We agree to be responsible for the school instrument listed above. It is our responsibility to maintain the
current condition of the instrument and case listed above. We understand that repair due to natural “wear
and tear” of the instrument will be handled by Folwell. We also understand that any damage or loss
caused by misuse, abuse, or negligence on the part of the student will be the responsibility of the student
and / or parent / guardian.
There is a $40.00, non-refundable rental fee for the academic school year for each instrument rented. The
summer rental fee is $20.00. Make checks payable to Folwell School Band or Strings. Please put the fee
and contract form in an envelope marked with student name(s) and room number. Each child needs
his/her own contract. Please write checks separately for either the String Dept. or the Band Dept.
Instruments may not leave school until contract and deposit are received. If paying the entire fee at once
may be too imposing, contact your student’s instrumental teacher and a payment plan may be set up on an
individual basis.

Acordamos ser responsables del instrumento de la escuela enumerado arriba. Es nuestra responsabilidad mantener la
condición actual del instrumento y encajonar mencionado arriba. Entendemos que cualquier reparación debido al
“desgaste natural y al rasgón” del instrumento será manejada por Folwell. También entendemos que cualquier daño
o pérdida causada por el uso erróneo, el abuso, o la negligencia de parte del estudiante será la responsabilidad del
estudiante y/o del padre/del guarda.
El precio para rentar el instrument durante el año escolar va a ser de $40 el cual no ex reembolsable. El costo de
rentarlo durante el verano sera de $20. Haga los cheques a nombre de: Folwell Band o Strings. Por favor ponga la
quota y la forma de contrato en un sobre marcado con el nombre del alumno y el numero de su salon. Cada niño
necesita su contrato. Por favor escriba los cheques directamnete para el departamento de cuerdas o el
departamento de la banda. Los instrument no pueden ir a su casa hasta que el contrato o deposito haya sido
recibido. Usted no debe pagar la suma total si no puede. Podemos empezar un programa de pago. Puede llamar la
escuela y acordar un plan de pago individualmente para usted.
