Für Schäden, die durch Nichtbeachtung dieser Gebrauchsanweisung entstehen, lehnt der Hersteller jede Haftung ab. 1. Produktbeschreibung Alle miniSKY Implantate sind durchmesserreduzierte Titanimplantate mit selbstschneidendem Doppelgewinde zur Inserierung in den Kieferknochen. mini1SKY 2.8: Das einteilige mini1SKY Implantat verfügt über einen Kugelkopf zur Fixierung von herausnehmbaren Prothesen. Weiterhin können mit zementierten Abutments festsitzende Versorgungen realisiert werden. mini2SKY 2.8 und 3.2: Das zweiteilige mini2SKY Implantat wird in zwei verschiedenen Varianten angeboten und verfügt über einen externen Torx, welcher mit verschiedenen Abutments und Aufbauteilen versorgt wird. 2. Indikation / Einsatzbereich mini1SKY 2.8: • Fixierung von Vollprothesen im atrophierten Kiefer • Feste Referenzpunkte (FRP) für die Implantatplanung zur Fixierung der Scan- und Bohrschablone. Diese Implantate können mit zementierten Abutments in die Brückenversorgung integriert werden. • Als temporäres Implantat zur Fixierung der provisorischen Versorgung. mini2SKY 2.8: • schmale Einzelzahnlücken UK Front (32-42) und OK seitlicher Schneidezahn (12, 22) • Verblockte Versorgungen bzw. Brücken UK Front (32-42) • Fixierung von Vollprothesen im atrophierten UK mit miniSKY Locator mini2SKY 3.2: • Einzelzahnlücken UK Front (32-42) und OK seitlicher Schneidezahn (12,22) • Einzelzahnlücken UK (35-45) und OK (15-25) • Verblockte Versorgungen bzw. Brücken im stark atrophierten Kiefer • Fixierung von Vollprothesen im atrophierten Kiefer (OK und UK) mit miniSKY Locator 3. Kontraindikation Das miniSKY Implantatsystem ist nicht geeignet für Patienten mit den in der Implantologie üblichen Kontraindikationen, wie z. B. Herz- und Kreislaufprobleme, Bruxismus, Osteoporose, starke Raucher, Alkoholiker, Diabetes und Stoffwechselerkrankungen, mangelnde Primärstabilität der Implantate. Bei Verdachtsmomenten auf Unverträglichkeit darf dieses Produkt nur nach vorheriger allergologischer Abklärung und Nachweis des Nichtbestehens einer Allergie verwendet werden. ACHTUNG Sind die nachfolgenden Anforderungen nicht erfüllt, ist die Anwendung von miniSKY kontraindiziert. • Um Frakturen bei Brücken- und Einzelzahnversorgungen zu vermeiden sind Extensionen nicht zulässig. • Der Knochen um das Implantat sollte eine Mindestwandstärke von 1,5 mm aufweisen. • Für mini1SKY 2.8 und mini2SKY 2.8: Bei Totalprothesen darf im Gegenkiefer keine natürliche Bezahnung vorhanden sein. • Bei der Fixierung einer Totalprothese mit mini1SKY 2.8 Implantaten muss darauf geachtet werden, dass die Hauptbelastung auf die Schleimhaut übertragen wird. Eine Überprüfung im mindestens jährlichen Recall ist notwendig. • Mit den mini2SKY uni.fit Abutments dürfen individuelle Abutments nur aus yttriumstabilisiertem Zirkonoxid im CAD/CAM Verfahren hergestellt werden. Es dürfen mit dem mini2SKY uni.fit Abutment nur Einzelabutments hergestellt werden. Brückenversorgungen auf individuellen Zirkonabutments sollten nur in statisch ausgewogenen Situationen durchgeführt werden, mit einer Relation von 2 Implantaten zu 1 Brückenglied. Die Brückenversorgung ist außerdem ohne Extensionen zu gestalten. Die Belastung muss axial in das Implantat-Abutmentsystem eingeleitet werden. Restaurationen mit einer Angulation größer 20° zur Implantatachse sind kontraindiziert. Die Länge der Restauration zur Länge des Implantats darf das Verhältnis von 1:1.25 nicht übersteigen. Die Breite der Restauration darf das 1,5 fache des Implantatdurchmessers nicht überschreiten. • Die prothetische Versorgung muss so gestaltet sein, dass sie gut ausbalanciert ist. • Einzelzahnversorgungen müssen flächig an den Nachbarzähnen abgestützt werden. Zusätzlich muss die Krone außer Okklusion gestellt werden. Auch unter Belastung dürfen keine Okklusionskontakte entstehen. • Ein möglicher Knochenabbau ist durch einen regelmäßigen Recall zu kontrollieren. Übersteigt der Knochenabbau um das Implantat mehr als ein Drittel der Implantatlänge, sollte eine Explantation des betroffenen Implantats erfolgen, um eine unkontrollierte Fraktur zu vermeiden. 4. Gefahren- und Sicherheitshinweise Die Lotnummern aller verwendeten Produkte müssen zur Gewährleistung der Rückverfolgbarkeit und Reklamationsansprüchen dokumentiert werden. 5. Lagerungs- und Haltbarkeitshinweise Die Implantate sind steril verpackt und müssen in der Originalverpackung trocken gelagert werden. Sterilverpackungen sind auf Unversehrtheit zu überprüfen. Beschädigte Verpackungen dürfen nicht mehr verwendet werden. Sterilprodukte dürfen nach Ablauf des Haltbarkeitsdatums nicht mehr verwendet werden (siehe Etikett). Die Abutments und Aufbauteile sind im Schlauchbeutel verpackt, werden unsteril angeliefert und müssen trocken, staubfrei gelagert werden. 6. Verarbeitung ACHTUNG Wir empfehlen grundsätzlich eine gründliche chirurgische und prothetische Planung sowie eine situations- und versorgungsabhängige Auswahl der Prothetikteile. Die im Labor verwendeten Halteschrauben dürfen nicht klinisch eingesetzt werden, da diese Belastungen im Labor unterliegen, wodurch der langfristige und sichere Sitz nicht gewährleistet werden kann. Für die Arbeit im Labor bietet bredent medical spezielle Laborschrauben an. Alle Teile sind vor der Eingliederung zu desinfizieren (siehe Punkt 9.). Implantation: Abb. 1: Chirurgisches Protokoll 1. Mit minimalinvasiver Eröffnung wird der Knochenkamm dargestellt. Mit dem Pilotbohrer (REF SKY-DP08) wird die Position des Implantates festgelegt und ca. 6,7 mm versenkt. 2. Nun wird mit dem Twistdrill (REF SKYDT13L) ø 1,3 mm die Richtung und Implantattiefe festgelegt. Die Graduierungen der Twistdrills entsprechen den Bohrtiefen: 6-8-10-12-14-16 mm. 3. Abhängig von der Knochenqualität wird mit dem Twistdrill (REF SKYDT23L) ø 2,25 mm das Implantatbett (siehe Abb. 1) aufbereitet. Je nach Implantatlänge verhindern die entsprechenden Tiefenstopps (REF SKY-ST06, SKY-ST08, SKY-ST10, SKY-ST12, SKY-ST14) eine zu tiefe Aufbereitung. 4. Für die mini2SKY 3.2 Implantate muss die weitere Aufbereitung je nach Knochenqualität wie folgt durchgeführt werden: D1 Knochen: Im harten Knochen wird mit dem SKY Bohrer für harte Knochen (REF SKYD1235) das Implantatbett bei 300 U/min auf die endgültige Länge abschließend aufbereitet. D2-D3 Knochen: Die Aufbereitung von mittleren und weichen Knochenqualitäten wird mit dem SKY Bohrer für mittleren und weichen Knochen (REF SKYD3435) bei 300 U/min auf die Implantatlänge durchgeführt. D4 Knochen: Im weichen Knochen ist die Aufbereitung mit dem Twistdrill (REF SKYDT23K, SKYDT23L) abgeschlossen. Je nach Implantatlänge stehen dem Anwender geeignete Tiefenstopps ø 4,0 mm (REF SKYS0840, SKYS1040, SKYS1240, SKYS1440) zur Verfügung, welche die Kavität vor zu tiefer Aufbereitung sichern. Die miniSKY Implantate sind steril verpackt. Nach dem Entfernen der Schutzfolie kann mit Hilfe des Implantatträgers das Implantat manuell 2-3 Umdrehungen eingeschraubt werden. Zur vollständigen Insertion wird das Implantat mit dem Einbringinstrument (REF mSKYXWM6, mSKYXWM7) aufgenommen. Das Einbringinstrument kann direkt mit dem Winkelstück (15 – 25 U/min) oder mit der Ratsche in Kombination mit dem Ratschenadapter (REF SKYADAP6) verwendet werden. Dabei ist darauf zu achten, dass das Implantat schonend und ohne Überhitzung bis zur definitiven Länge (bis zum Ende der Osseo-Connect-Oberfläche) eingedreht ist. Das max. Drehmoment beträgt 45 Ncm. 6. Bei den mini1SKY Implantaten wird die Prothese an der Implantatposition hohl gelegt. Die Retentionselemente werden erst nach ca. 12 Wochen eingearbeitet. Bei den mini²SKY Implantaten wird während der Osseointegration der Gingivaformer (REF m2SKYGF3) verwendet. 7. Die Sofortbelastung von miniSKY Implantaten ist nur bei temporären Versorgungen zulässig. 8. Für die spätere Eingliederung der prothetischen Versorgung sind die folgenden Anzugsdrehmomente zu berücksichtigen: • mini²SKY MD-Abutment Titan/BioXS, miniSKY Locator®Abutments, mini²SKY uni.fit Abutment: 20 Ncm • mini²SKY Gingivaformer, mini²SKY Abformabutment: 10 Ncm Abformung: mini1SKY: Zur Abformung der mini1SKY Implantate wird der O-Ring (REF SKY-OR55) über den Kugelkopf gesteckt (siehe Abb. 2). Anschließend wird der Torx der mini1SKY Implantate und die O-Ringe mit Abformmaterial brecision implant light (REF 580 BL05 0) vollständig umspritzt. Parallel wird das Abformmaterial brecision implant heavy (REF 580 BH38 0) in einen Abformlöffel gegeben und auf den abzuformenden Bereich appliziert. Nach der Aushärtung des Abformmaterials kann der Abformlöffel vorsichtig entnommen werden. Abb. 2 Die O-Ringe verbleiben in der Abformung. Zur Anfertigung des Meistermodells werden die mini²SKY Implantatanaloge (REF m2SKYXIA) mit dem mini²SKY Abformabutment verschraubt und das Modell mit Superhartgips hergestellt. Prothetik: mini1SKY: Bei Verwendung der mini1SKY Implantate wird zum Einpolymerisieren der Matrizen zuvor die Außenfläche des Matrizengehäuses mit Aluminiumoxid 110 µm konditioniert. Die Unterschnitte und Papillenzwischenräume sind zu schließen, um sicherzustellen, dass kein Kunststoff in untersichgehende Bereiche eindringen kann und eine Entnahme der Prothese verhindert. Bei der Verwendung der SKY Edelmetallmatrize ist die „Gebrauchsanweisung SKY Edelmetallmatrize“ zu beachten. mini1SKY und mini2SKY: Die miniSKY MD-Abutments können mit den in der Zahntechnik üblichen Methoden individualisiert werden. Das Interface Implantat-Abutment darf nicht individualisiert werden. Bei Reduzierung der Höhe bzw. Individualisierung des Abutments darf die Abutmentschraube nicht beschädigt werden. Es ist darauf zu achten, dass eine ausreichende Wandstärke und Abutmenthöhe in Abhängigkeit zur Kronenlänge erhalten bleibt. mini2SKY: Die miniSKY Locator® Abutments dürfen in keinem Fall beschliffen werden und können sofort nach der Implantatinsertion als Gingivaformer aufgeschraubt werden. Eine Sofortbelastung ist nicht zulässig. Die Prothese muss hohl gelegt werden. Erst nach erfolgter Osseointegration werden die Retentionselemente eingearbeitet. Dabei muss das schwarze Verarbeitungselement durch einen entsprechenden Retentionseinsatz mit definierter Abzugskraft ersetzt werden. ACHTUNG Zum Einpolymerisieren der Matrizen wird zuvor die Oberfläche des Matrizengehäuses mit Aluminiumoxid 110 µm konditioniert und das Locator®Abutment durch einen Kofferdamm abgedeckt. Dabei empfehlen wir, den Kofferdamm an der Stelle des Implantates nicht zu perforieren, um sicherzustellen, dass kein Kunststoff in untersichgehende Bereiche eindringt und eine Entnahme der Prothese verhindert. mini2SKY uni.fit Abutment: Konstruktion Für die Bearbeitung in einer CAD Software muss der Datensatz des mini2SKY uni.fit Abutments in der Datenbank hinterlegt und im Workflow abrufbar sein. Für die Erstellung eines individuellen Abutments sind die Herstellerangaben der Softwarehersteller zu beachten. Sollte ein Zementspaltautomatismus aktiviert sein, ist darauf zu achten, dass ein Zementspalt im fertigen Zirkonabutment von 30-50 μm nicht überschritten wird. Gegebenenfalls ist ein Prüfkörper herzustellen, mit dem die Passung für das mini2SKYuni.fit Abutment kalibriert wird. Der Prüfkörper / Zirkonabutment muss sich ohne Friktion auf die Titanklebebasis aufschieben lassen. Sollte ein geringes bis größeres Rotationsspiel beim Prüfkörper/ Zirkonabutment vorhanden sein, ist der Zementspalt in der CAD Software zu reduzieren. Es ist ratsam das zu scannende Meistermodell mit einer abnehmbaren, scanbaren und großzügig gestalteten Zahnfleischmaske herzustellen. Um Nacharbeiten am fertiggestellten Zirkonabutment weitestgehend zu vermeiden, sollte das individuelle Abutment bereits in der Konstruktion mit dem nötigen Emergenzprofil gestaltet werden. Das Abutmentdesign sollte eine Höhe von ca. 10 mm nicht überschreiten. CAM/Software/Fertigung Das konstruierte Abutment sollte im Nestingmodul so platziert werden, dass der Kavitätenbereich des Abutments, der einerseits den Raum zur Aufnahme der mini2SKY uni.fit Abutment Titanklebebasis bildet und andererseits den Schraubenkanal, in Z-Richtung auf 0° eingestellt ist. Das mini2SKY uni.fit Abutment ist so gestaltet, dass es auch mit 3+1 Achs Fräsmaschinen herstellbar ist. Der Schrumpfungsfaktor des verwendeten Zirkons ist unbedingt zu beachten. Falsch eingegebene Größenskalierungen können zu Passungsungenauigkeiten zwischen Klebebasis und Zirkonabutment führen. Verklebung Die Verklebung des individuellen Abutments mit der mini2SKY uni.fit Titanklebebasis empfehlen wir im Labor wie folgt durchzuführen: • Aufrauen der Klebestellen der mini2SKY uni.fit Titanklebebasis und des individuellen Abutments durch Sandstrahlen mit 110 μm Aluminiumoxid. Der Torx sowie der Gingivaanteil dürfen nicht gestrahlt werden! Zum Schutz des Torx wird die mini2SKY uni.fit Titanklebebasis auf ein Modellanalog aufgeschraubt sowie der Gingivaanteil abgedeckt und dann der Strahlvorgang durchgeführt. • Wir empfehlen als Kleber das Sortiment DTK-Kleber Abutment (REF 540 0117 5, bredent GmbH & Co. KG) oder Panavia™ F 2.0. Beim mini2SKY uni.fit Abutment ist mit größtmöglicher Sorgfalt vorzugehen, um Folgeschäden unsachgemäßer Verarbeitung zu vermeiden. Eingliedern Vor dem Eingliedern müssen die Teile desinfiziert werden. Die mini2SKYuni.fit Abutments werden analog der Position auf dem Modell im Implantat ausgerichtet und mit einem Drehmoment von 20 Ncm festgeschraubt. Verarbeitung als FRP Insertion der miniSKY-FRP Die miniSKY-FRP werden 2-3 Wochen vor dem geplanten Implantationstermin transgingival inseriert. Bei der Wahl der FRP-Position sind die definitiven Implantatpositionen, die Schnittführung und zu augmentierende Bereiche zu berücksichtigen. Um eine Verletzung wichtiger anatomischer Strukturen zu vermeiden ist eine adäquate Länge der miniSKY-FRP unbedingt zu beachten. Für eine stabile Abstützung der Planungshilfen und chirurgischen Hilfen sind mindestens 3 miniSKY-FRP pro Kiefer notwendig. Das chirurgische Vorgehen ist analog Abb. 1. Die mini1SKY Implantate können im UK sofort als Basis für die temporäre Versorgung verwendet werden. Dazu wird die bestehende Prothese an den Implantatpositionen ausgeschliffen und die SKY O-Ring Matrizen (REF SKY-OR50) bzw. die SKY Edelmetallmatrizen (REF SKYGM225) einpolymerisiert. Labor FRP Auf dem Modell werden die Scanschablonen und Bohrschablonen nach den üblichen zahntechnischen Verfahren hergestellt. Für die Herstellung der Zähne in der Scanschablone empfehlen wir das Produkt X-resin CT/DVT (REF 540 0115 9). Zur Fixierung der Scan- und Bohrschablone werden die weißen mini1SKY Planungsmatrizen verwendet. Diese werden in die Scan- und Bohrschablone einpolymerisiert. 7. At least one recall appointment must be made each year. • The mini2SKY uni.fit abutments may only be used for individual abutments made of yttriumstabilized zirconium oxide in the CAD/CAM procedure. The mini2SKY uni.fit abutment may only be used for the fabrication of single abutments. Bridge restorations on individual zirconium abutments should only be used in situations in which adequate stability is ensured (relation of 2 implants to 1 bridge unit). The bridge restorations should not have any extensions. Axial load transfer into the implant-abutment system must be ensured. Restorations with angles of more than 20° to the implant axis are contraindicated. The ratio of the length of the restoration to the length of the implant must not exceed 1:1.25. The width of the restoration must not be more than one and a half times the diameter of the implant. • The prosthetic restoration must be designed in a way to ensure that it is well balanced. • Single tooth restorations need to be supported on the surfaces of adjacent teeth. Moreover, the crown must be out of occlusion. Occlusion contacts must be avoided even under stress (loading). • Regular recalls help to check for potential bone resorption. If bone resorption of more than one third of the implant length occurs around an implant, explantation is required in order to avoid uncontrolled fracture. 4. Safety and hazard information The lot numbers of all components should be recorded to ensure traceability and to handle complaints. 5. Storage and durability The implants are supplied in the sterile condition and dry storage in the original packaging must be ensured. Sterile packagings must be checked to make sure that they are not damaged. Damaged packagings must not be used any longer. Sterile products must not be used after the date of expiration (see label). The abutments and suprastructure components are packed in pouches, supplied in a non-sterile condition and need to be stored under dry and dust-free conditions. 6. Processing / Use ATTENTION Generally, we recommend thorough surgical planning and selection of the prosthetic components depending on the situation and type of restoration. The retention screws used in the laboratory are not suitable for the clinical use, since they are subject to stress in the laboratory; as a result long-term and reliable fit can no longer be guaranteed. For the use in the laboratory, bredent medical offers special laboratory screws (brown anodized). All prosthetic components must be disinfected prior to the insertion (see item 9.). Die weitere Modellherstellung erfolgt nach den üblichen zahntechnischen Methoden. Für die sichere Anwendung ist es notwendig, dass das Weichgewebe unterhalb des Torx abschließt. Dadurch werden eine sichere Abformung und die sichere Retention des O-Ringes gewährleistet. Bei zu hoher Gingiva steht der O-Ring unter Spannung, was zu Ungenauigkeiten bei der Abformung und zu frühzeitigem Verschleiß bei der Prothesenfixierung führt. Nicht wiederverwenden 9. Sonstige Hinweise Das Produkt darf nur von Zahnärzten, Zahntechnikern und entsprechend geschultem Fachpersonal verwendet werden. Für die Verarbeitung sind nur Originalwerkzeuge und -teile zu verwenden. Alle Teile sind vor dem Eingliedern im Mund des Patienten zu desinfizieren. Es dürfen nur Instrumentendesinfektionsmittel benutzt werden, z. B. Dürr ID 212 forte. Die Herstellerangaben zum Gebrauch des Desinfektionsmittels sind zu beachten. Die Teile müssen nach dem Desinfizieren gründlich von Desinfektionsmittel befreit werden (siehe dazu Anleitung des Desinfektionsmittelherstellers, z. B. unter fließendem Wasser bei ID212 forte). ACHTUNG Beim jährlichen Recall der Patienten ist der korrekte Sitz der prothetischen Versorgung bzw. des Halteelements zu überprüfen. Die axiale Position der Kontaktpunkte muss ebenfalls kontrolliert werden da extraaxiale Belastungen zu Schraubenlockerungen und damit zu Ermüdungsbrüchen führen, wodurch Fragmente aspiriert oder verschluckt werden können. Werden die Abutments zur Reinigung entfernt, sind neue Halteschrauben zu verwenden, um Schraubenlockerungen und Schraubenbrüche zu vermeiden. Alle Produkte, die mit dem Symbol gekennzeichnet sind, sind ausschließlich für den einmaligen Gebrauch vorgesehen. Falls dieses Produkt dennoch mehrmals verwendet wird, können folgende Risiken entstehen: Kreuzkontamination, Fehlfunktion, Passungenauigkeit etc. Diese Gebrauchsanweisung entspricht dem aktuellen Stand der Technik und unseren eigenen Erfahrungen. Alle mit ® oder ™ gekennzeichneten Bezeichnungen sind geschützte Marken und/oder Unternehmenskennzeichen von fremden Rechteinhabern. miniSKY implant system The Locator® retention elements 0°-10° (REF LOCLAB10) cannot be used together with the miniSKY Locator® abutments. For polymerization of the matrixes, first the surface of the matrix housing is conditioned with aluminium oxide (110 µm) and the Locator® abutment is covered with rubber dam. It is recommended not to perforate the rubber dam at the position of the implant in order to avoid penetration of resin into undercuts and to ensure safe removal of the denture. mini2SKY uni.fit abutment: Design For processing in a CAD software, the dataset of the mini2SKY uni.fit abutments must be stored in the database and retrieved in the workflow. The information provided by the software producer must be observed when fabricating an individual abutment. If „automatic cement gap“ is activated, the cement gap in the completed zirconium abutment should be within the range of 30-50 μm. It may be required to prepare a specimen to calibrate the fit of the mini2SKYuni.fit abutment. It should be possible to slide the specimen / zirconium abutment onto the titanium adhesive base whilst avoiding friction. It is recommended to fabricate the master model to be scanned with a removable, scannable and adequately sized gingival mask. To avoid reworking of the completed zirconium abutment, the individual abutment should already have the required emergency profile in the design process. The abutment design should have a maximum height of approx. 10 mm. CAM/Software/Fabrication The designed abutment should be placed in the nesting module so that the cavity section of the abutment which forms the space to receive the mini2SKY uni.fit abutment titanium adhesive base and the screw canal is set to 0° in Z-direction. The design of the mini2SKY uni.fit CAD abutment enables the fabrication in 3+1 axle milling machines. The shrinkage factor of the zirconium in use must be strictly adhered to. When entering incorrect size scales, inadequate fit of the adhesive base and the zirconium abutment may result. Luting/bonding The individual abutment should be luted/bonded to the mini2SKY uni.fit CAD titanium adhesive base in the laboratory as follows: • Sandblast with aluminium oxide (110 μm) to roughen the bonding surfaces of the mini2SKY uni.fit CAD titanium adhesive base and the individual abutment. The torx and the gingival area must not be sandblasted! To protect the torx, the mini2SKY uni.fit CAD titanium adhesive base is screwed on a model analog and sandblasting is performed subsequently. • We recommend to use DTK-Kleber Abutment as an adhesive (REF 540 0117 5, bredent GmbH & Co. KG) or Panavia™ F 2.0. Utmost precision and care are required for the mini2SKY uni.fit CAD Abutment to avoid consequential damage caused by improper processing. Insertion The components must be disinfected prior to the insertion. The mini2SKY uni.fit CAD abutments are aligned in the implant in accordance with the position on the model and tightened at a torque of 20 Ncm. Processing as FRP Insertion on the miniSKY-FRP 2-3 weeks prior to the scheduled date of implant placement, the miniSKY-FRP are inserted transgingivally. When selecting the positions of the miniSKY-FRP, the final implant positions, incision and areas to be augmented must be taken into consideration. To avoid injuring essential anatomical structures, adequate length of the miniSKY-FRP must be ensured. At least 3 miniSKY-FRPs per jaw are required to achieve stable support of the auxiliary planning and surgical devices. The surgical procedure is identical with the one shown in fig. 1. In the lower jaw, the mini1SKY implants can be immediately used as a basis for the temporary restoration. For this purpose, the existing denture is ground at the positions of the miniSKY implants and the SKY O-ring matrixes (REF SKY-OR50) or the SKY precious metal matrixes (REF SKYGM225) are integrated (polymerized). Laboratory - FRP The standard dental-technical procedures are used to produce the scan and drilling templates on the model. We recommend X-resin CT/DVT (REF 540 0115 9) for the preparation of the teeth in the scan template. The white mini1SKY planning matrixes are used for fixing the scan and drilling templates. They are polymerized into the scan and drilling templates. REF GB Before using the product, please read these instructions for use carefully! The manufacturer will not accept any liability for damage resulting from non-compliance with these instructions for use. 1. Description of the product All miniSKY implants are diameter-reduced titanium implants with self-tapping double threads for insertion into the jaw bone. mini1SKY 2.8: The one-piece mini1SKY implant features a ball head for fixation of removable dentures. Moreover, cemented abutments can be used for the fabrication of fixed restorations. mini2SKY 2.8 and 3.2: The two-piece mini2SKY implant is available in different types and features an external torx to fasten various abutments and suprastructure components. 2. Indication / Application areas miniSKY implants and miniSKY prosthetics: mini1SKY 2.8: • Fixation of full dentures in the atrophied ridge • Fixed reference points (FRP) for implant planning for fixation of the scan and drilling template. These implants can be integrated into the bridge restoration using cemented abutments. • As a temporary implant for fixation of the temporary restoration. mini2SKY 2.8: • Narrow single tooth gaps, lower anterior teeth (32-42) and upper lateral incisor (12, 22) • Splinted restorations or lower anterior bridges (32-42) • Fixation of full dentures in the atrophied ridge with miniSKY Locator mini2SKY 3.2: • Single tooth gaps, lower anterior teeth (32-42) and upper lateral incisor (12, 22) • Single tooth gaps, lower jaw (35-45) and upper jaw (15-25) • Splinted restorations or bridges in the severely atrophied jaw • Fixation of full dentures in the atrophied jaw (lower and upper jaws) with miniSKY Locator 3. Contraindication The miniSKY implant system is not suitable for standard contraindications in implant dentistry such as patients with cardiovascular problems, bruxism, osteoporosis, heavy smokers, alcoholics, diabetes and metabolic diseases and insufficient primary stability of the implants. The product should not be used for patients with a suspected allergy until allergy tests have been completed to confirm that the patient is not allergic to the materials contained in the product. 1. The ridge is exposed using a minimally invasive procedure. The position of the implant is determined with the pilot drill (REF SKY-DP08) and the implant is inserted in the cavity to a depth of approx. 6.7 mm. 2. Then the direction and implant depth are determined with the twistdrill (REF SKY-DT13L) ø 1.3 mm. The graduations of the twistdrills correspond to the drilling depths: 6 -8 -10 -12 -14 -16 mm. 3. The implant bed is prepared with the twistdrill (REF SKY-DTL23L) ø 2.25 mm, depending on the bone quality (see fig.1). Depending on the implant length, the corresponding drill stops (REF SKY-ST06, SKY-ST10, SKY-ST12, SKY-ST14) avoid excessive depth of the preparation. 4. For the mini2SKY 3.2 implants, further preparation needs to be carried out in the following way (depending on the bone quality): D1 bone: In hard bone, the SKY drill for hard bone (REF SKYD1235) is used to prepare the implant site (300 rpm) until the definitive length is reached. D2-D3 bone: Medium and soft bone qualities are prepared up to the implant length using the SKY drill for medium and soft bone (REF SKYD3435) and a speed of 300 rpm. D4 bone: Preparation in the soft bone is completed with the Twistdrill (REF SKYDT23K, SKYDT23L). Depending on the implant length, suitable depth stops ø 4.0 mm (REF SKYS0840, SKYS1040, SKYS1240, SKYS1440) are available to avoid excessive depth during cavity preparation. ATTENTION Tilting during the drilling process or repeated drilling must be avoided to ensure the primary stability of the miniSKY implants. 5. The miniSKY implants are supplied in a sterile packaging. After removing the protective film, the implant can be screwed in manually (2-3 turns) using the implant carrier. The implant is taken up (fig. 1) with the insertion instrument (mounter) (REF mSKYXWM6, mSKYXWM7)) to insert it completely. The insertion instrument is suitable for use with the contra-angle (15 - 25 rpm) or with the ratchet in combination with the ratchet adapter (REF SKYADAP6). Whilst avoiding overheating, the implant is carefully turned in until the definitive length (up to the end of the osseo-connect surface) is reached. The max. torque is 45 Ncm. 6. The denture is hollowed out at the implant position when using the mini SKY implants. The retention elements are integrated only after approx. 12 weeks. The gingiva former (REF m2SKYGF3) is used for the mini²SKY implants during osseointegration. 7. Immediate loading of miniSKY implants is only permissible for temporary restorations. 8. For insertion of the prosthetic restoration later on, the following tightening torques must be observed: • mini2SKY MD-Abutment titanim/BioXS, miniSKY Locator®Abutments, mini²SKY uni.fit Abutment: 20 Ncm • mini2SKY Gingiva former: 10 Ncm Impression: mini1SKY: For the impression of the mini1SKY implants, the O-ring (REF SKY-OR55) is placed on the ball head (see fig. 2). Then brecision implant light (REF 580 BL050) impression material is injected around the torx of the mini1SKY implants and the O-rings. brecision implant heavy (REF 580 BH380) impression material is filled into an impression tray and applied to the impression area. Once the impression material has hardened, the impression tray can be removed carefully. The O-rings are left in the impression. Fig. 2 When repositioning the implant analogs (REF m1SKYXIA) make sure to match the torx geoPositioning metry in the impression material with the alignment of the torx from the implant analog. O-ring ATTENTION The implant analogs need to be connected with Multisil-Mask hard (REF 540 0113 4) gingival material to ensure accurate placement of the implant analogs during the entire model fabrication process. Standard dental-technical methods are used for the remaining steps of model fabrication. To ensure reliable and safe use, it must be ensure that the soft tissue ends below the torx. This way, an accurate impression and reliable retention of the O-ring are ensured. In cases of excessively high gingival lines, the O-ring is subjected to stress which leads to inaccuracy of the impression and premature wear when fixing the denture. mini²SKY: For the impression of the mini²SKY implants, the mini²SKY impression abutment, open tray (REF m2SKYPA1) is screwed on the implants (torque: 10 Ncm) and the impression is taken using the standard procedure for open trays. For the fabrication of the master model, the mini²SKY implant analogs (REF m2SKYXIA) are screwed on the mini²SKY impression abutment and the model is prepared using super-hard stone. Prosthetic restoration: mini1SKY: For polymerization of the matrixes, first the outer surface of the matrix housing is conditioned with aluminium oxide (110 µ). Undercuts and papillary spaces must be sealed to avoid penetration of resin in undercuts and enable reliable removal of the denture. Immediate loading is not permissible. The denture must be hollowed out. The retention elements are only integrated after successful osseointegration. The manufacturer‘s instructions on the use of the disinfectant must be observed. After disinfecting, disinfectant residues must be thoroughly removed from the components (see the instructions of the manufacturer of the disinfectant; for example under running water - for ID212 forte). Ill.: Protocole chirurgical ATTENTION During the patient‘s annual recall appointment, proper fit of the prosthetic restoration or retention elements needs to be checked. The axial position of the contacts must also be controlled since extra-axial loading may result in loosening of screws and hence in fatigue fractures and fragments may be aspirated or swallowed. If the abutments are removed for cleaning purposes, new retention screws must be used to avoid loosening of screws and screw fracture. All products labelled are intended for single use only. However, if this product is used several times, the following risks may arise: cross contamination, malfunction, inaccurate fit, etc. These instructions for use are based on state-of-the-art methods and equipment and our own experience. The product may only be used for the indication described under item 2. The user himself is responsible for processing the product. Liability for incorrect results shall be excluded since the manufacturer does not have any influence on further processing. Contre-indications Le système implantaire miniSKY n’est pas adapté pour les patients concernés par les contre-indications habituelles en implantologie, comme par ex. les problèmes cardiaques et de la circulation sanguine, le bruxisme ou l’ostéoporose, le tabagisme, l’alcoolisme, le diabète, les maladies d’ordre métabolique et en cas d’un manque de stabilité primaire des implants. En cas de doute en matière d’incompatibilité, le produit ne doit être utilisé qu’après clarification et mise en évidence qu’il n’y a pas de risque d’allergie. MISE EN GARDE Si les conditions suivantes ne sont pas remplies, l’utilisation des implants miniSKY est contre-indiquée. • Pour éviter des fractures en cas de bridges et de restaurations de dents unitaires, les extensions ne sont pas autorisées. • Les tissus osseux autour de l’implant devraient présenter une épaisseur de paroi d’au moins 1,5 mm. • Pour mini1SKY 2.8 et mini2SKY 2.8: En cas de prothèse complète du maxillaire supérieur, il ne doit pas y avoir de dentition naturelle de la mandibule. • Lors de la fixation d’une prothèse complète avec des implants mini1SKY 2.8 il faut veiller à ce que les contraintes principales soient transférées sur la muqueuse. Une vérification est nécessaire lors de contrôles au moins tous les ans. • Avec les piliers mini2SKY uni.fit on est uniquement autorisé à réaliser des piliers personnalisés en oxyde de zircone stabilisés à l’yttrium dans le procédé CAO/FAO. Avec le pilier mini2SKY uniquement la réalisation de piliers unitaires est autorisée. Des restaurations par bridges sur des piliers personnalisés en zircone ne doivent être effectuées que dans des situations équilibrées au point de vue statique avec une relation de 2 implants pour 1 élément de pont. D’autre part la restauration par bridge est à réaliser sans extensions. La contrainte doit être introduite de façon axiale dans le système implant-pilier. Les restaurations présentant une angulation supérieure à 20° par rapport à l’axe de l’implant sont contre-indiquées. La longueur de la restauration par rapport à la longueur de l’implant ne doit pas dépasser le rapport 1:1.25. La largeur de la restauration ne doit pas dépasser 1,5 fois le diamètre de l’implant. • La restauration prothétique doit être étudiée de telle sorte à être bien équilibrée. • En cas d’utilisation en tant qu’implant unitaire, les restaurations des dents unitaires doivent être soutenues par les dents avoisinantes sous forme de larges surfaces de contact. De plus la couronne doit se trouver entièrement en dehors de l’occlusion. Même sous charge il ne doit pas y avoir de contacts d’occlusion. • Une possible résorption osseuse est à contrôler par un suivi régulier. Quand une résorption osseuse autour de l’implant est supérieure à un tiers de la longueur de l’implant, il faut réaliser une explantation afin d’éviter une fracture incontrôlée. 4. Dangers et consignes de sécurité Les numéros de lot de toutes les pièces utilisées doivent être documentés pour assurer la traçabilité et pourvoir exercer des droits de réclamation. 5. Consignes de stockage et date de péremption Les implants sont sous emballage stérile et sont à stocker au sec dans leur emballage original. Veuillez vérifier que les emballages stériles soient intacts. Les emballages endommagés ne doivent plus être utilisés. Les produits stériles ne doivent plus être utilisés après la date de péremption (voir étiquette). Les piliers et pièces de reconstitution sont emballés dans un sachet tubulaire, ils sont livrés non stériles et sont à conserver au sec et à l’abri de la poussière. 6. Mode d‘emploi MISE EN GARDE Nous recommandons par principe une planification chirurgicale méticuleuse et une sélection des implants et des pièces prothétiques en fonction de la situation et de la restauration. Les vis de maintien utilisées au laboratoire ne doivent pas être sujettes à une mise en œuvre clinique car elles sont soumises à des contraintes au laboratoire et de ce fait leur assise sûre et à long terme ne pourrait plus être assurée. Pour le travail au laboratoire bredent medical dispose de vis de laboratoire spéciales anodisées en brun. Toutes les pièces doivent être désinfectées avant leur insertion (cf. point 9.). 1. Dégager la crête osseuse en réalisant une ouverture minimalement invasive. Avec le foret pilote (REF SKY-DP08) déterminer la position de l’implant qu’on introduit d’env. 6,7 mm. 2. A l’aide du Twistdrill (REF SKY-DT13L) ø 1,3 mm déterminer l’orientation et la profondeur d’implantation. Les graduations des Twistdrills correspondent aux profondeurs de forage: 6 -8 -10 -12 -14 -16 mm. 3. En fonction de la qualité osseuse on procède à la préparation de la loge implantaire à l’aide du Twistdrill (REF SKY-DT23L) ø 2,25 mm, (cf. Fig. 1). Selon la longueur de l’implant les butées de profondeur correspondantes (REF SKY-ST06, SKY-ST08, SKY-ST10, SKY-ST12, SKY-ST14) empêchent une préparation trop profonde. 4. Pour les implants mini2SKY 3.2, selon la qualité osseuse, il faut poursuivre la préparation comme suit: Tissus osseux D1 : Dans les tissus osseux durs on poursuit la préparation du lit implantaire avec le foret SKY pour tissus osseux durs (REF SKYD1235) à 300 t/min. pour atteindre la longueur finale. Tissus osseux D2-D3 : La préparation de qualités osseuses moyennes et molles se fait avec le foret SKY pour tissus osseux moyens et mous (REF SKYD3435) à 300 t/ min. pour atteindre la longueur de l’implant. Tissus osseux D4 : On termine la préparation osseuse dans les tissus mous avec le Twistdrill (REF SKYDT23K, SKYDT3L). Selon la longueur de l’implant, l’utilisateur dispose de butées de profondeur adaptées de Ø 4,0 mm (REF SKYS0840, SKYS1040, SKYS1240, SKYS1440) pour assurer que la préparation se soit pas trop profonde. MISE EN GARDE Eviter un balancement durant le processus de forage ou un forage répété afin de ne pas nuire à la stabilité primaire des implants miniSKY. 5. Les implants sont dans un emballage stérile. Après avoir enlevé la feuille de protection, on peut à l’aide du porte-implant visser l’implant à la main 2 à 3 tours. Pour l’insertion complète de l’implant on se sert de l’instrument de pose (REF mSKYXWM6, mSKYXWM7). L’instrument de pose s’utilise directement avec le contre-angle (15 – 25 t/min) ou la clé à cliquet en combinaison avec l’adaptateur de clé à cliquet (REF SKYADAP6). Il faut veiller à insérer l’implant de façon à ménager les tissus et sans surchauffe jusqu’à la longueur définitive (jusqu’à la fin de la surface osséo-connect). Le couple de rotation maximal est de 45 Ncm. 6. Pour les implants mini1SKY on pose la prothèse de façon creuse à la position de l’implant. Les éléments de rétention ne sont à intégrer qu’au bout d’env. 12 semaines. Pour les implants mini²SKY on se sert des vis de cicatrisation (REF m2SKYGF3) durant l’osséointégration. 7. La mise sous contrainte d’implants miniSKY n’est admissible que dans le cas de restaurations provisoires. 8. Pour une insertion ultérieure de la restauration prothétique il faut respecter les couples de serrage suivants: • Piliers MD mini2SKY titane/BioXS, piliers miniSKY Locator®, piliers mini²SKY uni.fit: 20 Ncm • Coiffes de cicatrisation mini2SKY, transferts mini2SKY: 10 Ncm Prise d’empreinte: mini1SKY: Pour la prise d’empreinte des implants mini1SKY, on place la bague en O (REF SKYOR55) sur la pièce mâle, (voir figure 2). Ensuite on enveloppe complètement le Torx des implants mini1SKY ainsi que les bagues en O de matériau d’empreinte brecision implant light (REF 580 BL05 0). Parallèlement on utilisera un porte-empreinte avec le matériau d’empreinte brecision implant heavy (REF 580 BH38 0) pour l’appliquer sur la zone à reproduire. MISE EN GARDE A l’aide de la fausse gencive Multisil-Mask dure (REF 540 0113 4) il faut bloquer les analogues d’implants ce qui permettra de les positionner de façon exacte durant l’élaboration du modèle. Le modèle sera ensuite réalisé selon les règles habituelles en prothèse dentaire. Pour une utilisation sûre, il est nécessaire que les tissus mous terminent sous le Torx. Ainsi on assure une prise d’empreinte sûre tout comme une rétention sûre de la bague en O. Si la gencive était trop élevée, la bague en O serait sous tension, ce qui conduirait à des imprécisions lors de la prise d’empreinte et à une usure prématurée de la fixation de la prothèse. mini2SKY: Pour la prise d’empreinte des implants mini2SKY on positionne le porte-empreinte spécial transfert mini2SKY (REF m2SKYAOL) avec 10 Ncm sur les implants et on procède à la prise d’empreinte traditionnelle. Pour la réalisation du maître-modèle on repositionne les analogues d’implant mini2SKY (REF m2SKYXIA) dans les transferts mini2SKY pour réaliser le modèle avec du plâtre extra-dur. Prothèse: mini1SKY: Quand on utilise des implants mini1SKY, pour la fixation par polymérisation des pièces femelles, on conditionne préalablement la surface extérieure du logement de la pièce femelle à l’oxyde d’aluminium 110 µm. Les parties en retrait et les papilles interdentaires sont à fermer afin d’assurer que de la résine ne puisse pas pénétrer dans les parties en retrait et empêche de retirer la prothèse. Lors de l’utilisation des pièces femelles SKY en métal précieux, il y a lieu d’observer le „Mode d’emploi Pièces femelles SKY en métal précieux“. mini1SKY et mini2SKY: Les piliers miniSKY MD se prêtent à la personnalisation selon les méthodes habituelles en prothèse dentaire. Ne pas personnaliser l’interface implant/pilier. En réduisant la hauteur du pilier et/ou en le personnalisant, ne pas endommager la vis du pilier. Il faut veiller à ce qu’une épaisseur de paroi et hauteur de pilier suffisantes subsiste en fonction de la longueur osseuse. mini2SKY: Il ne faut en aucun cas meuler les piliers miniSKY Locator®. Immédiatement après l’insertion des implants, on peut les mettre en place par vissage en tant que vis de cicatrisation. Une mise sous contrainte immédiate n’est pas admissible. Il faut poser la prothèse de façon creuse. Les éléments de rétention doivent seulement être intégrés après l’osséointégration. Il faut alors remplacer l’élément de mise en œuvre noir par un insert de rétention adéquat présentant une force de rétention définie. MISE EN GARDE Les éléments de rétention Locator® 0°-10° (REF LOCLAB10) ne doivent pas être utilisés avec les piliers miniSKY Locator®. Pour la fixation par polymérisation des pièces femelles, on conditionne préalablement la surface du logement de la pièce femelle avec de l’oxyde d’aluminium 110 µ et on recouvre le pilier Locator® d’une digue. Pour cela nous recommandons de ne pas perforer la digue à l’endroit correspondant à l’implant afin d’assurer que de la résine ne puisse pas pénétrer dans les parties en retrait et empêche de retirer la prothèse. Pilier mini2SKY uni.fit: Construction Pour la gestion dans un logiciel CAO il faut que l’ensemble des données du pilier mini²SKY uni.fit soit stocké dans la banque de données et soit disponible dans le workflow. Pour la réalisation d’un pilier personnalisé, veuillez respecter les recommandations du fabricant du logiciel. Au cas où un automatisme de joint de scellement serait activé, veillez à ce que le joint de scellement dans le pilier en zircone terminé ne dépasse pas 30 - 50 μm. Le cas échéant, il faut réaliser un spécimen de contrôle permettant de calibrer l’adaptation avec le pilier mini2SKYuni.fit. Il faut que le spécimen de contrôle / pilier en zircone se laisse coulisser sans friction sur la base de collage en titane. Au cas où le spécimen de contrôle / pilier en zircone présenterait un jeu trop faible ou trop important, il faut réduire le joint de scellement dans le logiciel CAO. Il est conseillé de réaliser le maître-modèle à réaliser à l’aide d’une fausse gencive amovible se laissant scanner et à formes généreuses. Pour éviter autant que possible des retouches du pilier en zircone terminé, prévoir le profil d’émergence adapté lors de son design. Le design du pilier ne devrait pas dépasser une hauteur d’env. 10 mm. CAO/Logiciel/Réalisation Le pilier conçu est à placer dans le module Nesting de telle sorte que la zone de cavité du pilier qui d’une part forme l’espace pour accueillir la base titane de collage du pilier mini2SKY uni.fit et d’autre part le canal de vis en direction Z soit réglé sur 0°. Le pilier mini2SKY uni.fit est conçu de telle sorte à aussi pouvoir être réalisé avec des appareils de fraisage 3+1 Achs. Il faut absolument tenir compte du facteur de rétraction de la zircone utilisée. En entrant des échelles incorrectes, il peut y avoir des imprécisions d’adaptation entre la base de collage et le pilier en zircone. Collage Nous conseillons de procéder au laboratoire au collage du pilier personnalisé avec la base de collage en titane mini2SKY uni.fit comme suit: • Conférer de la rugosité aux surfaces de collage de la base de collage titane de mini2SKY uni.fit et du pilier personnalisé par sablage avec de l’oxyde d’aluminium de 110 μm. Ne pas sabler le Torx ainsi que la partie gingivale! Pour protéger le Torx on visse la base de collage en titane mini2SKY uni.fit sur un analogue de modèle et la partie gencive est recouverte d’une protection et ensuite on procède au sablage. • Comme colle nous recommandons l‘assortiment Colle DTK-Kleber pilier (REF 540 0117 5, bredent GmbH & Co. KG) ou Panavia™ F 2.0. Il faut procéder avec grand soin avec le pilier mini2SKY uni.fit afin d’éviter des dommages subséquents à une mise en œuvre mal appropriée. Insertion Désinfecter les pièces avant l’insertion. Les piliers mini2SKYuni.fit sont à aligner dans l’implant de façon analogue à leur position sur le modèle et à fixer par vissage avec un couple de 20 Ncm. Mise en œuvre en tant que FRP Insertion des miniSKY-FRP L’insertion des vis miniSKY-FRP est à réaliser sur la face transgingivale 2-3 semaines avant la date d’implantation. Lors de la sélection de la position des FRP, il faut tenir compte de la position définitive des implants, de la ligne de coupe et des zones où une augmentation sera nécessaire. Pour ne pas blesser les structures anatomiques importantes, il faut absolument veiller à une longueur adéquate des miniSKY-FRP. Pour un soutien stable des auxiliaires chirurgicaux et de planification, au moins 3 miniSKYFRP sont nécessaires par maxillaire. Le protocole chirurgical correspond à celui illustré dans la Fig. 1. A la mandibule les mini1SKY peuvent servir immédiatement de base à une restauration provisoire. A cet effet, on meule des ouvertures sur la prothèse existante aux endroits correspondants à la position des implants et on y fixe par polymérisation les pièces femelles SKY à bague en O (REF SKY-OR50) ou les pièces femelles SKY en métal précieux (REF SKYGM225). Laboratoire FRP Les gabarits Scan et les gabarits de forage sont réalisés sur le modèle selon les procédés habituels en prothèse. Pour la confection des dents dans le gabarit Scan, nous recommandons X-resin CT/DVT (REF 540 0115 9). Pour la fixation des gabarits Scan et des gabarits de forage, on se sert des pièces femelles de planification mini1SKY. Elles seront polymérisées dans les gabarits Scan et les gabarits de forage. 7. Descrizione del prodotto Tutti gli impianti miniSKY sono in titanio, con diametro ridotto e doppia filettatura autofilettante. mini1SKY 2.8: L’impianto monofasico mini1SKY è dotato di un attacco a sfera per l’ancoraggio di protesi rimovibili. Inoltre con gli abutment cementati è possibile realizzare protesi fisse. mini2SKY 2.8 und 3.2: L’impianto bifasico mini2SKY è disponibile in due diverse versioni ed è dotato di un Torx esterno, che permette la riabilitazione con diversi tipi di abutment e componenti per sovrastrutture. 2. Indicazioni / Campo d’applicazione Impianti miniSKY e componenti protesici miniSKY: mini1SKY 2.8: • Fissaggio di protesi totali su arcate atrofiche • Punti fissi di riferimento (FRP) per la pianificazione implantare, per il fissaggio della dima radiologica e chirurgica. Controindicazioni Il sistema di impianti miniSKY non è indicato per pazienti che presentano le normali controindicazioni per l’inserimento implantare, come p. es. problemi di cuore e di circolazione, bruxismo, osteoporosi, forti fumatori, alcolisti, diabete, malattie del ricambio ed insufficiente stabilità primaria per gli impianti. In caso di sospetto di intolleranze, questo prodotto deve essere utilizzato solo dopo previo accertamento allergologico ed attestazione dell’assenza di allergie. ATTENZIONE Se non vengono soddisfatte le seguenti condizioni, l’uso degli impianti miniSKY è controindicato. • Per evitare fratture su strutture a ponte o su elementi singoli non sono consentite estensioni. • L’osso intorno all’impianto deve avere una parete con uno spessore minimo di 1,5 mm. • Per mini1SKY 2.8 e mini2SKY 2.8: in caso di protesi totale dell’arcata superiore, nell’arcata inferiore non deve essere presente la dentatura naturale o protesi fisse su impianti. • In caso di ancoraggio di una protesi totale agli impianti mini1SKY 2.8, fare attenzione che il carico maggiore venga trasferito sui tessuti molli. È necessario un controllo, effettuando almeno un recall all’anno. • Utilizzando gli abutment mini2SKY uni.fit con il procedimento CAD/CAM possono essere realizzati abutment individuali solo in ossido di zirconio stabilizzato all’ittrio. Con l’abutment mini2SKY uni. fit possono essere realizzati solo abutment singoli. Le strutture a ponte su abutment individuali in zirconio dovrebbero essere realizzate solo in situazioni di equilibrio statico, con un rapporto di 2 impianti per 1 elemento del ponte. Inoltre la struttura a ponte deve essere realizzata senza estensioni. Il carico deve essere distribuito in modo assiale sul sistema impianto-abutment. Sono controindicati restauri con un’angolazione superiore a 20° rispetto all’asse dell’impianto. La lunghezza del restauro rispetto alla lunghezza dell’impianto deve avere un rapporto non superiore a 1:1.25. La larghezza del restauro non deve superare di una volta e mezza il diametro dell’impianto. • La soluzione protesica deve essere realizzata in modo tale da essere ben equilibrata. • Nel caso d’utilizzo di impianti per elementi singoli, la corona deve essere provvista di un’ampia superficie di contatto con i denti adiacenti. Inoltre la corona deve essere posizionata al di fuori dell’occlusone. Anche sotto carico non devono esserci contatti occlusali. • È necessario effettuare regolari recall per verificare l’insorgenza di possibili riassorbimenti ossei. Se il riassorbimento osseo ricopre l’impianto per più di un terzo della sua lunghezza, è necessario eseguire un’estrazione dell’impianto interessato, per evitare una frattura incontrollata. 4. Avvertenze sui pericoli e per la sicurezza I numeri di lotto di tutti i prodotti utilizzati devono essere documentati per garantire la rintracciabilità ed in caso di reclamo. 5. Avvertenze per la conservazione e la durata Gli impianti vengono forniti in confezione sterile e devono essere conservati nella confezione originale ed in un luogo asciutto. Non utilizzare mai confezioni danneggiate. I prodotti sterili non devono essere più utilizzati dopo la data di scadenza (vedere etichetta). Gli abutment ed i componenti per sovrastruttura sono confezionati in una busta sigillata, non sono sterili e devono essere conservati in un luogo asciutto e privo di polveri. 6. Lavorazione ATTENZIONE Consigliamo di eseguire una pianificazione chirurgica di massima e di scegliere i componenti protesici in base alla situazione ed al tipo di riabilitazione protesica pianificata. Le viti di fissaggio utilizzate in laboratorio non possono essere impiegate in ambito clinico, poiché queste in laboratorio sono sottoposte a carichi, e quindi non è più possibile garantire una sede sicura e di lunga durata. Per la lavorazione in laboratorio la bredent medical fornisce speciali viti da laboratorio, in colore marrone anodizzato. Tutti i componenti devono essere disinfettati prima del loro inserimento (vedere paragrafo 9). Après la désinfection, enlever soigneusement le produit de désinfection de toutes les pièces (voir à cet effet le mode d’emploi du fabricant du produit de désinfection, par exemple sous de l’eau courante pour ID212 forte). MISE EN GARDE Lors des examens de contrôle annuels veuillez contrôler l’assise correcte de la restauration prothétique et des éléments de sustentation. Contrôler également la position axiale des points de contact, vu que des sollicitations extra-axiales peuvent engendrer des desserrements de vis et ainsi à des fractures dues à la fatigue pouvant entrainer l’aspiration ou l’ingestion de fragments. Con un sollevamento mininvasivo dei lembi viene esposta la cresta ossea. Con la fresa pilota (REF SKY-DP08) viene determinata la posizione dell’impianto e viene eseguita una perforazione di ca 6,7 mm. 2. Con la fresa Twistdrill (REF SKY-DT13L) ø 1,3 mm viene determinata la direzione e la profondità dell’impianto. Le demarcazioni della fresa Twistdrill corrispondono alle diverse profondità di perforazione: 6 -8 -10 -12 -14 -16 mm. 3. In base al tipo di densità ossea viene eseguita la preparazione con il Twistdrill (REF SKY-DT23L) ø 2,25 mm dell’alveolo implantare (vedere fig. 1). Con lo stop di perforazione (REF SKY-ST06, SKY-ST08, SKY-ST10, SKY-ST12, SKY-ST14) corrispondente alla lunghezza dell’impianto si esclude una perforazione più profonda. 4. Per gli impianti mini2SKY 3.2 è necessario proseguire la preparazione, in base alla densità ossea, come segue: Densità ossea D1: In osso duro si esegue la preparazione dell’alveolo implantare con la fresa SKY per osso duro (REF SKYD1235) a 300 giri/min per l’intera lunghezza dell’impianto. Densità ossea D2-D3: La preparazione in presenza di osso medio-duro e morbido si esegue con la fresa SKY per osso medio-duro e morbido (REF SKYD3435) a 300 giri/ min per la lunghezza dell’impianto. Densità ossea D4: in osso morbido la preparazione viene terminata con il Twistdrill (REF SKYDT23K, SKYDT23L). In base alla lunghezza dell’impianto, sono disponibili i corrispondenti stop di perforazione con ø 4,0 mm (REF SKYS0840, SKYS1040, SKYS1240, SKYS1440), che prevengono una perforazione più profonda della cavità. ATTENZIONE Durante la perforazione evitare di perforare più volte, per non compromettere la stabilità primaria degli impianti miniSKY. 5. Gli impianti sono confezionati in modo sterile. Dopo aver rimosso la pellicola protettiva, con l’aiuto dell’apposito mounter avvitare l’impianto manualmente eseguendo 2-3 rotazioni. Per l’inserimento definitivo, l’impianto viene inserito con il mounter per micromotore chirurgico (REF mSKYXWM6, mSKYXWM7). Il mounter può essere utilizzato direttamente con il micromotore chirurgico (15 – 25 giri/min) o con la chiave chirurgica ed il relativo adattatore (REF SKYADAP6). In questa fase si deve fare attenzione che l’impianto venga avvitato in modo delicato e senza surriscaldamento fino alla profondità stabilita (fino alla fine della superficie Osseo-Connect). Il momento torcente massimo è di 45 Ncm. 6. Con gli impianti mini1SKY la protesi viene scaricata in corrispondenza degli impianti per evitare che durante la fase dell’osteointegrazione le fixture siano sottoposte a carico. Gli elementi di ritenzione vengono aggiunti solo dopo ca. 12 settimane. Con gli impianti mini²SKY durante l’osteointegrazione viene utilizzato l’adattatore gengivale (REF m2SKYGF3). 7. Il carico immediato degli impianti miniSKY è consentito solo in caso di protesi provvisorie. 8. Per il successivo inserimento della protesi si devono considerare i seguenti momenti torcenti: • abutment MD mini²SKY in titanio/BioXS, abutment miniSKY Locator®, abutment mini²SKY uni.fit: 20 Ncm • adattatore gengivale mini²SKY, transfert da impronta mini²SKY: 10 Ncm Presa dell’impronta: mini1SKY: Per la presa dell’impronta degli impianti mini1SKY viene inserito un anello O-Ring (REF SKY-OR55) sull’attacco a sfera (vedere fig. 2). Successivamente il Torx degli impianti mini1SKY e gli anelli O-Ring vengono completamente coperti con il materiale da impronta brecision implant light (REF 580BL050). Contemporaneamente viene versato il materiale da impronta brecision implant heavy (REF 580BH380) in un cucchiaio e viene applicato sulla zona dove deve essere presa l’impronta. Quando il materiale da impronta si è indurito è possibile estrarre il cucchiaio con cautela. Gli anelli O-Ring Fig.2 rimangono nell’impronta. Posizionamento Al momento del riposizionamento degli analoghi implantari (REF m1SKYXIA) è necesdell’anello sario fare attenzione che la geometria del Torx nel materiale da impronta concordi O-Ring con la direzione del Torx dell’analogo implantare. ATTENZIONE Bloccare gli analoghi implantari con l’aiuto di una mascherina per finte gengive Multisil-Mask hart (REF 540 0113 4), al fine di garantire la posizione esatta degli analoghi implantari durante tutta la fase di modellazione. Successivamente proseguire la modellazione come di consueto. Per una lavorazione sicura, è necessario che i tessuti molli siano al di sotto del Torx. In tal modo si garantisce una presa dell’impronta precisa e una ritenzione sicura dell’anello O-Ring. In caso di altezza gengivale troppo elevata, l’anello O-Ring può essere sottoposto a tensione, causando imprecisioni durante la presa dell’impronta o una prematura usura al momento del fissaggio della protesi. mini²SKY Per la presa dell’impronta degli impianti mini²SKY, viene avvitato il transfert da impronta mini²SKY per impronte finestrate (REF m2SKYPA1) con un momento torcente di 10 Ncm sugli impianti e si prende l’impronta seguendo la consueta procedura per la tecnica delle impronte finestrate. Per realizzare il modello master vengono avvitati gli analoghi implantari mini²SKY (REF m2SKYXIA) con il transfert da impronta mini²SKY e viene realizzato il modello con un gesso extra-duro. Protesi: mini1SKY: Con gli impianti mini1SKY, prima di assemblare le matrici nella resina e polimerizzarle, viene condizionata la superficie esterna del contenitore per matrici con biossido di alluminio da 110 µm. I sottosquadri e gli spazi intermedi delle papille devono essere scaricati per garantire, che la resina non penetri negli spazi sottostanti, evitando che la protesi si disinserisca. In caso di utilizzo delle matrici in lega aurea SKY è necessario leggere attentamente le „Istruzioni d’uso delle matrici in lega aurea SKY. mini1SKY e mini2SKY: Gli abutment MD miniSKY possono essere individualizzati seguendo le consuete metodiche odontotecniche. L’interfaccia impianto-abutment non può essere individualizzato. Durante la riduzione dell’altezza o l’individualizzazione dell’abutment si deve fare attenzione a non danneggiare la vite dell’abutment. E’ necessario verificare, che lo spessore della parete e l’altezza dell’abutment siano sufficienti rispetto alla lunghezza della corona. mini2SKY: Gli abutment miniSKY Locator® non devono mai essere rifiniti e possono essere avvitati subito dopo l’inserimento implantare come adattatori gengivali. Il carico immediato non è consentito. La protesi deve essere scaricata in corrispondenza degli impianti per evitare che durante la fase dell’osteointegrazione le fixture siano sottoposte a carico. Solo dopo l’avvenuta osteointegrazione vengono aggiunti gli elementi ritentivi. In questa fase deve essere sostituita la matrice di ricambio da taratura nera con la corrispondente matrice in base al grado di frizione stabilito. Se deberá realizar un Recall una vez al año. • Con el aditamento uni.fit mini2SKYsolo se podrá realizar aditamentos individuales sobre óxido de circonio de itri estabilizado en el proceso de CAD/CAM. Solo se podrá realizar con el aditamento uni.fit mini2SKY aditamentos unitarios. Trabajos de puentes sobre aditamentos individuales siempre que haya una estabilidad completa correcta sobre 2 implantes y una pieza intermedia. El puente se deberá realizar sin ninguna extensión. La carga deberá realizarse de forma axial sobre el sistema de aditamentos del implante. La restauración con una angulación mayor de 20° sobre el eje del implante están contraindicados. La longitud de la restauración hacia la longitud del implante no deberá sobre pasar la relación de 1:1,25. El ancho de la restauración no deberá sobre pasar el ancho del implante en 1,5. • La solución protésica deberá estar diseñada de tal forma que resulte bien equilibrada. • Las prótesis de un solo diente deben apoyarse en superficies grandes de los dientes adyacentes. Además debe disponerse la corona de modo que no haya oclusión. Ni siquiera con carga deben producirse contactos por oclusión. • La posible resorción ósea se deberá controlar mediante revisiones periódicas. Si la resorción alrededor del implante es superior a un tercio de la longitud del implante, debería procederse a explantar el implante afectado para evitar una fractura incontrolada. 4. Consejo de almacenamiento y durabilidad Todos los números de lote de las piezas utilizadas se deberán documentar para poder garantizar su trazabilidad y poder así tener derecho a realizar una reclamación. 5. Consejos de almacenamiento y durabilidad Los implantes están envasados en estado estéril y deben almacenarse en un lugar seco en su embalaje original. Hay que comprobar que los envases con productos estériles estén intactos. No está permitido el uso de productos de envases dañados. Los productos estériles no pueden aplicarse una vez alcanzada su fecha de caducidad (consultar etiqueta). Los aditamentos y elementos de montaje están envasados en bolsas tubulares, se suministran de forma no estéril y deben almacenarse en un lugar seco y libre de polvo. 6. Procesamiento ATENCIÓN Recomendamos por principio realizar una planificación quirúrgica minuciosa y una elección de los implantes y los elementos protésicos dependiendo de la situación y la solución indicada. Los tornillos de sujeción utilizados en el laboratorio no pueden aplicarse en el ámbito clínico ya que están supeditados a las cargas soportadas en el laboratorio, por lo que no puede garantizarse una fijación segura a largo plazo. Todas las piezas deberán desinfectarse antes de su inserción (véase el punto 9). Contraindicaciones El sistema de implantes miniSKY no es adecuado para pacientes que presentan las contraindicaciones habituales en implantología, como por ejemplo: afecciones cardiocirculatorias, bruxismo, osteoporosis, fumadores empedernidos, alcohólicos, diabetes y enfermedades metabólicas, así como estabilidad primaria insuficiente de los implantes. En el caso de sospecha de intolerancia sólo se aplicará este producto tras haber realizado las pruebas alergológicas pertinentes y haber demostrado la inexistencia de una alergia. Non riutilizzabile Per assemblare la matrice nella resina e polimerizzarla, prima viene condizionata la superficie del contenitore della matrice con biossido di alluminio da 110 µ e l’abutment Locator® viene coperto con una diga in gomma. In questo caso si consiglia, di non perforare la diga in gomma sulla zona dell’impianto, per garantire, che la resina non penetri nella zona sottostante e che la protesi non si disinserisca. mini2SKY uni.fit Abutment: Costruzione Per la lavorazione con un Software CAD è necessario inserire il record di dati degli abutment mini2SKY uni. fit nel database, che deve essere reso disponibile per il Workflow. Per realizzare un abutment individuale é necessario attenersi ai dati del produttore del software. Se dovesse essere attivato l’automatismo per il gap virtuale del cemento, si deve fare attenzione che il gap del cemento nell’abutment in zirconio non superi i 30-50 μm. Se necessario realizzare un campione, con il quale calibrare l’adattamento dell’abutment mini2SKYuni.fit. Il campione / abutment in zirconio deve essere applicato senza frizione sulla base da incollaggio in titanio. Se con il campione / abutment in zirconio dovesse verificarsi un gioco di rotazione, sia pur minimo, è necessario ridurre il gap del cemento nel software CAD. Si consiglia di realizzare una mascherina per finte gengive rimovibile del modello, che sia anch’essa scansionabile. Per evitare di rifinire l’abutment in zirconio ultimato, è necessario che l’abutment individuale venga realizzato nella costruzione già con il profilo di emergenza necessario. Il design dell’abutment non dovrebbe superare un’altezza di ca. 10 mm. CAM/Software/Realizzazione L’abutment realizzato deve essere posizionato nel modulo Nesting, in modo tale che la zona delle cavità dell’abutment, che da un lato crea lo spazio per l’inserimento della base da incollaggio mini2SKY uni. fit e dall’altro il canale della vite, sia regolata in direzione Z su 0°. L’abutment mini2SKY uni.fit viene pianificato in modo tale da poter essere realizzato anche con un fresatore a 3 assi + 1. È assolutamente necessario fare attenzione al valore di contrazione dello zirconio utilizzato. Se le dimensioni in scala indicate sono errate possono portare ad imprecisioni tra la base da incollaggio e l’abutment in zirconio. Incollaggio Per l’incollaggio in laboratorio dell’abutment individuale con la base in titanio mini2SKY uni.fit si consiglia di procedere come segue: • sabbiare le zone di incollaggio della base in titanio mini2SKY uni.fit e dell’abutment individuale con 110 μm di biossido di alluminio. Il Torx e la zona dei tessuti molli non devono essere sabbiati! Per proteggere il Torx, la base da incollaggio in titanio mini2SKY uni.fit viene avvitata sull’analogo del modello, viene coperta la zona dei tessuti molli e poi viene eseguita la sabbiatura. • si consiglia di utilizzare il set dell’adesivo DTK per l’incollaggio di abutment individuali in zirconio (REF 540 0117 5, della ditta bredent GmbH & Co. KG) o l’adesivo Panavia™ F 2.0. Con l’abutment mini2SKY uni.fit si deve procedere con la massima prudenza possibile, per evitare danni dovuti ad una lavorazione errata. Inserimento Prima dell’inserimento i componenti devono essere disinfettati. Gli abutment mini2SKYuni.fit vengono posizionati sull’impianto in modo analogo alla posizione sul modello ed avvitati con un momento torcente di 20 Ncm. Lavorazione come sistema FRP Inserimento degli impianti miniSKY-FRP Gli impianti miniSKY-FRP vengono inseriti, a livello transgengivale, 2 - 3 settimane prima del termine programmato per l’inserimento implantare. Al momento della scelta degli impianti FRP si deve tener conto, delle posizioni degli impianti, della direzione d’inserzione e delle sezioni di innesto. Onde evitare una lesione a strutture anatomiche importanti, si deve assolutamente fare attenzione che i miniSKYFRP abbiano una lunghezza adeguata. Per un sostegno stabile degli strumenti ausiliari chirurgici sono necessari almeno 3 impianti miniSKY-FRP per arcata. Il procedimento chirurgico è analogo a quello illustrato nella fig. 1. Gli impianti mini1SKY possono essere utilizzati subito nell’arcata inferiore come base per la protesi provvisoria. A questo scopo la protesi preesistente viene scaricata sulle posizioni degli impianti e vengono inseriti e polimerizzati gli anelli SKY O-Ring (REF SKY-OR50) e le matrici in lega aurea SKY (REF SKYGM225). Laboratorio per il sistema FRP Sul modello vengono realizzate la dima radiologica e chirurgica come di consueto. Per realizzare i denti nella dima radiologica si consiglia di utilizzare il prodotto X-resin CT/DVT (REF 540 0115 9). Per il fissaggio della dima radiologica e chirurgica vengono utilizzate le matrici di posizionamento bianche mini1SKY. Queste vengono inserite e polimerizzate nella dima radiologica e chirurgica. 7. Ulteriori avvertenze Il prodotto deve essere utilizzato solo da odontoiatri, odontotecnici e personale specializzato, opportunamente addestrato. Per la lavorazione si devono utilizzare solo strumenti e componenti originali. Tutti i componenti devono essere disinfettati prima della loro applicazione nel cavo orale del paziente. Si devono utilizzare solo appositi disinfettanti per strumenti, p.es. Dürr ID 212 forte. Attenersi alle istruzioni per l‘uso del produttore del disinfettante utilizzato. Dopo la disinfezione rimuovere ogni traccia di disinfettante dai componenti (vedere le istruzioni d’uso del produttore del disinfettante utilizzato, p.es. nel caso di ID212 forte, ATTENZIONE In fase di recall del paziente è necessario verificare che la protesi e gli elementi di tenuta abbiano una sede corretta e stabile. E’ opportuno ricontrollare i punti di contatto, poichè i carichi fuori asse possono provocare allentamenti delle viti e quindi possono verificarsi fratture da sovraccarico, i cui frammenti possono essere accidentalmente aspirati od ingeriti. Se gli abutment con avvitamento occlusale devono essere rimossi per un intervento di igiene professionale, è necessario sostituire le viti, per evitare possibili allentamenti o fratture. Tutti i prodotti contrassegnati con il simbolo sono previsti per un unico utilizzo. Nel caso il prodotto venisse utilizzato più volte, possono insorgere i seguenti rischi: contaminazione crociata, difetti funzionali, imprecisioni ecc. Descripción del producto Todos los miniSKY Implantes son implantes de titanio de diámetro reducido con doble rosca auto cortante para la inserción en el hueso maxilar. mini1SKY 2.8: El implante mini1SKY monofásico dispone de una bola para la fijación de la prótesis removible. Al parte de eso se podrá utilizar un aditamento para cementar la estructura y realizar un trabajo fijo. mini2SKY 2.8 y 3.2: El implante mini2SKY bifásico se ofrece en dos variantes y dispone de un Torx externo, que se podrá manipulado con diferentes aditamentos y piezas de montaje. 2. Indicaciones miniSKY Implante y miniSKY Prótesis: mini1SKY 2.8: • Fijación de completas en el maxilar atrofiado • Puntos de referencia fijo este (FRP) para planificación de implantes, para fijar la férula de escaneo y de perforación. Estos implantes se podrán integrar con los aditamentos en la estructura de puente cementándolos. • Como implantes temporales para la fijación de la prótesis temporal. mini2SKY 2.8: • en espacios estrechos en el maxilar inferior (32-42) y superior grupo anterior (12, 22) • Bloqueo de tratamientos en grupos anteriores inferior (32-42) • Fijación de completas en el hueso atrofiado en el maxilar inferior con el miniSKY Locator mini2SKY 3.2: • en espacios estrechos en el maxilar inferior (32-42) y superior grupo anterior (12, 22) • Pieza unitaria inferior (35-45) y superior (15-25) • Bloqueo de puentes en un maxilar muy atrofiado • Fijación de completas en el hueso atrofiado en el maxilar inferior con el miniSKY Locator 1. Se descubre la cresta ósea mediante una incisión mínimamente invasiva. Con la fresa piloto (REF SKY-DP08) se determina la posición del implante y se hunde aproximadamente 6,7 mm. 2. A continuación se determina la dirección y la profundidad del implante con el twistdrill (REF SKY-DT13L ) de ø 1,3 mm. La graduación de los twistdrills se corresponde con las profundidades de perforación: 6 -8 -10 -12 -14 -16 mm. 3. Dependiendo de la calidad ósea se prepara la base del implante con el twistdrill (REF SKY-DT23L) de ø 2,25 mm (ver imagen 1).Según la longitud del implante , utilizar los topes de profundidad correspondiente (REF SKY-ST06, SKY-ST10, SKY-ST12, SKY-ST14). 4. Para los implantes mini2SKY 3.2 se deberá realizar la siguiente preparación según el tipo de hueso que se tenga: D1 Hueso: En el hueso duro se preparará con la fresa SKY para este tipo de hueso (REF SKYD1235) la base del implante a unas 300 rev/min a la longitud correspondiente definitiva. D2-D3 Hueso: La preparación del hueso medio y blando se utilizará la fresa SKY correspondiente para hueso medio y blando (REF SKYD3435) a 300 rev/min a la longitud correspondiente. D4 Hueso : En hueso blando se deberá realizar la preparación con el Twistdrill (REF SKYDT23K, SKYDT23L) Según la longitud del implante se utilizará el tope de profundidad correspondiente de ø 4,0 mm (REF SKYS0840, SKYS1040, SKYS1240, SKYS1440) con la cual se preparará la cavidad para asegurar la profundidad. ATENCION mini²SKY Para la toma de impresión del implante mini²SKY se utilizará el aditamento de impresión para mini²SKY para toma de impresión abierta (REF m2SKYPA1), que se atornillará sobre el implante a 10Ncm y se tomará la impresión como de costumbre. Para la creación del modelo maestro se utilizarán los análogos de implantes (REF m2SKYXIA) con el aditamento de impresión mini²SKY que se atornillarán encima del implante y se vaciará con escayola súper dura. Prótesis: mini1SKY: Si se utiliza el implante mini1SKY se arenará con óxido de aluminio 110 µm el exterior de la caja de hembra antes de polimerizarla en la prótesis. Las zonas retentivas y las zonas de las papilas se deberán aliviar, para evitar que la resina pueda colarse en esas zonas e imposibilite la extracción de la prótesis. Al aplicar la matriz de metal noble SKY habrá que seguir las “Instrucciones de uso para la matriz de metal noble SKY. mini1SKY y mini2SKY: Los aditamentos miniSKY MD pueden personalizarse con los métodos habituales de la técnica dental. La conexión entre implante y aditamento no permite la personalización. Al reducir la altura o personalizar el aditamento no debe dañarse el tornillo del aditamento. Hay que tener presente que es necesario que se mantenga un grosor del tabique y una altura del aditamento suficientes que se correspondan con la longitud de la corona. mini2SKY: Los aditamentos miniSKY Locator® no se deben limar en ningún caso y pueden atornillarse inmediatamente tras la inserción del implante a modo de formadores de encía. No se permite una carga inmediata. La prótesis debe disponerse hueca. Una vez terminada la integración ósea se adaptan los elementos de retención. Para ello se sustituye el elemento de trabajo negro por el correspondiente suplemento de retención con el par de apriete determinado para ello. ATENCION Los elementos de retención Locator® 0°-10° (REF LOCLAB10) no pueden utilizarse junto con los aditamentos miniSKY Locator®. Para polimerizar las matrices se prepara previamente la superficie de la carcasa de la matriz con óxido de aluminio 110 µm y el aditamento Locator® se cubre con un dique de contención. Recomendamos que el dique de contención no se perfore en el lugar del implante para garantizar que no penetre resina en zonas superpuestas, obstaculizando así la extracción de la prótesis. mini2SKY Aditamento uni.fit: Construcción Para poder trabajar con el CAD Software se deberá tener en el banco de datos el aditamento uni.fit del mini2SKY y poder estar disponible en el Workflow. Para la creación de un aditamento individual se tener en cuenta los datos de fabricante del software. Se se hubiese habilitado un espacio de cemento automáticamente , se deberá tener en cuenta que el espacio del cemento en un aditamento de circonio no deberá sobre pasar los 30 a 50 µm. Se aconseja realizar un elemento de prueba con el ajuste del aditamento del uni.fit para el mini2SKY. El elemento de prueba / aditamento de circonio deberá entrar sin fricción sobre la base de titanio. Si hubiese una leve a gran rotación del elemento de prueba / aditamento de circonio sobre la interface de titaniio se deberá calibrar el CAD Software para reducir el espacio del cemento. Se aconseja realizar el modelo a escanear con una encía falsa removible. Para evitar tener que retocar el aditamento de circonio, se deberá incluir la construcción en el modelo el aditamento individual con un perfil de emergencia. El diseño del aditamento no deberá sobrepasar una altura de aprox. 10 mm. CAM/Software /Terminación La construcción del aditamento deberá estar guardado en el Nestingmodulo de tal manera que las cavidades del aditamento , que forma el espacio de la posición del aditamento uni.fit interface de tiantio min2SKY con el canal del tornillo, en posición Z a 0°. El aditamento uni.fit mini2SKY esta creado de tal manera que se pueda fabricar en maquinas de fresado de 3+1 eje. El factor de contracción del circonio utilizado se deberá tener en cuenta. Cualquier escalado tamaño incorrecto puede conducir a desajustes entre la base adhesiva y el pilar de circonio. Adhesivos La adhesión del aditamento individual uni.fit interface de titanio min2SKYse aconseja realizar en el laboratorio: • Arenar las zonas del aditamento individual con la interface de titanio del uni.fit mini2SKY con óxido de aluminio de 110 µm. Las zonas del Torx y zonas gingivales NO se deberán arenar. Para la protección del Torx se colocará la interface de titanio uni.fit del mini2SKYsobre un análogo y se protegerá la zona gingival y proceder con el arenado. • Aconsejamos como adhesivo el Surtido DTK (REF 540 0117 5 bredent GmbH & Co.KG) o Panavia TM F 2.0. Se deberá ir con cuidado en el aditamento uni.fit mini2SKY para evitar una incorrecta manipulación de la pieza. Introducción Antes de la introducción de las piezas se deberán desinfectar. Los aditamentos uni.fit min2SKY se posicionarán en la misma posición que estaba en el modelo y se enroscará con un momento de torsión de 20 Ncm. Trabajo con FRP Inserción de los miniSKY-FRP Los miniSKY-FRP se insertan de 2 a 3 semanas antes de la fecha planificada para la implantación. Para elegir la posición FRP hay que tener en cuenta las posiciones definitivas de los implantes, el abordaje de la incisión y las zonas que requieren aumento óseo. Para evitar lesionar estructuras anatómicas importantes es absolutamente imprescindible tener en cuenta la longitud adecuada de los miniSKY-FRP. Para conseguir un apoyo estable de los elementos auxiliares de planificación y quirúrgicos se requiere al menos 3 miniSKY-FRP por mandíbula. El procedimiento quirúrgico es análogo a lo demostrado en la fig. 1. Los mini1SKY pueden utilizarse en el maxilar inferior inmediatamente como base para soluciones provisionales. Para ello se rebaja la prótesis existente en las posiciones del implante y se polimerizan las matrices con junta tórica SKY (REF SKY-OR50) o las matrices de metal noble SKY (REF SKYGM225). Laboratorio FRP Las plantillas para escanear y perforar se confeccionan sobre el modelo mediante los procedimientos técnico dentales habituales. Para la confección de los dientes en la plantilla de escaneo recomendamos el producto X-resin CT/DVT (REF 540 0115 9). Para la fijación de la plantilla de escaneo y perforación se usan las matrices de planificación blancas mini1SKY. Estas se polimerizan dentro de la plantilla de escaneo y perforación. 7. Los implantes están esterilizados dentro de su envase. Tras apartar la lámina protectora puede atornillarse el implante manualmente dándole 2 a 3 vueltas con la ayuda de un soporte para implantes. Para completar la inserción se acopla el implante con el instrumento de inserción (REF mSKYXWM6, mSKYXWM7). El instrumento de inserción puede utilizarse directamente con el contra ángulo (15 – 25 rev/min) o con la carraca en combinación con el adaptador para la carraca (REF SKYADAP6). Hay que procurar que el implante se enrosque con cuidado y sin sobrecalentamiento hasta la longitud definitiva (hasta el final de la superficie Osseo-Connect). El momento de giro máximo es de 45 Ncm. 6. Cuando se usa los implantes mini1SKY, la prótesis se dispone hueca en la posición del implante. Los elementos de retención se adaptan transcurridas aproximadamente 12 semanas. Con los implantes mini²SKY se utiliza el formador de encía (REF m2SKYGF3) durante la integración ósea. 7. La carga inmediata en los implantes miniSKY sólo está permitida para prótesis provisionales. 8. Para la posterior incorporación de la prótesis se deberá tener en cuenta los siguientes pares de apriete: • mini2SKY MD-Aditamento Titanio/BioXS, miniSKY Locator®Aditamento, mini²SKY, aditamento uni. fit: 20 Ncm • mini2SKY Formador de encía, mini²SKY pilar de impresión: 10 Ncm Toma de impresión: mini1SKY: La toma de impresión del implante mini1SKY se realizará con el O-Ring (REF SKYOR55) que se colocará sobre la cabeza de la bola (ver imagen 2). A continuación se cubrirá el torx del implante mini1SKY y el O-Ring con material de impresión brecision implant light (REF 580 BL05 0). Al mismo tiempo se preparará la cubeta con material de impresión brecision implant heavy (REF 580 BH38 0) y posicionado sobre la zona donde se quiera tomar la impresión. Una vez haya fraguado la silicona se retira con Imagen 2: cuidado. Los O-Ring se quedarán en la impresión. Posición del Durante la reposición de los análogos de implante (REF m1SKYXIA) se deberá tener O-ring en cuenta que la geometría del Torx coincida con el Torx del análogo del implante. ATENCION Con ayuda de la encía falsa Multisil-Mask duro (REF 540 0113 4) se deberán bloquear los análogos de los implantes, garantizando la correcta posición de los análogos de implantes durante toso el proceso de creación del modelo. La terminación de creación del modelo se realizará con los métodos normales en la técnica dental. Para una correcta manipulación, deberá estar la encía blanda por debajo del Torx, sellando la zona. Así se garantiza una impresión segura y la correcta fijación del O-ring. Otras observaciones Este producto sólo puede ser utilizado por odontólogos, técnicos dentales y personal cualificado con formación específica. Para el procesamiento sólo se utilizará herramientas y piezas originales. Todas las piezas deberán desinfectarse antes de su implantación en la boca del paciente. Sólo está permitido el uso de desinfectantes para instrumental como, por ejemplo, Dürr ID 212 forte. Es necesario seguir las indicaciones del fabricante a la hora de utilizar el desinfectante. Después de la desinfección deberá eliminarse a fondo el desinfectante de las piezas (consultar para ello las indicaciones del fabricante del desinfectante como, por ejemplo, lavar bajo el grifo de agua en el caso de ID212 forte). ATENCION Durante el Recall anual del paciente se deberá comprobar el correcto asentamiento de los elementos de retención . La Posición axial de los puntos de contacto se deberán comprobar también, para evitar cualquier aflojamiento de tornillo, que llevan a posible rotura del mismo y poder ser tragados o aspirado . Si se retirasen los aditamentos para su higiene se deberá utilizar nuevos tornillos de fijación, para evitar aflojamientos de tornillos o roturas. Todos los productos marcados con el símbolo son exclusivamente de un solo uso desechables. Si se reutilizase el producto otra vez podría producirse los siguientes riesgos: contaminación cruzada, fallo de funcionamiento y desajuste, etc..
