CTX-M-producing Escherichia coli and Klebsiella pneumoniae isolated from community-acquired urinary tract infections in Valledupar, Colombia. Martinez P1, Garzón D, Mattar S. ESBL production was detected in 12 (11.7%) E. coli and four (19%) K. pneumoniae isolates. TEM ESBLs were detected in seven E. coli and three K. pneumoniae isolates. SHV ESBLs were found in four K. pneumoniae isolates. CTX-M-1 phylogenetic subgroup was positive in seven E. coli and three K. pneumoniae isolates. CMY-2 β-lactamase gene was detected in nine E. coli and one K. pneumoniae isolates. A significant association of ESBL expression in E. coli was observed with resistance to tobramycin (p≤0.001), tetracycline (p=0.043), and ciprofloxacin (p≤0.001). In K. pneumoniae isolates, significant association was found with resistance to tobramycin and ciprofloxacin (p=0.006), and trimethoprim-sulfamethoxazole (p=0.043). Emergence of resistance to third generation cephalosporins by Enterobacteriaceae causing community-onset urinary tract infections in hospitals in Colombia. Authors’ conclusions No differences were observed between the classes of antimicrobials included in this review for the symptomatic cure of acute uncomplicated UTI. Fluoroquinolones proved more effective than beta-lactams for the short-term bacteriological outcome, probably with little clinical significance. Individualised treatment should take into consideration the predictable susceptibility of urinary pathogens in local areas, possible adverse events and resistance development, and patient preference. It is a common event in otherwise healthy, non-pregnant adult women. A large range of antimicrobials are used in the treatment of cystitis. Twenty one good quality studies, enrolling 6016 participants, which used different Ertapenem versus ceftriaxone for the treatment of complicated infections: a meta-analysis of randomized controlled trials. Bai N1, Sun C2, Wang J1, Cai Y2, Liang B1, Zhang L3, Liu Y3, Wang R4. Eight RCTs, involving 2 883 patients, were included in our meta-analysis. Ertapenem was associated with similar clinical treatment success with ceftriaxone for complicated infections (1 326 patients, fixed-effect model, OR: 1.13, 95% CI: 0.75-1.71). There was no difference between the compared treatment groups with regard to the microbiological treatment success, and no difference was found with regard to the incidence of clinical and laboratory drug-related adverse events between ertapenem and ceftriaxone groups. As to local tolerability, overall, there was no difference between the compared groups; however, in the subgroup analysis, local reaction was significantly less in the ertapenem subgroup than the ceftriaxone plus ceftriaxone subgroup. CONCLUSIONS: Ertapenem can be used as effectively and safely as ceftriaxone for the treatment of complicated infections. No hay diferencia entre betalactamicos Use el de menos espectro, no se asocia a fallo terapéutico ERROR # 5: DAR ANTIBIOTICOS PARA REDUCIR LA POSIBILIDAD DE ITU UNA VEZ INSERTADO UN CATETER VESICAL Antibiotic prophylaxis for short-term catheter bladder drainage in adults To determine if certain antibiotic prophylaxes are better than others in terms of prevention of urinary tract infections, complications, quality of life and cost-effectiveness in short-term catheterisation in adults. . SELECTION CRITERIA: All randomised and quasi-randomised trials comparing antibiotic prophylaxis for short-term (up to and including 14 days) catheterisation in adults. MAIN RESULTS: Six parallel-group randomised controlled trials with 789 participants met the inclusion criteria. All six trials compared antibiotic prophylaxis versus no prophylaxis. Studies presented a low to unclear risk of bias with similar interventions and measured outcomes.The primary outcome of bacteriuria was less common in the prophylaxis group amongst surgical patients with asymptomatic bacteriuria (I(2) = 0; risk ratio (RR) 0.20; 95% confidence interval (CI) 0.13 to 0.31) . Two nonsurgical studies could not be combined in a meta-analysis due to heterogeneity and only one showed significantly fewer cases of bacteriuria (RR 0.19; 95% CI 0.09 to 0.37).Two trials of surgical patients with asymptomatic bacteriuria only (255 participants) compared one type of antibiotic prophylaxis with another and neither study showed a significant difference in cases of bacteriuria.One study (78 participants) compared antibiotic prophylaxis in patients at catheterisation only versus antibiotic prophylaxis throughout catheterisation period with asymptomatic bacteriuria. Antibiotics at catheterisation only, resulted in significantly fewer cases of bacteriuria than giving prophylaxis throughout the catheterisation period (RR 0.29 95% CI 0.09 to 0.91).Secondary data of pyuria were provided by two surgical studies (255 participants). When studies were pooled, pyuria occurred in significantly fewer cases in the prophylactic antibiotic group (RR 0.23, 95% CI 0.13 to 0.42). The number of gram-negative isolates in patients' urine just before catheter removal in one study (RR 0.05, 95% CI 0.00 to 0.79) and six weeks after hospital discharge (RR 0.36, 95% CI 0.23 to 0.56) were significantly lower. There were no events in the treatment group before catheter removal. When pooled data from two studies showed significantly reduced febrile morbidity in those receiving antibiotic prophylaxis (RR 0.53 95% CI 0.31 to 0.89).Although all studies assessed microorganisms isolated from the urine specimens the data were too heterogenous to pool in a meta-analysis and have been provided in a narrative form. Further secondary data such as economic analysis, length of stay and quality of life were not covered in detail. AUTHORS' CONCLUSIONS: The limited evidence indicated that receiving prophylactic antibiotics reduced the rate of bacteriuria and other signs of infection, such as pyuria, febrile morbidity and gram-negative isolates in patients' urine, in surgical patients who undergo bladder drainage for at least 24 hours postoperatively. There was also limited evidence that prophylactic antibiotics reduced bacteriuria in non-surgical patients. AUTHORS' CONCLUSIONS: No eligible trials were identified that compared alternative routes of catheter insertion. The data from eight trials comparing different antibiotic policies were sparse, particularly when intermittent catheterisation was considered separately from indwelling catheterisation. Possible benefits of antibiotic prophylaxis must be balanced against possible adverse effects, such as development of antibiotic resistant bacteria. CONCLUSIONS: Patients admitted to hospital who undergo short term urinary catheterization might benefit from antimicrobial prophylaxis when the catheter is removed as they experience fewer subsequent urinary tract infections. Material y método Estudio de cohortes prospectivo, de pacientes mayores de 18 años de edad, atendidos en un servicio de urgencias hospitalario entre el 01/02 y el 31/05 de 2007 por síntomas miccionales, acompañados de piuria (>10leucocitos/mm3 de orina no centrifugada) o tira reactiva positiva para nitritos-leucocitos y urocultivo con >103 unidades formadoras de colonias. Se consideró que el tratamiento antibiótico prescrito empíricamente era adecuado si la bacteria aislada era sensible y se había prescrito por un tiempo correcto. Se consideró que hubo fracaso terapéutico cuando persistía la sintomatología tras 5 días de tratamiento. Se registraron la edad, el sexo, la presencia de sonda vesical permanente, el tipo de ITU, hospitalización en los 3 meses previos, la procedencia del paciente y las enfermedades asociadas. Resultados Se aislaron 177 bacterias en 168 pacientes. En 29 casos (17,3%) el microorganismo aislado era resistente al antibiótico prescrito. En tan solo 6 pacientes hubo fracaso terapéutico, aunque no precisaron ingreso hospitalario. Los pacientes que vivían en una residencia de ancianos (13,7 vs 2,2%, p=0,015) o que habían sido ingresados durante los 3 meses previos (20,6 vs 4,3%, p=0,039) presentaron un mayor riesgo de recibir tratamiento inadecuado.
