______ I understand Mass attendance is vital to my family’s spiritual growth and well being. I also understand that
it will be expected of our family to use the Mass Offering envelopes to track our mass attendance. Seventy five
percent (75%) attendance is expected and credit for the school may not be given if the minimum number of mass
dates is not met.
______ Tengo entendido que la asistencia a Misa es vital para el crecimiento spiritual y el bien de mi familia.
También tengo entendido que va ser esperado de nuestra familia usar los sobres para la colección en la Misa para
poder estar al tanto de nuestra asistencia. Setenta y cinco por ciento de asistencia es esperada y crédito para la
escuela pueda que no se dé si no cumplimos con lo mínimo de asistencias a misa.
PARENT MEETING ATTENDANCE

______ I understand regular attendance to class and PARENT MEETINGS is mandatory and my child may not
receive credit for the school year with a poor attendance record.
______ Entiendo que la asistencia regular a clase y JUNTAS DE PADRES es mandatorio y mi niño no recibirá
crédito para el año escolar si tiene asistencia pobre.
YOUTH MINISTRY ATTENDANCE

______ I understand that Youth Ministry is a required part of my child’s Sacrament Preparation. I will enroll my
child in either the Middle School Teen Club for 6 th-8th grades which meets on Tuesday evenings 6pm to 8pm, or the
High School Youth Group Ministry which meets on Wednesday evenings from 6pm-8pm. These ministries begin
the week of September 18th.
______ Tengo entendido que el Ministerio de Jóvenes es requerido como parte de la Preparación para el
Sacramento de mi hijo/a. Voy a inscribir a mi hijo/a en Middle School Teen Club para grados de 6to a 8vo que se
juntan Martes en las tardes de las 6pm a las 8pm, o en High School Youth Group que se juntan los Miércoles en las
tardes de las 6pm a las 8pm. Los ministerios empiezan la semana del 18 de septiembre.

______ I realize tardiness is a distraction to the class and will get my child to class on time. In
the event of a tardy I understand it is MY responsibility to walk my child to the RE office, fill
out a tardy slip and escort my child to the classroom. Realizo que llegar tarde es una distracción
para la clase y traeré a mi hijo/a a la clase a tiempo. En caso de que lleguemos tarde, es MI
responsabilidad de caminar a mi niño a la oficina de Educación Religiosa, llenar un pase y
llevar a mi hijo/a a su salón.
______ I understand the use of cell phones are not allowed by students and should be turned
OFF before entering class. Cell phones will be confiscated and withheld for a full week and/or
$10 fine. Entiendo que el uso de teléfonos celulares no es permitido para los estudiantes y
deberían de ser apagados antes de entrar a la clase. Los teléfonos celulares serán confiscados
por una semana entera y se les cobrara una tarifa de $10.
______ I allow my child’s Photo to be taken and used with proper discretion for use and
promotion of Parish ministries. Permito que la foto de mi hijo/a sea tomada y usada con la
discreción apropiada para uso y promoción del programa de Educación Religiosa.
______ I give permission for my child to take part in the Safe Environment Training as
recommended by the Diocese of Phoenix. This course is an age appropriate one time
informational offering given once during the school year on the topic of sex abuse and how the
children can better protect themselves. Permito que mi niño tome parte en el programa de
Ambiente Seguro como recomendación de la Diócesis de Phoenix. Este curso es informacional,
de edad apropiado, dado una vez durante el año escolar sobre el tema de abuso sexual y como
los jóvenes puedan protegerse mejor.
