Registration Information
All student registration is done on line.
1. Check webpage http://mom.sweetwaterschools.org for registration dates and times.
2. You will be mailed a letter that will provide a temporary password in order to start the On-line
Registration.
3. Using temporary password, Logon to: https://olr.sweetwaterschools.org
4. Once you complete the On-line Registration, you will need to print three forms:
1) Signature Page
2) Affidavit Form
3) Student Consent and Wavier
5. Return three (3) forms along with your proof of residency and the student shot record during
registration dates.
6. You’re Done!
Still need help? Montgomery Middle School will have computers available for parents to do the Online Registration. You are welcome to come in during the designated dates and times. Please check
webpage for dates and times.
Procedimiento de Inscripción por Internet
Toda inscripción de los alumnos es por internet.
1. Revise la página de http://mom.sweetwaterschools.org para fechas y horario de inscripción.
2. Se le enviará una carta por correo que le proporcionará una contraseña temporal con el fin de iniciar la
inscripción por internet
3. Usando la contraseña temporal entre en la pagina: https://olr.sweetwaterschools.org y siga las
instrucciones
4. Cuando complete la inscripción por internet, tendrá que imprimir tres formas:
Página de firmas (Signature Page)
Declaración jurada (Affidavit Form)
Consentimiento del estudiante (Student Consent and Wavier)
5. Entregue las tres formas con su comprobante de residencia y la tarjeta de vacunación durante las fechas
de inscripción
6. ¡Ha completado el proceso!
¿Aún necesita ayuda? La escuela Montgomery Middle tendrá computadoras disponibles para que los
padres puedan completar la inscripción por internet. Usted puede venir durante las fechas de
inscripciones.
