Los procesos inflamatorios no son los responsables de la tendinopatía crónica. (Bahr et al.,2006; Cook, Khan, & Purdam, 2011; Koen, & Roeland, 2005; Norregard et al., 2007). Son los procesos degenerativos los que conllevan una modificación de la estructura y de las propiedades de las fibras de colágeno en el tendón. (Langberg et al, 2007). BIOMECÁNICA DEL GESTO INCORRECTA REPETICIÓN EXCESIVA DEL GESTO EXCESO DE CARGA (músculo muy fuerte o muy débil) SOBREUSO Suele aparecer cuando existen aumentos bruscos en la intensidad, volumen o densidad del entrenamiento o la competición (Koen et al.,2005). INCIDENCIA Lesión común especialmente en aquellos deportes de salto, con gran implicación de gestos encuadrados en la manifestación de fuerza explosiva y en carrera de larga distancia. Prevalencia de hasta un 45% en alto nivel y un 14.4% en deportes recreacionales (Norregard, Larsen, Bieler, & Landberg, 2007; Roos, Engström, Lagerquist, & Söderberg, 2004). INCIDENCIA En deportistas de elite - 40% y un 50% en jugadores de voleibol, - 35% y un 40% en jugadores de baloncesto (Bahr, Fossan, Loken, Engebresten, 2006) - Entre el 11% y el 24% en corredores de larga distancia (Wasielewski, Kotsko, 2005) ¿RODILLA DEL SALTADOR O RODILLA DEL CAEDOR? El tendón responde al stress progresivo y controlado incrementando su fuerza de tensión, facilitando el aumento de colágeno y participando de esta forma en la remodelación. (Stanish, Rubinovich, & Curwin, 1986) ¿POR QUÉ EJERCICIO EXCÉNTRICO? La carga mecánica es beneficiosa para la salud del tendón, influyendo en la estructura, composición química y propiedades mecánicas del mismo (Kongsgaard et al., 2010; Magnusson, Langberg, & Kjaer, 2010) ¿CÓMO FUNCIONA? DISEÑO Estudio experimental con medidas pre-post de casos aleatorios y medidas repetidas durante un periodo de 4 semanas en periodo competitivo y sin restricción de entrenamiento. Es un entrenamiento válido pero poca evidencia con deportistas en periodo competitivo, poca extrapolación. (Cook et al., 2012; Visnes et al., 2005; Young et al., 2005). Este estudio se realizó en jugadores jóvenes, en formación. Existen estudios que valoran la mejoría al cabo de varios meses, tras 12 meses después de finalizado el protocolo (Bahr et al., 2006; Young, Cook, Purdam, Kiss & Alfredson, 2005) e incluso 36 meses después (Jonsson, Alfredson, 2005). CONCLUSIONES No es posible determinar mejoras significativas. El dolor y funcionalidad no empeoran manteniendo nivel de entreno y competición. A pesar de no ser significativas se ven mejoras en todos los casos excepto en uno. Los tratamientos podrían servir para controlar la evolución de la lesión.
