BUS PASS APPLIC ATION INSTRUC TIONS
If you are seeking busing for your child in the 2014-2015 school year, please read the following
information.
At the start of every school year, new applications and documents are needed to ensure that your child
receives transportation to and from school. No student will be allowed to board any bus
without a picture- identified district issued bus pass. This will ensure the continued safety of
your child while riding our buses, as well as ensuring that students are dropped off at the correct
locations both in the morning and afternoon.
In order for your child to ride the bus on the first day of school, you will need to do the
following:
1. Please review all of the safety rules contained in the Guidelines for Transportation with your
child. It is extremely important that they understand all of the bus rules and safety procedures.
2. Complete and sign the Transportation Application, using the List of Bus Stops to find the safest
bus stop for your student to use.
NOTE: Students attending Monroe Middle School living North of Fruitdale must use the
Moorpark stops, and those living South of Fruitdale must use the Sherman Oaks / Leigh &
Fruitdale stops.
3. MAIL OPTION: If you are paying by check or money order and your student was enrolled in
the district during the 2013-2014 school year, you may mail the application to – Campbell USD
Transportation 240 Harrison Ave., Campbell, Ca 95008. Your students bus pass will be
available for pick up from the bus driver on the first day of school. Please be sure to include
your email address so that we can send you confirmation of receipt.
Please keep in mind that:
•
A separate form MUST be turned in for each family.
•
Students will be assigned to the bus stop closest to their residence address. Students are only
eligible for transportation services to the school of their attendance area.
•
When filling out the application, please list the names of ALL children (grades K-8) who will be
needing transportation on one application form.
•
Please fill out all information requested on the application. The Transportation Department
cannot complete your request for transportation without the application filled out completely.
•
If you are applying for free transportation, please fill out the Application for Free
Transportation in addition to the original Transportation Application. You will also need to
verify your family’s eligibility for free transportation (please see the form for instructions on
materials to bring for verification).

Turn in your completed application (if you are applying for free transportation, please have both
applications filled out completely and all verification paperwork with you).
b. Pay any fees due (please make checks payable to C ampbell Union School District). Checks,
money orders, or cash are the accepted methods of payment.
c.
Have your student’s picture taken if they were not a district student during the 2013-2014 school
year. All students will need picture- identified bus passes to be transported this
school year, so it is very important that the student is present on this day to take their picture.

year. This will be paid each time a student requests a replacement bus pass **
District transportation can be purchased by semester or full year only. The purchase of day, week,
month, and one-way transportation is not available.
INSTRUC C IONES PARA SOLIC ITAR UN PASE DE AUTOBÚS
Si usted está buscando el transporte escolar para su hijo en el año escolar 2014-2015, por favor lea la
siguiente información.

NOTA: Los estudiantes que asisten a la Escuela Monroe y viven Norte de Fruitdale deben utilizar
las paradas de Moorpark, y los del Sur de Fruitdale deben utilizar las paradas de Sherman Oaks /
Leigh y Fruitdale.
5. OPC IÓN DE C ORREO: Si usted está pagando con cheque o giro postal y el estudiante
estaba inscripto en el distrito durante el año escolar 2013-2014, puede enviar la solicitud por
correo a -. Campbell USD Transportation 240 Harrison Ave, Campbell, Ca 95008. El pase de
autobús de su estudiante estará disponible para recoger del conductor del autobús el primer día
de clases. Por favor, asegúrese de incluir su dirección de correo electrónico para que podamos
enviarle la confirmación de recibo.
Por favor, tenga en cuenta que:

•

transporte gratuito, además de la solicitud original de Transporte. Usted también tendrá que
comprobar la elegibilidad de su familia para el transporte gratuito (vea por favor el formulario
para obtener instrucciones sobre materias que tiene que llevar para verificación).

Entregar su solicitud completada (si está aplicando para el transporte gratuito, por favor tengan
ambas solicitudes llenadas completamente y todos los trámites de verificación con usted).
b. Pagar cualquier cuota que corresponda (por favor haga los cheques a nombre de C ampbell Union
School District). Los cheques, giros postales, o dinero en efectivo son los métodos de pago
aceptados.

semanas, meses, y transporte unidireccional no está disponible.
