Patient safety involves everyone – patients, their families, volunteers and staff. In offering our families the Condition HELP option, we want you to know that you are our partners in care. The Condition HELP team can respond to calls from patients, family, visitors, volunteers or employees when there is a concern that a patient may be in danger of having a medical emergency while in the hospital. When Can You Call a “Condition HELP”? When you feel that a medical situation or condition is worsening and: • The healthcare team does not seem to understand or respond to your concerns. • There seems to be confusion or lack of coordination among the caregivers. • You are not sure what is wrong, but something just doesn’t seem right. While you are a patient in the hospital, your rights include the following: n You have the right to considerate and respectful care. n You have the right to reasonable privacy. n You have the right to visitors and to send or receive mail and telephone calls. n You have the right to be well informed about your illness and treatment options. n You have the right to an appropriate assessment and a pain management plan. n You have the right to formulate advance directives and to have hospital staff comply with these directives. n You have the right to participate in development and implementation of your plan of care. n You have the right to consent to or refuse treatment, as permitted by law. n You have the right to expect that treatment records are confidential. n You have the right to access information in your medical records. n You have the right to be told of realistic care alternatives when hospital care is no longer appropriate. n You have the right to a second opinion. n You have the right to know about hospital rules that affect you and your treatment and about charges and payment methods. n You have the right to receive care in a safe setting, free from restraints. n You have the right to know if this hospital has relationships with outside parties as applicable to your treatment and care. n You have the right to have a family member or representative of your choice notified promptly of admission to the hospital. Your Rights as a Hospital Patient You have the right to request appropriate language assistance when you have Limited English Proficiency (LEP) or have a disability which limits your meaningful access and opportunity to participate in your care. Your request for communication assistance using auxiliary aids or services will be provided consistent with the Office of Civil Rights, U.S. Department of Health and Human Services regulation. YRMC provides this service to the patient free of charge. YRMC PATIENT INFORMATION GUIDE 75 PATIENT RESPONSIBILITIES n You are responsible for providing information about your health. n You are responsible for asking questions when you do not understand information or instructions. n You are responsible for being considerate of the needs of other patients and staff. n You are responsible for recognizing the effects of life-style on your personal health. n You are responsible for providing information for insurance and for working with the hospital to arrange payment, when needed. Hospitals work to improve people’s health; treat people with injury and disease; educate health professionals, patients, and community members; and improve understanding of health and disease. In carrying out these activities, this institution works to respect your values and dignity. Advance Directives Formal advance directives are documents written in advance of serious illness. The documents state your choices for healthcare, or name someone to make those choices if you become unable to make decisions. Through advance directives, such as a Living Will, Medical Power of Attorney for healthcare and Mental Health Care Power of Attorney, you can make legally valid decisions about your future medical treatment. If you have an advance directive you should be able to provide a copy upon hospitalization or be able to state the intent of the contents of your advance directive. Upon admission to the hospital you will be asked to provide this information. If you would like to make an advance directive upon admission to the hospital, your admitting nurse can help you do so. The state of Arizona has a web site which allows you to create and store your advance directive. You can access this site at www.azsos.gov. The most important thing you can do to ensure that the healthcare decisions you have made in advance are followed is to talk about them. Talk to your family, friends, neighbors, clergy, doctors and other health care providers. Let them know what you do and do not want when you cannot speak for yourself. 8 YRMC PATIENT INFORMATION GUIDE Concern and Grievance Policy and Procedure Yuma Regional Medical Center (YRMC) encourages good communication between patients, families and the health care team. If you are dissatisfied with any of our services, please discuss your concerns with the nurse providing your care. Our goal is to respond quickly and fairly to your concerns; therefore, if your nurse is unable to resolve your concerns ask to speak with the Resource Coordinator, the Unit Director, or the Administrative Hospital Supervisor. If your concern cannot be or is not resolved by our staff then it becomes a grievance; we ask that you contact the YRMC Patient Advocacy Office by calling 928-336-2002. You can also file a grievance by sending a letter to: The Patient Advocate, Department of Spiritual Care and Patient Advocacy, Yuma Regional Medical Center, 2400 South Avenue A, Yuma, AZ 85364. We will acknowledge your grievance in writing within 14 workdays of receiving it. After our investigation we will let you know what actions we took on your behalf to resolve your grievance. If you are not satisfied, you may then request that the Patient Advocate submit your grievance to the YRMC Grievance Committee. The YRMC Grievance Committee will respond to you in writing within one week of completing its investigation. If you are still not satisfied, you can submit your grievance to the Arizona Department of Health Services, Office of Healthcare Licensure, 150 North 18th Avenue, Suite 450, Phoenix, Arizona 85007, (602) 364-2536. YRMC will not discriminate against any patient or visitor on the basis of sex, race, creed, national origin, handicap, or ability to pay nor will it retaliate against a patient who files a grievance with the Patient Advocacy Office. This Concern and Grievance Policy & Procedure does not prevent a patient or visitor from filing a grievance of discrimination on the basis of handicap with the U.S. Department of Health, San Francisco, California, 94120, or call (415) 336-7600. Protected Health Information–Patients Breach of Privacy Yuma Regional Medical Center is required under the Health Insurance Portability and Accountability Act (HIPAA) to protect patients’ healthcare information. Please tell us about any problems or concerns you have with your privacy rights if you believe the Hospital inappropriately used or disclosed your medical information. Please contact the Privacy Officer at Yuma Regional Medical Center at (928) 336-7600. If for some reason the Hospital cannot resolve your concern, you may file a complaint with the federal government. We will not penalize you or retaliate against you in any way for filing a complaint with the federal government. YRMC PATIENT INFORMATION GUIDE 95 GENERAL INFORMATION FOOD and DINING OPTIONS Room Service Dining YRMC is pleased to offer complimentary Room Service Dining for patients from 7:00 am to 6:30 pm. This hotel style dining program allows you to select meals from a menu designed to make you feel as though you were dining at home or at your favorite restaurant. Some diet restrictions may apply. Call Ext. 5500 to place your order. Cafeteria Hours The cafeteria is located on the ground floor of YRMC. ■ Breakfast is served from 7:00 am – 9:30 am ■ Lunch is served from 11:00 am – 1:30 pm ■ Dinner is served from 4:00 am – 6:30 pm ■ A light meal is available between serving times Daily Grind The Daily Grind is located near the Gift Shop at the main entrance. It offers burritos, sandwiches, snacks and made-to-order beverages, including espresso drinks. Winter hours are 7:00 am to 5:00 pm weekdays; 8:00 am to 2:00 pm weekends. Summer hours may vary. Corner Stork Cafe The Corner Stork Café is located near Women and Children Services. Baby gifts and also offering burritos, snacks and beverages. Vending Machines Beverages and snacks may be purchased at the vending machines 24 hours a day. Ask a caregiver for the closest location. Automated Teller Machines (ATM) are located n Women & Children’s lobby n Ground Floor near the cafeteria 10 YRMC PATIENT INFORMATION GUIDE 9 GENERAL INFORMATION Accreditation Yuma Regional Medical Center is proud to comply with all accreditation and certification standards. The national initiatives YRMC participates in focus on the quality of care and safety of our patients. These quality and performance measurements are used to evaluate the care provided by our organization. Admission Process Admitting services are available 24 hours, 7 days a week. You will be escorted to your assigned room and will be requested to complete all necessary admission paperwork including personal and insurance information. Care Teams During your stay at YRMC, you have the right to know the identity of all hospital staff involved in your care. Many of these caregivers will visit you several times during your stay to attend to your needs. Your care team may include: n Registered and Licensed Nurses n Social Workers n Clinical Associates (Nursing assistants) n Chaplains n Business Associates (Unit Support Rep.) n Nutrition Services n Housekeeping n Therapy Staff n Case Management n Patient Safety Companions Hospitalist Care at YRMC If you are admitted to the hospital your doctor may not be the doctor that sees you every day. You may be assigned to a Hospitalist. A Hospitalist is a doctor that specializes in the day to day care of patients in a hospital. They coordinate your care and work with any specialist that may be needed to provide you with the best treatment plan possible. When you are discharged you will be referred back to your doctor for follow-up. Case Management Case Management Services are available seven days a week to provide assistance with your inpatient stay and discharge. This includes home health care; durable medical equipment; transfers to facilities out of Yuma or to other health facilities in town such as Yuma Rehabilitation Hospital, skilled nursing or assisted living facilities; Adult Protective and Child Protective Services; and referrals to community agencies. Case Management provides services to patients of all ages and in all patient areas. To contact Case Management staff, call Ext. 7480 or ask your nurse for assistance. Chapel The chapel is located on the ground floor by the Heart Center. The chapel is open 24 hours to patients and visitors of all faiths for prayer and meditation. YRMC PATIENT INFORMATION GUIDE 11 GENERAL INFORMATION Childbirth and Babies n Birth Certificates: Information for birth certificates is collected when babies are born at YRMC. n Breastfeeding Support: A board-certified lactation consultant is available for breastfeeding support, please call (928) 336-1211. n Family Support Services: Classes include childbirth preparation, breastfeeding, sibling and newborn care. For information or to register, call 928-336-7058. Please speak with a member of the healthcare team for above topics. Gift Shops Yuma Regional Medical Center Volunteer Services operates two convenient gift shop locations. The main Gift Shop is located near the Main Lobby and carries a full array of unique gifts, cards, flowers and sundries items. The Corner Stork Cafe is located near the Women and Children Services lobby and offers refreshments and a variety of baby and children’s gifts. Healing Garden 5 The Healing Garden is located just outside our Intensive Care Unit and is designed to provide patients and families with a relaxing outdoor environment. This garden area is a collaborative project between the Foundation of YRMC and the Palliative Care team and is funded by private donations. It features electrical outlets and benches that allow patients with IV pumps to sit outside and enjoy the area. Hand Hygiene – clean hands save lives! Wash your hands with warm water • Add soap • Scrub all surfaces well for 20 seconds • Rinse completely • Dry with clean paper towel, throw it away • Turn faucet off with a clean paper towel • Wash your hands before and after going to the bathroom, commode or bedpan • Before and after you eat • If they become soiled Cough Etiquette Cover all coughs or sneezes with a tissue. If there is no tissue, cough into your sleeve or elbow. Sanitize your hands or wash with soap and water after every cough or sneeze. Family members who have a fever, cough, runny nose, sore throat, diarrhea or any flu symptoms should not come to visit patients. Patients are more likely to pick up germs of those who visit. Our goal is for you to recover and return home. 12 YRMC PATIENT INFORMATION GUIDE GENERAL INFORMATION Internet Usage Yuma Regional Medical Center is pleased to offer free wireless Internet connection during your hospital stay. For information about accessing the wireless network using your personal laptop computer or wireless device, please ask your patient care team or call our Computer Services at Ext. 7290. Language Services at YRMC YRMC will provide notice to all deaf or hard of hearing patients and/or companions of its policy to provide appropriate auxiliary aids and services throughout a patient’s period of treatment. You have the right to request language assistance when you have a disability which limits your meaningful access and opportunity to participate in your care. YRMC does not discriminate on the basis of disability and appropriate auxiliary aids and services shall be provided free of charge to deaf and hard of hearing patients and/or companions; provides a grievance procedure for filing and resolving complaints about disability discrimination prohibited by Section 504, including complaints regarding the failure to provide appropriate auxiliary aids and services consistent with the Office of Civil Rights, U.S. Department of Health and Human Services regulation. In requesting appropriate auxiliary aids or services, YRMC staff shall take into account all relevant facts and circumstances, including without limitation the following: 1. the nature, length and importance of thecommunication issue; 2. the individual’s disability, communication skills and knowledge; 3. patient’s health status; 4. patient’s and/or companion’s preference for a stated need for auxiliary aid and service. Appropriate auxiliary aids and services include, but are not limited to: qualified sign language interpreters, Video Remote Interpreting/American Sign Language, written materials, pictographs, assistive listening devices and systems, open and closed captioning, wireless internet and TTY/TDD. Interpreters Yuma Regional Medical Center has access to 24 hours/7 days a week interpreter services for a number of foreign languages for those who are limited-English proficient. Interpreters available through these services are trained in medical interpretation. Please ask your nurse for assistance. Library Services The Medical Library is open to patients and families from 8:30 a.m. to 5:00 p.m., Monday-Friday. It is closed on holidays. The library’s telephone number is (928) 336-7181. The library staff can help you find information. If you are looking for a magazine or a good book to read, the Volunteer Department has a book cart that visits patient care areas. Ask your nurse for help if you miss the book cart or call (928) 336-7462. YRMC PATIENT INFORMATION GUIDE 13 GENERAL INFORMATION Pain Management As a patient at YRMC, you can expect: n Information about pain and pain relief measures. n A concerned staff committed to pain prevention and management. n Health professionals who respond quickly to reports of pain. As a patient at YRMC, we expect you: n To ask for pain relief when pain begins. n To help your doctors and nurses assess your pain. n To tell your doctors and nurses if your pain is not relieved. n To tell your doctors and nurses about any worries you have about taking pain medication. Palliative Care The focus of palliative care is symptom management for whatever disease you may have. This may include pain management and symptom control throughout a person’s lifespan including end of life. For information, call Ext. 1529. Parking Visitor/Patient parking areas are located around the hospital and are identified by signs. Our Volunteer Department also offers a free courtesy parking shuttle. Hours may vary. Call Ext. 7927, Monday–Friday between 7 am–5 pm for pick-up. Patient Education To help patients become well informed about their illness and treatment options, YRMC offers information and education for both the patient and the patient’s family. It is important that family members be included whenever possible. Ask your nurse if you have questions about your illness and want to learn how to take care of yourself or a family member after leaving the hospital. Individual instruction is available for patients with diabetes, heart disease, lung disease, or other conditions. Patient Education videos on demand and various health programs may be seen on Channels 82-87. Channel 89 features soft music and photos. A printed program of topics is available from your nurse. To access videos, dial Ext. 5300 on your room telephone and follow the instructions. Paying Your Hospital Bill Yuma Regional Medical Center will bill most types of insurance, including Medicare, Medicaid and other commercial insurance. Patient Financial Services will assist you in making financial arrangements regarding your hospital bill. Total hospital charges are not always available at the time of discharge. These charges will be on the itemized bill sent to your home or to your insurance carrier. Long distance calls may be placed through the hospital operator by dialing “0.” Long distance calls must be charged to your home phone, your credit card/calling card or placed collect. Charges for your long distance calls cannot be added to your hospital bill. Valuables and Personal Property Send valuables home so they don’t get lost or stolen. If you have valuables and cannot arrange to send them home, please ask your nurse for assistance to place these items in the hospital safe. YRMC cannot be responsible for your valuable belongings left unsecured. If you do have property lost or stolen, please ask a member of our staff to contact Security to submit a report, or you can call Security at Ext. 2400. Other personal items should be stored in the bedside table when not in use. Visitation It is the philosophy of Yuma Regional Medical Center that visitation is patient and family centered. Nursing staff will collaborate and communicate with you and your family or significant others to coordinate a visiting plan that individualizes and promotes this support for you and your family. Patient Safety and Falls The danger of falling is very real, especially for hospital patients who have difficulty walking. Several factors increase the risk of falls, including: n Being in a new and unfamiliar environment n Taking certain medicines n Taking certain preparations for procedures n Previous history of falls n Physical or mental impairments (blindness, hearing loss) We will do all we can to ensure your safety and comfort during your stay at YRMC, however we believe that fall prevention is everyone’s responsibility. If your nurse or doctor identifies you as having an increased risk of falling, you may: n Be moved closer to the nurses’ station n Receive a yellow wrist band n Have a falling star placed above your bed n Be instructed to use your call light for help getting out of bed n Encourage a family member to stay with you Preventing Pressure Sores (Ulcers) Pressure sores can develop quickly, even in healthy skin. Taking pressure off the skin is the first step. That means changing positions often, supporting the body and avoiding rubbing and sliding. Keeping the skin clean, eating well and stretching the joints and muscles can also help prevent pressure sores. n Changing positions often allows blood to get to the skin and keep the tissue healthy n Supporting the body spreads pressure over a larger area n Rubbing and sliding cause the skin to break down more easily n Keeping the skin clean and soft helps prevent pressure sores n Eat enough calories to maintain a stable weight n Get plenty of protein, vitamins and iron; drink lots of fluids daily n Get out of the bed or chair as much as possible YRMC PATIENT INFORMATION GUIDE 17 SPEAK UP FOR YOUR HEALTH & SAFETY Everyone has a role in making healthcare safe–nurses, physicians, healthcare executives and technicians. Mientras usted sea paciente de este hospital, sus derechos son los siguientes: n Tiene derecho a recibir una atención atenta y respetuosa. n Tiene derecho a una privacidad razonable. n Tiene derecho a recibir visitas y a enviar o recibir correspondencia y llamadas. n Tiene derecho a estar bien informado sobre su enfermedad y las opciones de tratamiento. n Tiene derecho a recibir una evaluación y un plan de control del dolor. n Tiene derecho a formular instrucciones anticipadas y a que el personal del hospital las siga. n Tiene derecho a participar en el desarrollo y la implementación de su plan de atención. n Tiene derecho a aceptar o rechazar un tratamiento, según lo permita la ley. n Tiene derecho que los archivos de tratamiento sean confidenciales. n Tiene derecho a acceder a la información de su archivo médico. n Tiene derecho a que se le indiquen alternativas realistas de atención cuando la atención hospitalaria ya no sea adecuada. n Tiene derecho a una segunda opinión. n Tiene derecho a saber las reglas del hospital que tienen efecto sobre usted y su tratamiento, acerca de los cargos y métodos de pago. n Tiene derecho a recibir atención en un establecimiento seguro y sin limitaciones. n Tiene derecho a saber si este hospital tiene relación con entidades externas según corresponda a su tratamiento o atención. n Tiene derecho a que se le avise rapidamente a un familiar o representante sobre su hospitalización. Sus derechos como un Paciente del hospital Usted tiene el derecho a solicitar la asistencia de lenguaje apropiado cuando usted tiene dominio limitado del inglés (LEP) o tiene una discapacidad que limita su acceso efectivo y la oportunidad de participar en su cuidado. Los hospitales trabajan para mejorar la salud de las personas; para dar tratamiento a las personas con heridas y enfermedades; para educar a los profesionales de salud, a los pacientes y a los miembros de la comunidad; y para mejorar la comprensión de la salud y de la enfermedad. En el desempeño de estas actividades, esta institución se esfuerza para respetar sus valores y su dignidad. Directivas Anticipadas Las directivas anticipadas formales son documentos que se escriben antes de que ocurra una enfermedad grave. Los documentos establecen sus opciones respecto de la atención de salud o nombran a alguna persona para que elija esas opciones si usted no puede tomar decisiones. A través de las directivas anticipadas, como Voluntad de vida. Poder médico para atención de salud y Poder de atención de salud mental, usted puede tomar decisiones legalmente válidas sobre su futuro tratamiento médico. Si tiene una directiva anticipada, debe ser capaz de proporcionar una copia cuando es ingresado en el hospital o ser capaz de establecer la intención de los contenidos de su directiva anticipada. Al ser ingresado en el hospital, se le pedirá que proporcione esta información. Si desea realizar una directiva anticipada al ser ingresado en el hospital, el enfermero de admisión puede ayudarle con este tránsmite. GUÍA DE INFORMACIÓN PARA EL PACIENTE DE YRMC 31 Política y procedimiento de inquietudes y quejas Yuma Regional Medical Center (YRMC) alienta la buena comunicación entre pacientes, familias y el equipo de atención de salud. Si no está satisfecho con cualquiera de nuestros servicios, hable al respecto con el enfermero que le proporciona atención. Nuestro objetivo es responder con rapidez y equidad a sus inquietudes; por lo tanto si su enfermero no puede resolver sus inquietudes, solicite hablar con el Coordinador de Recursos, el Director de la Unidad o el Supervisor Administrativo del Hospital. Si nuestro personal no puede resolver o no soluciona su inquietud, ésta se convierte en una queja; le solicitamos comunicarse con la Oficina de Defensa al Paciente de YRMC llamando al 928-336-2002. También puede presentar una queja enviando una carta a: The Patient Advocate, Department of Spiritual Care and Patient Advocacy, Yuma Regional Medical Center, 2400 South Avenue A, Yuma, AZ 85364. Acusaremos recibo de su queja por escrito dentro de los catorce días después de recibir su carta. Luego de nuestra investigación, le comunicaremos qué acciones llevamos a cabo en su nombre para resolver su queja. Si no está satisfecho, podrá solicitar al Defensor del Paciente que presente su queja al Comité de Quejas de YRMC. El Comité de Quejas de YRMC le responderá por escrito dentro de la semana después de haber completado su investigación. YRMC no discriminará a ningún paciente ni visitante respecto de sexo, raza, credo, nacionalidad, discapacidad o capacidad para pagar. Ni tendrá represalias contra un paciente que presente una queja con la Oficina de Defensa del Paciente. Información de salud protegida – Incumplimiento de privacidad del paciente Según la Ley de Responsabilidad y Transferibilidad de Seguros Médicos (Yuma Regional Medical Center (Health Insurance Portability and Accountability Act, HIPAA) Yuma Regional Medical Center debe proteger la información de atención de salud de los pacientes. No dude en comunicar cualquier problema o inquietud que tenga sobre sus derechos de privacidad si considera que el Hospital utilizó o divulgó su información médica de manera inapropiada. No lo sancionaremos ni tomaremos represalias contra usted de ningún modo por presentar una demanda ante el gobierno federal. 32 GUÍA DE INFORMACIÓN PARA EL PACIENTE DE YRMC INFORMACIÓN GENERAL OPCIONES DE ALIMENTOS Y CENA Servicio a la Habitación YRMC tiene el agrado de ofrecer servicio a la habitación de cortesía para los pacientes de 7:00 am a 6:30 pm. Este programa de comidas tipo hotel le permite elegir comidas de un menú diseñado para hacerlo sentir igual como si estuviera cenando en su casa o en su restaurante favorito. Dependiendo de la dieta recetada por su médico, cuando haya elegido del menú, llame a la extensión 5500. La comida se entregará en su habitación dentro de un plazo de 45 minutos. También se dispone de bandejas de comida para las visitas por un precio nominal. Pregúntele a su equipo de atención. Horario de la Cafetería La cafetería está localizada en la planta baja. El desayuno se sirve de 7:00 am a 9:30 am El almuerzo se sirve de 11:00 am a 1:30 pm La cena se sirve de 4:00 pm a 6:30 pm Un aperitivo esta disponible entre comidas. Daily Grind (Tienda de Refrigerios Daily Grind) La Tienda de Refrigerios Daily Grind se encuentra cerca de la tienda de regalos. Allí puede comprar burritos, sándwiches preparados a pedido, bocadillos, bebidas, y también distintas combinaciones de café expreso. El horario de invierno es de 7:00 am a 5:00 pm los días de semana y de 8:00 am a 2:00 pm los fines de semana. El horario de verano varía. Corner Stork Café (Tienda de Refrigerios) El Corner Stork Cafe está ubicado cerca de Servicios para mujeres y niños. Ofrece regalos para bebés y también burritos, bocadillos y bebidas. Máquinas Expendedoras Las máquinas expendedoras están disponibles las 24 horas del día. En ellas se pueden comprar bebidas, bocadillos, refrigerios o meriendas. Cajeros Automaticos (ATM) están localizados n Vestíbulo de Mujeres y Niños n Planta baja cerca de la caféteria GUÍA DE INFORMACIÓN PARA EL PACIENTE DE YRMC 33 INFORMACIÓN GENERAL Acreditación El Centro Médico Regional de Yuma (YRMC pos sus siglas en Inglés) está orgulloso de cumplir con todos los estándares en certificatión y acreditación. Las iniciativas nacionales en que participa YRMC se enfocan en la calidad de cuidado y en la seguridad de nuetros pacientes. Estas medidas de calidad y rendimiento se utilizan para evaluar el cuidado que proporciona nuestra organización. Proceso de Admisión Los servicios de admisión se encuentran disponibles durante las 24 horas del día, los 7 días de la semana. Se le acompañará a su cuarto asignado y se le pedirá que llene toda la documentación de admisión necesaria que incluye la información personal y de aseguranza. Equipos de Atención Durante su hospitalización en YRMC, tiene el derecho de conocer la identidad del personal hospitalario que participan en su atención. Muchos de estos cuidadores lo visitarán varias veces durante su hospitalización para atender sus necesidades. Su equipo de atención puede incluir: n Enfermeras n Trabajadores Sociales n Socios Clínicos (asistentes de enfermería) n Capellanes n Socios Comerciales (Representante de apoyo a la unidad) n Servicios Nutricionales n Labores domésticas n Terapeutas n Administración de Casos n Asistente de Pacientes Atención de médicos hospitalistas en YRMC Si es ingresado en el hospital, es probable que su médico no sea el profesional que lo atiende todos los días. Se le podrá asignar un médico hospitalista. El médico hospitalista es un profesional que se especializa en la atención diaria de pacientes en un hospital. Coordina su atención y trabajo con cualquier otro especialista que se pueda necesitar para proporcionarle el mejor plan de tratamiento posible. Cuando se le da el alta, se lo volverá a derivar a su médico para el seguimiento. Administración de Casos Los servicios de Administración de Casos están disponibles los siete días de la semana para ofrecer ayuda con la hospitalización y del alta. Esto incluye la asistencia médica de casa, el equipo médico duradero, los traslados a instalaciones fuera de Yuma o a otras instalaciones en la ciudad como el Hospital de Rehabilitación de Yuma (Yuma Rehabilitation Hospital), enfermería capacitada o establecimientos de vivienda asistida, servicios de protección de adultos y niños y remisiones a agencias comunitarias. La Administración de Casos ofrece servicios a pacientes de todas las edades y en todas las áreas. Para comunicarse con el personal de Administración de Casos, llame a la extensión 7480 o consulte a su enfermera para asistencia. 34 GUÍA DE INFORMACIÓN PARA EL PACIENTE DE YRMC INFORMACIÓN GENERAL Capilla La capilla está localizada en la planta baja cerca del Centro Para el Tratamiento de Enfermedades Cardíacas. La capilla está abierta las 24 horas para los pacientes y las visitas de todas las religiones para que oren y mediten. Parto y Recién Nacidos n Fotos del bebé: YRMC tiene fotografías disponibles de los recién nacidos. n Certificados del Nacimiento: La información para los certificados de nacimiento se reúne cuando los bebés nacen en YRMC. n Apoyo al amamantamiento: Un consultor en lactancia certificado por la junta está disponible para brindar ayuda sobre la lactancia. Llame al (928) 336-1211. n Servicios de apoyo familiar: Las clases incluyen: preparación para el nacimiento, lactancia, atención de hermanos y del recién nacido. Tiendas de Regalos El Servicio voluntario de YRMC posee dos tiendas de regalos que se encuentran en lugares convenientes. La tienda de regalos principal está localizada cerca del pasillo principal y tiene una amplia variedad de regalos especiales, tarjetas, flores y varios artículoz. La Tienda de Refrigerios Corner Stork Café se encuentra cerca del vestíbulo de Mujeres y Niños en donde puede comprar refrescos, bocadillos y tiene una gran variedad de regalos para bebés y niños. Jardín de Curación El Jardín de Curación está localizado justo afuera de la Unidad de Cuidados Intensivos y está diseñado para proporcionar a los pacientes y sus familias un ambiente relajante al aire libre. Esta área del jardín es un proyecto de colaboración entre la fundación de YRMC y el equipo de cuidados paliativos y se financió con donaciones privadas. Tiene tomacorrientes y bancos que le permiten a los pacientes con bombas intravenosas sentarse al aire libre y disfrutar del área. Higiene de las manos: lavarse las manos salva vidas. Lávese las manos con agua calientes • Agregue jabón • Frótese bien todas las superficies durante 20 segundos • Enjuáguese completamente • Séquese las manos con toallas de papel limpias que luego eliminará. • Cierre la canilla con una toalla de papel limpia • Lávese las manos antes y después de ir al baño, tocar el inodoro o la chata • Antes y después de comer • Si se ensucian GUÍA DE INFORMACIÓN PARA EL PACIENTE DE YRMC 35 INFORMACIÓN GENERAL Instrucciones para cuando tose Cúbrase al toser o al estornudar con un papel tisú. Si no tiene papel, tosa sobre la manga o el codo. Desinféctese las manos o lávelas con jabón y agua después de cada vez que tose o estornuda. Los miembros de la familia que tengan fiebre, tos, goteo nasal, dolor de garganta, diarrea o cualquier síntoma de gripe no deben venir a visitar pacientes. Es más probable que los pacientes pesquen un germen de aquellos quienes los visitan. Nuestro objetivo es que se recupere y regrese a su casa. Uso de Internet: YRMC ofrece Internet inalámbrico gratis durante su permanencia en el hospital. Para información sobre como puede tener acceso a Internet inalámbrico usando su computadora personal portátil, por favor preguntele al equipo de cuidado de paciente o llame a nuestra area de tecnología a la extensión 7290. Intérpretes Yuma Regional Medical Center tiene acceso a servicios de intérprete las 24 horas, los 7 días de la semana, para un número de idiomas extranjeros para aquellos que tienen un dominio limitado del inglés. Los intérpretes disponibles a través de estos servicios están capacitados en interpretación médica. Solicite asistencia a su enfermero. Servicios de idioma en YRMC YRMC dará un aviso a todos los pacientes y/o acompañantes sordos o con dificultades auditivas de esta política para proporcionar la ayuda y los servicios adecuados durante el período de tratamiento del paciente. Tiene derecho a solicitar asistencia de idioma cuando tenga una discapacidad que limite su acceso y oportunidad significativos para participar en su atención. YRMC no discriminará con respecto a una discapacidad y se proporcionarán, de forma gratuita, la ayuda y los servicios adecuados a pacientes y/o acompañantes sordos o con problemas auditivos; proporcionará un procedimiento de queja para presentar y resolver reclamos sobre discriminación por discapacidad prohibida por la Sección 504, incluidos los reclamos con respecto a no proporcionar ayuda ni servicios adecuados consistentes con la reglamentación de la Oficina de Derechos Civiles, Departamento de Salud y Servicios Humanos. 36 GUÍA DE INFORMACIÓN PARA EL PACIENTE DE YRMC INFORMACIÓN GENERAL In requesting appropriate auxiliary aids or services, YRMC staff shall take into account all relevant facts and circumstances, including without limitation the following: 1. the nature, length and importance of thecommunication issue; 2. the individual’s disability, communication skills and knowledge; 3. patient’s health status; 4. patient’s and/or companion’s preference for a stated need for auxiliary aid and service. Appropriate auxiliary aids and services include, but are not limited to: qualified sign language interpreters, Video Remote Interpreting/American Sign Language, written materials, pictographs, assistive listening devices and systems, open and closed captioning, wireless internet and TTY/TDD. Servicios de biblioteca La Biblioteca Médica está abierta para pacientes y familias de 8:30 a.m. a 5:00 p.m., de lunes a viernes. Está cerrada los feriados. Control del dolor Como paciente de YRMC, puede esperar: n recibir información acerca del dolor y de las medidas para aliviarlo; n un equipo comprometido en la prevención del dolor; n obtener una respuesta rápida a sus síntomas por parte de los profesionales de la salud. Como paciente de YRMC, esperamos que usted: n pida alivio para el dolor cuando recién comience; n ayude a sus médicos y enfermeras a evaluar su dolor; n decirle a sus médicos y enfermeras si el dolor no se alivia; n decirle a sus médicos y enfermeras acerca de las preocupaciones que tenga en relación con los analgésicos que toma para el dolor. Cuidados Paliativos Los cuidados paliativos se concentran en el manejo de los síntomas de cualquier enfermedad que tenga. Esto puede incluir el control del dolor y los síntomas a lo largo de la vida de la persona incluyendo la etapa final, llame a la extensión 1529. GUÍA DE INFORMACIÓN PARA EL PACIENTE DE YRMC 37 INFORMACIÓN GENERAL Estacionamiento Las áreas de estacionamiento para visitas y pacientes se encuentran por todo el hospital y están señaladas. Nuestro Departamento de Voluntario también ofrece servicios de transporte gratis al estacionamiento.Los horarios varían. Educación para el Paciente Para ayudar a los pacientes a estar bien informados sobre sus enfermedades y opciones de tratamiento, YRMC ofrece información y educación para el paciente y su familia.
