What To Bring To Your First Appointment
Please be sure that you bring the following information and items to your first appointment:
 Your health insurance information. This includes Medicare, Medicaid (Public Aid) or private
insurance information. We will need to make copies of your insurance card if you have
coverage.
 Your Social Security Number (if you have one)
 The names and dosages of any prescription medications that you take
 Your primary physician’s complete name, full address and phone number
 Your best estimate of your household size and income.
