 Must live in Tuloso Midway District
 Must be 5 yrs old by September 1, 2014
The following documents are required for registration: (If you are missing documents you will

1. Proof of Residency- A current utility bill or lease or mortgage agreement
2. Updated immunization record
3. Birth certificate (official state issued)
4. Social Security card (official state issued)
5. Valid ID (parent or legal guardian)
Registration for all other grades or for all transfers will begin on May 01, 2013.
Residents will register at the Primary School and Transfers will register at the administration office

 Tiene que tener 5 años de edad antes del 1ro de septiembre de 2014
 Tiene que vivir en el distrito escolar de Tuloso-Midway
Las pruebas necesarias para la matriculación:
1. Comprobante de Residencia - recibo actual de servicios, copia de la hipoteca de la casa, o contrato
de vivienda
2. Comprobante de vacunas
3. Registro de nacimiento
4. Tarjeta de seguro social
5. Identificación válida (padre/madre - guardián legal)
Si no tiene todos los documentos necesarios no podrán continuar la matriculación.
