Paciente Masculino 58 años. Al examen físico se observa esta lesión. No sabe tiempo de evolución. ¿ CUÁL ES SU DIAGNÓSTICO ? “ ” Esta lesión es muy poco conocida pero muy frecuente, tiene un patrón dermatoscópico clásico, aparece en pieles foto dañadas y en zonas de lentigos múltiples. Fue descrita por la Dra. Jean Bolognia en 1992, a quien tendremos oportunidad de oír en Colombia el próximo mes. “…The "ink spot" lentigo is distinguished clinically by its color and wiry or beaded, markedly irregular outline. It has a reticulated pattern and most resembles a spot of ink on the skin. In this series of patients, the lesions were limited to sun-exposed areas of the body and had a distribution pattern similar to that of solar lentigines. Although all the patients were of Celtic ancestry and had numerous solar lentigines, they usually had only one black lentigo (range, one to four; mean, 1.6; median, one). Histologic evaluation, including electron microscopy and dopa-incubated vertical sections, demonstrated lentiginous hyperplasia of the epidermis, marked hyperpigmentation of the basal layer with "skip" areas that involved the rete ridges, and a minimal increase in the number of melanocytes.” “ ” March 1996 RETICULATED EPHELIDES: 'INKSPOTS' REVISITED Stacy R. Wolf, Edgar Rieger, Helmut Kerl …Resumen —El lentigo en «mancha de tinta» se caracteriza clínicamente por su color negro y por sus bordes marcadamente irregulares, que recuerdan a una mancha de tinta china sobre la piel. Es una lesión pigmentada de reciente descripción y poco conocida, que puede originar problemas de diagnóstico diferencial, tanto clínico como histopatológico. Histopatológicamente consiste en una hiperpigmentación del extremo inferior de las crestas epidérmicas, con escaso pigmento melánico en la epidermis entre crestas. No se observa proliferación de melanocitos. Se trata de una lesión benigna y sin capacidad de malignizarse” “ El motivo de presentar este caso es porque es frecuente y poco reconocido. Desde que lo conocí, gracias a los cursos del Dr. Merlano, que nos introdujo en esa nueva semiología de las imágenes dermatoscópicas, lo veo frecuentemente. Se mejoran fácilmente con una crio corta.
