If you are interested in applying for the
Head Start Program:
 Your child must be 3 or 4yrs. old on or
before September 1st, 2009.
 Your child may be non-English speaking.
(A language proficiency test will be given
to your child)
 Your child may have or you suspect a
special need or disability
Please bring the following documents with
your application







Signed Head Start application
Child’s birth Certificate
Child’s Social Security Card
Child’s updated shot Records
Proof of residency (light bill, water bill)
Medical insurance (Medicaid or Private)
Proof of family income (previous 4 pay
stubs, W2 or tax documents)
If applicable





Notarized letter of living arrangement
Notarized statement of proof of residency
Guardian / Legal Custody Documentation
Most recent ARD / IEP documents
SSI / TANF / Food Stamps / Grants /
Child Support
 Your child may be eligible to receive bus
transportation
Si a usted le interesa aplicar para el
Programa de Head Start:
 Su niño(a) debe tener 3 ó 4 años en o antes
del 1ro de Setiembre del 2009.
 Su niño(a) puede ser que no entienda ni hable
el idioma inglés (Una prueba de lenguaje se le
administrará a su niño(a)
 Su niño(a) tiene o usted sospecha una
necesidad especial o discapacidad.
Por Favor traiga los siguientes documentos
con su aplicación:




Aplicación de Head Start firmada
Certificado de nacimiento del niño(a)
Tarjeta de Seguro Social del niño(a)
Tarjeta de vacunas del niño(a)
 Prueba de Residencia (recibo del agua o luz)
 Medicaid Chip o cualquier otro seguro médico
del niño(a)
 Prueba de ingresos de la familia (los últimos 4
talonarios de cheque / W2 o los taxes)
Si le aplica
 Carta notariada de Establecimiento de
vivienda familiar
 Declaración de prueba de residencia notariada
 Documentación de custodia legal
 Documentos mas recientes del ARD / IEP
 SSI / TANF/ Estampillas de comida / Becas /
Manutención
 Su niño(a) podría recibir servicios de
transportación de bus.
