High INTENDED USE This assay is intended for the in-vitro measurement of specific, high avidity IgG autoantibodies against double stranded deoxyribonucleic acid (dsDNA) present in human serum, as an aid to the diagnosis of systemic lupus erythematosus (SLE), in conjunction with other serological test results and clinical findings. Sufficient materials are supplied to allow a maximum of 40 samples to be tested in duplicate or 88 in single, with a calibration curve and positive, negative and single stranded DNA controls. This kit can be run either as a fully quantitative assay, or as a screening assay, using the cut-off control. BACKGROUND The use of anti-dsDNA antibodies as a diagnostic marker for SLE and in the subsequent monitoring of disease activity has been of great benefit to clinicians1 since first being recognised by Ceppelini et al in 19572 and confirmed by its inclusion in the 1982 revised American College of Rheumatology criteria for the classification of SLE3. There are three commonly used assay methods employed today that can be differentiated on the basis of the avidity of the dsDNA antibodies detected4: Enzyme linked immunosorbant assay (ELISA), of which there are many variants, classically detect a wide spectrum of antibody avidities. The Crithidia luciliae immunofluorescent assay (IFA) detects antibodies biased to those with lower avidities and Farr radioimmunoassays (RIA)5 classically measure only the high avidity antibodies by virtue of the ammonium sulphate precipitation step which precludes low avidity antibodies. The FARRZYME assay incorporates a more stringent set of assay conditions8, which are intended to remove the low avidity weakly bound antibodies. Some studies6,7 have shown a relationship between antibody avidity and the progression of the disease, where the selection of the assay facilitates the differentiation between patients with a benign form of SLE and those with skin and renal involvement, particularly in cases of lupus nephritis 8,9. Generally there is good agreement among all the assays using sera from patients with ‘definite’ systemic lupus erythematosus (SLE), however there is significant discordance in patients with inactive disease, or those that do not meet the standard classification for SLE. 3 PRINCIPLE OF THE ASSAY Microwells are pre-coated with calf thymus dsDNA antigen. The calibrators, controls and diluted patient samples are added to the wells and autoantibodies recognising the dsDNA antigen bind during the first incubation. After washing the wells to remove all unbound proteins, purified peroxidase labelled rabbit antihuman IgG (γ chain specific) conjugate is added. The conjugate binds to the captured human autoantibody and the excess unbound conjugate is removed by a further wash step. The bound conjugate is visualised with 3,3’, 5,5’ tetramethylbenzidine (TMB) substrate which gives a blue reaction product, the intensity of which is proportional to the concentration of autoantibody in the sample. Phosphoric acid is added to each well to stop the reaction. This produces a yellow end point colour, which is read at 450nm. 4 PRECAUTIONS 4.1 WARNING • • Kathon Sodium Azide ProClin™ 300 Bromonitrodioxane Methylisothiazone 0.02% 0.099% 0.045% 0.002% 0.002% All human sera supplied have been tested at donor level and found negative for Hepatitis B surface antigens and antibodies to HIV 1 and 2 and Hepatitis C virus. However, these tests cannot guarantee the absence of infectious agents. Proper handling and disposal methods should be established and only personnel qualified in handling of potentially infectious materials should be permitted to use this kit. Sodium azide may react with lead and copper plumbing to form explosive Kathon is an irritant and may cause sensitisation by skin contact. The stop solution contains 3M phosphoric acid which is an irritant. Avoid contact with skin and eyes. Any deviation may affect assay performance, and the results obtained. Pay attention to specific ‘Notes’ and warnings throughout these Instructions for Use. Reagents from different batch numbers of kits are NOT interchangeable. If large numbers of tests are performed care should be taken to ensure that all reagents are from the SAME batch. All strips used must be taken from the same foil pouch. Substitution of any component may lead to incorrect results. To avoid reagent contamination, only use new or clean plastic / glassware. Never return unused reagents to the bottles. Do not leave reagent bottles uncapped; any resulting evaporation or contamination will lead to inconsistent results. TMB substrate must not be exposed to light or water. Microbially contaminated, haemolysed or lipaemic serum and specimens containing particulate matter should not be used. The use of calibrated pipettes and appropriate internal QC samples is recommended. The use of automated assay systems, sample dilutors and other automated equipment may lead to differences in results when compared to the manual procedure. It is the responsibility of any laboratory to fully validate the system, and ensure the results fall within the limits as defined in this insert and associated QC certificate. All equipment used must be calibrated and maintained according to the manufacturer’s instruction. 4.3 STORAGE AND STABILITY • • • 5 • • • • 6 The kit should be stored at 2-8°C and should not be frozen. Inappropriate storage temperatures will affect the results. Diluted wash buffer can be stored at room temperature (20-24°C) for a maximum of 4 weeks. The expiry date of the kit is shown on the outer label. SAMPLE COLLECTION AND STORAGE Blood samples should be collected by venepuncture allowed to clot naturally and the serum separated. The serum may be stored at 2-8°C for up to 7 days prior to assay10, or for prolonged storage, aliquoted and stored at -20°C or below. Repeated thawing and freezing should be avoided. Serum samples should not be heat-inactivated, as this may give false positive results. MATERIALS 6.1 MATERIALS SUPPLIED • • • • • • • • • • • • • Instruction leaflet: Giving full assay details. Coloured yellow, ready to use. dsDNA Wash Buffer (10x Concentrate): 2 bottles containing 50mL of a 10fold concentrated buffer for washing the wells. dsDNA Calibrators: 5 vials each containing 1.2mL of diluted human serum, with the following concentrations of anti-dsDNA autoantibody: 1000, 333, 111, 37, 12.3 IU/mL. Ready to use. dsDNA Cut-off Control: 1 bottle containing 1.2mL of diluted human serum. The expected value is given on the QC certificate. Insert Code: E072, Version: 11 December 2008, Page 1 of 13 6.2 ADDITIONAL MATERIALS AND EQUIPMENT – not supplied • • • • • • 7 Automatic microplate plate washer: This is recommended, however, plate washing can be performed manually. Plate reader: Capable of measuring optical densities at 450nm referenced on air. Distilled or deionised water: This should be of the highest quality available. Calibrated micropipettes: For dispensing 1000, 100 & 10µL. Multichannel pipette: Recommended for dispensing 100µL volumes of conjugate, substrate and stop solution. Glass/plastic tubes: For sample dilution. ASSAY METHOD 7.1 PRE-ASSAY STEPS 1. • • Bring the kit to room temperature The kit is designed for room temperature operation (20-24°C). Remove the kit from storage and stand at room temperature for approximately 60 minutes. Wells must not be removed from the foil bag until they have reached room temperature. Note: The kits may be maintained at room temperature for up to 1 week. 2. Kit components Gently mix each kit component before use. 3. Wash buffer (10 x concentrate) dilution Add 100mL of the wash buffer concentrate to 900mL of distilled water (1 in 10 dilution) into a clean container and mix. Smaller volumes can be diluted as appropriate. Note: Diluted wash buffer can be stored at room temperature for up to 4 weeks, therefore only dilute the appropriate amount. 4. Sample dilution Dilute 10µL of each sample with 1000µL of sample diluent (1:100) and mix well. Note: Diluted sample must be used within 8 hours. 5. Strip and frame handling Place the required number of wells in the strip holder, position from well A1, filling columns from left to right across the plate. When handling the plate, squeeze the long edges of the frame to prevent the wells falling out. Note: Return unused wells to the foil bag immediately with the two desiccant pouches and re-seal tightly to minimise exposure to moisture. Take care not to puncture or tear the foil bag, see below. WARNING: Exposure of wells to moisture or contamination by dust or other particulate matter will result in antigen degradation, leading to poor assay precision and potentially false results. 7.2 ASSAY METHOD Maintain the same dispensing sequence throughout the assay. 1. 2. Sample addition Dispense 100µL of each calibrator (quantitative assay) or cut-off control (qualitative assay), assay controls and diluted (1:100) sample into the appropriate wells of the plate provided. Note: Samples should be added as quickly as possible to the plate to minimise assay drift, and the timer started after the addition of the last sample. Incubate at room temperature for 30 minutes. Washing The washing procedure is critical and requires special attention. An improperly washed plate will give inaccurate results, with poor precision and high backgrounds. After incubation remove the plate and wash wells 3 times with 250-350µL wash buffer per well. Wash the plate either by using an automatic plate washer or manually as indicated below. After the final automated wash, invert the plate and tap the wells dry on absorbent paper. 8 2. 3. Conjugate addition Dispense 100µL of conjugate into each well, blot the top of the wells with a tissue to remove any splashes. Note: To avoid contamination, never return excess conjugate to the reagent bottle. Incubate at room temperature for 30 minutes. 4. Washing Repeat step 2. 5. Substrate (TMB) addition Dispense 100µL of TMB substrate into each well, blot the top of the wells with a tissue to remove any splashes. Note: To avoid contamination never return excess TMB to the reagent bottle. Incubate at room temperature in the dark for 30 minutes. 6. Stopping Dispense 100µL of stop solution into each well. This causes a change in colour from blue to yellow. 7. Optical density measurement Read the optical density (OD) of each well at 450nm on a microplate reader, within 30 minutes of stopping the reaction. Calculate mean optical densities (For assays run in duplicate only). For each calibrator, control and sample calculate the mean OD of the duplicate readings. The percentage coefficient of variation (%C.V.) for each duplicate OD should be less than 15%. 8.2 SCREEN ASSAY – CALCULATION OF RESULTS 1. Quality assessment of the optical densities • The OD of the positive control should be greater than 0.5 and greater than the OD of the cut-off control. • The OD of the negative control should be less than the OD of the cut-off control. • The OD of the cut-off control should be greater than that of the negative control but less than 0.5. • The OD of the single stranded control should be less than the OD of the cutoff control. 2. Results interpretation Using the cut off control, interpret the results according to the following table: RESULT OD equal to or greater than cut-off control OD less than cut-off control INTERPRETATION Suspected of having anti-dsDNA antibodies ACTION Test in the quantitative assay Negative Report as negative 8.3 QUANTITATIVE ASSAY - CALCULATION OF RESULTS 1. Plot calibration curve The calibration curve can be plotted either automatically or manually as follows by plotting the anti-dsDNA autoantibody concentration on the log scale against the OD on the linear scale for each calibrator: • Automatic - use appropriately validated software, and the curve fit that best fits the data. • Manual - using log/linear graph paper, draw a smooth curve through the points (not a straight line or point to point). 2. Treatment of anomalous points If any one point does not lie on the curve, it can be removed. If the absence of this point means that the curve has a shape dissimilar to that of the sample calibration curve, or more than one point appears to be anomalous, then the assay should be repeated. 3. Calculation of autoantibody levels in controls and diluted samples Read the level of the anti-dsDNA autoantibody in the controls and diluted samples directly from the calibration curve. The control values should fall within the range given on the QC Certificate. Note: The calibrator values have been adjusted by a factor of 100 to account for a 1:100 sample dilution. No further correction is required. 4. Assay calibration The assay is calibrated in IU/mL against the WHO reference calibrator Wo806. 5. • Limitations This assay, by virtue of its ability to detect only high avidity anti-dsDNA antibodies, may miss some forms of SLE where patients have only antibodies of low avidity. It might not be suitable for applications where a total anti-dsDNA antibody measurement is required. This kit is used to aid diagnosis only. A positive result suggests certain diseases, which must be confirmed by clinical findings and other serological tests. The results obtained from this assay are not diagnostic proof of the presence or absence of disease. Automated plate washers should not be programmed to include a soak step. Plates can be washed manually as follows: a. Flick out the contents of the plate into a sink. b. Tap the wells dry on absorbent paper. c. Fill each well with 250-350µL of wash buffer using a multichannel pipette. d. Gently shake the plate on a flat surface. e. Repeat a-d twice. f. Repeat a and b Do not leave the wash in the wells for longer than it takes to fill the whole plate. RESULTS AND QUALITY CONTROL 8.1 QUANTITATIVE/QUALITATIVE ASSAYS 1. Quality control In order for an assay to be valid, all the following criteria must be met: • Calibrators /cut-off control (as applicable) and positive, negative and single stranded controls must be included in each run. • The values /ODs obtained for all the controls should be in the ranges specified on the QC Certificate. • Quantitative assay only: the curve shape should be similar to the calibration curve, shown on the QC Certificate. If the above criteria are not met, the assay is invalid and the test should be repeated. • • 9 EXPECTED VALUES The following ranges have been established based on the results obtained from testing samples from both and healthy blood donors (n=150) and SLE patients (n = 224) on FARRZYME: INTERPRETATION Negative result ≤ 30 IU/mL > 30 IU/mL Positive result The 150 sera from normal blood donors all gave results below 17.0 IU/mL, with 146 (97%) of the results below 12.3 IU/mL which is the lower measuring limit of the kit. 22.8% of the SLE patient sera gave positive FARRZYME results (>30 IU/mL). The results shown below are of a comparision study between the same 224 SLE samples of the FARRZYME and a conventional anti-dsDNA EIA assay, which measures both low and high avidity antibodies to dsDNA. Samples giving borderline results in the conventional dsDNA assay were treated as negative when calculating positive and negative percentage agreement and overall agreement. By contrast, in 100 patients with various connective tissue diseases and other autoimmune diseases the incidence was 4% and 5% respectively for the two assays. Unit values obtained with the FARRZYME assay may not agree with those obtained in other assays reporting in IU/mL, as FARRZYME only measures the high avidity anti-dsDNA antibody subset. The ranges are provided as a guide only. ELISA assays are very sensitive and capable of detecting small differences in sample populations. It is recommended that each laboratory determine its own normal range, based on the population techniques and equipment employed. 10 10.1 PERFORMANCE CHARACTERISTICS MEASURING RANGE The measuring range of the assay is 12.3 – 1000 IU/mL. 10.2 CONFIRMATION OF ASSAY SENSITIVITY Confirmation that the FARRZYME assay can distinguish between two samples with values close to the bottom of the measuring range (120 and 165% of the lowest calibrator) was obtained by statistical analysis (Student’s t-test) of the results obtained from testing multiple replicates of each sample. 10.3 SPECIFICITY, AGREEMENT 189 samples from patients with SLE, together with 35 samples positive for dsDNA antibodies by Crithidia or ELISA and 28 samples from healthy normals were tested by FARRZYME, by Crithidia luciliae immunofluorescent assay and also in a conventional dsDNA ELISA. + Positive percent agreement Negative percent agreement Overall agreement FARRZYME 14 183 + *Positive percent agreement Negative percent agreement Overall agreement 67.3% 92.9% 87.3% Conventional dsDNA ELISA + Border-line 47 3 1 33 51 117 58.8% 97.7% 85.3% INTERFERING SUBSTANCES Various serum types were assayed to test the possible effect of interfering substances using an Interference Check A plus kit (Kokusai, Japan). Positive percent agreement was reduced because the latter assay is known to detect anti-DNA antibodies of low avidity11, unlike the FARRZYME assay. Die Kalibratoren, Kontrollen und verdünnten Patientenproben werden in den Vertiefungen inkubiert, so dass in diesem ersten Inkubationsschritt dsDNA-Autoantikörper an das immobilisierte Antigen binden können. Nach dem Waschen der Vertiefungen - um ungebundenes Protein zu entfernen - wird Peroxidase-markiertes Kaninchen anti-Human-IgG-Konjugat zugegegeben. Das gebundene Konjugat wird mit Hilfe von 3,3’,5,5’ Tetramethylbenzidin (TMB) sichtbar gemacht. TMB ergibt ein blaues Reaktionsprodukt, dessen Intensität proportional zur Autoantikörper-Konzentration der Probe ist. Um die Reaktion zu stoppen, wird in jede Vertiefung Phosphorsäure pipettiert. Das daraus resultierende gelbe Reaktionsprodukt wird bei 450nm gemessen. 4 WARNUNGEN UND VORSICHTSMAßNAHMEN 4.1 WARNUNGEN • • • • EINFÜHRUNG Die Verwendung von anti-dsDNA-Antikörper als diagnostischer Marker und zur Verlaufskontrolle der Krankheitsaktivität bei SLE hat seit der erstmaligen Beschreibung von Ceppelini et al im Jahr 19572 den behandelnden Ärzten1 große Verbesserungen gebracht. Diese Erkenntnisse wurden 1982 bestätigt, indem sie vom American College of Rheumatology in die überarbeiteten Klassifikationskriterien für SLE3 aufgenommen wurden. Es werden heute drei allgemeine Testmethoden eingesetzt, die sich darin unterscheiden, dass anti-dsDNA-Antikörper unterschiedlicher Avidität erfasst werden4. Enzym-Immunoassays (ELISA), die es in verschiedenen Varianten gibt, erfassen in der Regel Antikörper von sehr unterschiedlicher Avidität. In der indirekten Immunfluoreszenz (IFT) mit Crithidia luciliae werden Antikörper mit niedriger Avidität auch miterfasst, wohingegen im Farr Radioimmunoassay (RIA)5 nur die hoch-aviden Antikörper erfasst werden, da aufgrund eines Fällungsschrittes mit Ammoniumsulfat die niedrig-aviden Antikörper entfernt werden. Bei dem FARRZYME-Test wurden stringentere Testbedingungen gewählt8, wodurch die nur schwach bindenden niedrig-aviden Antikörper entfernt werden. In einigen Studien6,7 konnte ein Zusammenhang zwischen der Antikörper-Avidität und der Krankheitsprogression gezeigt werden, wobei die Wahl der Testmethode die Unterscheidung zwischen Patienten mit einer benignen Form des SLE und solchen mit Haut- und Nierenbeteiligung, besonders bei einer Lupus-Nephritis8,9, erleichtert. Im allgemeinen zeigen alle Methoden bei Seren von Patienten mit bestätigtem systemischen Lupus erythematodes (SLE) eine gute Übereinstimmung, aber eine signifikante Varianz bei Patienten mit inaktivem SLE, oder bei Patienten, die nicht der Standardklassifikation von SLE entsprechen. Das Ausgangsmaterial zur Erstellung der Kalibratoren und Kontrollen stammt aus menschlichem Blut. Jede Einzelspende wurde bezüglich Antikörpern gegen Human-Immunschwäche-Virus (HIV 1 & 2), Hepatitis C-Virus und Hepatitis-B-Oberflächenantigen (HBsAG) untersucht und als negativ befunden. Es gibt aber zur Zeit keine absolut sicheren Testmethoden zum Auschluss von Infektionsträgern. Deshalb sollten die Reagenzien als potentiell infektiös behandelt werden. Umgangs- und Entsorgungsmethoden sollten denen für potentiell infektiösem Materi al entsprechen und der Test nur von entsprechend geschultem Personal durchgeführt werden. Kathon wirkt reizend und kann bei Hautkontakt zu einer Sensibilisierung führen. Die Stopp-Lösung enthält 3M Phosphorsäure. Sie ist reizend, deshalb Kontakt mit Haut und Augen vermeiden. Verschüttete Reagenzien entsprechend beseitigen, dabei sind ökologische und gesetzliche Bestimmung zu beachten. • Dieser Test sollte nur von entsprechend geschultem Laborpersonal durchgeführt werden. Testdurchführung strikt nach Arbeitsanleitung. Jegliche Abweichung kann die Testqualität und Ergebnisse beeinflussen. Beachten Sie die spezifischen ‚Hinweise‘ und Warnungen in dieser Arbeitsanleitung. Reagenzien unterschiedlicher Chargen dürfen NICHT untereinander gemischt oder gemeinsam verwendet werden. Bei großem Testdurchsatz muss darauf geachtet werden, dass alle Reagenzien der GLEICHEN Charge entstammen. Alle Mikrotiterstreifen müsen aus demselben Folienbeutel entnommen werden. Der Austausch einer Testkomponente kann inkorrekte Ergebnisse zur Folge haben. Zur Vermeidung von Kontaminationen der Reagenzien immer saubere Plastik- oder Glasgefäße verwenden. Niemals unbenutztes Reagenz in die Flaschen zurückgeben. Reagenzienflaschen nicht für längere Zeit offen stehenlassen. Die daraus resultierende Verdunstung oder Verunreinigung führt zu widersprüchlichen Ergebnissen. TMB darf weder mit Licht noch mit Wasser in Berührung kommen! Die Verwendung von mikrobiell oder mit Partikeln verunreinigter, hämolytischer oder lipämischer Seren vermeiden. Es wird empfohlen kalibrierte Pipetten und geeignete interne Kontrollen zu verwenden. Die Verwendung von ELISA-Automaten, Pipettierautomaten oder anderen automatisierten Geräten kann im Vergleich zur manuellen Testdurchführung zu abweichenden Ergebnissen führen. Jedes Labor muss das System vollständig validieren und sicherstellen, dass die Ergebnisse innerhalb des in dieser Arbeitsanleitung und dem beigefügten QC-Zertifikat Spezifikationen liegen. Die verwendeten Geräte und Pipetten müssen gemäß der Herstellerangaben kalibriert und gewartet werden. 4.3 LAGERUNG UND STABILITÄT • • • 5 • • • • 6 Den Kit bei 2-8°C lagern, nicht einfrieren. Lagerung bei falscher Temperatur beeinflusst die Ergebnisse. Der verdünnte Waschpuffer ist in einem sauberen Gefäß bei Raumtemperatur (20-24°C) bis maximal 4 Wochen haltbar. Die Haltbarkeit des Kits wird auf dem Außenetikett angegeben. PROBENENTNAHME UND -VORBEREITUNG Blutproben über Venenpunktur sammeln und auf natürliche Weise gerinnen lassen. Serum vom Gerinnsel trennen. Die Seren können bei 2-8°C bis zu 7 Tage vor dem Test gelagert werden10. Für eine längere Lagerung empfiehlt es sich, die Seren unverdünnt und aliquotiert bei mindestens -20°C einzufrieren. Wiederholtes Einfrieren und Auftauen der Proben vermeiden. Serumproben nicht hitzeinaktivieren, dies kann zu falsch-positiven Ergebnissen führen. MATERIALIEN 6.1 GELIEFERTE MATERIALIEN • Arbeitsanleitung: mit genauen Angaben zur Testdurchführung. • QC-Zertifikat: mit Angabe der Leistungsdaten der Charge. • dsDNA Coated Wells (dsDNA-beschichtete Mikrotiterstreifen): 12 Mikrotiterstreifen mit 8 vereinzelbaren Vertiefungen, die mit KalbsthymusdsDNA beschichtet sind. Die Streifen sind in einem wiederverschließbaren Folienbeutel, der ein Trockenmittel enthält, verpackt. Type III Sample Diluent (Probendiluens Typ III): 2 Flaschen mit je 50mL gebrauchsfertigem Puffer zur Probenverdünnung. Farbcodierung: gelb. dsDNA Wash Buffer (10x Concentrate) (Waschpuffer dsDNA 10-fach Konzentrat): 2 Flaschen mit 50mL eines 10-fach konzentrierten Puffers zum Waschen der Vertiefungen. dsDNA Calibrator(s) (dsDNA-Kalibratoren): 5 Flaschen mit je 1,2mL verdünntem Humanserum mit den folgenden Konzentrationen an antidsDNA-Autoantikörpern: 1000; 333; 111; 37; 12,3 IU/mL. Reaktionsende 100µL Stopp-Lösung in jedes Well pipettieren. Die Farbe schlägt von blau nach gelb um. 7. Farbmessung Die optische Dichte (OD) bei 450nm in jedem Well mit einem Mikrotiterplatten-Reader innerhalb von 30 Minuten nach Zugabe der StoppLösung bestimmen. 6.2 BENÖTIGTE, NICHT IM KIT ENTHALTENE MATERIALIEN • • • • • • 7 Automatisches Mikrotiterplatten-Waschgerät: ist empfehlenswert, aber die Platten können auch manuell gewaschen werden. Mikrotiterplatten-Reader: Messung der optischen Dichte in den Vertiefungen bei 450nm. Destilliertes Wasser: Es sollte von höchster Qualität sein. Kalibrierte Micropipetten: zum Pipettieren von 1000, 100 & 10µL. Mehrkanal-Pipette: zum Pipettieren von Volumina von 100µL. Glas-oder Plastikgefäße: zur Probenverdünnung. TESTDURCHFÜHRUNG 7.1 TESTVORBEREITUNG 1. • • Kit auf Raumtemperatur erwärmen: Die optimale Temperatur für die Testdurchführung liegt bei 20-24°C. Den Kit nach Entnahme aus dem Kühlschrank ca. 60 Minuten bei Raumtemperatur stehen lassen. Die Wells dürfen erst nach Errreichen der Raumtemperatur aus dem Folienbeutel entnommen werden. Hinweis: Der Kit kann bis zu 1 Woche bei Raumtemperatur gelagert werden. 2. Kit-Komponenten Jede Kit-Komponente vor Gebrauch kurz schütteln. 3. Waschpuffer (10-fach-Konzentrat) verdünnen Dazu 100mL Waschpuffer-Konzentrat und 900mL destilliertes Wasser (1/10Verdünnung) in ein sauberes Gefäß geben und mischen. Es können auch entsprechend kleinere Volumina verdünnt werden. Hinweis: Der verdünnte Puffer ist bei Raumtemperatur bis zu 4 Wochen haltbar; deshalb nur die benötigte Menge verdünnen. 4. 5. Verdünnung der Proben 10µL jeder Probe mit 1000µL Probendiluens verdünnen (1:100) und gut mischen. Hinweis: Die verdünnten Proben müssen innerhalb von 8 h gemessen werden. Umgang mit Streifen und Rahmen Die benötigte Anzahl Wells aus dem Folienbeutel entnehmen und in den Rahmen setzen. Dabei von Position A1 ausgehend die Spalten von links nach rechts auffüllen. Achten Sie darauf, die langen Seiten des Rahmens zusammenzudrücken, damit die Wells nicht herausfallen können. Hinweis: Um die Kontaktzeit mit der Luftfeuchtigkeit zu minimieren die nicht verwendeten Wells sofort in den wiederverschließbaren Folienbeutel mit dem Trockenmittel geben und gut verschließen. Darauf achten, dass der Folienbeutel nicht beschädigt wird. ACHTUNG: Kontakt mit Feuchtigkeit oder Verunreinigung durch Staub oder anderen Partikeln kann einen Antigen-Abbau verursachen. Daraus resultiert eine schlechte Präzision und potentiell falsche Ergebnisse. 8 1. • • • 2. 1. Pipettieren der Kalibratoren, Kontrolle und Proben 100µL von jedem Kalibrator (quantitative Bestimmung) oder Cut-Off-Kontrolle (qualitative Bestimmung), jeder Kontrolle und verdünnten (1:100) Proben in das dafür laut Plattenschema vorgesehene Well pipettieren. Hinweis: Die Proben müssen so schnell wie möglich pipettiert werden um eine ‚Assay-Drift’ zu vermeiden; die Inkubationszeit läuft ab Zugabe der letzten Probe. 30 Minuten bei Raumtemperatur inkubieren. 2. Waschen der Platte Sorgfältiges Waschen der Wells ist wichtig für eine hohe Präzision und genaue Testergebnisse. Bei nicht korrekt gewaschenen Wells können ungenaue Ergebnisse mit schlechter Präzision und hohem Hintergrund auftreten. Nach der Inkubation die Wells dreimal mit 250-350µL Waschpuffer waschen. Das Waschen kann mit einem automatischen Platten-Waschgerät oder manuell erfolgen. Nach dem letzten Waschschritt die Wells durch leichtes Klopfen auf eine absorbierende Unterlage (z.B. Papiertücher) trocknen. 1. • • • • 2. Lassen Sie den Waschpfuffer nur solange in den Wells, bis alle Wells gefüllt sind. 3. Pipettieren des Konjugats 100µL Konjugat in jedes Well pipettieren. Die Ränder der Wells mit Filterpapier abtrocknen um Spritzer zu entfernen. Hinweis: Um Verunreinigungen zu vermeiden, niemals überschüssiges Konjugat in die Konjugat-Flasche zurückgeben. 30 Minuten bei Raumtemperatur inkubieren. Berechnung der mittleren optischen Dichte (OD) (nur bei Doppelbestimmung): Für jeden Kalibrator und jede Kontrolle und Probe die mittlere optischen Dichte (OD) der Doppelbestimmungen berechnen. Der Variationskoeffizient (%VK) der Doppelbestimmungen sollte jeweils unter 15% liegen. Qualitätskriterien für die OD Die OD der Positivkontrolle sollte größer als 0,5 und größer als die OD der Cut-Off-Kontrolle sein. Die OD der Negativkontrolle sollte kleiner als die OD der Cut-Off-Kontrolle sein. Die OD der Cut-Off-Kontrolle sollte größer als die der Negativkontrolle aber kleiner als 0,5 sein. Die OD der Einzelstrangkontrolle sollte kleiner als die OD der Cut-OffKontrolle sein. Interpretation der Ergebnisse Bei Verwendung der Cut-Off-Kontrolle sind die Ergebnisse wie in der folgenden Tabelle angegeben zu interpretieren: OD ≥ Cut-Off-Kontrolle Ergebnis Interpretation Verdacht auf antidsDNA-Autoantikörper Aktion Im quantitativen Test messen OD < Cut-Off-Kontrolle Negativ Befund: Negativ 8.3 QUANTITATIVE PROBE - BERECHNUNG VON RESULTATEN 1. Erstellen der Kalibrationskurve Die Kalibrationskurve kann entweder automatisch oder manuell erstellt werden. Dazu wird die anti-dsDNA-Autoantikörper-Konzentration auf der logarithmischen Skala gegen die OD-Werte auf der linearen Skala für jeden Kalibrator aufgetragen. • Automatisch: Geeignete, validierte Software verwenden. Kalkulationsmodus verwenden, der am besten zu den Daten passt. • Manuell: Halblogarithmisches Millimeterpapier verwenden, eine Ausgleichskurve durch die Punkte legen (keine Gerade oder Punkt-zu-PunktVerbindung). 2. Umgang mit abweichenden Kalibrationspunkten Liegt ein Messpunkt nicht auf der Kalibrationskurve, so kann er weggelassen werden. Der Test sollte wiederholt werden, wenn die daraus resultierende Kalibrationskurve deutlich von der im QC-Zertifikat angegebenen Kurvenform abweicht oder, wenn mehrere Kalibrationspunkte zu starke Abweichungen aufweisen. 3. Berechnung der Autoantikörper-Konzentrationen in den Kontrollen und verdünnten Proben Die anti-dsDNA-Autoantikörper-Konzentrationen der Kontrollen und verdünnten Proben direkt aus der Kalibrationskurve ablesen. Die Kontrollwerte müssen in den im QC-Zertifikat angegebenen Bereich fallen. Hinweis: Die Standard-Probenverdünnung von 1:100 ist in der Kalibrationskurve bereits berücksichtigt. Es sind keine weiteren Berechnungen nötig. 4. Kalibration des Tests Der Test ist in IU/mL gegen den Referenzkalibrator Wo80 der WHO kalibriert6. 5. • Grenzen des Tests Mit diesem Test, der nur hoch-avide anti-dsDNA-Antikörper erfasst, können einige Formen von SLE, bei denen die Patienten nur niedrig-avide Antikörper produzieren, als negativ gefunden werden. Daher kann der Test für solche Fälle, in denen die gesamte Population der anti-dsDNA-Antikörper erfasst werden soll, nicht eingesetzt werden. Dieser Test dient nur zur Unterstützung der Diagnose. Ein positives Ergebnis ist ein Hinweis auf bestimmte Erkrankungen, es muss aber durch den klinischen Befund und andere serologische Untersuchungen bestätigt werden. Die Testergebnisse können nicht als diagnostischer Beweis für das Vorliegen bzw. Nichtvorliegen einer Krankheit verwendet werden. Bei Verwendung von automatischen Mikrotiterstreifen-Waschgeräte sollte keine Standzeit einprogrammiert werden. Manuelles Waschen: a. Den Inhalt der Wells in den Ausguss schütten, dabei die langen Seiten des Rahmens zusammendrücken. b. Die Wells auf einer absorbierenden Unterlage (z.B. Papiertücher) leicht ausklopfen. c. 250-350µL Waschpuffer in der Arbeitskonzentration mit einer Mehrkanalpipette in die Wells pipettieren. d. Die Mikrotiterplatte auf einer flachen Unterlage stehend leicht schütteln e. Zweimaliges Wiederholen der Schritte a bis d. f. Die Wells wie unter a und b entleeren. Qualitätskontrolle Nur, wenn alle nachfolgend aufgeführten Kriterien erfüllt sind, ist der Test gültig. Kalibratoren/Cut-off-Kontrolle (je nach Ansatz) und die Kontrollen (Positiv, Negativ, Einzelstrang) müssen bei jedem Testansatz mitgeführt werden. Das Ergebnis/OD der Kontrollen muss in dem angegebenen Wertebereich (siehe QC-Zertifikat) liegen. Nur bei quantitativer Bestimmung: Die berechnete Kalibrationskurve sollte der auf dem QC-Zertifikat abgebildeten Kalibrationskurve ähnlich sein. Ist dies nicht der Fall, ist der Test ungültig und muss wiederholt werden. 8.2 Screening Test - Berechnung der Ergebnisse 7.2 TESTDURCHFÜHRUNG Alle Reagenzien in der gleichen Reihenfolge pipettieren. ERGEBNISSE UND QUALITÄTSKONTROLLE 8.1 Quantitativer/ Qualitativer Test • • Insert Code: E072, Version: 11 December 2008, Page 5 of 13 9 ERWARTETE WERTE 10.5 Die nachfolgenden Bereiche wurden anhand von Proben von 150 gesunden Blutspendern und 224 SLE-Patienten, mit FARRZYME ermittelt. INTERPRETATION Negativ ≤ 30 IU/mL > 30 IU/mL Positiv PRÄZISION Die Intra- und Inter-Assay-Präzision wurden anhand von 6 Proben, deren Konzentrationen im Bereich der Kalibrationskurve lagen, ermittelt. Der Mittelwert und der Variationskoeffizient VK% sind nachfolgend für jede Probe aufgelistet. Die Intra-Assay Präzision wurde durch 20-fache Messung in einem Assay bestimmt. Bei allen 150 Blutspendern wurden Konzentrationen unterhalb von 17,0 IU/mL gefunden, wobei 146 (97%) unterhalb von 12,3 IU/mL, der unteren Nachweisgrenze des Tests, lagen. 22,8% der SLE-Seren waren im FARRZYME-Test postiv (>30 IU/mL). In der nachfolgenden Tabelle ist eine Vergleichsstudie mit 224 SLE-Seren zwischen FARRZYME und einem konventionellen anti-dsDNA ELISA, der sowohl niedrig- als auch hoch-avide Antikörper erfasst, zusammengefasst. Proben, die in dem konventionellen anti-dsDNA ELISA ein grenzwertiges Ergebnis aufwiesen, wurden für die Berechnung der positiven und negativen Übereinstimmung und der gesamten Übereinstimmung, als negativ bewertet. FARRZYME Positiv Negativ Die oben angegebenen Werte dienen nur der Orientierung. ELISAs sind sehr sensitiv und in der Lage kleine Unterschiede in der ,Probenpopulation’ festzustellen. Daher wird empfohlen, dass jedes Labor auf der Basis der lokalen Population und der verwendeten Geräte und Techniken seinen eigenen Normalbereich ermittelt. 10 10.1 LEISTUNGSDATEN MESSBEREICH Der Messbereich dieses Kits ist 12,3 – 1000 IU/mL. 10.2 SENSITIVITÄT Es wurde durch eine statistische Analyse (Student-t-Test) bestätigt, dass der FARRZYME-Test zwischen zwei Proben mit Konzentrationen im untersten Messbereich (120% bzw. 165% des niedrigsten Kalibrators) unterscheiden kann. Hierzu wurde jede Probe mehrfach gemessen. 10.3 SPEZIFITÄT UND ÜBEREINSTIMMUNG 189 Proben von SLE-Patienen wurden zusammen mit 35 Proben, die entweder auf Crithidien oder im ELISA dsDNA-positiv waren, und 28 Proben von gesunden Blutspendern mit dem FARRZYME, in dem IFT auf Crithidia luciliae und in einem konventionellen anti-dsDNA ELISA gemessen. + Positive Übereinstimmung % Negative Übereinstimmung % Gesamt-Übereinstimmung FARRZYME + *Positive Übereinstimmung % Negative Übereinstimmung % Gesamt-Übereinstimmung 67,3% 92,9% 87,3% Konventioneller dsDNA ELISA + Grenzwertig 47 3 1 33 51 117 58,8% 97,7% 85,3% *Für die Berechnung der Übereinstimmung der beiden Tests, wurden die im konventionellen ELISA im grenzwertigen Bereich liegenden Proben als negativ eingestuft. Tous les équipements doivent être calibrés et doivent respecter les instructions du fabricant. • • • • 1 INDICATIONS Ce coffret permet de quantifier in vitro les autoanticorps IgG spécifiques de haute affinité dirigés contre l’acide désoxyribonucléique double brin (ADN db) dans le sérum humain. Il apporte une aide au diagnostic du lupus érythémateux disséminé (LED) en conjonction avec des résultats d’autres tests sérologiques et les aspects cliniques. Le matériel fourni est suffisant pour tester au maximum 40 échantillons en double ou 88 échantillons en simple avec une courbe de calibration, un contrôle positif, un contrôle négatif et un contrôle ADN simple brin. Ce coffret peut-être utilisé comme test quantitatif ou de dépistage en utilisant un contrôle seuil cut-off. 2 PRESENTATION GENERALE L’utilisation des autoanticorps anti-ADN double brin en tant que marqueur diagnostic du LED et de suivi de l’activité de la maladie est un grand bénéfice pour les cliniciens1 depuis qu’ils ont été découverts par Ceppelini et al en 19572 et confirmés par leur inclusion en 1982 par l’institut américain de rhumatologie comme critères de classification du LED3. Il existe trois méthodologies couramment utilisées actuellement qui peuvent être différenciées par l’avidité des anticorps ADNdb détectés4: La technique ELISA qui comporte plusieurs variantes détecte un large éventail d’avidité d’anticorps. L’immunofluorescence (IFA) sur lame de Crithidia luciliae détecte les anticorps de faible avidité et le test de Farr RIA5 mesure seulement les anticorps de haute avidité par précipitation avec du sulphate d’ammonium excluant ainsi les anticorps de faible avidité. Le test FARRZYME comprend un ensemble de conditions d’analyses plus strictes8, qui sont destinées à enlever les anticorps faiblement liés et de faible avidité. Quelques éudes6,7 ont montré un lien entre l’avidité des anticorps et la progression de la maladie, où la sélection de l’analyse facilite la différenciation entre les patients avec une forme bénigne de LED et ceux avec complications rénales ou de peau, particulièrement dans le cas des néphrites lupiques8,9. En générale, il existe une bonne corrélation entre toutes les techniques pour des sérums provenant de malades ayant un lupus érythémateux disséminé (LED) défini. Par contre pour des sérums provenant de patients dont la maladie est non active ou de patients non classifiables, il existe une discordance significative. 3 PRINCIPE Les micropuits sont recouverts de l’antigène ADN db de thymus de veau. Les calibrateurs, les contrôles et les échantillons dilués sont déposés dans les puits permettant ainsi la liaison spécifique de l’anticorps à l’ADN db fixé. Après avoir rincé les puits pour éliminer toute trace de protéines non accrochées, un anticorps de lapin anti IgG humaines (spécifique de la chaîne γ) purifié et conjugué à la péroxydase est déposé. Au cours de l’incubation, le conjugué enzymatique se lie aux IgG ayant reconnu l’ADN db. L’excès de conjugué marqué non accroché est éliminé par des lavages. Le conjugué accroché est visualisé en utilisant du 3,3',5,5' tétraméthyl benzidine (TMB) qui donne un produit de réaction bleu, l’intensité est proportionnelle à la concentration d’autoanticorps dans l’échantillon. L’acide phosphorique est ajouté à chaque puits pour arrêter la réaction. Le produit final induit est coloré en jaune et la densité optique est lue à 450nm. 4 • • • • • 4.3 STOCKAGE ET STABILITE • Tous les sérums des donneurs humains ont subi un dépistage négatif pour les anticorps anti-VIH 1 et 2, les anticorps anti-VHC et l’Ag Hbs. Toutefois, ces tests ne peuvent garantir l’absence totale d’agents infectieux. Tous les échantillons doivent donc être manipulés comme des produits potentiellement infectieux. Seul un personnel qualifié dans la manipulation d’échantillons potentiellement infectieux est autorisé à utiliser ce coffret. L’azide de sodium peut réagir avec les tuyauteries en plomb ou cuivre pour former des azides de métaux explosifs. Il est nécessaire d’ajouter de grands volumes d’eau pour éviter la formation d’azides. Les tampons et sérums fournis dans ce coffret contiennent plusieurs inhibiteurs d’enzymes listés ci-dessous. Les manipuler avec précaution. Le coffret doit être stocké à 2-8°C et ne doit pas être congelé. Un stockage à des températures non appropriées peut entraîner de faux résultats. Le tampon de lavage dilué peut être stocké à température ambiante (2024°C) pendant 4 semaines. La date de péremption figure sur l’étiquette du coffret. • • 5 • • • • 6 ECHANTILLONS Les échantillons doivent être prélevés par ponction veineuse et le sérum doit être séparé après coagulation. Les sérums peuvent être conservés à 2-8°C pendant 7 jours avant les tests10 ou aliquotés et congelés à -20°C minimum pour une conservation plus longue. Eviter les congélations et décongélations répétées. Les sérums ne doivent pas être inactivés par la chaleur, ceci peut induire de faux positifs. Pipette multicanaux : recommandée pour la distribution de volumes de 100µL de conjugué, de substrat et de solution d’arrêt. Tubes en verre ou en plastique : pour la dilution des échantillons. PROCEDURE 7.1 ETAPES PREALABLES 1. • • Ramener le coffret à température ambiante Ces coffrets sont opérationnels à une température comprise entre 20 et 24°C. Avant utilisation, laisser le coffret à température ambiante pendant environ 60 minutes. Ne pas retirer les barrettes de leur sachet aluminium durant cette période. Attendre que les barrettes soient à température ambiante. Note : Les coffrets peuvent être gardés à température ambiante pendant une semaine. 2. Composants du coffret Mélanger chaque composant du coffret avant utilisation. 3. Dilution du tampon lavage (concentré 10 fois) Ajouter 100mL de tampon de lavage concentré à 900mL d’eau distillée (dilution au 1/10) dans un recipient propre et mélanger. De plus petits volumes peuvent être dilués. Note : Le tampon de lavage dilué peut être stocké à température ambiante pendant 4 semaines. Il est recommandé de ne diluer que le volume nécessaire. 4. Dilution des échantillons Diluer 10µL de chaque échantillon avec 1000µL de diluant (dilution 1:100) et mélanger. Note : Les échantillons dilués doivent être utilisés dans les 8 heures suivant la dilution. 5. Manipulation des barrettes Sortir le nombre nécessaire de puits et les enfiler sur le cadre. A partir de la position A1, remplir les colonnes de gauche à droite. Lors de la manipulation du cadre, le maintenir dans le sens de la longueur afin d’empêcher les barrettes de sortir de leur logement. Note : les puits non utilisés doivent être immédiatement replacés dans leurs étuis contenant un dessicateur. Fermer hermétiquement les étuis afin de minimiser l’exposition à l’humidité. Faire attention de ne pas percer l’étui. AVERTISSEMENT : L’exposition des puits à l’humidité ou à la contamination avec de la poussière ou des particules de matière induit une dégradation de l’antigène, induisant une faible précision des résultats et potentiellement des résultats faux positifs. 7.2 PROCEDURE 8 2. 2. Dépôt de l’échantillon Déposer 100µL de chaque calibrateur (test quantitatif) ou de contrôle seuil (test qualitatif), contrôle et échantillon dilué (1:100) dans les puits appropriés suivant le plan de plaque. Note : Les échantillons doivent être déposés sur la plaque aussi rapidement que possible afin de minimiser les écarts, le décompte du temps doit commencer aprés l’addition du dernier échantillon. Incuber pendant 30 minutes à température ambiante. Lavage de la plaque La procédure de lavage est très importante et requiert une attention spéciale. Une plaque mal lavée donnera des résultats incorrects avec une précision faible et un bruit de fond important. Après incubation, laver trois fois les puits avec 250 à 350µL de tampon de lavage en utilisant un laveur automatique ou manuellement comme indiqué ci-dessous. Après le lavage final, renverser la plaque et sécher les puits en tapant la plaque sur du papier absorbant. Ne pas programmer d’étape de trempage pour les laveurs de plaques. Pour un lavage manuel : a. Jeter le contenu des plaques dans un évier. b. Tapoter les puits sur un papier absorbant. c. Remplir chaque puits avec 250 à 350µL de tampon de lavage à l’aide d’une pipette multicanaux. d. Agiter délicatement la plaque sur une surface plate. e. Répéter 2 fois cette procédure. f. Terminer en tapotant la plaque sur du papier absorbant. Ne pas laisser la solution de lavage dans les puits plus longtemps que le temps nécessaire au remplissage total de la plaque. 3. Addition du conjugué Déposer 100µL de conjugué par puits. Note : Afin d’éviter les contaminations, ne jamais remettre l’excès de conjugué dans le flacon d’origine. Incuber à température ambiante pendant 30 minutes. 4. Lavage de la plaque (cf. 2) 5. Addition du substrat TMB Déposer 100µL de TMB dans chaque puits. Cette étape doit être effectuée rapidement. Note : Afin d’éviter les contaminations, ne jamais remettre l’excès de TMB dans le flacon d’origine. Incuber 30 minutes à température ambiante dans l’obscurité. 6. Arrêt de la réaction Ajouter 100µL de solution d ‘arrêt dans chaque puits. Ceci induit un changement de couleur du bleu au jaune. 7. Mesure de la densité optique La densité optique (DO) de chaque puits doit être lue à 450nm à l’aide d’un lecteur de plaques dans les 30 minutes suivant l’arrêt de la réaction. Calcul des moyennes de DO (Pour les tests ayant été faits en double.) Pour chaque calibrateur, contrôle et échantillon, calculer la DO moyenne. Le pourcentage du coefficient de variation (% CV) pour chaque duplicat doit être inférieur à 15%. 8.2 TEST DE DEPISTAGE – CALCUL DES RESULTATS 1. Evaluation de la qualité des densité optiques • La DO du contrôle positif doit être supérieure à 0.5 et supérieure à la DO du contrôle seuil (cut-off). • La DO du contrôle négatif doit être inférieure à la DO du contrôle seuil. • La DO du contrôle seuil doit supérieure à la DO du contrôle négatif mais inférieure à 0.5. • La DO du contrôle simple brin doit être inférieure à la DO du contrôle seuil. 2. Interpretation des résultats En utilisant le contrôle seuil, interpréter les résultats en accord avec le tableau ci-dessous. DO inférieure à la DO du Négatif Rendre négatif cut-off 8.3 ANALYSE QUANTITATIVE - CALCUL DES RÉSULTATS 1. • • Etablissement de la courbe de calibration La courbe de calibration peut être tracée automatique ou manuellement en plaçant les concentrations en anticorps anti-ADN db (échelle logarithmique) sur l’axe des X et la densité optique de chaque calibrateur sur l’axe des Y (échelle linéaire). Automatique : choisir un tracé de courbe pour lequel les résultats sont les meilleurs. Manuel : utiliser du papier log/linéaire, tracer une courbe qui joints les points (pas une droite, ni une courbe point à point). 2. Traitement des points anormaux Si seul un point n’est pas sur la courbe, il peut être supprimé. Si en l’absence de ce point, la courbe a une forme différente de celle du certificat de contrôle de qualité ou si plusieurs points ne sont pas sur la courbe, le test doit être répété. 3. Calcul des concentrations d’autoanticorps dans les contrôles et les échantillons dilués Lire la concentration en autoanticorps anti-ADN db des contrôles et des échantillons dilués à partir de la courbe de calibration. Les valeurs des contrôles doivent être comprises dans les limites indiquées sur le certificat de contrôle qualité. Note : Les valeurs des calibrateurs ont été ajustées d’un facteur 100 pour tenir compte de la dilution des échantillons (1:100). Aucune autre correction n’est nécessaire. 4. Calibration du test Le test est calibré en UI/mL par rapport au calibrateur de référence OMS Wo806. 5. • Limites Ce test, par sa faculté de détecter uniquement les anticorps anti-ADNdb de hautes avidités, peut omettre quelques formes de LED chez les patients présentant uniquement des anticorps de faibles avidités. Il peut ne pas être convenable si le taux global des anticorps anti-ADNdb doit être mesuré. Ce coffret est utilisé pour aider au diagnostic seulement. Un résultat positif suggère certaines maladies qui doivent être confirmées par les données cliniques et par d’autres tests sérologiques. Les résultats obtenus ne sont pas une preuve diagnostique de la présence ou de l’absence de maladies. Suivre la même séquence de dépôt durant tout le test. 1. RESULTATS ET CONTROLE DE QUALITE 8.1 QUANTITATIVE/QUALITATIVE ASSAYS 1. Contrôle de qualité Pour valider le test, tous les critères suivants doivent être respectés: • Les calibrateurs/contrôles seuil (si besoin) le contrôle positif ADN db, le contrôle négatif et le contrôle ADN sb doivent être inclus dans chaque test. • Les valeurs obtenues pour tous les contrôles doivent être dans les gammes spécifiées sur la fiche de contrôle qualité. • Test quantitatif uniquement: La forme de la courbe doit être similaire à celle fournie sur le certificat de contrôle qualité. Si les critères ci-dessus ne sont pas respectés, le test est invalide et doit être répété. • • 9 VALEURS NORMALES Les gammes suivantes ont été établies sur la base des résultats obtenus à partir d’échantillons de donneurs de sang sains (n=150) et de patients présentant un LED (n=224) avec FARRZYME. INTERPRETATION Résultat négatif ≤ 30 UI/mL > 30 UI/mL Résultat positif Les 150 sérums de donneurs de sang sains ont donné des résultats inférieurs à 17,0 UI/mL avec pour 146 (97%) des résultats inférieurs à 12,3 UI/mL, ce qui est la limite basse de détection du coffret. 22,8% des sérums de patients présentant un LED ont donné des résultats positifs sur FARRZYME (> 30 UI/mL). Les résultats ci-dessous montrent une étude comparative entre 224 échantillons LED analysés par FARRZYME et un test ELISA anti-ADNdb classique qui mesure les anticorps anti-ADNdb de haute et de basse avidité. Les échantillons donnant des résultats douteux avec le test ELISA conventionnel ont été traités comme négatifs lors du calcul des pourcentages de négatifs et postifs et pour la corrélation d’ensemble. Se suministra material suficiente para analizar 40 muestras en duplicado (o bien 88 muestras sin duplicado), con una curva de calibración, control positivo y negativo y de cadena simple de ADN. Este kit se puede ejecutar como ensayo totalmente cuantitativo, o como screening, usando el control cut-off. RESUMEN Y EXPLICACIÓN El uso de anticuerpos anti-dsDNA como marcadores diagnósticos del LES y la consecuente monitorización de la actividad de la enfermedad ha sido de gran beneficio para los profesionales de laboratorio clínico1 desde que se reconoció por primera vez en Ceppelini et al en 19572 y fue confirmado en 1982 en los criterios revisados del American College of Rheumatology para la clasificación del LES3. Hoy en día hay tres métodos de análisis de uso común que se diferencian según la avidez de los anticuerpos dsDNA detectada4: El análisis inmunoabsorbente ligado a enzimas (ELISA), del que hay muchas variantes, detecta generalmente un amplio espectro de avidez de anticuerpos. El análisis por inmunofluorescencia Crithidia luciliae (IFA) detecta los anticuerpos unidos a aquellos de más baja avidez y los radioinmunoensayos Farr (RIA)5 determinan generalmente sólo aquellos anticuerpos de alta avidez mediante el paso de precipitación de sulfato de amonio que impide la baja avidez de anticuerpos. El análisis FARRZYME incorpora un conjunto de condiciones analíticas más estrictas8, que tienen como objetivo eliminar los anticuerpos débilmente unidos. Algunos estudios6,7 han demostrado que la avidez de anticuerpos está relacionada con la progresión de la enfermedad. La selección de un análisis u otro facilita la diferenciación entre pacientes con una forma benigna de LES y pacientes con problemas renales y dermatológicos, especialmente en casos de lupus de riñón8,9. Por lo general hay concordancia entre todos los análisis que usan suero de pacientes con lupus eritematosos sistémicos (LES) ‘definidos’, sin embargo hay una discrepancia significativa entre pacientes con la enfermedad inactiva, o que no cumplen los parámetros estándard de clasificación para LES. 3 PRINCIPIO DEL ENSAYO Los pocillos de la placa están tapizados con antígeno (ADN de doble cadena de timo de ternera). Los calibradores, controles y muestras del paciente diluidas se añaden a los pocillos, los autoanticuerpos presentes que reconozcan el antígeno (ADN de doble cadena) se unirán durante la primera incubación. Después de lavar los pocillos para eliminar las proteínas no unidas, se añade un anticuerpo de conejo contra IgGs humanas (específico de cadena γ) marcado con peroxidasa purificada. El conjugado se unirá a los anticuerpos capturados en la placa y el conjugado que está en exceso y no se ha unido se eliminará con los siguientes pasos de lavado. El conjugado unido se visualizará con sustrato 3,3’,5,5’ tetrametilbenzidina (TMB) que da lugar a un producto de reacción de color azul, la intensidad de éste es proporcional a la concentración de autoanticuerpos en la muestra. Se añade ácido fosfórico a cada pocillo para parar la reacción. La adición del ácido da lugar a un color amarillo final que se leerá a 450nm. 4 • • • PRECAUCIONES • • • • • • • • Todos los sueros humanos suministrados se han testado a nivel de donante y se han encontrado negativos para antígenos de superficie de Hepatitis B y para anticuerpos contra HIV 1 y 2 y virus Hepatitis C. Por otra parte, estos tests no pueden garantizar la ausencia de agentes infecciosos. Se debe utilizar métodos correctos de manipulación y de eliminación de residuos. Este kit solo debe utilizado por personal cualificado en la manipulación de material potencialmente infeccioso. La azida sódica puede reaccionar con el plomo y el cobre produciendo azidas metálicas explosivas. Para la eliminación de residuos con azida, lavar con agua abundante. Los tampones y los sueros suministrados con el kit contienen varios inhibidores enzimáticos que están detallados a continuación. Se deben manipular cuidadosamente. Kathon es un irritante y puede causar sensibilización por contacto con la piel. La solución de parada contiene ácido fosfórico 3M y es un irritante. Evitad el contacto con la piel y los ojos. Si se derrama algún reactivo se debe limpiar de forma apropiada y eliminar los residuos siguiendo la normativa local y la legislación medioambiental vigente. Este producto sólo debe ser utilizado por personal adecuadamente preparado. Se recomienda seguir estrictamente el protocolo. Cualquier variación sobre el protocolo puede afectar a los resultados. Prestad especial atención a las Notas específicas y a todas las advertencias presentes en estas Instrucciones de Uso. No se pueden intercambiar reactivos de distintos lotes y/o kits. Si se deben realizar muchos tests, prestad atención a que todos los reactivos sean del mismo lote. Todas las tiras de pocillos deben provenir de una misma bolsa. La substitución de alguno de los componentes del kit puede dar lugar a resultados incorrectos. Para evitar que se contaminen los reactivos se debe usar siempre material plástico o de cristal nuevo o limpio. No se debe retornar el reactivo no utilizado a la botella original. Nunca se deben dejar las botellas destapadas, cualquier evaporación o contaminación puede dar lugar a resultados incorrectos. El sustrato TMB no puede estar expuesto a la luz ni al agua. Los sueros que muestren contaminación bacteriana, que sean lipémicos o hemolizados o bien que contengan partículas deberán ser descartados. Se recomienda usar pipetas calibradas y controles de calidad internos. El uso de sistemas automáticos de ensayo, dilutores de muestras y otros equipos automáticos puede dar lugar a diferencias cuando se comparan resultados con el método manual. Es responsabilidad del laboratorio validar completamente el sistema utilizado y asegurarse de que los resultados estén dentro de los límites que se han definido en este protocolo y en el certificado de Control de Calidad lo acompaña. Todo el equipamiento se debe calibrar y mantener siguiendo las instrucciones del fabricante. 4.3 ALMACENAMIENTO Y ESTABILIDAD • • • 5 • • • • 6 El kit se debe almacenar entre 2-8°C y no debe ser congelado. Una temperatura de almacenamiento inapropiada puede afectar los resultados obtenidos con el kit. El tampón de lavados diluido se debe almacenar a temperatura ambiente (20-24°C) como máximo 4 semanas. La fecha de caducidad del kit está detallada en la etiqueta. RECOLECCIÓN Y ALMACENAMIENTO DE LAS MUESTRAS Las muestras de sangre se deben recoger por punción venosa, permitir que coagule de forma natural y separar el suero. El suero puede conservarse a 2-8°C durante un máximo de 7 días10, o para periodos más largos, distribuir el suero en alícuotas y conservarlo a -20°C, o temperatura inferior. Se debe evitar realizar congelaciones y descongelaciones de forma repetida. Las muestras de suero no deben ser inactivadas por calor, esto podría producir falsos positivos. Lector de placas: para medir densidades ópticas a 450nm en aire. Agua destilada o desionizada: de la mejor calidad posible. Micropipetas calibradas: para dispensar 1000, 100 y 10µL. Pipeta multicanal: recomendada para dispensar volúmenes de 100µL de conjugado, sustrato y solución parada. Tubos de plástico/vidrio: para la dilución de las muestras. MÉTODO ENSAYO 7.1 PASOS PREVIOS AL ENSAYO 1. Dejar el kit a temperatura ambiente Este kit está pensado para ser procesado a temperatura ambiente (20-24°C). Sacar el kit del almacenamiento y dejarlo a temperatura ambiente unos 60 minutos. Los pocillos no se deben sacar de la bolsa hasta que no se hayan atemperado. Nota: Este kit se puede mantener a temperatura ambiente durante una semana. 2. Componentes del kit Mezclar generosamente cada componente del kit antes de ser utilizado. 3. Tampón de lavado (concentrado 10 x) dilución. Añadir 100mL del tampón de lavado concentrado a 900mL de agua destilada (dilución 1 en 10) en un contenedor limpio y mezclar. Se pueden diluir volúmenes pequeños de forma apropiada. Nota: El tampón de lavados diluido se puede almacenar a temperatura ambiente durante 4 semanas, se recomienda diluir solo la cantidad necesaria. 4. Dilución de la muestra Diluir 10µL de cada muestra con 1000µL de diluyente de muestras (1:100) y mezclar bien. Nota: La muestra diluida se debe usar en las 8 horas siguientes. 5. Manipulación del marco y las tiras Disponer el número necesario de pocillos en la tira. Disponerlos desde la posición A1, rellenando columnas de izquierda a derecha a lo ancho de la placaAl manipular la placa, apretar los extremos del marco para evitar que caigan los pocillos. Nota: Los pocillos no usados se deben volver a guardar en la bolsa con autocierre con los dos desecantes rápidamente, de esta forma se evitará una exposición excesiva a la humedad. Cuidado al manipular la bolsa, no agujerearla o cortarla. ATENCION: Si los pocillos se exponen a la humedad o a la contaminación por polvo, el antígeno puede degradarse, esto daría lugar a un ensayo con una baja precisión y resultados potencialmente erróneos. 7.2 MÉTODO DE ENSAYO Mantener la misma secuencia de dispensación a lo largo de todo el ensayo. 1. 2. Dispensación de la muestra Dispensar 100µL de cada calibrador (análisis cuantitativo), o control cut-off (análisis cualitativo), controles del ensayo y muestra diluida (1:100) en los pocillos de la placa. Nota: Las muestras se deben dispensar rápidamente a la placa para disminuir el tiempo de procesado de la placa y también la diferencia de tiempo entre la dispensación de la primera y la última muestra. Incubar a temperatura ambiente durante 30 minutos. Lavados El procedimiento de lavados es un punto crítico y requiere una atención especial. Un mal lavado de la placa puede dar lugar a resultados poco precisos, con una baja precisión y mucho fondo. Después de la incubación, lavar los pocillos 3 veces con 250-350µL cada pocillo con tampón de lavados. La placa se puede lavar de forma automática o manual. Después del lavado automático invertir la placa sobre un papel de filtro y dar unos golpecitos sobre los pocillos para permitir que se sequen. 6. Parada de la reacción Dispensar 100µL de la solución de parada a cada pocillo. Esto provoca un cambio de color pasando del azul al amarillo. 7. Determinación de la densidad óptica Leer la densidad óptica (DO) de cada pocillo a 450nm en un lector de microplacas, durante los 30 minutos siguientes a la parada de la reacción. 8 8.1 ENSAYOS CUANTITATIVOS/CUALITATIVOS 1. Control de calidad Para que un ensayo se considere válido, debe cumplir los siguientes criterios: • Calibradores / control cut-off (según convenga), control positivo y negativo y el control de cadena simple de ADN siempre se deben incluir cada vez que se realiza un ensayo. • Los valores obtenidos con los controles deben estar dentro del rango descrito en el Certificado de Control de Calidad. • Sólo análisis cuantitativo: La forma de la curva debe ser similar a la curva que se incluye en el Certificado de Control de Calidad. Si no se cumplen estos criterios, el ensayo no se puede considerar válido y el test se debe repetir. 2. Calcular la media de las densidades ópticas (Sólo para ensayos que se realicen por duplicado) Para cada calibrador, control y muestra, calcular la media de las DO de los duplicados de las lecturas. El porcentaje del coeficiente de variación (%CV) para cada duplicado de DO debe ser inferior al 15%. 8.2 SCREENING - CÁLCULO DE RESULTADOS 1. Evaluación de calidad de las densidades ópticas • La DO del control positivo debería ser mayor que 0,5 y superior a la DO del control cut-off. • La DO del control negativo debería ser menor que la DO del control cut-off. • La DO del control cut-off debería ser superior al del control negativo pero menos que 0,5. • La DO del control de cadena simple debería ser menor que la DO del control cut-off. 2. Dispensación del sustrato (TMB) Dispensar 100µL de sustrato TMB en cada pocillo, secar la parte superior de los pocillos con un pañuelo para eliminar cualquier posible salpicadura. Nota: Para evitar contaminaciones no devolver nunca el TMB no utilizado a la botella de reactivo. Incubar a temperatura ambiente, en la oscuridad, durante 30 minutos. Dibujar la curva de calibración La curva de calibración se debe dibujar de forma automática o manual como sigue: dibujar la concentración de anticuerpos contra ADN de doble cadena en la escala logarítmica y la DO en la escala lineal para cada uno de los calibradores. Automática – usar un Software validado y dibujar la curva que mejor represente a los datos. Manual – usando un papel de gráfica semi-logarítmico, dibujar una curva entre los puntos (no una recta o punto a punto). 2. Tratamiento de los puntos anómalos Si alguno de los puntos está muy alejado de la curva, éste se puede eliminar. Si la eliminación de este punto, hace variar mucho la curva y ésta no se asemeja en forma a la curva de calibración o bien hay más de un punto anómalo, se deberá repetir el ensayo. 3. Cálculos de los niveles de autoanticuerpos y muestras diluidas. Leer el nivel de los anticuerpos anti ADN de doble cadena a partir de la curva de calibración. Los valores de los controles deben estar dentro del rango definido en el Certificado de Control de Calidad. Nota: Los valores de los calibradores se deben ajustar por un factor 100 para compensar la dilución 1:100 de la dilución de la muestra. No se necesitan más correcciones. 4. Calibración del ensayo El ensayo está calibrado en IU/mL contra un calibrador de referencia OMS (Wo80)6. 5. • Limitaciones Este análisis, dada su habilidad para detectar únicamente los anticuerpos anti-dsDNA de alta avidez, puede no funcionar en algunas formas de LES en las que el paciente sólo presenta anticuerpos de baja avidez. Puede no ser adecuado para aplicaciones en las que se precisa la determinación total de anticuerpos anti-dsDN. Este kit debe ser usado tan solo como una ayuda al diagnóstico. Un resultado positivo sugiere la presencia de determinadas enfermedades, pero debe ser confirmado por los síntomas clínicos y otros tests serológicos. Los resultados obtenidos con este ensayo no son una prueba diagnóstica de la presencia o ausencia de enfermedad. En los lavadores automáticos no se debe programar ningún paso en el que se deje la placa en remojo. Las placas se pueden lavar manualmente de la siguiente forma: a. Eliminar el contenido de la placa en un fregadero. b. Dejar secar la placa invertida en un papel absorbente. c. Llenar cada pocillo con 250-350µL de tampón de lavados usando una pipeta multicanal. d. Agitar la placa sobre una superficie plana. e. Repetir a-d dos veces. f. Repetir a y b. No dejar el tampón de lavados en los pocillos más tiempo que el necesario para llenar toda la placa. RESULTADOS Y CONTROL DE CALIDAD • • 9 VALORES ESPERADOS Los siguientes rangos se han establecido en base a los resultados obtenidos con FARRZYME del análisis de muestras de donantes de sangre sanos (n=150) y de pacientes con LES (n = 224): INTERPRETACION Resultado negativo ≤ 30 IU/mL > 30 IU/mL Resultado positivo Todas las 150 muestras de suero de donantes normales dieron resultados por debajo de 17,0 IU/mL, con 146 (97%) de los resultados por debajo de 12,3 IU/mL que es el límite inferior de medición del kit. El 22,8% de los sueros de pacientes de LES dieron resultados positivos con FARRZYME (>30 IU/mL). Los análisis ELISA son muy sensibles y capaces de detectar pequeñas diferencias entre distintas poblaciones de muestras. Se recomienda que cada laboratorio determine su propio rango normal, en base a la población, las técnicas y los equipos empleados. 10 CARACTERÍSTICAS DE FUNCIONAMIENTO 10.1 RANGO DE MEDICIÓN El rango de medición del análisis es 12,3 – 1000 IU/mL. 10.2 CONFIRMACIÓN DE LA SENSIBILIDAD DEL ANÁLISIS La confirmación de que el análisis FARRZYME puede distinguir entre dos muestras con valores cercanos al final del rango de medición (120 y 165% del calibrador inferior) se obtuvo por análisis estadístico (el t-test de Student) de los resultados obtenidos al analizar múltiples réplicas de cada muestra. 10.3 ESPECIFICIDAD, CONCORDANCIA 189 muestras procedentes de pacientes con LES, junto con 35 muestras positivas en anticuerpos dsDNA por Crithidia o ELISA y 28 muestras normales sanas, fueron analizadas con FARRZYME, con el análisis por inmunofluorescencia Crithidia luciliae y también con un ELISA dsDNA convencional. + Porcentaje de concordancia positiva Porcentaje de concordancia negativa Concordancia global FARRZYME + 37 18 Crithidia IFA 14 183 67,3% 92,9% 87,3% EIA FARRZYME demostró buena concordancia negativa con el análisis por inmunofluorescencia Crithidia luciliae. El ELISA convencional detecta anticuerpos tanto de alta como de baja avidez, dando como resultado una concordancia positiva reducida con el análisis FARRZYME. 10.4 SUSTANCIAS INTERFERENTES Se analizaron varios tipos de suero para determinar el posible efecto de sustancias interferentes mediante el kit plus Interference Check A (Kokusai, Japón). En cambio, en 100 pacientes con enfermedades varias del tejido conectivo y otras enfermedades autoinmunes, la incidencia fue del 4% y 5% respectivamente para los dos análisis. Los valores unitarios obtenidos con el análisis FARRZYME pueden no ser acordes a los obtenidos con otros análisis que dan resultados en IU/mL, ya que FARRZYME sólo determina el subconjunto de anticuerpos anti-dsDNA de alta avidez.
