BUS PASS APPLICATION PROCESS
1. Complete Application - a) Online at fullertonsd.org OR
b) In person on the day of your appointment
2. Address Verification - Need a most recent utility bill
3. Student Picture - Need a recent picture of student (Picture will be taken the day
of your appointment if student is present.)
4. Bus Pass Fees - Please refer to the Instructions and Fee Schedule information
sheet
a. If applying for financial assistance, income verification is required for each
working adult (current income tax returns and two most recent pay stubs)

1. Completar solicitud – a) En nuestro sitio web fullertonsd.org
b) En persona el dia de su cita.
2. Verificación de domicilio - necesita traer una reciente factura de servicios públicos
3. Fotografía de estudiante - necesita traer una fotografía reciente del estudiante (La
fotografía se puede tomar el mismo dia de su cita si el estudiante está presente.)
4. Tarifas para el pase del autobús - por favor, consulte la hoja de instrucciones y
lista de tarifas
a. Si esta solicitando asistencia financiera, necesita traer verificación de
ingresos para cada adulto que trabaja (declaracion de impuestos actual y
dos recientes talones de pago)
