September 13, 2013
Dear Parents,
We will be conducting hearing and visions screenings on Wednesday, September 25, 2013. This is a simple
procedure and will only take a few minutes per student. If you do not want your child’s hearing or vision
screened, please let your child’s teacher know before the screening date.
If your child is absent or does not pass the hearing and or vision screening, we will rescreen them in 30-45 days.
If your child has still not passed one or both of the screenings at this time, we will notify you.
If there are any complications, e.g. having a hearing aid, that could affect your child’s hearing screening, please
complete the form below and return it to the main office before the screening date.

Vamos a realizar exámenes de audición y visiones el Miércoles, 25 de septiembre 2013. Se trata de un
procedimiento sencillo y sólo tardará unos minutos por estudiante. Si no desea que la audición o la visión de su
hijo proyectado, por favor, deje que el maestro de su hijo antes de la fecha de selección.
Si su hijo está ausente o no pasa la audiencia o exámenes de la vista, vamos a seleccionar de nuevo en 30-45
días. Si su hijo aún no ha aprobado una o ambas de las proyecciones en este momento, se lo notificaremos.
Si hay alguna complicación, por ejemplo, tener un audífono, que podría afectar la prueba de audición de su hijo,
por favor completa el siguiente formulario y enviarlo a la oficina principal antes de la fecha de selección.
