[UK] 823360/05 – 04/2008 microtest-cell plate [COMP] complement Intended Use The microtest cell plate [MZP] Lymphoscreen ABC 60 is used for screening and differentiation of complement-dependent cytotoxic HLA-ABC antibodies (1,5). Each [MZP] contains a panel of deep-frozen HLA-ABC typed lymphocytes (6). Summary and Explanation Cytotoxic HLA-ABC antibodies can be formed after blood transfusions, after organ transplantations and during pregnancy (2,3,4). Using Lymphoscreen ABC 60, sera from pregnant women, from transfused or transplanted patients can be screened rapidly. The determination of cytotoxic HLA-ABC antibodies against a panel of HLA typed lymphocytes is a method which can be carried out in any HLA laboratory. Principle of the Microlymphocytotoxicity Test Lymphocytes with known antigens are incubated with an unknown serum and rabbit complement. If the corresponding antigens are present, the addition of the serum result in the presence of complement in lysis of the lymphocytes. This is rendered visible by staining (e.g. Eosin Y). The lysed and vital lymphocytes are assessed using an inverted phase contrast microscope. Reagent Description In Lymphoscreen ABC 60, peripheral blood lymphocytes from various, healthy blood donors with defined HLA antigens are deep-frozen in a microtest plate [MZP]. The membrane of the frozen lymphocytes is stabilized using DMSO (dimethyl sulphoxide). To perform the microlymphocytotoxicity test, Lymphoscreen ABC 60 is thawed at room temperature. The DMSO is removed using a washing medium, as it is highly cytotoxic at room temperature. One package of Lymphoscreen ABC 60 consists of 6 [MZP]. Each [MZP] contains a deep-frozen panel of 56 HLA-ABC typed lymphocytes for screening of one sample and 2 lymphocyte pools (each 2 wells) provided for positive and negative double controls. The control positions are marked on the plate. Each well of the plate contains 2 μl of lymphocyte suspension in 13 µl freezing medium. The cell count is approx. 7000 lymphocytes per well. Washing Procedure 1. Washing procedure by using of absorbent cellulose: - Allow the Lymphoscreen ABC 60 to thaw briefly at room temperature (18...22°C). Note: Ice should still be visible in the wells! Do not thaw more than 2 [MZP] at once! - Pipette 20μl washing medium immediately into each well, using the Terasaki dispenser. - Allow the [MZP] to stand for 15 minutes at room temperature (18...22°C). - Using the absorbent cellulose, soak up the washing medium from one corner of the [MZP]. With a second piece of cellulose remove the remaining washing medium row by row. Care: If the washing medium is not completely removed weakened reactions may occur owing to dilution of the sera being tested. - Cover the Lymphoscreen ABC 60 with cover oil (approx. 5 µl per well). or alternatively R ef er en ce Statement of Precautions [IVD] For in Vitro Diagnostic Use The test must be performed by well-trained and authorised laboratory technicians. All materials of human origin used in this product have been tested and found to be non-reactive for HBsAg, anti-HCV and anti-HIV-1/2 using FDA licensed test kits. However, all products of human origin should be considered to be potential transmitters of hepatitis, HIV or other infectious agents. Appropriate safety measures are recommended. Preparation of the Reagents - Prepare the washing medium: 10% (V/V) FCS in Lymphostabil. - Reconstitute the control HLA (pos.) in the volume of distilled water as stated on the label = positive control. - Reconstitute the control HLA (neg.) in the volume of distilled water as stated on the label = negative control. y [MZP] nl 823 300 O [REF] 7 se For the detection of HLA-ABC antibodies Additional Materials Required 1. * Lymphostabil (McCoy's medium 5A modified according to Park and Terasaki, Ca++ and Mg++ free) 2. Fetal calf serum (FCS), heat-inactivated, e.g. Sigma 3. * Control HLA (pos) 4. * Control HLA (neg) 5. Eosin Y (dissolved 5% in distilled water and filtered), e.g. Merck 6. Formaldehyde for histology, 37%, acid-free (filtered and adjusted to pH 7.2 ± 0.2 with 0.1 N sodium hydroxide solution), e.g. Merck 7. * Cover oil 8. Absorbent cellulose 9. * Cover slips 50x75 mm 10. Microlitre syringe, e.g. Hamilton no. 710 (0.100 ml syringe) 11. Volume dispenser, e.g. Hamilton PB 600-1 12. Terasaki dispenser, e.g. Hamilton no. 1725 (6 x 0.250 ml syringes) 13. Microdispenser for microtest plates, e.g. Greiner 14. Inverted phase contrast microscope only for alternative washing procedure with microplate centrifuge 15. Microplate centrifuge 16. 6-channel aspiration head, e.g. Greiner 17. Water-jet pump U Biotest Lymphoscreen ABC 60 Fo r Storage Lymphoscreen ABC 60 is stable up to the date stated on its label. Lymphoscreen ABC 60 must be stored at -70° C or below. The packages, which are delivered in dry ice, must be transferred immediately into storage at -70°C. Opened Lymphoscreen ABC 60 packages must not be stored together with dry ice. Indication of Deterioration If the negative control shows over 10% lysed lymphocytes and the background toxicity is >10%, the test should be repeated. If the positive control shows less than 80% lysed lymphocytes, the test must be repeated. Specimen Collection and Preparation Obtain the serum sample from: - approx. 3 ml native blood or - approx. 3 ml heparinised blood ( convert plasma to serum by adding thrombin) or - approx. 3 ml EDTA or ACD blood (convert plasma to serum by adding calcium chloride) The serum sample should either be used fresh or stored frozen at -20°C or below. Procedure Materials Provided Lymphoscreen ABC 60 (1 microtest plate [MZP] for 1 sample) lyophilised rabbit complement [COMP] (3x1 ml are added to each package) worksheet (6 are added to each package) 2. Washing procedure by using of microplate centrifuge: - Allow the Lymphoscreen ABC 60 to thaw briefly at room temperature (18...22°C). Note: Ice should still be visible in the wells! Do not thaw more than 2 [MZP] at once! - Pipette 20μl washing medium immediately into each well, using the Terasaki dispenser. - Centrifuge [MZP] immediately at 1000 x g for 30 seconds (without braking). - Carefully aspirate off the washing medium using the 6-channel aspiration head attached to the water-jet pump. Warning: Place manifold tips at the edge of the wells, so that only the washing medium is aspirated off, and not the sedimented cells. - Cover Lymphoscreen ABC 60 with cover oil (approx. 5 µl per well). Lymphoscreen ABC 60 is now ready to be used for the microlymphocytotoxicity test. Microlymphocytotoxicity Test Procedure - Pipette 2μl of the negative control into each of the wells 1A and 1B. - Pipette 2μl of the positive control into each of the wells 10A and 10B. The control positions are marked on each [MZP]. - Pipette 2μl of the serum sample into each of the remaining wells (1C up to 10C). - Incubate for 30 minutes at room temperature (18...22°C). - Reconstitute the complement [COMP]15 minutes before the incubation period is over in the volume of distilled water as stated on the label, under gentle swirling. DO NOT REFREEZE! DISCARD ANY RESIDUAL COMPLEMENT! - Add 5-6μl complement [COMP] per well and incubate for 60 minutes at room temperature (18...22°C). - Stain the cells for approx. 3 minutes with 3-4 μl of 5% Eosin Y per well. - Fix the reactions by adding of 6-7 µl of buffered formaldehyde per well. - If necessary, oil should be added to the microtest plates. - Cover the completed plate with a cover slip and read the results at least 30 minutes after completion of the test. If stored at 2...8°C evaluation will still be possible for up to 3 days. Read the tray in the following serpentine pattern which corresponds to the worksheet. 1A through 1F; 2F through 2A; 3A through 3F; etc. Quality Control The negative controls are in positions 1A and 1B and are used to test the viability of the lymphocytes. The viability should be higher than 90%. The positive controls are in position 10A and 10B and are used to check the complement activity. The positive control should yield more than 80% lysed lymphocytes. * available from Biotest FOR REFERENCE USE ONLY: DO NOT USE in place of package inserts provided with each product. nl O se U ef er en ce Limitations If the negative control shows over 10% lysed lymphocytes and the background toxicity is >10%, the test should be repeated. Possible reasons are: 1. Lymphoscreen ABC 60 was exposed to room temperature too long during unpacking. 2. Lymphoscreen ABC 60 was stored at a temperature less than -70°C. 3. Lymphoscreen ABC 60 was completely thawed before adding the washing medium. If the positive control shows less than 80% lysed lymphocytes, the test must be repeated. Possible reasons are: 1. Complement activity inadequate. 2. Dilution of the antibody in question through inadequate removal of the washing medium. 3. Opened packages or individual [MZP] have been stored together with dry ice. The guidelines of ASHI (American Society for Histocompatibility and Immunogenetics) and EFI (European Federation of Immunogenetics) do not stipulate which HLA anigens should be represented on a panel used for HLA antibody screening. Therefore based on the donors available at the time, we select those that will give us the most complete representation of the known HLA antigens. The phenotype frequencies for HLA-ABC will vary among different populations. The represented antigens in Lymphoscreen ABC may be different to the incidence of antigens in the population being tested. In Lymphoscreen ABC 60 mainly cells of Caucasian source are used. The test should only be used for initial HLA antibody determination. Other clinical and diagnostic findings should be used in addition when determining suitability for transplant. y Results The reactions are read under an inverted phase contrast microscope. Viable lymphocytes are bright and shining (= negative reaction); lysed lymphocytes are dark and larger (= positive reaction). The percentage of lysed lymphocytes in the total lymphocyte count is expressed as score values. % lysed cells evaluation 0 10% = score 1 negative 11 20% = score 2 doubtful negative 21 50% = score 4 weakly positive 51 80% = score 6 positive 81 100% = score 8 strongly positive score 0 unreadable The reactions read off must be entered in the appropriate column of the worksheet. The antigens of the positively reacting cells can be marked in the antigen table with a coloured marking pen. By selection of the antigens which are completely marked, the HLA specificity of the antibody is defined. If the specificity cannot be determined, the total of the positive reactions is expressed as a percentage of the size of the panel (% PRA = % Panel Reactive Antibodies). Number of positive reactions %PRA = ----------------------------------------- x 100 Number of cells in the panel Fo r R Specific Performance Characteristics Lymphoscreen ABC 60 for screening and differentiation of complementdependent cytotoxic HLA-ABC antibodies is preparaed using lymphocytes from donors whose cells have been tested with appropriate antisera and shown to possess or lack the antigens listed on the accompanying worksheet. Each lot is tested with at least 10 wellknown selected anti-HLA-ABC test sera, whereby the proportion of 10% false positive and 5% false negative reactions must not be exceeded. On testing with a negative control serum, the viability of the lymphocytes must be at least 90%. On testing with a positive control serum, at least 90% of the cells in each well must be lysed. Comparison data of Lymphoscreen ABC 60 and cell panel trays produced under guidelines and regulations of ASHI: 78/ 101 were non-reactive for both assays 22/ 101 were positive in both assays 1/ 101 were positive only in Lymphoscreen ABC 60 References 1. Jede [MZP] enthält ein Panel tiefgefrorener, HLA-ABC-typisierter Lymphozyten (6). Einleitung Zytotoxische HLA-Antikörper können nach Bluttransfusionen, nach Organtransplantationen und während einer Schwangerschaft entstehen (2,3,4). Mit Hilfe von Lymphoscreen ABC 60 ist ein schnelles Screening von Seren schwangerer Frauen, von transfundierten oder transplantierten Patienten möglich. Die Bestimmung von zytotoxischen HLA-ABC-Antikörpern gegen ein Panel HLA-typisierter Lymphozyten ist eine Methode, die in jedem HLA-Labor durchgeführt werden kann. Prinzip des Mikrolymphozytotoxizitätstests Lymphozyten mit bekannten HLA-Antigenen werden mit einem Serum unbekannter HLA-Spezifität und Komplement vom Kaninchen inkubiert. Im Falle der Anwesenheit des korrespondierenden Antigens führt die Zugabe des Serums in Gegenwart von Komplement zur Lyse der Lymphozyten. Diese wird mit einem Farbstoff (z.B.Eosin Y) sichtbar gemacht. Die Beurteilung der lysierten und vitalen Lymphozyten erfolgt mit einem inversen Phasenkontrastmikroskop. Waschvorgang 1. Waschvorgang unter Verwendung von saugfähigem Zellstoff - Lymphoscreen ABC 60 bei Raumtemperatur (18...22°C) kurz antauen lassen. Achtung: Eis sollte in den Kavitäten noch zu sehen sein! Nicht mehr als 2 [MZP] auf einmal auftauen! - 20 μl Waschmedium mit dem Terasaki-Dispenser zügig in jede Kavität pipettieren. - Die [MZP] 15 Minuten bei Raumtemperatur (18...22°C) stehen lassen. - Mit saugfähigem Zellstoff das Waschmedium von einer Ecke der [MZP] absaugen. Mit einem zweiten Tupfer das restliche Waschmedium Reihe für Reihe durch Abtupfen entfernen. Achtung: Falls das Waschmedium nicht komplett entfernt wird, können abgeschwächte Reaktionen durch Verdünnung der zu testenden Seren auftreten. - Lymphoscreen ABC 60 mit Decköl beschichten (ca. 5 µl pro Kavität). oder alternativ en ce Produktbeschreibung In Lymphoscreen ABC 60 sind periphere Lymphozyten von verschiedenen, gesunden Blutspendern mit definierten HLA-Antigenen in einer [MZP] tiefgefroren. Die Membran der gefrorenen Lymphozyten ist mit DMSO (Dimethylsulfoxid) stabilisiert. Für die Durchführung des Mikrolymphozytotoxizitätstests wird Lymphoscreen ABC 60 bei Raumtemperatur aufgetaut. DMSO wird mit einem Waschmedium entfernt, da es bei Raumtemperatur stark zytotoxisch wirkt. Eine Packung Lymphoscreen ABC 60 beinhaltet 6 [MZP]. Jede [MZP] enthält ein Panel von 56 tiefgefrorenen, HLA-typisierten Lymphozyten für das Screening eines Probanden und 2 Lymphozytenpools ( je 2 Kavitäten), die für positive und negative Doppelkontrollen vorgesehen sind. Die Positionen der Kontrollen sind auf jeder [MTP] farblich markiert. Jede Kavität enthält 2 μl Lymphozytensuspension in 13 µl Einfriermedium. Die Zellzahl beträgt ca. 7000 Lymphozyten pro Kavität. Vorbereitung der Reagenzien Waschmedium herstellen: 10% (V/V) FCS in Lymphostabil. Kontroll-HLA (pos.) in dem auf dem Etikett angegebenen Volumen destillierten Wassers rekonstituieren = Positivkontrolle. Wasserstrahlpumpe U Biotest Lymphoscreen ABC 60 Fo r R ef er Vorsichtsmaßnahmen Reagenz nur zur in-vitro-Diagnostik [IVD] Der Test ist nur von geschultem und autorisiertem Fachpersonal durchzuführen. Alle für dieses Produkt verwendeten Materialien humanen Ursprungs wurden mit FDA-lizenzierten Testkits auf HBsAg, Anti-HCV und Anti-HIV-1/-2 geprüft und erwiesen sich als nicht reaktiv. Jedoch sollten alle Produkte humanen Ursprungs als potentielle Überträger von Hepatitis, HIV oder anderen infektiösen Krankheitserregern betrachtet werden. Angemessene Sicherheitsvorkehrungen werden empfohlen. Lagerung Lymphoscreen ABC 60 ist bis zu dem auf dem Etikett angegebenen Datum haltbar. Lymphoscreen ABC 60 muß bei -70°C oder kälter gelagert werden. Die in Trockeneis angelieferten Packungen sollten unmittelbar in die -70°C Lagerung überführt werden. Geöffnete Lymphoscreen ABC 60 Packungen dürfen nicht zusammen mit Trockeneis gelagert werden. Warnhinweis Zeigt die Negativkontrolle mehr als 10 % lysierte Lymphozyten und ist die Grundtoxizität >10 % sollte die Testung wiederholt werden. Zeigt die Positivkontrolle weniger als 80 % lysierte Lymphozyten muß der Test wiederholt werden. Probenvorbereitung Gewinnung des Probandenserums aus: - ca. 3 ml Nativblut oder - ca. 3 ml Heparinblut (Plasma durch Thrombinzugabe in Serum überführen) oder - ca. 3 ml EDTA- oder ACD-Blut (Plasma durch Zugabe von Calciumchlorid in Serum überführen) Das Probandenserum sollte entweder frisch eingesetzt oder bei mindestens -20°C gelagert werden. Methode vorhandenes Material Lymphoscreen ABC 60 (1 [MZP] für 1 Probe) Lyophilisiertes [COMP] vom Kaninchen (3x1 ml sind je Packung beigelegt) Worksheets (6 Stück sind je Packung beigelegt) 2. Waschvorgang mit einer Plattenzentrifuge - Lymphoscreen ABC 60 bei Raumtemperatur (18...22°C) kurz antauen lassen. Achtung: Eis sollte in den Kavitäten noch zu sehen sein! Nicht mehr als 2 [MZP] auf einmal auftauen! - 20 μl Waschmedium mit dem Terasaki-Dispenser zügig in jede Kavität pipettieren. - [MZP] sofort bei 1000 x g für 30 Sekunden zentrifugieren (ohne Bremse). - Waschmedium mit 6fach Absaugkopf über Wasserstrahlpumpe vorsichtig absaugen. Achtung: Absaugkopf am Rand der Vertiefung ansetzen, damit nur das Medium und nicht die sedimentierten Zellen abgesaugt werden. - Lymphoscreen ABC 60 mit Decköl beschichten (ca. 5 µl pro Kavität). Lymphoscreen ABC 60 ist jetzt für den Mikrolymphozytotoxizitätstest gebrauchsfertig. Mikrolymphozytotoxizitätstest - Je 2 μl der Negativkontrolle in die Kavitäten 1A und 1B pipettieren. - Je 2 μl der Positivkontrolle in die Kavitäten 10A und 10B pipettieren. Die Kontrollpositionen sind auf der [MZP] markiert. - Je 2 μl des Probandenserums in die restlichen Kavitäten (1C bis 10C) pipettieren. - 30 Minuten bei Raumtemperatur (18...22°C) inkubieren. - [COMP] 15 Minuten vor Ablauf der Inkubationszeit mit dem auf dem Etikett angegebenen Volumen destillierten Wassers unter leichtem Schwenken rekonstituieren. NICHT WIEDER EINFRIEREN! KOMPLEMENTREST VERWERFEN! - 5-6 μl [COMP] je Kavität zugeben und 60 Minuten bei Raumtemperatur (18...22°C) inkubieren. - Mit 3-4 μl 5%iges Eosin Y je Kavität die Zellen ca. 3 Minuten anfärben. - Mit 6-7 μl gepufferter Formaldehydlösung je Kavität die Reaktionen fixieren. - Bei Bedarf die Platten zusätzlich ölen. - Die fertige Platte mit einem Deckglas abdecken und frühestens 30 Minuten nach Beendigung des Tests ablesen. Bei Aufbewahrung bei 2..8°C ist eine Beurteilung noch bis zu 3 Tagen möglich. Das Ablesen erfolgt entsprechend der beigelegten Worksheets mäanderförmig; von 1A nach 1F, von 2F nach 2A, von 3A nach 3F usw. Qualitätskontrolle Die Negativkontrolle befindet sich in den Positionen 1A und 1B und überprüft die Vitalität der Lymphozyten. Die Vitalität sollte mehr als 90 % betragen. Die Positivkontrolle befindet sich in den Positionen 10A und 10B und überprüft die Komplementaktivität. Die Positivkontrolle sollte mehr als 80 % lysierte Lymphozyten enthalten. * von Biotest lieferbar y nl O r R ef er en ce Fehlerquellen Zeigt die Negativkontrolle mehr als 10 % lysierte Lymphozyten und ist die Grundtoxizität >10 % sollte die Testung wiederholt werden. Mögliche Ursachen können sein: 1. Lymphoscreen ABC 60 wurde beim Umpacken zu lange der Raumtemperatur ausgesetzt. 2. Lymphoscreen ABC 60 wurde wärmer als bei -70°C gelagert. 3. Lymphscreen ABC 60 war völlig aufgetaut, bevor das Waschmedium zugegeben wurde. Zeigt die Positivkontrolle weniger als 80 % lysierte Lymphozyten muß der Test wiederholt werden. Mögliche Ursachen können sein: 1. Komplementaktivität nicht ausreichend. 2. Verdünnung des nachzuweisenden Antikörpers durch unzureichende Entfernung des Waschmediums. 3. Geöffnete Packungen oder einzelne [MZP] wurden gemeinsam mit Trockeneis gelagert. Weder in den ASHI (American Society for Histocompatibility and Immunogenetics)- noch in den EFI (European Federation of Immunogenetics)Richtlinien werden die HLA-Antigene benannt, die in einem Panel zur Differenzierung von HLA-ABC-Antikörpern vorhanden sein sollten. Deshalb werden die vorhandenen Spender so ausgewählt, dass möglichst alle bekannten HLAABC-Antigene vorhanden sind. Die Phenotyp-Frequenzen variieren zwischen den verschiedenen Populationen. Deshalb können die in Lymphoscreen ABC 60 repräsentierten Antigene unterschiedlich zur Antigenhäufigkeit der Population sein, aus der die getestete Probe stammt. Vitale Lymphozyten sind hell und leuchtend (=negative Reaktion), lysierte Lymphozyten sind dunkel und größer (=positive Reaktion). Fo Spezifische Charakteristika In Lymphoscreen ABC 60 werden zum Screening und zur Differenzierung komplementabhängiger, zytotoxischer HLA-ABC-Antikörper Lymphozyten von Spendern eingesetzt, die mit den entsprechenden HLA-Antiseren getestet wurden. Das Vorhandensein der Antigene ist im beigelegten Worksheet aufgelistet. Jede Charge wird mit mindestens 10 ausgewählten Anti-HLA-ABC-Testseren überprüft. Dabei darf der Anteil 10% falsch positiver und 5% falsch negativer Reaktionen nicht überschritten werden. Bei Testung mit einem negativen Kontrollserum muß sich eine Vitalität der Lymphozyten von mindestens 90% ergeben. Bei Testung mit einem positiven Kontrollserum müssen mindestens 90% der Zellen je Kavität lysiert sein. Lymphoscreen ABC 60 contient un panel de lymphocytes congelés dont les groupes HLA-ABC ont été typés (6). Introduction On peut observer l’apparition d’anticorps HLA cytotoxiques après une transfusion sanguine, une greffe d’organe et pendant la grossesse (2,3,4). L’utilisation de Lymphoscreen ABC 60 permet un dépistage rapide dans les sérums, prélevés chez des femmes enceintes et chez des patients transfusés ou transplantés. La détermination des anticorps cytotoxiques par un panel de lymphocytes dont les groupes HLA-ABC ont été typés est une méthode qui peut être mise en oeuvre dans chaque laboratoire HLA. Principe du test de microlymphocytotoxicité On incube des lymphocytes, portant des antigènes HLA connus, avec un sérum de spécificité HLA non connu et du complément de lapin. Si l’antigène correspondant est présent, l’ajout du sérum en présence de complément provoque une lyse des lymphocytes. Cette lyse est révélée par un colorant (par exemple éosine Y). Les lymphocytes lysés et intacts sont évalués à l'aide d'un microscope à contraste de phase inverse. Processus de lavage 1. Processus de lavage à l’aide de papier absorbant - Laisser brièvement décongeler Lymphoscreen ABC 60 à température ambiante (18…22°C ). Mise en garde : Des cristaux de glace devront rester visibles dans les puits ! Ne pas décongeler plus de deux plaques [MZP] à la fois. - Distribuer sans délai 20 µl de milieu de lavage dans chaque puits à l’aide du distributeur de Terasaki . - Laisser la plaque [MZP] pendant 15 minutes à température ambiante (18…22°C). - Aspirer le milieu de lavage à partir d’un coin de la plaque [MZP] à l’aide de papier absorbant. Enlever à l’aide d’un deuxième papier absorbant le milieu de lavage restant rangée par rangée. Mise en garde : Si le milieu de lavage n’est pas complètement enlevé, on peut observer des réactions atténuées par dilution des sérums à tester. - Répartir de l’huile dans chaque puits (environ 5 µl par puits). autre possibilité en ce Description du produit Lymphoscreen ABC 60 contient des lymphocytes congelés de différents donneurs ayant des antigènes HLA définis congelés sur microplaque [MZP]. La membrane des lymphocytes congelés est stabilisée à l'aide de DMSO (diméthylsulfoxyde). Pour la réalisation du test de microlymphocytotoxicité, on décongèle Lymphoscreen ABC 60 à température ambiante. On élimine le DMSO à l'aide d'un milieu de lavage, car ce produit est fortement toxique pour les cellules à température ambiante. Un conditionnement Lymphoscreen ABC 60 contient 6 plaques [MZP]. Chaque plaque [MZP] contient un panel de 56 lymphocytes congelés dont le groupe HLA a été determiné pour le dépistage du sérum d’un sujet et de deux pools lymphocytaires (chaque fois deux puits), prévus pour les témoins positifs et négatifs en double. La position des témoins est indiquée sur chaque plaque par un code couleur. Chaque puits contient 2 µl de suspension de lymphocyte dans 13 µl de milieu de congélation. Le nombre de lymphocytes est de l’ordre de 7000 lymphocytes par puits. Préparation des réactifs Préparer le milieu de lavage : SVF à 10 % (V/V) dans Lymphostabil Reconstituer le témoin HLA (positif) dans le volume d'eau distillée indiqué sur l'étiquette = témoin positif. Le témoin positif est un sérum antilymphocytaire de cheval qui réagit sur le plan cytotoxique avec tous les lymphocytes d'origine humaine. Reconstituer le témoin HLA (négatif) dans le volume d'eau distillée indiqué sur l'étiquette = témoin négatif. Tous les matériaux d'origine humaine, utilisés pour ce produit, ont été contrôlés à l'aide de trousses, brevetées par la FDA, pour l'AgHBs, les anticorps anti-HCV et anti-HIV-1/-2 et ont donné des réactions négatives. Tous ces produits d'origine humaine devront toutefois être considérés comme transmetteurs potentiels des virus de l'hépatite, du HIV et d'autres agents pathogènes infectieux. Des mesures de précaution appropriées sont recommandées. Stockage Lymphoscreen ABC 60 est utilisable jusqu’à la date de péremption indiquée sur les étiquettes. Lymphoscreen ABC 60 doit être stocké à une température inférieure ou égale à 70°C. Les emballages livrés sur carboglace doivent être congelés immédiatement à -70°C. Les emballages de Lymphoscreen ABC 60 entamés ne doivent pas être stockés dans de la carboglace. Problèmes qualités Si le témoin négatif présente plus de 10 % de lymphocytes lysés et si la toxicité de fond est supérieure à 10 %, le test devra être répété. Si le témoin positif présente moins de 80 % de lymphocytes lysés, le test devra être répété. Préparation des prélèvements Prélèvement du sérum à partir de: - environ 3 ml de sang complet ou bien - environ 3 ml de sang hépariné (transformer le plasma en sérum paraddition de thrombine) ou bien - environ 3 ml de sang sur EDTA ou ACD (transformer le plasma en sérum par addition de chlorure de calcium) Utiliser un sérum fraîchement prélevé ou après stockage congelé à une température inférieure ou égale à -20°C. Lymphoscreen ABC 60 est maintenant prêt à l'emploi pour le test de microlymphocytotoxicité. Test de microlymphocytotoxicité - Distribuer 2 µl du témoin négatif dans les puits 1A et 1B - Distribuer 2 µl du témoin positif dans les puits 10A et 10B. Les positions des témoins sont marquées sur la plaque [MZP]. - Distribuer 2 µl du sérum prélevé dans les puits restants (1C à 10C). - Incuber pendant 30 minutes à température ambiante (18...22°C). - Reconstituer le complément [COMP] 15 minutes avant la fin de la durée d'incubation à l'aide du volume d'eau distillée, indiqué sur l'étiquette, en agitant légèrement. NE PAS RECONGELER! JETER LE COMPLEMENT RESTANT! - Ajouter 5 à 6 µl de complément [COMP] dans chaque puits. - Incuber pendant 60 minutes à température ambiante (18...22°C). - Colorer les cellules pendant environ 3 minutes à l'aide de 3 à 4 µl d'éosine Y 5% par puits. * pouvant être livrés par Biotest y nl Fo r R ef er en ce Sources d'erreur Si le témoin négatif présente plus de 10 % de lymphocytes lysés et si la toxicité de fond est supérieure à 10 %, le test devra être répété. Les causes peuvent en être les suivantes: 1. Lymphoscreen ABC 60 a été exposé trop longtemps à la température ambiante lors de la livraison. 2. Lymphoscreen ABC 60 a été stocké à une température supérieure à -70°C 3. Décongélation complète de Lymphoscreen ABC 60 avant l’ajout du milieu de lavage. Si le témoin positif présente moins de 80 % de lymphocytes lysés, le test devra être répété. Les causes peuvent en être les suivantes: 1. Réactivité insuffisante du complément 2. Dilution de l'anticorps à déceler du fait d'une élimination non satisfaisante du milieu de lavage. 3. Emballages entamés ou stockage des plaques [MZP] dans de la carboglace; Les antigènes HLA qui devront faire partie d’un panel visant à différencier les anticorps HLA-ABC ne sont cités ni dans les directives de l’ASHI (American Society for Histocompatibility and Immunogenetics) ni dans celles de l’EFI (European Federation of Immunogenetics). C’est pourquoi on choisit les donneurs de façon à comprendre si possible tous les antigènes HLA-ABC connus. Les fréquences des phénotypes varient entre les différentes populations. Les antigènes représentés dans Lymphoscreen ABC peuvent ainsi être différents de la fréquence des antigènes de la population à partir de laquelle provient le prélèvement à étudier. Lymphoscreen ABC 60 utilise principalement des lymphocytes prélevés chez des sujets de race blanche. O Interprétation des réactions Les plaques sont lues au microscope à contraste de phase inverse. Les lymphocytes vivants ont une couleur claire et sont brillants (= réaction négative), les lymphocytes lysés sont foncés et de plus grande dimension (= réaction positive). La part des lymphocytes lysés sur le nombre total de lymphocytes est % de cellules lysées Interprétation 0 -10% = Score 1 négatif 11 -20% = Score 2 négativité discutable 21 -50% = Score 4 faiblement positif 51 -80% = Score 6 positif 81 -100% = Score 8 fortement positif = Score 0 non interprétable Les réactions lues devront être inscrites dans la colonne correspondante du formulaire de travail. Les antigènes des cellules présentant une réaction positive peuvent être marqués sur le tableau des antigènes à l'aide d'un stylo de couleur. La spécificité anti-HLA de l'anticorps est définie par la sélection des antigènes présentant un marquage complet. S'il n'est pas possible d'indiquer une spécificité, la somme des réactions positives est indiquée sous forme de pourcentage par rapport à la totalité du panel (% PRA = pourcentage des anticorps réactionnels vis à vis du panel). Si la plaque est conservée à des températures de 2 à 8°C, l’évaluation est encore possible dans un délai de 3 jours. La lecture se fait en conformité avec les formulaires joints selon la séquence suivante de 1A à 1F, de 2F à 2A, de 3A à 3F, etc. Caractéristiques spécifiques Pour le dépistage et le diagnostic différentiel des anticorps HLA-ABC cytotoxiques, dépendants du complément, Lymphoscreen ABC 60 contient des lymphocytes provenant de donneurs testés à l’aide des antisérums HLA correspondants. Les différents antigènes de Lymphoscreen ABC 60 sont énumérés dans le formulaire de travail joint en annexe. Chaque lot est contrôlé à l'aide d'au moins 10 sérums-tests anti-HLA-ABC sélectionnés. Un pourcentage de 10 % de réaction à tort positives et de 5 % de réactions à tort négatives ne devra pas être dépassé. Lors du contrôle à l'aide d'un sérum témoin négatif, la viabilité des lymphocytes doit être au moins de 90 %. Lors du contrôle à l'aide d'un sérum témoin positif, au moins 90 % des cellules par puits doivent être lysées. Ciascuna piastra [MZP] contiene un panel di linfociti congelati e tipizzati per HLAABC (6). Introduzione Dopo trasfusioni di sangue, trapianti di organi e durante una gravidanza possono formarsi anticorpi-HLA citotossici(2,3,4). Con l’aiuto di Lymphoscreen ABC 60 è possibile un rapido screening dei sieri di donne in gravidanza e di pazienti trasfusi o trapiantati. La determinazione degli anticorpi HLA-ABC citotossici contro un panel di linfociti HLA tipizzati è una metodica che può essere eseguita in ogni laboratorio con esperienza nel settore del sistema HLA. Principio del test di microlinfocitotossicità I linfociti con antigeni-HLA noti sono incubati con un siero di specificità HLA sconosciuta e complemento di coniglio. In caso di presenza dell’antigene corrispondente, l’aggiunta di siero in presenza di complemento porta alla lisi dei linfociti. Questa viene evidenziata mediante l’uso di una sostanza colorante (es. eosina Y) che penetra all’interno dei linfociti. La valutazione dei linfociti lisati e di quelli vitali viene effettuata con un microscopio a contrasto di fase invertito. Procedimento di lavaggio 1. Procedimento di lavaggio con impiego di carta assorbente. - Portare le piastre di Lymphoscreen ABC 60 a temperatura ambiente (18...22°C). Attenzione: Si deve ancora vedere ghiaccio nei pozzetti! Non scongelare più di 2 piastre [MZP] per volta! - Pipettare in ogni pozzetto, con la microsiringa Terasaki, 20 µl della soluzione di lavaggio. - Lasciare le piastre [MZP] per 15 minuti a temperatura ambiente (18...22°C). - Assorbire da un angolo della piastra [MZP], con la carta assorbente, la soluzione di lavaggio. Tamponare, fila per fila, la rimanente soluzione di lavaggio. Attenzione: se la soluzione di lavaggio non viene rimossa completamente, è possibile un indebolimento delle reazioni a causa della diluizione del siero in esame. - Ricoprire le piastre di Lymphoscreen ABC 60 con olio (ca. 5µl per ogni pozzetto). oppure, in alternativa ce Descrizione del prodotto Nel Lymphoscreen ABC 60, i linfociti tipizzati del sangue periferico, proveniente da diversi donatori, vengono congelati in micropiastre [MZP]. La membrana dei linfociti congelati viene stabilizzata con DMSO (dimetilsulfossido), Tale composto è tossico per le cellule, e pertanto deve essere rimosso immediatamente, mediante opportuni lavaggi, non appena le micropiastre vengono portate a temperatura ambiente per lo scongelamento. Una confezione di Lymphoscreen ABC 60 contiene 6 piastre [MZP]. Ogni piastra [MZP] contiene un panel di 56 linfociti congelati, a tipizzazione HLA nota, necessari per lo screening di un campione e 2 pool di linfociti (ognuno in 2 pozzetti) previsti per i doppi controlli positivo e negativo. Le posizioni dei controlli sono segnate su ogni piastra. Ciascun pozzetto contiene 2 µl di sospensione di linfociti e 13 µl di medium di congelamento. Tutti i materiali di origine umana impiegati per questo prodotto sono stati testati con kit approvati dalla FDA per rilevare l'eventuale presenza di : HBsAg, anti-HCV ed anti-HIV-1/2 con esito negativo. Tuttavia, tutti i prodotti di origine umana devono essere considerati potenzialmente in grado di trasmettere epatiti, HIV o altre malattie infettive. Si raccomanda di attenersi alle procedure di sicurezza opportune. Conservazione Le Lymphoscreen ABC 60 sono stabill fino alla data dichiarata sull' etichetta. Il Lymphoscreen ABC 60 deve essere conservato ad almeno -70°C. Le confezioni consegnate in ghiaccio secco devono essere immediatamente trasferite in un congelatore a -80°C. Le confezioni di Lymphoscreen ABC 60, una volta aperte, non devono essere conservate insieme al ghiaccio secco. Segnali di deterioramento Se il controllo negativo indica più del 10% di linfociti lisati e la tossicità di base è >10%, è necessario ripetere il test. Se il controllo positivo indica meno dell'80% di linfociti lisati, è necessario ripetere il test. Preparazione dei campioni Il siero del paziente può essere ottenuto da: circa 3 ml di sangue intero, oppure circa 3 ml di sangue eparinato (trasformare il plasma in siero mediante aggiunta di trombina), oppure circa 3 ml di sangue trattato con EDTA o con ACD (trasformare il plasma in siero mediante aggiunta di cloruro di calcio). Il siero del paziente puo’ essere conservato ad almeno -20°C. Metodica Materiale disponibile Lymphoscreen ABC 60 (1 piastra-microtest [MZP] per 1 campione) Complemento [COMP] di coniglio liofilizzato (in ogni confezione sono acclusi 3 x 1 ml) Worksheets (in ogni confezione sono acclusi 6 fogli). 2. Procedimento di lavaggio con una centrifuga per piastre. - Portare le piastre di Lymphoscreen ABC 60 a temperatura ambiente (18...22°C). Attenzione: Si deve ancora vedere ghiaccio nei pozzetti! Non scongelare più di 2 piastre [MZP] per volta! - Pipettare in ogni pozzetto, con la microsiringa Terasaki, 20 µl della soluzione di lavaggio. - Centrifugare immediatamente le piastre [MZP] a 1000 x g per 30 secondi (senza freno). - Aspirare accuratamente la soluzione di lavaggio,con il pettine a 6 canali, mediante la pompa a getto d'acqua. Attenzione: aspirare attentamente la soluzione di lavaggio tenendosi sul bordo del pozzetto, onde evitare di aspirare il sedimento cellulare. - Ricoprire le piastre Lymphoscreen ABC 60 con l'olio (ca. 5µl per ogni pozzetto). Il Lymphoscreen ABC 60 è ora pronto per l'uso per il test di microlinfocitotossicità. Test di microlinfocitotossicità - Pipettare 2 µl di controllo negativo nei dei pozzetti 1 A e 1B. - Pipettare 2 µl di controllo positivo nei pozzetti 10A e 10B. Le posizioni dei controlli sono indicate sulla piastra [MZP]. - Pipettare 2 µl di siero del paziente in ognuno dei rimanenti pozzetti (da 1C fino a 10C). - Incubare per 30 minuti a temperatura ambiente (18...22°C). - 15 minuti prima del termine del tempo di incubazione, ricostituire il complemento [COMP] con il volume di acqua distillata indicato sull'etichetta, agitando leggermente. NON CONGELARE NUOVAMENTE! ELIMINARE IL COMPLEMENTO RESIDUO! - Aggiungere in ogni pozzetto 5-6 µl di complemento [COMP]. - Incubare per 60 minuti a temperatura ambiente (18...22°C). - Aggiungere in ogni pozzetto 3-4 µl di 5% eosina Y e lasciar colorare le cellule per ca. 3 minuti. - Bloccare la reazione con 6-7 µl di formaldeide per ogni pozzetto. - Coprire con un vetrino coprioggetto la piastra ultimata e leggere non prima di 30 minuti. La valutazione è ancora possibile per un massimo di 3 giorni, se le piastre vengono conservate a 2...8°C. La lettura avviene in modo conforme ai Worksheets di forma meandrica acclusi; dall’1 A all’1 F, dal 2 F al 2 A, dal 3 A al 3 F, ecc. * disponibile presso Biotest Controllo di qualità Il controllo negativo si trova nelle posizioni 1 A e 1 B e verifica la vitalità dei linfociti. La vitalità dovrebbe essere superiore al 90%. Il controllo positivo si trova nelle posizioni 10 A e 10 B e verifica l’attività del complemento. Il controllo positivo dovrebbe contenere più dell’ 80% di linfociti lisati. nl O se U R ef er en ce Possibilità di errori Se il controllo negativo indica più del 10% di linfociti lisati e la tossicità di base è >10%, è necessario ripetere il test. Possibili cause : 1. Il Lymphoscreen ABC 60 è stato lasciato troppo a lungo a temperatura ambiente all’apertura dell’imballaggio 2. Il Lymphoscreen ABC 60 è stato conservato ad una temperatura inferiore a -70°C. 3. Lymphoscreen ABC 60 era completamente scongelato, prima di essere aggiunto il tampone di lavaggio. Se il controllo positivo indica meno dell'80% di linfociti lisati, è necessario ripetere il test. Possibili cause : 1. Insufficiente attività del complemento. 2. Diluizione degli anticorpi eventualmente rilevabili per insufficiente rimozione della soluzione di lavaggio. 3. Le confezioni sono state conservate insieme al ghiaccio secco. Né nelle direttive dell’ASHI (American Society for Histocompatibility and Immunogenetics) né in quelle dell’EFI (European Federation of Immunogenetics) sono nominati gli antigeni-HLA che dovrebbero essere presenti in un panel per la differenziazione degli anticorpi-HLA-ABC. Perciò i donatori presenti vengono scelti in modo tale che tutti gli antigeni-HLA-ABC conosciuti siano presenti il più possibile. Le frequenze dei fenotipi variano da una popolazione all’altra. Per questo gli antigeni rappresentati in Lymphoscreen ABC possono essere diversi dalla frequenza antigenica della popolazione da cui deriva il campione esaminato. In Lymphoscreen ABC 60 sono contenute prevalentemente cellule di origine caucasica. y Valutazione delle reazioni La lettura viene eseguita con un microscopio a contrasto di fase invertito. I linfociti vitali sono chiari e luminosi (= reazione negativa), i linfociti lisati sono scuri e più grandi (= reazione positiva). Nell'esame di un siero di controllo positivo, almeno il 90% delle cellule di ogni pozzetto deve essere lisato. Se contiene un grupo ultracongelado de linfocitos tipificados según HLA (6). Introducciòn Los anticuerpos HLA citotóxicos se pueden originar después de transfusiones de sangre, trasplantes de órganos o durante el embarazo (2,3,4). Lymphoscreen ABC 60 hace posible una rápida exploración de los sueros de mujeres embarazadas o de pacientes a los que se les ha transfundido sangre o trasplantado un órgano. La determinación de los anticuerpos citotóxicos contra un grupo de linfocitos tipificados según HLA representa un método que puede aplicarse en cualquier laboratorio de HLA. Principio del la prueba de microlinfocitotoxicidad Los linfocitos con antígenos HLA conocidos son incubados con un suero de especificidad HLA desconocida y con un complemento procedente del conejo. Si el antígeno correspondiente está presente, la adición del suero en presencia del complemento conduce a la lisis de los linfocitos. Esta se visualiza con un colorante (p. ej., eosina Y). La valoración de los linfocitos lisados y vitales tiene lugar con un microscopio inverso de contraste de fases. Procedimiento de lavado 1. Lavado con celulosa absorbente - Se descongela brevemente Lymphoscreen ABC 60 a temperatura ambiente (18...22°C). Atención: ¡debe apreciarse hielo en los pocillos! ¡No se deben descongelar más de dos placas [MZP] a la vez! - Se pipetean de inmediato 20 µl del medio de lavado con el dispensador de Terasaki en cada pocillo. - Se deja en reposo la placa [MZP] durante 15 minutos a la temperatura ambiente (18...22°C). - Se aspira el medio de lavado de los bordes de la placa [MZP] con celulosa absorbente. Se retira el resto del medio con un segundo paño, pasándolo por cada fila. Atención: si no se elimina completamente el medio de lavado, puede reducirse la reacción, por dilución de los sueros analizados. - Se recubre Lymphoscreen ABC 60 con aceite (approx. 5µl a cada pocillo). o bien er en ce Descripción del producto Los linfocitos periféricos de los diferentes donantes con antígenos HLA definidos se encuentran ultracongelados en las placas [MZP] de microensayo de Lymphoscreen ABC 60. La membrana de los linfocitos congelados se estabiliza con DMSO (dimetilsulfóxido). Para realizar el ensayo de microtoxicidad linfocítica se descongela Lymphoscreen ABC 60 a la temperatura ambiente. DMSO se elimina con una solución de lavado, ya que posee un efecto tóxico celular muy intenso a la temperatura ambiente. Un envase de Lymphoscreen ABC 60 contiene 6 placas [MZP]. Cada placa [MZP] contiene un panel de 56 linfocitos ultracongelados y tipificados en HLA para la exploración de una muestra, así como 2 bandejas de linfocitos (de 2 cavidades cada una), previstas para controles dobles positivos y negativos. Las posiciones de los controles se hallan marcados con colores en cada placa. Cada cavidad contiene 2 μl de suspensión linfocítica en 13 μl de medio congelante. El número de células asciende a aproximadamente 7.000 linfocitos por cavidad. Preparación de los reactivos Preparación del medio de lavado: SBF al 10% (v/v) en Lymphostabil. Bomba de aire de chorro de agua U Biotest Lymphoscreen ABC 60 Fo r R ef Medidas de precaución [IVD] El reactivo sólo debe utilizarse para el diagnóstico in vitro. El ensayo debe ser realizado por técnicos de laboratorio bien preparados y autorizados. Todo material de origen humano utilizado para este producto ha sido sometido a análisis de HBsAg, anti-HCV y anti-VIH-1/2, con lotes aprobados por la FDA, obteniendo un resultado negativo. De todas maneras, todo producto de origen humano puede, en teoría, transmitir la hepatitis, VIH y otras enfermedades infecciosas. Por eso, se recomiendan las medidas generales de seguridad. Conservación El producto es estable hasta la fecha indicada en la etiqueta. Lymphoscreen ABC 60 se debe conservar, como mínimo, a -70°C. Los envases, suministrados en hielo seco, se trasladarán inmediatamente al congelador de 70°C. Los envases abiertos de Lymphoscreen ABC 60 no se pueden conservar con hielo seco. Indicaciones de deterioración Si el control negativo contiene más de 10% de linfocitos lisados y la toxicidad de fondo es >10%, se repetirá el ensayo. Si el control positivo contiene menos de 80% de linfocitos lisados, se repetirá el ensayo. Preparación de muestras Obtención del suero del probando a partir de: aproximadamente 3 ml de sangre nativa o aproximadamente 3 ml de sangre heparinizada (llevar el plasma al suero, añadiendo trombina) o aproximadamente 3 ml de sangre tratada con EDTA o ACD (pasar el plasma al suero, añadiendo cloruro cálcico) El suero del probando debe analizarse en fresco o conservarse a una temperatura mínima de -20°C. Método Material que se suministra Lymphoscreen ABC 60 (1 microplaca [MZP] de análisis para 1 muestra) Complemento [COMP] liofilizado del conejo (3x1 ml en cada envase) Hojas de trabajo (6 unidades en cada envase) 2. Lavado con una centrífuga de placas - Se descongela brevemente Lymphoscreen ABC 60 a temperatura ambiente (18...22°C). Atención: ¡debe apreciarse hielo en los pocillos! ¡No se deben descongelar más de dos placas [MZP] a la vez! - Se pipetean de inmediato 20 µl del medio de lavado con el dispensador de Terasaki en cada pocillo. - Se centrifuga inmediatamente las placas [MZP] a 1000 x g durante 30 segundos (sin frenos). - Se aspira cuidadosamente el medio de lavado con un cabezal aspirador de 6 tomas a través de una bomba de aire con chorro de agua. Atención: el cabezal de aspiración debe colocarse en los bordes de la concavidad, para que sólo se aspire el medio y no las células sedimentadas. - Se recubre Lymphoscreen ABC 60 con aceite (approx. 5µl a cada pocillo). Lymphoscreen ABC 60 está ya preparado para el ensayo de microcitotoxicidad linfocitaria. Prueba de microlinfocitotoxicidad - Se pipetean 2 µl de los controles negativos cada vez en los pocillos 1A y 1B. - Se pipetean 2 µl de los controles positivos cada vez en los pocillos 10A y 10B. Se marcan sobre la placa [MZP] los controles positivos. - Se pipetean 2 µl del suero del probando en los demás pocillos (1C a 10C). - Se incuba durante 30 minutos a temperatura ambiente (18...22°C). - El complemento [COMP] se reconstituye 15 minutos antes de que termine el periodo de incubación con el volumen de agua destilada señalada en la etiqueta, balanceando ligeramente el recipiente. ¡NO VOLVER A CONGELAR! ¡DESECHAR LOS RESTOS DE COMPLEMENTO! - Se añaden 5-6 µl del complemento [COMP] a cada pocillo. - Se incuba durante 60 minutos a temperatura ambiente (18...22°C). - Se tiñen las células de cada cavidad con 3-4 µl de 5% eosina Y durante aprox. 3 minutos. - Se fijan los reactivos de cada cavidad con 6-7 µl de formaldehído. - Se tapa la placa preparada con un cubreobjetos y la lectura se realiza, como pronto, 30 minutos después de que concluya el ensayo. Si se conserva la placa a 2...8°C, también se valora incluso durante tres días. La lectura se realiza conforme a las hojas de trabajo que se adjuntan, en forma de meandro: de 1A a 1F, de 2F a 2A, de 3A a 3F, etc. * Proveedor: Biotest y nl O Fo r R ef er en ce Posibilidades de error Si el control negativo contiene más de 10% de linfocitos lisados y la toxicidad de fondo es >10%, se repetirá el ensayo. Las posibles causas comprenden: 1. Lymphoscreen ABC 60 se mantuvo expuesto durante mucho tiempo a temperatura ambiente después de desempaquetarlo. 2. Lymphoscreen ABC 60 se conservó a una temperatura menos fría que 70ºC. 3. Lymphoscreen ABC 60 estaba totalmente descongelado antes de añadir el medio de lavado. Si el control positivo contiene menos de 80% de linfocitos lisados, se repetirá el ensayo. Entre las posibles causas se encuentran: 1. Actividad insuficiente del complemento. 2. Dilución del anticuerpo detector por eliminación inadecuada del medio de lavado. 3. Los envases abiertos o algunas placas [MZP] se conservaron conjuntamente con el hielo seco. Ni en las directivas de la ASHI (American Society for Histocompatibility and Immunogenetics) ni en las de la EFI (European Federation of Immunogenetics) se nombran los antígenos HLA que deberían estar incluidos en un panel de identificación de anticuerpos HLA-ABC. Por eso, los donantes se escogen de tal modo que, en lo posible, estén presentes todos los antígenos HLA-ABC conocidos. Las frecuencias de fenotipos varían en función de las diferentes poblaciones. Por eso, los antígenos representados en Lymphoscreen ABC 60 pueden presentar diferencias con respecto a la frecuencia de antígenos de la población de la que procede la muestra sometida a prueba. En Lymphoscreen ABC 60 se han utilizado eminentemente células de origen caucasoide. se Valoración de las reacciones La lectura se realiza con un microscopio invertido de contraste de fases. Los linfocitos vitales aparecen claros y brillantes (= reacción negativa) y los lisados, oscuros y de mayor tamaño (= reacción positiva). El porcentaje de linfocitos lisados, con respecto a la población total de linfocitos, se indica como valor índice. % de celulas lisadas Valoración 0 - 10% = Índice 1 negativo 11 - 20% = Índice 2 dudosamente negativo 21 - 50% = Índice 4 débilmente positivo 51 - 80% = Índice 6 positivo 81 - 100% = Índice 8 muy positivo Índice 0 no valorable Las reacciones valoradas se anotarán en la columna correspondiente de la hoja de trabajo. Los antígenos de las células con reacción positiva se pueden marcar con un rotulador en la tabla antigénica. La especificidad HLA del anticuerpo se puede definir por la selección de los antígenos que están completamente marcados. Si no se puede valorar la especificidad, se indicará la suma de las reacciones positivas como porcentaje de todo el grupo (% PRA = % Panel Reactive Antibodies - % de anticuerpos reactivos del grupo). U Control de calidad Las posiciones del control negativo son 1A y 1B, y permiten comprobar la vitalidad de los linfocitos. La vitalidad debería ser superior al 90%. Las posiciones del control positivo son 10A y 10B, y permiten comprobar la actividad del complemento. El control positivo debería contener más de un 80% de linfocitos lisados. Características específicas Para la exploración e identificación de anticuerpos HLA-ABC citotóxicos y dependientes del complemento, en Lymphoscreen ABC 60 se han utilizado linfocitos de donantes que previamente se han sometido a pruebas con los correspondientes antisueros HLA. La hoja de trabajo adjunta incluye una lista de los antígenos incluidos. Cada lote se debe examinar con un número mínimo de 10 sueros problema antiHLA-ABC escogidos. El porcentaje de resultados falsamente positivos no debe exceder del 10% y el de falsamente negativos no debe hacerlo del 5%. Si se examina un suero control negativo, la vitalidad de los linfocitos a de resultar, como mínimo, del 90%. Si se examina un solo control positivo, deberán aparecer como mínimo, un 90% de células lisadas en cada pocillo.
