The test system is to be used as an aid in the detection of antibodies associated with autoimmune vasculitis. Antineutrophil cytoplasmic autoantibodies (ANCA) are a group of antibodies that react with cytoplasmic antigens in human neutrophils. Although these antibodies were originally reported in 1964 (1), the first report linking these antibodies to disease was in 1982, when Davies et al reported the antibodies in eight patients with segmental necrotizing glomerulonephritis (2). In 1984, four more patients with vasculitis and glomerulonephritis were reported. In 1985, van der Woude et al showed that ANCA had a high association with Wegener’s granulomatosis, and that antibody titer correlated with disease activity (3). In 1988, Falk and Jennette reported that ANCA have more than one antigen specificity (4). A subsequent report showed that the specificity of ANCA correlated with the pathologic features of vasculitides (5). In the immunofluorescent test for ANCA, several patterns of cellular staining may be seen. Two major patterns of staining have been described and well characterized when ethanol-fixed neutrophils are used in the immunofluorescent ANCA test. Autoantibodies that show a fine granular cytoplasmic pattern, called cANCA, are usually directed against a serine protease, Proteinase 3 (PR-3). These autoantibodies have been shown to have a high association with Wegener’s granulomatosis. The other major pattern of staining, the perinuclear, or pANCA pattern, which is usually due to antibodies directed against myeloperoxidase (MPO), has been associated with systemic vasculitis and idiopathic necrotizing and crescentic glomerulonephritis (4). The pANCA pattern is an artifact induced by the use of ethanol as a fixative (4). If the neutrophils are fixed in formalin, myeloperoxidase (the major antigen responsible for the pANCA pattern in ethanol fixed cells) remains associated with the primary (alpha) granules, and shows a granular cytoplasmic distribution. Proteinase-3 (PR-3) remains associated with the primary (alpha) granules in either ethanol or formalin fixatives. PRINCIPLE OF THE TEST The Immuno Concepts antineutrophil cytoplasmic autoantibodies (ANCA) test uses the indirect fluorescent antibody (IFA) technique first described by Weller and Coons (6). Diluted patient samples are incubated with human neutrophils, which are fixed on glass microscope slides, to allow specific binding of ANCA. If ANCA are present, the autoantibodies bind to neutrophilic antigens. After washing to remove non-specific antibodies, the substrate is incubated with anti-human IgG conjugated to fluorescein. When results are positive there is the formation of a stable three-part complex consisting of fluorescent anti-human antibody bound to human ANCA which are bound to antigen located in the cells. This complex can be visualized with the aid of a fluorescent microscope. Samples that are positive for cANCA will show a distinctive granular cytoplasmic staining of the neutrophils on both ethanol and formalin fixed slides. In samples that are positive for pANCA, a diffuse or peripheral nuclear staining of the neutrophils will be seen on ethanol fixed cells, and a granular staining of the cytoplasm will be seen on formalin fixed cells. If the sample is negative for ANCA, specific staining of the neutrophils will not be seen. SYSTEM COMPONENTS (MATERIALS PROVIDEd) ANCA Substrate Slides:* Catalog No. 10010-11 (Ethanol fixed) or 10010-12 (Formalin fixed). Ten well slides containing human neutrophils stabilized and fixed directly on the test wells. Unique moat slide design minimizes cross contamination of wells during testing. Slides include four control wells designated pANCA, cANCA, negative, and blank, to aid in proper interpretation of results. ANCA Sample Diluent: Catalog number 10100 (100 ml) Proprietary buffered sample diluent used to dilute patient samples. cANCA Positive Control: Catalog number 10021-12. Ready to use dropper vial containing 1.0 ml of cANCA positive human control serum. This serum demonstrates granular staining of the cytoplasm between the nuclear segments of the neutrophils on both ethanol and formalin fixed slides. This reagent contains 0.1% sodium azide as a preservative. pANCA Positive Control: Catalog number 10021-11. Ready to use dropper vial containing 1.0 ml of pANCA positive human control serum. This serum demonstrates diffuse or peripheral nuclear staining of the neutrophils on ethanol fixed slides and granular cytoplasmic fluorescence on formalin fixed slides. This reagent contains 0.1% sodium azide as a preservative. cANCA Titratable Control: Catalog number 10026-12. Ready to use vial containing 0.25 ml of liquid stable cANCA positive human control serum. This reagent contains 0.1% sodium azide as a preservative. This control should be treated as an undiluted patient sample. See label for mean titer information. pANCA Titratable Control: Catalog number 10026-11. Ready to use vial containing 0.25 ml of liquid stable pANCA positive human control serum. This reagent contains 0.1% sodium azide as a preservative. This control should be treated as an undiluted patient sample. See label for mean titer information. Negative Control: Catalog number 10031. Ready to use dropper vial containing 1.0 ml of ANCA negative human control serum. This serum demonstrates low intensity, nonspecific dull green fluorescence of the neutrophils. This reagent contains 0.1% sodium azide as a preservative. Fluorescent Antibody Reagent-IgG Specific: Catalog number 10009 (9ml). Goat anti-human IgG conjugated to fluorescein isothiocyanate (FITC). This reagent contains Evans Blue as a counterstain. Reagent comes ready to use in precision dropper bottles. This reagent contains 0.1% sodium azide as a preservative. *This slide is protected by one or more of the following U.S. and foreign patents: 4387972, D-274261, D-273261, Canadian patent 1171302, and other patents pending. NON-REACTIVE REAGENTS PBS Buffer: Catalog number 1011. Phosphate-buffered saline powder (0.01 M, pH 7.4 ± 0.2). Each package contains sufficient buffer powder to make 1 liter. (One package of buffer powder is supplied for each five slides in complete test kits). Preparation: Dissolve one package of buffer powder in 1 liter of deionized or distilled water and store refrigerated between 2-10° C for up to 4 weeks or until signs of contamination or other visible changes occur. Semi-Permanent Mounting Medium: Catalog number 1111. Ready to use dropper vial containing 5 ml of glycerol-based mounting medium, pH 9.1 ± 0.2. This reagent contains 0.1% sodium azide as a preservative. Coverslips: Catalog number 1041. Each packet contains 10 24 x 60 mm No. 1 glass coverslips. Use: All components come ready to use with no aliquoting or reconstitution required (except the PBS buffer which must be dissolved in deionized or distilled water before use). ADDITIONAL MATERIALS REQUIRED (BUT NOT PROVIDED) Storage: All components can be stored under refrigeration at 2-10°C. After reconstitution, PBS buffer should be stored in screw-cap containers under refrigeration at 2-10°C for up to four weeks or until signs of contamination or other visible changes occur. Stability: All components remain stable at least 12 months from date of manufacture. Do not use any component beyond its expiration date. The pANCA positive control should show bright apple-green diffuse or peripheral nuclear staining of the neutrophils on ethanol fixed slides, and granular cytoplasmic fluorescence on formalin fixed slides. The negative control should not exhibit any bright green fluorescence. Low intensity, nonspecific dull green fluorescence may be observed in the negative control. The blank control is used to observe nonspecific fluorescence of the antibody reagent, and should not exhibit any green fluorescent staining. The counterstain provided in the conjugate may stain the cells a dull red color. If the controls do not appear as described, the test is invalid and should be repeated. PRECAUTIONS 1. All human source material used in the preparation of controls for this product has been tested and found to be negative (not repeatedly reactive) for antibody to human immunodeficiency virus-1 and human immunodeficiency virus-2 (HIV-1 & HIV-2), antibody to hepatitis C virus (HCV), and for hepatitis B surface antigen (HBsAg) by an FDA approved method. No test method can offer complete assurance that HIV-1, HIV-2, hepatitis C virus, hepatitis B virus, or other infectious agents are absent. Thus, all control sera should be handled in the same manner as potentially infectious materials. 2. All patient samples should be handled at the Biosafety Level 2 as recommended for any potentially infectious human serum or blood specimen in the Centers for Disease Control/National Institutes of Health Manual: Biosafety in Microbiological and Biomedical Laboratories, 1984 Edition. 3. Dilution of the components or substitution of components other than those provided in this system may yield inconsistent results. 4. Sodium azide (0.1%) is used as a preservative. Sodium azide may react with lead or copper plumbing and form explosive metal azide salts. When disposing of reagents, flush with ample volumes of tap water to prevent potential residues in plumbing. Sodium azide is a poison and may be toxic if ingested. 5. This kit is for in vitro diagnostic use. 6. The titratable control serum is intended for use in monitoring lot to lot and run to run reproducibility. It is not intended as a measurement of overall sensitivity or specificity of the assay. 7. Do not smoke, eat, or drink in areas where specimens or kit reagents are handled. 8. Avoid splashing or generation of aerosols at all times. 9. Incubation times and temperatures other than those specified may give erroneous results. 10. Cross contamination of reagents or samples may give false results. 11. Reusable glassware must be washed and thoroughly rinsed free of detergents prior to use. All glassware must be clean and dry before use. 12. Bring all reagents, slides, and specimens to room tempera-ture (18-24°C) prior to use. 13. Wear disposable latex gloves when handling specimens and reagents, and wash hands thoroughly afterwards. 14. Microbial contamination of reagents or samples may give false results. 15. Never pipette by mouth and avoid contact of reagents and specimens with skin and mucous membranes. If contact occurs, wash with a germicidal soap and copious amounts of water. OPTIONAL TITRATABLE CONTROL When reading titers, many laboratories begin reading with the well that contains the most dilute sample and read “backwards” to the 1:20 dilution. The first well in which clearly discernible cytoplasmic staining is visible is the titer end-point. We recommend this technique for determining titer end-points. The mean titer and titer range (± one dilution on either side of the mean) determined for each lot number were established in our laboratory and are stated as a guide. This control is provided to allow each laboratory to assess the reproducibility (precision) of its ANCA testing. Since this control is not intended to be an indicator of titer accuracy, each laboratory should establish its own mean titer end-point for this sample, and should use this information to assess run-to-run reproducibility (precision). Through multiple testing of this titratable control, using the Immuno Concepts Fluorescent ANCA Test Systems, a mean titer value has been established for each lot number. The lot number, mean titer and titer range (± one twofold dilution on either side of the mean) are stated on the vial label and should be used as a guide for the test system performance. The values obtained in our laboratory may differ from your values. Some of the many factors that can affect your results may include, but are not limited to: 1. Type of light source: Mercury light sources will produce greater excitation energy at 495 nm than Quartz/Halogen. The 50-watt, 100-watt, and 200watt Mercury light sources differ little in excitiation energy at 495 nm. The 100-watt Quartz/Halogen light sources will produce greater excitation energy at 495 nm than 50-watt Quartz/Halogen. 2. Condition and age of the light source: This is particularly true for Mercury light sources which generally exhibit a gradual reduction in excitation energy at 495 nm prior to burning out. This gradual reduction in excitation energy can result in a significant loss in sensitivity over several weeks. This problem can be avoided by keeping a time log. For best results, replace 50-watt mercury bulbs at 100 hours, and 100 or 200-watt mercury bulbs at 200 hours. Quartz/Halogen light sources generally do not exhibit a gradual reduction in excitation energy prior to burning out. 3. Type of exciter filter used: Interference exciter filters provide greater sensitivity over a much narrower wavelength than absorption exciter filters. Refer to your fluorescent micro-scope manual or sales representative for more information. 4. Proper alignment of the microscope light path: Refer to your fluorescent microscope manual for instructions. 5. Numerical aperture of the objective: With incident light fluorescence (Epi), fluorescence is increased exponentially as the numerical aperture (NA) of the objective is increased additively. This may cause a 40X objective with a NA of 0.65 to read one or more dilutions lower than a 40X objective with a NA of 0.85. The numerical aperture is printed on the side of the objective. Sensitivity of transmitted light fluorescent microscopy is not affected by NA. 6. Supression filters: Supression filters reduce specific wavelengths of excitation and may be used in reducing sensitivity. Refer to your fluorescent microscope manual or sales representative for more information. 7. Precision and accuracy of dilution technique, equipment, and performance of the test procedures. SPECIMEN COLLECTION Collection: Serum is the preferred specimen. Approximately 5 ml of whole blood should be collected aseptically by venipuncture using a sterile vacuum collection tube or other suitable collection system. Allow blood to clot at room temperature (18-24°C). Serum should be separated from the clot as soon as possible to minimize hemolysis. Interfering Substances: Sera exhibiting a high degree of hemolysis, icterus, lipemia, or microbial growth should not be used because a decrease in antibody titer may occur on positive samples. Specimens with very high levels of lipid may cause a non-specific fluorescent film to form over the cell substrate. Use of fasting specimens or clearing specimens by ultracentrifugation may eliminate this problem. Specimens containing visible particulate matter should be clarified by centrifugation before testing. Storage: Sera may be stored at 2-10°C up to one week. If testing is further delayed, sera should be stored frozen at -20°C or lower. Serum should not be stored in a selfdefrosting freezer. CAUTION: Repeated freeze/thawing of patient samples may yield false positive or false negative results. INTERPRETATION OF RESULTS QUALITY CONTROL Positive, negative and blank controls should be run in the wells provided for quality control on each slide. The INTERPRETATION OF PATIENT RESULTS 3 400X total magnification is recommended for viewing the neutrophils. Among the normal samples there were 22 samples that were positive for ANA, and were considered uninterpretable for ANCA. The remaining discrepant samples were tested for antibodies to MPO and PR3 using ELISA tests to determine the true antibody status. Negative: A serum is considered negative for ANCA if, at a dilution of 1:20, green fluorescent staining of the cells is less than or equal to the negative control well. Non-specific background staining due to heterophile antibodies or autoantibodies may be observed in the neutrophils. SERA PREVIOUSLY DETERMINED TO BE POSITIVE FOR ANCA BY INDIRECT IMMUNOFLUORESCENCE Serum samples which had been judged to be positive for ANCA using in-house IFA assays were obtained from reference laboratories in the USA, the United Kingdom, and Australia. A total of 383 sera, which we expected to be positive for ANCA, were examined in this part of the study. These samples were tested in parallel using the Immuno Concepts ANCA kit and another kit in commercial distribution. Discrepant samples were tested for ANA using Immuno Concepts HEp-2 ANA Test System, and for antibodies to MPO and PR3 using ELISA tests. Positive: A serum is considered positive for ANCA if, at a dilution of 1:20, the cells in each field demonstrate granular cytoplasmic fluorescence similiar to that seen with the cANCA control on either ethanol or formalin fixed slides. Alternatively, a serum is considered positive for ANCA if, at a dilution of 1:20, the cells demonstrate diffuse or peripheral nuclear fluorescence similar to that seen with the pANCA control on ethanol fixed slides. REPORTING OF RESULTS Screening: Results should be reported as positive or negative at the 1:20 dilution. Combining the results of the normal population and the abnormal population, we obtained the following data in the initial comparison for the Immuno Concepts ANCA Test System with Ethanol Fixed Human Neutrophils (Table 1): Staining Patterns: Many autoantibodies can cause staining of the cytoplasm and/or nucleus of the neutrophils. There are two major patterns of specific staining: Comparison Method cANCA (classical or cytoplasmic staining): Staining of the alpha (primary) granules in the cytoplasm shows a consistent speckled cytoplasmic staining pattern, often with a concentration of staining between the lobes of the nucleus. This sample was judged to be a non-specific reaction. All other sera in this population were negative on both the ethanol fixed and formalin fixed neutrophils. Since many autoantibodies can react non-specifically with human neutrophils (8), positive immunofluorescent tests should always be confirmed using specific assays for the ANCA-associated antibodies. These data yield the following comparative statistics: Relative sensitivity : 98.3% Relative specificity: 97.7% Overall agreement: 98.6% SERUM SAMPLES FROM PATIENTS WITH KNOWN VASCULITIDES Samples obtained from 102 patients with clinically characterized vasculitides were tested using the Immuno Concepts ethanol fixed neutrophils and the Immuno Concepts formalin fixed neu-trophils. The results of this comparison are shown in Table 5: REPRODUCIBILITY Intra-run, inter-day and inter-lot studies were performed to demonstrate the reproducibility of the Immuno Concepts ANCA Test System with Ethanol Fixed Human Neutrophils and the Immuno Concepts ANCA Test System with Formalin Fixed Human Neutrophils. The sera used in this study included three pANCA positive samples (one that exhibited strong staining, one that exhibited moderate staining and one that exhibited weak staining), and three cANCA positve samples (one each that showed strong, moderate or weak staining). Six samples which were negative for ANCA were also used. In the intra-run study these 12 sera were tested in replicates of six wells each. For inter-day and inter-lot reproducibility, these 12 sera were each run on three lot numbers of kits on three separate occasions. The six negative samples were negative on all slides tested, and the positive samples were positive, with consistent fluorescent intensities, on all slides tested. CROSS REACTIVITY Sera from 57 patients with various autoimmune disorders were tested on both the Immuno Concepts ANCA Test System with Ethanol Fixed Human Neutrophils and the Immuno Concepts ANCA Test System with Formalin Fixed Human Neutrophils. Three of these samples showed an atypical type of nuclear staining, which resembled the pANCA pattern, on the ethanol fixed neutrophils. All three of these samples were from patients with SLE, were positive for anti-DNA antibodies and showed a positive ANA with a homogeneous pattern. RECONSTITUTE BUFFER (PBS) Dissolve contents of one buffer package in 1 liter of deionized or distilled water. Cover and store at 2-10° C for up to four weeks or until signs of contamination or other visible changes occur. 2. DILUTE PATIENT SAMPLES Screening: Dilute patient samples to 1:20 by adding 0.05 ml (50 µl) of serum to 0.95 ml (950 µl) of Sample Diluent. Semi-Quantitative Titering: To make twofold serial dilutions of screening samples (e.g. 1:40, 1:80, 1:160...1:640), remove 0.5 ml of the 1:20 dilution and mix with 0.5 ml of sample diluent to achieve a 1:40 dilution, and continue serial dilutions in this fashion. 3. 4. 5. DILUTE OPTIONAL TITRATABLE CONTROL Treat the optional titratable control as an undiluted patient sample. Dilute the control 1:20 by adding 0.05 ml (50 µl) of the control serum to 0.95 ml of Sample Diluent. Make two-fold serial dilutions of the titratable control as outlined above. PREPARE SUBSTRATE SLIDES (20-25 µl/well) Remove slide(s) from pouch(es) and place control sera on control wells as follows: Invert control dropper bottle and squeeze gently until drop is visible at the tip. Gently touch the drop to appropriate control well while avoiding direct contact of dropper tip with slide surface. Place pANCA positive control on well marked “pANCA”, cANCA positive control on well marked “cANCA”, negative control on well marked “Neg”, and one drop of Sample Diluent on well marked “Blank”. Add 1 drop (20-25 µl) of diluted patient sample to numbered wells. CAUTION: DIRECT CONTACT OF DROPPER TIP WITH SLIDE SURFACE MAY RESULT IN DAMAGE TO THE ANTIGEN SUBSTRATE. INCUBATE SLIDES (30 ± 5 minutes at room temperature, i.e. 18-24°C) Place slide(s) into a moist covered chamber (a petri dish with moistened paper toweling will be adequate). Incubate, with lid in place, for 30 minutes (± 5 minutes) at room temperature (18-24°C). 6. PBS RINSE Remove slide(s) from incubator tray and rinse briefly with PBS using a squirt bottle, Pasteur, or serological pipette. Do not squirt buffer directly on the wells. NOTE: To avoid cross contamination on 10well slides, direct PBS stream along midline of slide, tilting first toward wells 1-3 followed by tilting toward wells 4-6. 7. PBS WASH (10 minutes) Wash slide(s) 10 minutes with PBS in a slide staining dish or Coplin jar. Gentle agitation is recommended at slide immersion, midpoint, and removal. This wash may be extended up to 30 minutes with no variability in final test results. Discard PBS wash solution after use. For optimal results, change PBS at midpoint and use a magnetic stirrer. 8. FLUORESCENT ANTIBODY REAGENT (Cover the wells with 10-12 drops) Remove one slide at a time from PBS and dip 3-5 times in deionized or distilled water. Tap slide on its side against bibulous paper or paper toweling to remove excess water. Immediately return slide to the incubation chamber and cover the wells completely using fluorescent antibody reagent; begin by placing a drop over each well. Repeat for each slide. Fluorescent antibody reagent has been titered to compensate for residual deionized or distilled water remaining on the slide after rinsing. 6 NOTE: It is important that slide wells do not dry during this procedure or damage to the substrate may occur. DO NOT BLOT OR DRY THE SLIDE IN ANY MANNER OR ALLOW SLIDE TO SIT WITHOUT FLUORESCENT ANTIBODY REAGENT FOR LONGER THAN 15 SECONDS. 9. INCUBATE SLIDES (30 ± 5 minutes at room temperature, i.e. 18-24°C) Place lid on incubation chamber and cover with a paper towel to prevent exposure to light if the chamber is not opaque. Allow slide(s) to incubate 30 minutes (± 5 minutes) at room temperature (18-24°C). 10. PBS RINSE Remove slide(s) from incubator tray and rinse briefly with PBS. Do not squirt buffer directly on the wells. 11. PBS WASH (10 minutes) Wash slide(s) 10 minutes with PBS in a slide staining dish or Coplin jar. En 1985, van der Woude et al ont montré que l’ANCA était fortement associé à la granulomatose de Wegener et que le titre de l’anticorps était corrélé à l’activité de la maladie (3). En 1988, Falk et Jennette ont rapporté que l’ANCA avait plusieurs spécificités antigéniques (4). Une étude ultérieure a démontré que la spécificité de l’ANCA était corrélée aux caractéristiques pathologiques des vascularites (5). Les auto-anticorps qui présentent une image cytoplasmique granulaire fine, appelés cANCA, sont généralement dirigés contre une sérine protéase, la protéinase 3 (PR-3). Flacon compte-gouttes prêt à l’emploi contenant 1,0 ml de sérum de contrôle humain négatif ANCA. Ce sérum présente une fluorescence vert sombre non spécifique peu intense des neutrophiles. Ce réactif contient 0,1 % d’azide de sodium (conservateur). Le test des auto-anticorps anti-neutrophiles cytoplasmiques (ANCA) d’Immuno Concepts utilise la technique d’immunofluorescence indirecte (IFA) initialement décrite par Weller et Coons (6). Les échantillons de patient dilués sont mis à incuber avec des neutrophiles humains qui sont fixés sur des lames de microscope en verre pour permettre la liaison spécifique des ANCA. En présence d’ANCA, les auto-anticorps se lient aux antigènes neutrophiles. Après le rinçage destiné à éliminer les anticorps non spécifiques, le substrat est mis à incuber avec de l’IgG anti-humaine conjuguée à de la fluorescéine. Lorsque les résultats sont positifs, on observe la formation d’un complexe tripartite stable composé d’un anticorps anti-humain fluorescent lié à un auto-anticorps anti-neutrophile cytoplasmique humain, lui-même lié à un antigène situé dans les cellules. Ce complexe peut être visualisé à l’aide d’un microscope à fluorescence. Les échantillons positifs au cANCA présentent une fluorescence cytoplasmique granulaire caractéristique des neutrophiles sur les lames fixées à l’éthanol et au formol. Dans les échantillons positifs au pANCA, on observe une fluorescence nucléaire diffuse ou périphérique des neutrophiles sur les cellules fixées à l’éthanol et on note une fluorescence granulaire du cytoplasme sur les cellules fixées au formol. Si l’échantillon est négatif à l’ANCA, aucune fluorescence spécifique des neutrophiles n’est observée. Réactif immunofluorescent - Spécifique à l’IgG : Référence catalogue 10009 (9 ml). Globuline de chèvre anti-IgG humaine conjuguée à de l’isothiocyanate de fluorescéine (FITC). Ce réactif contient du bleu d’Evans comme contre-colorant. Le réactif est fourni dans des flacons compte-gouttes de précision prêts à l’emploi. Ce réactif contient 0,1 % d’azide de sodium (conservateur). * Cette lame est protégée par un ou plusieurs des brevets suivants déposés aux États-Unis et à l’étranger : 4387972, D-274261, D-273261, brevet canadien 1171302 et autres brevets en attente. RÉACTIFS NÉGATIFS Tampon PBS : Référence catalogue 1011. Solution saline en poudre tamponnée au phosphate (0,01 M, pH 7,4 ± 0,2). Chaque sachet contient une quantité suffisante de poudre tampon pour préparer 1 litre de solution. Chaque kit de test complet contient un sachet de poudre tampon pour cinq lames. Substances interférentes : Les sérums présentant un degré élevé d’hémolyse, d’ictère, de lipémie ou de prolifération microbienne doivent être écartés car une réduction du titre de l’anticorps peut survenir sur les échantillons positifs. Les échantillons dont les niveaux de lipide sont très élevés peuvent entraîner la formation d’un film fluorescent non spécifique sur le substrat cellulaire. L’utilisation d’échantillons prélevés à jeun ou d’échantillons clarifiés par ultracentrifugation peut éliminer ce problème. Les échantillons contenant des particules visibles doivent être clarifiés par centrifugation avant de procéder au test. PRÉCAUTIONS 1. Tous les matériels d’origine humaine utilisés dans la préparation des contrôles pour ce produit ont été testés et se sont révélés négatifs (non-réactivité répétée) vis-à-vis des anticorps des virus de l’immunodéficience humaine 1 et 2 (vih 1 et 2), de l’anticorps du virus de l’hépatite C (hcv) et de l’antigène de surface de l’hépatite B (hbsag), selon les méthodes approuvées par la fda. Aucune méthode de test ne peut assurer totalement l’absence de vih-1, vih-2, virus de l’hépatite C, virus de l’hépatite C ou d’autres agents infectieux. Par conséquent, tous les sérums de contrôle doivent être manipulés de la même manière que des matériels considérés comme potentiellement infectieux. 2. Tous les échantillons de patient doivent être manipulés conformément aux recommandations du niveau de biosécurité 2, comme pour tout échantillon de sérum ou de sang humain potentiellement infectieux, telles qu’indiquées dans le manuel du Centers for Disease Control/National Institutes of Health : Biosafety in Microbiological and Biomedical Laboratories, 1984 Edition. 3. La dilution des composants ou l’utilisation de composants autres que ceux fournis dans ce système peut donner lieu à des résultats incohérents. 4. L’azide de sodium (0,1 %) est utilisé comme conservateur. Il est possible que l’azide de sodium réagisse au contact des canalisations en plomb ou en cuivre et forme des sels d’azides métalliques explosifs. Lors de l’élimination des réactifs, rincer abondamment les canalisations avec de l’eau afin d’éviter toute accumulation de résidus. L’azide de sodium est un poison et peut être toxique en cas d’ingestion. 5. Ce kit est destiné à une utilisation diagnostique in vitro. 6. Le sérum de contrôle titrable est destiné à être utilisé pour surveiller la reproductibilité inter-lot et inter-analyse. Il n’est pas destiné à mesurer la sensibilité ou la spécificité globale de l’essai. 7. Ne pas fumer, manger ni boire dans les zones où des échantillons ou des réactifs du kit sont manipulés. 8. Éviter toute éclaboussure ou pulvérisation d’aérosols à tout moment. 9. Les durées et températures d’incubation autres que celles spécifiées peuvent donner des résultats erronés. 10. La contamination croisée des réactifs ou des échantillons peut donner de faux résultats. 11. Avant utilisation, la verrerie réutilisable doit être lavée et rincée soigneusement afin d’éliminer tout détergent. Toute la verrerie doit être propre et sèche avant utilisation. 12. Avant utilisation, porter les réactifs, lames et échantillons à température ambiante (18-24 °C). 13. Porter des gants en latex jetables pour manipuler les échantillons et réactifs puis se laver soigneusement les mains par la suite. 14. La contamination microbienne des réactifs ou des échantillons peut donner de faux résultats. 15. Ne pas pipeter avec la bouche et éviter tout contact de la peau et des muqueuses avec les réactifs et les échantillons. En cas de contact, laver abondamment avec un savon germicide et de l’eau. Conservation : Les sérums peuvent être conservés entre 2 et 10 °C pendant une semaine maximum. Si le test est reporté, ils doivent être congelés à -20 °C minimum. Le sérum ne doit pas être conservé dans un congélateur à dégivrage automatique. ATTENTION : Les congélations et décongélations successives des échantillons de patient peuvent induire des résultats faussement positifs ou faussement négatifs. INTERPRÉTATION DES RÉSULTATS CONTRÔLE DE QUALITÉ Les contrôles positif, négatif et blanc doivent être effectués dans les puits dédiés au contrôle qualité sur chaque lame. Le contrôle positif cANCA doit présenter une fluorescence cytoplasmique granulaire vert pomme intense des neutrophiles, entre les segments nucléaires sur les lames fixées à l’éthanol ou au formol. Le contrôle positif pANCA présente une fluorescence nucléaire diffuse ou périphérique vert pomme intense des neutrophiles sur les lames fixées à l’éthanol et une fluorescence cytoplasmique granulaire sur les lames fixées au formol. Le contrôle négatif ne doit pas présenter de fluorescence vert intense. Une fluorescence vert sombre non spécifique peu intense peut être observée dans le contrôle négatif. Le contrôle à blanc est utilisé pour observer la fluorescence non spécifique du réactif anticorps et ne doit pas présenter d’image fluorescente verte. Le contre-colorant fourni dans le conjugué peut donner aux cellules une couleur rouge sombre. Si les contrôles n’apparaissent pas tel que décrit, le test n’est pas valable et doit être recommencé. CONTRÔLE TITRABLE OPTIONNEL Lors de la lecture des titres, de nombreux laboratoires commencent par lire le puits qui contient l’échantillon le plus dilué puis poursuivent « à rebours » jusqu’à la dilution 1:20. Le premier puits dans lequel une fluorescence cytoplasmique clairement perceptible est visible, est le résultat du titrage. Nous recommandons cette technique pour la détermination des résultats de titrage. Le titre moyen et la plage de titrage (± une dilution de part et d’autre de la moyenne) déterminés pour chaque numéro de lot ont été établis dans notre laboratoire et sont communiqués à titre indicatif. Ce contrôle est fourni pour permettre à chaque laboratoire d’évaluer la reproductibilité (précision) de son test ANCA. Étant donné que ce contrôle n’est pas destiné à être un indicateur de la précision du titrage, chaque laboratoire doit établir sa propre référence de titrage moyen pour cet échantillon et utiliser cette information pour évaluer la reproductibilité inter-analyse (précision). État et durée de vie de la source lumineuse : Cela est particulièrement vrai pour les sources de lumière au mercure qui affichent généralement une réduction progressive de l’énergie d’excitation à 495 nm avant de griller. La valeur escomptée dans la population saine est négative à une dilution de test de 01:20:00. RECONSTITUTION DU TAMPON (PBS) Dissoudre le contenu d’un sachet de tampon dans un litre d’eau désionisée ou distillée. Couvrir et conserver entre 2 et 10 °C pendant 4 semaines maximum ou jusqu’à ce que des signes de contamination ou de modifications visibles apparaissent. 2. DILUTION DES ÉCHANTILLONS DES PATIENTS Test : Diluer les échantillons des patients à 1:20 en ajoutant 0,05 ml (50 µl) de sérum à 0,95 ml (950 µl) de diluant pour échantillon. Mettre soigneusement la goutte en contact avec le puits de contrôle approprié en évitant tout contact direct de l’embout du comptegouttes avec la surface de la lame. Placer le contrôle positif pANCA sur le puits marqué « pANCA », le contrôle positif cANCA sur le puits marqué « cANCA » et le contrôle négatif sur le puits marqué « Nég » puis une goutte de diluant pour échantillon sur le puits marqué « Blanc ». INCUBATION DES LAMES (30 minutes ±5 minutes à température ambiante, soit 18-24 °C) Placer les lames dans une chambre humide couverte (une boîte de Pétri avec des serviettes en papier humidifiées conviendra). Ce lavage peut être prolongé jusqu’à 30 minutes sans que les résultats des tests finaux n’en soient affectés. Jeter la solution de lavage PBS après utilisation. Pour obtenir des résultats optimums, changer le tampon PBS à mi-parcours et utiliser un agitateur magnétique. 8. RÉACTIF IMMUNOFLUORESCENT (couvrir les puits avec 10 à 12 gouttes) Retirer les lames une à une du tampon PBS et les immerger 3 à 5 fois dans de l’eau désionisée ou distillée. Tapoter la tranche de la lame sur du papier absorbant ou des serviettes en papier pour éliminer l’excès d’eau. Remettre immédiatement la lame dans la chambre d’incubation et recouvrir complètement les puits de réactif immunofluorescent. Commencer par placer une goutte sur chaque puits. Recommencer l’opération pour chaque lame. Le réactif immunofluorescent a été titré de façon à compenser l’eau désionisée ou distillée résiduelle restant sur la lame après le rinçage. REMARQUE : Il est important que les puits de la lame ne se dessèchent pas pendant cette procédure sous peine d’altérer le substrat. NE PAS SÉCHER LA LAME OU OUBLIER DE LA RECOUVRIR DE RÉACTIF IMMUNOfluorescent PENDANT PLUS DE 15 SECONDES. 9. INCUBATION DES LAMES (30 minutes ±5 minutes à température ambiante, soit 18-24 °C) Placer le couvercle sur la chambre d’incubation puis, si la chambre n’est pas opaque, la couvrir d’une serviette en papier pour l’abriter de la lumière. Laisser la ou les lames incuber pendant 30 minutes (±5 minutes) à température ambiante (1824 °C). 10. RINÇAGE EN TAMPON PBS Sortir les lames du plateau d’incubation et les rincer rapidement avec du PBS. Ne pas asperger le tampon directement sur les puits. 11. Questo sistema di analisi deve essere usato come ausilio nella determinazione di anticorpi associati alla vasculite autoimmune. Gli anticorpi anticitoplasma dei neutrofili (ANCA) sono un gruppo di anticorpi reagenti con gli antigeni citoplasmatici nei neutrofili umani. Sebbene questi anticorpi furono riportati per la prima volta nel 1964,(1) la prima volta che fu riportato un collegamento degli stessi con la patologia fu nel 1982, quando Davies et al. riportarono gli anticorpi in otto pazienti con glomerulonefrite segmentale necrotizzante.(2) Nel 1984, furono riportati altri quattro pazienti con vasculite e glomerulonefrite. L’altro importante pattern della colorazione, quello perinucleare, o pattern pANCA, che è in genere dovuto agli anticorpi diretti contro la mieloperossidasi (MPO), è stata associato alla vasculite sistemica e alla glomerulonefrite idiopatica necrotizzante e semilunare.(4) Il pattern pANCA è un artefatto indotto dall’uso di etanolo come fissativo.(4) Se i neutrofili sono fissati in formalina, la mieloperossidasi (il maggior antigene responsabile del pattern pANCA nelle cellule fissate con etanolo) resta associata ai granuli primari (alfa), e mostra una distribuzione citoplasmatica granulare. La proteinasi-3 (PR-3) resta associata ai granuli primari (alfa) sia nei fissativi con etanolo che in quelli con formalina. PRINCIPIO DEL TEST Il sistema di analisi degli anticorpi anticitoplasma dei neutrofili (ANCA) della IC utilizza la tecnica dell’immunofluorescenza indiretta, descritta per la prima volta da Weller e Coons.(6) I campioni dei pazienti, diluiti, sono incubati con neutrofili umani che sono fissati sui vetrini del microscopio per consentire lo specifico legame degli ANCA. Se sono presenti ANCA, gli anticorpi si legano agli antigeni neutrofili. Dopo il lavaggio per rimuovere anticorpi non specifici, il substrato viene incubato con anti-IgG umane coniugate con fluoresceina. Quando i risultati sono positivi, avviene la formazione di un complesso stabile in tre parti composto dall’anticorpo fluorescente antiumano legato agli ANCA umani che sono legati all’antigene presente nelle cellule. Tale complesso può essere visualizzato con l’ausilio di un microscopio a fluorescenza. I campioni positivi per cANCA avranno una particolare colorazione citoplasmatica granulare dei neutrofili sia sui vetrini fissati in etanolo che su quelli fissati in formalina. Nei campioni positivi per pANCA, si noterà una colorazione diffusa o periferica dei neutrofili sulle cellule fissate con etanolo ed una colorazione granulare del citoplasma in quelle fissate con formalina. Se il campione è negativo per gli ANCA, non si vedrà alcuna colorazione specifica dei neutrofili. COMPONENTI DEL SISTEMA (MATERIALI FORNITI) Uso: tutti i componenti sono pronti per l’uso senza che siano necessarie la suddivisione in aliquote o la ricostituzione (tranne il tampone PBS che deve essere sciolto in acqua deionizzata o distillata prima dell’uso). Conservazione: tutti i componenti possono essere conservati alla temperatura di 2-10°C. Dopo la ricostituzione, il tampone PBS deve essere conservato in contenitori con tappo a vite alla temperatura di 2-10°C per massimo quattro settimane o fino a che si presentano segni di contaminazione o altri cambiamenti visibili. 12 Vetrini per substrato ANCA:* numero di catalogo 10010-11 (fissati in etanolo) o 10010-12 (fissati in formalina). Dieci vetrini a pozzetto contenenti neutrofili umani stabilizzati e fissati direttamente sui pozzetti per il test. L’esclusivo design a fossa del vetrino minimizza la contaminazione incrociata dei pozzetti durante l’analisi. I vetrini comprendono quattro pozzetti definiti pANCA, cANCA, negativo e bianco per favorire l’esatta interpretazione dei risultati. Diluente per campione ANCA: numero di catalogo 10100 (100 ml). (Nei kit di analisi completi, viene fornita una confezione di polvere tamponata per cinque vetrini). Preparazione: sciogliere una confezione di polvere tamponata in un (1) litro di acqua deionizzata o distillata e conservare in frigorifero ad una temperatura di 2-10° per massimo 4 settimane o fino a che non compaiono segni di contaminazione o altri cambiamenti visibili. Tutti i materiali di origine umana usati per la preparazione dei controlli per questo prodotto sono stati analizzati e trovati negativi (non ripetutamente reattivi) per gli anticorpi del virus dell’immunodeficienza umana tipo 1 (HIV-1), del virus della immunodeficienza umana tipo 2 (HIV-2), del virus dell’epatite C (HCV) e per l’antigene di superficie dell’epatite B (HBsAg) con un metodo approvato dalla FDA. Nessun metodo di analisi è in grado di garantire con completa sicurezza che siano assenti HIV-1, HIV-2, virus dell’epatite C, virus dell’epatite B o altri agenti infettivi. Quindi, tutti i sieri di controllo vanno maneggiati secondo le stesse modalità utilizzate per i materiali potenzialmente infettivi. 2. Tutti i campioni dei pazienti devono essere maneggiati osservando le precauzioni di sicurezza biologica di livello 2, come raccomandato per ogni siero umano potenzialmente infettivo o per campioni di sangue nel manuale pubblicato per i CDC/NIHM (Centers for Disease Control/National Institutes of Health Manual, Centri per il Controllo delle Infezioni/ Istituti Nazionali per la Sanità): Biosafety in Microbiological and Biomedical Laboratories, Edizione 1984. 3. La diluizione di componenti o la sostituzione di componenti diversi da quelli forniti in questo sistema può dare risultati non coerenti. 4. Il sodio azide (0,1%) viene usato come conservante. Il sodio azide può reagire nelle tubature di piombo o rame formando sali metallici di azide esplosivi. Quando si eliminano i reagenti, far scorrere grandi quantità di acqua del rubinetto per evitare la formazione di potenziali residui nelle tubature. Il sodio azide è un veleno e può essere tossico se ingerito. 5. Questo kit è per uso diagnostico in vitro. 6. Il siero di controllo titolabile è destinato ad essere usato nel monitoraggio da lotto a lotto e nella riproducibilità interfase. Non è destinato alla misurazione della sensibilità complessiva o della specificità del dosaggio. 7. Prima dell’uso, tutta la vetreria di laboratorio deve essere pulita e asciutta. 12. Prima dell’uso, portare tutti i reagenti, i vetrini e i campioni a temperatura ambiente (18-24°C). 13. Indossare guanti a perdere in lattice quando si maneggiano campioni e reagenti e dopo lavare accuratamente le mani. 14. La contaminazione microbica dei reagenti o dei campioni può dare origine a risultati falsi. 15. Non pipettare mai con la bocca ed evitare il contatto dei reagenti e dei campioni con la cute e le mucose. In caso di contatto, lavare abbondantemente con un sapone germicida e acqua. ambiente (18-24°C). Non appena possibile, il siero deve essere separato dal coagulo in modo da ridurre al minimo l’emolisi. Sostanze interferenti: non vanno usati sieri che mostrano un alto grado di emolisi, ittero, lipemia o crescita microbica, perché può verificarsi una diminuzione del titolo anticorporale nei campioni positivi. Campioni con livelli di lipidi molto alti possono causare la formazione di una pellicola fluorescente non specifica sul substrato cellulare. L’uso di campioni a digiuno o diafanizzanti per ultracentrifugazione può eliminare questo problema. Campioni contenenti sostanze particellari visibili vanno chiariti per centrifugazione prima dell’analisi. Conservazione: i sieri possono essere conservati a 2-10°C fino ad una settimana. Se l’analisi viene ulteriormente rimandata, i sieri devono essere conservati congelati a -20°C o a una temperatura inferiore. Il siero non deve essere conservato in freezer autosbrinanti. ATTENZIONE: il ripetuto congelamento e scongelamento dei campioni dei pazienti può dare origine a falsi risultati positivi o a falsi risultati negativi. INTERPRETAZIONE DEI RISULTATI CONTROLLO DELLA QUALITÀ I controlli positivo, negativo e del bianco devono essere analizzati nei pozzetti previsti per il controllo qualità su ciascun vetrino. Il controllo positivo cANCA dovrebbe mostrare una colorazione citoplasmatica granulare dei neutrofili verde mela brillante tra i segmenti nucleari, sia sui vetrini fissati in etanolo che su quelli fissati in formalina. Il controllo positivo pANCA dovrebbe mostrare una colorazione nucleare dei neutrofili diffusa o periferica, sui vetrini fissati in etanolo e a fluorescenza citoplasmatica granulare sui vetrini fissati in formalina. Il controllo negativo non dovrebbe mostrare alcuna fluorescenza verde brillante. Nel controllo negativo può essere osservata una fluorescenza verde spento, non specifica. Il controllo del bianco si usa per osservare la fluorescenza non specifica da parte del reagente anticorpo, e non dovrebbe mostrare alcuna colorazione fluorescente verde. Il colorante di contrasto fornito nel coniugato può dare alle cellule una sbiadita colorazione rossa. Se i controlli non appaiono come descritto, il test non è valido e deve essere ripetuto. CONTROLLO TITOLABILE OPZIONALE Nel leggere i titoli, molti laboratori iniziano a leggere dal pozzetto contenente il campione più diluito e leggono “all’indietro” fino alla diluizione 01:20. Il primo pozzetto in cui è visibile una colorazione citoplasmatica chiaramente distinguibile è il punto di equivalenza della reazione. Raccomandiamo questa tecnica per determinare i punti di equivalenza della reazione. Il titolo medio e la gamma del titolo (± una diluizione su ciascun lato della media) determinati per ciascun numero di lotto sono stati stabiliti nel nostro laboratorio e sono indicati come guida. Questo controllo è fornito per consentire a ciascun laboratorio di valutare la riproducibilità (di precisione) del proprio test ANCA. Dal momento che questo controllo non è destinato ad essere un indicatore dell’accuratezza del titolo, ciascun laboratorio deve stabilire il proprio punto medio di equivalenza della reazione per questo campione e deve usare tali informazioni per valutare la riproducibilità interfase (di precisione). Attraverso analisi multiple di questo controllo titolabile, usando il sistema di analisi in fluorescenza degli ANCA della Immuno Concepts, è stato stabilito un valore medio del titolo per ciascun numero di lotto. Il numero di lotto, il titolo medio e la gamma del titolo (± una duplice diluizione su ciascun lato della media) sono indicati sull’etichetta della fiala e vanno usati come guida della performance del sistema di analisi. I valori ottenuti nel nostro laboratorio possono essere diversi dai vostri. Di seguito sono indicati alcuni tra i fattori che possono influenzare i risultati. 1. Il tipo di fonte luminosa. Fonti luminose al mercurio producono una maggiore energia di eccitazione a 495 nm rispetto a quelle al quarzo/alogene. Le fonti luminose al mercurio da 50 watt, 100 watt e 200 watt differiscono poco nell’energia di eccitazione a 495 nm. Fonti luminose al quarzo/alogene da 100 watt generano una maggiore energia di eccitazione a 495 nm rispetto a quelle al quarzo/alogene da 50 watt. 2. La condizione e l’età della fonte di luce. Questo in particolare per le fonti luminose al mercurio RACCOLTA DI CAMPIONI Raccolta: il siero è il campione preferito. Devono essere prelevati con tecnica asettica circa 5 ml di sangue intero per venopuntura usando una provetta di raccolta sterile a vuoto o un altro sistema di raccolta adatto. Lasciare che il sangue si coaguli a temperatura 13 3. 4. 5. 6. 7. che in genere mostrano una graduale riduzione nell’energia di eccitazione a 495 nm prima di esaurirsi. Questa riduzione graduale dell’energia di eccitazione può comportare una significativa perdita di sensibilità dopo diverse settimane. Questo problema può essere evitato tenendo una registrazione dei tempi. Per risultati ottimali, sostituire le lampadine a mercurio da 50 watt dopo 100 ore e quelle da 100 o 200 watt dopo 200 ore. Le fonti luminose al quarzo/alogene in genere non mostrano una graduale riduzione dell’energia di eccitazione prima di esaurirsi. Il tipo di filtro di eccitazione usato. I filtri eccitatori ad interferenza danno maggiore sensibilità in una lunghezza d’onda molto più ridotta rispetto ai filtri eccitatori ad assorbimento. Con fluorescenza a luce incidente (Epi), la fluorescenza cresce in modo esponenziale man mano che aumenta anche l’apertura numerica (NA) dell’obiettivo. Questo può far sì che un obiettivo da 40X con una NA di 0,65 legga una o più diluizioni più basse rispetto ad un obiettivo da 40X con un NA di 0,85. L’apertura numerica è stampata sul lato dell’obiettivo. La sensibilità del microscopio a luce fluorescente trasmessa non è influenzata da NA. I filtri a soppressione. I filtri a soppressione riducono le specifiche lunghezze d’onda dell’eccitazione e possono essere usati per ridurre la sensibilità. La precisione e l’accuratezza della tecnica di diluizione, dell’apparecchiatura e della performance delle procedure di analisi. INTERPRETAZIONE DEI RISULTATI DEL PAZIENTE LIMITI DEL TEST 1. Le diagnosi non possono essere fatte solo sulla base del rilevamento dell’anticorpo anti-citoplasma dei neutrofili. Il medico deve interpretare questi risultati confrontandoli con l’anamnesi e i sintomi del paziente, i dati fisici e altre procedure diagnostiche. 2. La cura non va iniziata sulla sola base di un test positivo per gli anticorpi anti-citoplasma dei neutrofili. Prima di iniziare qualunque trattamento devono essere considerate indicazioni cliniche, altri risultati di laboratorio e l’impressione clinica del medico. 3. Gli anticorpi antinucleari presenti nei campioni dei pazienti possono reagire con il substrato cellulare. Se sono presenti anticorpi antinucleari, i risultati ANCA possono spesso essere interpretati per il diverso carattere della specifica colorazione ANCA, per i titoli diversi e/o per la diversa intensità delle reazioni. Se la reazione dell’anticorpo antinucleare fluorescente è abbastanza forte da oscurare la colorazione ANCA, l’analisi va riportata come “non interpretabile”, e va usato un metodo alternativo per stabilire lo status degli ANCA del paziente. 4. A causa delle numerose opzioni disponibili nei microscopi a fluorescenza, si raccomanda di standardizzare le fonti luminose, i filtri e gli strumenti ottici in caso di confronto dei titoli dei pazienti tra i vari laboratori. 5. Per stabilire lo status clinico del paziente, i risultati di questa analisi vanno usati in connessione alle informazioni disponibili a partire da valutazioni cliniche ed altre procedure diagnostiche. VALORI ATTESI Il valore atteso nella popolazione normale è negativo in una diluizione di screening di 01:20. In pazienti con patologie, sono stati riportati titoli alti fino 1:640.(7) CARATTERISTICHE PRESTAZIONALI CAMPIONI NORMALI Per visualizzare i neutrofili si raccomanda un ingrandimento totale di 400X. Negativo: un siero è considerato negativo per ANCA se, ad una diluizione di 1:20, la colorazione verde fluorescente delle cellule è inferiore o uguale al pozzetto provetta di controllo negativo. Nei neutrofili si può osservare colorazione non specifica di fondo dovuta ad anticorpi eterofili o autoanticorpi. Positivo: un siero si considera positivo per gli ANCA se, ad una diluizione di 1:20, le cellule in ciascun campo mostrano una fluorescenza citoplasmatica granulare simile a quella osservata con il controllo cANCA su vetrini fissati in etanolo o in formalina. Alternativamente, un siero si considera positivo per gli ANCA se, ad una diluizione di 1:20, le cellule mostrano una fluorescenza diffusa o periferica simile a quella osservata con il controllo pANCA su vetrini fissati in etanolo. RIPORTO DEI RISULTATI Screening: i risultati devono essere riportati come positivi o negativi alla diluizione di 01:20. Pattern della colorazione: molti anticorpi possono causare la colorazione del citoplasma e/o del nucleo dei neutrofili. Esistono due importanti pattern di colorazione specifica. Furono testati in parallelo, usando il kit ANCA della Immuno Concepts e un altro kit disponibile in commercio, campioni di siero di 497 donatori di sangue (247 maschi e 250 femmine). Tutti i campioni che erano positivi su vetrini fissati in etanolo in questo gruppo furono testati anche per gli anticorpi antinucleari (ANA) usando il sistema di analisi degli ANA HEp-2 della Immuno Concepts. Tra i campioni normali ce n’erano 22 positivi per ANA e vennero considerati non interpretabili per ANCA. I rimanenti campioni discrepanti furono testati per gli anticorpi a MPO e PR3 usando le analisi ELISA per definire il vero status degli anticorpo. RICOSTITUZIONE DEL TAMPONE (PBS) Sciogliere il contenuto di una confezione di tampone in un litro di acqua deionizzata o distillata. Coprire e conservare alla temperatura di 2-10° fino a quattro settimane o fino a che non si presentino segni di contaminazione o altri cambiamenti visibili. 2. DILUIZIONE DEI CAMPIONI DEI PAZIENTI Screening: diluire i campioni del paziente a 01:20 aggiungendo 0,05 ml (50 µl) di siero a 0,95 ml (950 µl) di diluente per campioni. Titolo semi-quantitativo: per preparare diluizioni seriali duplici di campioni di screening (ad es. 1:40, 1:80, 1:160...1:640), rimuovere 0,5 ml della diluizione 1:20 e mescolarla con 0,5 ml di diluente per campioni per ottenere una diluizione a 01:40, e continuare le diluizioni seriali in tal modo. 3. DILUIZIONE OPZIONALE DEL CONTROLLO TITOLABILE Trattare il controllo opzionale titolabile come un campione del paziente non diluito. Diluire il controllo a 01:20 aggiungendo 0,05 ml (50 µl) di siero di controllo a 0,95 ml diluente per campioni. Preparare diluizioni seriali duplici del controllo titolabile, come sopra descritto. 4. PREPARAZIONE DEI VETRINI DEL SUBSTRATO (20-25 µl/pozzetto) Rimuovere il/i vetrino/i dal contenitore/i e porre i sieri di controllo sui pozzetti di controllo come descritto di seguito. Capovolgere il flacone contagocce di controllo e premere delicatamente fino a che in punta è visibile la goccia. Appoggiare delicatamente la goccia sul pozzetto di controllo appropriato evitando il contatto diretto della punta del contagocce con la superficie del vetrino. Mettere il controllo pANCA positivo sul pozzetto contrassegnato come “pANCA”, il controllo cANCA positivo sul pozzetto indicato come “cANCA”, il controllo negativo sul pozzetto indicato come “neg” e una goccia di diluente per campione sul pozzetto indicato come “bianco”. Aggiungere 1 goccia (20-25 µl) di campione diluito del paziente ai pozzetti numerati. ATTENZIONE: IL CONTATTO DIRETTO DELLA PUNTA DEL CONTAGOCCE CON LA SUPERFICIE DEL VETRINO PUÒ DANNEGGIARE IL SUBSTRATO ANTIGENE. 5. INCUBAZIONE DEI VETRINI (30±5 minuti a temperatura ambiente, ovvero 18-24°C) Mettere il/i vetrino/i in una camera umida, coperta (una capsula di Petri con carta assorbente inumidita andrà bene). Incubare, col coperchio, per 30 minuti (±5 minuti) a temperatura ambiente (18-24°C). 6. RISCIACQUO COL PBS Rimuovere il/i vetrino/i dal piatto dell’incubatore e sciacquare brevemente con PBS usando una bottiglia a zampillo, una pipetta di Pasteur o una pipetta sierologica. Non far zampillare il tampone direttamente sui pozzetti. NOTA: per evitare la contaminazione incrociata sui vetrini con 10 pozzetti, orientare il flusso di PBS lungo la linea mediana del vetrino, inclinando prima verso le provette 1-3 e poi verso quelle 4-6. 7. LAVAGGIO COL PBS (10 minuti) Lavare il/i vetrino/i per 10 minuti con PBS in un piatto per la colorazione del vetrino o una vaschetta Coplin. Si raccomanda di agitare con delicatezza all’immersione del vetrino, al punto mediano e alla rimozione. Questo lavaggio può durare fino a 30 minuti senza che i risultati finali dell’analisi varino. Eliminare la soluzione di lavaggio PBS dopo l’uso. Per risultati ottimali, cambiare il PBS al punto mediano e usare un agitatore magnetico. 16 8. REAGENTE ANTICORPO FLUORESCENTE (coprire i pozzetti con 10-12 gocce) Rimuovere uno alla volta i vetrini dal PBS e immergerli 3-5 volte in acqua deionizzata o distillata. Tamponare il vetrino sul lato con carta bibula o con carta assorbente in modo da eliminare l’acqua in eccesso. Riportare immediatamente il vetrino nella camera di incubazione e coprire completamente i pozzetti usando il reagente anticorpo fluorescente; iniziare mettendo una goccia su ogni pozzetto. Ripetere l’operazione per ciascun vetrino. Il reagente anticorpo fluorescente è stato titolato al fine di compensare l’acqua deionizzata o distillata residua che resta sul vetrino dopo il risciacquo. NOTA: è importante che i pozzetti dei vetrini non si asciughino durante questa procedura onde evitare il danneggiamento del substrato. TAMPONARE O ASCIUGARE IL VETRINO IN MANIERA CHE LO STESSO NON RESTI SENZA REAGENTE ANTICORPO FLUORESCENTE PER PIÙ DI 15 SECONDI. 9. INCUBAZIONE DEI VETRINI (30±5 minuti a temperatura ambiente, ovvero 18-24°C) Mettere il coperchio sulla camera di incubazione e coprire con carta assorbente per impedire l’esposizione alla luce se la cella non è opaca. Lasciare incubare il/i vetrino/i per 30 minuti (±5 minuti) a temperatura ambiente (18-24°C). 10. RISCIACQUO COL PBS Rimuovere il/i vetrino/i dal piatto dell’incubatore e sciacquare brevemente con PBS. Non far zampillare il tampone direttamente sui pozzetti. 11. TAMPONARE O ASCIUGARE IL VETRINO IN MANIERA CHE NON RIMANGA SENZA COPRIVETRINO PER PIÙ DI 15 SECONDI. Aggiungere 4-5 gocce di mezzo di fissaggio semipermanente lungo la linea mediana di ciascun vetrino. Posizionare con attenzione il vetrino coprioggetto, evitando vuoti d’aria, abbassando delicatamente il vetrino coprioggetto da un estremo all’altro del vetrino. NOTA: una quantità eccessiva di mezzo di fissaggio sul vetrino può causare un’alta fluorescenza di fondo, dovuta alla dispersione della luce o alla mancanza di risoluzione chiara delle cellule (immagine sfocata). L’eccesso del mezzo di fissaggio può essere eliminato dal vetrino tamponando delicatamente il vetrino coprioggetto con carta assorbente o carta per pulire lenti evitando qualsiasi movimento diretto del vetrino coprioggetto. PER ASSISTENZA TECNICA: +1 916-363-2649 oppure a mezzo e-mail: [email protected] SISTEMA DE DETECCIÓN DE ANCA PARA USO DIAGNÓSTICO IN VITRO RESUMEN Y EXPLICACIÓN DE LA PRUEBA Uso previsto: Se trata de una determinación indirecta por inmunofluorescencia para la detección semicuantitativa de autoanticuerpos citoplásmicos antineutrófilos (ANCA) en suero humano. Este sistema de análisis ha de emplearse como ayuda en la detección de anticuerpos asociados a las vasculitis autoinmunitarias. Los autoanticuerpos citoplásmicos antineutrófilos (ANCA) son un grupo de anticuerpos que reaccionan con los antígenos citoplásmicos de los neutrófilos humanos. Aunque aparecieron por primera vez en un informe de 1964 (1), su primera vinculación con enfermedades apareció en 1982, cuando Davies y cols. informaron de su presencia en ocho pacientes con glomerulonefritis necrotizante segmentaria (2). En 1984, se informó de otros cuatro pacientes con vasculitis y glomerulonefritis. En 1985 van der Woude y cols. demostraron que los ANCA tenían una elevada asociación a la granulomatosis de Wegener, y que el título de anticuerpos se correlacionaba con la actividad de la enfermedad (3). En 1988, Falk y Jenette publicaron que los ANCA tenían más de una especificidad antigénica (4). En un informe ulterior se comunicó que la especificidad de los ANCA se correlacionaba con las características anatomopatológicas de las vasculitis (5). En el análisis de ANCA por inmunofluorescencia se pueden observar varios patrones de tinción celular. Se han descrito dos principales; han sido bien caracterizados con neutrófilos fijados en etanol, y se emplean en la detección de ANCA por inmunofluorescencia. Los autoanticuerpos que muestran un patrón citoplásmico granular fino, denominados cANCA, suele ir dirigidos contra una serina proteasa, la proteinasa 3 (PR-3). Se ha demostrado que estos autoanticuerpos presentan un elevado grado de asociación a la granulomatosis de Wegener. El otro patrón de tinción importante, el perinuclear o pANCA, que se suele deber a autoanticuerpos dirigidos con la mieloperoxidasa (MPO), se ha asociado a las vasculitis sistémicas y a la glomerulonefritis necrotizante idiopática y con semilunas (4). El patrón pANCA es un artefacto inducido por el uso de etanol como fijador (4). Si los neutrófilos se fijan con formol la mieloperoxidasa (el principal antígeno responsable del patrón pANCA en células fijadas con etanol) sigue asociado a los gránulos primarios (alfa), y muestra una distribución citoplásmica granular. La proteinasa 3 (PR-3) sigue asociada a los gránulos primarios (alfa) tanto si se emplea etanol como fijador como si se emplea formol. PRINCIPIO DE LA PRUEBA El análisis de autoanticuerpos citoplásmicos antineutrófilos (ANCA) de Immuno Concepts emplea la técnica indirecta de detección de anticuerpos por inmunofluorescencia (IFA) descrita por primera vez por Weller y Coons (6). Se incuban muestras de pacientes diluidas con neutrófilos humanos, que se fijan en portaobjetos de vidrio para permitir la unión específica de los ANCA. Si hay ANCA, los autoanticuerpos se unen a los antígenos de los neutrófilos. Tras el lavado para retirar los anticuerpos unidos de forma inespecífica, se incuba el sustrato con IgG antihumana conjugada con fluoresceína. Si los resultados son positivos se forma un complejo estable con tres partes: el anticuerpo antihumano fluorescente unido al ANCA humano, unido a su vez al antígeno localizado en las células. Este complejo se puede visualizar con la ayuda de un microscopio de fluorescencia. Las muestras positivas para ANCA mostrarán una tinción citoplásmica granular de los neutrófilos característica, tanto en los portaobjetos fijados con etanol como en los fijados con formol. En las muestras positivas para pANCA se observará un patrón de tinción difusa o periférica de los neutrófilos en las células fijadas con etanol, y una tinción granular del citoplasma en las fijadas con formol. Si la muestra es negativa para ANCA, los neutrófilos no se teñirán de forma específica. COMPONENTES DEL SISTEMA (MATERIALES SUMINISTRADOS) Utilización: Todos los componentes se entregan listos para su empleo, sin necesidad de fragmentarlos ni prepararlos (excepto el tampón PBS, que debe ser disuelto en agua desionizada o destilada antes de utilizarlo). Conservación: Todos los componentes deben conservarse en refrigerador a 2-10 ºC. Una vez preparado, el tampón PBS debe conservarse en envases con tapón de rosca, en refrigerador a 2-10 ºC durante cuatro semanas como máximo, hasta que aparezcan signos de contaminación u otros cambios visibles. Estabilidad: Todos los componentes son estables al menos durante 12 meses a partir de la fecha de fabricación. No utilice los componentes pasada la fecha de caducidad. OTROS MATERIALES NECESARIOS (PERO QUE NO SE SUMINISTRAN) OBTENCIÓN DE MUESTRAS Obtención: La muestra ideal es suero. Se obtendrán aproximadamente 5 ml de sangre por venipunción aséptica, con un tubo de vacío estéril u otro sistema de obtención adecuado. Deje que la sangre coagule a temperatura ambiente (18-24 ºC). Se separará el suero del coágulo cuanto antes, para que no se produzca hemólisis. Sustancias que interfieren: No se utilizarán sueros que muestren grados elevados de hemólisis, ictericia, lipemia o crecimiento microbiano, pues en todas esas circunstancias se pueden producir una reducción el título de anticuerpos en muestras positivas. Las muestras con niveles de lípidos muy elevados pueden presentar una película fluorescente inespecífica sobre el sustrato celular. Este problema se puede eliminar utilizando muestras tomadas en ayunas o aclarándolas por ultracentrifugado. Si la muestra presenta partículas visibles, éstas deben ser eliminadas por centrifugado antes de la prueba. No se utilizarán refrigeradores ni congeladores con sistema “auto-frost” (eliminación automática de la escarcha). PRECAUCIONES 1. Todos los materiales de procedencia humana utilizados en este producto han sido analizados en busca de anticuerpos con el virus de la inmunodeficiencia humana-1 (VIH-1), el virus de la inmunodeficiencia humana-2 (VIH-2), el virus de la hepatitis C (VHC) y el antígeno de superficie de la hepatitis B (HBsAG) con métodos aprobados por la FDA, obteniendo resultados negativos (no reactivos en varias ocasiones) en todos los casos. Pero no existe ningún método de análisis que pueda garantizar por completo la ausencia de VIH-1, VIH-2, hepatitis C, hepatitis B u otros agentes infecciosos. Por eso, todos los sueros de control deben ser manipulados como si fueran infecciosos. 2. Todas las muestras de paciente deben ser manipuladas según el nivel 2 de bioseguridad, según se recomienda en el manual de los Centers for Disease Control/National Institutes of Health para toda muestra de suero o sangre humana potencialmente infecciosa: Biosafety in Microbiological and Biomedical Laboratories, 1984 Edition. 3. La disolución de los componentes o su sustitución por otros distintos de los suministrados con el sistema puede arrojar resultados incoherentes. 4. Se emplea azida sódica (0,1%) como conservante. La azida sódica puede reaccionar con el plomo o con las conducciones de cobre y formar sales de azidas metálicas explosivas. Al eliminar los reactivos, lavar con grandes volúmenes de agua del grifo para evitar que queden residuos en las tuberías. La azida sódica es venenosa y puede ser tóxica en caso de ingestión. 5. Este kit es para uso diagnóstico in vitro. 6. El suero de control titulable debe utilizarse para controlar la reproductibilidad entre lotes y entre ciclos. No está concebido para medir la sensibilidad o la especificidad generales del ensayo. 7. Esta prohibido fumar, comer o beber en las zonas de manipulación de las muestras o los reactivos del kit. 8. Evite salpicaduras y la generación de aerosoles en todo momento. 9. Si los tiempos de incubación y las temperaturas no son los especificados, los resultados pueden ser erróneos. 10. La contaminación cruzada de los reactivos o de las muestras puede dar resultados falsos. 11. Los elementos de vidrio reutilizables deben ser lavados y enjuagados a fondo para eliminar los detergentes antes de su uso. Todos los elementos de vidrio deben estar limpios y secos antes de su uso. 12. Todos los reactivos, portaobjetos y muestras deben estar a temperatura ambiente (18-24 ºC) antes de su uso. 13. Para manipular las muestras y los reactivos debe utilizar guantes de látex desechables, y cuando acabe deberá lavarse bien las manos. 14. La contaminación microbiana de los reactivos o de las muestras puede dar resultados falsos. 15. No pipetée nunca con la boca y evite el contacto de los reactivos y las muestras con la piel y las mucosas. En caso de contacto, lávese con un jabón germicida y agua abundante. 18 PRECAUCIÓN: Si las muestras son sucesivamente congeladas y descongeladas, se pueden obtener resultados falsos positivos y negativos. INTERPRETACIÓN DE LOS RESULTADOS CONTROL DE CALIDAD Para el control de calidad se dispondrán los controles positivo, negativo y vacíos en los pocillos al efecto que se encuentran en todos los portaobjetos. El control positivo cANCA debe dar una tinción citoplásmica granular fluorescente de color verde manzana brillante entre los segmentos nucleares de los neutrófilos, tanto en los portaobjetos fijados con etanol como en los fijados con formol. El control positivo pANCA muestra una tinción nuclear difusa o periférica de color verde manzana brillante los neutrófilos en los portaobjetos fijados con etanol, y una fluorescencia citoplásmica granular en los fijados con formol. El control negativo no debe mostrar fluorescencia verde brillante. En el control negativo se puede observar una fluorescencia de color verde mate inespecífica y baja intensidad. El control vacío se emplea para observar la tinción inespecífica del reactivo de anticuerpos, y no debe dar fluorescencia verde. La contratinción que se facilita con el conjugado puede teñir las células de color rojo mate. Si los controles no se ven como se ha descrito, la prueba no es válida y debe repetirse. CONTROL TITULABLE OPCIONAL Para interpretar los títulos, muchos laboratorio empiezan por el pocillo que contiene la muestra más diluida, y van “hacia atrás” hasta la dilución 1:20. El primer pocillo en el que se aprecie un patrón discernible de tinción del citoplasma corresponde al título que se considera como criterio de valoración. Recomendamos utilizar esta técnica para determinar los criterios de valoración de los títulos. El título medio y el rango de títulos (± una dilución a cada lado de la media) determinados para cada número de lote han sido establecidos en nuestro laboratorio, y se consideran la norma. Este control se facilita para que cada laboratorio pueda evaluar la reproductibilidad (precisión) de sus análisis de ANCA. Como quiera que este control no ha sido diseñado como indicador de la exactitud de la titulación, cada laboratorio deberá establecer su propio criterio de valoración del título medio para cada muestra, utilizando esta información para evaluar la reproductibilidad (precisión) de unos ciclos a otros. Mediante la reiterada evaluación de este control titulable utilizando el sistema de análisis de ANCA por inmunofluorescencia de Immuno Concepts, se ha establecido un título medio para cada número de lote. El número de lote, el título medio y el rango de títulos (± una dilución doble a cada lado de la media) figuran en la etiqueta del vial y deben servir como guía para el funcionamiento del sistema de análisis. Puede que los valores obtenidos en nuestro laboratorio difieran de los suyos. He aquí algunos de los muchos factores que pueden afectar a sus resultados: 1. Tipo de fuente de luz: Las fuentes de luz de mercurio producen una energía de excitación a 495 nm mayor que las de cuarzo/halógenas. Las fuentes de luz de mercurio de 50, 100 y 200 vatios difieren poco en la energía de excitación a 495 nm. Las fuentes de luz de cuarzo/halógenas de 100 vatios producen una energía de excitación a 495 nm superior a la proporcionada por las de cuarzo/ halógenas de 50 vatios. 2. El estado de la fuente de luz y su edad: Esto es especialmente cierto para las fuentes de luz de mercurio, que suelen experimentar una reducción gradual de la energía de excitación a 495 nm antes de agotarse. Esta gradual reducción de la energía de excitación puede producir una pérdida significativa de la sensibilidad en un plazo de semanas. El problema se puede evitar con un registro cronológico. Para que los resultados sean óptimos, cambie las bombillas de mercurio de 500 vatios cada 100 horas, y las de 100 ó 200 vatios cada 200 horas. Por lo general, las fuentes de luz de cuarzo/halógenas no experimentan una reducción gradual de la energía de excitación antes de agotarse. 3. El tipo de filtro excitador utilizado: Los filtros excitadores por interferencia proporcionan mayor sensibilidad en una longitud de onda mucho menor que los filtros excitadores por absorción. Si desea más información, consulte el manual de su microscopio de fluorescencia, o con su delegado de ventas. 4. Correcta alineación del haz de luz del microscopio. Consulte las instrucciones del manual del microscopio de fluorescencia. 5. La apertura numérica del objetivo. Si se emplea fluorescencia por luz incidente (Epi), la fluorescencia aumenta exponencialmente con la apertura numérica (NA) del objetivo. Así, puede que un objetivo de 40X con una apertura de 0,65 interprete una o más diluciones como inferiores a las determinadas con un objetivo de 40X con una apertura numérica de 0,85. La apertura numérica va impresa a un lado del objetivo. La apertura numérica no afecta a la sensibilidad del microscopio de luz fluorescente transmitida. 6. Filtros de supresión. Los filtros de supresión reducen longitudes de onda de excitación concretas, y se pueden utilizar para reducir la sensibilidad. Si desea más información, consulte el manual de su microscopio de fluorescencia, o con su delegado de ventas. 7. Precisión y exactitud de la técnica de dilución, del equipo y de la realización de los procedimientos de análisis. INTERPRETACIÓN DE LOS RESULTADOS DEL PACIENTE Para ver los neutrófilos se recomienda un aumento total de 400X. Negativos: Se considera que un suero es negativo para ANCA si a una dilución de 1:20 la tinción fluorescente verde de las células es igual o inferior a la del pocillo de control negativo. En los neutrófilos se puede observar una tinción de fondo inespecífica, debida a los anticuerpos heterófilos o a los autoanticuerpos. Positivos: Se considera que un suero es positivo para ANCA si a una dilución de 1:20 las células de todos los campos muestran fluorescencia citoplásmica granular similar a la observada en el control cANCA, tanto en los portaobjetos fijados con etanol como en los fijados con formol. Por otro lado, se considera que un suero es positivo para ANCA si a una dilución de 1:20 las células muestran fluorescencia nuclear difusa o periférica similar a la observada en el control pANCA en los portaobjetos fijados con etanol. NOTIFICACIÓN DE LOS RESULTADOS Selección: Se indicará si el resultado es positivo o negativo a la dilución 1:20. Patrones de tinción: Muchos autoanticuerpos puede teñir el citoplasma y/o el núcleo de los neutrófilos. Hay dos patrones principales de tinción específicos: cANCA (tinción clásica o citoplásmica): La tinción de los gránulos alfa (primarios) del citoplasma muestra un patrón de tinción citoplásmica moteada homogéneo, a menudo con una concentración de la tinción en los lóbulos del núcleo. El moteado citoplásmico se observa tanto en los neutrófilos fijados con etanol como en los fijados con formol pANCA (tinción perinuclear): Tinción lisa u homogénea de los núcleos multilobulados, a menudo con una notable tinción periférica de los lóbulos nucleares de los neutrófilos fijados con etanol. En los neutrófilos fijados con formol estos anticuerpos muestran una tinción citoplásmica granular. Los anticuerpos antinucleares y otros autoanticuerpos pueden reaccionar con los neutrófilos, por lo que en todas las muestras positivas se practicará un análisis de ANA con sustrato del células HEp-2 antes de declarar la presencia de ANCA. Titulación opcional: Los resultados se notificarán como la inversa de la dilución del último pocillo en el que se observa reacción positiva. LIMITACIONES DEL ANÁLISIS 1. No es posible hacer un diagnóstico basándose sólo en la detección de anticuerpos citoplásmicos antineutrófilos. El médico debe interpretar estos resultados en el contexto de la historia y los síntomas del paciente, los hallazgos físicos y otros procedimientos diagnósticos. 2. No se debe iniciar un tratamiento basándose exclusivamente en un resultado positivo del análisis de anticuerpos citoplásmicos antineutrófilos. Antes de iniciar un tratamiento hay que tener en cuenta las indicaciones clínicas, otros hallazgos de laboratorio y la impresión clínica del médico. 3. Los anticuerpos antinucleares presentes en las muestras de los pacientes pueden reaccionar con el sustrato celular. Si hay anticuerpos antinucleares, a menudo se pueden interpretar los resultados ANCA debido al diferente carácter de la tinción ANCA específica, a que los títulos son diferentes, y/o a la intensidad de las reacciones. Si la reacción fluorescente de los anticuerpos antinucleares es tan intensa que oscurece la tinción ANCA se declarará la prueba como “no interpretable” y se utilizará otro método para determinar el estado ANCA del paciente. 4. Como quiera que existen muchas opciones en los microscopios de fluorescencia, se recomienda estandarizar las fuentes de luz, los filtros y los medios ópticos para comparar los títulos de los pacientes obtenidos en diferentes laboratorios. 5. Los resultados de esta prueba, junto con la información obtenida de la evaluación clínica y otros procedimientos diagnósticos, servirán para determinar la situación clínica del paciente. VALORES ESPERADOS El valor previsto en la población normal es negativo a la dilución 1:20 utilizada para la selección. En pacientes con enfermedades, se han notificado valores de hasta 1:640 (7). CARACTERÍSTICAS DEL RENDIMIENTO MUESTRAS NORMALES Se analizaron en paralelo muestras de suero de 497 donantes de sangre (247 varones y 250 mujeres) con el kit ANCA de Immuno Concepts y otro kit comercializado. Todas las muestras que dieron positivo en portaobjetos fijados con etanol en esta población también fueron sometidas a la detección de anticuerpos antinucleares (ANA) mediante el sistema de análisis de ANA HEp-2 de Immuno Concepts. Entre las muestras normales, 22 dieron positivo para ANA y fueron consideradas no interpretables para ANCA. Las demás muestras discrepantes fueron sometidas a la detección de anticuerpos anti MPO y PRE mediante ELISA para determinar la situación real de los anticuerpos. SUEROS ANTERIORMENTE DETERMINADOS COMO POSITIVO PARA ANCA POR INMUNOFLUORESCENCIA INDIRECTA Las muestras de suero que fueron consideradas positivas para ANCA mediante prueba IFA locales se obtuvieron de laboratorios de referencia de los Estados Unidos, el Reino Unido y Australia. En esta parte del estudio se examinó un total de 383 suero, que esperábamos que dieran positivo para ANCA. Estas muestras fueron evaluadas en paralelo con el kit ANCA de Immuno Concepts y otro kit comercializado. PREPARACIÓN DEL TAMPÓN (PBS) Disuelva el contenido de una bolsa de tampón en un litro de agua desionizada o destilada. Tape y conserve en refrigerador a 2-10 ºC durante 4 semanas como máximo, o hasta que aparezcan signos de contaminación u otras alteraciones visibles. 2. DILUCIÓN DE LAS MUESTRAS DE PACIENTE Selección: Disuelva las muestras de pacientes a 1:20 añadiendo 0,05 ml (50 µl) de suero a 0,95 ml (950 µl) de disolvente de muestras. Titulación semicuantitativa: Para preparar series de diluciones dobles de las muestras de selección (p. ej., 1:20, 1:40, 1:80...1:640), saque 0,5 ml de la dilución 1:20 y mézclelos con 0,5 ml de disolvente de muestras para obtener una dilución 1:40, y así para las sucesivas diluciones. 3. DILUCIÓN DEL CONTROL TITULABLE OPCIONAL Considere que el control titulable opcional es una muestra de paciente no diluida. Diluya el control a 1:20 añadiendo 0,05 ml (50 µl) del suero de control a 0,95 ml de disolvente de muestras. Prepare diluciones dobles del control titulable como se ha descrito. 4. PREPARACIÓN DE LOS PORTAOBJETOS CON SUSTRATO (20-25 µl/pocillo) Saque los portaobjetos de las bolsas y disponga los sueros de control en los pocillos de control, como se indica: Invierta el frasco cuentagotas de control y apriete hasta que se vea una gota en la punta. Toque suavemente el pocillo de control con la gota, evitando que la punta del cuentagotas entre en contacto directo con la superficie del portaobjetos. Coloque el control positivo pANCA en el pocillo designado “pANCA”, el control positivo cANCA en el pocillo designado “cANCA”, el control negativo en el pocillo designado “Neg” y una gota de disolvente de muestras en el pocillo designado “Blank”. Añada una gota (20-25 µl) de muestra de paciente diluida a los pocillos numerados. PRECAUCIÓN: SI LA PUNTA DEL CUENTAGOTAS TOCA DIRECTAMENTE LA SUPERFICIE DEL PORTAOBJETOS, SE PUEDE DAÑAR EL SUSTRATO DE ANTÍGENO. 5. INCUBACIÓN DE LOS PORTAOBJETOS (30±5 minutos a temperatura ambiente, es decir, entre 18 y 24 °C) Ponga los portaobjetos en una cámara cubierta y húmeda (bastará con una placa de Petri con una toalla de papel humedecida). Incube con la tapa puesta durante 30 minutos (±5 minutos) a temperatura ambiente (18-24 ºC). 6. ENJUAGUE CON PBS Saque los portaobjetos de la bandeja de la incubadora y aclárelos un poco con PBS utilizando una jeringa o una pipeta de Pasteur o serológica. No utilice la jeringa directamente sobre los pocillos. REACTIVO FLUORESCENTE PARA DETECTAR ANTICUERPOS (cubra los pocillos con 10-12 gotas) Saque los portaobjetos del PBS de uno en uno y sumérjalos 3-5 veces en agua desionizada o destilada. Pase un papel secante o una toalla de papel por el borde del portaobjetos para retirar el agua sobrante. Vuelva a colocar de inmediato el portaobjetos en la cámara de incubación y cubra los pocillos por completo con el reactivo fluorescente para detectar anticuerpos; empiece por poner una gota en cada pocillo. Repita la operación en cada portaobjetos. El reactivo fluorescente para detectar anticuerpos ha sido titulado para compensar el agua desionizada o destilada residual que queda en el portaobjetos tras el enjuague. NOTA: Es importante que los pocillos del portaobjetos no se sequen durante este procedimiento, pues se podría dañar el sustrato. NO SEQUE EL PORTAOBJETOS NI DEJE QUE PASEN MÁS DE 15 SEGUNDOS SIN AÑADIR EL REACTIVO. 9. INCUBACIÓN DE LOS PORTAOBJETOS (30±5 minutos a temperatura ambiente, es decir, entre 18 y 24 °C) Ponga la tapa de la cámara de incubación y cúbrala con una toalla de papel para impedir la exposición a la luz, si la cámara no es opaca. Deje que los portaobjetos se incuben 30 minutos (±5 minutos) a temperatura ambiente (18-24 ºC). 10. ENJUAGUE CON PBS Retire los portaobjetos de la bandeja de la incubadora y enjuáguelos un poco con PBS. No utilice la jeringa directamente sobre los pocillos. 11. Coloque el cubreobjetos con cuidado, evitando que se formen bolsas de aire, haciendo bajar suavemente el cubreobjetos de un lado del portaobjetos al otro. NOTA: Si pone demasiado medio de preparación puede que la fluorescencia de fondo sea elevada debido a la diseminación de la luz, o que la resolución de las células no sea clara (imagen borrosa). Si ha puesto demasiado medio de preparación, puede retirarlo del portaobjetos secando suavemente el cubreobjetos con papel secante o para lentes, evitando cualquier movimiento directo del cubreobjetos. ASISTENCIA TÉCNICA: 916-363-2649 o correo electrónico: [email protected] 21 ANCA TESTSYSTEM NUR ZUR IN VITRO-DIAGNOSTIK ZUSAMMENFASSENDE ERKLÄRUNG DES TESTS Indikation: Dies ist ein indirekter Fluoreszenz-Antikörpertest für den semiquantitativen Nachweis von antineutrophilen Zytoplasma-Antikörpern (ANCA) in humanem Serum. Der Test dient als Hilfestellung beim Nachweis von Antikörpern, die mit Autoimmun-Vaskulitis in Zusammenhang gebracht werden. Antineutrophile Zytoplasma-Autoantikörper (ANCA) zählen zu einer Gruppe von Antikörpern, die mit Zytoplasma-Antigenen in Human-Neutrophilen reagieren. Obwohl über diese Antikörper bereits 1964 berichtet wurde (1), wurde eine Verbindung zu Krankheiten erst 1982 hergestellt, als Davies et al über die Antikörper bei acht Patienten mit segmentaler nekrotisierender Glomerulonephritis berichteten (2). 1984 wurde über vier weitere Patienten mit Vaskulitis und Glomerulonephritis berichtet. 1985 wiesen van der Woude et al den starken Zusammenhang zwischen ANCA und WegenerGranulomatose nach und zeigten die Korrelation der Krankheitsaktivität mit der Antikörpertitrierung auf (3). 1988 berichteten Falk und Jennette, dass ANCA mehr als eine Antigen-Spezifität aufweisen (4). Eine nachfolgende Untersuchung hat gezeigt, dass die Spezifität von ANCA mit den pathologischen Ausprägungen von Vaskulitiden korreliert (5). Im Immunofluoreszenz-Test für ANCA sind verschiedene Muster der Zellverfärbung zu sehen. Zwei wesentliche Verfärbungsmuster und deren Eigenschaften wurden beschrieben; dazu wurden im ImmunofluoreszenzANCA-Test Äthanol-fixierte Neutrophile verwendet. Autoantikörper, die ein feines Granularmuster des Zytoplasma aufweisen, werden cANCA genannt und sind im Allgemeinen gegen eine Serin-Protease, Proteinase 3 (PR-3), gerichtet. Diese Autoantikörper wiesen einen starken Zusammenhang mit Wegener-Granulomatose auf. Das andere wichtige Verfärbungsmuster, das perinukleäre bzw. pANCA-Muster, das in der Regel auf gegen Myeloperoxidase (MPO) gerichtete Antikörper zurückzuführen ist, wurde mit systemischer Vaskulitis und idiopathischer nekrotisierender und anwachsender Glomerulonephritis in Zusammenhang gebracht (4). Bei dem pANCA-Muster handelt es sich um einen Artifakt, hervorgerufen durch die Verwendung von Äthanol als Fixativ (4). Wenn die Neutrophile in Formalin fixiert sind, bleibt die Verbindung zwischen Myeloperoxidase (das für das pANCA-Muster in Äthanol-fixierten Zellen hauptverantwortliche Antigen) und den primären (alpha) Granularen, die durch eine Granularverteilung des Zytoplasma gekennzeichnet ist. Proteinase-3 (PR-3) bleibt mit den primären (alpha) Granularen in Äthanolund Formalin-Fixativen verbunden. Aufbewahrung: Alle Komponenten können gekühlt bei 2-10 °C aufbewahrt werden. Nach Rekonstitution sollte PBS-Puffer in Behältern mit Schraubverschluss bei 2-10 °C bis zu vier Wochen aufbewahrt werden, bis Anzeichen von Kontamination oder andere sichtbare Veränderungen auftreten. Stabilität: Alle Komponenten sind bis mindestens 12 Monate nach Herstellungsdatum stabil. Keine Komponenten nach Überschreiten des Verfallsdatums verwenden. REAKTIVE REAGENZIEN ANCA Substratträger:* Katalognummer 10010-11 (Äthanol-fixiert) oder 10010-12 (Formalin-fixiert). Objektträger für zehn Vertiefungen, die Human-Neutrophile enthalten, welche direkt auf den Testvertiefungen fixiert sind. Durch eine spezielle Konstruktion wird die Gefahr der Kreuzkontamination der Vertiefungen bei den Tests minimiert. Die Objektträger beinhalten vier Kontrollvertiefungen für pANCA, cANCA, negative und Leerwerte zur Erleichterung der korrekten Interpretation der Ergebnisse. *Dieser Objektträger ist durch mindestens eines der folgenden US- und anderen Patente geschützt: 4387972, D-274261, D-273261, Patent 1171302 in Kanada, andere Patente angemeldet. Dieses Reagens enthält 0,1 % Natriumazid als Konservierungsmittel. cANCA Titrierbare Kontrolle: Katalognummer 1002612. Gebrauchsfertiger Tropfer mit 0,25 ml positivem cANCA Human-Kontrollserum in flüssiger stabiler Form. Dieses Reagens enthält 0,1 % Natriumazid als Konservierungsmittel. Diese Kontrolle ist als unverdünnte Patientenprobe zu behandeln. Siehe Etikett für mittleren Titrierungsbereich. TESTPRINZIP Das antineutrophile Zytoplasma-Autoantikörper Testsystem (ANCA) von Immuno Concepts arbeitet mit der Methode der indirekten Fluoreszenz-Antikörper (IFA), wie sie von Weller und Coons zuerst beschrieben wurde (6). Verdünnte Patientenproben werden mit HumanNeutrophilen verdünnt, die auf Glas oder MikroskopObjektträgern fixiert sind, um spezifische Bindungen von ANCA zu ermöglichen. Falls ANCA vorhanden sind, werden die Autoantikörper an neutrophile Antigene gebunden. Nach dem Waschen, bei dem nicht-spezifische Antikörper entfernt werden, wird das Substrat mit einem anti-Human-IgG, das an Fluoreszein konjugiert ist, inkubiert. Bei positivem Ergebnis bildet sich ein stabiler dreiteiliger Komplex, bestehend aus an humane ANCA gebundenen anti-Human-Fluoreszenz-Antikörpern, welche ihrerseits an Antigen in den Zellen gebunden sind. Dieser Komplex kann mit Hilfe eines Fluoreszenz-Mikroskops sichtbar gemacht werden. Für cANCA positive Proben weisen eine deutliche Granular-Zytoplasmaverfärbung der Neutrophile auf Äthanol- und Formalin-fixierten Objektträgern auf. In für pANCA positiven Proben ist eine diffuse oder periphere Zellkernverfärbung des Neutrophils auf Äthanol-fixierten Zellen zu sehen und auf Formalin-fixierten Zellen ist eine Granularverfärbung des Zytoplasma zu sehen. Wenn die Probe für ANCA negativ ist, zeigen die Neutrophile keine spezifische Verfärbung. pANCA Titrierbare Kontrolle: Katalognummer 1002611. Gebrauchsfertiger Tropfer mit 0,25 ml positivem pANCA Human-Kontrollserum in flüssiger stabiler Form. Dieses Reagens enthält 0,1 % Natriumazid als Konservierungsmittel. Diese Kontrolle ist als unverdünnte Patientenprobe zu behandeln. Siehe Etikett für mittleren Titrierungsbereich. Negativkontrolle: Katalognummer 10031. Gebrauchsfertiger Tropfer mit 1,0 ml negativem ANCA Human-Kontrollserum. Dieses Serum weist eine wenig ausgeprägte, nichtspezifische mattgrüne Fluoreszenz der Neutrophile auf. Dieses Reagens enthält 0,1 % Natriumazid als Konservierungsmittel. Fluoreszenz-Antikörper-Reagens – spezifisch für IgG: Katalognummer 10009 (9ml). Anti-Human-IgG (Ziege), an Fluoreszein-Isothiocyanat (FITC) konjugiert. Dieses Reagens enthält Evans Blau als Gegenfärbung. Das Reagens wird gebrauchsfertig in Präzisionstropffläschchen geliefert. Dieses Reagens enthält 0,1 % Natriumazid als Konservierungsmittel. NICHT-REAKTIVE REAGENZIEN THALTEN) PBS-Puffer: Katalognummer 1011. Phosphat-gepuffertes Kochsalzpulver (0,01 M, pH 7,4 ± 0,2). Jeder Beutelinhalt reicht für 1 Liter gebrauchsfertigen Puffer. (Ein Beutel Pufferpulver für je fünf Objektträger ist im Lieferumfang des Testkits enthalten). Verwendung: Sämtliche Komponenten werden gebrauchsfertig geliefert, ohne dass Aliquotieren oder eine Rekonstitution erforderlich sind (mit Ausnahme des PBS-Puffers, der vor Gebrauch in deionisiertem oder destilliertem Wasser aufgelöst werden muss). Herstellung: Einen Beutel Pulver in 1 Liter deionisiertem oder destilliertem Wasser auflösen und gekühlt bei 2-10 °C bis zu 4 Wochen aufbewahren, oder bis Anzeichen von Kontamination oder andere sichtbare Veränderungen zu erkennen sind. Fluoreszenz-Mikroskop mit 495 nm Erregerfilter und 515 nm Sperrfilter. SICHERHEITSHINWEISE 1. Sämtliche zur Herstellung von Kontrollseren für dieses Produkt verwendeten Materialien humanen Ursprungs wurden nach von der FDA anerkannten Methoden negativ (nicht wiederholt reaktiv) auf Antikörper gegen HIV-1, HIV-2, Hepatitis-C (HCV) und Hepatitis-B-Oberflächenantigen (HBsAG) getestet. Keine Testmethode kann jedoch mit absoluter Sicherheit nachweisen, dass keine HIV-1, HIV-2, Hepatitis-C oder Hepatitis-B-Viren oder andere infektiöse Agenten vorhanden sind. Daher sollten alle Kontrollseren wie potenziell infektiöse Materialien gehandhabt werden. 2. Alle Patientenproben sollten nach den Anforderungen für Biosafety Level 2 behandelt werden, wie für potenziell infektiöses humanes Serum und andere Blutbestandteile empfohlen in: Centers for Disease Control / National Institutes of Health Manual: Biosafety in Microbiological and Biomedical Laboratories, Ausgabe 1984. 3. Ein Verdünnen der Bestandteile oder eine Zugabe von nicht zum Kit gehörenden Reagenzien kann die Qualität der Ergebnisse beeinträchtigen. 4. Einige Reagenzien enthalten Natriumazid (0,1 %) als Konservierungmittel. Natriumazid kann mit Blei- oder Kupferinstallationen reagieren und hochexplosive Metallazidsalze bilden. Beim Entsorgen der Reagenzien mit reichlich Leitungswasser nachspülen, damit im Abfluss keine Rückstände verbleiben. Natriumazid ist giftig und kann bei Verschlucken toxisch wirken. 5. Der Kit ist ausschließlich zur In vitro-Diagnostik bestimmt. 6. Das titrierbare Kontrollserum ist für die Überwachung der Chargen- und Testlauf-übergreifenden Reproduzierbarkeit bestimmt. Es ist nicht zur Messung der Gesamtsensibilität oder Spezifität des Tests bestimmt. 7. In Bereichen, in denen mit Patientenproben oder Kitreagenzien gearbeitet wird, nicht rauchen, essen oder trinken. 8. Verspritzen von Reagenzien und Erzeugung von Aerosolen vermeiden. 9. Die angegebenen Inkubationszeiten und Temperaturwerte genau einhalten, andernfalls könnten die Ergebnisse verfälscht werden. 10. Eine Kreuzkontamination der Reagenzien oder Proben kann ebenfalls zu falschen Ergebnissen führen. 11. Wiederverwendbare Glasartikel müssen vor Gebrauch gewaschen und gründlich ausgespült werden, um sämtliche Waschmittelrückstände zu entfernen. Die Glasartikel müssen vor Gebrauch sauber und trocken sein. 12. Vor der Testdurchführung müssen alle Reagenzien, Objektträger und Proben auf Zimmertemperatur (18-24 °C) gebracht werden. 13. Beim Arbeiten mit Proben und Reagenzien sind grundsätzlich Einmal-Latexhandschuhe zu tragen. Danach gründlich Hände waschen. 14. Eine mikrobielle Kontamination der Reagenzien oder Proben kann das Ergebnis verfälschen. 15. Niemals mit dem Mund pipettieren und Kontakt der Reagenzien und Proben mit Haut und Schleimhäuten vermeiden. Bei Kontakt mit viel Wasser und desinfizierender Seife waschen. PROBENGEWINNUNG Probenentnahme: Nach Möglichkeit sollten Serumproben hergestellt werden. Dazu durch Venenpunktion in ein steriles Vakuumröhrchen oder durch ein anderes geeignetes Blutentnahmesystem aseptisch ca. 5 ml Vollblut entnehmen. Das Blut bei Zimmertemperatur (18-24 °C) gerinnen lassen. Das Serum muss so bald wie möglich abgetrennt werden, um Hämolyse zu vermeiden. Störsubstanzen: Stark hämolytische, lipämische oder durch Mikrobenwachstum verunreinigte Seren sowie Seren von Ikteruspatienten dürfen nicht verwendet werden, weil bei positiven Proben eine Abnahme der Antikörper-Titrierungen auftreten könnte. Proben mit hohen Lipidanteilen können einen nicht-spezifischen fluoreszenten Film über dem Zellsubstrat bilden. Die Verwendung von Fixierungs- oder Reinigungsproben durch Ultrazentrifugieren kann dieses Problem beheben. Proben mit sichtbaren Verunreinigungen müssen vor Verwendung zentrifugiert werden. Aufbewahrung: Serumproben können bei einer Temperatur von 2-10 °C maximal eine Woche lang aufbewahrt werden. Sollen die Proben länger aufbewahrt werden, müssen sie bei mindestens –20 °C eingefroren werden. Serum darf nicht in einem Gefrierschrank mit Abtauautomatik gelagert werden. ACHTUNG: Wiederholtes Einfrieren / Auftauen von Patientenproben ist zu vermeiden. Andernfalls können falsch-positive oder falsch-negative Ergebnisse auftreten. INTERPRETATION DER ERGEBNISSE QUALITÄTSSICHERUNG In den für die Qualitätssicherung vorgesehenen Vertiefungen sollten für jeden Objektträger Positiv-, Negativ- und Leer-Kontrollläufe durchgeführt werden. Die cANCA Positivkontrolle sollte zwischen den Kernsegmenten auf Äthanol- oder Formalin-fixierten Objektträgern eine helle apfelgrüne fluoreszierende GranularZytoplasmaverfärbung im Neutrophil aufweisen. Die pANCA Positivkontrolle sollte eine helle apfelgrüne diffuse oder periphere Zellkernverfärbung des Neutrophils auf Äthanol-fixierten Objektträgern aufweisen und eine Granular-Fluoreszenz des Zytoplasma auf Formalin-fixierten Objektträgern. Die Negativkontrolle sollte keine hellgrüne Fluoreszenz aufweisen. In der Negativkontrolle ist u.U. eine nicht-spezifische mattgrüne Fluoreszenz von geringer Intensität festzustellen. Die leere Kontrollvertiefung wird zur Beobachtung nicht-spezifischer Verfärbungen durch das Antikörper-Reagens verwendet und sollte keine grüne Fluoreszenz aufweisen. Die Gegenfärbung des Konjugats kann eine mattrote Verfärbung der Zellen hervorrufen. Wenn die Kontrollvertiefungen nicht die beschriebenen Merkmale aufweisen, ist der Test ungültig und muss wiederholt werden. OPTIONALE TITRIERBARE KONTROLLE Beim Auswerten der Titrierungen beginnen viele Labore mit der Vertiefung, die die Probe mit der größten Verdünnung enthält, und fahren mit der Auswertung „nach hinten” zur 1:20-Verdünnung fort. Die erste Vertiefung, in der eine deutliche Verfärbung des Zytoplasma sichtbar ist, ist der Titrierungsendpunkt. Wir empfehlen diese Vorgehensweise zur Bestimmung des Titrierungsendpunkts. In unserem Labor wurde die mittlere Titrierung und der mittlere Titrierungsbereich (± eine Verdünnung nach oben oder unten, vom Mittel ausgehend) für diese Chargennummer festgelegt; dieser gilt als Richtwert. Mit Hilfe dieser Kontrolle kann jedes Labor die Reproduzierbarkeit (Präzision) seiner ANCA-Tests selbst einschätzen. Da diese Kontrolle nicht als Indikator für die Titrierungsgenauigkeit vorgesehen ist, muss jedes Labor seinen eigenen mittleren Titrierungsendpunkt für diese Probe festlegen und sollte diese Informationen zur Bewertung der Reproduzierbarkeit (Präzision) ihrer Testläufe heranziehen. Für jede Chargennummer wurde durch mehrere Testläufe mit dieser titrierbaren Kontrolle, unter Verwendung des ANCA-Fluoreszenz-Testsystems von Immuno Concepts, ein mittlerer Titrierungswert ermittelt. Chargennummer, mittlere Titrierung und Titrierungsbereich (± eine Zweifach-Verdünnung nach oben oder unten, ausgehend vom Mittel) sind auf dem Etikett des Fläschchens verzeichnet und sollten zur Messung der Systemleistung herangezogen werden. Die in unserem Labor ermittelten Werte können von Ihren eigenen Werten abweichen. Zahlreiche Faktoren 23 können Einfluss auf Ihre Ergebnisse nehmen; hier einige Beispiele: lin-fixierten Neutrophilen zu sehen. 1. Die Art der verwendeten Lichtquelle: Quecksilber-Lichtquellen produzieren bei 495 nm mehr Erregerenergie als Quarz/Halogen. QuecksilberLichtquellen mit 50 Watt, 100 Watt und 200 Watt weisen bei 495 nm nur geringe Unterschiede in der Erregerenergie auf. Quarz/Halogen-Lichtquellen mit 100 Watt erzeugen bei 495 nm mehr Erregerenergie als solche mit 50 Watt. 2. Zustand und Alter der Lichtquelle: Dies gilt besonders für Quecksilber-Lichtquellen, bei denen in der Regel vor dem Durchbrennen eine allmähliche Reduzierung der Erregerenergie bei 495 nm zu beobachten ist. Dieser allmähliche Rückgang der Erregerenergie kann im Verlauf mehrerer Wochen zu einem deutlichen Sensibilitätsverlust führen. Das Problem kann durch Führen eines Lampenprotokolls vermieden werden. Für beste Ergebnisse empfiehlt es sich, 50 Watt-Quecksilberbirnen nach 100 Stunden und 100 oder 200 Watt-Quecksilberbirnen nach 200 Stunden auszuwechseln. Bei Quarz/Halogen-Lichtquellen tritt in der Regel vor dem Durchbrennen keine allmähliche Reduzierung der Erregerenergie auf. 3. Die Art des verwendeten Erregerfilters: InterferenzErregerfilter bieten höhere Sensibilität über eine viel schmalere Wellenlänge als Absorptions-Erregerfilter. Präzision und Genauigkeit der Verdünnungstechnik, der Ausrüstung und der Ausführung der Testverfahren. INTERPRETATION DER PATIENTENERGEBNISSE pANCA (perinukleäre Verfärbung): Glatte oder homogene Verfärbung des mehrlappigen Nukleus, oft mit deutlicher peripherer Verfärbung der Nukleus-Lappen auf Äthanol-fixierten Neutrophilen. Auf Formalin-fixierten Neutrophilen weisen diese Antikörper eine Granularfärbung des Zytoplasma auf. Antinukleäre und andere Autoantikörper können mit den Neutrophilen reagieren; es sollten deshalb alle positiven Proben mit einem Hep-2 Zellsubstrat auf ANA getestet werden, bevor das Ergebnis als Vorhandensein von ANCA gewertet wird. Optionale Titrierung: Die Ergebnisse sind als reziprok zur Verdünnung der letzten Vertiefung, in der eine deutliche positive Reaktion sichtbar ist, anzugeben. EINSCHRÄNKUNGEN DES TESTS 1. Es ist nicht möglich, lediglich auf der Basis des Nachweises antineutrophiler Zytoplasma-Antikörper eine Diagnose zu stellen. Der Arzt muss die Ergebnisse im Zusammenhang mit Krankengeschichte und Symptomen des Patienten, den Ergebnissen der körperlichen Untersuchung und anderen diagnostischen Methoden interpretieren. 2. Lediglich auf Grund eines positiven Testergebnisses für antineutrophile Zytoplasma-Antikörper mit diesem Test sollte keine Behandlung initiiert werden. Für eine Behandlung müssen auch klinische Symptome, andere Laborergebnisse und der Gesamteindruck des Patienten auf den behandelnden Arzt herangezogen werden. 3. In Patientenproben vorhandene antinukleäre Antikörper können mit dem Zellsubstrat reagieren. Wenn antinukleäre Antikörper vorhanden sind, können die ANCA-Ergebnisse oft aufgrund unterschiedlicher Eigenschaften der spezifischen ANCA-Verfärbung, unterschiedlicher Titrierungen und/oder Reaktionsintensität interpretiert werden. Falls die FluoreszenzReaktion antinukleärer Antikörper so stark ist, dass die ANCA-Verfärbung überdeckt wird, sollte der Test als „nicht interpretierbar“ gewertet und eine alternative Methode zur Bestimmung des ANCAStatus des Patienten gewählt werden. 4. Wegen der zahlreichen für Fluoreszenz-Mikroskope verfügbaren Optionen empfehlen wir, bei Vergleichen von Patienten-Titrierungen zwischen verschiedenen Laboren mit standardisierten Lichtquellen, Filtern und optischen Geräten zu arbeiten. 5. Um den klinischen Zustand des Patienten zu bestimmen, sollten die Ergebnisse des Tests im Zusammenhang mit den übrigen verfügbaren Informationen zur klinischen Einschätzung und anderen diagnostischen Methoden interpretiert werden. ZU ERWARTENDE WERTE Zum Betrachten der Neutrophile wird eine 400-fache Vergrößerung empfohlen. Der zu erwartende Wert in der normalen Population ist negativ bei einer Screening-Verdünnung von 1:20. Bei Patienten mit vorhandenen Erkrankungen ergaben sich Titrierungen von bis zu 1:640 (7). Negative Reaktion: Ein Serum ist als negativ für Antikörper gegen ANCA zu werten, wenn bei einer Verdünnung von 1:20 die Zell-Fluoreszenz geringer als oder gleich der negativen Kontrollvertiefung ist. In den Neutrophilen ist u.U. eine nicht-spezifische Hintergrundverfärbung, bedingt durch heterophile Antikörper oder Autoantikörper, zu beobachten. LEISTUNGSFÄHIGKEIT DES TESTS NORMALE PROBEN Positive Reaktion: Ein Serum ist als positiv für ANCA zu werten, wenn bei einer Verdünnung von 1:20 die Zellen in jedem Feld eine Granular-Fluoreszenz des Zytoplasma aufweisen, die der cANCA-Kontrolle auf Äthanol- oder Formalin-fixierten Objektträgern ähnelt. Alternativ ist ein Serum als positiv für ANCA zu werten, wenn bei einer Verdünnung von 1:20 die Zellen eine diffuse oder periphere Zellkern-Fluoreszenz aufweisen, die der pANCAKontrolle auf Äthanol-fixierten Objektträgern ähnelt. AUSWERTUNG DER ERGEBNISSE Screening: Die Ergebnisse des Tests sind als positiv oder negativ bei 1:20-Verdünnung anzugeben. Serumproben von 497 Blutspendern (247 Männer und 250 Frauen) wurden parallel mit dem Immuno Concepts ANCA Kit und einem anderen im Handel erhältlichen Kit getestet. Alle Proben dieser Population, die auf Äthanol-fixierten Objektträgern positiv waren, wurden außerdem mit dem HEp-2 ANA Testsystem von Immuno Concepts auf antinukleäre Antikörper (ANA) getestet. Unter den normalen Proben waren 22 Proben, die für ANA positiv und als nicht interpretierbar für ANCA zu werten waren. Die übrigen Proben mit Diskrepanzen wurden auf Antikörper gegen MPO und PR3 mithilfe der ELISA-Tests zur Bestimmung des tatsächlichen Antikörper-Status getestet. PUFFER (PBS) REKONSTITUIEREN Inhalt eines Beutels mit Pufferpulver in einem Liter deionisiertem oder destilliertem Wasser auflösen. Abdecken und bei 2-10 °C bis zu vier Wochen aufbewahren, oder bis Anzeichen von Kontamination oder andere sichtbare Veränderungen zu erkennen sind. 2. Patientenproben verdünnen Screening: Patientenproben durch Zugabe von 0,05 ml (50 µl) Patientenserum zu 0,95 ml (950 µl) Probenverdünner 1:20 verdünnen. Semiquantitative Titrierung: Zur Herstellung von zweifachen Reihenverdünnungen der Screening-Proben (z.B. 1:40, 1:80, 1:160...1: 640), 0,5 ml von der 1:20 Verdünnung trennen und mit 0,5 ml Probenverdünnung mischen, um eine Verdünnung von 1:40 zu erzielen, anschließend auf diese Weise weitere Reihenverdünnungen herstellen. 3. VERDÜNNEN DES OPTIONALEN TITRIERBAREN KONTROLLSERUMS Die optionale titrierbare Kontrolle als unverdünnte Patientenprobe behandeln. Das Kontrollserum 1:20 verdünnen, indem 0,05 ml (50 µl) Kontrollserum zu 0,95 ml Probenverdünnung zugegeben werden. Zweifache Reihenverdünnungen des titrierbaren Kontrollserums wie oben beschrieben herstellen. 4. SUBSTRATTRÄGER VORBEREITEN (20-25 µl/Vertiefung) Objektträger aus der Folie entnehmen und Kontrollseren wie folgt in den Kontrollvertiefungen platzieren: Tropfflasche mit Kontrollserum umdrehen und vorsichtig drücken, bis an der Spitze ein Tropfen austritt. Den Tropfen vorsichtig in die entsprechende Kontrollvertiefung geben, dabei direkten Kontakt der Tropferspitze mit der Oberfläche des Objektträgers vermeiden. Die pANCA Positivkontrolle in die mit „pANCA” markierte Vertiefung geben, die cANCA Positivkontrolle in die mit „cANCA” markierte Vertiefung, die Negativkontrolle in die mit „Neg“ markierte Vertiefung geben und einen Tropfen Probenverdünnung in die mit „Blank” markierte Vertiefung geben. 1 Tropfen (20-25 µl) verdünnter Patientenprobe in die nummerierten Vertiefungen geben. ACHTUNG: DIREKTER KONTAKT DER TROPFERSPITZE MIT DEM OBJEKTTRÄGER KANN DAS ANTIGEN-SUBSTRAT BESCHÄDIGEN. 5. OBJEKTTRÄGER INKUBIEREN (30 ± 5 Minuten bei Zimmertemperatur, 18-24 °C) Objektträger in eine feuchte, bedeckte Schale legen (z. B. Petrischale mit angefeuchtetem Papierhandtuch). 30 Minuten (±5 Minuten) bei Zimmertemperatur (18-24 °C) abgedeckt inkubieren lassen. 6. PBS-SPÜLUNG Objektträger aus der Inkubationsschale nehmen und mit Spritzflasche, Pasteur- oder serologischer Pipette kurz mit PBS spülen. Den Puffer nicht direkt in die Vertiefungen einspritzen. HINWEIS: Um bei Objektträgern mit 10 Vertiefungen eine Kreuzkontamination zu vermeiden, den PBS auf die Mittellinie des Objektträgers spritzen, dabei den Träger zuerst in Richtung Vertiefungen 1-3 neigen, anschließend in Richtung Vertiefungen 4-6. 7. PBS-WASCHVORGANG (10 Minuten) Objektträger in einem entsprechenden Gefäß 10 Minuten mit PBS waschen (Glaszylinder, Schale). Beim Eintauchen, in der Mitte des Waschvorgangs und beim Herausnehmen leicht hin- und herbewegen. Dieser Waschvorgang kann ohne Auswirkung auf die Testergebnisse auf bis zu 30 Minuten ausgedehnt werden. PBS-Waschlösung nach Gebrauch entsorgen. Für optimale Ergebnisse sollte nach Ablauf der halben Waschzeit der PBS-Waschpuffer gewechselt und ein magnetischer Rührer verwendet werden. 26 8. FLUORESZENZ-ANTIKÖRPERREAGENS (Vertiefungen mit 10-12 Tropfen befüllen) Jeweils einen Objektträger aus dem PBSPuffer nehmen und 3-5 Mal in deionisiertes oder destilliertes Wasser tauchen. Überschüssiges Wasser durch seitliches Ausklopfen auf Saugpapier oder Papierhandtuch entfernen. Objektträger sofort wieder in die Inkubationskammer geben und die Vertiefungen vollständig mit FluoreszenzAntikörperreagens füllen; beginnend mit einem Tropfen pro Vertiefung. Vorgang für alle Objektträger wiederholen. Das Fluoreszenz-Antikörperreagens wurde titriert, um für auf dem Objektträger verbliebenes deionisiertes oder destilliertes Wasser zu kompensieren. HINWEIS: Es ist wichtig, dass die Vertiefungen auf dem Objektträger während dieses Vorgangs nicht austrocknen, andernfalls kann das Substrat beschädigt werden. DEN OBJEKTTRÄGER NICHT TROCKEN TUPFEN ODER WISCHEN, ODER LÄNGER ALS 15 SEKUNDEN OHNE FLUORESZENZ-ANTIKÖRPERREAGENS STEHEN LASSEN. 9. OBJEKTTRÄGER INKUBIEREN (30±5 Minuten bei Zimmertemperatur, 18-24 °C) Deckel auf Inkubationskammer setzen und mit Papierhandtuch abdecken, um das Eindringen von Licht zu vermeiden, falls die Kammer durchsichtig ist. Objektträger 30 Minuten (±5 Minuten) bei Zimmertemperatur (18-24 °C) inkubieren lassen. 10. PBS-SPÜLUNG Objektträger aus der Inkubationsschale nehmen und kurz mit PBS spülen. Den Puffer nicht direkt in die Vertiefungen einspritzen. 11. PBS-WASCHVORGANG (10 Minuten) Objektträger in einem entsprechenden Gefäß 10 Minuten mit PBS waschen (Glaszylinder, Schale). Beim Eintauchen, in der Mitte des Waschvorgangs und beim Herausnehmen leicht hin- und herbewegen. Dieser Waschvorgang kann ohne Auswirkung auf die Testergebnisse auf bis zu 30 Minuten ausgedehnt werden. 12. DECKGLAS AUFSETZEN Jeweils einen Objektträger aus dem PBSPuffer nehmen und 3-5 Mal in deionisiertes oder destilliertes Wasser tauchen. Überschüssiges Wasser durch seitliches Ausklopfen auf Saugpapier oder Papierhandtuch entfernen. Testsystemet skall användas som hjälpmedel för detektion av antikroppar associerade med autoimmun vaskulit. Antineutrofila cytoplasmiska autoantikroppar (ANCA) är en grupp antikroppar som reagerar med cytoplasmiska antigener i humana neutrofiler. Även om dessa antikroppar ursprungligen rapporterades 1964 (1), associerades de med en sjukdom för första gången i en rapport 1982 när Davis o.a. rapporterade att antikropparna förekom hos åtta patienter med segmental nekrotiserande glomerulonefrit (2). 1984 rapporterades ytterligare fyra patienter med vaskulit och glomerulonefrit. 1985 visade van der Woude o.a. att ANCA är starkt förknippad med Wegeners granulomatos och att antikroppnivåerna står i relation till sjukdomens aktivitet (3) 1988 rapporterade Falk och Jennette att ANCA har mer än en antigenspecificitet (4). En efterföljande rapport visade att ANCA-specificiteten står i relation till de patologiska kännetecknen för vaskulitid (5). I det immunfluorescerande ANCA-testet kan flera cellformiga färgningsmönster noteras. Två betydande färgningsmönster har beskrivits och blivit väl karaktäriserade med användande av etanolbeständiga neutrofiler i det immunfluorescerande ANCA-testet. Autoantikroppar som uppvisar ett finkornigt cytoplasmiskt mönster (cANCA) riktas vanligtvis mot en serinproteas, proteinas 3 (PR-3). Dessa autoantikroppar har visat sig i hög grad vara förknippade med Wegeners granulomatos. Det andra betydande färgningsmönstret - det perinukleära mönstret, eller pANCA-mönstret - som vanligtvis beror på antikroppar mot myeloperoxidas (MPO) har associerats med systemisk vaskulit och idiopatisk nekrotiserande och halvmåneformad glomerulonefrit (4). pANCA-mönstret är en artefakt som uppkommit genom användning av etanol som fixeringsmedel (4). Proteinas-3 (PR-3) förblir associerad med primära (alfa-) korn i antingen etanol- eller formalinfixeringsmedel TESTPRINCIP Immuno Concepts antineutrofila cytoplasmiska autoantikroppanalys (ANCA) använder den indirekta fluorescerande antikroppteknik (IFA) som först beskrivits av Weller och Coons (6). Spädda patientprover odlas med humana neutrofiler som är fixerade på objektglas på glasmikroskop för att tillåta specifik ANCA-bindning. Om det förekommer ANCA binder autoantikropparna till de neutrofila antigenerna. Efter tvättning för att avlägsna ospecifika antikroppar odlas substratet med antihuman IgG konjugerad med fluoroscein. Om resultatet är positivt bildas ett stabilt komplex i tre delar bestående av en fluorescerande antikropp bunden till en human ANCA, som i sin tur är bunden till en antigen belägen i cellerna. Detta komplex kan studeras i fluorescerande mikroskop. Prover som är cANCA-positiva uppvisar en särskiljande kornig cytoplasmisk färgning av neutrofilerna på både etanol- och formalinbeständiga objektglas. I prover som är pANCA-positiva syns en diffus eller perifer nukleär färgning av neutrofilerna på etanolbeständiga celler och en kornig färgning av cytoplasman på formalinbeständiga celler. Om provet är ANCA-negativt syns ingen specifik färgning av neutrofilerna. SYSTEMKOMPONENTER (MATERIAL SOM MEDFÖLJER) Användning: Alla komponenter levereras bruksfärdiga utan krav på delning eller rekonstitution (förutom PBS-bufferten som måste lösas upp i avjoniserat eller destillerat vatten före användning). Allt material av humant ursprung som använts för att förbereda kontroller för denna produkt har testats med en FDA-godkänd metod och visat sig vara negativt (inte upprepat reaktivt) för antikroppar mot humant immunbristvirus-1, humant immunbristvirus-2 (HIV-1 och HIV-2), antikroppar mot hepatit C-virus (HCV) samt hepatit B ytantigen (HBsAg) . Ingen testmetod kan helt garantera att det inte förekommer HIV-1, HIV-2, hepatit-C-virus, hepatit-B-virus eller andra smittämnen. Därför skall allt kontrollsera hanteras som potentiellt smittsamt. 2. Alla patientprover på biosäkerhetsnivå 2 skall hanteras enligt rekommendationerna för potentiellt smittsamt humanserum eller blodprov i Centrum för Sjukdomskontroll/Nationella hälsoinstitutets manual: Biosäkerhet i mikrobiologiska och biomedicinska laboratorier, 1984 års upplaga. 3. Spädning av komponenter eller byte till andra komponenter än de som medföljer detta system kan ge motsägande resultat. 4. Natriumazid (0,1 %) används som konserveringsmedel. Natriumazid kan reagera med ledningsrör av bly eller koppar och bilda explosiva metallazidsalter. När reagenser kasseras skall man spola med rikliga mängder kranvatten för att skölja bort eventuella rester i avloppsledningarna. Natriumazid är ett gift och kan vara toxiskt vid förtäring. 5. Denna sats är avsedd för diagnostisk användning in vitro. 6. Det titrerbara kontrollserumet är avsett för användning vid övervakning av reproducerbarheten mellan olika loter eller serier. Det är inte avsett för mätning av den totala sensitiviteten eller analysens specificitet. 7. Undvik att röka, äta eller dricka i områden där prover eller satsreagenser hanteras. 8. Undvik alltid stänk och alstring av aerosoler. 9. Andra inkubationstider och temperaturer än de angivna kan ge felaktiga resultat. 10. Korskontamination mellan reagenser och prover kan ge felaktiga resultat. 11. Återanvändningsbart glas måste tvättas och noggrant sköljas från rengöringsmedel innan det används. Allt glas måste rengöras och torkas före användning. 12. Placera alla reagenser, objektglas och prov i rumstemperatur (18-24 °C) före användning. 13. Använd engångshandskar av latex vid hantering av prover och reagenser, och tvätta händerna noggrant efteråt. 14. Mikrobisk kontamination av reagenser eller prover kan ge felaktiga resultat. 15. Pipettera aldrig med munnen och undvik att komma i kontakt med reagenser och prov med hud eller slemhinnor. Tvätta med bakteriedödande tvål och rikligt med vatten om sådan kontakt inträffat. PROVTAGNING Provtagning: Serum rekommenderas som prov. Cirka 5 ml helblod skall tas aseptiskt genom venpunktion med hjälp av ett sterilt vakuumblodtagningsrör eller annat lämpligt blodtagningssystem. Låt blodet koagulera i rumstemperatur (18-24 °C). Serum skall så snart som möjligt separeras från koagler för att minimera hemolys. Störande substanser: Sera som uppvisar en hög grad av hemolys, ikterus, lipemi eller mikrobiell tillväxt skall inte användas, eftersom antikroppnivån kan minska i positiva prover. Prover med mycket höga lipidnivåer kan leda till att en ospecifik fluorescerande film bildas över cellsubstratet. Detta problem kan undvikas genom användning av prover som får fast form eller klar färg genom ultracentrifugering. Prover som innehåller synliga partiklar bör klargöras genom centrifugering före analysen. Förvaring: Sera kan förvaras i 2-10 °C under högst en vecka. Om analysen fördröjs ytterligare, skall sera frysas i –20 °C eller lägre. Serum bör inte förvaras i självavfrostande fryslagerrum. VARNING: Upprepad frysning/upptining av patientprover kan ge falskt positiva eller negativa resultat. 28 Positiva, negativa och färglösa kontroller bör köras i de brunnar som tillhandahålls för kvalitetskontroll på varje objektglas. Den cANCA-positiva kontrollen skall uppvisa ljust äppelgrön fluorescerande kornig cytoplasmisk färgning av neutrofilerna mellan de nukleära segmenten på antingen etanol- eller formalinbeständiga objektglas. Den pANCA-positiva kontrollen skall uppvisa ljust äppelgrön diffus eller perifer nukleär färgning av neutrofilerna på etanolbeständiga objektglas och kornig cytoplasmisk fluorescens på formalinbeständiga objektglas. Den negativa kontrollen skall inte uppvisa någon ljusgrön fluorescens. Den negativa kontrollen kan uppvisa låg intensitet och en matt grön fluorescens. Den färglösa kontrollen används för att observera ospecifik fluorescens av antikroppreagensen och bör inte uppvisa någon grön fluorescerande färgning. Den motfärg som ingår i konjugatet kan färga brunnarna i en matt röd färg. Om kontrollerna inte ser ut enligt beskrivningarna är testet ogiltigt och bör göras om. TILLHÖRANDE TITRERBAR KONTROLL Vid läsning av antikroppnivåerna börjar många laboratorier med att läsa den brunn som innehåller det mest spädda provet och läser „baklänges“ till spädningen 1: 20. Den första brunnen med klart urskiljbar cytoplasmisk färgning är antikroppnivåns ändpunkt. Vi rekommenderar denna teknik för att fastställa antikroppnivåns ändpunkter. Det medelvärde och spridningsområde för antikroppnivån (± en spädning på var sin sida om medelvärdet) som bestämts för varje lotnummer har fastställts i vårt laboratorium och uppges för vägledning. Denna kontroll tillhandahålls för att varje laboratorium skall ha tillgång till ANCA-testernas reproducerbarhet (precision). Genom upprepade analyser av denna titrerbara kontroll med användande av Immuno Concepts fluorescerande ANCA-testsystem har ett medelantikroppvärde fastställts för varje lotnummer. Lotnumret och antikroppnivåns medelvärde och spridningsområde för (± en dubbel spädning på vardera sidan om medelvärdet) står angivna på flaskans etikett och skall användas som vägledning för testsystemets prestanda. De värden som erhållits i vårt laboratorium kan skilja sig från era. Några av de faktorer som kan påverka resultaten kan vara, men är inte begränsat till: 1. Typ av ljuskälla: Ljuskällor av kvicksilver ger högre exciteringsenergi vid 495 nm än kvarts/halogen. Ljuskällor av kvicksilver på 50 watt, 100 watt och 200 watt skiljer sig något i exciteringsenergi vid 495 nm. Kvarts-/halogenljuskällor på 100 watt ger högre exciteringsenergi vid 495 nm än kvarts-/ halogenljuskällor på 50 watt. 2. Ljuskällans skick och ålder: Detta gäller framför allt ljuskällor av kvicksilver som i allmänhet uppvisar en gradvis minskning i exciteringsenergi vid 495 nm, innan de smälter ned. Denna gradvisa minskning i exciteringsenergi kan leda till en avsevärd förlust i känslighet över flera veckors tid. Detta problem kan undvikas genom att föra en tidsloggbok. För bästa resultat: Byt ut 50 watts glödlampor av kvicksilver efter 100 timmar och 100 eller 200 watts glödlampor av kvicksilver efter 200 timmar. Kvarts-/ halogenljuskällor uppvisar i allmänhet ingen gradvis minskning i exciteringsenergi, innan de smälter ned. 3. Vilken typ av matarfilter som används: Störningsmatarfilter ger större känslighet över en mycket smalare våglängd än absorptionsmatarfilter. Objektivets numeriska bländaröppning: Med infallande ljusfluorescens (Epi) ökar fluorescensen exponentiellt, medan den numeriska bländaröppningen (NA) ökar additivt. Detta kan göra att ett 40X-objektiv med en NA på 0,65 läser en eller flera spädningar lägre än ett 40X-objektiv med en NA på 0,85. Den numeriska bländaröppningen står angiven på sidan av objektivet. REKONSTITUTION AV BUFFERT (PBS) Lös upp innehållet i en buffertförpackning i 1 liter avjoniserat eller destillerat vatten. Täck och förvara i 2-10 °C i upp till fyra veckor eller tills det syns tecken på kontamination eller andra synliga förändringar. 2. SPÄDNING AV PATIENTPROV Screening: Späd patientprovet till 1:20 genom tillsättande av 0,05 ml (50 µl) serum till 0,95 ml (950 µl) provspädningsvätska. Semikvantitativ bestämning av antikroppnivå: För att framställa dubbla seriespädningar av screeningprover (t ex 1:40, 1:80, 1:160…1:640, avlägsnas 0,5 ml av spädningen 1:20 och blandas med 0,5 ml provspädningsvätska för att uppnå spädningen 1:40. Fortsätt därefter med seriespädningarna på detta sätt. 3. SPÄDNING AV TILLHÖRANDE TITRERBAR KONTROLL Behandla den tillhörande titrerbara kontrollen som ett outspätt patientprov. Späd kontrollen 1:20 genom att tillsätta 0,05 ml (50 µl) kontrollserum till 0,95 ml provspädningsvätska. Framställ dubbla spädningar av den titrerbara kontrollen (se skiss nedan). 4. IORDNINGSTÄLLANDE AV OBJEKTGLAS FÖR SUBSTRAT (20-25 µl/brunn) Avlägsna objektglaset/objektglasen från påsen/påsarna och placera kontrollsera på kontrollserabrunnar enligt följande: Vänd upp och ned på pipettflaskan och kläm försiktigt tills det syns en droppe på spetsen. För försiktigt droppen mot rätt kontrollbrunn, men undvik direktkontakt mellan pipettspetsen och objektglasets yta. Placera pANCA positiv kontroll i brunnen markerad „pANCA“, cANCA positiv kontroll i brunnen markerad „cANCA“, negativ kontroll i brunnen markerad „Neg“ samt en droppe provspädningsvätska i brunnen markerad „färglös“. Tillsätt 1 droppe (20-25 µl) utspätt patientprov i de numrerade brunnarna. VARNING: DIREKTKONTAKT MELLAN PIPETTSPETSEN OCH OBJEKTGLASETS YTA KAN LEDA TILL ATT ANTIGENSUBSTRATET TAR SKADA. 5. ODLING AV OBJEKTGLAS (30 ± 5 minuter i rumstemperatur, dvs 18-24 °C) Placera objektglaset/-n i en fuktig täckt kammare (en petriskål med fuktad pappershandduk duger). Odla, med locket på, i 30 minuter (±5 minuter) i rumstemperatur (18-24 °C). 6. PBS-SKÖLJNING Avlägsna objektglaset/-n från inkubatorbrickan och skölj hastigt med PBS genom att använda en sprutflaska, Pasteur, eller serologisk pipett. Spruta inte buffert direkt på brunnarna. OBSERVERA: Led PBS-flödet längs objektglasets mittlinje för att undvika korskontamination på tiobrunnars objektglas genom att först luta glaset mot brunnarna 1-3 och därefter mot brunnarna 4-6. 7. PBS-TVÄTTNING (tio minuter) Tvätta objektglaset/-n i tio minuter med PBS i en objektglasfärgskål eller ett Coplin-kärl. Försiktig omskakning rekommenderas vid nedsänkning, mittpunkt och avlägsnande av objektglaset. Denna tvättning kan förlängas med upp till 30 minuter utan att de slutliga testresultaten påverkas. Kassera PBS-tvättlösningen efter användning. För optimala resultat skall PBS ändras vid mittpunkten och en magnetisk blandare användas. 8. FLUORESCERANDE ANTIKROPPREAGENS (täck brunnarna med 10-12 droppar) Avlägsna ett objektglas åt gången från PBS och doppa det 3-5 gånger i avjoniserat eller destillerat vatten. Knacka objektglasets sida mot läskpapper eller pappershandduk för att avlägsna överskottsvatten. Återför genast objektglaset till inkubationskammaren och täck brunnarna helt med fluorescerande antikroppreagens. Börja med att placera en droppe över varje brunn. Upprepa detta för varje objektglas. Den fluorescerande antikroppreagensen har titrerats för att kompensera för det avjoniserade eller destillerade vatten som är kvar på objektglaset efter sköljning. OBSERVERA: Det är viktigt att objektglasbrunnarna inte torkar under detta förfarande, annars tar substratet skada. TORKA ALDRIG OBJEKTGLASET MED LÄSKPAPPER ELLER ANNAT FÖREMÅL OCH LÅT ALDRIG OBJEKTGLASET STÅ UTAN FLUORESCERANDE ANTIKROPPREAGENS LÄNGRE ÄN FEMTON SEKUNDER. 9. ODLING AV OBJEKTGLAS (30 ± 5 minuter i rumstemperatur, dvs 18-24 °C) Placera locket på inkubationskammaren och täck med en pappershandduk för att förhindra att det utsätts för ljus, om kammaren inte är ogenomskinlig. Odla objektglaset/-n i 30 minuter (±5 minuter) i rumstemperatur (18-24 °C). 10. PBS-SKÖLJNING Avlägsna objektglaset/-n från inkubatorbrickan och skölj hastigt med PBS. Spruta inte buffert direkt på brunnarna. 11. PBS-TVÄTTNING (tio minuter) Tvätta objektglaset/-n i tio minuter med PBS i en objektglasfärgskål eller ett Coplin-kärl. TORKA ALDRIG OBJEKTGLASET MED LÄSKPAPPER ELLER ANNAT FÖREMÅL OCH LÅT ALDRIG OBJEKTGLASET STÅ UTAN SKYDDSREMSA LÄNGRE ÄN 15 SEKUNDER. Tillsätt 4-5 droppar halvpermanent monteringsmedium längs mittlinjen på varje objektglas. Sätt försiktigt skyddsremsan på plats och undvik luftfickor genom att försiktigt lägga ned skyddsremsan från ena änden av objektglaset till den andra. OBSERVERA: Överflödigt monteringsmedium på objektglaset kan leda till hög bakgrundsfluorescens på grund av ljusspridning eller brist på tydlig upplösning av cellerna (suddig bild). Överflödigt monteringsmedium kan avlägsnas från objektglaset genom att skyddsremsan försiktigt torkas med läskpapper eller linspapper. Undvik att röra direkt vid skyddsremsan.
