Candidates registered for Confirmation II 2016-2017 are required to have Sacraments of Initiation documents on file at Our Lady of
Mt. Carmel Catholic Community (OLMC) by the time the 2016-2017 program year starts. Please return completed checklist at the
Second Thursday Gifts of the Holy Spirit Session, November 10, 2016.
Es requerido que los candidatos registrados en el programa de Confirmación II (2016-2017) tengan su documentación de los
Sacramentos de Iniciación en los archivos de nuestra parroquia (Nuestra Señora del Monte Carmelo) al comenzar las clases del 20162017. Favor de regresar esta forma ya completa a la Second Thursday Gifts of the Holy Spirit Session, 10 Noviembre 2016.
Parents and candidates will need to meet with Estela Flores, Secretary & Sacramental Records, 713-645-6673, to submit copies of
the Baptism certificate and First Communion certificate if the candidates received the sacraments from another parish.
Si los candidatos recibieron los Sacramentos de Bautismo y/o Primera Comunión en otra parroquia, los padres y candidatos deben de
comunicarse con Estela Flores, Secretaria de la Parroquia, al 713-645-6673, para entregar copias de los certificados y obtener su
firma en esta forma.
