including private-pay families, up to ten (10) days per fiscal year July - June. If you indicate more than ten
(10) days, Children’s Resource and Referral (CRR) will apply the first ten (10) days indicated on this form.
A copy of the provider handbook must be attached to this form in order to receive reimbursement. CRR
must be notified of changes to the holiday / non-operational days listed on this form prior to the holiday /
non-operation date listed. CRR will not make changes retroactive. This form must be updated by June 30th
of each year in order to receive all reimbursement for all days indicated.
