Emergency Medical Authorization
I am aware of the risks, hazards and inherent dangers that may arise
due to my child’s participation in the 2013 University of Akron Zips
Basketball School and Zips Individual Instruction/Offensive Skills Camp
for Advanced Players being held at The University of Akron (collectively
referred to as “UNIVERSITY”) on the following dates: June 17–20, 2013,
June 24–27, 2013, July 8–11, 2013 and July 22–25, 2013.
In consideration for being allowed to participate in said activity, I hereby
release, waive and discharge UNIVERSITY, its instructors, agents and
employees from every claim, liability or demand of any kind sustained,
whether caused by the negligence of the UNIVERSITY or otherwise. This
release shall be binding upon any heirs, administrators, executors and
assigns of mine.
I further agree to indemnify the UNIVERSITY from any loss, liability,
damage or cost it may incur due to my participation in said activity in any
way whether caused by the UNIVERSITY or otherwise.
In the event of illness or injury resulting or arising directly or indirectly
out of said activity, I hereby give my consent and authorization for (1) the
administration of emergency first aid care and treatment at the scene of
an emergency by faculty, staff members or volunteers of UNIVERSITY or
(2) the administration of any treatment deemed necessary by a licensed
physician or dentist and (3) the transfer to any hospital reasonably accessible. This authorization is not intended to cover major surgery unless the
medical opinions of two (2) licensed physicians or dentists, concurring in
the necessity for such surgery, are obtained prior to the performance of
such surgery.
I further declare and warrant that I am covered by sufficient medical
and dental insurance and that such insurance will remain in effect during
my child’s participation in said activity.

The Offensive Skills Camp is for players, boys and girls,
from grades 3–12. Each player will have detailed instruction, film analysis, shooting instruction, and Division I
college basketball practice each day. There will be four
sessions by grade level, with 30 spots open per session
with emphasis on offensive skills.

Tuition for the Individual Instruction/Offensive Skills Camp
is $190.00.
* An early registration discount of $15.00 will be applied if students
register for the Zips Basketball School and Offensive Skills Camp

Tuition for the Basketball School is $165.00, which includes
an Akron Zips t-shirt. A $50.00 non-refundable deposit,
which is applied toward your tuition, is required with your
application form. Camp store will have food for campers to
purchase (pizza, drinks, etc.).

The 2013 UA Basketball Camps will be held on the UA
Campus. Games will be played in RHODES ARENA and the
STUDENT RECREATION CENTER.
Registration/Deadlines
Registration will be held in the lobby of Rhodes Arena on
the first day of each basketball school. The deadline for
registration is one week prior to each session. Payment
in full must accompany each camp application prior to
the start of your camp session. We now have registration
