2006 Annual Report Mary’s Center’s family tree grows with the community The mission of Mary’s Center is to build better futures for families through the delivery of health care, education, and social services. The center embraces culturally diverse communities to provide them with the highest quality of care, regardless of their ability to pay. By providing clients with an effective, integrated set of culturally competent services in collaboration with the center’s partners, each family member benefits from a variety of programs offered. In this way, Mary’s Center strives to save lives and create stronger communities, one family at a time. Established in 1988. A Federally Qualified Health Center since 2005. National Council of La Raza Affiliate. A Letter from Maria Gomez, President & CEO Mary’s Center experienced many accomplishments and successes in 2006. The center increased the number of patients served, improved the programs and services it offers, streamlined operational processes to further facilitate service delivery, and solidified plans to open an additional facility in Maryland. Mary’s Center’s goals for 2007 continue to be ambitious and we are confident that they will be achieved with measurable success. The center is looking forward to extending its services beyond the District of Columbia with the opening of a Maryland clinic in late summer; broadening development outreach to secure additional sources of funding and support; further improving operational efficiency and client tracking to facilitate outcomes performance measurements; and identifying new partners to establish yet another site in the Commonwealth of Virginia. Such aggressive growth and planning presents numerous new challenges, and every day the dedicated staff of Mary’s Center works to produce positive outcomes for our clients. We look forward to a successful 2007. Programs Adult Medicine Mary’s Center provides primary and prenatal care to adults and adolescents including physical exams, prenatal care, family planning, and sick visits. We promote wellness through health screening, cancer screening, health education and support, management of acute and chronic illnesses, and other health conditions. 4,091 adults served in 2006 1,092 Women received prenatal care 476 Infants delivered 467 Infants born at a healthy birth weight Infants born prematurely (prior to 37 weeks) 9 1,341 Women received annual exams with Pap smear and breast cancer screenings 261 Men received annual exams 5,057 Family planning visits 3,556 Sick and follow-up visits 293 Adults received skin tests for tuberculosis 125 Female patients received a colposcopy Pediatrics Mary’s Center serves children ages 12 and younger with complete well-baby and well-child care, preventing illness and injury while fostering optimal growth and development. The center emphasizes prevention and management of chronic conditions by immunizing all patients according to schedule. 2,394 Well-child check ups 2,177 Sick and follow-up visits 1,331 Children received blood lead level testing 1,575 Children received tuberculosis tests 2,781 children served in 2006 Programs School-Based Health Center Mary’s Center’s School-Based Health Center offered primary care services to students at Brightwood Elementary School, staffed by a Family Nurse Practitioner. Services provided included well-child checks/physical exams, vaccinations, lab work, sick visits, treatment (including dispensing/prescribing medications), and referrals. In-classroom health education for students and health education programs for parents were also included. The program concluded on June 30, 2006. 276 Patient visits 30 Health education classes 12 Parent group meetings 1 Health fair 199 students served in 2006 587 teens served in 2006 Teen Program Mary’s Center provides comprehensive health, education, and social services to young adults between the ages of 13 and 21. Through a youth driven/youth development model, we promote primary health care, family planning and pregnancy prevention services, health education, academic support and college preparation, HIV/STD testing, counseling, case management, and referrals. Mary’s Center works to help adolescents make responsible decisions and realize their full potential. 2,379 Teen visits 24 Saturday Teen Clinics 25 Teens participating in the Peer Educators Program 64 Teens participated in the Urbanitos Summer Program 12 Monthly birthday activities Programs Family Social Services Mary’s Center’s bilingual family support team addresses social and emotional needs of patients and seeks to help patients and their families become self-sufficient. Family support workers educate and assist patients and their families in accessing available services such as housing and food programs, education, financial assistance, employment, legal and mental health services. Additionally, the staff ensures a continuity of care by serving as a liaison among physicians, mental health professionals, and other social service agencies. 1,258 Client visits 300 Referrals to other Mary’s Center services 87 Referrals to services outside of Mary’s Center 148 Prenatal class participants 118 Educational Baby Shower Program participants 111 Trained in car seat safety 504 clients served in 2006 396 families served in 2006 Healthy Start Healthy Families (Home Visitation Services) Healthy Start Healthy Families (HSHF) provides intensive, long-term home visitation services to pregnant and postpartum women and their families. Outreach, assessment, and support staff work with a registered nurse, an inhome mental health provider, and educational specialists to provide education, support, and advocacy services. This team ensures that participants receive quality comprehensive medical care and social services. HSHF serves District of Columbia residents until the child reaches the age of five. HSHF demonstrates a combined effort of the Healthy Families America home visiting model, a national movement to prevent child abuse and neglect, and the Federal Healthy Start program, an initiative to reduce infant mortality. HSHF’s mission is to partner with families to ensure children are healthy, safe, and ready for school through home visitation and connections with community resources. 323 Assessments completed 6,522 Family support worker home visits 663 Registered nurse visits 97% Babies born with healthy birth weights 94% Children linked with a medical provider and health insurance 95% Children enrolled are up to date with their immunizations 99% Families with no substantiated case of child abuse and neglect 97% Teen parents did not have an additional birth within two years of their first child Programs Early Intervention Program The Early Intervention Program provides education, service coordination, advocacy, psychosocial support, home visitation, and translation services for parents of children up to three years old with disabilities or developmental delays who are eligible for the DC Infants and Toddlers with Disabilities Program. All families who request assistance are helped with enrolling their children in daycare, preschool or public school programs. Mary’s Center hosts weekly playgroups for all parents and their children with a special focus placed on children ages 0–5 who are at risk for developmental delays. The program also facilitates a support group called “Somos Unidos” for Spanish-speaking parents of children with disabilities. 106 Children had a developmental evaluation completed 97 Children were eligible and enrolled in the program and provided access to therapies 23 Families participated in the playgroup 24 Families participated in the support group 143 children and families served in 2006 10 454 participants served in 2006 Even Start/ESF Public Charter School Mary’s Center’s Even Start Multicultural Family Literacy Program provides parents and their children ages 0-7 with the tools needed to be successful in school and in the work place. Parent and Child Together Time (PACT Time) supports parent’s understanding of their role in their child’s educational development, reinforcing the impact that parents have on educational success. In September 2006, Education Strengthens Families Public Charter School (ESF) opened. Mary’s Center partners with ESF to strengthen the family literacy services available to the immigrant population of the District of Columbia by enhancing its quality and long-term financial stability that would not be possible as a stand-alone program. The center offers technical assistance and step-by-step guidance in the complex licensing process, providing individual support problem solving, interacting with agencies on behalf of the participants, and assistance as providers prepare for licensure exam and final inspection. Classes are taught in Amharic, Arabic, Creole, English, French, Spanish, Tigrigna, Turkish, and Vietnamese. 12 44 Participants received the Child Development Association Credential 70 Participants had internships at daycare centers 8 Child care homes were licensed 18 Participants continue the licensing process in 2007 WIC Program Special Supplemental Nutrition Program for Women, Infants, and Children is a federally funded program that provides pregnant women, new mothers, infants, and children under the age of five years with education about healthy foods and nutrition. Mary’s Center strives to facilitate the process of receiving WIC benefits. 15 Educational trips to farmer’s markets to teach participants how to use vegetable vouchers 250 Nutrition and health classes given 3,468 clients served in 2006 Since the first Mary’s Center WIC site was opened in June 2002, the program has expanded its services through satellite sites around the District of Columbia. In addition to the center’s main site in Adams Morgan, the program also serves the neighborhoods of Ward 4, Chinatown, and Brookland, so that residents do not have to travel far to receive benefits. 13 Programs Health Promotion Mary’s Center provides health screenings, HIV, STD and tuberculosis testing, a comprehensive set of counseling services, and educational programs related to disease prevention and other health related topics. The center’s staff delivers such programs using popular academic methodologies based on community-focused models. 2,767 clients served in 2006 534 Breast health educational chats facilitated 47 Waiting room educational chats given on other health and prevention topics 1,924 HIV one-on-one counseling, education and testing provided 31 Peer Educators received training on STDs, HIV, basic human anatomy, and body function systems 3 Diabetes management classes were conducted 3 Childhood asthma management classes conducted 2 14 Cancer related workshops conducted 928 clients served in 2006 Mama & Baby Bus Mary’s Center’s Mama & Baby Bus is a mobile health clinic that travels throughout the District of Columbia to provide families with health care and related services such as health screenings, insurance enrollment, pregnancy and STD/HIV testing, health education, vaccinations, dental exams, and referrals to other health and social services. 74 Pregnancy tests administered 427 HIV tests administered 879 Social service intakes 828 Dental exams and/or cleanings provided 22 Children given immunizations 15 Programs Bilingual Health Access Project Mary’s Center’s Bilingual Health Access Project (BHAP) helps families apply for Medicaid, food stamps, Temporary Assistance for Needy Families (TANF), and the District of Columbia’s Children Health Insurance Program. The center’s staff gathers and translates all necessary documents for enrollment and helps families complete their applications in a timely manner. Outreach workers spend numerous hours in community settings educating the public about the advantages and the process of enrolling in public benefit programs. The BHAP staff speaks Amharic, English, Spanish and Vietnamese. 7,210 Client encounters 227 New applicants for Medicaid 394 New applicants for DC Healthy Families 223 New applicants for Temporary Assistance for Needy Families 363 New applicants for Food Stamps 2,485 Translation and/or interpretation services provided 6,379 clients served in 2006 16 Mary’s Center: Investment in Lives and Partnerships 2006 was a successful year for Mary’s Center, and many of the accomplishments made are reflected in the center’s Strategic Plan prepared with support from Venture Philanthropy Partners (VPP). With the strong commitment of VPP, the center was able to increase the number of patients served, improve the programs and services offered, streamline operational processes to facilitate service delivery, and solidify plans to open an additional facility in Maryland to meet the needs of residents in Montgomery County. The center’s partnership with VPP is a multi-year commitment with the goal of Mary’s Center’s continued expansion until all populations throughout the Washington, DC region are effectively provided with quality health care services. 17 Programs Federally Qualified Health Center As a Federally Qualified Health Center (FQHC), Mary’s Center has been able to achieve many goals that otherwise would not have been attainable. FQHC status has enabled the center to increase the number of clients to 9,864 and to improve access for families to a host of health and social services, leading to healthier lives. In addition, it has provided for staff trainings, as well as permitting Mary’s Center to be covered under the Federal Torts Claims Act which allows funds previously spent on malpractice insurance to be used to expand and improve programs. 18 Special Events at Mary’s Center Mary’s Center utilizes special events to spread the word about the services it provides in a fun and enjoyable way, and to raise funds to help support the center’s programs and services. Established in 1998, Noche Tropical, Mary’s Center’s annual gala, has raised close to $1 million, providing much needed funding for many of the center’s programs and services.
