Purpose: Students attending the Injury Prevention & Safety First Aid Class will participate in a unit on WATER SAFETY.
Dear parent/guardian,
I am Vic Sbarbaro, one of the instructors for the Summer Connection Program. The WATER SAFETY unit includes a related video: on water
rescues, information about water safety, water hazards, personal flotation devices (life jacket relays), and quizzes. During their class I want to
administer a survey on Water Safety that I intend to publish. The purpose of the survey is (1) a tool for students to assess their own water safety
experiences, and (2) a tool to help school decision-makers determine the extent of drowning/water accidents and ways to reduce drowning/water
accidents. This survey will take about 10–15 minutes to complete. Permission from you that your son/daughter take this survey is voluntary. If your
son/daughter chooses not to take the survey there is no penalty. Your son/daughter can still attend this class without taking the survey.

Students in this class will also view October Sky. Based on a true story in the late 50’s and early 60’s, the Rocket Boys with the help of their teacher
overcome seemingly impossible odds to fulfill a vision in a timeless inspiring story. Similar to this movie, overcoming obstacles and accomplishing
goals will be addressed in this class. Because this movie is rated PG, your student will not be allowed to view it without your written
permission.

Estimados Padres,
Propósito: Los estudiantes que participen en la clase de primeros auxilios también recibirán información de cómo estar seguros mientras nadan.
Yo soy Vic Sbarbaro, uno de los instructores del programa de verano, Summer Connection Program. La unidad de seguridad en el agua, WATER
SAFETY, incluye videos sobre: rescates en el agua, información sobre la seguridad dentro del agua, peligros en el agua, equipo de flotación personal
(salvavidas) y un examen del material al final. Durante la clase, yo quisiera administrarles un estudio sobre la seguridad en el agua que quisiera
publicar. El propósito del estudio es (1) para que sirva como una guía para que los estudiantes evalúen su propia experiencia sobre la seguridad en el
agua, y (2) como herramienta para ayudar al personal encargado de las escuelas a determinar el grado de peligrosidad de ahogamiento y accidentes en
el agua y maneras de reducirlos. Este estudio tomara aproximadamente 10-15 minutos para completar. El permiso de usted para que su hijo/a
participe en este estudio es voluntario. Si su hijo/a no desea participar no hay problema. Su hijo/a todavía puede asistir a esta clase sin participar en
el estudio.

Los estudiantes en esta clase también estarán viendo el video October Sky, Película basada en la vida real de los 50’s y 60’s, los “Rocket Boys” con
la ayuda de su profesor vencieron obstáculos y lograron sus sueños. En esta clase estaremos discutiendo sobre las metas y como cumplirlas y por esa
razón estaremos viendo escenas de esta película. No veremos toda la película, solo escenas relacionadas a la clase. Los estudiantes participarán en
una discusión de cómo llegar a decisiones positivas después de ver estas escenas. Si prefiere que su hijo(a) no participe, por favor indíquelo en la
parte apropiada y se le ofrecerá otra actividad académica durante esa actividad. Su hijo/hija no podrá ver la película sin su permiso.

While attending the Summer Connection Program you need to stay in the supervised areas.
This means that during breakfast and lunch, you need to stay in the cafeteria.
You many NOT go out to the balcony during breakfast and lunch.
During class time, you need to ask permission to leave the room. Only one person may leave the classroom at one time
to use the restroom.
Please do not wander off to explore on your own. Rattlesnakes are a dangerous reality at Butte College!!
You may not take any food out of cafeteria. FDA inspectors monitor and we could lose the breakfast and lunch
program if their regulations are not followed.
You may only buy food after you have taken the free meal.
You may not play with the arcade machines at all!

Usted no puede sacar comida fuera de la cafetería. Inspectores del programa del FDA supervisan y podríamos perder el
programa del desayuno y del almuerzo si no seguimos sus reglamentos.
Sólo después de obtener su alimento, podría comprarse comida de la cafetería a su gusto.
No pueden jugar en las máquinas de juego!

Conduct. I understand that it is imperative that I abide by these rules for my safety. I also understand that a discipline referral
resulting from breaking any of these rules may lead to dismissal from the program.
