FIEBRE SIN FOCO < 3 AÑOS DE EDAD 1. Definición: - Fiebre < 36 horas de evolución, sin causa aparente, y tras anamnesis y exploración física completa. - Escala Yale <10: bajo riesgo enfermedad bacteriana invasiva; 10-16: riesgo intermedio; >16: riesgo alto. PARÁMETRO NORMAL (1) PERFUSIÓN PERIFÉRICA Sonrosado Extremidades calientes Relleno capilar < 2´ CALIDAD LLANTO Vigoroso RESPUESTA SOCIAL REACCIÓN PADRES HIDRATACIÓN NIVEL CONCIENCIA PUNTUACIÓN Sonríe y activo Se calma o no llora Normal Normal <10 RI riesgo IBPG AFECTACIÓN MODERADA (3) Piel moteada Extremidades frías Relleno capilar 24´ Lloriqueando ó Sollozando Brevemente alerta Llanto intermitente Boca seca + Somnoliento 10-16: riesgo medio IBPG AFECTACIÓN GRAVE (5) Palidez Relleno capilar >5´ Débil ó muy agudo No sonríe, no alerta Llanto continuo Mucosas secas ++ Estupor o no se despierta ≥16 riesgo IBPG ESCALA YALE < 3 MESES 2. Manejo: - < 3 meses de edad: DERIVACIÓN hospital. - > 3 meses Yale < 10 (Buen aspecto) * Tª <39,5º: Control 24 horas * Tª ≥ 39,5º: ≤ 2 dosis vacuna neumococo: DERIVAR hospital ≥ 2 dosis vacuna neumococo: Combur orina (Tª >39º) (Niños<1 año y Niñas <2) con bolsa estéril. Lavar la zona genital previamente y cambiar la bolsa cada 30 minutos. *Si combur N: Control en 24 horas. *Si combur tiene leucocituria y/o nitritos +: DERIVAR hospital *Si ≥1año (niños) ó ≥2 años (niñas): Control 24h Yale > 10 (sobre todo si esta valoración se hace tras 1 hora de la administración de un antipirético): DERIVAR hospital.
