Family – Back to School Questionnaire
Dear Parent(s)/Guardian(s),
As I stated in the welcome letter you received before the beginning of the school
year, I would be sending a questionnaire home for you to complete. Please take some
time, in the next few days, to respond to as many of the following questions as possible.
These questions pertain to your child. By answering the questions, I can discover as
much important information as possible about your child, his/her life, and her/his leaning.
The information you share is for my eyes only, and will only be shared, when deemed
important, with Daly Elementary staff involved in supporting your child at school. If you’d
prefer not to record a written response to a question, but discuss such a matter over the
phone, please email me so we can arrange a time to speak.
Please return this questionnaire to me no later than the end of the first week of
school. You can send this hard copy in with your child, or as an attachment to an email
(the document can be found on my website).

Como he dicho en la carta de bienvenida que recibió antes del inicio del año
escolar, ME podría enviar un cuestionario para que lo complete. Por favor tome algún
tiempo, en los próximos días, a fin de responder a muchas de las preguntas siguientes
como sea posible. Estas preguntas se refieren a su hijo. Al responder a las preguntas,
puedo descubrir que mucha información importante como sea posible acerca de su hijo,
su vida, y su inclinación. La información que usted comparte es sólo para mis ojos, y sólo
se comparte, cuando se considera importante, Daly Elemental personal involucrado en el
apoyo al niño en la escuela. Si prefieres no hacer constar por escrito una respuesta a una
pregunta, pero hablar de ello a través del teléfono, por favor envíeme un email para que
podamos organizar el momento de hablar.
Por favor devolver este cuestionario a mí a más tardar al final de la primera
semana de clases. Este archivo se puede enviar copia en disco duro con el niño, o como
un archivo adjunto a un mensaje de correo electrónico (el documento se puede encontrar
