Elementary Faith Formation Registration Information
Early registration for families currently enrolled in our program will begin June 1. You may register by stopping by the
Faith Formation Office during regular office hours Monday-Thursday: 9:00 – 5:00pm, calling in your registration at (972)
423-4715, or by registering online at our website www.stmarkplano.org. If your registration is called in, please mail
payment to the Faith Formation Office as soon as possible, so we may process your registration; we also accept on-line
payments.
Faith Formation classes begin Sun. Sept. 9 and Wed. Sept. 12. Registrations are processed on a first-come, first
served basis and since class size is limited, be sure to take advantage of early registration to get your time preference.

The fees noted above apply to families with children and/or teens enrolled in either elementary or youth faith formation –
one fee structure per child and/or family. Full payment is due at time of registration unless other arrangements are made
and any prior balances must be paid. All families enrolled in our program must be registered with the parish or pay out of
parish fees.
2012-2013 Elementary Faith Formation Classes: Scheduled Day and Times

Classes meet for 1 ¼ hr.
Preschool [PK3] through 5 grade
8:45-10:00am [English only], 10:30-11:45am [English/Spanish/Bilingual]
1:15-2:30pm [Spanish/Bilingual only] (additional times may be added as needed.)
Wednesday
1 – 5 grade
6:45-8:00pm – English/Spanish/Bilingual

If you have a child entering the 6 grade or above, there are separate registration and permission forms that need to be
filled out; these are available in the Youth Ministry Office.

Programa Primario de Formación de Fe
Información de las Inscripciones de 2012-2013
Las inscripciones para las familias que fueron matriculadas en nuestros programas de formación de fe en el año 20112012 inician el día 1 de junio. Pueden registrarse durante horas de oficina de lunes a jueves de 9:00 AM a 5:00 PM,
pueden matricularse en persona o llamando a la oficina de formación de fe al numero (972) 423-4715, también por el
Internet al sitio www.stmarkplano.org. Si se matriculan por teléfono, por favor envíe su pago a la oficina lo mas pronto
posible (St Mark Formación de Fe 1105 W 15th Street, Plano, TX 75075) o también se aceptan pagos por la Internet, y
así poder iniciar su matricula en cuanto recibamos su pago.
Las clases de Formación de Fe comienzan el domingo 9 de septiembre y el miércoles, 12 de septiembre. Las
clases tendrán cupo limitado, así que aproveche matricularse temprano para conseguir el horario que usted desee. El
pago completo es requerido al momento de su registración; al menos que hagan algún arreglo con la coordinadora.
Todas las familias que se inscriban en nuestro programa deberán estar registradas en la parroquia, o si no es así,
deberán pagar una cuota para personas no registradas.

Si tiene a un niño que entra en el séptimo grado o arriba, ha que matriculado separado, las formas que se necesitan ha
llenar están disponibles en la Oficina del Ministerio de Juventud.
