•If your child has CenCal Health and has not had an eye exam in two years you can call any office listed below
and make an appointment.
• If your child has had an eye exam in the last two years, you will need to get a referral from your child's
pediatrician before making an appointment.
•Optometrists who accept BOTH CenCal Health and/or State Medi-Cal from the CHDP GATEWAY
