During the window of May 9 - May 27, 2011, all
Bakersfield City School District, State Pre-Kindergarten
sites will offer pre-registration for the 2011-12
Pre-Kindergarten Program between the hours of:
8:45 a.m. - 10:45 a.m. and 12:45 p.m. – 2:45 p.m.
Please post and distribute attached flier as needed. Thank you.
2011-2012 Pre-Registration
Parents, your child may qualify for a FREE Pre-Kindergarten (Pre-K) education program
if your family meets the California income guidelines and State funding is available.
COMPLETE AN APPLICATION for the 2011-12
school year between the hours of
8:45 - 10:45 a.m. or 12:45 - 2:45 p.m.
May 9th through May 27th, 2011

Parent and/or adult volunteers with current Tuberculosis Clearance (TB) on file will participate
7 hours per month in child’s classroom.
Bring ALL of the following documents (required for enrollment):
Child’s completed physical form (MUST be less than 1 year old);
Copy of child’s birth certificate (MUST be Age 3 or 4 on or before December 2, 2011);
Copy of birth certificate for all children living in the home 18 and under;
Child’s immunization record (current);
Child’s TB clearance (within 1 year);
Parent and/or adult volunteer TB clearance (within 1 year);
Photo ID (identification) for each parent and/or adult volunteer;
Proof of income (one current month for each adult living in the home);
Completion of employment verification form; and
Current proof of address (utility bill or official letter).

Un padre y/o adulto voluntarios con una prueba actual de estar libre de tuberculosis participará
7 horas por mes en la clase del niño.
Traiga TODOS los siguientes documentos (necesarios para la inscripción):
Formulario completado del examen físico del niño (DEBE haberse realizado hace menos de 1 año);
Copia del acta de nacimiento del niño (DEBE tener 3 ó 4 años el 2 de diciembre o antes del 2011);
Copia de la acta de nacimiento de cada niño/a de 18 o menor que vive en casa;
Registro de vacunas del niño (debe estar al día);
Prueba de que el niño no tiene tuberculosis (hace menos de 1 año);
Prueba de que el padre/adulto voluntario no tiene tuberculosis (desde hace menos de 1 año);
Identificación con foto para cada padre y/o adulto voluntario;
Comprobante de ingresos (un mes actual para cada adulto que vive en el hogar);
Completar el formulario de verificación de empleo; y
Comprobante de domicilio (factura de la compañía de servicios públicos o carta oficial)
El programa de prekinder empieza con la escuela regular el 22 de agosto del 2011
