Congratulations! I am writing to confirm that you were chosen to participate in an immigration
project hosted by Orrick, Herrington & Sutcliffe LLP, the University of California, Irvine Law School
and the Legal Aid Foundation of Los Angeles. As we mentioned to you during our recent telephone
conversation, a group of attorneys and law students will meet with you to help prepare your green card
application, travel document, and work permit.
You will meet with the attorneys and law students on Tuesday, October 6th at the Orrick office
in downtown Los Angeles. The office is located 777 South Figueroa Street, Suite 3200, Los Angeles.
Please see the attached map for directions. Parking is free in the parking structure labeled “Target”
located on 8th Street. Please bring your ticket with you to the office on Oct. 6th and we will validate it.
Please arrive at 10 a.m. and plan to stay until about 3:00 p.m. We will serve you a light
breakfast and lunch. If you have children who are 14 years of age or older that are applying for a green
card, please bring them with you as they will need to sign their own application forms. If you are unable
to arrange for childcare for children under 14 years old, you may bring them with you to the office.
When you meet with the attorneys and law students on October 6th, they will help you fill out
application forms to get a work permit, a travel document and your green card (permanent resident card).
The attorneys will also ask you about your case and go over the documents that you will bring. After they
review your documents, lunch will be served. After lunch, the attorneys and law students will work on
your applications. There will be a space for you to wait with your children while they prepare your forms.
Once the forms are finished, they will meet with you again to discuss your case. Please plan to stay at the
office until approximately 3:00 p.m.
There are a few things you need to do to prepare for the application process. Please look at the
attached instructions page on what you should bring with you to the clinic. Some of the documents may
take extra time to collect, so please start locating the documents today. You also need to get a medical
exam by a civil surgeon. The letter on the next page explains the exam and where to find a civil surgeon
in your area.

Enclosed please find a copy of the Form I-693, Report of Medical Examination and Vaccination
Record and a copy of part of the list of USCIS approved civil surgeons in your area. USCIS generally
requires all applicants filing for adjustment of status to that of a permanent resident to submit this form.
Please be advised that only a doctor who has been specially designated by USCIS as a civil
surgeon may perform the medical exam. USCIS will not accept a Form I-693 completed by a doctor who
is not a currently designated civil surgeon. The current complete list of civil surgeons is available online
at www.uscis.gov, under the Civil Surgeon Locator Tool located under “Related Links” in the upper right
hand corner of the web page. You may also obtain the names and phone numbers of designated civil
surgeons by calling the USCIS National Customer Service Center at (800) 375-5238. I have included a
list of civil surgeons in your area.
You must pay all costs of the medical exam, including the cost of any follow-up tests or
treatments that may be required. The prices of each doctor on the list provided may vary. Please make
sure to ask how much the doctor charges before you schedule an appointment for your medical exam.
You must fill out Part 1 of the I-693 form. Do not sign the form until you are in front of a doctor. Part 2
through 6 will be filled out by the doctor, clinic, or the health department receiving the referral. Take any
medical records that you may have at the time of your visit to the doctor. Also, take a photoidentification with you to the exam.
The doctor is required to give you the completed form must be submitted in a sealed envelope.
Please do not open the envelope. USCIS will return the form to you if it is not in an envelope or if it has

Any receipts or letters showing you receive public assistance such as CalWORKs, Cal Fresh (also
called food stamps) or Medi-Cal.
If you do not receive public assistance but cannot afford the filing fee, please bring documents
showing your economic need. Bring proof of your income and expenses.
Medical examination results. These will be given to you in a sealed (unopened) envelope, please
do not open the envelope.
If you have ever been arrested or charged with a crime please bring all documents relating to the
arrest or charge. You do not need documents for traffic incidents that did not result in an arrest. If
you have a traffic incident involving drugs or alcohol please submit documents for those
instances. Example documents include
o Police reports
o Arrest record
o Official statement or court order confirming arrest but no charges filed
o Dismissal order, conviction record, or acquittal order
If you were convicted or placed in an alternative sentencing program or rehabilitative program
(such as drug treatment or community service) please bring the sentencing record for each
incident and evidence you completed the sentence (this can be a copy of probation or parole
record, or a letter from the program)
If you were granted a U Visa, please also bring:

Any documents showing you helped the police investigate your case. This can be emails
from the police or district attorney, court orders or receipts, or any other documents.
Any letters or forms showing your U Visa was approved.
School records for all children (for the past three years).

Phone, electricity, television, internet, and other bills (for the past three years).
Tax filings or forms (for the past three years).
Bank statements and/or paychecks (for the past three years).

Directions for October 6th
Please arrive at the Orrick office by 10:00 a.m. for the clinic. The office is located at 777 South Figueroa
Street, Suite 3200, Los Angeles. The office is located in the tall white building on the corner of 8th Street
and Figueroa Street.
There is free parking in the parking structure labeled “Target” on 8th St. If you park in any other parking
structure except the Target structure, we will not be able to validate your parking. There is also a metro
station located one block away from the office. The closest metro station is the 7th Street Metro center
station. On the subway, the Metro Purple Line (805) and Metro Red Line (802) will take you to this
station. Using the light rail, the Metro Blue Line (801) and Metro Expo Line (806) will also bring you to
this station.
When you get to the metro station, walk down Flower Street toward 8th Street (towards a tall light blue
building and away from the Coffee Bean & Tea Leaf). Turn right on 8th Street and walk one block up to
Figueroa Street. The office is in the tall white building on your right hand side.

Permanency Project Overview To be eligible for a U Visa, you must be a victim of crime who has cooperated with law enforcement. The clients you will see have all received U Visa status. Now we are helping them apply for permanent residence. Forms The following forms will be filled out in the session:  I‐485 – Application to register for permanent residence  G‐325(a) – Biographical information  I‐131 – Application for travel document  I‐765 ‐ Application for Employment Authorization  I‐912 – Fee waiver  AR‐11 – Alien change of address form [if their address is different than listed on their U Visa app]  G‐28 – Attorney representation authorization You will also need to prepare:  Declaration (see sample)  Cover Letter for the application (see sample – prepare on own before second session) Important questions to ask:  Exact date and location of entry for EACH entry into the US (both before and after the client applied for a U Visa)  Has the client had any interaction with the police (especially after they were granted U Visa status)? o Check for arrests without charges o Traffic stops – ask if includes drugs or alcohol  Has the client has left the US since being granted a U Visa?
