Dieses Produkt trägt das CE-Zeichen, d. h. es erfüllt somit die grundlegenden Anforderungen, die durch die Medizinprodukterichtlinie der EG festgelegt worden sind. Art, Zusammensetzung und Anwendung jedes Distraktionssystems werden vom Anwender nach Beurteilung der jeweiligen anatomischen und funktionalen Ansprüche des Patienten bestimmt und liegen somit in der Verantwortung des Anwenders (behandelnder Arzt bzw. operierender Chirurg). • Gebrüder Martin ist nicht verantwortlich für Komplikationen durch falsche Indikation, falsche Implantatauswahl, falsche Kombination von Systemkomponenten und Operationstechnik sowie Grenzen der Behandlungsmethode oder unzureichende Hygiene. • Alle Distraktionskomponenten, Instrumente und Zubehör müssen in einsatzbereitem Zustand verfügbar sein. • Das Produkt muss indikationsgemäß angewendet werden. 8 V. 3.3 Gebrauchsanweisung RPE-Distraktoren 1.3 Gewährleistung Es gelten unsere Allgemeinen Verkaufsbedingungen in der jeweils gültigen Fassung. Davon abweichende Vereinbarungen schränken die gesetzlichen Rechte des Käufers nicht ein. Eine darüber hinausgehende Garantie bedarf der vertraglichen Form und schließt Vandalismus an Bauteilen, Software-Updates sowie Verbrauchsartikel aus. Wichtige Hinweise Die Instandsetzung des Produkts darf nur durch Gebrüder Martin oder durch eine von Gebrüder Martin ausdrücklich dazu ermächtigte Person oder Firma durchgeführt werden. Erfolgt die Instandsetzung durch eine von Gebrüder Martin autorisierte Person oder Firma, so wird der Betreiber des Produkts aufgefordert, vom Instandsetzer eine Bescheinigung über Art und Umfang der Instandsetzung zu verlangen. Diese Bescheinigung muss das Datum der Instandsetzung sowie die Firmenangabe mit Unterschrift aufweisen. Falls die Instandsetzung nicht vom Hersteller selbst durchgeführt wird, müssen instand gesetzte Produkte zusätzlich das Kennzeichen des Instandsetzers erhalten. Bei unsachgemäßen Eingriffen oder Veränderungen durch Dritte während der Verjährungsfrist erlöschen jegliche Gewährleistungsansprüche. Nicht autorisierte Aktionen am Produkt führen zum Verlust des Haftungsanspruchs gegenüber Gebrüder Martin. 1.4 Eingangskontrolle Überprüfen Sie die Lieferung sofort nach Empfang auf Vollständigkeit und Unversehrtheit. V. 3.3 9 Gebrauchsanweisung RPE-Distraktoren 2 Hinweise zu diesem Dokument Bei Nichtbeachtung dieses Dokuments besteht die Möglichkeit einer schweren oder sogar tödlichen Verletzung des Patienten oder des Anwenders! Sorgen Sie dafür, dass die folgenden Hinweise verstanden und beachtet werden! • Jeder Anwender muss dieses Dokument vollständig lesen und beachten. • Insbesondere alle Vorsichts-, Warn- und Gefahrenhinweise beachten. • Dieses Dokument muss dem Anwender jederzeit zugänglich sein. • Der vorliegende Text bezieht sich gleichermaßen auf männliche und weibliche Personen. Diese Anweisung erhalten Sie mit Anlieferung der Osteosynthese-Schrauben. Unsachgemäße Handhabung und Pflege sowie zweckentfremdeter Gebrauch können zu vorzeitigem Verschleiß und / oder Risiken für Patient und Anwender führen! Sorgen Sie dafür, dass die folgenden Hinweise verstanden und beachtet werden! 10 V. 3.3 Gebrauchsanweisung RPE-Distraktoren 2.1 Symbolik in diesem Dokument Wichtige Informationen wie allgemeine oder sicherheitsrelevante Hinweise werden in diesem Dokument mit folgenden Symbolen und Signalwörtern gekennzeichnet: WARNUNG Todesgefahr oder Gefahr schwerer Körperverletzung! Bei Nichtbeachtung droht möglicherweise Tod oder schwere Körperverletzung! VORSICHT Gefahr der leichten Körperverletzung! Bei Nichtbeachtung droht Körperverletzung! HINWEIS Sachschadenrisiko! V. 3.3 11 Gebrauchsanweisung RPE-Distraktoren 3 Anwender • Das Distraktionssystem darf nur von medizinischem Fachpersonal angewendet werden, das aufgrund seiner fachlichen Ausbildung, den Kenntnissen, der Erfahrung sowie der Kenntnisse der einschlägigen Bestimmungen für den Einsatz dieses Systems qualifiziert ist. • Die Regeln der ärztlichen Kunst, der Stand der Wissenschaft und die Inhalte einschlägiger wissenschaftlicher Veröffentlichungen der medizinischen Autoren müssen dem Anwender bekannt sein. • Die Vermeidung bzw. Reduzierung allgemeiner Risiken bei chirurgischen Eingriffen obliegt dem Anwender. • Der Anwender: — trägt die Verantwortung für die sachgemäße Durchführung des chirurgischen Eingriffs — muss Operationstechnik, Implantatsortiment sowie Instrumentarium und Zubehör kennen und beherrschen — ist verantwortlich für die Zusammenstellung der Implantatkomponenten und deren sachgemäße Implantation HINWEIS Empfehlung zur leichten Rückverfolgung des eingesetzten Implantats. In der Patientenakte können die verwendeten Implantatkomponenten mit Artikelnummern, Artikelbezeichnung sowie Lot- und/oder Seriennummern dokumentiert werden. Nur die Dokumentation der Lot- und/oder Seriennummern gewährleistet die eindeutige Rückverfolgung der eingesetzten Implantate. 4 Implantatwerkstoffe Hinweise zu Implantatwerkstoffen finden Sie auf unserer Homepage: www.klsmartin.com 12 V. 3.3 Gebrauchsanweisung RPE-Distraktoren 5 Indikationen • Extreme transversale, uni- oder bilaterale Oberkieferengstände, bei Syndrom- und nicht Syndrompatienten. • Anteriore Zahnengstände und „buccal corridors” (dead spaces). Transversale Oberkieferengstände bei Jugendlichen und Erwachsenen kommen sowohl bei Syndrompatienten als auch bei nicht Syndrompatienten und Spaltpatienten häufig vor. Uniund bilaterale Oberkieferengstände können mit Hilfe der chirurgisch unterstützten Gaumennahtsprengung (SARPE) behandelt werden. Die Behandlung stellt eine Kombination aus Kieferorthopädie und chirurgischer Vorgehensweise dar. Sie ermöglicht eine verbesserte Ausrichtung von Zähnen und Dentalimplantaten am Zahnbogen. Das Verfahren ermöglicht außerdem auch eine erhebliche Vergrößerung der maxillären Wurzelbasis und der palatalen Wölbung. Der zur Verfügung stehende Raum für die Zunge und für korrektes Schlucken wird vergrößert, ein erneutes Zusammenfallen des Oberkiefers wird verhindert. Darüber hinaus kann eine eindeutige Verbesserung der nasalen Atmung, in Verbindung mit einer Vergrößerung der Nasenflügel hin zu normalen Werten beobachtet werden. Frühere Formen der Expansionsdistraktoren sind zahngetragen (beispielsweise die Hyraxschraube). Diese besitzen jedoch einige schwerwiegende Nachteile. • Zahnmedizinische Probleme, wie bukkale Wurzelresorption und Fenestration der Kortikalis. • Abkippen von ganzen Zahnsegmenten oder den Ankerzähnen • Zahnkaries bei Syndrompatienten mit schlechter Mundhygiene Im Gegensatz dazu werden knochengetragene Distraktoren an einer höheren Stelle in der palatalen Wölbung appliziert. Diese maxillären Expansionen sind orthopädisch und können mechanisch besser gesteuert werden. Außerdem wirken die Kräfte direkt auf den Knochen; ein Abkippen der Zähne oder andere unerwünschte Nebeneffekte werden verhindert. Die Entfernung des Distraktors nach abgeschlossener Distraktions- und Konsolidierungszeit kann sehr leicht und unter lokaler Anästhesie durchgeführt werden. V. 3.3 13 Gebrauchsanweisung RPE-Distraktoren 6 Kontraindikationen WARNUNG Bei Nichtbeachtung der Kontraindikationen besteht die Möglichkeit einer schweren oder sogar tödlichen Verletzung des Patienten! • Manifeste Infektionen • Verdacht auf Materialempfindlichkeit oder Allergien gegen die verwendeten Implantatwerkstoffe • Autoimmunerkrankungen • Durchblutungsstörungen, Systemerkrankungen und Stoffwechselstörungen • Unzureichendes quantitatives bzw. qualitatives Hart- und Weichgewebe • Knochentumore im Bereich der Implantatverankerung • Begleiterkrankungen, z. B. degenerative Krankheitsprozesse mit negativem Einfluss auf den Heilungserfolg • Verwendung in Bereichen, die nicht angemessenen Kraft- oder Gewichtsbelastungen ausgesetzt sind • Patienten, die aufgrund ihrer mentalen, neurologischen oder physischen Verfassung nicht bereit oder in der Lage sind, die Anweisungen für die postoperative Handhabung zu befolgen • Patienten mit schlechtem Allgemeinzustand im Sinne einer allgemeinen Kontraindikation (z. B. Bestrahlungspatienten) • Osteoporose oder Osteomalazie und andere schwerwiegende Schädigungen der Knochenstrukturen, die einer stabilen Verankerung der Implantatkomponenten entgegenstehen • Manifester Medikamenten-, Drogen- und / oder Alkoholmissbrauch • Extrem niedriger Gaumen: Im Falle eines niedrigen Gaumens verlieren die Abstützplatten Halt und der Distraktor verliert seine Stabilität. • Überbisse der Klasse II: Der Distraktor oder die Aktivierungsspindel könnten mit den Zähnen des Unterkiefers in Kontakt kommen. Dies kann vermieden werden, indem der Distraktor höher (distal) in der palatalen Wölbung angebracht wird oder indem während der Distraktions- und Konsolidierungsperiode eine Beißschiene getragen wird. 14 V. 3.3 Gebrauchsanweisung RPE-Distraktoren 7 Mögliche Nebenwirkungen und Komplikationen • Abnehmen der Knochendichte und / oder Knochennekrosen aufgrund von Belastung • Gefäßveränderungen • Luxationen und Subluxationen • Früh- und Spätinfektionen, Oberflächen- und / oder Tiefeninfektionen • Nerven- und Gefäßverletzungen • Hämatome und Wundheilungsstörungen • Periartikuläre Verkalkungen • Eingeschränkte Gelenkfunktion und Beweglichkeit • Eingeschränkte Gelenkbelastung und Gelenkschmerzen • Allergische Reaktion oder Empfindlichkeit gegen das Implantatmaterial • Nervenschäden durch chirurgisches Trauma • Unzureichende Knochenbildung oder verzögerte Knochenheilung, Osteoporose, Osteolyse, Osteomyelitis, gehemmte Revaskularisierung oder Infektionen, Knochennekrose, die zum Lockern, Verbiegen, Reißen oder Brechen des Distraktionssystems führen können • Nicht angemessene Kraft- oder Gewichtsbelastung kann zu Verschleiß, Bruch, Verbiegen, Lockerung oder Migration des Distraktionssystems führen. • Distraktionssysteme können nach der Konsolidierungsphase sich lockern, korrodieren, sich verschieben oder Schmerzen verursachen. Dies ist besonders bei jungen, körperlich aktiven Patienten der Fall. • Auch nach der Entfernung des Distraktionssystems muss der Patient eine entsprechende postoperative Betreuung erhalten, um eine Fraktur des neuen Knochengewebes zu vermeiden. • Wachstumseinschränkungen • Schmerzen, Beschwerden, anormale Empfindungen oder Tastbarkeit aufgrund des Implantats • Gesteigerte Reaktion des Weichgewebes im Umfeld der Osteotomie und / oder des Distraktionssystems • Unzulängliche Heilung • Gewebeverfärbungen aufgrund von Kontakt mit Implantatmaterial. • Fragmente und mikroskopisch kleine Partikel können sich vom Distraktionssystem lösen und sich vom Implantationssitus wegbewegen. Auch nach der Entfernung eines Metallimplantats kann es vorkommen, dass solche Metallpartikel im Körper verbleiben. Die langfristigen Wirkungen solcher Partikel sind derzeit nicht bekannt. Abgesehen von den o. g. Nebenwirkungen sind bei jedem chirurgischen Eingriff Komplikationen möglich, wie z. B. Infektionen, Nervenverletzungen und Schmerzen, die nicht auf das Distraktionssystem zurückzuführen sind. V. 3.3 15 Gebrauchsanweisung RPE-Distraktoren 8 Verpackung • Nach Anlieferung des Produktes muss die Originalverpackung und das Verpackungssiegel auf Unversehrtheit geprüft werden. Verpackungen mit aufgebrochenem Siegel sind von der Rücknahme ausgeschlossen. • Distraktionssysteme, die nicht steril verpackt sind, müssen zur Aufbereitung aus der Verpackung genommen werden. • Sterilverpackte Implantate können ohne weitere Vorbehandlung direkt aus ihrer Sterilverpackung appliziert werden. • Sterilimplantate mit (auch versehentlich) geöffneter oder beschädigter Sterilverpackung, sowie Implantate, die aus Verpackungen mit abgelaufenem Verfallsdatum entnommen wurden, müssen als unsteril angesehen werden und sind vor ihrem Einsatz einem validierten Aufbereitungsverfahren zu unterziehen. 9 Aufbau und Funktionsweise des Distraktionssystems Der RPE-Distraktor (RPE = Rapid Palatal Expander) von KLS Martin ist ein knochengetragener Distraktor. Er kann sehr leicht appliziert und aktiviert werden. Die Funktionsweise beruht auf zwei gegenläufigen Gewinden mit identischer Steigung. Beim Aktivieren des Distraktors bohren sich die 2 mm langen Pins der Abstützplatte direkt in den Knochen, die Vorrichtung wird selbständig stabilisiert. Eine zusätzliche Fixierung des Distraktors am Knochen mit Hilfe der beiden mitgelieferten maxDrive®-Drill-Free-Schrauben ist erforderlich. Nach jeder Aktivierung / Distraktion muss der Distraktor durch Festdrehen der grauen Gegenmutter (3) blockiert werden, damit dieser vor unerwünschtem Verdrehen gesichert ist. (Siehe Übersicht Typ-Distraktoren, Seite 6). 10 Auswahl des Distraktionssystems WARNUNG Kombination verschiedener Materialien oder Systeme vermeiden! Distraktionssysteme von KLS Martin dürfen nicht mit Systemen anderer Hersteller kombiniert werden. Werden Instrumente verwendet, die nicht spezifisch für den Gebrauch in Verbindung mit Implantaten vorgesehen sind, kann es zu einem Versagen des Distraktionssystems kommen. 16 V. 3.3 Gebrauchsanweisung RPE-Distraktoren 11 Aufbereitung, Reinigung, Desinfektion und Sterilisation Distraktionssysteme sind für eine maschinelle Aufbereitung / thermische Desinfektion geeignet. Sie können mit den Programmabläufen aufbereitet werden, die für chirurgisches Instrumentarium freigegeben sind. Für die Reinigung sind die Angaben der Hersteller der Reinigungsautomaten sowie der Hersteller der Behandlungsmittel zu beachten. Der Prozess inklusive der Beladung muss die ausreichende Entfernung von Rückständen gewährleisten. WARNUNG Infektionsgefahr durch unsterile Handhabung! Unsachgemäße Sterilisation sowie unsterile Handhabung des Distraktionssystems kann zu schweren gesundheitlichen Risiken für den Patienten führen. Die Sterilisation hat nach einem validierten Dampfsterilisationsverfahren, z. B. in einem Sterilisator gemäß EN 285:2009 und validiert gemäß ISO 17665-1:2006, zu erfolgen. HINWEIS Die Verantwortung für die Reinigung, Desinfektion und Sterilisation von Distraktionskomponenten liegt beim Betreiber / Produktanwender. Nationale Regelungen, auch Einschränkungen hierzu, müssen unbedingt beachtet werden. Für die Sterilisation, den nachfolgenden Transport und die Lagerung sind entsprechende zugelassene Sterilisierverpackungen (z. B. gemäß EN 868, ISO 11607) einzusetzen. Kontaminierte und / oder benutzte Implantat-Komponenten dürfen nicht wieder verwendet und deshalb auch nicht wieder aufbereitet werden. V. 3.3 17 Gebrauchsanweisung RPE-Distraktoren 12 Vor der Implantation Hinweise • Distraktionssysteme müssen vor ihrer Anwendung den gesamten Aufbereitungsgang geeigneter Reinigungs-, Desinfektions- und Sterilisationsverfahren durchlaufen. • Sterilverpackte Implantate können ohne weitere Vorbehandlung direkt aus ihrer Sterilverpackung appliziert werden. • Um zu vermeiden, dass die Verpackung nicht passender steriler Implantate geöffnet wird, kann der Einsatz von Schablonen sehr hilfreich sein, um sich vor Öffnung der Schutzverpackungen mit den anatomischen Gegebenheiten am OP-Situs vertraut zu machen und um die Einschätzung der knochenspezifischen Abmessungen im Vorfeld zu überprüfen. Schablonen für dieses Distraktionssystem sind zu diesem Zweck bei KLS Martin erhältlich. • Vor der Implantation ist das Distraktionssystem einer Sichtprüfung und einer Probeaktivierung zu unterziehen: Stellen Sie sicher, dass sich der Aktivierungsspindel (2) des Distraktionssystems drehen lässt (Abbildung siehe Übersicht Typ-Distraktoren, Seite 6), und dass sich der Distraktor dabei korrekt bewegt. • Vor der Implantation empfiehlt sich die Durchführung eines computertomographischen 3D-Scans und / oder die Herstellung eines stereolithografischen Modells, um die optimale Position des Distraktionssystems zu ermitteln bzw. Vor-ab-Adaptionen vorzunehmen. Individuelle anatomische Modelle können bei KLS Martin bestellt werden (www.klsmartin.com). HINWEIS Mögliche Fehlinterpretation der Untersuchung! Distraktionssysteme können sowohl bei der Computertomografie (CT) als auch bei der Magnet-Resonanz-Tomografie (MRT) störende Artefakte verursachen. WARNUNG Gefahr der Verbrennung bzw. unbeabsichtigten Bewegung des Implantats beim Einsatz von MRT! Aufgrund der Weiterentwicklung und weiter steigenden Energiedichte der MRT-Geräte ist ein negativer Einfluss auf Implantate zukünftig nicht auszuschließen. Im Zweifelsfall ist eine MRT-Untersuchung vor dem Ausschluss einer möglichen Patientenschädigung nicht zulässig. • Bei der MRT besteht die Gefahr des Erhitzens und der Bewegung von möglicherweise auf das Magnetfeld reagierenden Implantaten durch das Magnetfeld. • Bei externen Fixateuren aus Stahl ist die MRT-Kompatibilität im Einzelfall zu überprüfen. • Stahlimplantate nach ISO 5832-1 werden als para- oder unmagnetisch eingestuft. • Titan ist vollständig unmagnetisch. 18 V. 3.3 Gebrauchsanweisung RPE-Distraktoren 13 Intraoperatives Vorgehen Die Operation wird unter Vollnarkose, vorzugsweise mit endotrachealer Intubation durchgeführt. Standardkortikotomien der anterioren, lateralen und medialen Maxilla werden durchgeführt. Im Falle einer posterioren (parallelen) Expansion sollte zusätzlich eine Trennung im pterygomaxillären Bereich (posteriore Osteotomie) durchgeführt werden. Die Durchtrennung der Nasenscheidewand wird unter Ärzten kontrovers diskutiert. Die Schleimhaut der Prämolaren wird mit einem Anästhetikum und einem Vasokonstringenz injiziert. Der RPE-Distraktor wird mit Abstützplatten an der Mukosa über den Wurzeln der zweiten Prämolaren (im Falle einer anterioren (3:2) Expansion) und der ersten Molaren (im Falle einer posterioren Expansion) befestigt. Die Aktivierungsspindel befindet sich mittig, sie darf nicht mit der Zahnokklusion zusammenstoßen. Der Distraktor wird leicht aktiviert, damit die Abdrücke der Pins deutlich auf der Mukosa sichtbar sind. Die Mukosa an der anterioren okklusalen Seite wird nun eingeschnitten. Der Distraktor wird deaktiviert und wieder entfernt. Die Gaumenschleimhaut wird entfernt, dabei wird eine örtliche Hämostase durchgeführt. Der RPE-Distraktor wird nun wieder, diesmal jedoch mit den Abstützplatten direkt am Knochen, appliziert. Der Distraktor wird dabei leicht aktiviert, so dass die Plattenpins in den Knochen eindringen können. Es gilt dabei zu beachten, dass die Osteosyntheselöcher der Abstützplatte nach anterior ausgerichtet sind. Nun wird eine zusätzliche Sicherung vorgenommen, indem die beiden mitgelieferten Drill-FreeSchrauben durch die Löcher der Abstützplatten in den Knochen eingeschraubt werden. Um etwaigen Bewegungen am Distraktionskörper während der Konsolidierungsphase entgegenzuwirken, ist es erforderlich, die Gegenmutter festzuziehen. V. 3.3 19 Gebrauchsanweisung RPE-Distraktoren WARNUNG Gefahr des Systemversagens durch unsachgemäße Handhabung! Unsachgemäße Handhabung (Bearbeitung) des Distraktionssystems durch den Anwender kann zu Systemversagen und Verletzungen führen! Beachten Sie daher: • Das Distraktionssystem darf nie gebogen oder ankonturiert werden. • In Fällen, bei denen die maximale Distraktionslänge angestrebt wird, muss vor dem Abtrennen des Aktivators am Ende der kompletten Distraktion der Aktivator um eine halbe Umdrehung zurückgedreht werden. Diese Vorgehensweise vermeidet eine Beschädigung der Aktivierungsspindel durch versehentliches Drehen beim Trennvorgang. WARNUNG Erstickungsgefahr! Löst sich der Distraktor, so besteht die Gefahr den Distraktor versehentlich zu verschlucken oder zu inhalieren. Nach erfolgter Implantation den Distraktor zusätzlich mit Hilfe der beiden mitgelieferten DrillFree-Schrauben sichern. Dazu die Drill-Free-Schrauben durch die Löcher der Abstützplat-ten in den Knochen schrauben. WARNUNG Gefahr der unzureichenden Mobilisierung der Knochensegmente! Eine unzureichende Mobilisierung führt zu einer Erhöhung der Kräfte am Distraktionssystem, die ein Verbiegen bzw. den Bruch des Distraktionssystems zur Folge haben können. Beachten Sie daher: • Die zu distrahierenden Knochensegmente müssen vor der Distraktion mobilisiert werden (Kortikotomie bzw. Komplettosteotomie). Zur Prüfung der Mobilisierung muss am implantierten Distraktionssystem intraoperativ eine Probeaktivierung von mindestens 1 – 3 mm durchgeführt werden, die einen durchgehenden Osteotomiespalt aufweist. • Das Distraktionssystem darf sich während der Distraktion nicht verbiegen. 20 V. 3.3 Gebrauchsanweisung RPE-Distraktoren 14 Vor der Distraktion (Latenzphase) • Für die erstmalige Stabilisierung muss der RPE-Distraktor leicht aktiviert werden. Bitte beachten Sie dabei, dass beide Platten von der Grundstellung parallel aufdistrahieren und diese nicht einseitig ausgefahren werden. • Nach erfolgter Probeaktivierung den Distraktor wieder in die komplett zurückgedrehte Ausgangsposition drehen. Aussagen zum Distraktionsprotokoll sind in der Literatur sehr heterogen beschrieben, sehen aber üblicherweise nachfolgende Parameter vor. Abweichend dazu können therapeutische oder patientenspezifische Faktoren zu Änderungen des Distraktionsprotokoll führen, z. B. Alter, Anamnese, Weichgewebssituation, Bezahnung bzw. kieferorthopädische Behandlungskonzepte etc. Nach der Implantation eines Distraktionssystems ist zuerst eine Latenzphase einzuhalten, bevor die eigentliche Distraktionsphase beginnt. Die Latenzphase ist indikationsbedingt vom Operateur zu beurteilen und beträgt ca. 5 – 7 Tage (abhängig von Patient und Indikation). V. 3.3 21 Gebrauchsanweisung RPE-Distraktoren 15 • Während der Distraktion (Distraktionsphase) Der Distraktor kann sehr einfach mit Hilfe des Aktivierungsschlüssels (Art. Nr. 1-565-90-07 oder Art. Nr. 51-565-95-07) aktiviert werden. Eine Drehung um 120° (bis zur nächsten Farbkodierung) entspricht einem Distraktionsweg von 0,33 mm. Pro Tag wird eine Umdrehung von 120 – 240 ° empfohlen. Dies entspricht einem Distraktionsweg von 0,33 – 0,66 mm pro Tag. Der Distraktionsfortschritt muss durch eine fortlaufende ärztliche Kontrolle überwacht werden. Zum Beispiel kann ein zu großer Weichgewebszug zu einer Verformung des Distraktionssystems führen. Diese mögliche Verformung ist durch regelmäßige Röntgenkontrollen zu kontrollieren. Wird trotz Verformung die Distraktion fortgeführt, kann dies zu einem Überlastungsbruch bzw. zu einem Ausreißen der Schrauben aus dem Knochen führen. WARNUNG Gefahr des Systemversagens durch patientenseitige falsche Handhabung! Falsche Handhabung des Distraktionssystems durch den Patienten kann zu Systemdefekt oder Verletzung des Patienten führen. Beachten Sie daher: • Patient unterweisen und postoperativ betreuen. • Das Distraktionssystem darf nicht verbogen oder in anderer Form verändert werden. • Während der kompletten Behandlungsdauer sind alle körperlichen Aktivitäten, welche das Distraktionssystem oder die mit diesem verbundenen Knochenelemente beeinträchtigen könnten, zu unterlassen. Dies gilt insbesondere für das Tragen von Lasten (Sturzgefahr) und sportliche Betätigungen, die einen Stoß oder Schlag verursachen könnten. • Durch protektive Maßnahmen (wie Schutzmanschette, Schiene, Helm etc.) ist sicherzustellen, dass der Distraktionssitus nicht überbelastet wird (Schlafphase eingeschlossen). • Korrekte Aktivierungsrichtung (Richtungs-Pfeile auf dem Distraktionskorpus) beachten. • Nach jeder Aktivierung / Distraktion muss der Distraktor durch Festdrehen der grauen Gegenmutter blockiert werden, damit dieser vor unerwünschtem Verdrehen gesichert ist. 22 V. 3.3 Gebrauchsanweisung RPE-Distraktoren 16 Nach Abschluss der Distraktionsphase (Konsolidierungsphase) Hinweise • Die Konsolidierungsphase beträgt ca. 3 – 6 Monate, abhängig vom Patienten. • Längere Konsolidierungsphasen helfen das Distraktionsergebnis zu stabilisieren und ein Rezidiv zu vermeiden. • Das Distraktionssystem darf erst nach vollständiger knöcherner Durchbauung (Konsolidierung) entfernt werden, um das Distraktionsergebnis nicht zu gefährden. • Die physiotherapeutische Behandlung kann schon während der Konsolidierungsphase aufgenommen werden, um eine Adduktionskontraktur zu vermeiden. • Die kieferorthopädische Behandlung kann schon während der Konsolidierungsphase aufgenommen werden. • Nach Abschluss der Konsolidierungsphase sind sämtliche Distraktorkomponenten zu entfernen. • Art und Umfang der postoperativen Behandlung und / oder Rehabilitationsmaßnahmen sind vom behandelnden Arzt mit dem Patienten abzustimmen. WARNUNG Mögliches Systemversagen durch Wiederverwendung! Beschädigte oder gebrauchte Distraktionssysteme können zu Verletzungen führen und dürfen nicht wieder verwendet werden. Only documentation of the batch and / or serial numbers ensures clear traceability of the implants used. 4 Implant Materials For implant material details, please refer to our website: www.klsmartin.com V. 3.3 31 Instructions for Use RPE Distractors 5 Indications • Extreme transversal, unilateral or bilateral maxilla crowdings in syndrome and nonsyndrome patients. • Anterior tooth crowdings and buccal corridors (dead spaces). Transversal maxilla crowdings in adolescents and adults often occur in syndrome patients as well as in non-syndrome and cleft patients. Unilateral and bilateral maxilla crowdings can be treated by surgically assisted rapid palatal expansion (SARPE). The treatment is a combination of orthodontics and surgical procedure. It allows better alignment of teeth and dental implants with the dental arch. The procedure moreover allows significant expansion of the maxillary root base and the palatal arch. The available space for the tongue and for correct deglutition is expanded, and another collapse of the maxilla can be avoided. Furthermore, clear improvement of nasal respiration in connection with expansion of the nasal alae towards normal values can be observed. Earlier version of expansion distractors are tooth-supported (for example the Hyrax screw). However, they have some severe disadvantages. • Dental problems such as buccal root resorption and fenestration of the corticalis. • Tilting of complete tooth segments or anchor teeth. • Dental caries in syndrome patients with poor oral hygiene. By contrast, bone-supported distractors are applied at a higher location of the palatal arch. These maxillary expansions are orthopedically appropriate and can be better controlled mechanically. Moreover, the forces are directly applied to the bone; tilting of the teeth or other undesired side effects can be avoided. The removal of the distractor after the distraction and consolidation period can be done easily under local anesthesia. 32 V. 3.3 Instructions for Use RPE Distractors 6 Contraindications WARNING Non-observance of the contraindications poses a danger of serious or even lethal patient injury! • Obvious infections • Suspected sensitivity or allergies to the implant materials used • Autoimmune diseases • Circulatory problems, systemic diseases and metabolic disorders • Insufficient or inadequate hard and soft tissue • Bone tumors located in the implant base region • Secondary diseases such as degenerative processes that may adversely affect the healing process • Regions exposed to inadequately high forces or excessive weight loads • Patients unwilling or unable to follow instructions during the postoperative phase due to their mental, neurological or physical condition • Patients in a poor condition of health in the sense of a general contraindication (e.g. irradiation patients) • Osteoporosis or osteomalacia or other severe structural bone damage that prevents the stable fixation of implant components • Obvious drug or alcohol abuse • Extremely low palate: In case of an extremely low palate, the supporting plates of the distractor lose their grip, and the distractor loses its stability. • Class II overbites: The distractor or the activation spindle can come into contact with the mandible. This can be avoided by mounting the distractor higher in the palatal arc in distal direction or by wearing a mouth-guard during the distraction and consolidation period. V. 3.3 33 Instructions for Use RPE Distractors 7 Potential Side Effects and Complications • Reduced bone density and/or bone necroses due to mechanical stress • Vascular changes • Dislocations and partial dislocations • Early and late infections, surface and/or deep infections • Nerve and vascular injuries • Hematomas and slow wound healing • Periarticular calcification • Reduced joint function and mobility • Reduced joint load-bearing capacity; joint pain • Allergic response or sensitivity to the implant material • Nerve damage due to surgical trauma • Insufficient ossification or delayed bone healing, osteoporosis, osteolysis, osteomyelitis, slow revascularization, infections or bone necroses, all of which may cause the distraction system to loosen, crack, fracture or become bent. • Inadequately high forces or weight loads may lead to wear, fracture, deformation, loosening or migration of the distraction system. • After completion of the consolidation phase, distraction systems may come loose, corrode, dislocate or cause pain, especially in young, physically active patients. • To prevent fracture of the new bone tissue, it is important to provide the patient with adequate postoperative care even after removal of the distraction system. • Growth restriction • Pain, discomfort, anomalous sensations caused by the implant; implant palpability • Increased reaction of the soft tissue around the osteotomy and/or the distraction system • Poor healing • Tissue discoloration due to contact with the implant material • Fragments and microscopically small particles may come off the distraction system, migrating away from the implantation site. These metal particles may remain in the patient’s body even after removing the implant. The long-term effects of such particles are presently unknown. Apart from the side effects described above, it should be noted that any surgical intervention can lead to complications not necessarily caused by the distraction system (such as infections, nerve injuries and pain). 34 V. 3.3 Instructions for Use RPE Distractors 8 Packaging • Upon receipt of the product, the original packaging and its seal must be checked for integrity. Packages with a broken seal cannot be taken back. • Distraction systems delivered non-sterile must be removed from their packaging before processing them for use. • Sterile-packed implants can be applied immediately after removing them from their sterile packaging, no pretreatment required. • Sterile implants whose sterile packaging has been opened or damaged for any reason, as well as implants taken from expired packages, must be considered non-sterile and have to be sent through a validated reprocessing cycle prior to use. 9 Design and Working Principle of the Distraction System The RPE Distractor (RPE = Rapid Palatal Expander) by KLS Martin is a bone-supported distractor. It can be easily applied and activated. Its mode of operation is based on two threads with identical pitch that run in opposite directions. When the distractor is activated, the 2-mm-long pins of the supporting plates drill directly into the bone, and the system stabilizes automatically. Additional fixation of the distractor at the bone using the two supplied maxDrive® Drill Free screws is required. After every activation / distraction, the distractor must be blocked by tightening the gray lock nut (3) so that it is secured against unintentional twisting. (See Overview of Typ distractors, page 6). 10 Selecting the Distraction System WARNING Do not use different materials or systems in combination! KLS Martin distraction systems must not be combined with systems from other manufacturers! Using instruments not specifically intended for use with implants can result in failure of the distraction system! V. 3.3 35 Instructions for Use RPE Distractors 11 Processing, Cleaning, Disinfection and Sterilization Distraction systems are suitable for machine processing/thermal disinfection. They can be processed with the same programs that have been released for surgical instruments. As regards cleaning, be sure to follow the instructions provided by the manufacturer of your washer-disinfector(s) as well as those provided by the manufacturers of the cleaners and disinfectants used. The process (including loading) must guarantee sufficient removal of residues. WARNING Danger of infection due to non-sterile handling! Improper sterilization and non-sterile handling of the distraction system can lead to serious health hazards for patients. Sterilization must be carried out according to a validated steam sterilization process, for example in a sterilizer complying with EN 285:2009 and ANSI/AAMI ST 79, validated in accordance with ISO 17665-1:2006. ANSI/AAMI ST 79 recommends a minimum cycle time of 4 minutes at 132 °C (270 °F) for dynamic air removal steam sterilization cycles. Please follow the instructions of the user manual of your steam sterilizer. NOTICE The responsibility for proper cleaning, disinfection and sterilization of distraction components rests with the operator / product user. Be sure to observe your national/local regulations, including potential restrictions. Use appropriate and approved sterile packaging (e.g. in accordance with EN 868, ISO 11607) for sterilization, subsequent transportation and storage. Contaminated and / or used implant components may never be reused and therefore must not be reprocessed. 36 V. 3.3 Instructions for Use RPE Distractors 12 Prior to Implantation Notices • Prior to use, distraction systems must be sent through the entire processing cycle of suitable cleaning, disinfecting and sterilization procedures. • Sterile-packed implants can be applied immediately after removing them from their sterile packaging, no pretreatment required. • To prevent the opening of packages containing non-suitable implants, the use of templates can be very helpful because they allow you to check the anatomical conditions of the surgical site and verify bone dimensions prior to opening the protective packaging. Templates for the distraction system are available from KLS Martin. • Prior to implantation, the distraction system must be visually inspected and an activation test must be carried out: Ensure that the activation spindle (2) of the distraction system can be turned (see illustration in Overview of Typ distractors, page 6) and that the distractor moves correctly when doing so. • We also recommend you to perform a 3-D CT scan and/or have a stereolithographic model created prior to implantation to determine the optimal position for the distraction system or make adjustments in advance. Patient-specific anatomical models are available from KLS Martin on request (www.klsmartin.com). NOTICE Potential misinterpretation of examination results! Distraction systems can cause irritating artifacts in CT scans and MRIs. WARNING Danger of burns or accidental implant movement when using magnetic resonance imaging (MRIs)! Due to the further development and increasingly higher energy density of MRI systems, an adverse effect on implants cannot be ruled out in the future. Therefore, MRIs are not permitted unless potential patient injury can definitely be ruled out. • Due to the magnetic field, MRIs pose a danger of heating up or dislocating potentially susceptible implants. • For external fixators made of steel, MRI compatibility must be checked in each individual case. • Steel implants acc. to ISO 5832-1 are classified as paramagnetic or non-magnetic. • Titanium is completely non-magnetic. V. 3.3 37 Instructions for Use RPE Distractors 13 Intraoperative Procedure The surgery is performed under general anesthesia, preferably with endotracheal intubation. Standard corticotomies of the anterior, lateral and medial maxilla are carried out. In cases of a posterior (parallel) expansion, an additional separation should be performed in the pterygomaxillary area (posterior osteotomy). There is some controversy among physicians regarding the severing of the nasal septum. The mucus membrane of the premolar teeth is injected with an anesthetic and a vasoconstrictor. The RPE Distractor is fixed at the mucosa above the roots of the second premolar teeth (in cases of anterior (3:2) expansion) and the first molar teeth (in cases of posterior expansion). The activation spindle sits in the middle and must not come into contact with the dental occlusion. The distractor is activated slightly so that the marks of the pins are clearly visible on the mucosa. Now the mucosa on the anterior occlusal side is cut into. The distractor is deactivated and removed. The palate mucosa is removed and a local hemostasis is carried out. The RPE Distractor is applied again. This time, however, the supporting plates sit directly on the bone. The distractor is activated to a degree that the plate pins can penetrate into the bone. Make sure that the osteosynthesis holes of the supporting plates are aligned in anterior direction. Now the distractor is secured additionally by screwing in both supplied Drill Free screws through the holes of the supporting plates into the bone. To avoid any movement on the distraction body during the consolidation phase, the lock nut must be tightened properly. 38 V. 3.3 Instructions for Use RPE Distractors WARNING Danger of system failure due to improper handling! Improper handling (non-permitted adaptation) of the distraction system by the user can lead to failure of the system and injury! Therefore, please note: • NEVER bend or manipulate the distractor body in an attempt to adapt it to bone contours! • In cases where the maximum distraction length is used, the activator must be rotated backwards by half a turn before cutting if off once the distraction process has been completed. This prevents damage to the activation spindle due to unintended rotation during the process of separation. WARNING Danger of asphyxiation! If the distractor loosens, there is danger of accidentally swallowing or inhaling the distractor. Therefore, after successful implantation secure the distractor additionally using the two supplied Drill Free screws. Screw in the Drill Free screws through the holes of the supporting plates into the bone. WARNING Danger of insufficient bone segment mobilization! Insufficient mobilization will result in higher forces acting upon the distraction system, which could lead to deformation or fracture of the distraction system. Therefore, please note: • The bone segments to be distracted must be mobilized prior to distraction (corticotomy or complete osteotomy). To verify sufficient mobilization, an activation test must be performed intraoperatively on the implanted distraction system. The test activation must cover at least 1 – 3 mm and a complete osteotomy gap must be produced in the process. • Deformation of the distraction system must be prevented during the distraction process. V. 3.3 39 Instructions for Use RPE Distractors 14 Prior to Distraction (Latency Phase) • For initial stabilization, the RPE Distractor must be slightly activated. Please make sure that both plates distract in parallel from the initial position and are not extended unilaterally. • After test activation, fully turn the distractor back into the initial position. The distraction protocol iscommented on very heterogeneously in the literature. However, the following parameters are usually used. In deviation from this, therapeutic or patient-specific factors can lead to changes in the distraction protocol, e. g. regarding age, anamnesis, situation of the soft tissue, dentition and orthodontic treatment concepts, etc. Once the distraction system has been implanted, a latency period must be observed before starting the distraction process. The length of the latency period depends on the indication and must be determined by the surgeon. It usually covers approx. 5 – 7 days (depending on patient and indication). 40 V. 3.3 Instructions for Use RPE Distractors 15 • During Distraction (Distraction Phase) The distractor can be easily activated using the activation key (item no. 515659007 or item no. 515659507). Rotation by 120° (up to the next color code) corresponds to a distraction path of 0.33 mm. We recommend one rotation of 120 – 240° per day. This corresponds to a distraction path of 0.33 – 0.66 mm per day. The distraction progress must be monitored by regular medical checks. For example, excessive soft-tissue pull can lead to deformation of the distraction system. Such potential deformation must be monitored by regular X-ray checks. If the distraction process is continued in spite of deformation, this could lead to fracture due to overload or the screws being torn out of the bone. WARNING Danger of system failure due to wrong handling by patient! Improper handling of the distraction system by the patient can lead to a system defect or patient injury. Therefore, heed the following: • Be sure to provide adequate patient instruction and postoperative care. • The distraction system may never be bent or modified in any other way. • All physical activities which could adversely affect the distraction system or bone sections connected with it must be strictly avoided throughout the treatment. This particularly applies to load carrying (danger of fall) and sports posing a physical impact risk (blow or knock). • Suitable measures must be taken (such as wearing a protective cuff, splint or helmet) to prevent the distraction site from overload (also during sleep). • Observe correct direction of activation (see direction arrows on the distraction corpus). • After every activation / distraction, the distractor must be blocked by tightening the gray lock nut so that it is secured against unintentional twisting. V. 3.3 41 Instructions for Use RPE Distractors 16 After Distraction (Consolidation Phase) Notices • The consolidation phase takes approx. 3 – 6 months, depending on patient. • In order not to jeopardize the distraction result, the distraction system must be left in situ until complete osseous consolidation (“knitting”) has been achieved. • The activator should be removed after completion of the distraction phase. • To prevent an adduction contracture, physiotherapy can already be started during the consolidation phase. • Orthodontic treatment can already be started during the consolidation phase. • Upon completion of the consolidation phase, all distractor components must be removed. • The type and scope of the postoperative therapy and/or rehabilitation measures must be determined by the physician according to patient needs. WARNING Possible system failure in case of reuse! V. 3.3 49 Instrucciones de uso Distractores RPE 3 Usuarios • El sistema de distracción únicamente deberá ser utilizado por personal médico especializado que, debido a su formación específica, su experiencia y su conocimiento de las disposiciones vigentes aplicables estén cualificados para la utilización del sistema. • El usuario deberá conocer las reglas de la práctica médica, así como el estado de la ciencia y los contenidos de publicaciones científicas pertinentes de autores médicos sobre la intervención. • La prevención o la reducción de los riesgos generales inherentes a las intervenciones quirúrgicas incumbe al usuario. • El usuario: — es responsable de realizar la intervención quirúrgica conforme a las reglas del arte; — deberá conocer y dominar la técnica quirúrgica, el surtido de implantes, así como el instrumental y los accesorios; — es responsable de la combinación de los componentes del implante y de su implantación conforme a las prescripciones. INDICACIÓN Recomendación para un seguimiento sencillo del implante insertado En la historia clínica del paciente pueden documentarse los componentes del implante utilizados con los números de artículo, las denominaciones y los números de lote y de serie. Solo la documentación de los números de lote y de serie garantiza el seguimiento inequívoco de los implantes insertados. 4 Materiales del implante Encontrará indicaciones sobre los materiales de los implantes en nuestro sitio web: www.klsmartin.com 50 V. 3.3 Instrucciones de uso Distractores RPE 5 Indicaciones • Estrecheces transversales unilaterales o bilaterales extremas del maxilar superior en pacientes con y sin síndrome. • Estrecheces dentales anteriores y corredores bucales (espacios muertos). Las estrecheces transversales del maxilar superior en adolescentes y adultos, suelen ser frecuentes tanto en pacientes con y sin síndrome como en pacientes con labio leporino. Las estrecheces unilaterales y bilaterales del maxilar superior pueden tratarse por medio de la expansión rápida de paladar asistida quirúrgicamente (SARPE). El tratamiento representa una combinación de odontología y procedimiento quirúrgico. Posibilita una alineación mejorada de las piezas dentales y de los implantes dentales en el arco dental. Asimismo, el procedimiento permite una ampliación considerable de la base radicular maxilar y de la bóveda palatina. De esta manera, también se amplía el espacio disponible para la lengua y para una deglución correcta y se evita un nuevo colapso del maxilar superior. Además, puede observarse una clara mejora de la respiración nasal asociada con la ampliación de las aletas nasales hasta alcanzar los valores normales. Las formas anteriores de los distractores de expansión son de anclaje dental (por ejemplo los tornillos Hyrax). No obstante, poseen algunas desventajas graves. • Problemas odontológicos tales como reabsorción radicular bucal y fenestración cortical. • Inclinación de segmentos dentales completos o de los dientes de anclaje. • Caries en pacientes con síndrome y una higiene bucal deficiente. Por el contrario, los distractores de anclaje óseo se aplican en una posición superior en la bóveda palatina. Estas expansiones maxilares son ortopédicas y pueden controlarse mejor mecánicamente. Además, las fuerzas actúan directamente sobre los huesos, por lo que se impide la inclinación de los dientes u otros efectos secundarios no deseados. La extracción del distractor una vez finalizado el periodo de distracción y consolidación puede realizarse de forma muy sencilla y con anestesia local. V. 3.3 51 Instrucciones de uso Distractores RPE 6 Contraindicaciones ADVERTENCIA ¡En caso de no tener en cuenta las contraindicaciones existe la posibilidad de una lesión grave o incluso mortal para el paciente! • Infecciones manifiestas. • Sospecha de sensibilidad o alergias contra los materiales utilizados en la fabricación de los implantes. • Enfermedades autoinmunes. • Trastornos circulatorios, enfermedades sistémicas y anomalías metabólicas. • Insuficiencia cuantitativa o cualitativa de tejidos compactos o blandos. • Tumores óseos en la zona de anclaje del implante. • Enfermedades secundarias, p. ej. procesos patológicos degenerativos con efecto negativo sobre el éxito curativo. • Utilización en sectores que no están expuestos a cargas de fuerza o de peso adecuados. • Pacientes que, debido a su estado mental, neurológico o físico, no estén dispuestos o no sean capaces de seguir las indicaciones para el manejo postoperatorio. • Pacientes con estado general deficiente en el sentido de una contraindicación generalizada (p. ej. pacientes sometidos a radioterapia). • Osteoporosis u osteomalacosis y otros deterioros graves de las estructuras óseas, que evitan un anclaje estable de los componentes del implante. • Abuso manifiesto de medicamentos, drogas y/o alcohol. • Paladares extremadamente bajos: en estos casos, las placas de sostén pierden su apoyo y el distractor pierde estabilidad. • Supraoclusión de clase II: el distractor o el huso de activación podrían entrar en contacto con los dientes del maxilar inferior. Esta circunstancia puede impedirse colocando el distractor en una posición superior (distal) en la bóveda palatina o utilizando una férula oclusal durante el periodo de distracción y consolidación. 52 V. 3.3 Instrucciones de uso Distractores RPE 7 Posibles efectos secundarios y complicaciones • Reducción de la densidad ósea y/o osteonecrosis debido a cargas. • Modificaciones vasculares. • Luxaciones y subluxaciones. • Infecciones primarias y secundarias, infecciones superficiales y/o profundas. • Traumatismos nerviosos y vasculares. • Hematomas y alteraciones en la cicatrización de heridas. • Calcificaciones periarticulares. • Función articular y motilidad limitada. • Carga articular limitada y dolores articulares. • Reacción alérgica o sensibilidad frente al material del implante. • Lesiones nerviosas por traumatismos quirúrgicos. • Osificación insuficiente o regeneración ósea retardada, osteoporosis, osteolisis, osteomielitis, revascularización inhibida o infecciones, osteonecrosis, que puedan llegar a aflojar, deformar, desgarrar o fracturar el sistema de distracción. • Las cargas de fuerza o de peso inadecuadas pueden conducir al desgaste, la rotura, la deformación, el aflojamiento o la migración del sistema de distracción. • Los sistemas de distracción pueden aflojarse, oxidarse, desplazarse o causar dolores después de la fase de consolidación. Esto se da sobre todo en el caso de pacientes jóvenes, físicamente activos. • Incluso después de eliminar el sistema de distracción, el paciente deberá obtener la correspondiente asistencia médica postoperatoria para evitar una fractura del tejido óseo de nueva formación. • Limitación del crecimiento. • Dolores, molestias, sensaciones o palpabilidad anómala debidos al implante. • Reacción incrementada del tejido blando en el entorno de la osteotomía y/o del sistema de distracción. • Curación insuficiente. • Alteraciones cromáticas del tejido debido al contacto con el material del implante. • Existe la posibilidad de un desprendimiento de fragmentos y partículas de tamaño microscópico del sistema de distracción, pudiendo estos llegar a desplazarse del lugar de implantación. Esta es la razón por la que pueden permanecer partículas metálicas dentro del cuerpo incluso después de haber eliminado el implante metálico. Hasta el momento se desconocen los efectos a largo plazo de estas partículas. A parte de los efectos secundarios nombrados anteriormente, en cada intervención quirúrgica pueden darse complicaciones como, por ejemplo, infecciones, lesiones nerviosas y dolores, que no son atribuibles al sistema de distracción. V. 3.3 53 Instrucciones de uso Distractores RPE 8 Embalaje • Después de la recepción del producto deberá controlarse que el embalaje original y el precinto del mismo no estén dañados. Los embalajes que tengan el precinto roto no podrán retornarse. • Los sistemas de distracción que no estén embalados en estado estéril, deberán extraerse del embalaje para su tratamiento previo. • Los implantes embalados en estado estéril pueden aplicarse directamente después de extraerlos del embalaje estéril sin ningún tratamiento previo. • Los implantes estériles que presenten un envase estéril abierto (también involuntariamente) o dañado, así como aquellos que hayan sido extraídos de envases cuya fecha de caducidad haya sido rebasada, deberán considerarse no estériles, siendo necesario someterlos a un ciclo de reprocesado validado antes de su utilización. 9 Estructura y principio de actuación del sistema de distracción El distractor RPE (RPE = Rapid Palatal Expander = expansor palatal rápido) de KLS Martin es un distractor de anclaje óseo de aplicación y activación muy sencilla. El funcionamiento se basa en dos roscas de movimiento opuesto con una inclinación idéntica. Al activarse el distractor, los pines de 2 mm de longitud de la placa de sostén perforan directamente el hueso, con lo que el aparato se estabiliza por sí solo. No obstante, se precisa una fijación adicional del distractor en el hueso con ayuda de los tornillos Drill-Free de maxDrive® suministrados. Tras cada activación/distracción, debe bloquearse el distractor apretando la contratuerca gris (3) para que quede asegurado contra un giro accidental. (Véase Sinopsis de los distractores Typ, en la página 6). 10 Selección del sistema de distracción ADVERTENCIA ¡Evite la combinación de diferentes materiales o sistemas! Los sistemas de distracción de KLS Martin no deben combinarse con los sistemas de otros fabricantes. Si se utilizan instrumentos que no estén específicamente previstos para su utilización con implantes, esto podría conducir a una falla total del sistema de distracción. 54 V. 3.3 Instrucciones de uso Distractores RPE 11 Reprocesado, limpieza, desinfección y esterilización Los sistemas de distracción son apropiados para un reprocesado mecánico / una desinfección térmica. Pueden ser reprocesados con las secuencias de programa homologadas para el instrumental quirúrgico. Para la limpieza deberán observarse las instrucciones del fabricante de la máquina de lavado, así como del fabricante de los productos de tratamiento utilizados. El proceso, incluyendo la carga, debe asegurar la eliminación suficiente de suciedad e incrustaciones. ADVERTENCIA ¡Peligro de infección por manipulación no estéril! La esterilización inadecuada y la manipulación no estéril del sistema de distracción pueden conducir a graves riesgos de salud para el paciente. La esterilización deberá realizarse mediante un proceso de esterilización por vapor de agua validado, p. ej. en un esterilizador conforme a la norma EN 285:2009 y validado conforme a la norma ISO 17665-1:2006. INDICACIÓN El propietario / usuario del producto es responsable de la limpieza, la desinfección y la esterilización de los componentes del sistema de distracción. En este sentido es prescriptivo que se observen las regulaciones nacionales y también las restricciones respectivas. Para la esterilización, el consiguiente transporte y el almacenamiento se tienen que utilizar los embalajes de esterilización correspondientes y aprobados (por ejemplo, conforme a las normas EN 868, ISO 11607). Los componentes del implante contaminados y/o utilizados no deben reutilizarse bajo ningún concepto y, en consecuencia, tampoco deberán reprocesarse. V. 3.3 55 Instrucciones de uso Distractores RPE 12 Antes de la implantación Indicaciones • Antes de su utilización, los sistemas de distracción deberán pasar el ciclo completo de reprocesado con los correspondientes procedimientos adecuados de limpieza, desinfección y esterilización. • Los implantes embalados en estado estéril pueden aplicarse directamente después de extraerlos del embalaje estéril sin ningún tratamiento previo. • Para evitar la apertura del embalaje de implantes estériles no apropiados puede resultar muy útil la utilización de plantillas para familiarizarse con las condiciones anatómicas en el sitio de operación y controlar previamente las estimaciones de las dimensiones específicas del hueso antes de abrir los embalajes protectores. Para esta finalidad pueden obtenerse plantillas para este sistema de distracción a través de la sociedad KLS Martin. • Antes de la implantación deberá realizarse una inspección visual y una activación de prueba del sistema de distracción: asegúrese de que el huso de activación (2) del sistema de distracción pueda girarse (véase la imagen de Sinopsis de los distractores Typ, en la página 6) y de que el distractor se mueva correctamente. • Antes de la implantación se recomienda la realización de un barrido en 3D mediante tomógrafo computerizado y/o la elaboración de un modelo estereolitográfico para determinar la posición óptima del sistema de distracción o realizar adaptaciones previas. En caso necesario podrá encargar modelos anatómicos individuales a KLS Martin (www.klsmartin.com). INDICACIÓN ¡Posible interpretación errónea de la exploración! Los sistemas de distracción pueden generar artefactos perturbadores tanto en la tomografía computerizada (TC) como en la tomografía por resonancia magnética nuclear (TRM). 56 V. 3.3 Instrucciones de uso Distractores RPE ADVERTENCIA ¡Peligro de quemaduras o desplazamiento involuntario del implante en el caso de utilizar la TRM! Debido al desarrollo ulterior y a la densidad energética creciente de los equipos para la TRM, en el futuro no puede descartarse una influencia negativa sobre los implantes. Una TRM sólo será admisible en el caso de que pueda excluirse definitivamente cualquier lesión del paciente. • En la TRM existe el peligro de calentamiento y de desplazamiento de los implantes por influencia del campo magnético generado. • En el caso de fijadores externos de acero deberá comprobarse la compatibilidad con la TRM en cada caso particular. • Los implantes de acero conforme a la norma ISO 5832-1 están catalogados como paramagnéticos o antimagnéticos. • El titanio es totalmente antimagnético. V. 3.3 57 Instrucciones de uso Distractores RPE 13 Procedimiento intraoperativo La operación se realiza con anestesia general, preferentemente con intubación endotraqueal. Se realizan las corticotomías estándar en las partes anterior, lateral y medial del maxilar. En caso de expansión posterior (paralela), debe realizarse una separación adicional en la región pterigomaxilar (osteotomía posterior). La separación del tabique nasal es objeto de discusión médica. A continuación, se inyectan un anestésico y un vasoconstrictor en la mucosa de los premolares. El distractor RPE se fija a la mucosa con placas de sostén por encima de las raíces de los segundos premolares (en caso de una expansión anterior [3:2]) y de los primeros molares (en caso de una expansión posterior). El huso de activación se encuentra en la parte central y no debe entrar en contacto con la oclusión dental. El distractor se activa ligeramente para que las impresiones de los pines sean claramente visibles en la mucosa. Tras esto, se realiza una incisión en la mucosa, en la parte oclusal anterior. El distractor se desactiva y se retira. Se retira la mucosa palatina y se realiza una hemostasia local. A continuación, vuelve a colocarse el distractor RPE, pero esta vez con las placas de sostén directamente sobre el hueso. Se activa ligeramente el distractor, de modo que los pines de las placas puedan penetrar en el hueso. Para ello, debe tenerse en cuenta que los orificios de osteosíntesis de la placa de sostén estén orientados hacia anterior. A continuación, se realiza una fijación adicional atornillando en el hueso los dos tornillos DrillFree suministrados a través de los orificios de las placas de sostén. Para contrarrestar posibles movimientos en el cuerpo de distracción durante la fase de consolidación, es necesario apretar la contratuerca. 58 V. 3.3 Instrucciones de uso Distractores RPE ADVERTENCIA ¡Peligro de un fallo del sistema debido a la manipulación indebida! ¡La manipulación indebida (adaptación indebida) del sistema de distracción por el usuario puede conducir a un fallo del sistema y a posibles lesiones! Por ello deberá observarse que: • el sistema de distracción nunca debe doblarse ni contornearse ligeramente; • en aquellos casos en los que desee conseguir la longitud máxima de distracción, deberá tenerse en cuenta que antes de eliminar el activador al finalizar la fase de distracción, éste deberá girarse media revolución en dirección contraria a la distracción (dirección antihorario); este proceder evita un desperfecto en el husillo de activación debido a un giro involuntario durante el proceso de desacoplamiento. ADVERTENCIA ¡Peligro de asfixia! Si se suelta el distractor, existe riesgo de atragantamiento o de inhalación accidental. Una vez implantado el distractor, asegurarlo adicionalmente con los dos tornillos Drill-Free suministrados. Para ello, atornillar los tornillos Drill-Free en el hueso a través de los orificios de las placas de sostén. ADVERTENCIA ¡Peligro de una movilización insuficiente de los segmentos óseos! Una movilización insuficiente conduce a un incremento inadmisible de las fuerzas en el sistema de distracción, que podrían tener como consecuencia un doblado o la rotura de éste. Por ello deberá observar: • que los segmentos óseos a extender deben movilizarse antes de la distracción (corticotomía u osteotomía completa). Para controlar la movilización deberá realizarse una activación de prueba de como mínimo 1-3 mm del sistema de distracción durante la operación, debiendo presentarse después de ésta una hendidura de osteotomía ininterrumpida; • que el sistema de distracción nunca deberá doblarse durante el proceso de distracción. V. 3.3 59 Instrucciones de uso Distractores RPE 14 Antes de la distracción (fase de latencia) • Para la primera estabilización, debe activarse ligeramente el distractor RPE. Tenga en cuenta que ambas placas se separan paralelamente de la posición inicial y que no se desplazan de forma unilateral. • Tras realizar la activación de prueba, volver a girar completamente el distractor hasta alcanzar la posición de inicio. Las conclusiones relativas al protocolo de distracción están descritas de manera muy heterogénea en la literatura, pero suelen seguir los siguientes parámetros. No obstante, los factores terapéuticos o específicos del paciente pueden alterar lo dispuesto, como p. ej. la edad, la anamnesis, la situación de los tejidos blandos, la dentadura o los conceptos de tratamiento de ortodoncia. Después de la implantación del sistema de distracción, en primer lugar deberá cumplirse una fase de latencia antes de iniciar la fase de distracción propiamente dicha. La fase de latencia deberá ser valorada por el cirujano conforme a la indicación y por regla general dura entre 5-7 días (dependiendo del paciente y de la indicación). 60 V. 3.3 Instrucciones de uso Distractores RPE 15 Durante la distracción (fase de distracción) El distractor puede activarse muy fácilmente por medio de la llave de activación (n.º de art. 51-565-90-07 o n.º de art. 51-565-95-07). Un giro de 120° (hasta el siguiente código cromático) corresponde a un recorrido de distracción de 0,33 mm. Se recomienda un giro de 120-240° al día, lo que equivale a un recorrido de distracción de 0,33-0,66 mm al día. El avance de la distracción debe estar sometido a un control médico continuo. Por ejemplo, una tracción demasiado fuerte de los tejidos blandos puede conducir a una deformación del sistema de distracción. Esta posible deformación debe verificarse mediante controles radiológicos periódicos. Si se prosigue con la distracción a pesar de presentarse una deformación, podría darse una rotura por sobrecarga del distractor o llegar a arrancarse los tornillos del hueso. ADVERTENCIA ¡Peligro de un fallo del sistema debido a la manipulación indebida por parte del paciente! La manipulación errónea del sistema de distracción por parte del paciente puede conducir a un defecto del sistema o a una lesión del paciente. Por ello deberá observar lo siguiente: • Informe al paciente y asístale después de la operación. • El sistema de distracción no debe doblarse ni modificarse de ningún otro modo. • Durante todo el tiempo del tratamiento deberá prescindirse de cualquier actividad física que pueda menoscabar el sistema de distracción o los elementos óseos acoplados a éste. Esto es especialmente válido para el transporte de cargas (peligro de caídas) y cualquier actividad deportiva que pueda causar golpes o impactos. • Mediante medidas protectoras adecuadas (p. ej. manguito de protección, férula, casco, etc.) deberá asegurarse que el lugar de distracción no sea sometido a sobrecargas (incluyendo el periodo de sueño). • Observe la dirección de activación correcta (las flechas indicadoras se encuentran en el cuerpo de distracción). • Tras cada activación/distracción, debe bloquearse el distractor apretando la contratuerca gris para que quede asegurado contra un giro accidental. V. 3.3 61 Instrucciones de uso Distractores RPE 16 Después de la distracción (fase de consolidación) Indicaciones • La fase de consolidación dura aprox. 3-6 meses, dependiendo del paciente. • El sistema de distracción sólo deberá eliminarse después de la consolidación ósea completa, de modo que no se ponga en peligro el resultado de distracción. • No obstante, una vez concluida la fase de distracción debería eliminarse el activador. • El tratamiento fisioterapéutico ya puede iniciarse durante la fase de consolidación para evitar una contracción de aducción. • El tratamiento de ortodoncia ya podrá iniciarse durante la fase de consolidación. • Una vez concluida la fase de consolidación deberán eliminarse todos los componentes del distractor. • El tipo y el alcance del tratamiento postoperatorio y/o las medidas de rehabilitación deberán ser consensuadas entre el médico asistente y el paciente. ADVERTENCIA ¡Posible fallo del sistema en caso de reutilización! Le type, la composition et l’application de chaque système de distraction sont déterminés par l’utilisateur après évaluation des exigences anatomiques et fonctionnelles respectives du patient et sont donc de la responsabilité de l’utilisateur (médecin traitant, chirurgien). • Gebrüder Martin ne sera pas responsable de complications résultant d’une indication erronée, du choix des mauvais implants, d’une mauvaise combinaison de composants du système et d’une technique opératoire non adaptée, ainsi que des limites de la méthode de traitement ou d’une hygiène insuffisante. • Tous les composants de distraction, instruments et accessoires doivent être disponibles dans un état permettant leur utilisation immédiate. • Le produit devra être utilisé conformément aux indications. 66 V. 3.3 Mode d’emploi Distracteurs RPE 1.3 Garantie Nos conditions générales de vente dans la version respectivement en vigueur sont applicables. Des dispositions en divergeant ne limiteront pas les droits légaux de l’acheteur. Toute garantie allant au-delà nécessitera la forme contractuelle et exclura le vandalisme sur des composants, les mises à jour logicielles, ainsi que les consommables. Indications importantes Seul Gebrüder Martin ou une personne ou une société expressément habilitée par Gebrüder Martin sera autorisé à réparer le produit. Si la réparation est effectuée par une personne ou une société autorisée par Gebrüder Martin, l’exploitant du produit sera tenu d’exiger du réparateur un certificat stipulant le type et l’étendue de la réparation. La date de la réparation, ainsi que l’entreprise et la signature doivent figurer sur ce certificat. Si la réparation n’est pas effectuée par le fabricant, l’identification du réparateur devra en outre être apposée sur les produits réparés. Toute manipulation et entretien non conformes et toute utilisation contraire à l’usage prévu peuvent entraîner une usure précoce et / ou des risques pour le patient et l’utilisateur ! V. 3.3 69 Mode d’emploi Distracteurs RPE 3 Utilisateur • Seul un personnel médical compétent, qualifié de par sa formation spécialisée, ses enseignements, son expérience, ainsi que sa connaissance des dispositions inhérentes, sera autorisé à utiliser ce système. • L’utilisateur devra avoir connaissance des règles de l’art médical, de l’état actuel de la science et des contenus de publications scientifiques afférentes d’auteurs médicaux. • Il incombera à l’utilisateur d’éviter et/ou de minimiser les risques généraux liés à des interventions chirurgicales. • L’utilisateur : — sera responsable de l’exécution correcte de l’intervention chirurgicale — devra connaître et maîtriser la technique opératoire, la gamme d’implants, ainsi que les instruments et accessoires — sera responsable de la composition des composants de l’implant et de leur implantation correcte NOTICE Recommandation pour une meilleure traçabilité de l'implant utilisé. Dans le dossier du patient, il est possible de consigner les composants implantaires utilisés en y mentionnant leurs références, leurs désignations ainsi que leurs numéros de lot et/ou de série. Seule une consignation des numéros de lot et/ou de série permet de garantir une traçabilité claire des implants utilisés. 4 Matériaux des implants Vous trouverez des informations relatives aux matériaux de l’implant sur notre site Internet : www.klsmartin.com 70 V. 3.3 Mode d’emploi Distracteurs RPE 5 Indications • Chevauchements de la mâchoire supérieure transversaux, uni- ou bilatéraux extrêmes chez les patients à syndrome ou non. • Chevauchements dentaires antérieurs et “corridors buccaux” (espaces morts). Les chevauchements de la mâchoire supérieure transversaux chez les jeunes et les adultes sont aussi bien fréquents chez les patients à syndrome que chez les patients sans syndrome et les patients présentant des divisions. Les chevauchements de la mâchoire supérieure uni- et bilatéraux peuvent être traités par recours à une expansion palatine rapide assistée chirurgicalement (SARPE). Le traitement allie orthodontie et procédure chirurgicale. Il permet une meilleure orientation des dents et des implants dentaires sur l'arcade dentaire. Le procédé permet en outre d'élargir considérablement la base radiculaire maxillaire ainsi que la voûte palatine. L'espace disponible pour la langue et nécessaire à une déglutition correcte est élargi, évitant ainsi un nouvel affaissement de la mâchoire supérieure. Ce procédé entraîne également une nette amélioration de la respiration nasale par élargissement des narines. Les anciens distracteurs d'expansion sont des distracteurs à appui dentaire (vis Hyrax par ex.). Ceux-ci présentent cependant certains inconvénients à ne pas négliger. • Problèmes médicaux dentaires tels qu'une résorption radiculaire buccale ou une fenestration de la corticale. • Basculement de segments dentaires entiers ou des prothèses • Caries dentaires chez les patients à syndrome avec mauvaise hygiène buccale Contrairement à ces distracteurs, les distracteurs à appui osseux sont appliqués à un niveau plus élevé dans la voûte palatine. Ces expansions maxillaires sont orthopédiques et sont plus simples à commander mécaniquement. En outre, les forces agissent directement sur les os ; un basculement des dents ou d'autres effets secondaires indésirables sont évités. Le retrait du distracteur au terme des phases de distraction et de consolidation peut être effectué très facilement, sous anesthésie locale. V. 3.3 71 Mode d’emploi Distracteurs RPE 6 Contre-indications Le non-respect du mode d’emploi peut entraîner des blessures graves, voire mortelles du patient ! • Infections manifestes • Soupçon de sensibilité ou allergies connues aux matériaux de l’implant • Maladies auto-immunes • Troubles de vascularisation, maladies systémiques et troubles du métabolisme • Tissu dur et mou en quantité et de qualité insuffisantes • Tumeurs osseuses dans la région d’ancrage de l’implant • Maladies associées, par ex. processus pathologiques dégénératifs compromettant le succès thérapeutique • Utilisation dans des régions soumises à des poids ou efforts excessifs • Patients qui, du fait de leur état mental, neurologique ou physique, ne sont pas prêts à ou ne sont pas en mesure de suivre les instructions données pendant le suivi postopératoire • Patients dans un mauvais état général au sens d’une contre-indication générale (par ex. patients subissant une radiothérapie) • Ostéoporose ou ostéomalacie et autres lésions graves des structures osseuses qui s’opposent à un ancrage stable des composants de l’implant • Abus manifeste de médicaments, de drogues et/ou d’alcool • Palais extrêmement bas : En cas de palais bas, les plaques d'appui perdent tout soutien et la stabilité du distracteur est amoindrie. • Surocclusions de classe II : Le distracteur ou la broche d'activation pourrait entrer en contact avec les dents de la mandibule. Ceci peut être évité en plaçant le distracteur à un niveau plus élevé (distalement) dans la voûte palatine ou en portant une gouttière au cours des phases de distraction et de consolidation. 72 V. 3.3 Mode d’emploi Distracteurs RPE 7 Effets secondaires et complications possibles • Diminution de la densité osseuse et/ou nécroses osseuses dues à des charges • Modifications des vaisseaux • Luxations et subluxations • Infections précoces et tardives, infections superficielles et/ou profondes • Lésions nerveuses et vasculaires • Hématomes et troubles de la cicatrisation • Calcifications péri-articulaires • Fonction articulaire et mobilité limitées • Charge articulaire restreinte et douleurs articulaires • Réaction allergique ou sensibilité aux matériaux de l’implant • Lésions nerveuses dues à un traumatisme chirurgical • Ostéogenèse insuffisante ou cicatrisation osseuse ralentie, ostéoporose, ostéolyse, ostéomyélite, revascularisation inhibée ou infections, nécrose osseuse pouvant entraîner le relâchement, la déformation, l’arrachement ou la rupture du système de distraction. • Une charge de force ou de poids inadaptée peut entraîner l’usure, la rupture, la déformation, le relâchement ou la migration du système de distraction. • Après la phase de consolidation, les systèmes de distraction peuvent se relâcher, rouiller, se déplacer ou provoquer des douleurs, ce qui est particulièrement le cas chez les patients jeunes et sportifs. • Même après le retrait du système de distraction, le patient doit bénéficier d’un suivi postopératoire adapté afin d’éviter une fracture du nouveau tissu osseux • Restriction de la croissance • Douleurs, inconfort, sensations anormales ou implant palpable • Réaction accrue du tissu mou dans la zone de l’ostéotomie et/ou du système de distraction • Cicatrisation insuffisante • Décoloration des tissus due au contact avec le matériau de l’implant. • Des fragments et de petites particules microscopiques peuvent se détacher du système de distraction et s’éloigner du site d’implantation. Même après avoir enlevé l’implant, ces particules peuvent encore rester dans le corps. Leurs effets à long terme ne sont pas connus à ce jour. Outre les effets secondaires susmentionnés, certaines complications, comme par ex. infections, lésions nerveuses et douleurs n’étant pas forcément dues au système de distraction, sont possibles lors de chaque intervention chirurgicale. V. 3.3 73 Mode d’emploi Distracteurs RPE 8 Emballage • Après la livraison du produit, contrôler que l’emballage original et le sceau de l’emballage sont intacts. Les emballages dont le sceau est endommagé ne seront pas repris. • Les systèmes de distraction, qui ne sont pas emballés de manière stériles, doivent être déballés à des fins de traitement. • Les implants, conditionnés dans des emballages stériles, peuvent être utilisés directement en sortant de leur emballage stérile, sans autre traitement préalable. • Les implants stériles dont l’emballage stérile est ouvert (même par inadvertance) ou endommagé, ainsi que ceux sortis d’emballages dont la date limite d’utilisation est expirée devront être considérés comme non stériles et subir une procédure de traitement validée avant utilisation. 9 Structure et fonctionnement du système de distraction Le distracteur RPE (RPE = Rapid Palatal Expander) de KLS Martin est un distracteur à appui osseux. Il peut être appliqué et activé très facilement. Son fonctionnement repose sur deux filetages contraires de pas identique. À l'activation du distracteur, les broches de 2 mm de longueur de la plaque d'appui viennent directement s'implanter dans l'os, le dispositif se stabilise automatiquement. Une fixation supplémentaire du distracteur sur l'os à l'aide des deux vis Drill Free maxDrive® fournies est nécessaire. Après chaque activation / distraction, le distracteur doit être bloqué en serrant le contre-écrou gris (3) afin de le sécuriser contre toute torsion indésirable. (voir Aperçu distracteurs types, page 6). 10 Sélection du système de distraction Éviter de combiner différents matériaux ou systèmes ! Les systèmes de distraction KLS Martin ne doivent pas être combinés avec des systèmes d’autres fabricants. L’application éventuelle d’instruments qui ne sont pas spécifiquement prévus pour être utilisés en liaison avec des implants est susceptible de provoquer une défaillance du système de distraction. 74 V. 3.3 Mode d’emploi Distracteurs RPE 11 Traitement, nettoyage, désinfection et stérilisation Les systèmes de distraction sont adaptés à un traitement mécanique/à une désinfection thermique. Ils peuvent être traités avec les cycles de programmes homologués pour les instruments chirurgicaux. Il convient de respecter les informations données par les fabricants des automates de nettoyage, ainsi que celles des fabricants des produits de traitement. Le processus, chargement inclus, doit garantir l’enlèvement suffisant de résidus. Risque d’infection suite à une manipulation non stérile ! Une stérilisation incorrecte, ainsi qu’une manipulation non stérile du système de distraction peuvent entraîner de graves risques pour la santé du patient. La stérilisation doit être effectuée selon un procédé de stérilisation à la vapeur validé, par exemple dans un stérilisateur conforme à EN 285:2009, et validé conformément à ISO 17665-1:2006. INDICATION La responsabilité pour le nettoyage, la désinfection et la stérilisation de composants de distraction incombe à l’exploitant/l’utilisateur du produit. Les réglementations nationales, y compris les limitations respectives, doivent absolument être respectées. Pour la stérilisation, le transport et le stockage, utiliser les emballages de stérilisation homologués correspondants (p. ex. selon EN 868, ISO 11607). Des composants d’implants contaminés et/ou déjà utilisés ne devront en aucun cas être réutilisés et ne doivent donc pas être retraités. V. 3.3 75 Mode d’emploi Distracteurs RPE 12 Avant l’implantation Indications • Avant d’être utilisés, les systèmes de distraction doivent traverser toutes les étapes du traitement, à savoir les procédures de nettoyage, de désinfection et de stérilisation adéquates. • Les implants, conditionnés dans des emballages stériles, peuvent être utilisés directement en sortant de leur emballage stérile, sans autre traitement préalable. • Afin d’éviter d’ouvrir tout emballage avec des implants stériles non adaptés, il est possible d’utiliser des pochoirs permettant de se familiariser avec les conditions anatomiques sur le site opératoire et de vérifier au préalable l’estimation des dimensions osseuses avant d’ouvrir les emballages de protection. À ce sujet, des pochoirs pour ce système de distraction sont disponibles auprès de KLS Martin. • Avant l’implantation, le système de distraction doit être soumis à un contrôle visuel et à une activation d’essai : Assurez-vous que la broche d'activation (2) du système de distraction tourne (figure, voir Aperçu distracteurs types, page 6) et que le distracteur bouge correctement. • Avant l’implantation, il est recommandé d’effectuer une tomodensitométrie et/ou de fabriquer un modèle stéréolithographique afin de déterminer la position optimale du système de distraction et/ou de procéder à des adaptations préalables. Des modèles anatomiques individuels peuvent être commandés chez KLS Martin (www.klsmartin.com). INDICATION Interprétations erronées possibles de l’examen ! Les systèmes de distraction peuvent provoquer des artefacts gênants tant lors de la tomodensitométrie (TDM) que lors de la tomographie par résonance magnétique (IRM). 76 V. 3.3 Mode d’emploi Distracteurs RPE Risque de brûlure et/ou de mouvement intempestif de l’implant lors de l’utilisation de la tomographie par résonance magnétique (IRM) ! En raison du perfectionnement et de la densité énergétique croissante des appareils IRM, il est impossible d’exclure à l’avenir toute influence négative sur les implants. En cas de doute, un examen IRM ne sera autorisé que si un dommage pour le patient est définitivement exclu. • Lors de l’IRM, le champ magnétique risque de provoquer un réchauffement et un déplacement des implants susceptibles de réagir à ce champ magnétique. • Dans le cas de fixateurs externes en acier, la compatibilité à l’IRM devra être vérifiée au cas par cas. • Les implants en acier conformes à ISO 5832-1 sont classifiés comme paramagnétiques ou non magnétiques. • Le titane n’est absolument pas magnétique. 13 Procédure peropératoire L'opération a lieu sous anesthésie générale, de préférence avec intubation endotrachéale. Corticotomies standard des maxillaires antérieurs, latéraux et médiaux. En cas d'expansion postérieure (parallèle), effectuer également une séparation dans la zone ptérygo-maxillaire (ostéotomie postérieure). La dissection de la cloison nasale suscite la controverse chez les médecins. Un anesthésique et un vasoconstricteur sont injectés dans la muqueuse des prémolaires. Le distracteur RPE est fixé à l'aide de plaques d'appui sur la muqueuse au-dessus des racines des secondes prémolaires (en cas d'expansion antérieure (3:2)) et des premières molaires (en cas d'expansion postérieure). La broche d'activation se situe au centre, elle ne doit pas entrer en contact avec l'occlusion dentaire. Le distracteur est légèrement activé afin que les empreintes de la broche soient clairement visibles sur la muqueuse. La muqueuse sur la partie occlusale antérieure est à présent incisée. Le distracteur est désactivé puis retiré. La muqueuse du palais est retirée, une hémostase locale est effectuée. Le distracteur RPE est ensuite directement appliqué sur l'os, avec les plaques d'appui cette fois. Le distracteur est à cet effet légèrement activé de manière à ce que les broches des plaques puissent pénétrer dans l'os. Veiller ici à ce que les trous d'ostéosynthèse de la plaque d'appui soient orientés vers l'avant. Les deux vis Drill Free fournies sont ensuite vissées dans l'os à travers les trous des plaques d'appui. Afin de contrecarrer tout mouvement éventuel au niveau du corps de distraction au cours de la phase de consolidation, serrer fermement le contre-écrou. V. 3.3 77 Mode d’emploi Distracteurs RPE Risque de défaillance du système suite à une manipulation incorrecte ! Une manipulation incorrecte (adaptation non autorisée) du système de distraction par l’utilisateur peut entraîner une défaillance du système ! Il faut donc respecter ce qui suit : • Défense impérative de plier le système de distraction ou de le manipuler en vue de l’adapter au contour de l’os. • Dans les cas où il est prévu d’utiliser la longueur de distraction maximale, il faut, avant de détacher l’activateur après la distraction intégrale, donner un demi-tour vers l’arrière à l’activateur afin de ne pas endommager la broche d’activation en tournant intempestivement au moment de la séparation. Risque de suffocation ! Si le distracteur venait à se détacher, celui-ci risquerait alors d'être ingéré ou inhalé par mégarde. L'implantation une fois terminée, sécuriser le distracteur à l'aide des deux vis Drill Free fournies. Pour cela, visser les vis Drill Free dans l'os à travers les trous des plaques d'appui. Risque de mobilisation insuffisante des segments osseux ! Une mobilisation insuffisante entraîne une augmentation des forces exercées sur le système de distraction, qui peuvent provoquer une déformation et/ou la rupture du système de distraction. Il faut donc veiller à ce qui suit : • Les segments devant subir la distraction doivent être mobilisés avant la distraction (corticotomie et/ou ostéotomie complète). Pour vérifier la mobilisation, il faut effectuer pendant l’opération au niveau du système de distraction implanté une activation d’essai d’au moins 1 à 3 mm affichant une fente d’ostéotomie continue. Le système de distraction ne doit pas se déformer pendant la distraction. 78 V. 3.3 Mode d’emploi Distracteurs RPE 14 Avant la distraction (phase de latence) • Pour la première stabilisation, le distracteur RPE doit être légèrement activé. Veillez à ce que les deux plaques défilent parallèlement par rapport à leur position de base et à ce qu'elles ne soient pas déployées d'un seul côté. • L'activation test une fois terminée, tourner entièrement le distracteur pour le ramener en position initiale. Les déclarations relatives au protocole de distraction sont décrites de manière très hétérogène dans les ouvrages de référence, mais prévoient généralement les paramètres suivants. Certains facteurs thérapeutiques ou spécifiques au patient peuvent également venir modifier le protocole de distraction, par ex. âge, anamnèse, situation des tissus mous, dentition ou concepts de traitement orthodontaires, etc. Après l’implantation d’un système de distraction, il faut d’abord respecter une phase de latence avant la phase de distraction en soi. La phase de latence doit être évaluée par le chirurgien en fonction de l’indication et dure environ 5 à 7 jours (en fonction du patient et de l’indication). V. 3.3 79 Mode d’emploi Distracteurs RPE 15 Pendant la distraction (phase de distraction) Le distracteur peut être activé très facilement à l'aide de la clé d'activation (réf. 51-565-90-07 ou réf. 51-565-95-07). Une rotation de 120° (jusqu'au codage couleur suivant) correspond à une course de distraction de 0,33 mm. Il est recommandé d'effectuer une rotation de 120 à 240° par jour. Ceci correspond à une longueur de distraction quotidienne de 0,33 à 0,66 mm. Le progrès de la distraction devra faire l’objet d’un suivi médical continu. Une traction trop importante des tissus mous peut par exemple entraîner une déformation du système de distraction. Cette déformation éventuelle doit être surveillée par des radios régulières. Si la distraction persiste malgré une déformation, risque de provoquer une rupture de surcharge et/ou un arrachement des vis de l’os. Risque de défaillance du système due à une mauvaise manipulation par le patient ! Une mauvaise manipulation du système de distraction par le patient peut entraîner une défaillance du système ou une blessure du patient. Il faut donc veiller à ce qui suit : • Donner des instructions au patient et le faire bénéficier d’un suivi postopératoire. • Le système de distraction ne doit pas être déformé ou modifié d’une autre manière. • Pendant toute la durée du traitement, toutes les activités physiques qui pourraient altérer le système de distraction ou les éléments osseux reliés à ce dernier sont proscrites. Ceci est en particulier valable pour le fait de porter des charges (risque de chute) et les activités sportives qui pourraient être à l’origine d’un impact ou d’un coup. • Des mesures de protection (par ex. manchette de protection, attelle, casque etc.) doivent être prises pour s’assurer que le site de distraction ne subira pas de surcharge (y compris pendant le sommeil). • Sens d’activation correct (flèches indiquant la direction sur le corps de distraction). • Après chaque activation / distraction, le distracteur doit être bloqué en serrant le contreécrou gris afin de le protéger contre toute torsion indésirable. 80 V. 3.3 Mode d’emploi Distracteurs RPE 16 Après la distraction (phase de consolidation) Indications • La phase de consolidation dure de 3 à 6 mois, en fonction du patient. • Le système de distraction ne doit être retiré qu’après la consolidation osseuse complète afin de ne pas compromettre le résultat de la distraction. • L’activateur devrait être enlevé à la fin de la phase de distraction. • La physiothérapie peut commencer dès la phase de consolidation afin d’éviter toute contracture en adduction. • Le traitement orthodontique peut également commencer dès la phase de consolidation. • À la fin de la phase de consolidation, tous les composants du distracteur doivent être enlevés. • Le type et l’étendue du traitement postopératoire et/ou des mesures de rééducation doivent faire l’objet d’une concertation entre le médecin traitant et le patient. Défaillance possible du système en cas de réutilisation ! Les systèmes de distraction endommagés ou déjà utilisés peuvent être à l’origine de blessures et ne doivent donc pas être réutilisés. Questo prodotto è certificato con marchio CE, ovvero è conforme ai requisiti fondamentali posti dalla direttiva CE in materia di prodotti a uso medico. È responsabilità dell'utente (medico curante o chirurgo operante) determinare e selezionare il tipo e la configurazione corretti del sistema di distrazione da usare, conformemente alle esigenze anatomiche e funzionali del paziente. • La Gebrüder Martin non è responsabile per le avvenute complicazioni causate da indicazione erronea, scelta sbagliata dell’impianto, combinazione errata di componenti del sistema e tecnica operatoria inadeguata, nonché per i limiti del metodo terapeutico o per l’igiene insufficiente. • Tutti i componenti, gli strumenti e gli accessori della distrazione devono essere in condizioni pronte all'uso • Il prodotto deve essere utilizzato conformemente alle indicazioni. V. 3.3 85 Istruzioni per l’uso Distrattori RPE 1.3 Garanzia Valgono le nostre condizioni generali di vendita nella rispettiva versione vigente. Gli accordi divergenti non limitano i diritti dell’acquirente previsti per Legge. Per altri tipi di garanzia si richiede la forma contrattuale. Si escludono atti di vandalismo sui componenti, agli aggiornamenti software e agli articoli di consumo. Avvertenze importanti Le operazioni di riparazione del prodotto possono essere effettuate esclusivamente da parte dell’azienda Gebrüder Martin oppure da parte di personale o di una ditta espressamente autorizzati dall’azienda Gebrüder Martin. Nel caso in cui la riparazione venga eseguita da parte di personale o di una ditta autorizzati dall’azienda Gebrüder Martin, il gestore del prodotto è tenuto a farsi consegnare dal personale di manutenzione un'attestazione relativa al tipo e all'entità della riparazione effettuata. Il certificato deve contenere la data della riparazione, nonché i dati relativi alla ditta e la firma. Qualora le operazioni di riparazione non vengano eseguite direttamente da parte del produttore, è inoltre indispensabile che i prodotti sottoposti a riparazione vengano dotati del contrassegno della ditta responsabile della riparazione. Qualsiasi diritto di garanzia decade in caso di manipolazioni e modifiche inadeguate da parte di terze persone durante il periodo coperto da garanzia. Azioni non autorizzate sul prodotto comportano la perdita di rivendicazioni di responsabilità nei confronti dell’azienda Gebrüder Martin. 1.4 Verifica alla consegna Al momento della ricezione della merce consigliamo di verificarne immediatamente l’integrità e di assicurarsi che non abbia subito danni. Fare quindi attenzione che le seguenti indicazioni vengano comprese e rispettate! • Ciascun utente deve leggere e osservare integralmente questo documento. • In modo particolare occorre rispettare tutte le indicazioni relative alla precauzione, alla cautela ed ai pericoli. • L’utente deve poter accedere in qualsiasi momento a questo documento. • È responsabilità dell’utente informare il paziente in merito a questo documento, sulle precauzioni, sulla cautela e sui pericoli menzionati nelle presenti istruzioni ed assicurarsi che le abbia comprese. • Prestare inoltre attenzione alle istruzioni per l’uso degli impianti da osteosintesi. V. 3.3 87 Istruzioni per l’uso Distrattori RPE 2.1 Simboli utilizzati in questo documento In questo documento le informazioni importanti, così come le indicazioni generali o rilevanti per la sicurezza vengono contrassegnate con i seguenti simboli e segnalazioni: AVVERTENZA Pericolo di morte o di lesioni gravi! In caso di inosservanza sussiste il rischio di danno materiale (perdita di tempo, perdita di dati, guasto della macchina, ecc.)! 88 V. 3.3 Istruzioni per l’uso Distrattori RPE 3 Utenti • Il sistema di distrazione può essere utilizzato unicamente da personale medico specializzato che, sulla base di formazione specialistica, esperienza e conoscenza delle disposizioni rilevanti in materia, sia qualificato per l’impiego di questo sistema. • All'utente devono essere note le regole della pratica medica, gli aggiornamenti scientifici ed i contenuti rilevanti di pubblicazioni scientifiche ad opera di autori medici. • È responsabilità dell’utente evitare o ridurre i rischi generici nel corso degli interventi chirurgici. • L’utente: — è responsabile della corretta attuazione dell’intervento chirurgico — deve avere padronanza della tecnica operatoria, conoscere l’assortimento di impianti, nonché gli strumenti e gli accessori utilizzati — è responsabile della selezione dei componenti dell’impianto e del loro montaggio conforme. NOTICE Consigli per una facile rintracciabilità dell'impianto utilizzato. Nella cartella del paziente è possibile registrare i componenti utilizzati dell'impianto con il relativo codice-articolo, la denominazione articolo e il numero di lotto e/o numero di serie. Solo la documentazione dei numeri di lotto e/o dei numeri di serie garantisce la rintracciabilità univoca dell'impianto utilizzato. 4 Materiali degli impianti Per ulteriori dettagli relativi alle indicazioni sui materiali dell’impianto rinviamo al nostro sito web: www.klsmartin.com V. 3.3 89 Istruzioni per l’uso Distrattori RPE 5 Indicazioni • Estrema malocclusione trasversale, mono o bilaterale della mascella superiore in pazienti sindromici e non sindromici. • Affollamento dentale anteriore e “buccal corridors” (spazi che si creano tra l’interno della guancia e i denti). Le malocclusioni trasversali della mascella superiore nei ragazzi e negli adulti si verificano nei pazienti sindromici, in quelli non sindromici e in quelli con diastemi. Le malocclusioni mono e bilaterali della mascella superiore possono essere trattate con l'ausilio dell'espansione chirurgica del palato (SARPE). Il trattamento rappresenta una combinazione tra procedura ortodontica e procedura chirurgica. Esso permette di allineare meglio i denti e gli impianti sull'arcata dentale. La procedura permette inoltre di ingrandire notevolmente la base delle radici nel seno mascellare e nella volta palatale. Lo spazio disponibile per la lingua e per una corretta deglutizione si ingrandisce, evitando così un nuovo restringimento della mascella superiore. Si può osservare inoltre un marcato miglioramento della respirazione nasale, insieme ad un ingrandimento ai valori normali della pinna nasale. Le prime forme dei distrattori ad espansione sono sostenute dai denti (ad esempio la vite Hyrax). Essi possiedono però alcuni notevoli svantaggi. • Problemi di medicina dentale, come il riassorbimento delle radici dentarie e la fenestrazione della corticale. • Inclinazione di interi segmenti dentali o dei denti di ancoraggio • Carie dentali nei pazienti sindromici che presentano una scarsa igiene orale Diversamente da questi tipi di distrattore, quelli sostenuti dall'osso vengono applicati in un punto più alto della volta palatale. Queste espansioni della mascella sono di tipo ortopedico e possono essere gestite meglio meccanicamente. Inoltre le forze agiscono direttamente sull'osso, evitando l'inclinazione dei denti o altri effetti collaterali indesiderati. È possibile infine rimuovere il distrattore molto facilmente e in anestesia locale a fasi di distrazione e di consolidamento concluse. 90 V. 3.3 Istruzioni per l’uso Distrattori RPE 6 Controindicazioni AVVERTENZA La mancata osservanza delle controindicazioni può comportare il rischio di lesioni gravi o addirittura letali per il paziente! • Infezioni manifeste • Sospetto di intolleranza o di allergie ai materiali utilizzati per l’impianto • Malattie autoimmuni • Disturbi circolatori, malattie sistemiche e disturbi del metabolismo • Tessuto duro o tessuto molle insufficienti per quantità o qualità • Tumori ossei nella zona di ancoraggio dell’impianto • Affezioni concomitanti, ad es. processi degenerativi con influsso negativo sulla guarigione • Utilizzo in zone sottoposte a sollecitazioni inadeguate di forza o a peso eccessivo • Pazienti che, a causa del loro stato mentale, neurologico o fisico, non sono disposti o non sono in grado di seguire le indicazioni per l’assistenza postoperatoria • Pazienti in precarie condizioni generali di salute, nel senso di una controindicazione generale (p.es. pazienti sottoposti a radiazioni) • Osteoporosi od osteomalacia e altri danni gravi delle strutture ossee che sono di impedimento allo stabile ancoraggio dei componenti dell’impianto • Abuso manifesto di medicinali, sostanze stupefacenti e/o alcoliche • Palato estremamente basso: in caso di palato basso le placche di sostegno perdono la loro tenuta e il distrattore perde la sua stabilità. Overbite di classe II: il distrattore o il mandrino di attivazione potrebbero venire in contatto con i denti della mascella inferiore. Ciò può essere evitato applicando il distrattore nella volta palatale in una posizione superiore (in direzione distale), oppure adottando un paradenti durante la fase di distrazione e di consolidamento. V. 3.3 91 Istruzioni per l’uso Distrattori RPE 7 Possibili effetti collaterali e complicazioni • Perdita di densità e/o necrosi ossee dovute a carico • Alterazioni vascolari • Lussazioni e sublussazioni • Infezioni precoci e tardive, infezioni superficiali e/o in profondità • Lesioni nervose e vascolari • Ematomi e problemi di cicatrizzazione • Calcificazioni periarticolari • Funzioni articolari e mobilità limitate • Carico articolare limitato e dolori articolari • Reazione allergica o ipersensibilità al materiale dell’impianto • Danni al sistema nervoso in seguito a trauma operatorio • Formazione ossea insufficiente o guarigione ossea rallentata, osteoporosi, osteolisi, osteomielite, rivascolarizzazione inibita o infezioni e necrosi ossee che potrebbero causare l’allentamento, l’incurvatura, il lacerarsi o la rottura del sistema di distrazione. • Un carico non adeguato di forza o di peso può causare il logorio, la rottura, l’incurvatura, l’allentamento o il dislocamento del sistema di distrazione. • I sistemi di distrazione, dopo la fase di consolidamento, possono allentarsi, corrodersi, dislocarsi o causare dolori. Ciò avviene soprattutto nel caso di pazienti giovani, fisicamente attivi. • Anche dopo la rimozione del sistema di distrazione il paziente deve ricevere un’adeguata assistenza postoperatoria per evitare una frattura del nuovo tessuto osseo. • Limitazioni della crescita • Dolori, disturbi e anomalie della sensibilità e del tatto a causa dell’impianto • Maggiore reazione del tessuto molle nella zona dell’osteotomia e/o del sistema di distrazione • Guarigione insufficiente • Colorazioni del tessuto in seguito a contatto con il materiale dell’impianto • Frammenti e particelle microscopiche possono staccarsi dal sistema di distrazione e migrare dal sito dell’impianto. Anche dopo la rimozione di un impianto metallico può succedere che queste particelle metalliche rimangano nel corpo. Attualmente non sono noti gli effetti a lungo termine di queste particelle. A prescindere dai suddetti effetti collaterali, in ogni intervento chirurgico si possono verificare complicazioni, quali ad esempio infezioni, lesioni nervose e dolori non imputabili al sistema di distrazione. 92 V. 3.3 Istruzioni per l’uso Distrattori RPE 8 Imballaggio • Al ricevimento del prodotto è necessario verificare che l’imballaggio originale e il relativo sigillo siano intatti. Le confezioni con sigillo rotto non saranno riprese in consegna. • I sistemi di distrazione non provvisti di imballaggio sterile devono essere prelevati dalla confezione prima di essere sottoposti a trattamento. • Gli impianti con imballaggio sterile possono essere applicati senza trattamento preventivo direttamente dopo essere stati estratti dall’imballaggio sterile. • Gli impianti sterili con imballaggio sterile aperto o danneggiato (anche inavvertitamente), nonché gli impianti rimossi da imballaggi con data di scadenza superata, devono essere considerati non sterili e, prima del loro utilizzo, devono essere sottoposti ad un procedimento di sterilizzazione convalidato. 9 Costruzione e funzionamento del sistema di distrazione Il distrattore RPE (RPE = Rapid Palatal Expander, espansore rapido del palato) della KLS è un distrattore sostenuto dall'osso. Esso può essere applicato e attivato molto facilmente. Il suo funzionamento si basa su due filettature dal passo identico ma con direzioni opposte. Attivando il distrattore, i pin della lunghezza di 2 mm della placca di sostegno perforano direttamente l'osso, il dispositivo si stabilizza così automaticamente. È necessario fissare ulteriormente il distrattore sull'osso con l'ausilio delle due viti Drill-Free maxDrive® in dotazione. Dopo ciascuna attivazione/distrazione bloccare il distrattore, in modo da proteggerlo da eventuali torsioni indesiderate, fissando il controdado grigio (3) (vedere Panoramica tipi di distrattore, pagina 6). 10 Selezione del sistema di distrazione AVVERTENZA Non è consentita la combinazione di diversi materiali o sistemi! I sistemi di distrazione KLS Martin non possono essere combinati con sistemi di altri produttori! L’uso di strumenti non specificamente previsti per l’impiego congiuntamente ad impianti può causare un guasto del sistema di distrazione! V. 3.3 93 Istruzioni per l’uso Distrattori RPE 11 Trattamento, pulizia, disinfezione e sterilizzazione I sistemi di distrazione sono idonei al trattamento meccanico/alla disinfezione termica. Essi possono essere sterilizzati con programmi autorizzati per gli strumenti chirurgici. Per la pulizia vanno osservate le disposizioni del fabbricante dei macchinari di pulizia automatica e del fabbricante del prodotto di trattamento (detergente, disinfettante). Il processo, compreso il corretto caricamento, deve garantire la sufficiente asportazione dei residui. AVVERTENZA Pericolo di infezione a causa di uso non sterile! Una sterilizzazione non conforme e l’uso non sterile del sistema di distrazione possono causare gravi rischi per la salute del paziente. La sterilizzazione deve avvenire secondo un procedimento convalidato di sterilizzazione a vapore, p.es. in uno sterilizzatore conforme alla norma EN 285:2009 e convalidato conformemente alla norma ISO 17665-1:2006. NOTA La responsabilità della pulizia, della disinfezione e della sterilizzazione conformi dei componenti della distrazione è a carico del gestore/dell’utente del prodotto. Pertanto, è assolutamente necessario rispettare le normative nazionali, nonché le limitazioni esistenti. Per la sterilizzazione, il successivo trasporto e il magazzinaggio vanno applicati gli imballaggi sterili appositamente omologati (p. es. ai sensi della EN 868, ISO 11607). I componenti dell’impianto contaminati e/o usati non possono essere riutilizzati e quindi non devono essere sottoposti a trattamento. 94 V. 3.3 Istruzioni per l’uso Distrattori RPE 12 Prima dell’impianto Indicazioni • rima dell’uso, i sistemi di distrazione devono attraversare tutto il ciclo di preparazione delle procedure idonee di pulizia, disinfezione e sterilizzazione. • Gli impianti con imballaggio sterile possono essere applicati senza trattamento preventivo direttamente dopo essere stati estratti dall’imballaggio sterile. • Per evitare che venga aperto l’imballaggio di impianti sterili non adatti può essere molto utile l’uso di mascherine, per acquisire familiarità con le caratteristiche anatomiche direttamente sul sito operatorio prima dell’apertura degli imballaggi di protezione e per verificare preliminarmente le valutazioni relative alle dimensioni specifiche ossee. Le mascherine per questo sistema di distrazione sono reperibili allo scopo presso KLS Martin. • Prima dell’impianto occorre sottoporre il sistema di distrazione ad un controllo visivo e ad un test di attivazione: Sincerarsi che il mandrino di attivazione (2) del sistema di distrazione possa ruotare (per la figura vedere Panoramica tipi di distrattore, pagina 6), e che nel far ciò il distrattore si muova correttamente. • Prima di procedere all’impianto, si consiglia di realizzare una tomografia tridimensionale computerizzata, e/o di un modello stereolitografico, per calcolare la posizione ottimale del sistema di distrazione e per effettuare eventuali adattamenti preliminari. I modelli anatomici individuali possono essere ordinati presso la KLS Martin (www.klsmartin.com). NOTA Possibile interpretazione errata dei risultati dell’analisi! I sistemi di distrazione possono causare artefatti irritanti sia nella tomografia computerizzata (TAC) che durante la tomografia a risonanza magnetica (RMT). AVVERTENZA Pericolo di ustione o di movimento accidentale dell’impianto durante l’effettuazione di una tomografia a risonanza magnetica (RMT)! A causa dell’ulteriore sviluppo e dell’aumento della densità di energia degli apparecchi RMT, non è possibile escludere in futuro ripercussioni negative sugli impianti. In caso di dubbio, non è consentito un esame con apparecchi RMT, al fine di evitare possibili danni al paziente. • Con gli apparecchi RMT sussiste infatti il pericolo del riscaldamento e dello spostamento all’interno del campo magnetico degli impianti, che possono reagire al campo magnetico. • Per quanto concerne i fissatori esterni in acciaio, la compatibilità RMT deve essere verificata nei singoli casi. • Gli impianti in acciaio ai sensi della norma ISO 5832-1 vengono classificati come paramagnetici o non magnetici. • Il titanio è un materiale completamente non magnetico. V. 3.3 95 Istruzioni per l’uso Distrattori RPE 13 Procedura intraoperatoria L'intervento viene effettuato in anestesia totale, preferibilmente con intubazione endotracheale. Effettuare corticotomie standard della parete anteriore, laterale e mediale dell'osso mascellare. In caso di espansione posteriore (parallela) si dovrà inoltre effettuare un sezionamento nella zona pterigo-mascellare (osteotomia posteriore). La separazione del setto nasale è oggetto di controversia tra i medici. Iniettare un anestetico e un vasocostrittore nella mucosa dei premolari. Fissare il distrattore RPE alla mucosa al di sopra delle radici del secondo premolare (in caso di espansione anteriore (3:2)) e del primo molare (in caso di espansione posteriore) con le placche di sostegno. Il mandrino di attivazione si trova al centro e non deve urtare l'occlusione dentale. Attivare leggermente il distrattore in modo che le impronte dei pin siano chiaramente visibili sulla mucosa. Incidere ora la mucosa sul lato occlusale anteriore. Disattivare e rimuovere nuovamente il distrattore. Rimuovere la mucosa del palato effettuando a tal fine un'emostasi locale. Applicare ora nuovamente il distrattore RPE, questa volta però con le placche di sostegno direttamente sull'osso. Durante questa operazione attivare leggermente il distrattore di modo che i pin della placca possano penetrare nell'osso. Tenere presente che i fori per osteosintesi della placca di sostegno sono rivolti verso la parte anteriore. Effettuare ora un ulteriore fissaggio di sicurezza avvitando sull'osso le due viti Drill-Free in dotazione attraverso i fori delle placche di sostegno. Per controbilanciare eventuali movimenti del corpo di distrazione durante la fase di consolidamento è necessario serrare il controdado. 96 V. 3.3 Istruzioni per l’uso Distrattori RPE AVVERTENZA Pericolo di guasto del sistema a causa dell’uso non conforme! L’uso non conforme (adattamento non consentito) del sistema di distrazione da parte dell’utente può causare guasti del sistema e provocare lesioni! Si prega di osservare quanto segue: • Il sistema di distrazione non deve essere mai piegato o adattato ai contorni ossei. • Nei casi in cui si usa la lunghezza massima di distrazione, prima di rimuovere l’attivatore a conclusione del processo di distrazione, lo si deve ruotare di mezzo giro all’indietro. Questo modo di procedere evita un danneggiamento del mandrino di attivazione in seguito a rotazione accidentale in fase di distacco. AVVERTENZA Pericolo di soffocamento! Se il distrattore si allenta, sussiste il pericolo di ingestione o di inalazione involontaria del distrattore. Ad impianto effettuato, fissare ulteriormente il distrattore con l'ausilio delle due viti Drill-Free in dotazione. A tal fine avvitare le viti Drill-Free nell'osso attraverso i fori delle placche di sostegno. AVVERTENZA Pericolo a causa di insufficiente mobilizzazione dei segmenti ossei! La mobilizzazione insufficiente causa un aumento delle forze che agiscono sul sistema di distrazione, che possono avere come conseguenza la deformazione o la rottura del sistema di distrazione stesso. Si prega pertanto di osservare quanto segue: • I segmenti ossei da sottoporre a distrazione devono essere mobilizzati prima della distrazione (corticotomia od osteotomia totale). Per verificare la mobilizzazione occorre eseguire, durante l’operazione, un test di attivazione di almeno 1 – 3 mm sul sistema di distrazione impiantato, che crei un'incisura continua dell'osteotomia. • Evitare la deformazione del sistema di distrazione durante il processo di distrazione. V. 3.3 97 Istruzioni per l’uso Distrattori RPE 14 Prima della distrazione (fase di latenza) • Per la prima stabilizzazione attivare leggermente il distrattore RPE. Tener presente che le due placche effettuano la distrazione parallelamente dalla posizione di base, e non effettuano la loro corsa solamente da un lato. • Ad avvenuta attivazione di prova ruotare nuovamente il distrattore in senso opposto riportandolo in posizione iniziale. Le affermazioni in merito al protocollo di distrazione nella letteratura sono piuttosto eterogenee, prevedono tuttavia di norma i seguenti parametri. Diversamente da ciò, fattori terapeutici o specifici del paziente possono modificare il protocollo di distrazione, ad esempio età, anamnesi, situazione dei tessuti molli, dentatura, oppure metodi ortodontici di trattamento, ecc. Dopo l’impianto di un sistema di distrazione, occorre osservare innanzitutto una fase di latenza, prima di iniziare la fase di distrazione vera e propria. La fase di latenza deve essere valutata dal chirurgo in base al tipo di indicazione e dura normalmente ca. 5 – 7 giorni (a seconda del paziente e dell’indicazione). 98 V. 3.3 Istruzioni per l’uso Distrattori RPE 15 Durante la distrazione (fase di distrazione) Il distrattore può essere attivato molto facilmente con l'ausilio della chiave di attivazione (Cod. Art. 515659007 oppure Cod. Art. 515659507). Una rotazione di 120° (fino alla successiva codifica a colori) corrisponde a una lunghezza di distrazione di 0,33 mm. Si consiglia una rotazione di 120° – 240° al giorno, pari a una lunghezza di distrazione di 0,33 – 0,66 mm al giorno. Il progresso della distrazione deve essere seguito tramite controlli medici effettuati regolarmente. Una trazione eccessiva del tessuto molle, ad esempio, può causare una deformazione del sistema di distrazione. Questa possibile deformazione deve essere controllata tramite esami radiografici regolari. Se nonostante la deformazione si prosegue con la distrazione, può sopravvenire una rottura da sovraccarico del distrattore o un distacco delle viti dall’osso. AVVERTENZA Pericolo di guasto del sistema a causa dell’uso errato da parte del paziente! L’uso errato del sistema di distrazione da parte del paziente può causare guasti del sistema e provocare lesioni al paziente stesso. Si prega pertanto di osservare quanto segue: • Istruire il paziente ed assisterlo dopo l’operazione. • Il sistema di distrazione non deve essere curvato o modificato. • Durante l’intero periodo del trattamento è necessario tralasciare tutte le attività fisiche che potrebbero avere ripercussioni sul sistema di distrazione o sugli elementi ossei ad esso collegati. Ciò vale in particolare per il carico di pesi (pericolo di caduta) e per le attività sportive che potrebbero causare urti o colpi. • Adottando misure di protezione (quali mascherina, stecche, casco, ecc.), occorre accertarsi che il sito di distrazione non venga sovraccaricato (anche durante il sonno). • Osservare la direzione corretta di attivazione (freccia direzionale sul corpo di distrazione). • Dopo ciascuna attivazione/distrazione bloccare il distrattore, in modo da proteggerlo da eventuali torsioni indesiderate, fissando il controdado grigio. V. 3.3 99 Istruzioni per l’uso Distrattori RPE 16 Dopo la distrazione (fase di consolidamento) Indicazioni • La fase di consolidamento è di circa 3 – 6 Monate, a seconda del paziente. • Il sistema di distrazione può essere rimosso solo in seguito al completo consolidamento osseo, per non compromettere il risultato della distrazione. • Una volta terminata la fase di distrazione, l’attivatore dovrebbe tuttavia essere rimosso. • Il trattamento fisioterapeutico può essere iniziato già durante il periodo di consolidamento, per evitare una contrattura da adduzione. • Il trattamento ortodontico della mandibola può essere iniziato già durante la fase di consolidamento. • Quando è terminata la fase di consolidamento, occorre rimuovere tutti i componenti del distrattore. • Il tipo e l’entità del trattamento postoperatorio e/o delle misure di riabilitazione devono essere accordate con il paziente dal medico curante. AVVERTENZA Possibile guasto del sistema in seguito a riutilizzo! De gebruiker (behandelend arts of opererend chirurg) bepaalt afhankelijk van beoordeling van de desbetreffende anatomische en functionele eisen van de patiënt soort, samenstelling en gebruik van elk distractiesysteem en is derhalve hiervoor verantwoordelijk. • Gebrüder Martin is niet verantwoordelijk voor complicaties door een verkeerde indicatie, een verkeerd gekozen implantaat, een verkeerde combinatie van systeemcomponenten en operatietechniek evenals grenzen van de behandelingsmethode of onvoldoende hygiëne. • Alle distractiecomponenten, instrumenten en toebehoren moeten zodanig beschikbaar zijn dat ze direct gereed zijn voor gebruik. • Het product moet conform indicatie worden toegepast. 104 V. 3.3 Gebruiksaanwijzing RPE distractoren 1.3 Garantie Hier gelden onze algemene verkoopvoorwaarden in de telkens geldige versie. Overeenkomsten die daarvan afwijken, beperken de wettelijke rechten van de koper niet. Een garantie die verder gaat, dient contractueel te worden vastgelegd en sluit vandalisme aan componenten, software-updates en verbruiksartikelen uit. Belangrijke opmerkingen Het product mag uitsluitend worden gerepareerd door Gebrüder Martin of door een persoon of firma die hiertoe door Gebrüder Martin uitdrukkelijk is gemachtigd. Indien de reparatie wordt uitgevoerd door een door Gebrüder Martin geautoriseerde persoon of firma, dan wordt de exploitant van het product dringend verzocht een verklaring over aard en omvang van de reparatie te verlangen van deze persoon of firma. Deze verklaring moet zijn voorzien van de datum van de reparatie, de gegevens van de firma en een handtekening. Indien de reparatie niet door de fabrikant zelf wordt uitgevoerd, moeten gerepareerde producten bovendien zijn voorzien van het kenmerk van degene die de reparatie heeft uitgevoerd. Bij ondeskundige ingrepen of veranderingen door derden tijdens de verjaringstermijn vervallen alle aanspraken op garantie. Handelingen die zonder autorisatie aan het product worden uitgevoerd, hebben tot gevolg dat de aanspraak op garantie tegenover Gebrüder Martin verloren gaat. 1.4 Ontvangstcontrole Controleer de levering direct na ontvangst op volledigheid en beschadigingen. V. 3.3 107 Gebruiksaanwijzing RPE distractoren 3 Gebruiker • Het distractiesysteem mag uitsluitend worden gebruikt door medisch geschoold personeel dat op grond van zijn vakopleiding, kennis, ervaring en kennis van de desbetreffende voorschriften voor het gebruik van dit systeem is gekwalificeerd. • De gebruiker dient bekend te zijn met de regels van de geneeskunst, de stand van de wetenschap en de inhoud van desbetreffende wetenschappelijke publicaties van de medische auteurs. • De gebruiker is verantwoordelijk voor het vermijden resp. reduceren van algemene risico's bij chirurgische ingrepen. • De gebruiker: — draagt de verantwoordelijkheid voor het vakkundig uitvoeren van de chirurgische ingreep — moet operatietechniek, implantaatassortiment evenals instrumentarium en toebehoren kennen en beheersen — is verantwoordelijk voor de samenstelling van de implantaatcomponenten en hun vakkundige implantatie NOTICE Aanbeveling voor gemakkelijke traceerbaarheid van het ingezette implantaat. In het dossier van de patiënt kunnen de gebruikte implantaatcomponenten met artikelnummers, artikelbenaming en lot- en/of serienummers gedocumenteerd worden. Alleen de documentatie van de lot- en/of serienummers garandeert de eenduidige traceerbaarheid van de ingezette implantaten. 4 Implantaatmaterialen Opmerkingen met betrekking tot implantaatmaterialen vindt u op onze internetpagina: www.klsmartin.com 108 V. 3.3 Gebruiksaanwijzing RPE distractoren 5 Indicaties • Extreme transversaal, uni- of bilateraal te smalle bovenkaken, bij syndroom en nietsyndroom patiënten. • Nauwe stand van de anterieure tanden en “buccal corridors” (donkere ruimtes tussen tandboog en wang, dead spaces). Transversaal te smalle bovenkaken bij jongeren en volwassenen komen zowel bij syndroom patiënten als bij niet-syndroom patiënten en schisispatiënten vaak voor. Uni- en bilateraal te smalle bovenkaken kunnen met behulp van de chirurgisch ondersteunde maxillaire expansie (SARPE) behandeld worden. De behandeling bestaat uit een combinatie van kaakorthopedie en chirurgie. Ze maakt een verbeterde uitrichting van tanden en dentale implantaten aan de tandboog mogelijk. De procedure maakt bovendien ook een aanzienlijke expansie van de basis van de wortels van de maxillaire tanden en van de palatale welving mogelijk. De beschikbare ruimte voor de tong en om correct te slikken wordt vergroot, en er wordt verhinderd dat de bovenkaak opnieuw inzakt. Daarnaast kan een eenduidige verbetering van de nasale ademhaling gepaard met een vergroting van de neusvleugels tot volledig normale waarden worden vastgesteld. Vroegere vormen van de expansiedistractoren zijn dentaal gedragen (bijvoorbeeld de Hyraxschroef). Deze bezitten echter enkele ernstige nadelen. • Tandheelkundige problemen, zoals buccale wortelresorptie en fenestratie van de corticalis. • Omkantelen van hele tandsegmenten of van de ankertanden • Cariës bij syndroom patiënten met slechte mondhygiëne In tegenstelling daartoe worden botgedragen distractoren aangebracht op een hogere plaats in de palatale welving. Deze maxillaire expansies zijn orthopedisch en kunnen mechanisch beter gestuurd worden. Bovendien werken de krachten direct op het bot; omkantelen van tanden of andere ongewenste neveneffecten worden verhinderd. De verwijdering van de distractor na afgelopen distractie- en consolidatietijd kan heel gemakkelijk en onder lokale verdoving worden uitgevoerd. V. 3.3 109 Gebruiksaanwijzing RPE distractoren 6 Contra-indicaties Bij negeren van de contra-indicaties bestaat gevaar voor ernstig of zelfs dodelijk letsel van de patiënt! • Manifeste infecties • Verdenking van materiaalgevoeligheid of allergieën tegen de gebruikte implantaatmaterialen • Auto-immuunziekten • Doorbloedingsstoornissen, systemische aandoeningen en stofwisselingsstoornissen • Onvoldoende kwantitatief of kwalitatief hartweefsel en weke delen • Bottumoren in het gebied van de implantaatverankering • Secundaire aandoeningen, bijv. degeneratieve ziekteprocessen met een negatieve uitwerking op het genezingssucces • Gebruik in gebieden die aan onredelijke kracht- of gewichtsbelastingen zijn blootgesteld • Patiënten die op grond van hun mentale, neurologische of fysieke gesteldheid niet bereid of in staat zijn de instructies in de postoperatieve fase op te volgen • Patiënten met een slechte algemene toestand in de zin van een algemene contra-indicatie (bijv. patiënten die worden bestraald) • Osteoporose of osteomalacie en andere ernstige beschadigingen van de botstructuren die een stabiele verankering van de implantaatcomponenten in de weg staan • Manifest medicijn-, drugs- en/of alcoholmisbruik • Extreem laag verhemelte: In het geval van een laag verhemelte verliezen de steunplaten houvast en de distractor verliest zijn stabiliteit. • Schaarbeten van de klasse II: De distractor of de activeringsspil zouden met de tanden van de onderkaak in aanraking kunnen komen. Dit kan worden vermeden door de distractor hoger (distaal) in de palatale welving aan te brengen of door tijdens de distractie- en consolidatieperiode een splint te dragen. 110 V. 3.3 Gebruiksaanwijzing RPE distractoren 7 Mogelijke bijwerkingen en complicaties • Afnemen van de botdichtheid en/of botnecrosen op grond van belasting • Vaatveranderingen • Luxaties of subluxaties • Vroege en late infecties, oppervlakkige en/of diepe infecties • Zenuw- en vaatletsel • Hematomen en stoornissen in de wondgenezing • Peri-articulaire verkalkingen • Beperkte gewrichtsfunctie en beweeglijkheid • Beperkte gewrichtsbelasting en gewrichtspijn • Allergische reactie of gevoeligheid tegen het implantaatmateriaal • Zenuwschade door chirurgisch trauma • Onvoldoende botvorming of vertraagde botgenezing, osteoporose, osteolyse, osteomyelitis, geremde revascularisatie of infecties, botnecrosen, wat allemaal loslaten, verbuigen, scheuren of breken van het distractiesysteem tot gevolg kan hebben • Onredelijke kracht- of gewichtsbelasting kan slijtage, breuk, verbuigen, loslaten of migratie van het distractiesysteem tot gevolg hebben • Distractiesystemen kunnen na de consolidatiefase loslaten, corroderen, verschuiven of pijn veroorzaken. Dit is vooral bij jonge, lichamelijk actieve patiënten het geval • Ook na het verwijderen van het distractiesysteem moet de patiënt postoperatief goed worden begeleid, om een fractuur van het nieuwe botweefsel te vermijden • Groeibeperkingen • Pijn, ongemak en ongewone gewaarwordingen door het implantaat en tastbaarheid van het implantaat • Verhoogde reactie van de weke delen rond de osteotomie en/of van het distractiesysteem • Slechte genezing • Weefselverkleuringen door contact met implantaatmateriaal • Fragmenten en microscopisch kleine deeltjes kunnen loslaten van het distractiesysteem en van de implantatieplek migreren. Ook na verwijderen van een metalen implantaat kan het voorkomen dat dergelijke metaaldeeltjes in het lichaam achterblijven. De uitwerkingen op lange termijn van dergelijke deeltjes zijn momenteel niet bekend Afgezien van de bovengenoemde bijwerkingen zijn bij elke chirurgische ingreep complicaties mogelijk, zoals bijv. infecties, zenuwletsel en pijn, die niet aan het distractiesysteem te wijten zijn. V. 3.3 111 Gebruiksaanwijzing RPE distractoren 8 Verpakking • Na levering van het product moet worden gecontroleerd of de originele verpakking en het verpakkingszegel intact zijn. Verpakkingen met een opengebroken zegel kunnen niet worden teruggenomen. • Distractiesystemen die niet steriel zijn verpakt, moeten voor voorbereiding uit de verpakking worden genomen. • Steriel verpakte implantaten kunnen zonder verdere voorbehandeling direct uit hun steriele verpakking worden aangebracht. • Steriele implantaten met (ook per ongeluk) geopende of beschadigde steriele verpakking evenals implantaten die uit verpakkingen met een verstreken vervaldatum werden genomen, moeten als niet steriel worden beschouwd en moeten vóór hun gebruik aan een gevalideerde voorbereidingsmethode worden onderworpen. 9 Opbouw en werkwijze van het distractiesysteem De RPE distractor (RPE = Rapid Palatal Expander) van KLS Martin is een botgedragen distractor. Hij kan heel gemakkelijk geappliceerd en geactiveerd worden. De werkwijze berust op twee contraroterende schroefdraden met identieke spoed. Bij het activeren van de distractor boren de 2 mm lange pennen van de steunplaat zich direct in het bot, het apparaat wordt automatisch gestabiliseerd. Een aanvullende fixering van de distractor aan het bot met behulp van de beide meegeleverde maxDrive® Drill-Free schroeven is vereist. Na elke activering/distractie moet de distractor geblokkeerd worden door de grijze contramoer (3) vast te draaien, opdat deze beveiligd is tegen ongewenst verdraaien. (Zie Overzicht Typ distractoren, pagina 6). 10 Keuze van het distractiesysteem Het combineren van verschillende materialen of systemen moet worden vermeden! Distractiesystemen van KLS Martin mogen niet worden gecombineerd met systemen van andere fabrikanten. Indien instrumenten worden gebruikt die niet specifiek voor gebruik in combinatie met implantaten zijn bedoeld, dan kan het distractiesysteem falen. 112 V. 3.3 Gebruiksaanwijzing RPE distractoren 11 Voorbereiding, reiniging, desinfectie en sterilisatie Distractiesystemen zijn geschikt voor machinale voorbereiding/thermische desinfectie. Ze kunnen worden bereid met de programma's die voor chirurgisch instrumentarium zijn goedgekeurd. Voor de reiniging moet goed nota worden genomen van de informatie van de fabrikanten van de reinigingsautomaten evenals van de fabrikanten van de behandelingsmiddelen. Het proces inclusief de belading moet garanderen dat alle resten afdoende worden verwijderd. Infectiegevaar door niet-steriele werkwijze! Verkeerde sterilisatie en een niet-steriel gebruik van het distractiesysteem kunnen resulteren in ernstige gezondheidsrisico's voor de patiënt. De sterilisatie dient te geschieden volgens een gevalideerde stoomsterilisatiemethode, bijv. in een sterilisator conform EN 285:2009 en gevalideerd conform ISO 17665-1:2006. OPMERKING De eigenaar/gebruiker van het product is verantwoordelijk voor de reiniging, desinfectie en sterilisatie van distractiecomponenten. Hierbij dient absoluut goed nota te worden genomen van nationale regelingen, inclusief eventuele beperkingen. Voor de sterilisatie, het erop volgende transport en opslag moeten dienovereenkomstige goedgekeurde sterilisatieverpakkingen (bijv. conform EN 868, ISO 11607) worden gebruikt. Besmette en/of gebruikte implantaatcomponenten mogen niet hergebruikt en derhalve ook niet opnieuw voorbereid worden. V. 3.3 113 Gebruiksaanwijzing RPE distractoren 12 Vóór de implantatie Opmerkingen • Distractiesystemen moeten vóór hun gebruik de gehele voorbereidingscyclus van geschikte reinigings-, desinfectie- en sterilisatiemethodes doorlopen. • Steriel verpakte implantaten kunnen zonder verdere voorbehandeling direct uit hun steriele verpakking worden aangebracht. • Om te voorkomen dat de verpakking van niet passende steriele implantaten wordt geopend, kan het gebruik van sjablonen erg handig zijn. Zo kan men zich vóór opening van de beschermende verpakkingen vertrouwd maken met de anatomische omstandigheden op de operatieplek en de inschatting van de botspecifieke afmetingen op voorhand controleren. Sjablonen voor dit distractiesysteem zijn voor dit doeleinde verkrijgbaar bij KLS Martin. • Vóór de implantatie moet het distractiesysteem visueel worden gecontroleerd en bij wijze van proef worden geactiveerd: overtuig u ervan dat activeringsspil (2) van het distractiesysteem gedraaid kan worden (afbeelding zie Overzicht Typ distractoren, pagina 6), en dat de distractor daarbij correct beweegt. • Er wordt aangeraden om vóór de implantatie een computertomografische 3D-scan uit te voeren en/of een stereolithografisch model te maken, om de optimale positie van het distractiesysteem te bepalen of op voorhand aanpassingen uit te voeren. Individuele anatomische modellen kunnen bij KLS Martin worden besteld (www.klsmartin.com). OPMERKING Mogelijke verkeerde interpretatie van de onderzoeksresultaten! Distractiesystemen kunnen zowel bij CT-scans (ComputerTomografie) als bij MRI-scans (Magnetic Resonance Imaging) storende artefacten veroorzaken. 114 V. 3.3 Gebruiksaanwijzing RPE distractoren Gevaar voor verbranding of onbedoelde beweging van het implantaat bij gebruik van MRI! Vanwege de verdere ontwikkeling en verder toenemende energiedichtheid van de MRIapparatuur kan een negatieve uitwerking op implantaten in de toekomst niet worden uitgesloten. Bij twijfel is een MRI-onderzoek niet toegestaan, wanneer mogelijk letsel voor de patiënt niet kan worden uitgesloten. • Bij het maken van MRI-scans bestaat het gevaar van verhitting en beweging van mogelijk op het magnetische veld reagerende implantaten door het magnetische veld. • Bij externe fixateurs van staal moet de MRI-compatibiliteit van geval tot geval worden gecontroleerd. • Stalen implantaten conform ISO 5832-1 worden als para- of niet-magnetisch geclassificeerd. • Titaan is absoluut niet magnetisch. 13 Intra-operatieve procedure De operatie wordt uitgevoerd onder volledige narcose, bij voorkeur met endotracheale intubatie. Er worden standaard-corticotomieën van het anterieure, laterale en mediale bovenkaakbeen uitgevoerd. In geval van een posterieure (parallelle) expansie moet bovendien een snede in het pterygomaxillaire bereik (posterieure osteotomie) worden uitgevoerd. Over het doorsnijden van het neustussenschot verschillen de artsen van mening. Het slijmvlies van de premolaren wordt geïnjecteerd met een anestheticum en een vaatvernauwende stof. De RPE distractor wordt met de steunplaten bevestigd aan de mucosa boven de wortels van de tweede premolaren (in geval van een anterieure (3:2) expansie) en van de eerste molaren (in geval van een posterieure expansie). De activeringsspil bevindt zich in het midden, hij mag niet tegen de tandocclusie aan stoten. De distractor wordt iets geactiveerd, opdat de afdrukken van de pennen duidelijk zichtbaar zijn op de mucosa. De mucosa aan de anterieure occlusale zijde wordt nu ingesneden. De distractor wordt gedeactiveerd en weer verwijderd. Het slijmvlies van het verhemelte wordt verwijderd, waarbij een lokale hemostase wordt uitgevoerd. De RPE distractor wordt nu opnieuw, ditmaal echter met de steunplaten, direct op het bot aangebracht. De distractor wordt daarbij iets geactiveerd, zodat de pennen van de platen in het bot kunnen dringen. Daarbij moet men erop letten dat de osteosynthesegaten van de steunplaat naar anterieur zijn gericht. Nu wordt voor een extra beveiliging gezorgd door de beide meegeleverde Drill-Free schroeven door de gaten van de steunplaten in het bot te schroeven. Om eventuele bewegingen bij de distractor tijdens de consolidatiefase tegen te gaan, is het vereist om de contramoer vast te draaien. V. 3.3 115 Gebruiksaanwijzing RPE distractoren Gevaar van systeemfalen door verkeerd gebruik! Verkeerd gebruik (verkeerde bewerking) van het distractiesysteem door de gebruiker kan systeemfalen en letsel tot gevolg hebben! Let daarom op het volgende: • Het distractiesysteem mag nooit worden gebogen of gemanipuleerd voor aanpassing aan de botcontouren. • In gevallen waarbij naar de maximale distractielengte wordt gestreefd, moet de activator een halve slag worden teruggedraaid, voordat deze aan het einde van de complete distractie wordt afgeknipt. Deze werkwijze voorkomt een beschadiging van de activeringsspil door per ongeluk draaien bij het afknippen. Verstikkingsgevaar! Als de distractor loskomt, dan bestaat het gevaar dat hij per ongeluk ingeslikt of ingeademd wordt. Na implantatie de distractor extra beveiligen met behulp van de beide meegeleverde Drill-Free schroeven. Schroef daartoe de Drill-Free schroeven door de gaten van de steunplaten in het bot. Gevaar van onvoldoende mobilisering van de botsegmenten! Onvoldoende mobilisering leidt tot een verhoging van de krachten bij het distractiesysteem, die verbuigen of breken van het distractiesysteem tot gevolg kunnen hebben. Let daarom op het volgende: • De uit te rekken botsegmenten moeten vóór de distractie worden gemobiliseerd (corticotomie of complete osteotomie). Ter controle van de mobilisering moet het geïmplanteerde distractiesysteem intra-operatief bij wijze van proef ten minste 1 – 3 mm worden geactiveerd, waarbij een doorlopende osteotomiespleet ontstaat. • Het distractiesysteem mag tijdens de distractie niet verbuigen. 116 V. 3.3 Gebruiksaanwijzing RPE distractoren 14 Vóór de distractie (latentiefase) • Voor de eerste stabilisatie moet de RPE distractor iets geactiveerd worden. Gelieve er daarbij op te letten dat beide platen vanuit de uitgangspositie parallel scheiden en dat deze niet eenzijdig worden uitgeschoven. • Na proefactivering de distractor weer helemaal terug in de uitgangspositie draaien. Uitspraken over het distractieprotocol zijn in de literatuur zeer heterogeen beschreven, voorzien echter in de regel de volgende parameters. In afwijking daarvan kunnen therapeutische of patiëntspecifieke factoren wijzigingen van het distractieprotocol tot gevolg hebben, bijv. leeftijd, anamnese, toestand van week weefsel, tanden resp. kaakorthopedische behandelingsconcepten enz. Na de implantatie van een distractiesysteem moet eerst een latentiefase worden aangehouden, voordat de eigenlijke distractiefase begint. De latentiefase moet afhankelijk van indicatie door de chirurg worden beoordeeld en bedraagt ca. 5 – 7 dagen (afhankelijk van patiënt en indicatie). V. 3.3 117 Gebruiksaanwijzing RPE distractoren 15 • Tijdens de distractie (distractiefase) De distractor kan heel eenvoudig met behulp van de activeringssleutel (art. nr. 1-565-90-07 of art. nr. 51-565-95-07) geactiveerd worden. Een draaiing van 120° (tot aan de volgende kleurcodering) komt overeen met een expansie van 0,33 mm. Per dag wordt een omdraaiing van 120 – 240° aanbevolen. Dit komt overeen met een expansie van 0,33 – 0,66 mm per dag. De voortgang van de distractie moet worden bewaakt door een voortdurende controle door de arts. Teveel trekken door de zachte delen kan bijvoorbeeld tot een vervorming van het distractiesysteem leiden. Deze mogelijke vervorming moet door regelmatige röntgencontroles in de gaten worden gehouden. Als ondanks vervorming de distractie wordt voortgezet, kan dit leiden tot een breuk door overbelasting of uitscheuren van de schroeven uit het bot. Gevaar van systeemfalen door verkeerd gebruik door de patiënt! Verkeerd gebruik van het distractiesysteem door de patiënt kan een systeemdefect *of letsel van de patiënt tot gevolg hebben. Let daarom op het volgende: • Instrueer en begeleid de patiënt na de operatie. • Het distractiesysteem mag niet verbogen of in een andere vorm veranderd worden. • Tijdens de gehele behandelingsduur moeten alle lichamelijke activiteiten die nadelige uitwerkingen op het distractiesysteem of de hiermee verbonden botelementen zouden kunnen hebben, achterwege worden gelaten. Dit geldt met name voor het dragen van lasten (gevaar voor vallen) en sportieve activiteiten die schokken of slagen zouden kunnen veroorzaken. • Door beschermende maatregelen (zoals beschermingsmanchet, spalk, helm enz.) moet worden gewaarborgd dat de distractielocatie niet wordt overbelast (ook tijdens het slapen). • Let op de correcte activeringsrichting (richtingpijlen op de distractor). Na elke activering/distractie moet de distractor geblokkeerd worden door de grijze contramoer vast te draaien, opdat deze beveiligd is tegen ongewenst verdraaien. 118 V. 3.3 Gebruiksaanwijzing RPE distractoren 16 Na voltooiing van de distractiefase (consolidatiefase) Opmerkingen • De consolidatiefase bedraagt ca. 3 – 6 maanden, afhankelijk van de patiënt. • Het distractiesysteem mag pas na een volledige botconsolidatie worden verwijderd, om het distractieresultaat niet in gevaar te brengen. • Na voltooiing van de distractiefase moet de activator worden verwijderd. • Met de fysiotherapeutische behandeling kan al tijdens de consolidatiefase worden begonnen, om een adductiecontractuur te vermijden. • Met de kaakorthopedische behandeling kan al tijdens de consolidatiefase worden begonnen. • Na voltooiing van de consolidatiefase moeten alle distractorcomponenten worden verwijderd. • Aard en omvang van de postoperatieve behandeling en/of revalidatiemaatregelen moeten door de behandelend arts in overleg met de patiënt worden bepaald. Mogelijk systeemfalen door hergebruik! Beschadigde of gebruikte distractiesystemen kunnen tot letsel leiden en mogen niet opnieuw worden gebruikt.
