Resumen Abstract Objetivo: Determinar los factores predictivos para el desarrollo de PTI crónica en pacientes pediátricos del Instituto Nacional de Salud del Niño, 1998- 2003. Materiales y métodos: Estudio observacional, analítico retrospectivo de casos y controles. Los controles incluyeron toda trombocitopenia aislada < 150.000/ mm3 y duración menor de 6 meses; los casos incluyeron trombocitopenia mayor a 6 meses. Fueron excluidos los recurrentes y otras causas de trombocitopenia. Las variables fueron: edad, sexo, recuento plaquetario al momento del diagnóstico, estación de inicio de los síntomas, antecedente de infección viral y/o inmunización, forma de inicio de los síntomas y el inicio de cortico terapia. Fue utilizado el sistema SPSS y la valoración de variables con Chi2, OR con IC95% (p<0.05); y para las variables estadísticamente significativas, análisis multivariado mediante regresión logística. Resultados: 42 pacientes al grupo de casos y 122 al control. Tanto el sexo como la edad al diagnóstico no mostraron asociación significativa con la enfermedad (p=1.000 y p=0.836 respectivamente), tampoco la estación de inicio de síntomas (p=0.502). Utilizando OR y Chi2, las variables que sí mostraron asociación fueron el antecedente de infección viral y/o inmunización (p=0.017) y la forma de inicio de los síntomas (p=0.001). La media analizó las variables recuento plaquetario al momento del diagnóstico y conteo de días desde los síntomas hasta el inicio de corticoterapia; sólo ésta última tuvo significancia, con la prueba de Kruskal-Wallis, (p=0.008). La regresión logística, determinó que 81.1% del impacto viene dado por la forma de inicio de los síntomas y el conteo de días desde la presentación de los síntomas hasta el inicio de corticoterapia. Conclusiones: Las variables asociadas fueron inicio de corticoterapia y forma de inicio, el antecedente de infección viral resultó ser factor protector. Las variables no asociadas fueron edad, sexo, estación de inicio de los síntomas y el recuento plaquetario al momento del diagnóstico, en las cuales nuestros resultados no mostraron significancia estadística en relación con la cronicidad. Conclusions: The variables were associated with initiation of corticosteroid therapy and mode of onset, history of viral infection was found to be protective. The associated variables were age, sex, season of onset of symptoms and platelet count at diagnosis, in which our results showed no statistical significance in relation to chronicity.
