Schedule an appointment with your child’s health care provider EARLY so the required
Physical Exam and Immunizations are complete in time for Kindergarten entry in August

A well-child exam is recommended for all incoming 6th grade students
(Required for participation in sports/Tdap booster required for 7th grade entry)


If your child has a health issue(s) that may require attention at school, ask your health
care provider to complete an Emergency Care Plan, available at:

Bring vaccine record to appointment

If medication must be taken at school, ask your health care provider to complete a
Medication Form for each medication, available at:

Programe CON ANTICIPACIÓN una cita con el proveedor de salud médica de su hijo/a para
que la examinación médica y las vacunas estén completas a tiempo para ingresar a Kindergarten
en agosto

Se recomienda una revisión de bienestar es recomendada para los estudiantes ingresando a
6º grado. (Requerido para la participación en deportes/Refuerzo Tdap requerido para entrar al 7º grado)

Si su hijo/a tiene un problema de salud que pueda requerir atención en la escuela, solicite
a su proveedor de salud que complete un Plan de Atención de Emergencia, disponible en:

Lleve su reporte de vacunas a su cita

