REGISTRATION APPOINTMENTS FOR SATURDAY MARCH 12, 2016 WILL BE
GIVEN OUT ON FRIDAY MARCH 11, 2016, 2:00 PM IN THE OFFICE.
APPOINTMENT CARDS AND TIMES WILL BE ON A FIRST COME FIRST SERVE BASIS.
Hours on Saturday March 12, 2016 will be 7:30 am – 12:00 pm
Registration will continue the following week by appointment only.
ALL of the following items must be presented in order to register on Saturday.

Immunization Records (must be current)
Copy of most recent physical: Due before entering the 1st grade
Verification of Address – (utility bills - gas, electric or water)
Incomplete registration packets WILL NOT BE ACCEPTED.
CVUSD HEALTH BASED CLINIC WILL BE ON CAMPUS
MARCH 10, 2016 AND MAY 26, 2016 FROM 8:30 AM – 11:30 AM
Please take advantage of this clinic that offers FREE health care for children with insurance and
low income/ uninsured children. This program assists with application assistance for Covered
California/Medi-Cal and other health insurance programs such as IEHP and Molina. This includes
Physicals, Immunizations and Sick Care Treatment. Children with insurance can take advantage
of Physicals and Sick Care Treatment but Vaccines are not covered.
TRANSITIONAL KINDERGARTEN
You will have the option of attending Transitional Kindergarten if your child turns 5 years of
age between September 2, 2016 and December 2, 2016. Transitional Kindergarten PreRegistration is at the Office of Student Support Services located at 13453 Ramona Avenue in
Chino. TK is a two year program. Upon completion of TK, the student will be required to attend
a full year of traditional kindergarten. Each student will return to his/her neighborhood school
for kindergarten.

El viernes, 11 de marzo del 2016, a partir de las 2:00 p.m. daremos las citas en la
oficina para las inscripciones del sábado, 12 de marzo del 2016.
Las citas con la hora serán dadas en orden de “el primero en llegar es el primero en servir”.
El horario para inscribir a su niño/a el sábado, 12 de marzo del 2015 es de 7:30am – 12:00pm
Las registraciones continuarán la semana siguiente, únicamente por cita .
Todo lo siguiente será necesario para la inscripción el sábado:
 Acta de Nacimiento
Registro de vacunas (debe ser reciente)
 Copia del examen físico mas reciente
 Comprobante de domicilio – (facturas de servicios públicos - gas, electricidad ó agua)

CLINICA DE SALUD CVUSD ESTARÁ EN LA ESCUELA
EL 10 DE MARZO Y EL 26 DE MAYO DE 8:30 AM A 11:30 AM
Por favor aproveche esta clínica que ofrece cuidado de salud gratis para niños con asuguranza
médica, niños de bajos ingresos/sin aseguranza médica. Este programa asiste con las
aplicaciones de Covered California/Medi-Cal y otros programas de aseguranza tales como IEHP y
Molina. También incluye físicos, vacunas, tratamiento de enfermedades. Los niños con
aseguranza médica pueden aprovechar los físicos y tratamiento de enfermedades pero las
vacunas no son cubiertas.
Kindergarten Transicional
Usted tendrá la oportunidad de que su hijo asista a Kinder Transicional si su hijo/a cumple 5
años de edad entre el 2 de septiembre y el 2 de diciembre, 2016. Pre-registraciones para
Kinder de Transición es en la Oficina de Servicios de Apoyo al Estudiante ubicada en 13453
Ramona Ave en Chino. Kinder de Transición es un programa de dos años. Al término de los
conocimientos del Kinder Transicional, se requerirá que el estudiante asista un año completo al
Kinder tradicional. Los estudiante regresarán a su escuela correspondiente para kinder
tradicional.
