Waiver, Release and Indemnity

Harding Auction Childcare through the City of El Cerrito. In doing so, I hereby absolve the Harding PTA, its officers, volunteer
auction committee members and volunteers from all liability that may arise as the result from above-named child’s participation
in the activities listed above. I recognize and acknowledge that there are certain risks inherent in my child’s participation in said
program and I agree to assume the risk of accident or injury sustained in connection with the program. I further release the
City of El Cerrito, its officers, employees, agents and volunteers from all liability that may arise as the result from above-named
child’s participation in the listed activities. I realize that the City of El Cerrito is not responsible for lost or stolen articles. I have
read and fully understand the terms of this Waiver, Release and Indemnity.
REFUNDS: There are no refunds for activities sponsored by the City of El Cerrito unless a medical emergency occurs or the
activity is cancelled due to lack of enrollment.

hijo(s) nombrados en los actividades mencionadas. Reconozco que hay riesgos inherentes en la participación de mi(s) hijo(s)
en el programa mencionado y estoy de acuerdo de asumir todo los riesgos de accidente o herida causado en conección
con el programa. Adicionalmente exonero el “City of El Cerrito” y sus oficiales, empleados, agentes y voluntarios de toda la
responsabilidad por la participacion de mi(s) hijo(s) nombrados en las actividades mencionadas. Entiendo que el “City of El
Cerrito” no tiene responsabilidad por articulos perdidos o robados. He leido y entiendo completamente los términos de esta
renuncia, liberación e indemnización.
REEMBOLSOS: No hay reembolsos para actividades patrociadas por el “City of El Cerrito” al menos que ocurra una emergencia
medica o la actividad seá cancelada por falta de participantes.
