Las resistencias a los fármacos antirretrovirales constituyen un problema importante en los pacientes VIH+. La información que proporciona su determinación resulta crucial en la clínica a la hora de instaurar el tratamiento más idóneo. Sin embargo, la interpretación del genotipo es compleja y está sujeta a cambios frecuentes, tanto por la incorporación de nuevos fármacos como por la aparición de nuevos patrones de resistencia. En las tablas que se exponen a continuación se presenta la interpretación simplificada de los genotipos de resistencia a cada uno de los antirretrovirales disponibles hasta la fecha. Con el fin de valorar las mutaciones más frecuentes en nuestros pacientes, se ha tenido en cuenta la prevalencia de las mutaciones de resistencia en la base de datos de la Red de Investigación en SIDA (RIS). Se ha ponderado el peso específico de cada mutación para cada fármaco, por lo que la interpretación viene dada por la suma del peso de cada mutación según la puntuación del grupo al que pertenezcan y el fármaco considerado (grupo 1, 2, 3). En esta nueva edición hemos incorporado información sobre resistencias a análogos de nucleós(t)idos (AN) e inhibidores de la polimerasa del virus de la hepatitis B (VHB) empleados en el tratamiento de la infección por este virus. Las mutaciones de resistencia a AN son específicas de fármaco, aunque también se han identificado mutaciones que pueden conferir resistencia cruzada entre los diferentes antivirales. La tasa de incidencia de aparición de resistencias en el VHB varía entre los diferentes fármacos y se incrementa progresivamente durante el tiempo de tratamiento, siendo la más alta para lamivudina (3TC) y la más baja para tenofovir (TDF). Teniendo en cuenta que en pacientes coinfectados VIH-VHB se emplea un régimen triple con dos fármacos activos también frente al VHB, hemos considerado interesante incluir un apartado sobre mutaciones de resistencias a AN en el VHB y su interpretación. Además, ante los avances en el tratamiento de la infección de la hepatitis crónica por el virus de la hepatitis C (VHC), se ha incorporado también un apartado sobre resistencias a los nuevos antivirales de acción directa (DAA). A pesar de la potencia antiviral demostrada por estos fármacos, la rápida emergencia de mutaciones de resistencia a los DAA podría comprometer la eficacia de estas nuevas terapias. Si tenemos en cuenta la extraordinaria variabilidad genética en la infección por VHC, la eficacia de estos fármacos podría variar entre los diferentes genotipos y subtipos del virus debido, principalmente, a la presencia de polimorfismos naturales asociados con resistencia a algunos de estos compuestos. Aunque la utilidad de los estudios de resistencia a estos fármacos para guiar las decisiones terapéuticas todavía está por determinar, Guía de Resistencias 2013 Introducción 7 de laboratorio que forman parte de la Plataforma de Resistencias de la RIS. Tras la buena acogida de las ediciones anteriores, pretendemos facilitar con esta actualización la interpretación de los genotipos de resistencia a los médicos responsables del manejo de estos pacientes en España. También puede accederse a esta guía y utilizar el algoritmo para la interpretación de resistencias de la RIS a través del programa que está disponible en la página web de la red: www.retic-ris.net. Guía de Resistencias 2013 es importante conocer los perfiles de resistencia a estas nuevas familias de fármacos con el fin de evaluar su posible impacto en la respuesta a las nuevas terapias y poder así optimizar su uso terapéutico. Los datos aquí presentados han sido fielmente contrastados con la bibliografía más recientemente publicada sobre resistencias fenotípicas con datos de respuesta clínica, otras guías internacionales, especialmente Stanford Drug Resistance Database, el Panel IAS-USA, Celera y Trugene, y la opinión de expertos clínicos y 8 – En todos los nuevos diagnósticos de infección por VIH se recomienda realizar un estudio de resistencias de la RT y la PRO • En el momento del diagnóstico • Antes de iniciar tratamiento antirretroviral * El estudio de resistencias en la región de la IN se realizará solo si existe sospecha de transmisión de resistencias a esta familia – En el fracaso virológico (CV > 500 copias/ml) * Se analizará RT y la PRO. El resto de mutaciones han sido seleccionadas en fracasos a ENF, aunque por sí solas se desconoce su impacto específico en la resistencia. Las mutaciones N126K, N137K y S138A son secundarias y se seleccionan en la región HR2 en combinación con mutaciones en la región HR1 (posiciones 36-45) incrementando el fitness viral. N42S es un polimorfismo y no confiere resistencia. S138A es un polimorfismo natural en variantes del grupo O y N del VIH-1. IF: inhibidores de fusión; ENF: enfuvirtida. Resistencia a maraviroc Se han descrito al menos dos mecanismos de resistencia: – Cambio en el uso del correceptor: emergencia de aislados X4 preexistentes como población minoritaria en el momento basal y que estaban por debajo del límite de detección – Selección de cambios en la glucoproteína de la envuelta gp120 (especialmente en la región V3): • Cambios en las posiciones 11 y 26 de V3 • Mutaciones 4L, 11R y 19S se han asociado con peor respuesta 15 Red de Investigación en SIDA (RIS) Resistencia a inhibidores de la fusión Antagonistas del correceptor CCR5 Recomendaciones clínicas y técnicas para la determinación genotípica del tropismo del VIH-1 Recomendaciones clínicas Indicación – En pacientes naive que vayan a iniciar TAR. Realizar el test lo más próximo al inicio del tratamiento siempre que se considere MVC como opción terapéutica – En pacientes pretratados, se recomienda la determinación del tropismo del VIH, tras el fracaso a cualquier línea de tratamiento – En pacientes que requieran un cambio de tratamiento y que presenten carga viral indetectable. En este caso se recomienda realizar la determinación del tropismo a partir de ADN proviral o a partir de una muestra de plasma congelada previa a la indetectabilidad Recomendaciones técnicas – Se recomienda el uso de geno2pheno con un FPR = 10% y/o PSSM, matrices X4/R5 y SI/NSI, en el caso de que el genotipado de V3 se realice en una única RT-PCR – Se recomienda el uso de geno2pheno con un FPR = 5,75% en el caso de que el genotipado de V3 se realice por triplicado Análisis de – Se recomienda considerar las mezclas nucleotídicas para la determinación del tropismo viral. Además, la mayor variabilidad de la secuencia V3 en subtipos no B puede provocar errores de interpretación – Interpretación igual que para subtipos B excepto para variantes de subtipo C en las que se recomienda usar la “matriz C” de PSSM ADN proviral – En pacientes con carga viral ≤ 500 copias/ml, o ausencia de amplificación en plasma se recomienda la realización del tropismo viral a partir de ADN proviral en CMSP – La interpretación se realizará utilizando geno2pheno FPR: 10-20% Informe de – Se recomienda informar el tropismo como “tropismo R5” o “tropismo X4”, incluyendo como comentario el FPR si la interpretación se realiza resultados con geno2pheno.
