The plaster and the corset El tratamiento ortopédico comprende: - Un tiempo de reducciôn mediante una 0 varias escayolas de Abbott Cmodificado por Y. Cotrel: EDF = elongaciôn, desrotaci6n, flexiôn). La reducciôn se lleva a cabo en un marco por medio de bandas de tracci6n y de desrotaciôn. La escayola presenta también zonas de apoyo reforzadas progresi va mente Weltros) en correspondencia con las gibosidades y ventanas de expansiôn al nivel de las depresiones. Nosotros intentamos realizar una correc ci6n tridimensional con elongaci6n débil de alrededor de 12 Kg, insistiendo sobre la inflexi6n lateral. - Una contenci6n que viene asegurada por un corsé polivalvo de P. Stagnara de plexidur y duraluminio. La secuencia habituai es: - Dos corsés de escayola durante 45 dias: alrededor de tres meses, - Un corsé ortopédico lionés hasta la madurez ôsea. - Una reeducaciôn especifica a 10 largo de toda la duraciôn dei tratamiento. En la actualidad, hemos modificado este protocolo para las escoliosis tratadas precozmente. El tiempo duranre el que debe llevarse la escayola se puede reducir hasta 45 dias, si la correcci6n es satisfactoria en principio. El corsé de escayola se realiza con una componente de tracci6n minima. Orthopaedic therapy includes: - A timefor reduction by one or some Abbottplasters (modified by Y. Cotrel. EDF = elongation, derotation, flexion), Reduction is done within a frame, by means of traction and derotation strips. The plaster has also support areas progressively reiriforced (felts) in relation to the humps, as well as expansion windows at the leuel ofthe depressions. We try toperform a three dimensional correction with a slight elongation of about 12 Kg, insisting on a lateral inflexion. - A contention which is assured by a P. Stagna ra 's polyvalve corset manufactured in plexidur and duraluminium. The usual sequence is: - Two plaster corsets during 45 days each: about three months, - One orthopaedic Lyonnais corset until bone maturity is reached. - A specifie re-education which lasts for the whole length of therapy. Presently, we have modified th isprotocolfor the early treated scolioses. The time dun'ng which the ptaster t~ being worn may be reduced to 45 days, ifthe correction is satisfactory from the heginning. The plaster corset is manufactured with a minimal traction component. Cuando la correccian sea muy satisfactoria (reduc cian total por ejemplo) y la escoliosis sea medianamente evoluriva, es posible prever que la ortesis no se Ueve de forma continuada sino a tiempo parcia!. El protocolo de vigilancia implica: - Un primer control al cabo de cuatro meses, - Posteriormente, un control cada seis meses. Si la angulacian permanece estable, se aumenta en dos horas el tiempo de ablaci6n en cada nueva visita. En el casa de curvaturas imeriores a 30°, el corsé se quitara al final dei crecimiento estatural, rnientras que, en los demas casos, sera necesario prolongar el tiempo con corsé durante un p]azo de 12 meses. La retirada de la ortesis sera progresiva: dos horas mas diarias cada 15 dias, hasta Hevarla sola mente de noche durante un mes. En la actualidad, el tratamiento se ha hecho casi ambulatorio, con hospitalizaci6n de dia para ]a realiza cian dei corsé de escayola, realizandose la ortesis to talmente en régimen extemo. En cambio, las bandas plâsticas no permiten reemplazar la escayola por una parte porque es insuficiente el moldeo y par otra parte porque no se dispone dei ancho de 20 cm. When the correction is very satisfactory (total reduction, for instance) and the scoliosis is fairly evolutionary, it is possible to foresee a wearing of the orthesis during part of the time only. The surueillance protocol includes: - A first control once four months have elapsed, - Afterwards, a control every six months. If the angulation remains stable, the time not wearing the orthesis is increased two hours at every visil. For curves below 3(J', the corset will be remo ved at the end of statural growth. Otherwise, prolonging ils wearing for 12 more months will be necessary. The weaning of the orthesis will be progressive: two more daily hours every 15 days and finally night wearing only during one month. Presently, the treatment is practically ambulatory, with day time hospitalization to manufacture theplaster corset. The orthesis is manufactured totally as an out patient. On the contrary, theplastic strips do not allow replacing the plaster: insufficient casting is one reason and another reason is that the 20 cm width is not available. Position for negatiue modelling ofthe cast LA ORTESIS DE PLEXI-DUR POLIVALVA THE PLEXIDUR POL YVAL VE ORTHESfS Se realiza sobre un positivo tras el moldeo y consiste en un casco pélvico compuesto por dos semi-valvas que se unen mediante tomillos y chamelas en los dos soportes. El conjunto se debe perfilar de tal forma que, estando el sujeto sentado, sea posible la flexi6n de los muslos a 90° sin compresi6n, al tiempo que, por detras, el borde inferior dei casco esté por 10 menos dos traveses de dedo por encima de la superficie de apoyo. Las masas gluteas se deben mantener sin compresi6n. El sopOite anterior es torneable a petici6n a nivel del t6rax y tiene una altura regulable. Es mediano, vertical y se fija abajo sobre las barras 0 pasadores de la arti culaci6n anterior de las dos valvas pélvicas. En la parte superior termina por un trébol de tope giratorio que no debe favorecer la espalda hundida. El soporte posterior es tomeable para restitulr la cifosis. Sobre estas dos soportes es donde van fijadas las valvas de apoyo y de contra-apoyo. La valva de apoyo dorsal se aplica sobre la superficie postero-Iateral de la gibosidad. Los bordes superiores e inferiores de la valva estân cortados siguiendo la oblicuidad de las costillas correspondientes a la vértebra mas alta y a la vértebra deI limite inferior. La valva de apoyo lumbar, a diferencia de la prece dente, debe ser posterior y aplicarse directamente sobre las masas paravertebrales latero-espinosas. El borde superior dei apoyo debe dejar libres las costillas co rrespondientes a la concavidad de la deformaciôn. Esta valva lumbar se prolongara a 10 largo de la cara anterior dei t6rax sin que tenga un apoyo verdadero. A este nivel, tiene un efecto de tope inspiratolio que obstaculiza la horizontalizaciôn de Jas costillas de la concavidad. Se tiene la ventaja de poder desarticular esta valva de apoyo lumbar dei contra-apoyo torâcico anterior, 10 que facilita los ajustes durante el tratamiento ortopédico. this valve must not lean on the spinousprocesses and the immediate paravertebral areas, as this wouldfavour a hollow hack. La valva de contra-a poyo axilar esta situada teniendo como referencia a las costillas terceras, cuartas y quin tas, que corresponden a la costilla de la vértebra limite superior de la curvatura toracîca. El apoyo debe ser riguroso y debe tener una forma que deje libres con amplitud a los arcos axilares anteriores y posteriores. Posteriormente y durante el periodo de tratamiento, se lIeva a cabo en régimen ambulatorio un control c1inico, radiol6gico, reeducativo y ortésieo. La reeducaci6n vertebral que se ha ensenado durante el periodo de adaptaci6n de la ortesis se continua diariamente durante 20 minutos y debe ser controlada por un einesirerapeuta una vez a la semana. La mana opuesta al costado dei corte se apoya en el piano de trabajo dei cuarto de escayolas. La mana situada en el lado dei corte se coloca sobre la cabeza dei nino. El corte es rectilineo, realizado con la sierra de escayolas y terminado con cizalla curva para el fieltro y el jersey. Después, la escayola se separa con la pinza de Hennig. El nino coloca entonces las dos manos sobre la cabeza para que se pueda quitar defini tivamente la escayola. Tras comprobar el estado de la piel, se coloca un jersey tubular muy cenido Chabitualmente de 15 cm). En cada cruce, se levanta el pecha par un movimiento de tracciôn deI jersey al nivel dei cuello. Las otras bandas se van desenvol viendo, siempre en oblicuo, insistiendo sobre las futuras zonas de apoyo dei corsé. Es necesario ajustar bien la parte esternal anterior y los salientes condro-costales. MOLDEO ORfENTADO DE UNA ORTESIS DE PLEXIDUR POLIVALVA ORIENTED CASTING OF A POLYVALVE PLEXfDUR ORTHESfS Se pide al nino que levante los hombros y tratamos manualmente de vol':'er a crear una cifosis toracica alta. Utilizamos bandas escayoladas de 20 cm de ancho, de forja rapida. Las bandas se desenvuelven empezando par abajo; la primera banda estabiliza el jersey al nivel de la pelvis y de las crestas iliacas, desenvolviéndose una segunda banda en '<ochÛ" oblicuo, pasando par encima deI hombro derecho y descendiendo bajo el saliente condro-eostal opuesto. Du rante el paso de la banda, se tira deI jersey hacia arriba, para levantar el pecha y dejar libre el saliente condro-costal. Se pasara también una banda sobre el hombro izquier do, volviendo bajo el saliente condro-costal derecho y realizando la rnisma acciôn de tracciôn dei pecho hacia arriba. Hay que modelar bien la parte esternal anterior y los salientes condro-costales. A continuacion, se desen vuelven las bandas transversalmente de forma que recubran completamente el jersey. Para un nino de talla media, utilizamos habitualmente seis bandas. Se evita con cuidado toda tension anormal. La escayola se extien de con un espesor algo mayor al nivel de las gibosidades. We ask the child to raise his or her shoulders and we try to manually recreate a high thoracic kyphosis. We use plastered quickforge stripes, 20 cm wide. The stripes are unrolled starting at the bottom. The first one stabilizes the jersey at the level of the pelvis and the iliac crests. A second stripe is unrol/ed in an «eight» shape in oblique, passing over the right shoulder and descending under the opposite chondro-costal joint. When passing the stripe, the jersey is pulled up, so as to lift the chest and leave the chondro-costaljoint free. A passing of a stripe will also be done over the left shoulder, coming back under the right chondro-costal joint and lifting the chest in the same way as before. Care must be taken to properly cast the anterior sternal area and the chondro-costal joints. Afterwards, the stripes are unrolled transversal/y, so as to total/y cover the jersey. For a child with an average waist, we general/y use six stripes. We carefully avoid any abnormal tension. The plaster is spread a little thicker at the leuel of the humps. Las crestas se senalan cuidadosamente con el borde cubital de la mano, realizando una cubeta al nivel de las costillas falsas. Una vez que la escayola comienza a secar, modela mos las gibosidades. En el ca 50 habituai de una escoliosis doble mayor, toracica derecha y lumbar izquierda, empezamos a modelar la convexidad lumbar izquierda marcando el triangulo de la cintma y apoyando lareralmente sobre las ap6fisis transversas de la izquierda de forma que se conserve una Jordosis fisiol6gica y se deshaga la rota cian de la escoliosis. El contra-apoyo se obtiene por delante al nive! de la hemi-pelvis derecha. A continuacian, modelamos la gibosidad toracica derecha, evitando acentuar la espalda hundida median te un apoyo lateral con contra-apoyo al nivel dei salien te condro-costal izquierdo. Modelamos cuidadosamente los apoyos sub-axilares y el apoyo giratorio de delante, para volver a crear una cifosis toracica aita. La mana derecha estabiliza los salientes condro-costales, al tiempo que la mana iz quierda realiza un movimiento cifosante al nivel de la charnela cérvico-toracica. We carefully cast the sub-axillary supports and the anterior ratatory support, in order to recreate CI high thoracic kyphosis. DISCUSIÔN DISCUSSION La muestra de enfermos con escoliosis cuyos resu[ta dos hemos estudiado es idéntica a la que siIvi6 para estudiar el pOlvenir socio-profesional de los mismos. Se trataba de tratamientos ortopédicos comp[etos ho mogéneos. Hemos eliminado a los ninos que hubieran tenido un tratamiento mixto (Milwaukee, Lionés) y a los que hubieran sufrido mas tarde una intelvenci6n quirlirgi ca. El criterio de selecci6n fue la respuesta a un cuestiona rio y la existencia de una radiografia reciente. Vo[untaria mente, hemos conservado los enfermos con escoliosis cuya angulaci6n inicial era superior a 50°. La poblaciôn actual de enfermos con escoliosis trata clos ortopéclicamente se caracteriza por una disminu ciôn de la angulaci6n inicial y de la eclacl de comienzo dei tratamiento debido a la detecciôn precoz, aSl como a un mejor conocimiento de los factores de evolutividacl de las escoliosis. Estos resultados son satisfactorios por 10 que se refiere al ângulo y confirman nuestras indica ciones actuales: - Cirugla para las formas de mâs cie 40° torâcicas en los chicos. - Para las otras formas anatomo-radiolôgicas, es legiti mo un ensayo de tratamiento ortopédico para las escoliosis cie menos de 50° Si la ganancia estrucrural en el momen to cie la retirada de los corsés de escayola es superior a 9° en una racliografla dei raquis en bipedestaciôn y de cara sin contenci6n, se podrâ proseguir con el tratamjento. - Para las escoliosis de mâs de 50°, en caso de rechazo de la cirugia, el tratamienro puede toclavla estabilizar la escoliosis y evitar las repercusiones funcionales graves en la eclad adulta, sobre toclo si se trata de una forma doble mayor que necesita una artrodesis casi total dei rdquis torâcico y lumbar. Para las escoliosis lumbares y tôraco-Iumbares bajas, es interesanre comparar los resultaclos de esta serie con Jos publicados por Caton y Michel. Para una angulaciôn irucial de 35°, la angulaciôn dos anos después de la retirada de la ortesis es de 31°, es decir, una ganancia estructural de 4°. Este resultado es inferioral cie 7,54° de ganancia estructural seis anos después cie la retirada de la oItesis, obtenjdo en nuestra muesrra. Es por esta raz6n por 10 que seguimos utilizando el tratamienro OItopédico lionés clâsico en cienas fonnas graves de escoliosis lumbares, sobre todo cuando hay una fueIte rotaci6n, porque en ta] casa las dos cintu ras escapular y péJvica se estabilizan mejor. Cuando esta estabiJizaci6n no nos parece indispensable, realiza mos desde 1980 un protocolo estricro consistenre en dos lumbostatos cie escayoJa asimétricos reductores bajo desplazamiento de Min Mehta durante tres meses y, pos teriolmenre, aplicaci6n rigurosa de una oItesis de plexidur cie tres valvas, ortesis que es, cie hecho, mâs dlfkiJ de adaptaI' que una ortesis polivalva. No nos han convencido los resultaclos cie las oItesis anglosajonas, que oprimen mucha al niIÎo y que, sobre todo, son muy cleslordosanrcs, por la que se ha demostrado el riesgo evolutivo hacia la cifosis t6raco-Iumbar en la edad aclulta. Nosotros realiza mos, pues, nuestras escayolas y nuestras oItesis cie pie, en our old therapies, even if the statistical results are penalized. The characteristics of the present population of ortbopaedically treated scolioticpatients are a decrease of the initial angle and of the age wben therapy is stm1ed. This is a consequence of an earlier detection and of a better knowledge of the factors injluencing the course of the scoliosis. This results are sati~factory wilh respect to the angle and confirm our present indications. - Surgery for thoracic forms above 4Cf', in boys. - For the remaining anatomo-radiological forms, a trial of orthopaedic lherapy is adequate for scolioses belmu 5(f. If the structuralgain after the ablation ofthe plaster corset is above 9' in a spine X-ray taken in standingposition,frontal viewand without contention, tbe therapy can be conlinued. - For scolioses above 5Cf', if surgery is refused, the tberapy may still slabilize the scoliosis and avoid its functional consequences whicb are serious in the adult age, especial/y so if it is the case ofa double, majorfonn which requires an almost total arlhrodesis ofthe thoracic and lumbar spine. For lumbar and low tboraco-lumbar scolioses, it is interesting to compare the results of this series wilh those published by CATON and MICHEL For an initial angle of 35", the angle two years ajter the ablation of the orthesis is 31°, i. e., a structural gain of 4°. This result is worse than tbe 7,54° ofstructural gain sixyears after the ablation ofthe orthesis obtained in our sample. It isfor this reason tbat we continue to use the classical Lyonnais orthopaedic tberapy in certain severe forms of lumbar scoliosis. We do so mainly when there is a strong rotation, because in sucb cases both the scapular and pelvic waists are better stabilized. When we do not consider this stabilization absolutely necessary, we peifonn since 1980 a strict protocol consisting in the use of two reducing, assymetric, plaster lumbostats with Min Mehta 's shift during three months, followed by the rigourous use ofa plexidur three valve orthesis which is actually more difficult to adapt than a po(yvalve orthesis. We have not been convinced by the results obtained wilh Anglo-Saxon ortheses; they are tao oppresivefor the ch ild and, above al!, very delordosing, when the evolutional risk towards a thoraco-lumbar kyphosis in the adult age has heen shown. Cuando los tratarnientos se han iniciado entre los 15 y los 16 afios, aun comprobamos una ganancia estruc tural de 5,70°, que es practicamente equivalente a la de las escoliosis cuyo comienzo de tratamiento se sitUa entre los 13 y los 14 afios (6,70°). Por 10 tanto, no es demasiado tarde para emprender un tratamiento ortopédico a esta edad y estabilizar aSI definitivamente una escoliosis, ya que la evoluciôn continua esponra neamente hasta alcanzar la madurez ôsea completa. Duprat ha estudiado en 1976 los dolores de las escoliosis dei adulto no tratadas ortopédicamente. Los principales resultados han sido los siguientes: - Una escoliosis de cada dos produce dolores en la edad adulta. - La edad de comienzo de los dolores se sitUa en los 26 afios como media. - El valor dei angulo de la escoliosis no influye en la aparici6n de los dolores. - Las escoliosis lumbares son las que producen mas dolores (63%). Se puede, pues, sefialar que la edad de apariciôn de los dolores es idéntica, se haya tratado la escoliosis 0 no. Por el contrario, las escoliasis lumbares son las menos doloro sas en nuestra serie, 10 que es un dato a favordel tratamiento ortapédico de las escoliosis en el adolescente. Nos parece que el tratamiento ortopédico lionés ha demostrado su eficacia a largo plazo y en algunos casas sigue siendo una altemativa valida al tratamiento quirUrgi co, aun a pesarde que, coma consecuenda de los progresos técnicos, la tendencia actuallegitima y justificada es la de reducir el umbral de las inclicaciones quirUrgicas. Therefore, it is not too late to start an orthopaedic therapy at this age and to definitively stabilize in this way a scoliosis, considering that the course continues spontaneously until total bone maturity. Dupratinvestigatedin 1976 thepain in adultscolioses not orthopaedically treated. His main results are the following: - One scoliosis ofevery two haspain in the adult age. - The age of start of the pain is 26 years, as an average. - The angle value ofthe scoliosis has no influence in the appearance ofthe pain. - Lumbar scolioses are the ones which cause more pain (63%). Therefore, we can see that the age ofinitiation ofthe pain is the same with treated or non treated scolioses. On the contrary, lumbar scolioses are the least painful ones in ourseries, whichfavours orthopaedic therapy of scoliosis in teenagers. We consider that the Lyonnais orthopaedic therapy bas proved its long term efficacy and, in certain cases, it remains as a valuable alternative to surgical therapy.
