PARENTS/PADRES
Upon receiving this registration form along with the completed Parent/Guardian Consent, Video/Photography
Consent, Medical Consent, and Liability Waiver forms, the Office of Vocations will send an acceptance letter
either by email or regular mail to participants and parents. Included in this letter will be an agenda for parents and
a list of things for participants to bring for the retreat. The original and completed forms must be given to the
Explore Staff upon arrival if copies were sent. Forms are available at www.houstonvocations.com/explore-retreat
I approve of my son's desire to participate in the Explore program. I will pay the $80.00 fee for the cost of
the retreat. I have also completed and signed the Parent/Guardian Consent, Video/Photography Consent, Medical
Consent, and Liability Waiver forms as requested by the Office of Vocations.
Al recibir esta forma de registro junto con las formas firmadas necesarias, la Oficina de Vocaciones enviará a los
participantes y padres una carta de aceptación via correo postal o email. En esta carta se incluirá un horario para padres y
