Crown House, Waller Road, Devizes, Wiltshire SN10 2GH, United Kingdom PRODUCT INFORMATION - English DESCRIPTION OMNIPORE® Surgical Implants are manufactured of porous high-density polyethylene, a biomaterial that is easily contoured or carved to suit the anatomical and functional requirements of the patient. The interconnecting pores of the Porous HDPE material permits fibrovascular ingrowth into the implant. Because of the white color of the material, it will not show through overlying soft tissues. In vitro and in vivo biocompatibility studies have shown OmniPore Surgical Implants to be free from any observable systemic or cytotoxic effects. Porous HDPE Surgical Implants are provided STERILE and NONPYROGENIC. INDICATIONS OmniPore Surgical Implants in block, sheet, and anatomical shapes are intended for non-weight bearing applications of craniofacial reconstruction/cosmetic surgery and repair of craniofacial trauma. OmniPore Surgical Implants are also intended for the augmentation or restoration of contour in the craniomaxillofacial skeleton. CONTRAINDICATIONS The following are specific contraindications, adverse effects, and warnings that must be understood by the surgeon and explained to the patient. Patients who are unwilling or unable to follow postoperative instructions due to accompanying conditions (mental or physical). Foreign body sensitivity. Where material sensitivity is suspected, appropriate tests should be made to rule out this possibility prior to implantation. Use in tissue that has been compromised by cancer therapies. Limitations in blood supply and/or systemic disorders that may slow healing and increase the possibility of infection and/or rejection of the implants. Any degenerative disease process that would adversely affect proper placement of the implants. Inadequate coverage with healthy tissue. Procedures in which a non-sterile environment exists, such as sinuses. Use in load bearing areas such as the temporal mandible joint. POSSIBLE ADVERSE EFFECTS 1. Vascular changes. 2. Allergic reaction to the implant. 3. Nerve damage due to surgical trauma. 4. Deformation or fracture of the implant. Migration or loosening of the implant. Pain, discomfort, and/or abnormal sensation due to presence of the implant. Superficial and/or deep infection. WARNINGS 1. OmniPore Surgical Implants must never be re-used or re-sterilized. 2. Adequate patient instruction is a key factor in determining the success of the surgical procedure. Postoperative follow-up and care are very important. The patient must be made aware that the implants are not as strong as natural bone and that excessive physical activity or load bearing may cause loosening, migration, deformation, or fracture of the implants. 3. The success of any implants is dependent upon careful handling and good surgical technique. When shaping and contouring the implant, sharp edges should be avoided to minimize trauma to the surrounding tissue. The implants must never be shaped by use of devices that produce intense heat such as electrosurgery devices, heated knives, or lasers. 4. Porous materials are particularly at risk of contamination by foreign materials and particulate matter, including glove powder, lint from draping material, and cleaning agents. All efforts should be made to limit handling of the implants. Do not place or contour the implant on any surface that could transfer contaminants to the implant. 5. The surgeon must be thoroughly familiar with the implants, the method of application, the instrumentation, and the surgical procedure. These implants are not designed for use in weight bearing or load bearing applications. The patient must be made aware of the risks of using the implants, including possible adverse effects. 6. Responsibility for completion of adequate training, proper patient selection, and choice and placement of the implants rests with the physician. INSTRUCTIONS FOR USE The following is a guide for informational uses only. Since a number of surgical techniques may be employed, the surgeon may wish to alter details in accordance with his or her clinical experience and medical judgment. The selection of the implant, its composition, size, and shape is made by the surgeon after evaluating the anatomical and functional requirements of the patient. SHAPING THE IMPLANT OmniPore Surgical Implants are provided as basic anatomical shapes, which may be customized by the surgeon to suit the implantation site. The material may be carved using scissors or scalpel without fearing of collapsing the pores, taking care to feather the edges of the implant where it transitions to bone. The implants may be recontoured by immersing them in a basin of sterile saline heated to its boiling point. At this temperature the implant will relax, allowing it to be gently formed to the desired shape. If the implant becomes difficult to bend, it should be returned to the hot saline. Once the desired shape is obtained, the implant should be held in that shape and allowed to cool. PLACING THE IMPLANT An ample tissue pocket should be prepared to ensure that tissue is not stretched over the implant. Incisions located directly over the implant site should be avoided. If the potential exists for the implant to move before tissue ingrowth, fixation can be accomplished with sutures, K-wire, or surgical fixation screws. Fixation screws should be tightened until they are flush with the surface of the implant. Multiple pieces of implant material may be stacked and sutured together. A cutting needle can easily penetrate the surface of the implant enabling the surgeon to re-suspend tissue or muscle by attaching it to the implant. If the implants are to be placed using the intaoral route, precautions should be taken to reduce, as far as possible, the risk of contaminating the implant. CONTRE-INDICATIONS Les contre-indications, les effets secondaires et les mises en garde que le chirurgien doit comprendre et expliquer au patient figurent ci-dessous. Les risques chirurgicaux généraux qui ne sont pas inclus devront également être expliqués au patient avant l'intervention chirurgicale. 1. Infections actives. 2. Ne jamais réutiliser ni restériliser les implants chirurgicaux OmniPore. 2. Des instructions adaptées à l'attention du patient sont un facteur clé de succès pour l'intervention chirurgicale. Le suivi et les soins postopératoires sont très importants. Le patient doit être informé que les implants ne sont pas aussi résistants que les os naturels et qu'une activité physique ou une charge excessive peut entraîner un relâchement, une migration, une déformation ou une fracture des implants. 3. Le succès des implantations dépend également de la manipulation et d'une bonne technique chirurgicale. Lorsque l'implant est façonné ou sculpté, éviter les angles vifs afin de réduire les traumatismes aux tissus avoisinants. Ne jamais façonner les implants à l'aide d'équipements produisant une chaleur intense comme des dispositifs électrochirurgicaux, des couteaux chauffants ou des lasers. 4. Les matériaux poreux présentent un risque important de contamination par corps étrangers ou particules comme la poudre sur les gants, les peluches des tissus et les produits nettoyants. Veiller à limiter la manipulation des implants au strict minimum. Ne pas placer ni façonner l'implant sur une surface susceptible de transférer des contaminants sur l'implant. 5. Le chirurgien doit connaître parfaitement les implants, la méthode d'application, les instruments et la procédure chirurgicale. Ces implants ne sont pas conçus pour une utilisation dans des applications porteuses. Le patient sera informé des risques liés à l'utilisation de l'implant, y compris les effets secondaires possibles. 6. Il appartient au professionnel de santé de suivre la formation adaptée, de bien sélectionner le patient et de poser convenablement les implants. MODE D'EMPLOI Ces conseils sont uniquement fournis à titre informatif. Comme différentes techniques chirurgicales peuvent être employées, le chirurgien peut modifier les détails en fonction de son expérience clinique et de son jugement médical. La sélection de l'implant, sa composition, sa taille et sa forme sont effectués par le chirurgien après avoir évalué les besoins anatomiques et fonctionnels du patient. FAÇONNER L'IMPLANT Les implants chirurgicaux OmniPore sont fournis dans des formes anatomiques de base qui peuvent être personnalisées par le chirurgien pour correspondre au site d'implantation. Le matériau peut être sculpté à l'aide de ciseaux ou d'un scalpel sans craindre d'affaisser les pores, en veillant à façonner les bords de l'implant à la transition avec l'os. Il est possible de refaçonner les implants en les immergeant dans une bassine de sérum physiologique stérile porté à ébullition. À cette température l'implant se détend pour être façonné doucement à la forme désirée. Replonger l'implant dans le sérum physiologique chaud dès que son façonnage devient difficile. Une fois que la forme désirée est obtenue, l'implant sera conservé dans cette forme pour le laisser refroidir. POSITIONNER L'IMPLANT Une grande poche de tissu sera préparée afin de s'assurer que le tissu n'est pas étiré au-dessus de l'implant. Éviter les Incisions situées directement au-dessus du site d'implantation. En cas de risque de déplacement de l'implant avant le développement des tissus, il est possible de fixer l'implant à l'aide de sutures, d'une broche de Kirschner ou de vis de fixation chirurgicales. Les vis de fixation seront serrées jusqu'à ce qu'elles soient à fleur avec la surface de l'implant. Il est possible d'empiler plusieurs implants et de les suturer ensemble. Une aiguille coupante peut facilement pénétrer dans la surface de l'implant afin de permettre au chirurgien de suspendre à nouveau le tissu ou le muscle en le fixant à l'implant. Si les implants doivent être positionnés par voie intraorale, prendre les précautions requises pour éviter le plus possible tout risque de contamination de l'implant. KONTRAINDIKATIONEN Nachfolgend werden spezifische Kontraindikationen, Nebenwirkungen und Warnhinweise aufgeführt, die vom Chirurgen verstanden und dem Patienten erklärt werden müssen. Patienten, die wegen (psychischer oder körperlicher) Begleiterkrankungen nicht bereit oder nicht in der Lage sind, postoperative Anweisungen zu befolgen. Fremdkörperüberempfindlichkeit. Wenn eine Materialüberempfindlichkeit vermutet wird, sollten geeignete Tests durchgeführt werden, um diese Möglichkeit vor der Implantation auszuschließen. Einsatz in Gewebe, das durch Krebstherapien geschädigt ist. Eingeschränkte Blutzufuhr und/oder systemische Erkrankungen, die zu einer verzögerten Heilung und erhöhten Infektionsgefahr und/oder zur Abstoßung des Implantats führen könnten. Jegliche degenerative Erkrankung, die eine korrekte Platzierung des Implantats nachteilig beeinflussen würde. Unzureichende Deckung mit gesundem Gewebe. Eingriffe in einem Gebiet mit nicht steriler Umgebung, wie z.B. Nasennebenhöhlen. Einsatz in lasttragenden Bereichen wie z.B. dem Kiefergelenk. MÖGLICHE NEBENWIRKUNGEN 1. Gefäßveränderungen. 2. Allergische Reaktion auf das Implantat. 3. Nervenverletzung durch den chirurgischen Eingriff. 4. Deformation oder Bruch des Implantats. 5. Migration oder Lockerung des Implantats. 6. Schmerzen, Unbehagen und/oder ungewöhnliche Empfindungen auf Grund der Anwesenheit des Implantats. 7. Oberflächliche oder tiefe Infektion. WARNHINWEISE 1. OmniPore Chirurgische Implantate niemals wiederverwenden oder sterilisieren. 2. Die angemessene Unterweisung der Patienten spielt eine entscheidende Rolle beim Erfolg des chirurgischen Eingriffs. Postoperative Nachuntersuchungen und Pflege sind von großer Bedeutung. Der Patient muss darüber aufgeklärt werden, dass Implantate nicht so fest wie natürliche Knochen sind, und dass übermäßige körperliche Aktivität oder Belastung zu Lockerung, Migration, Deformation oder Bruch des Implantats führen können. 3. Der erfolgreiche Einsatz aller Implantate hängt von umsichtiger Handhabung und guter chirurgischer Technik ab. Beim Formen und Schneiden des Implantats sollten scharfe Kanten vermieden werden, um die Verletzung angrenzender Gewebestrukturen zu minimieren. Die Implantate dürfen niemals mit Geräten mit starker Wärmeentwicklung geformt werden wie beispielsweise elektrochirurgische Geräte, beheizte Messer oder Laser. 4. Poröse Materialien sind besonders gefährdet für eine Kontamination mit Fremdmaterial und Partikeln einschließlich Staub aus Handschuhen, Flusen von Abdeckmaterial und Reinigungsmitteln. Die Handhabung der Implantate sollte möglichst auf ein Minimum beschränkt werden. Schneiden Sie das Implantat nicht auf einer Oberfläche, die den Eintrag von Verunreinigungen in das Implantat begünstigen könnte. 5. Der Chirurg muss mit den Implantaten, der Anwendungsmethode, den Instrumente und dem chirurgischen Verfahren vertraut sein. Diese Implantate sind nicht für die Anwendung unter Gewicht oder Belastung ausgelegt. Der Patient muss über die Risiken der Implantate einschließlich möglicher Nebenwirkungen aufgeklärt werden. 6. GEBRAUCHSANWEISUNG Die folgende Anleitung dient lediglich Informationszwecken. Da eine Vielzahl chirurgischer Techniken eingesetzt werden kann, kann der Chirurg Details je nach seiner oder ihrer klinischen Erfahrung und ärztlichem Urteil abändern. Der Chirurg trifft die Auswahl des Implantats, seiner Zusammensetzung, Größe und Form nach Auswertung der anatomischen und funktionellen Bedürfnisse des Patienten. FORMUNG DES IMPLANTATS OmniPore Chirurgische Implantate werden in grundlegenden anatomischen Formen geliefert, die vom Chirurgen an den Ort der Implantation angepasst werden können. Das Material kann mit Schere oder Skalpell geschnitten werden, ohne dass ein Zusammenfallen der Poren befürchtet werden muss; hierbei ist darauf zu achten, dass die Kanten an den Stellen abgeschrägt werden, wo sie am Knochen anliegen. Die Implantate können durch Eintauchen in ein Becken mit kochend heißer steriler Kochsalzlösung in eine neue Form gebracht werden. Bei dieser Temperatur entspannt das Implantat und lässt sich wie gewünscht formen. Sollte sich das Implantat schwer biegen lassen, legen Sie es erneut in die heiße Kochsalzlösung. Sobald die gewünschte Form vorliegt, sollte das Implantat in dieser Form gehalten werden bis es abgekühlt ist. PLATZIERUNG DES IMPLANTATS Eine großzügige Gewebetasche sollte präpariert werden um sicherzustellen, dass das Gewebe über dem Implantat nicht unter Spannung gerät. Inzisionen direkt über dem Implantationsgebiet sollten vermieden werden. Besteht die Gefahr, dass das Implantat vor dem Einwachsen von Gewebe wandert, kann es mittels Nähten, K-Draht oder chirurgischen Fixierungsschrauben gehalten werden. Die Fixierungsschrauben sollten angezogen werden, bis sie eben mit der Oberfläche des Implantats sind. Mehrere Lagen von Implantatmaterial können zusammen gestapelt oder genäht werden. Eine Schneidenadel kann die Implantatoberfläche leicht durchdringen, so dass der Chirurg Gewebe oder Muskel an das Implantat heften und wieder somit wieder aufhängen kann. Bei der Platzierung der Implantate über den intraoralen Weg sind Vorsichtsmaßnahmen zu treffen, um das Risiko einer Kontamination des Implantats zu vermindern. Gli studi di biocompatibilità In vitro e In vivo hanno dimostrato che gli impianti chirurgici OmniPore sono esenti da effetti sistemici o citotossici osservabili. INDICAZIONI Gli impianti chirurgici OmniPore in blocchetti e sagome anatomiche sono indicati per applicazioni non sostenenti carico di chirurgia ricostruttiva e cosmetica del distretto cranio-facciale nonché per la riparazione dei traumi del cranio e del volto. CONTROINDICAZIONI Le condizioni di seguito elencate sono controindicazioni specifiche, effetti avversi e avvertenze che devono essere comprese dal chirurgo e da questi spiegate al paziente. Gli impianti chirurgici Omnipore non devono mai essere riutilizzati né risterilizzati. 2. Un'adeguata informazione del paziente è un fattore cruciale per il successo dell'intervento chirurgico. Il follow-up e le cure post-operatorie sono della massima importanza. Gli impianti non devono mai essere modellati utilizzando dispositivi che producano un calore intenso, quali i dispositivi elettrochirurgici, e i bisturi "caldi" o i laser. 4. MODELLARE L'IMPIANTO Gli impianti chirurgici OmniPore sono forniti pre-sagomati nella forme anatomiche più comuni, personalizzabili dal chirurgo per adattarsi al sito di impianto. Una volta ottenuta la forma desiderata, l'impianto va fissato in quella forma e lasciato raffreddare POSIZIONAMENTO DELL'IMPIANTO Preparare un'ampia tasca di tessuto perché il tessuto non debba essere "stirato" sull'impianto. Le incisioni praticate subito sopra il sito di impianto devono essere evitate. Se si prevede che l'impianto possa spostarsi prima che il tessuto abbia il tempo d'infiltrarvisi, è possibile ottenere la fissazione mediante suture, filo di Kirschner, o viti chirurgiche di fissaggio. Le viti di fissaggio devono essere serrate fino a che siano a livello della superficie dell'impianto. È possibile impilare e suturare assieme più pezzi di materiale per impianto. Os Implantes Cirúrgicos OmniPore nunca devem ser reutilizados ou re-esterilizados. 2. Instruções adequadas para o paciente são o principal fator determinante do sucesso do procedimento cirúrgico. O acompanhamento e a assistência pós-cirúrgica são muito importantes. CÓMO DAR FORMA AL IMPLANTE Los implantes quirúrgicos OmniPore se suministran en distintas formas anatómicas básicas que puede conformar el cirujano para adaptarlas al lugar de implantación. El material se puede cortar con tijeras o un bisturí sin miedo a aplastar los poros, teniendo en cuenta que se deben biselar los bordes del implante donde entre en contacto con el hueso. Los implantes se pueden conformar sumergiéndolos en un recipiente con solución salina estéril calentada hasta el punto de ebullición. A esta temperatura, el implante pierde su rigidez y se le puede dar con cuidado la forma pertinente. Si resulta difícil doblar el implante, se debe volver a colocar en la solución salina caliente. Una vez se obtenga la forma adecuada, el implante debe mantenerse con esa forma y dejarse enfriar. COLOCACIÓN DEL IMPLANTE Debe crearse una bolsa de tejido amplia para garantizar que el tejido no quede estirado sobre el implante. No deben realizarse incisiones directamente sobre el sitio del implante. PRODUCTINFORMATIE - Nederlands BESCHRIJVING OMNIPORE® chirurgische implantaten worden vervaardigd van poreus polyethyleen met een hoge dichtheid, een biologisch materiaal dat gemakkelijk kan worden gevormd of gesneden om te voldoen aan de anatomische en functionele eisen van de patiënt. De onderling verbonden poriën van het poreuze HDPE-materiaal maken fibrovasculaire ingroei in het implantaat mogelijk. Vanwege de witte kleur is het materiaal niet zichtbaar onder het bedekkende zachte weefsel. In vitro en in vivo onderzoeken naar de biologische compatibiliteit hebben aangetoond dat OmniPore chirurgische implantaten geen waarneembare systemische of cytotoxische effecten hebben. Poreuze HDPE chirurgische implantaten worden STERIEL en NIET-PYROGEEN geleverd. INDICATIES OmniPore chirurgische implantaten in blok-, plaatje- en anatomische vormen zijn bedoeld voor geen-gewichtdragende toepassingen bij craniofaciale reconstructie/cosmetische chirurgie en reparatie van craniofaciaal trauma. OmniPore chirurgische implantaten zijn tevens bedoeld voor de augmentatie of reconstructie van contouren in het craniomaxillofaciale skelet. CONTRA-INDICATIES Onderstaande specifieke contra-indicaties, bijwerkingen en waarschuwingen moeten worden begrepen door de chirurg en uitgelegd aan de patiënt. De algemene chirurgische risico's die hier niet worden genoemd, moeten ook aan de patiënt worden uitgelegd voorafgaand aan de operatie. 1. Actieve infecties. 2. Patiënten die de postoperatieve instructies niet willen of kunnen opvolgen als gevolg van begeleidende aandoeningen (mentaal of fysiek). 3. Gevoeligheid voor lichaamsvreemde materialen. Wanneer gevoeligheid voor het materiaal wordt vermoed, dienen aangewezen tests te worden uitgevoerd om deze mogelijkheid voorafgaand aan de implantatie uit te sluiten. 4. Beperkingen in de bloedtoevoer en/of systemische aandoeningen die het helingsproces kunnen vertragen en het risico van infectie en/of afstoting van het implantaat kunnen vergroten. Vasculaire veranderingen. 2. Allergische reactie op het implantaat. 3. Zenuwbeschadiging als gevolg van chirurgisch trauma. 4. Deformatie of fractuur van het implantaat. 5. Migratie of losraken van het implantaat. 6. Pijn, ongemak en/of abnormaal gevoel als gevolg van de aanwezigheid van het implantaat. 7. Oppervlakkige en/of diepe infectie. WAARSCHUWINGEN 1. OmniPore chirurgische implantaten mogen nooit opnieuw worden gebruikt of opnieuw gesteriliseerd. 2. Adequate instructies aan de patiënt zijn van essentieel belang voor een geslaagde chirurgische procedure. Postoperatieve follow-up en zorg zijn zeer belangrijk. De patiënt moet ervan bewust worden gemaakt dat implantaten niet zo sterk zijn als lichaamseigen bot en dat overmatige lichamelijke activiteit of belasting het losraken, migratie, deformatie of fractuur van het implantaat kan veroorzaken. 3. Het succes van een implantaat is afhankelijk van de zorgvuldige hantering en een goede chirurgische techniek. Bij het vormen en aangeven van de contouren van het implantaat dienen scherpe randen te worden vermeden om het risico van trauma van omliggend weefsel te beperken. Implantaten mogen nooit worden gevormd met behulp van instrumenten die sterke hitte produceren, zoals elektrochirurgische instrumenten, verwarmde messen of lasers. 4. Poreuze materialen lopen een verhoogd risico op besmetting door vreemd materiaal en deeltjes, zoals handschoenpoeder, vezels van afdekmateriaal en reinigingsmiddelen. Er moet alles aan worden gedaan om hantering van het implantaat tot een minimum te beperken. Plaats of vorm het implantaat niet op een oppervlak dat verontreinigende stoffen kan overdragen op het implantaat. 5. De chirurg moet grondige kennis hebben van de implantaten, de toepassingsmethode, de instrumenten en de chirurgische procedure. Deze implantaten zijn niet ontworpen voor gebruik in gewichtdragende of lastdragende toepassingen. De patiënt moet bewust worden gemaakt van de risico's van het gebruik van de implantaten, met inbegrip van de mogelijk bijwerkingen. 6. De arts is verantwoordelijk voor het volgen van een adequate training, de correcte patiëntselectie en de keuze en plaatsing van de implantaten. GEBRUIKSAANWIJZING Het volgende is een leidraad en dient uitsluitend ter informatie. Aangezien er een aantal verschillende chirurgische technieken kunnen worden gebruikt, is het mogelijk dat de chirurg details wil aanpassen aan zijn of haar klinische ervaring en medisch oordeel. De selectie van het implantaat en de samenstelling, grootte en vorm ervan worden vastgesteld door de chirurg na beoordeling van de anatomische en functionele wensen van de patiënt. VORMING VAN HET IMPLANTAAT OmniPore chirurgische implantaten worden geleverd in anatomische basisvormen die door de chirurg kunnen worden aangepast aan de implantatielocatie. Het materiaal kan zonder gevaar voor samendrukken van de poriën worden gesneden met een schaar of scalpel, waarbij de randen moeten uitwaaieren waar ze aansluiten op het bot. De implantaten kunnen opnieuw worden gevormd door ze onder te dompelen in bak met een tot het kookpunt verhitte steriele zoutoplossing. Bij deze temperatuur ontspant het implantaat, zodat het voorzichtig in de gewenste vorm kan worden gebracht. Zodra het implantaat moeilijk te buigen is, moet het worden teruggelegd in de hete zoutoplossing. Na het verkrijgen van de gewenste vorm, moet het implantaat in die vorm worden gehouden en moet men het laten afkoelen. PLAATSING VAN HET IMPLANTAAT Bereid een ruime weefselpocket om ervoor te zorgen dat het weefsel niet wordt uitgerekt over het implantaat. Vermijd incisies direct over de implantaatlocatie. Indien het risico bestaat dat het implantaat zich verplaatst voordat weefselingroei heeft plaatsgevonden, kan het implantaat worden vastgezet met hechtingen, K-draad of chirurgische fixatieschroeven. Fixatieschroeven moeten worden vastgedraaid totdat ze vlak op het oppervlak van het implantaat liggen. Meerdere stukken implantaatmateriaal kunnen worden gestapeld en aan elkaar worden gehecht. Een hechtnaald kan gemakkelijk het oppervlak van het implantaat penetreren, zodat de chirurg het weefsel of de spier aan het implantaat kan bevestigen.
