Según el Dr. Sattler, formaron a la Academia para tratar la necesidad
de conseguir resultados superiores en la remodelación corporal
mediante más y mejor formación e información a los médicos. La
“formación es, ante todo, el primer y más importante papel de
La Academia. Fomentaremos una sólida base de conocimiento
básico sin el cual, estos procedimientos no se pueden aplicar con
éxito. Sobre éste fundamente presentamos nuevos métodos y
procedimientos.” El intercambio de ideas y de experiencia llevará
a más innovación y propulsará el estado de la industria hacia
adelante. “La finalidad es establecer condiciones indispensables
de modo que podamos compartir conocimiento, aumentar

Currently, Cynosure is the sole contributor
to The Academy, which was founded by
internationally- renowned physician Gerhard
Sattler, M.D., cosmetic surgeon and director
of the Rosenpark Klinik in Darmstadt,
Germany. “The body of empirical research
in liposuction and body contouring
technology has advanced considerably
since the early days and has fostered key
developments in technology and refinements in
technique,” he explained. “While the growing variety of available
technical options has led to a new understanding of the subject,
subsequently there is a greater need for proper training and
education, which patients increasingly demand from their
physicians. What’s more, the standards of safety and efficacy
we achieved even ten years ago with liposuction are no longer
acceptable to today’s patient.”
According to Dr. Sattler, The Academy was formed to address
the need for superior body contouring results through
better training and education of practitioners. “Education is

cannot be successfully applied. From that foundation we
present new methods and procedures as well.” The exchange
of ideas and experience will further drive innovation and

In addition to providing a forum for the exchange of ideas
from world leaders in body contouring science, The Academy’s

The original six watt Smartlipo 1064 nm Nd:YAG

inserta en el área que se va a tratar para licuar la grasa,
la cual, después se deja drenar o se succiona. La energía
láser describe un camino para la cánula, lo cual reduce
el trauma del tejido. El tamaño de la cánula y su facilidad
de manejo al moverse a través del tejido fibroso permite
realizar el procedimiento en consulta, con mucho menos
tiempo de convalecencia, comparado con la liposucción
tradicional. La cicatrización no es un problema porque
la incisión es minúscula y no requiere sutura. La técnica
es similar a la de la liposucción asistida por ultrasonido,
pero con algunas diferencias: menos dolor, mejoría de la
hemostasis y la posibilidad de dirigir la energía durante del
tratamiento. “Con la cánula de 1 mm, la más pequeña del

Smartlipo MPX utilizes Cynosure’s patented MultiPlex
technology to combine the benefits of the 1064 nm and
1320 nm wavelengths in one laser output, enabling them to be
blended, or fired sequentially, for optimal fat liquefaction and
skin tightening results through tissue coagulation. “The use
of the 30 watt, 1064 nm laser and the 16 watt, 1320 nm laser
together translates into higher peak power for better outcomes
and skin tightening benefits,” said Bruce Katz, M.D., medical
director of the Juva Skin and Laser Center in New York City, New
York, U.S.
According to Dr. Katz, treatment is performed under tumescent
anesthesia. The laser energy is delivered through a fiber inside
a small cannula. It is inserted into the treatment area to
liquefy the fat, which is then expressed or suctioned out. The
laser energy carves a path for the cannula which reduces
tissue trauma. The cannula size and ease with which it moves
through even heavily fibrotic tissue allows
the procedure to be performed in-office,
with much less downtime compared to
traditional liposuction. Scarring is not a
problem because the tiny incisions do not
require sutures. The technique is similar
to ultrasound-assisted lipolysis, but with
a few key differences: less pain, improved
hemostasis and the ability to direct

Smartlipo MPX ofrece tres ratios de combinaciones.

size and shape), allowing users to be as aggressive as needed.
Furthermore, you get the non-specific thermal component that
you lack with erbium fractional systems.”
Nine scan patterns (three different shapes and three different
sizes) are offered by the Performa CO2 and these are adjustable
on the fly with the push of a button on the handpiece. “This
helps to save time, so I can treat a complete face in about
30 minutes,” said Dr. Saluja. Users can also choose between
microwound densities (pitch) of 100, 200, or 500 spots per
centimeter in a square, rectangular or trapezoidal pattern.
“While the square is probably the most useful, the other
patterns are good for some facial zones such as the perioral
area, and it is very helpful to be able to adjust with the fingertip
controls.” Power and pulse duration are also adjustable, allowing
aggressiveness up to surgical ablation. “As we learn more and
more about this technology I think you’ll see experienced
physicians obtaining great results using very aggressive
settings,” Dr. Saluja continued.

How long have you been using the laser?
Comencé a utilizar el láser durante mi residencia, dónde hice
una rotación en Estados Unidos por diferentes hospitales
asociados a la Universidad de Harvard y a la Boston University.
Entonces a la vuelta se puso en marcha la Unidad de Láser
del Hospital Ramón y Cajal, que ha sido centro de referencia
nacional desde inicio de los años 90.
I started using lasers during my residency, when I was in
different hospitals associated with Harvard and Boston
Universities in United States.
When I came back, the laser unit from Ramón y Cajal Hospital
began to work. It has been a national reference centre since the
90’s

Tell us something about how you got started. From your point
of view, how has laser technology progressed since then? What
was the first laser you used? What kind of treatments did you
perform with it?
Tuve la suerte de formarme con

en ese campo en Estados Unidos
(Dr Rox Anderson, Dr Ken Arndt,
Dr Dover, etc). Era un campo
totalmente nuevo y apasionante.

se utilizaban para lesiones
vasculares y pigmentarias (láseres

Which ones do you currently use? Wich treatments for?
En la actualidad se tiende a combinar tratamientos láser
con otro tipo de tratamientos (botox, rellenos, peelings,
radiofrecuencia, terapia fotodinámica, etc.). Los láseres que más
utilizamos son los de depilación, vasculares, láser Q- Switched
para lesiones pigmentadas y tatuajes, láseres fraccionales para
rejuvenecimiento y láseres ablativos tipo CO2.
Nowadays is common to combine laser treatments with
other treatments (botox, refilling, peelings, radiofrequency,
photodynamic therapy, etc).
The lasers we use more are for hair removal, vascular,
Q-Switched laser for pigmented and tattoos lesions, fractionated
lasers for rejuvenation and ablative lasers such as CO2.
¿Qué es lo que más le gusta de los láser?
What do you like best about lasers?
Lo que más me gusta es tratar de aportar cosas nuevas, buscar
nuevas apliacaciones, encontrar la combinación adecuada de
parámetros para lesiones resistentes, entender histológicamente
que es lo que está pasando y como influye cada parámetro tanto
del láser como del tipo de lesión y sobretodo diseñar nuevas
tecnologías. He tenido la suerte de colaborar con compañias
americanas, trabajando conjuntamnte con bioingenieros, en el
diseño de nuevos láser y tecnologías aplicadas a dermatología.
Me gusta asimismo haber trabajado con numerosos residentes
que han rotado conmigo en todos estos años. Transmitirnos
mutuamente esta motivación por aprender y avanzar un paso
más allá.
What I like more is to try to contribute new things, to look for
new applications, to find appropriate parameters combination
to resistant lesions, to understand histologically what is
happening and how each parameter influence, both from laser
and lesion type, and especially, to design new technologies.
I am lucky I have advised many American companies working
to design new laser applications and technologies, working
together with bioengineers.
I also like to work with a lot of residents who have been with me
during all these years and to reciprocally pass on the motivation
to learn and go forwards.
¿Qué mejoras aporta para el paciente?
Se ha avanzado mucho aunque todavía queda muchísimo
por hacer. El único problema es que lo que limita el avance es
el mercado, que es el que marca las tendencias. Si un tipo de
tratamiento no es rentable, las compañias no invertiran allí.
Comente, por favor, qué le sugeriría a los médicos que se inician
en esta tecnología.
Como cualquier campo de la vida lo importante es estar
motivado para aprender y sobre todo para poder aportar algo
nuevo a través de nuestro conocimiento e imaginación. Que se
lo piensen dos veces antes de invertir en una tecnología y que
eviten convertirse en especuladores financieros.
What has the laser contributed to your patients?
There is a huge advance but there is too much to do. The only
problem is that what limits the advance is the market, which
marks tendencies. If a treatment is not profitable, companies will
not invest on it
Please tell us what you would say to practitioners who are just
beginning to use laser technology.
As in every life field, the most important thing is to be motivated
to learn and, especially, to contribute something new through
our knowledge and imagination.
They have to think twice before investing in a technology and
they have to prevent to become financial speculators.

vapourise the hair. This is down to
the practitioner and if they are not

rather than the melanin within the hair follicle.

We evaluate peolpe´s skin on the Fitzpatrick Scale.
Skin types are classified from one to six based on
your skin, eye and hair colour. One to four is classed
as white skin and four to six dark / ethnic skins.
Evaluamos la piel de los pacientes según
la escala Fitzpatrick. Los tipos de piel se
clasifican de uno a seis basándose en
la piel y el color de ojos y pelo. Del tipo
uno al cuatro se considera piel clara y del
cuatro al seis, oscura/pieles étnicas. Hay

Problems occur when practitioners use
the wrong laser or IPL. I have a lot of Assian
patients who have been throught six different
clinics with really poor results then come to us
because we specialise in ethnic skins.
If you use the wrong laser then it basically
heats the skin as well as the hair , sometimes
causing damage to the surrounding tissue,
and on -assian skin you can cause terrible dark
pigmentation that can last eighteen months
to two years if not longer.

hair but you can only treat grey hair to a
certain degree. We use a product containing
squid ink that dyes the hair so it can be treated

Trus: if it is done properly the there should be no side effects. The only
thing that can happen, althoug there is no major evidence of this, is
that if hair is destroyed in one place it can sometimes grow in another.
We have a lot of patients come to us with polycystic ovaries and we can
get rid of it on their faces but they might find that a smallpatch could
come up somewhear
else. However, most
find it is much more
beneficial to have it
removed from their

There is about an 80 per cent reduction with the Cynosure
lasers we use but we do tend to get a better reduction
here because we are small clinic and we pride ourselves on
getting the best results possible.
I have to be very careful to state to patients that we are doing
a hair reduction because a lot of people who come tu us don´t want to
see any hairs and we just can´t do that.
You are always going to have a very fine layer of downy hair that is

Body Contouring and Laser Lipolysis) en
septiembre de 2009 para dar respuesta a la
demanda de formación contínua por parte
de los médicos y a la seguridad en el caso
de los pacientes.
Durante los últimos años, el ámbito de la
liposucción se ha diversificado, por ello,
ha nacido la necesidad de contar con
una asociación que agrupe los nuevos
avances que dan respuesta a la demanda
de los pacientes. De ahí, que La Academia
se centre en la técnica de la láser-lipólisis
aplicada a la eliminación de la grasa
subcutánea y el tratamiento del tejido
conectivo que provocan la celulitis.

Formación: La formacion para el departamento de
dermatología del Hospital de la Universidad de Tromso. Todos
los dermatólogos del departamento atendieron la formación.
Tenían experiencia anterior con un láser antiguo de PDL
(Photogenica). El láser se rompió un año antes y ellos tienen
muchos pacientes en espera para el nuevo láser. Comenzamos
la formación con la teoría de Cynergy, explicando las diferencias
con solo el PDL, explicando cómo el Nd-YAG funciona (no
tienen experiencia anterior con el Nd-YAG), y cómo el múltiplex
funciona. Ventajas del sistema de Múltiplex contra solo el PDL
solamente o solo el Nd-YAG. La mayoría de los doctores van a
atender al EADV en Berlín. Ellos visitarán nuestra stand en el
Congreso
Training: A Training Session was held for the dermatology
department at the University Hospital of Troms. All the
dermatologists in the department attended the training session.
They had had previous experience with an older version of the
PDL laser (Photogenica). However, the laser broke down last
year. They have a lot of patients waiting for the new laser’s
arrival. We started the training session with the theory about
Cynergy, explaining the differences in utilizing solely the PDL,
explaining how the Nd-YAG operates (they don’t have any
previous experience with the Nd-YAG), and how Multiplex
operates. Advantages of the Multiplex System vs. solely the PDL
or solely the Nd-YAG. Most of the doctors are going to attend the
EADV in Berlin. They will visit our stand at the Congress.
Tratamientos: Tratamos a 15 pacientes con diversas
patologías: hemangiomas planos, malformaciones vasculares,
telangiectasias faciales, venas de la pierna, cicatrices y verrugas.
No podríamos realizar la eliminación del pelo porque la opción
de depilación no estaba disponible aunque la hayan comprado.
Una paciente no pudo ser tratada porque tenía una cicatriz
vieja con hiperpigmentación leve que no iba a responder al
tratamiento. Tres pacientes anularon la cita.
Treatments: We treated 15 patients with different pathologies:
PWS, vascular malformations, facial telangiectasia, leg veins,
scars and warts. We couldn’t perform hair removal because the
depilation option wasn’t available although they have bought it.
One patient couldn’t be treated as she had an old scar with mild
hyperpigmentation that would not respond to treatment. Three
patients cancelled their appointment.

mediante la simulación tridimensional Montecarlo. Este
software nos permite prever el comportamiento de un pulso
láser a partir de la simulación del movimiento de miles de
millones de fotones en el interior de un tejido, caracterizado por
parámetros matemáticos obtenidos empíricamente. Dicho más
simplemente, nos permite conocer como se va a distribuir la
energía de un disparo láser en el interior del tejido.
Para ello, es necesario averiguar determinados parámetros que
influyen en la difusión de la radiación en el interior de la piel.
Dichos parámetros son el índice de refracción, la absorción,
dispersión, anisotropía, así como el grosor, la posición y la
forma de la zona que se va a tratar. Sería posible introducir en
el modelo un mayor número de parámetros para conseguir
un modelo más exacto del tejido, pero a la vez, aumentamos
la complejidad de la simulación y, en consecuencia, el tiempo
de análisis. Como quiera que algunos de los parámetros
mencionados varían con la longitud de onda aplicada,
centraremos el estudio en una única longitud de onda, 755 nm,
que es la más utilizada en eliminación del pelo.
In this article, we refer to the results obtained through
Montecalro tridimensional simulation. This software enables
us to anticipate the behaviour of a laser pulse starting from
the simulation of the movement of thousands of millions
of photons inside a tissue characterised by mathematical
parameters which were empirically obtained. In other words, it
enables us to discover how the energy from a laser fire will be
distributed within the tissue.
Hence, specific parameters must be ascertained that influence
radiation diffusion within the skin. These parameters are the
index of refraction, absorption, dispersion, anisotropy as well
as the thickness, the position and the shape of the area that
will be treated. It would be possible to introduce a greater
number of parameters into the model in order to achieve a
more precise tissue model, but at the same time, we increase
the complexity of the simulation, and in consequence, the
analysis time. As we would like some of the parameters
mentioned to vary with the wavelength applied, we have
based the study on one single wavelength, 755 nm which is
the most commonly utilised in hair removal.

a uno de diámetro diferente? A partir de los datos numéricos
que ofrece la simulación, podemos obtener la siguiente tabla,
que relaciona la energía que entrega el spot indicado con
respecto al spot de 15mm a diferentes profundidades:

to a different diameter. Starting with the numerical data
provided by the simulation, we have obtained the following
table which shows the relationship between the energy that
the indicated spot delivers compared to the 15 mm spot at
different depths:

diferencias se observan a mayores profundidades, de forma
que debemos valorar a la hora de hacer nuestro tratamiento la
energía que vamos a utilizar, ya que si nuestro tejido diana se
encuentra a mucha profundidad, debemos tener en cuenta el
posible daño que podemos hacer en capas superficiales, que es
donde más energía se disipa, según el gráfica anterior.
Es posible que el lector piense que este estudio no aporta
ninguna conclusión desconocida hasta ahora, pero la simulación

clinics nationwide has confirmed
that the recession has had little
or no impact on their respective
businesses. This demonstrates
the buoyancy of the laser therapy
market and the potency and
efficacy of the Cynosure systems in
supporting laser clinics.

with the following statements about the future of the laser

Over 90% agreed that “The future looks bright for the laser

This positivity is replicated by the proactive steps clinics are
taking to ensure business continuity.75% have conducted an
advertising campaign during the recession and 50% of those
who participated in the survey said they had offered both
discounts on courses and introductory offers to entice new
clients, all of which had been a success.
Laser hair removal has consistently been regarded as the most
popular treatment in the past 12 months and is often seen as
the “bread and butter” of a clinic’s operation.In the same period
the clinics surveyed have also seen an increase in bookings
for laser hair removal, demonstrating that treatments that
offer proven results have not suffered during the economic
downturn.
When comparing the past 12 months to the preceding
12 month period, thread vein removal and pigmentation
enquiries have increased by around 41%, but again during that
period there was a 71% increase in enquiries about laser hair
removal.

A recent survey of Cynosure clinics nationwide has confirmed
that there has been an increase in men seeking laser therapy
for their health and grooming needs. This crosses a number of
procedures and is demonstrated right across the UK.
Men account for 25% of the business in Cynosure clinics
nationwide and this has been steadily increasing over the past
5 years.
In the past 12 months, laser hair removal has proved to be the
most popular treatment for men across all of the Cynosure
clinics with 100% of the clinics surveyed reporting this as the
most popular treatment for their male clients.
Additionally, men are also seeking laser therapy for antiageing, tattoo removal and thread vein removal. This typically
occurs when they have successfully treated excessive hair
growth using lasers.
Cynosure clinics are reporting that over the past 12 months
they have seen an increase in men enquiring about the
following treatments.

In the survey, clinics attribute

“Los no ablativos
están indicados para
mejorar la textura,

los semiablativos,
tratan la dermis

mi experiencia, lo
que mejor hacen los
láser semiablativos
es tratar las cicatrices,
sobre todo las
recientes”.
La Dra. Ribé además,
argumenta estos
resultados con
tinciones de un
estudio realizado por
ella misma mediante
biopsias en las que
muestra la dermis
antes y después del tratamiento
láser en las que se puede apreciar
una reorganización del colágeno, la
activación de éste y sus fibras.
En cuanto a la combinación de
láser, Ribé afirma que, “cuando
ya tienes experiencia con láser
siempre quieres hacer más y
crees que combinándolos puedes
mejorar”; por ello, presenta
durante su charla que con el
láser semiablativo Performa CO2,
debido a su afinidad por el agua, su
energía es absorbida por la dermis
y esto hace que sea muy efectivo
aunque, si las fluencias son muy
altas, el “downtime” también será
más duradero. Por su parte, con el
láser no ablativo fraccionado Affirm
–también de Cynosure- con sus dos
longitudes de onda, una profunda
que produce el estiramiento y
otra más superficial que estimula
el colágeno, se consiguen muy
buenos resultados. Por tanto,
la Dra. Ribé concluye en que al
combinarlos, “el Performa vaporiza
la epidermis con menos impacto
en la dermis y provoca menos daño
y mejor resultado; ya que, con el
Affirm- a altas fluencias- daña la
dermis y remodela el colágeno”.

therapy, Ribé

to do more and you believe that
combining them could yield
optimal results”; hence, she
explained during her talk that
the fractionated Performa CO2
laser, due to its affinity for water,
its energy is absorbed by the
dermis and this should be very
effective although if the fluences
are very high, the “downtime” will
also be longer. In turn, with the
fractioned non ablative Affirm laser
–also from Cynosure- with its two
wavelengths, reaches a depth which
provokes a tightening effect and
another more superficial depth that
stimulates collagen that achieves
very good results.
Thus, Dr. Ribé concluded that in
combination laser therapy, “the
Performa vaporizes the epidermis in
a fractionated way with less impact
on the dermis and provokes less
injury for better results; whereas
with the Affirm- at high fluences- it
injures the dermis and remodels the
collagen”.

efectivo eliminando arrugas, cerrando los poros, eliminando los
léntigos, mejorando la textura, unificando el color, estirando la
piel y con menor “downtime” que con el CO2 en solitario y con
una mayor cantidad de piel tratada, ya que juntos, abordan toda
la superficie”.
La Dra. Ribé presentó imágenes de pacientes y explicó casos
desde su práctica diaria, ya que “cuando una persona llega a mi
consulta y quiere parecer más joven, yo le comento todas las
posibilidades que existen. Con el láser, además de un aspecto
rejuvenecido y de disminuir las arrugas, también consigues una
uniformidad en la piel y un color y textura mejores”.
Cynosure continuará con su labor docente mostrando los
trabajos de los médicos que trabajan con láser tanto en España
como en Portugal.

Cynosure will continue with its knowledge job showing
clinical trials from doctors who works with laser in Spain and in
Portugal.
