Wilhelm Julius Teufel GmbH Orthopädietechnische Medizinprodukte GEBRAUCHSANLEITUNG Hüftorthese DynaCox® REF 22 300 ... 1 USER MANUAL MODO DE EMPLEO MANUALE D‘INSTRUZIONI PRODUKTBESCHREIBUNG Die Hüftorthese DynaCox vereint die körpernahe Wirkung einer Bandage (Propriozeption) mit den integrierten Stabilisationselementen einer Orthese im 3-Punkt-Prinzip. Das elastische Form-Gestrick bewirkt eine gleichmäßige Kompression bei optimalem Tragekomfort. Das ROM-Flex-Gelenk ermöglicht das Einstellen der Flexion/ Extension in Schritten von 15°. Die modulare Bauweise lässt eine schnelle Versorgung mit voller Anpassbarkeit zu. Die Hüftorthese DynaCox ist zugelassen zur Anwendung in der Klinik und im häuslichen Bereich. HMV-Nr. DynaCox mit ROM-Flex-Gelenk: 23.05.01.0002 INDIKATION • Nach Reposition luxierter Hüftgelenke • Instabilität nach T.E.P. • Muskuläre Dysbalance KONTRAINDIKATION • Fehlende Patienten-Compliance (z.B. verursacht durch fortgeschrittene Demenz) • Schwere Hautveränderungen (Wundheilungsstörung mit Fistelung) FUNKTION • Sicherung des OP-Ergebnisses •Luxationsprophylaxe • Frühfunktionelle Bewegungstherapie 2 SICHERHEIT Vor der Benutzung ist eine individuelle Anpassung der Orthese und eine Einweisung in den Gebrauch durch einen erfahrenen Orthopädietechniker entsprechend der ärztlichen Anweisung erforderlich. Falsche Auswahl, Anpassung, Anwendung und/oder mangelhafte Kontrolle der Passform des Produktes können zu gesundheitlichen Schäden führen. Die Orthese nie auf verletzter Haut tragen. Die Orthese vor jedem Anlegen auf Beschädigungen überprüfen (z.B. lockere Verbindungen, Verschlüsse, Gurte, etc.). Bei vermuteten Leistungsstörungen (z.B. dem Gefühl einer nachlassenden oder unzureichenden Wirkung) muss unverzüglich der Arzt oder Orthopädietechniker aufgesucht werden. Die Orthese muss regelmäßig von einem Arzt oder Orthopädietechniker auf ihre Funktion, korrekten Sitz und eventuelle Beschädigungen überprüft werden. Die Abstände der Kontrollen sind für jeden Patienten individuell festzulegen. Sollte das Produkt einer unverhältnismäßig starken Belastung ausgesetzt gewesen sein, muss es vor der weiteren Verwendung von einem Orthopädietechniker auf mögliche Schäden überprüft werden. Scharfkantige Ränder u.ä., die durch spanabhebende Bearbeitung des Produktes auftreten können, müssen zur Vermeidung von Hautverletzungen entgratet werden. Unzuträglicher Druck durch die Orthese kann zu Hautschädigungen und Durchblutungsstörungen führen und muss daher vermieden werden. Die Orthese ist durch die notwendige individuelle Anpassung (produktabhängige Formung und/oder spanabhebende Bearbeitung) nicht zur Wiederverwendung bei weiteren Personen geeignet! Lagerung/Entsorgung: Das Produkt enthält keine umweltgefährdenden Stoffe. Es kann über den Hausmüll entsorgt werden. 3 Warnung Achtung Hinweis BESCHREIBUNG Die DynaCox ist modular aufgebaut. Die Orthesen- und Bandagen-Komponenten sind untereinander kombinierbar. Die Orthesenbauteile (nicht Bandage) sind rechts/links verwendbar. Die DynaCox wird teilkonfektioniert geliefert. (Femurlänge = Distanz Kniespalt –Trochanter Major) Die Schenkelgurte sind untereinander austauschbar und separat bestellbar. Die Schenkelgurte erweitern/reduzieren den U2 (Umfang 2) um jeweils 3 cm. Wir empfehlen bei längerer Tragezeit der DynaCox die Ausstattung mit einer Wechselbandage (REF 22 301 ...). 25 cm 6 cm Umfang 6 cm über U1 Trochanter major 4 cm Höhe Trochanter major (Ausgangspunkt des Maßnehmens) Umfang 25 cm unter U2 Trochanter major cm Umfang 5 cm über U3 Patellamittecm LEGENDE Anatomischer Punkt Technischer Ansatzpunkt Auszuführende Tätigkeit ANWENDUNG/ ANPASSUNG Maße nehmen: 1 Der Ausgangspunkt im Beckenbereich ist der Trochanter Major (siehe auch Maßtabelle S. 4). 2 Umfang I Messen Sie zirkulär 6 cm über dem Trochanter. 3 Umfang II Messen Sie zirkulär 25 cm unter dem Trochanter. 4 Umfang III Messen Sie zirkulär 5 cm über der Patellaoberkante. 5 ANWENDUNG/ ANPASSUNG Anpassungen I Der Patient sollte zur besseren Einstellung der DynaCox mit leicht abduzierten Beinen stehen. Alle Schrauben besitzen Schlüsselweite 3 mm Inbus. 1 Öffnen Sie den Reißverschluss der Femurtasche und beide darunter sichtbaren Schrauben. 2 Halten Sie die Orthese zur Orientierung neben das betroffene Patientenbein. Die obere Kante der Femurpelotte sollte direkt unter dem Trochanter platziert werden. 3 Ziehen bzw. schieben Sie die Kondylenspange auf die gewünschte Länge. Schließen Sie die Schrauben und den Reißverschluss wieder. 4 Öffnen Sie den Klettverschluss der Schraubenabdeckung und die beiden mittleren Schrauben. 6 ANWENDUNG/ ANPASSUNG Anpassungen II 5 Halten Sie die Orthese zur Orientierung neben das betroffene Patientenbein. Die obere Kante der Femurpelotte sollte direkt unter dem Trochanter platziert werden. Die Beckenspange sollte zwischen dem Beckenkamm und dem Trochanter platziert werden. Wichtig: Die Beckenspange muss thermoplastisch angepasst werden. (s. S. 12 Thermoplastische Anpassung) 6 Ziehen bzw. schieben Sie die Beckenspange auf die gewünschte Länge. 7 Schließen Sie die Schrauben und den Klett der Schraubenabdeckung wieder. 8 Öffnen Sie beide lateralen Schrauben der Kondylenspange leicht. 9 Die Orthese ist nun zur Anprobe vorbereitet. 7 Tipp: Zum leichteren Anziehen kletten Sie die innere Beckenlasche mit ihrem Hakenverschluss nach außen auf der Femurtasche und die Schenkellasche von hinten auf ihren Hakenverschluss nach außen fest. ANWENDUNG/ ANPASSUNG Anpassungen III 10 Legen Sie dem Patienten die Orthese locker um. 11 Schließen Sie als erstes den Kniekehlengurt der Kondylenspange. Achten Sie auf die richtige Höhe, die obere Patellakante sollte ca. zwei Finger breit Abstand zur Kondylenspange aufweisen. 12 Schließen Sie mit Hilfe der Greiflasche den Klettverschluss der Bandage am Leib. 13 Kontrollieren Sie die flächige Anlage der Kondylenspange und schließen Sie beide Schrauben. 8 ANWENDUNG/ ANPASSUNG Anpassungen IV 14 Schließen Sie den Klettverschluss der Schenkellasche. 15 Kürzen Sie die Schenkel- und Beckenlasche mit einer Schere auf das Patientenmaß. 16 Schließen Sie den Klickverschluss und ziehen Sie die Beckengurte der Orthese beidseitig fest an. 17 Kürzen Sie die Beckengurte mit einer Schere auf das Patientenmaß. 9 ANWENDUNG/ ANPASSUNG Anpassungen V 18 Einstellen des ROM-FlexGelenkes Ziehen Sie den NeoprenGelenkschutz nach oben weg, so dass Sie die beiden Einstellschrauben erreichen können. Öffnen Sie immer nur eine der beiden Anschlagschrauben! Drehen Sie die Anschlagschraube komplett heraus. Auf der Skalierung können Sie durch das Sichtfenster den momentanen Winkelwert ablesen. Platzieren Sie die Anschlagschraube so in eines der Gewinde, dass die gewünschte Beugung bzw. Streckung erreicht werden kann. (Beispiel: max. 30° Flexion) Schließen Sie die Schraube wieder. Sie können Flexion und Extension in Schritten von 15° einstellen. Öffnen Sie die 2. Anschlagschraube und verfahren Sie gleichermaßen. Schließen Sie auch diese Schraube wieder. (Beispiel: max. 15° Extension) 10 ANWENDUNG/ ANPASSUNG Anpassungen V Hinweis Falls das Gelenk ohne Bewe- gungsbegrenzung eingesetzt werden soll, entfernen Sie bitte beide Anschlagschrauben. Gleichzeitig wird damit der rückseitige Ring gelöst und kann abgenommen werden. Dieser Ring kann bei Bedarf jederzeit wieder aufgeschraubt werden. Ziehen Sie zum Schluss den beiliegenden Neopren- Gelenkschutz wieder über das ROM-Flex-Gelenk. Dadurch wird ein eventuelles Einklemmen von Kleidung verhindert. 19 Die fertig angepasste Orthese. Kein Loctite nötig durch selbstsichernde Tuflock®-Schrauben. 11 ANWENDUNG/ ANPASSUNG Tipp zur Anpassung: Bei besonders adipösen Patienten kann zur Stabilisierung der Weichteile eine Kompressions-Innenbinde unter der DynaCoxBandage getragen werden. Wir empfehlen den Einsatz unserer Silva® Universal-Rumpfbandage REF 40 368 … (siehe Katalog „Orthetik“). Thermoplastische Anpassung: Die Kunststoffkomponenten können thermoplastisch nachgeformt werden. • Entfernen Sie das eingeklettete Polster. • Die zu verändernde Stelle sollte durch leichte Kreisbewegungen des Heißluftgebläses erwärmt werden. • Verarbeitungstemperatur: Beckenspange 140 °C/ Kondylenspange 160 °C. • Nachdem sich das Material an der Oberfläche wachsartig, glänzend darstellt, kann mit dem Umformen per Hand begonnen werden (Achtung Verbrennungsgefahr – Thermohandschuhe tragen!). • Nach kompletter Abkühlung das Polster wieder passgenau einkletten. Nachpassung: Im Einzelfall kann es erforderlich sein, die laterale Schiene distal der Femurplatte nachzubiegen. • Verwenden Sie runde Schränkeisen (MW 4). • Achtung: Falls die Schränkung der Schiene direkt distal der Femurplatte stattgefunden hat, ist eine Verkürzung durch Einschieben nicht mehr möglich. 12 MATERIAL Bandagenteil: Orthesenteil: 48% Polyester Hart PVC 36% CoolMaxPETG 16% ElasthanAluminium Stahlschienen KOMFORT/ HYGIENE Unter der DynaCox sollte ein Unterhemd/T-Shirt getragen werden. Der Slip sollte über der DynaCox getragen werden. KOMPATIBILITÄT Die DynaCox ist nicht zur Kombination mit anderen als in dieser Gebrauchsanleitung genannten Produkten bestimmt. ORTHESE ABSCHULEN Dem Therapieverlauf folgend, können die festen (orthetischen) Komponenten der DynaCox von der Bandage getrennt werden. 80 C PRODUKTPFLEGE/ REINIGUNG/ WARTUNG A Der Schenkelgurt kann zum Waschen von der Bandage 40 C abgenommen werden. Um die Therapie nicht zu unterbrechen, kann ein zweiter Gurt (REF 22 303 …) bestellt werden. C Wir95 empfehlen CLbei längerer Tragezeit der DynaCox die Ausstattung mit einer Wechselbandage (REF 22 301 …). C Die40 Bandage sollte zum Waschen vom Fachbetrieb gewechselt werden. 30° •Handwäsche • Nicht bügeln • Nicht im Trockner trocknen • Bleichen nicht erlaubt • Keine chemische Reinigung 13 • Orthesenteil mit einem feuchten Tuch abwischen PRODUCT DESCRIPTION The DynaCox hip orthosis combines the proximal effect of a bandage (proprioception) and the integrated stabilising elements of an orthosis working according to the three-point principle. The elastic, fully-fashioned knitted fabric ensures an even compression and optimal wearing comfort as well as completing the force system required to limit flexion. The ROM-Flex joint allows the adjustment of flexion or extension movements in steps of 15°. The modular design allows for quick treatment and full adaptability. The DynaCox hip orthosis is approved for clinical and domestic use. INDICATION • After reduction of dislocated joint • Instability after Total Hip Replacement (THR) CONTRAINDICATION • Mechanically forced dislocation due to biomechanical misalignment of the joint FUNCTION • Safeguarding surgical results • Dislocation prophylaxis • Early functional motion therapy 14 SAFETY Before using this device an individual adaptation of the orthosis and a familiarisation in its use by an experienced Orthopaedic Technician according to the medical instructions is essential. Incorrect selection, adaptation, application and/or failure to monitor the correct fit of this device can cause health damage. Never wear the orthosis on bruised skin. Check orthosis for damage (such as loose connections, fasteners, belts, etc) each time it is put on. Should any performance malfunctions (like an apparent reduction in or lack of corrective action) be suspected, immediately call at an Orthotist or an Orthopaedic Technician. The orthosis must be checked for function, correct fit and possible damage by an Orthotist or Orthopaedic Technician at regular intervals. The intervals between checks are to be individually determined for each patient. Should the product have been subject to disproportionately strong forces, it must be checked for possible damages by an Orthopaedic Technician before further use. Sharp edges or the like, which may occur when machining the product, must be deburred in order to avoid skin injuries. Intolerable pressure caused by the orthosis may cause skin injuries or a disturbance in the blood supply and must be avoided. Due to the required individual adaptation (productdependent forming and/or machining) of the orthosis it is not suitable for reuse by other persons! Storage/Disposal: The product does not contain any environmentally hazardous substances. It can be disposed of with household refuse. 15 Warning Attention Note DESCRIPTION The DynaCox has a modular design. Orthosis and bandage components can be combined with one another. Orthosis components (not bandage) can be used on right/left. The DynaCox will be delivered partly preassembled. (Femur length = Distance between lower edge of knee cap and greater trochanter) The thigh belts are interchangeable and can be ordered separately. The thigh belts extend/reduce U2 (Circumference 2) by 3 cm each. We recommend the use of a spare bandage when the DynaCox needs to be worn for extended periods of time (REF 22 301 ...). 25 cm 6 cm Circumference 6 cm U1 above greater trochanter 16 cm Height of greater trochanter (starting point for measuring) Circumference 25 cm U2 below greater trochanter cm Circumference 5 cm cm U3 above patella KEY Anatomic location Technical starting point Required action APPLICATION/ ADJUSTMENT Taking measurements: 1 The starting point in the pelvis area is the greater trochanter (see also size chart on p. 16). 2 Circumference I: Take measure circumferentially 6 cm above the trochanter. 3 Circumference II: Take measure circumferentially 25 cm below the trochanter. 4 Circumference III: Take measure circumferentially 5 cm above the upper patella edge. 17 APPLICATION/ ADJUSTMENT Adjustments I: The patient should be standing with slightly abducted legs to allow for easier adjustment of the DynaCox. All screws are Allen key size 3 mm. 1 Open the zip fastener of the femur bag and unscrew both screws visible underneath. 2 Hold the orthosis alongside the leg of the patient for orientation. The upper edge of the femur pad should be placed directly below the trochanter. 3 Pull or push the condylar brace to the desired length. Tighten the screws and close the zip fastener 4 Open the Velcro of the screw cover and unscrew both the middle screws. 18 APPLICATION/ ADJUSTMENT Adjustments II: 5 Hold the orthosis alongside the respective leg of the patient for orientation. The upper edge of the femur pad should be placed directly below the trochanter. The pelvis brace should be placed between the iliac crest and the trochanter. Important: The pelvis brace may be thermoplastically adjusted. (see p. 24 Thermoplastic Adjustment) 6 Pull or push the pelvis brace to the desired length. 7 Tighten the screws and close the Velcro of the screw cover. 8 Slightly open both lateral screws of the condylar brace. 9 The orthosis is now ready for fitting. Tip: To make putting it on easier, fasten the inner pelvis flap with its hook fastener to the outside onto the femur bag, and take the thigh flap from behind to fasten onto its hook fastener to the outside. 19 APPLICATION/ ADJUSTMENT Adjustments III: 10 Put the orthosis around the patient loosely. 11 First, close the popliteal fossa strap of the condylar brace. Be aware of the correct height. The upper patella edge should have a distance of about the width of two fingers to the condylar brace. 12 Close the Velcro of the bandage at the body by means of the grip tab. 13 Check if condylar brace fits flatly and close both screws. 20 APPLICATION/ ADJUSTMENT Adjustments IV: 14 Close the Velcro on the thigh flap. 15 Shorten the thigh and pelvis flap to the required measure for the patient by means of a pair of scissors. 16 Close the snap lock and pull the thigh belts of the orthosis tight. 17 Shorten the thigh belts to the required measure for the patient by means of a pair of scissors. 21 APPLICATION/ ADJUSTMENT Adjustments V: 18 Adjusting the ROM-Flex Joint Pull the neoprene joint guard upward to remove so that the two adjusting screws are accessible. Always loosen only one of the two stop screws at any given time! Take the stop screw out completely. The current angle value can be read from the scale through the viewing window. Place the stop screw in one of the threads such that the desired flexion or extension can be achieved. (Example: 30° flexion max.) Tighten the screw again. Flexion and extension are adjustable in increments of 15°. Loosen the 2nd stop screw and proceed likewise. Tighten this screw as well. (Example: 15° extension max.) 22 APPLICATION/ ADJUSTMENT Adjustments V: Note If the joint is to be inserted without movement limitation, please remove both stop screws. This releases the ring at the reverse side at the same time and it can be removed. If required, this ring can be screwed back on at any time. Finally, pull the enclosed neoprene joint guard back over the ROM-Flex joint. This will prevent items of clothing from becoming jammed. 19 The ready-made orthosis. No Loctite required through selflocking Tuflock® screws. 23 APPLICATION/ ADJUSTMENT Tip for the adjustment: With particularly obese patients an inner compression bandage can be worn underneath the DynaCox bandage. For this purpose we recommend our Silva® Universal Rib Belt REF 40 368 … (see “Orthotics” catalogue). Thermoplastic adjustment: The components made of plastics can be thermoplastically reshaped. • Remove the Velcro-fastened padding. • The area to be processed should be heated up by light circular movements of the hot-air blower. • Processing temperature: Pelvis brace 140 °C/Condylar brace 160 °C. • As soon as the surface of the material begins to look wax-like and shiny the manual reshaping can start (Attention – Risk of Burning – Please wear thermal gloves!). • After complete cooling fasten padding precisely in its former location. Readjustment: In some cases distally bending the lateral splint to the femur belt may be required. • Use round saw sets (MW/WAF 4). • Attention: If the offset of the splint has been done directly distal to the femur plate, then shortening by sliding it back in is no longer possible. 24 MATERIAL Bandage part: Orthosis part: 48% Polyester Rigid PVC 36% CoolMaxPETG 16% SpandexAluminium steel splints COMFORT/ HYGIENE Underneath the DynaCox an Undershirt/T-Shirt should be worn. The underpants should be worn over the DynaCox. COMPATIBILITY The DynaCox may not be combined with products other than those mentioned in this instruction manual. ORTHOSIS WEANING In accordance with the course of the therapy, the fixed (orthotic) components of the DynaCox can be separated from the bandage. 80 C PRODUCT CARE/ CLEANING/ MAINTENANCE A To allow for washing the thigh belt should be removed from the40bandage. To avoid interrupting the therapy, a second C bandage (REF 22 303 …) can be ordered. We recommend the use of an exchangeable bandage when 95 C the DynaCox CL needs to be worn for extended periods of time (REF 22 301 ...). 40 C To allow for washing the bandage should be replaced by your specialist. 30° • Hand wash • Do not iron • Do not tumble dry • Do not bleach • Do not dry clean 25 • Wipe orthosis part clean with a damp cloth DESCRIPCIÓN DEL PRODUCTO La órtesis de cadera DynaCox une el efecto de una banda ceñida al cuerpo (propiocepción) con los elementos estabilizadores integrados de una órtesis en el principio de 3 puntos. La elasticidad del tejido de la banda ejerce una compresión uniforme, al tiempo que garantiza una comodidad óptima. La articulación ROM-Flex permite ajustar la flexión/extensión en pasos de 15°. Su diseño modular hace posible una colocación rápida con una adaptabilidad total. La órtesis de cadera DynaCox está autorizada para su uso en los ámbitos clínico y doméstico. INDICACIONES • Después de reposición de articulaciones de cadera luxadas • Inestabilidad después de endoprótesis total • Desequilibrio muscular CONTRAINDICACIONES • Colaboración insuficiente del paciente (p.ej. debido a demencia avanzada) • Lesiones dermatológicas graves (trastorno en la cicatrización de las heridas con formación de fístulas) FUNCIÓN • Aseguramiento del resultado quirúrgico • Profilaxis de luxación • Terapia motriz funcional temprana 26 SEGURIDAD Antes de usar el producto es imprescindible que un técnico ortopédico experimentado ajuste la órtesis al paciente y le instruya en su uso conforme a las indicaciones médicas. Una selección, una adaptación, un uso y/o un control incorrectos del ajuste del producto puede provocar daños en la salud. Nunca coloque la órtesis si la piel del paciente está lesionada. Antes de usar la órtesis compruebe que no presenta daños (p.ej. uniones flojas, cierres, cinturones, etc.). Si sospecha que el funcionamiento del producto no es correcto (p.ej. sensación de que disminuye la efectividad o que esta es insuficiente), deberá consultar inmediatamente al médico o al técnico ortopédico. El médico o el técnico ortopédico deberá comprobar con regularidad el funcionamiento, el ajuste correcto y los posibles desperfectos de la órtesis. Los intervalos entre los controles deberán establecerse de forma individual para cada paciente. Si la órtesis ha sido sometida a una carga excesiva, el técnico ortopédico deberá comprobar que no presenta daños antes de continuar con su uso. Los bordes cortantes o similares que puedan resultar del procesamiento del producto deberán ser pulidos para evitar lesionar la piel. Es necesario evitar la presión excesiva de la órtesis porque podría causar lesiones cutáneas y problemas circulatorios. La órtesis debe ajustarse de forma individual (moldeado según el producto y/o acabado mediante arranque de virutas), por tanto, no está indicada para su reutilización por otras personas. Almacenamiento/eliminación: El producto no contiene sustancias peligrosas para el medioambiente. Puede desecharse a través de la basura doméstica. 27 Aviso Indicación Atención DESCRIPCIÓN DynaCox tiene un diseño modular. Los componentes de la órtesis y de la venda son combinables entre sí. Los componentes de la órtesis (no las bandas) se pueden utilizar para el lado derecho/ izquierdo. DynaCox se entrega parcialmente confeccionada. (Longitud de fémur = Distancia corva – trocánter mayor) Los cinturones del muslo son intercambiables entre sí y pueden solicitarse por separado. Estos cinturones amplían/reducen el D2 (diámetro 2) en 3 cm respectivamente. USO/ AJUSTE Ajustes I Para el ajuste óptimo de la órtesis DynaCox, el paciente deberá estar de pie con las piernas ligeramente abducidas. Todos los tornillos tienen un ancho de llave de 3 mm, hexagonal. 1 Abra la cremallera del bolsillo del fémur y afloje los dos tornillos que se ven debajo del bolsillo. 2 Para orientarse, mantenga la órtesis junto a la pierna afectada del paciente. El borde superior de la almohadilla del fémur deberá colocarse directamente por debajo del trocánter. 3 Tire o deslice la abrazadera condilar hasta la longitud deseada. Apriete nuevamente los tornillos y cierre la cremallera. 4 Abra el cierre de velcro de la cubierta de los tornillos y afloje los dos tornillos centrales. 30 USO/ AJUSTE Ajustes II 5 Para orientarse, mantenga la órtesis junto a la pierna afectada del paciente. El borde superior de la almohadilla del fémur deberá colocarse directamente por debajo del trocánter. Coloque la abrazadera pélvica entre la cresta de la pelvis y el trocánter. Importante: La abrazadera pélvica debe ser adaptada termoplásticamente. (Véase pág. 36 Adaptación termoplástica) 6 Tire o deslice la abrazadera pélvica hasta la longitud deseada. 7 Apriete nuevamente los tornillos y cierre la cubierta de los tornillos. 8 Afloje ligeramente los dos tornillos laterales de la abrazadera condilar. 9 La órtesis está preparada ahora para su prueba. Consejo: Para una colocación más sencilla, pegue la cinta pélvica interior con su cierre de gancho hacia fuera sobre el bolsillo femoral, y la cinta del muslo desde atrás sobre su cierre de gancho hacia fuera. 31 USO/ AJUSTE Ajustes III 10 Coloque la órtesis sobre el paciente sin apretarla. 11 Primeramente, cierre el cinturón para la corva de la abrazadera del cóndilo. Asegúrese de que la altura es correcta: la distancia entre el borde superior de la rótula y la abrazadera condilar debe ser aprox. el ancho de dos dedos. 12 Ciña bien la banda al cuerpo y ciérrela uniendo las dos partes del velcro® ayudándose del asa de la banda. 13 Compruebe que la abrazadera condilar está bien ajustada al cuerpo y apriete los dos tornillos. 32 USO/ AJUSTE Ajustes IV 14 Cierre el cierre de velcro de la cinta del muslo. 15 Acorte los cintas del muslo y de la pelvis con unas tijeras según las medidas del paciente. 16 Cierre el cierre de clic y ajuste firmemente el cinturón pélvico de la órtesis tirando de los dos lados. 17 Acorte el cinturón pélvico con unas tijeras según las medidas del paciente. 33 USO/ AJUSTE Ajustes V 18 Ajuste de la articulación ROM-Flex Deslice hacia arriba la protección de neopreno de la articulación de modo que tenga acceso a los dos tornillos de ajuste. ¡Afloje siempre uno sólo de los dos tornillos limitadores! Desatornille completamente el tornillo limitador. A través de la ventana de control podrá ver el valor actual del ángulo sobre la escala. Coloque el tornillo limitador en una de las roscas, de modo que sea posible conseguir la flexión o la extensión deseada. (Ejemplo: máx. 30° flexión) Apriete nuevamente el tornillo. Puede ajustar la flexión y la extensión en pasos de 15°. Desatornille el segundo tornillo limitador y proceda de la misma manera. Vuelva después a apretarlo. (Ejemplo: máx. 15° extensión) 34 USO/ AJUSTE Ajustes V Indicación Si la articulación se va a emplear sin límite en el movimiento, retire los dos tornillos limitadores. Al quitarlos se suelta también el anillo de la parte posterior. Si fuera necesario, este anillo puede volver a atornillarse en cualquier momento. Para finalizar, deslice de nuevo la protección de neopreno de la articulación sobre la articulación ROM-Flex. De este modo evitará que la ropa se quede pillada en la articulación. 19 La órtesis ajustada. Gracias a los tornillos de fijación cautivos Tuflock® no es necesario emplear Loctite. 35 USO/ AJUSTE Consejo para el ajuste: En el caso de pacientes especialmente adiposos, se puede utilizar una venda interior de compresión bajo la banda del DynaCox a fin de estabilizar las partes blandas. Le recomendamos emplear nuestra banda universal para tronco Silva® REF 40 368 … (véase el catálogo “Órtesis“). Ajuste termoplástico: Los componentes de plástico pueden moldearse termoplásticamente. • Despegue el acolchado. • Caliente la zona a moldear aplicando en ligeros círculos un soplete de aire caliente. • Temperatura para el procesamiento: abrazadera pélvica 140°C/abrazadera condilar 160°C. • Cuando la superficie del material tenga una consistencia similar a la cera y brille puede comenzar con el moldeado manual. (¡Atención! Peligro de quemaduras. Utilice guantes térmicos). • Una vez que el material se haya enfriado completamente, pegue de nuevo el acolchado. Adaptación posterior: En algunos casos puede ser necesario doblar con posterioridad la férula lateral distal a la placa femoral. • Utilice un triscador circular (MW 4). • Atención: Si la férula se ha triscado directamente en distal a la placa femoral, ya no es posible acortarla. 36 MATERIAL Banda: Órtesis: 48% poliéster PVC duro 36% CoolMax PETG 16% elastánAluminio Férulas de acero CONFORT/ HIGIENE Es necesario llevar una camiseta debajo de la órtesis DynaCox. El slip deberá ponerse por encima de la órtesis DynaCox. COMPATIBILIDAD DynaCox no esté prevista para su combinación con otros productos diferentes a los indicados en estas instrucciones de uso. RETIRAR LA ÓRTESIS Conforme a la evolución del tratamiento, los componentes rígidos (ortésicos) de la órtesis DynaCox se pueden quitar de la banda. 80 C CUIDADO, LIMPIEZA Y MANTENIMIENTO DE PRODUCTO A La banda del muslo puede quitarse del vendaje para lavarlo. Para 40no C interrumpir el tratamiento se puede solicitar un segundo cinturón (REF 22 303 …). Si la órtesis DynaCox se va a utilizar durante un periodo de 95 C CL tiempo prolongado, recomendamos disponer de una banda de recambio (REF 22 301 ...). 40 C La banda deberá ser sustituida por otra si se va a lavar en una tintorería. 30° • Lavado a mano • No planchar • No secar en la secadora • No blanquear • Limpieza química no permitida 37 • Limpiar los componentes ortésicos con un paño húmedo DESCRIZIONE DEL PRODOTTO L‘ortesi per anca DynaCox è un tutore che unisce i vantaggi di un bendaggio aderente al corpo (miglioramento della funzione propriocettiva) e di elementi di stabilizzazione integrati tipici di un‘ortesi basata sul principio a 3 punti. La struttura a maglia elastica favorisce una compressione uniforme che rende confortevole l‘utilizzo. In virtù dell‘apposita articolazione TRI-Flex, composta da più elementi, si evita altresì un‘incongruenza tra il punto di rotazione dell‘anca e l‘asse meccanico di articolazione. L‘articolazione ROM-Flex consente una regolazione graduata della flesso-estensione a passi di 15°. La struttura modulare permette di adattare velocemente e perfettamente l‘ortesi alle conformità anatomiche del paziente. INDICAZIONI • Dopo la riposizione dell‘anca lussata • Instabilità dopo il trattamento endoprotesico totale • Artrosi coxo-femorale • Sbilanciamento muscolare CONTROINDICAZIONI • Mancanza di compliance del paziente (ad es. a causa di demenza avanzata) • Gravi alterazioni cutanee (disturbi della guarigione delle ferite con formazione di fistole) FUNZIONE • Revisione ed assistenza dell‘anca a seguito di interventi chirurgici • Profilassi in caso di lussazione • Terapia di movimento prefunzionale 38 SICUREZZA Prima di utilizzare l’ortesi è assolutamente necessario consultare un tecnico ortopedico esperto che lo adatti alle esigenze individuali e vi istruisca circa l’uso in conformità alle prescrizioni mediche. Un’ortesi sbagliata, erroneamente adattata ed utilizzata, nonché un controllo insufficiente del posizionamento del prodotto possono causare gravi danni alla salute. Non applicare mai l’ortesi sulla cute lesionata. Prima di applicare l’ortesi controllare sempre che non sia danneggiata (ad es. punti di giuntura, chiusure e cinture laschi, etc.). Sospettando problemi all’ortesi (ad es. diminuzione o assenza degli effetti desiderati) consultare immediatamente il medico o il tecnico ortopedico. Un medico o un tecnico ortopedico dovranno controllare regolarmente che l’ortesi funzioni bene, sia ben posi zionata e non presenti possibili danni. La frequenza dei controlli deve essere fissata individualmente secondo le esigenze del paziente. In caso di sollecitazioni eccessive dell’ortesi, prima del riutilizzo, va consultato un tecnico ortopedico per veri ficare la presenza di eventuali danni. I bordi vivi o simili, che potrebbero formarsi per la specifica lavorazione del prodotto con asportazione di trucioli, vanno sbavati al fine di evitare lesioni cutanee. Una pressione eccessiva esercitata dall’ortesi può provocare gravi lesioni della cute nonché disturbi circolatori, e va quindi assolutamente evitata. Proprio a causa del necessario adattamento individuale dell’ortesi (messa in forma in funzione del prodotto e/o lavorazione con asportazione di trucioli) essa non è adatta al riutilizzo con altri pazienti! Immagazzinamento/Smaltimento: Il prodotto non contiene materiali nocivi per l’ambiente. Può essere smaltito nell’immondizia domestica. 39 Avvertenza Attenzione Avviso DESCRIZIONE DynaCox è stata concepita come sistema modulare. I componenti dell‘ortesi e dei bendaggi sono combinabili fra loro. I componenti dell’ortesi (non bendaggio) sono utilizzabili a destra / sinistra. DynaCox viene fornita confezionata nei suoi singoli componenti. Le cinghie coscia aumentano o diminuiscono la C2 (circonferenza 2) di rispettivamente 3 cm. Tutte le viti sono provviste di esagono cavo apertura 3 mm. 1 Aprire la chiusura lampo della tasca femorale e quindi svitare ambedue le due viti al di sotto ora visibili. 2 Per meglio regolarsi tenere l‘ortesi vicino alla gamba interessata del paziente. Il bordo superiore della pelotta femorale deve essere posizionato subito sotto il trocantere. 3 Tirare o spingere il s upporto condilare per raggiungere la lunghezza desiderata. Riavvitare le viti e richiudere la chiusura lampo. 4 Aprire la chiusura a velcro del copriviti e svitare ambedue le viti centrali. 42 APPLICAZIONE Adattamenti II 5 Per meglio regolarsi tenere l‘ortesi vicino alla gamba interessata del paziente. Il bordo superiore della pelotta femorale deve essere posizionato subito sotto il trocantere. Il supporto del bacino deve essere posizionato tra la crista iliaca ed il trocantere. Importante: Il supporto del bacino deve essere modellato con un procedimento termoplastico per adattarlo al paziente. (vedi pg. 48 Adattamento termoplastico). 6 Tirare o spingere il supporto del bacino per raggiungere la lunghezza desiderata. 7 Riavvitare le viti e richiudere il velcro del copriviti. 8 Allentare ambedue le viti laterali del supporto condilare. 9 Ora l‘ortesi è pronta per essere provata indosso. 43 Consiglio: Per far indossare più facilmente l‘ortesi fermare bene l‘aletta per bacino interna all‘ esterno, agganciandola per mezzo del suo stesso gancetto di chiusura alla tasca femorale e l‘aletta per coscia da dietro al suo stesso gancetto di chiusura. APPLICAZIONE Adattamenti III 10 Mettere l‘ortesi al paziente senza ancora stringerla o fissarla. 11 Chiudere per prima cosa la cinghia cavità poplitea del supporto condilare. Considerare l‘altezza giusta, il bordo super iore della patella dovrebbe trovarsi ca. due dita lontano dal supporto condilare. 12 Chiudere attorno al corpo la chiusura a velcro del bendaggio servendosi dell‘apposito passante di presa. 13 Controllare che il supporto condilare sia piano ed avvitare ambedue le vit. 44 APPLICAZIONE Adattamenti IV 14 Chiudere la chiusura a velcro della aletta per coscia. 15 Accorciare tagliandole con una forbice le alette per coscia e bacino dell‘ortesi a seconda delle misure del paziente. 16 Chiudere la chiusura a scatto e tirare bene le cinghie bacino dell‘ortesi su entrambi i lati. 17 Accorciare tagliandole con una forbice le cinghie bacino a seconda delle misure del paziente. 45 APPLICAZIONE Adattamenti V 18 Regolazione dell’articolazione ROM Rimuovere verso l’alto la protezione dell’articolazione in neoprene in modo da esporre le due viti di regolazione. Svitare sempre una sola delle due viti di fermo! Svitare completamente unaa vite di fermo. Sulla scala graduata è possibile leggere il valore d’angolo corrente attraverso la finestrella d’ispezione. Posizionare la vite di fermo in una delle filettature in modo da ottenere la flessione e/o estensione desiderata. (Esempio: flessione max. 30°) Serrare di nuovo la vite. È possibile regolare la flessione e l’estensione ad intervalli di 15°. Svitare la seconda vite di fermo e procedere nello stesso modo. Serrare di nuovo anche questa seconda vite. (Esempio: estensione max. 15°) 46 APPLICAZIONE Adattamenti V Avvertenza Se è previsto l’uso dell’articolazione senza limitazione del movimento, rimuovere entrambe le viti di fermo. Contemporaneamente si stacca anche l’anello posteriore, che può essere quindi prelevato. Se necessario, questo anello può essere riavvitato in qualsiasi momento. Per concludere, riapplicare la protezione dell’articolazione in neoprene in dotazione sull’articolazione ROM In tal modo si evita che i vestiti possano rimanere impigliati. 19 Ortesi regolata pronta. Non è necessario utilizzare Loctite date le viti Tuflock® autobloccanti. 47 APPLICAZIONE Consiglio sull‘adattamento: In caso di pazienti con eccessiva adiposità è possibile applicare sottoal bendaggio DynaCox una fascia interna di compressione per facilitare la stabilizzazione dell‘ortesi stessa. Consigliamo di utilizzare il nostro apposito bendaggio per tronco universale Silva® Universal REF 40 368 … (vedi catalogo „Ortetica“). Adattamento termoplastico: I componenti in materiale sintetico possono essere modificati in un secondo tempo con procedimento termoplastico. • Rimuovere l‘imbottitura fissata con il velcro. • La parte da cambiare va riscaldata con leggeri movimenti rotatori per mezzo di una soffiante ad aria calda. • Temperatura di lavorazione: Supporto del bacino 140 °C/ supporto condilare 160 °C. • Quando la superficie del materiale diventa come cera ed assume un aspetto lucido si può procedere a modellare la parte interessata con le mani (attenzione pericolo di ustioni – indossare guanti di protezione contro i rischi termici!). • Una volta che il materiale si è completamente raffre ddato riposizionare esattamente con le chiusure a velcro l‘imbottitura sull‘ortesi. Ulteriore adattamento: Nel singolo caso potrebbe risultare necessario piegare l‘asta laterale distalmente rispetto alla piastra femorale. • Utilizzare apposita pinza per strozzamento (MW 4). • Prudenza: Qualora lo strozzamento dell‘asta sia avvenuto distalmente rispetto alla piastra femorale non è più possibile un accorciamento mediante inserimento. 48 MATERIALE Bendaggio: Órtesis: 48% poliestere PVC duro 36% CoolMax PETG 16% elastan Alluminio Stecche di acciaio COMFORT/ IGIENE Sotto l‘ortesi DynaCox va indossata una canottiera/T-shirt. COMPATIBILITÀ La DynaCox non è stata concepita per un utilizzo con altri componenti che non siano quelli citati nelle seguenti istruzioni per l‘uso. FINE TERAPIA Seguendo il decorso della terapia è possibile separare i componenti fissi (ortesici) di DynaCox dal bendaggio. Lo slip va indossato sopra l‘ortesi DynaCox. In tal modo sussiste la possibilità di continuare ad assistere il paziente proseguendo la terapia con la compressione elastica esercitata dal bendaggio. CURA/ PULIZIA/ MANUTENZIONE DEL PRODOTTO La 80 cinghia coscia può essere C A staccata dal bendaggio, per lavarla, aprendo la chiusura lampo. Al fine di non interrompere la terapia si può applicare sullo stesso punto una seconda cinghia separata 40 C (REF 22 303 …). In caso di utilizzo prolungato di DynaCox consigliamo la dotazione che include un bendaggio di ricambio 95 C CL (REF 22 301 ...).
