HemosIL® Dimertest ® - 49738960 Dimertest ® - 49738960 ENGLISH - Insert revision 11/2014 INTENDED USE The DIMERTEST® Latex Assay is intended for the rapid qualitative or semi-quantitative evaluation of circulating derivatives of cross-linked fibrin degradation products (XL-FDP) in human plasma. SUMMARY AND EXPLANATION OF THE TEST During blood coagulation, fibrinogen is converted to fibrin by the activation of thrombin. The resulting fibrin monomers polymerize to form a soluble gel of non-cross-linked fibrin. This fibrin gel is then converted to cross-linked fibrin by thrombin activated Factor XIII to form an insoluble fibrin clot. Production of plasmin, the major clot-lysing enzyme, is triggered when a fibrin clot is formed. Fibrinogen and fibrin are both cleaved by the fibrinolytic enzyme plasmin to yield degradation products, but only degradation products from cross-linked fibrin contain D-dimer.1-3 Therefore, cross-linked fibrin degradation products (XL-FDP) are a specific marker of fibrinolysis. TEST PRINCIPLE DIMERTEST® Latex is a rapid agglutination assay utilizing latex beads coupled with a highly specific D-dimer monoclonal antibody. XL-FDP present in a plasma sample bind to the coated latex beads, which results in visible agglutination occurring when the concentration of D-dimer is above the threshold of detection of the assay. REAGENTS Composition R Latex Reagent: a 0.83% suspension of latex particles coated with murine anti-D-dimer monoclonal antibody, 10 mg/mL BSA and 0.1% sodium azide. P Positive Control: a solution containing purified human D-dimer fragment, 5 mg/mL BSA and 0.1% sodium azide. N Negative Control: a buffer solution containing 5 mg/mL BSA and 0.1% sodium azide. BBuffer: a 10 mM phosphate buffer solution containing 0.1% sodium azide. Warnings and Precautions • For In Vitro Diagnostic Use Only. The Dimertest kit is classified as: Hazard Class: Acute Tox. 4, H302 Hazard statements: H302: Harmful if swallowed. Precautionary statements: P270: Do no eat, drink or smoke when using this product. P301+312: IF SWALLOWED : Call a poison center or doctor/physician if you feel unwell. P330: Rinse mouth. • CAUTION: All reagents in DIMERTEST® Latex contain sodium azide (0.1%) as preservative. Do not ingest or allow to contact skin or mucous membranes. Sodium azide may form explosive azides in metal plumbing. Use proper disposal procedures. • CAUTION: The Positive Control in DIMERTEST® Latex contains components of human origin. Each individual blood donation intended for the production of this reagent is tested for Hepatitis B antigen, anti-HCV, anti-HIV1 and anti-HIV2. Only donations with negative findings are employed. As complete absence of infectious agents can never be assured, all materials derived from human blood should be treated as potentially infectious and handled with due care following the precautions recommended for biohazardous material. Storage and Stability Storage: Store at 2°C to 8°C. DO NOT FREEZE. Stability: Refer to outer package and vial labels for expiration date. Indication of Reagent Deterioration Reagent deterioration is indicated by failure of the Latex Reagent to agglutinate with the Positive Control, agglutination with the Negative Control, or evidence of microbial contamination. SPECIMEN COLLECTION AND PREPARATION Plasma prepared from whole blood anticoagulated with sodium citrate is recommended. The use of EDTA and heparin will result in an increased level of false positive reactions. After separation of the plasma by centrifugation, specimens may be tested directly for the presence of XL-FDP. Defibrination of the plasma is not recommended. Plasma storage/stability: -20°C: 2 weeks Thaw frozen specimens rapidly at 37°C and centrifuge before testing. Refer to CLSI (formerly NCCLS) publication H21-A5 for further instructions on specimen collection, handling and storage.4 PROCEDURE Materials Provided DIMERTEST® (Cat. ZUSAMMENFASSUNG QUALITY CONTROL It is recommended that both Positive and Negative Controls be included in each batch of tests to ensure proper functioning of the system. Control solutions should be tested by the same procedures as patient samples. For the qualitative screening procedure, a positive result (agglutination) can be obtained by substituting DIMERTEST® Positive Control for the plasma in steps 3-6. Conversely a negative result (no agglutination) can be obtained by substituting DIMERTEST® Negative Control for the plasma in steps 3-6. When performing the semiquantitative procedure, it is recommended that testing of serial dilutions of the Positive Control be conducted. Dimertest® Latex Positive Control consists of a solution of human D-dimer at a level of approximately 0.80 mg/L (800 ng/mL). RESULTS A. BBuffer: 10 mM Phosphatpufferlösung, die 0,1% Natriumazid enthält. 1:8 + LIMITATIONS OF THE PROCEDURE Clinical diagnosis should not be based on the result of DIMERTEST® Latex alone. Clinical signs and other relevant test information should be included in the diagnostic decision. EXPECTED VALUES A positive result, indicating active fibrinolysis, should be obtained with DIMERTEST® Latex when XL-FDP (D-dimer) levels are at or greater than approximately 0.20 mg/L (200 ng/mL). Plasma specimens from normal subjects are expected to give negative results because their plasma XL-FDP concentrations are typically less than 0.20 mg/L (200 ng/mL). Due to many variables that may affect results, each laboratory should establish its own normal range. Elevated levels of XL-FDP (containing the D-dimer domain) have been demonstrated in patients by a combination of immunoprecipitation and gel electrophoresis techniques.1-3,5,6 Monoclonal antibodies allow the specific detection of the D-dimer domain.7 Monoclonal antibody based D-dimer assays are of diagnostic value in disseminated intravascular coagulation (DIC) and acute vascular diseases, including pulmonary embolism (PE) and deep venous thrombosis (DVT), conditions that are difficult to detect reliably by clinical examination.8,9 The amount of XL-FDP detected in a specimen will depend on several interrelated factors in vivo, such as the severity of the thrombotic episode, the rate of cross linked fibrin formation, and the time elapsed after the thrombotic event until blood is drawn from the patient. Elevated levels of XL-FDP as an indication of reactive fibrinolysis have also been reported in surgery, trauma, sickle cell disease, liver disease, severe infection, sepsis, inflammation, and malignancy.10,11 D-dimer levels also rise during normal pregnancy but very high levels are associated with complications.12 DIMERTEST® Latex does not cross-react with fibrinogen, factor XIIIa cross-linked fibrinogen,13 or fibrinogen degradation products.8 Warnung • Dieses Produkt ist nur für die in vitro Diagnostik geeignet. Da das Vorhandensein von infektiösen Agentien nie mit hundertprozentiger Sicherheit ausgeschlossen werden kann, sollten sämtliche Materialien humanen Ursprungs als potentiell infektiös betrachtet und nach den für biogefährliches Material empfohlenen Richtlinien behandelt werden. Lagerung und Haltbarkeit Lagerung: Lagerung zwischen 2-8°C. NICHT EINFRIEREN. Haltbarkeit: Das Verfallsdatum befindet sich auf der Verpackung und den Etiketten. Anzeichen von Reagenzverfall Sollte bei der Positiv-Kontrolle keine Agglutination der Latex-Partikel auftreten, die Negativ-Kontrolle agglutinieren oder Anzeichen einer mikrobiellen Kontamination erscheinen, so ist dies ein Hinweis auf einen möglichen Verfall des Reagenzens. PROBENENTNAHME UND VORBEREITUNG Es wird empfohlen, Citrat-Plasma zu verwenden. Der Einsatz von EDTA oder Heparin kann zu vermehrt falsch positiven Ergebnissen führen. Nach dem Zentrifugieren kann das Plasma sofort auf das Vorkommen von XL-FDP getestet werden. Die Defibrination des Plasmas wird nicht empfohlen. Lagerung/Haltbarkeit Plasma: -20°C: 2 Wochen Gefrorene Proben bei 37°C rasch auftauen und vor dem Testen zentrifugieren. Hinweise zur Aufbereitung des Blutes sind den Empfehlungen des Deutschen Instituts für Normung - DIN 58 905 - oder dem CLSI (ehemals NCCLS) Dokument H21-A5 zu entnehmen.4 SPECIFIC PERFORMANCE CHARACTERISTICS TESTVERFAHREN Plasma from one hundred and seventy (170) apparently healthy, voluntary blood donors was tested using DIMERTEST® Latex. A negative result was obtained for one hundred and sixty-two (162) of the samples. This equates to a specificity of 95.3% (162/170). One hundred and forty-five (145) plasma samples from patients judged to be suffering from, or having a high probability for thrombotic episode, were tested by DIMERTEST® Latex and another agglutination reference method. The correlation coefficient was r = 0.94 and the regression equation was y = 1.19x. Intra-assay (within run) reproducibility was determined for 10 replicates of 3 plasma samples that contained different levels of XL-FDP. The results were equivalent for all replicates. Inter-assay (run-to-run) reproducibility was determined using 10 plasma samples with XL-FDP titers ranging from 1 to 16. In 10 runs, the replicates of these specimens did not vary by more than one titer. In an anticoagulant study of 50 parallel citrated, EDTA and heparin plasma samples, the correlation between the titers obtained with DIMERTEST® Latex and the expected titers (based on ELISA XL-FDP values) was r = 0.91 for citrated samples, r = 0.73 for EDTA samples and r = 0.78 for heparin samples. Citrate is the anticoagulant of choice. In a study of samples from patients with rheumatoid arthritis, 17 were found to agglutinate with DIMERTEST® Latex. In all 17 samples, the agglutination could be inhibited by the addition of the D-dimer specific monoclonal antibody DD3B6/22, but not with a non-specific monoclonal antibody of the same subgroup, IgG3K. This suggests that DIMERTEST® Latex is insensitive to rheumatoid factor disturbances. No assay interference was demonstrated with DIMERTEST® Latex with spiked specimens containing potential interferents at the following concentrations: Mitgelieferte Materialien DIMERTEST® (Art. Prepare serial dilutions of the test plasma with Buffer as follows: 1:2 dilution 100 μL plasma plus 100 μL Buffer solution 1:4 dilution 100 μL 1:2 dilution plus 100 μL Buffer solution 1:8 dilution 100 μL 1:4 dilution plus 100 μL Buffer solution 2. Test each dilution as described in the qualitative method. 5.0 mg/mL 0.06 g/mL Qualitative Method 1. Mark (or make note of) positions on the test slide for specimens and, as needed, for positive and negative controls. 2. Hold the Latex Reagent dropper bottle vertically and place one drop of the reagent within a well on a Test Card. 3. AVOID touching the surface of the Test Card. Accurately pipette 20 μL of undiluted plasma or add one drop of control solution inside the same well next to the drop of Latex Reagent. 4. Mix the Latex Reagent and sample with a stirrer until the Latex is uniformly distributed. 5. Rock the Test Card gently by hand for exactly 3 minutes. 6. At exactly 3 minutes, check for agglutination under a strong light source. Note: If test reading is delayed beyond 3 minutes, the latex suspension may dry out giving a false agglutination pattern. If this is suspected, the specimen must be retested. 7. Den Latexreagenz- und Probentropfen mit einem Kunststoffrührstab mischen, bis das Latexreagenz gleichmäßig verteilt ist. Objektträger für exakt 3 Minuten von Hand bewegen; keine mechanischen Mischer o.ä. verwenden. 6. Nach Ablauf der 3 Minuten sofort unter einer starken Lichtquelle auf Agglutination prüfen. Hinweis: Falls ein Ablesen des Tests erst nach mehr als 3 Minuten erfolgt, ist ein Austrocknen der Latex-Suspension möglich, was zu einem falschen Agglutinationsmuster führt. Wenn dies vermutet wird, muss die Probe neu etestet werden. 7. Testkarte und Rührer in einem Behälter für biogefährliches Material entsorgen. Nicht wiederverwenden. Semiquantitative Methode 1. Mit Phosphatpuffer Serienverdünnungen des Testplasmas wie folgt herstellen: Verdünnung 1:2 100 μL Plasma + 100 μL Pufferlösung Verdünnung 1:4 100 μL Verdünnung 1:2 + 100 μL Pufferlösung Verdünnung 1:8 100 μL Verdünnung 1:4 + 100 μL Pufferlösung 2. Jede Verdünnung wie für die qualitative Methode beschrieben testen. QUALITÄTSKONTROLLE Um die richtige Funktionalität des Systems sicherzustellen und die Qualität der Analyse zu überprüfen, wird empfohlen, in jeder Serie sowohl die Positiv- als auch die Negativ-Kontrolle mitlaufen zu lassen. Die Kontrolllösungen müssen mit derselben Methode wie die Patientenproben getestet werden. Wird das Ergebnis nur qualitativ ausgegeben kann ein positiver Befund (Agglutination) erzielt werden, indem statt Plasma die DIMERTEST® Positiv-Kontrolle in den Schritten 3-6 der Methodendurchführung eingesetzt wird. Dementsprechend kann ein Negativbefund (keine Agglutintion) durch Einsetzen der DIMERTEST® Negativ-Kontrolle in den Schritten 3-6 erzielt werden. Bei Ausführung der semiquantitativen Methode wird empfohlen, serielle Verdünnungen der Positiv-Kontrolle zu machen. Die DIMERTEST® Positiv-Kontrolle ist eine Lösung mit ungefähr 0,80 mg/L (800 ng/mL), humanem D-Dimer. ERGEBNISSE A. Qualitativer Test Beim qualitativen Testprotokoll sollte folgendes Ergebnismuster erscheinen: Unverdünntes Plasma D-Dimer (XL-FDP) Konzentration Negativ weniger als 0,20 mg/L (200 ng/mL) Positiv mehr als 0,20 mg/L (200 ng/mL) Hinweis: Bei den Angaben in mg/L (ng/mL) handelt es sich um Näherungswerte. B. Semiquantitativer Test Näherungsbereiche für XL-FDP D-Dimer-Spiegel der verschiedenen Plasmaverdünnungen werden in Tabelle 1 aufgeführt. Wie bei allen semiquantitativen Testmethoden kann eine gewisse Variabilität bezüglich der Dose-Response erwartet werden. EINSCHRÄNKUNGEN Eine klinische Diagnose sollte nicht auf einem DIMERTEST® Latex Ergebnis alleine basieren. Das klinische Bild und andere relevante Testinformationen sollten in die diagnostische Bewertung mit einbezogen werden. ZU ERWARTENDE WERTE Ein positives Resultat, welches eine aktive Fibrinolyse indiziert, sollte mit dem DIMERTEST® Latex erhalten werden, wenn die XL-FDP- (D-Dimer) Konzentrationen größer oder gleich 0,20 mg/L (200 ng/mL) sind. Plasmaproben von normalen Testpersonen sollten aufgrund von XL-FDP-Konzentrationen unterhalb 0,20 mg/L (200 ng/mL) ein negatives Testergebnis liefern. Aufgrund verschiedener Variablen, die das Testergebnis beeinflussen können, wird empfohlen, dass jedes Labor seinen eigenen Normalbereich ermittelt. Erhöhte Konzentrationen von XL-FDP (D-Dimer-Domänen beinhaltend) können bei Patienten durch eine Kombination von Immunpräzipitation und Gel-Elektrophorese-Techniken nachgewiesen werden.1-3,5,6 Monoklonale Antikörper7 erlauben den spezifischen Nachweis von D-Dimer-Domänen. Auf monoklonalen Antikörpern basierende D-Dimer Tests sind von diagnostischer Bedeutung vor allem bei disseminierter intravasaler Gerinnung (DIC) und akuten vaskulären Erkrankungen, wie Lungenembolien (PE) und tiefen Venenthrombosen (DVT), die mittels klinischer Untersuchungen nur schwer zuverlässig nachzuweisen sind.8,9 Die in einer Probe nachgewiesene Menge an XL-FDP, hängt von verschiedenen, in vivo in Bezug zueinander stehenden, Faktoren ab, wie des Schweregrads des thrombotischen Ereignisses, der Bildungsrate des quervernetzten Fibrins und der Zeitspanne zwischen Auftreten des thrombotischen Ereignisses und der Blutentnahme. Erhöhte XL-FDP-Konzentrationen als Indikation für eine reaktive Fibrinolyse sind ebenso bei Operationen, Traumen, Sichelzellenanämie, Lebererkrankungen, Sepsis, Entzündungen und malignen Tumoren beschrieben worden.10,11 Auch bei normal verlaufenden Schwangerschaften können erhöhte D-Dimer Spiegel auftreten, sehr hohe Werte sind jedoch meist mit Komplikationen verbunden.12 Beim DIMERTEST® Latex tritt keine Kreuzreaktion mit Fibrinogen, mit Faktor XIII stabilisiertem, quervernetzten Fibrinogen13 oder Fibrinogen Abbauprodukten auf.8 LEISTUNGSDATEN Plasma von 170 gesunden, freiwilligen Blutspendern wurde mit dem DIMERTEST® Latex getestet. Bei 162 Plasmen wurde ein negatives Testergebnis erzielt. Dies entspricht einer Spezifität von 95,3 % (162/170). 145 Plasmaproben von Patienten mit thrombotischen Ereignissen bzw. einer hohen Wahrscheinlichkeit dafür, wurden ebenfalls mit dem DIMERTEST® Latex und einer anderen Agglutinations-Referenz-Methode untersucht, wobei ein Korrelationskoeffizient von r= 0,94 und die Regressionsgleichung y=1.19x ermittelt wurde. Die Präzision wurde über zehn Läufe mit drei Plasmen unterschiedlicher D-Dimer Konzentration bestimmt. Die Ergebnisse waren bei allen Läufen vergleichbar. Die Präzision im Lauf wurde mit zehn Plasmen in den Verdünnungsstufen zwischen 1 und 16 bestimmt. In diesen zehn Läufen wichen die Plasmen nicht mehr als eine Verdünnungsstufe ab. In einer Antikoagulantien-Studie wurden 50 Plasmen parallel mit Citrat, EDTA und Heparin antikoaguliert und Korrelationen zwischen den mit dem DIMERTEST® Latex ermittelten und den erwarteten, auf ELISA D-Dimer Werten beruhenden, Titer erstellt. Die ermittelten Korrelations-koeffizienten r= 0,91 für Citrat-Plasmen, r=0,73 für EDTA-Plasmen und r= 0,78 für heparinisierte Plasmen führen zu der Empfehlung Citrat-Plasmen zu verwenden. In einer Studie mit Plasmen von Patienten mit rheumatoider Arthritis, agglutinierten 17 der Plasmen mit DIMERTEST® Latex. In allen 17 Proben konnte die Agglutination durch Zugabe von D-Dimer spezifischen monoklonalen Antikörpern DD3B6/22 gehemmt werden; mit nicht-spezifischen monoklonalen Antikörpern der selben Untergruppe, IgG3k, trat diese Hemmung nicht ein. Dies zeigt, dass der DIMERTEST® Latex insensitiv gegenüber Störungen des Rheumafaktors ist. LIMITACIONES DEL PROCEDIMIENTO El diagnóstico clínico no debe basarse únicamente en el resultado del DIMERTEST®. Para establecer un diagnóstico, los resultados del análisis deben ser usados conjuntamente con otra información, incluyendo el contexto clínico. VALORES ESPERADOS Cuando los niveles de XDP (Dímero-D) son superiores a 0,20 mg/L (200 ng/mL) el resultado se considerará positivo, indicando una fibrinolisis activa. Muestras de individuos normales deben dar resultados negativos ya que la concentración en su plasma de XDP es normalmente inferior a 0,20 mg/L (200 ng/mL). Debido a las variables que pueden afectar los resultados, cada laboratorio debe establecer su propio rango de normalidad. Niveles elevados de XDP (que contengan el dominio Dímero D) se han detectado en pacientes con la combinación de técnicas de inmunoprecipitación y electroforesis en gel.1-3,5,6 Anticuerpos monoclonales (Mab)7 permiten actualmente detectar específicamente el dominio Dímero D. Los ensayos de Dímero D, basados en anticuerpos monoclonales, tienen un gran valor diagnóstico en la coagulación intravascular diseminada (CID), y en patologías vasculares agudas tales como el embolismo pulmonar (EP) y trombosis venosa profunda (TVP), situaciones que son difíciles de detectar por examen clínico.8,9 La cantidad de XDP detectado en una muestra dependerá de diferentes factores in vivo, como son la severidad del episodio trombótico, el grado de formación de la malla de fibrina, y del tiempo transcurrido entre el episodio trombótico y la toma de muestra del paciente. También se han reportado niveles elevados de XDP como indicador de fibrinolisis reactiva en cirugía, traumatismos, en la anemia drepanocítica, hepatopatías, infección severa / sepsis, inflamación y procesos malignos.10 Durante un embarazo de desarrollo normal, los niveles de Dímero-D se elevan aunque mucho menos que en aquellos embarazos asociados a complicaciones.12 El Test de Látex DIMERTEST® no tiene reacciones cruzadas con el fibrinógeno, fibrinógeno unido al Factor XIIIa,13 o productos de degradación del fibrinógeno. CARACTERÍSTICAS FUNCIONALES Plasmas procedentes de 170 donantes de sangre aparentemente sanos se analizaron con el Test de Látex DIMERTEST®. Se obtuvieron 162 resultados negativos. La especificidad se estimó en un 95.3% (162/170). 145 Plasmas procedentes de pacientes que han sido diagnosticados o con alta probabilidad de sufrir un episodio trombótico, se analizaron con el Test de Látex DIMERTEST® y con otro método de aglutinación de referencia. El coeficiente de correlación fue de r = 0,94 y una ecuación de regresión de y = 1,19 x. La reproducibilidad intraserie se determinó a partir de 10 replicados de 3 plasmas que contenían distintos niveles de XDP. Los resultados fueron equivalentes para todos los replicados. La reproducibilidad interserie se determinó a partir de 10 plasmas que contenían una titulación desde 1 hasta 16. En 10 series, los replicados de estas muestras no variaron más de 1 título (o dilución). En un estudio sobre anticoagulantes 50 muestras de plasmas fueron tratadas paralelamente con Citrato Sódico, EDTA y Heparina. La correlación obtenida entre los títulos del Test de Látex DIMERTEST® y el título esperado (basado en valores de XDP obtenidos por ELISA) fue de r = 0,91 en las muestras citratadas, r = 0,73 en las muestras con EDTA y r = 0,78 en las muestras con Heparina. El Citrato Sódico es el anticoagulante óptimo. En un estudio con muestras procedentes de pacientes con artritis reumatoide, 17 muestras aglutinaron con el Test de Látex DIMERTEST®. En estas 17, la aglutinación se pudo inhibir con la adición de un anticuerpo monoclonal específico del Dímero D (DD3B6/22), pero no con un anticuerpo monoclonal inespecífico del mismo subgrupo, IgG3k. Esto sugiere que el Test de Látex DIMERTEST® es insensible a la presencia de factor reumatoide. No se ha demostrado interferencias con DIMERTEST® en muestras que contengan interferentes potenciales a las siguientes concentraciones: Bilirrubina 0,2 mg/mL Hemoglobina 5,0 mg/mL Lipemia (triglicéridos) 30 mg/mL Proteína (gamma globulina) 0,06 g/mL PRINCIPE Durant la coagulation, le fibrinogène est converti en fibrine par activation de la thrombine. La production de plasmine, enzyme majeure de lyse du caillot, est activée lorsqu’un caillot de fibrine est formé. Le fibrinogène et la fibrine sont clivés par la plasmine (enzyme fibrinolytique), mais seuls les produits de dégradation de la fibrine contiennent des D-Dimères.1-3 Enfin, les produits de dégradation (XL-FDP) de la fibrine stabilisée sont un marqueur de la fibrinolyse. DIMERTEST® Latex est un test sur lame basé sur l’agglutination de bille de latex couplées à un anticorps monoclonal très spécifique anti-D-Dimère. Les XL-FDP présents dans le plasma du patient se fixent sur les particules de latex adsorbées, et une agglutination apparaît lorsque la concentration en D-Dimères est supérieure au seuil décisionnel du test (« cut-off »). Indication de détérioration L’absence d’agglutination avec le contrôle positif, une agglutination avec le contrôle négatif ou la visualisation d’une contamination microbienne sont les témoins d’une détérioration du réactif latex. RECUEIL DES ECHANTILLONS ET PREPARATION Il est recommandé d’utiliser des plasmas préparés à partir de sang total anti-coagulé avec du citrate de sodium. L’utilisation d’ EDTA et d’héparine entraînera une augmentation du nombre de faux positifs. Les spécimens de plasmas doivent être analysés immédiatement après centrifugation. La défibrination du plasma n’est pas recommandée. Conservation/Stabilité du Plasma: 2 semaines à -20°C. Décongeler rapidement à 37°C les échantillons et centrifuger avant de réaliser le test. Pipeter avec précision 20 μl de plasma à tester non dilué ou ajouter une goutte de solution de contrôle à l’endroit précis où l’on a déposé le latex. 4. Mélanger le latex et l’échantillon à l’aide d’un agitateur jusqu’à obtention d’une répartition uniforme du latex. 5. Agiter manuellement et doucement la lame test pendant 3 minutes très exactement. 6. Après 3 minutes exactement, lire l’agglutination sous une source lumineuse. Note: Si la lecture du test est effectuée très au-delà de 3 minutes, la suspension de latex peut être asséchée et donner l’aspect d’une réaction faussement positive. Dans un tel cas, le test doit être réitérer. 7. Jeter la lame test et l’agitateur dans un conteneur prévu à cet effet – Ne pas réutiliser. Método semicuantitativo 1. Cuando se está realizando el procedimiento semicuantitativo, se recomienda testar también una serie de diluciones del Control Positivo. B. Test semi-quantitatif Les concentrations approximatives de XL-FDP contenant le domaine D-Dimère, pour les dilutions concernées, sont indiquées dans le tableau 1. Comme avec tout test semi-quantitatif, des variations de la réponse peuvent apparaître pour une même concentration. LIMITES DE LA TECHNIQUE Un diagnostic clinique ne peut se baser uniquement sur le résultat du DIMERTEST® Latex. Les informations cliniques et relatives à d’autres tests pertinents sont nécessaires au diagnostic et à la décision thérapeutique. VALEURS ATTENDUES Un résultat positif, indiquant une fibrinolyse active, peut être obtenu avec DIMERTEST® Latex quand la concentration de XL-FDP (D-dimères) est supérieure ou égale approximativement à 0,20 mg/l (200 ng/ml). Les spécimens de plasmas provenant de sujets normaux auront un résultat négatif, car les concentrations plasmatiques en XL-FDP sont typiquement inférieures à 0,20 mg/l (200 ng/ml). Du fait qu’un grand nombre de variables peuvent affecter les résultats, nous recommandons à chaque laboratoire d’établir ses propres valeurs normales. Des concentrations élevées de XL-FDP (contenant le domaine des D-Dimères) ont été observés chez des patients par une association de techniques d’immunoprécipitation et d’électrophorèse sur gel.1-3,5,6 Les techniques par anticorps monoclonaux (Mab)7 permettent maintenant une détection spécifique du domaine des D-Dimères. Les techniques de dosage des D-Dimères par anticorps monoclonaux ont une valeur diagnostique dans le cas des coagulations intravasculaire disséminées (CIVD) et des maladies vasculaires aiguës, incluant l’embolie pulmonaire (EP) et les thromboses veineuses profondes (TVP), conditions difficiles à détecter lors de l’examen clinique.8,9 La quantité de XL-FDP détectée dans un spécimen dépendra de différents facteurs in vivo, tels que, la sévérité de l’épisode thrombotique, le taux de formation de fibrine stable et le temps écoulé entre l’événement thrombotique et le prélèvement. Des concentrations élevées de XL-FDP, comme marqueur d’une fibrinolyse importante, ont été également trouvées dans des situations telles que, une chirurgie, un traumatisme, la Drépanocytose, les affections hépatiques, les infections sévères, les sepsis, l’inflammation et les cancers.10,11 . Les taux de D-Dimères sont aussi augmentés durant la grossesse normale mais des taux très élevés sont associés à des complications.12 Le test Latex DIMERTEST® n’a pas de réaction croisée avec le fibrinogène, la fibrine stabilisé par le facteur XIIIa,13 ou bien avec les produits de dégradation du fibrinogène.8 PERFORMANCES Des plasmas provenant de cent soixante dix (170) patients apparemment en bonne santé, donneurs volontaires, ont été testés avec le réactif Latex DIMERTEST®. Un résultat négatif a été obtenu pour cent soixante deux (162) d’entre eux, correspondant à une spécificité de 95,3% (162/170). Cent quarante cinq (145) plasmas de patients souffrant ou suspectés d’épisode thrombotique, ont été testés avec le réactif Latex DIMERTEST® ainsi qu’une autre méthode d’agglutination de référence. Le coefficient de corrélation était de 0,94 et l’équation de régression était y =1,19x. La reproductibilité intra-série (dans une série) a été déterminée sur 10 séries de tests effectuées sur 3 plasmas contenant différentes concentrations de XL-FDP. Les résultats étaient équivalents pour toutes les séries. La reproductibilité intersérie (de série à série) a été déterminée sur 10 plasmas de spécimens avec un titre de XL-FDP entre 1 et 16. Pour 10 séries, le réplicate de ces spécimens n’a pas varié de plus d’un titre. Dans une étude sur les anticoagulants portant en parallèle sur 50 plasmas citratés, EDTA et héparinés, la corrélation entre les titres obtenus avec le réactif Latex DIMERTEST® et les titres attendus (basés sur les valeurs XL-FDP ELISA) était r = 0,91 pour les plasmas citratés, r = 0,73 pour les spécimens EDTA et r = 0,78 pour les spécimens héparinés. Le citrate est donc l’anticoagulant de choix. Dans une étude portant sur des spécimens provenant de patients avec rhumatisme articulaire, 17 ont agglutiné avec le réactif Latex DIMERTEST®. Pour ces 17 spécimens, l’agglutination pouvait être inhibée par addition d’un anticorps monoclonal DD3B6/22 spécifique des D-Dimères, mais non inhibée avec un anticorps monoclonal non spécifique du même sous groupe, IgG3k. Ceci suggère que le réactif Latex DIMERTEST® est insensible aux perturbations dues au facteur rhumatoïde. Aucune interférence n’a été mise en évidence avec le réactif DIMERTEST® Latex lors d’une étude utilisant des plasmas surchargés avec des substances interférentes aux concentrations suivantes: Bilirubine 0,2 mg/ml (200 mg/l) Hémoglobine 5,0 mg/ml (5 g/l) Lipémie (triglycérides) 30 mg/ml (30 g/l) Protéine (gamma globuline) 0,06 g/ml Important ! • Amener les réactifs à température ambiante (20 - 25°C) avant utilisation. • Mélanger le réactif latex par inversion juste avant utilisation. • Essuyer l’embout des flacons compte-gouttes à l’aide d’un tissu avant chaque utilisation. • Les flacons compte-gouttes doivent être tenus verticalement lors de la distribution de réactifs. Pour une distribution correcte Assécher l’embout du flacon Método cualitativo 1. Marcar en la tarjeta de tests la Identificación de la muestra, así como los controles positivo y negativo. 2. Sostenga el frasco cuentagotas en posición vertical y ponga una gota del reactivo de látex en cada círculo de la tarjeta de test de reacción desechable. EVÍTESE tocar la superficie de la tarjeta. 3. Pipetee exactamente 20 μL del plasma a analizar sin diluir o añada una gota de solución control en el mismo círculo al lado de la gota de reactivo de látex. 4. Mezcle el látex y la muestra con un agitador de plástico hasta que el látex esté uniformemente distribuido. 5. Agite suavemente la tarjeta del test, durante 3 minutos exactamente. 6. Exactamente a los 3 minutos, compruebe si hay aglutinación, utilizando para ello una luz potente. Nota: Si en ese momento no se ha leído el test, la suspensión de látex puede secarse y mostrar una falsa aglutinación. Si se sospecha esta posibilidad, reanalizar la muestra. 7. Desechar la tarjeta de Test y el agitador en un contenedor de residuos de riesgo biológico. No reutilizar. CONTROL DE CALIDAD A. Test qualitatif Dans le cadre du protocole de test qualitatif, les indications suivantes peuvent être obtenues: Plasma non dilué D-Dimères (XL-FDP) Concentration CONTROLE DE QUALITE Il est recommandé que les deux contrôles positif et négatif soient inclus dans chaque série de tests afin de valider un fonctionnement correct du système. Ces solutions de contrôle devront être utilisées dans les mêmes conditions que les échantillons de patients. Pour la procédure de dépistage qualitatif, un résultat positif (agglutination) peut être obtenu en substituant le plasma par le contrôle positif DIMERTEST® dans les étapes 3 à 6 de la méthode qualitative. Inversement, un résultat négatif (pas d’agglutination) peut être obtenu en substituant le plasma par le contrôle négatif DIMERTEST® dans les étapes 3 à 6 de la méthode qualitative. Lors de la réalisation d’une procédure semi-quantitative, il est recommandé de tester une série de dilutions du contrôle positif.
