legal or beneficial interest or management control (direct or indirect) of or in any other Synergy Team Membership. This Group Addendum Form is
supplemental to the Contract as defined in the Membership Application Form and forms part of the agreement between the Member and the
Company relating to the Member's participation in the Synergy business opportunity. We hereby agree to be bound by the terms and conditions of
the Contract including the Policies and Procedures. We certify that each of the individuals named above are 18 years of age or older and are legally
able to enter into the Contract.
