Make your voice heard!
San Joaquin County is currently updating its Community Health Assessment, which
identifies the health-related needs and resources across the county. The goal is to have
a community-based process that seeks out and collects input from as many residents
as possible.
How You Can Help:
• Attend our Community Health Outreach Training to learn how to conduct
a focus group with your community members
• Promote and distribute the community health survey within your network

You’re invited!
Join us for the Community Health Outreach Training to learn how you can become
a Focus Group Champion! This is your opportunity to make sure that your community’s
voices are heard, and that their needs are identified and addressed through the planning
process.
During the training, you’ll be given step-by-step coaching and practical tips on how to
conduct a successful focus group and how to distribute the survey. You’ll also receive a
Focus Group Toolkit which includes the community health survey and additional details on
conducting community focus groups.

Campeón de Grupo de Enfoque! Este es su oportunidad de asegurar que las opiniones
de su comunidad están incluidas e asegurar que identificamos y respondemos a sus
necesidades durante el proceso de planificación.
