Section 8 Housing Choice Voucher Rental Assistance Lottery Entry Form
This entry form is for the lottery pool for our Section 8 Rental Assistance program in Canyon County.
You must return this form to our office (377 Cornell St., Middleton, Idaho 83644) by 3:30 pm on March
14, 2013 or have it postmarked no later than March 14, 2013. All entries received after these times will
not be accepted. All valid entries will be placed in a lottery pool for the purpose of selecting entries to
be placed on our Section 8 waiting list. Your entry will remain in the lottery pool for one year. We will,
from time to time during the ensuing year, advertise that we are drawing from the remaining lottery pool
to place entries on our waiting list. If you are selected, you will be notified by mail and will be invited
to complete an application for our program.
ONLY ONE ENTRY PER FAMILY IS ALLOWED – All other entries will be discarded.

14, Marzo, del 2013. Todas las entradas recibidas despues de esta fecha, seran rechazadas. Toda forma
de entrada valida sera puesta en la loteria con el proposito de selecionar las entradas para ser puestas en
la lista de espera de Seccion 8. De tiempo a tiempo, y durante el año, se anuciara en el periodico cuando
se llevara acabo un nuevo sorteo para abastecer nuestra lista de espera. Usted sera notificado por
correro, e invitado a llenar una aplicacion para nuestro programa si su aplicacion es elegida. Las
entradas se quedaran por un año.
