FORMULARIO DE SOLICITUD DE VISA VCA-4 DEBIDAMENTE
INTERESADO.(filled out visa aplication for CA-4 and signed)
2.
UNA FOTO RECIENTE, TAMAÑO PASAPORTE, FONDO BLANCO, DE FRENTE Y SIN GORROS, NI CUBRE
CABEZAS.(one recent passport size photograph with white background , no hat or head covered)
3.
COPIA DE LA PAGINA DE LOS DATOS DEL PASAPORTE, CON UNA VIGENCIA MINIMA DE SEIS
MESES.(copy of the first page of the Passport with at least six months validation)
4.
PRESENTAR BOLETO AEREO DE IDA Y VUELTA. (Round trip airfare ticket).
5.
RESIDENCIA O VISA AMERICANA. (Residency or U.S. Visa). PARA LOS CIUDADANOS DE LOS PAISES
CATEGORIA “C” DEBERAN PRESENTAR RESIDENCIA DE LOS ESTADOS UNIDOS DE AMERICA O DE
CUALQUIER PAIS CATEGORIA “A”.(citizens of countries in category “C” must prove U.S residency , or
residency in category “A” country)

REFERENCIAS BANCARIAS, ó SALARIALES, U OTRO TIPO DE INGRESO.
(Proof of income, bank statements)

1- FORMULARIO DE SOLICITUD DE VISA VCA-4 DEBIDAMENTE LLENADO Y FIRMADO POR EL
INTERESADO ( filled out visa application form CA-4 and signed )
2- DOS FOTOS,TAMAÑO PASAPORTE (Two Passport size photograph)
3- COPIA DE LA PAGINA DE LOS DATOS DEL PASAPORTE,CON UNA VIGENCIA MINIMA DE SEIS
MESES ( copy of the first page of the Passport with at least six months validation )
4- CARTA DE SOLICITUD DE VISA DEBIDAMENTE MOTIVADA ANTE LA REPRESENTACION
CONSULAR (visa application letter must mostrate interest in going to Nicaragua this will reviewed by the
consulate officer)
LA CARTA DEBE CONTENER: NOMBRES, APELLIDOS No DE IDENTIFICACION, EDAD, ESTADO
CIVIL, DOMICILIO, PROFESION U OFICIO, MOTIVO Y FECHA PREVISTA DEL VIAJE,DIRECCION No. DE
TELEFONO, Y REFERENCIA DE CONTACTO DEL LUGAR DONDE SE VA A HOSPEDAR EN
NICARAGUA, SEA HOTEL O CASA PARTICULAR, TIEMPO DE PERMANENCIA EN EL PAIS QUIEN
ASUMIRA LOS GASTOS DURANTE SU ESTADIA.( the letter must include: full name , identification number,
age, marital status, adress, profecion or trade, motive on inted , travel date, adress and phone number will be
cover the expensess during your stay)
5- CERTIFICADO DE ANTECEDENTES PENALES O POLICIALES DEL PAIS DE ORIGEN O
RESIDENCIA DE LOS ULTIMOS 3, AÑOS EN SU DEFECTO EL CERTIFICADO EL CERTIFICADO DE
INTERPOL EN NICARAGUA.( present pólice record for the last 3 years for the country or certificate of
the interpol in Nicaragua)
6- COMPROMISO DE MANUTENCION NOTARIZADA DEL SOLICITANTE.
(Proof of self sufificiency in country and notarized)
7- PRESENTAR BOLETO AEREO DE IDA Y VUELTA.

10- COPIA DE LAS CONSTANCIAS DE DGI, RUC Y MATRICULA DE LA ALCALDIA DEL LUGAR DONDE
SE ENCUENTRA UBICADO EL NEGOCIO. (copy of the enrollment record DGI, RUC, (must be
