STUART PLACE ELEMENTARY MEDICATION GUIDELINES
PARENTS: PLEASE SAVE THIS INFORMATION TO REVIEW WHEN YOUR CHILD
NEEDS TO TAKE PRESCRIPTION OR OVER THE COUNTER MEDICATION.
If at any time during the school year it becomes necessary for your child to be on
medication, the following guidelines must be followed:
1. For safety, an adult must bring the medication to the office in the morning
and an adult must pick up the medication at the end of the day. Children
will not be allowed to bring medication to school or take medication home
after school. Medications are an adult's responsibility. Medications are
NOT to be kept in the classroom or with the child. PLEASE HELP US
PROTECT YOUR CHILD!!
2. The medication must be in the original container whether it is a
prescription medication or an over the counter medication.
3. Any medication that is to be administered on a long term basis will require
written instructions from the prescribing physician.
4. Medication obtained outside the United States shall not be administered by
district employees to student unless written authorization is on file from a
physician licensed to practice in the United States.
5. Substances classified as dietary, natural remedies, or herbal medicines will
not be administered at school unless written authorization is on file from a
physician licensed to practice in the United States.

RECETADA POR UN MEDICO
Si es necesario que el estudiante tome medicinas durante las horas de escuela,
siga las siguientes reglas: Las enfermeras pueden administrar medicinas
SOLAMENTE SI:
1. Para la proteccion, todos los medicamentos deberan ser recogidos y
entregados por un adulto. Su hijo/a no podra traer y llevar medicamentos
de la escuela para la casa y viceversa. Los medicamentos son
responsabilidad del adulto. Las medicinas no deben llevarse a la clase ni
dejar que el estudiante las traiga con el/ella. ESTO SE HACE PARA LA
PROTECCION DEL ESTUDIANTE.
2. La medicina, recetada o no recetada, tiene que estar en su envase
original.
3. Medicinas que deben ser administrados por un termino largo requieren
instrucciones escritas por el medico que receto la medicina.
4. Medicinas obtenidas fuera de los Estados Unidos no seran administrada
por empleados del Distrito Escolar a los estudiantes a menos que haya
autorizacion escrito, en los archivos del Distrito Escolar, por un medico
con licencia para practicar medicina en los Estados Unidos.
5. Las substancias clasificadas como substancias dieteticas, remedios
naturales, o medicinas herbarias, no seran administradas en la escuela a
menos que hayas permiso en escrito, en los archivos del Distrito Escolar,
de un medico con licencia para practicar medicina en los Estados Unidos.
