in the LAST 21 DAYS?
*Guinea/Sierra Leone/Liberia
AND
Do you have any of the
following symptoms?
• Fever of greater than 100.4 F or 38.0 C
• Severe headache, muscle pain, diarrhea,
vomiting, abdominal pain or unexplained bleeding
• Close contact with someone who has traveled to
West Africa in the last 21 days
If you answered YES, notify the front desk IMMEDIATELY!
