Guiding You to Better Health
The staff from the MassHealth PCC Plan believe that you
would benefit from the Care Management Program (CMP).
This voluntary program provided by the PCC Plan and the
Massachusetts Behavioral Health Partnership (MBHP) will
help you or your caregiver learn more ways to care for your
health and improve the way you feel.
The CMP also provides you with unlimited access to a team of
caring, licensed medical professionals. Nurses and clinicians
are available 24 hours a day, 7 days a week. You don’t need
an appointment and you can call from the privacy and comfort
of home.
If you’re sick, hurt or have an urgent health question, call
toll-free 1-855-694-4382 for immediate assistance and advice
from a registered nurse. All calls are confidential.
If you’re having a life- or limb-threatening emergency,
call 911 or your local emergency services right away.

When you enroll in the program (at no charge to you), you will:
•Get help or answers to health questions between doctor’s
visits—without an appointment—from the CMP team.

•Learn more about your health and your health condition(s).
•Better understand and follow the treatment plan set by you
and your doctor.
•Learn more ways to care for your health.
•Have 24/7 access to a registered nurse for urgent health
