Please read this book carefully. This is your SoonerCare Choice Enrollment Guide and Provider Directory. As a SoonerCare Choice member, you will get an updated Member Handbook and Enrollment Guide/ Provider Directory in the mail every year. Make sure you tell your OKDHS worker your new address every time you move. What is SoonerCare Choice? SoonerCare Choice is a medical benefit plan featuring a medical home. It is run by the Oklahoma Health Care Authority (OHCA). As a SoonerCare Choice member, you can choose a SoonerCare Choice provider for yourself and each qualified member of your family. Your health care provider is called your Primary Care Provider (PCP). It’s easy! 1. STEP ONE: Read the SoonerCare Choice information. 2. STEP TWO: Decide which PCP is best for you and your family. If you do not choose a PCP, one will be chosen for you. 3. STEP THREE: Enroll. You are the only one who can enroll you and your family. Call the SoonerCare Helpline at 1-800-987-7767 to enroll. • Use the SoonerCare Choice Provider Directory to choose a PCP for each eligible member in your family. • You can use one PCP for the whole family, or each family member can have a different one. • The PCP you choose should be close to your home. • Please choose a PCP that serves your age group. You can call the SoonerCare Helpline for the most current listing. You can also look at the OHCA Web site, www.okhca.org PCPs can be Doctors, Physician Assistants, or Nurse Practitioners. American Indians who are eligible for Indian Health Services (IHS) must choose a SoonerCare Choice PCP. Many IHS, Tribal and Urban Indian Clinics are PCPs. These providers are listed in a special American Indian Provider section of the SoonerCare Choice Provider Directory. You can choose a health care provider you have seen before if he/she is in the Provider Directory or has signed up for SoonerCare Choice. If the provider you are seeing now is NOT in SoonerCare Choice, you will have to choose a different PCP from the directory. If the PCP you choose is not taking any more patients, you will be asked to make another choice or be placed with a different PCP. Please do not choose a PCP who has dismissed or dropped you from his/her practice. The Oklahoma Department of Human Services (OKDHS) decides if you qualify for SoonerCare. If you do, you will get a letter telling you when your SoonerCare benefits begin and end. You will also get a plastic Oklahoma Health Care Authority Medical I.D. Card. You will get your care in the SoonerCare Traditional (Fee-forService) program until you become a member of SoonerCare Choice. After you become a SoonerCare Choice member, you will get a Member Handbook in the mail. Your handbook will tell you about your benefits. There are some limits on health care services and some co-payments. It is important that you read all of your SoonerCare information. You can call the SoonerCare Helpline at 1800-987-7767 if you have questions. What if I don’t choose a PCP within two (2) weeks of applying for SoonerCare Choice? • SoonerCare Choice will choose a PCP for you and your family. You will get a letter that tells you which PCP was chosen for you. If you do not want this PCP, call the SoonerCare Helpline. • Keep your address current so we can reach you with news about SoonerCare, your PCP and your benefits. After I am in the SoonerCare Choice program, how do I change my PCP? • You may change your PCP four (4) times each year. • The change will not go into effect right away. Ask the SoonerCare Helpline when you can visit your new PCP. • You must go to your current PCP until the change happens. • You can call the SoonerCare Helpline to find out the name of your PCP. Call your OKDHS worker. • You will get a renewal notice, called the Benefit Review Form, in the mail each year from your local OKDHS office. • You need to return this form on time to keep your SoonerCare benefits. • If you do not return this form on time, your SoonerCare benefits will stop. How to Use the Provider Directory This directory has a list of Primary Care Providers (PCPs). PCPs are listed by city. American Indian PCPs are listed in a special section of the directory. Look for a city near you and choose a PCP. To find a provider by name, an index in the back of the Provider Directory lists PCPs in last name order. A PCP may have more than one office. To find out if a PCP has more than one office, look in the back of the Provider Directory. Choose a PCP in a city near you to get your health care. The SoonerCare Helpline has the most current list of providers. They can tell you if a provider is part of the program but not listed in the Provider Directory. IMPORTANT! You must choose a PCP. If you don’t, one will be chosen for you.
