RESUMEN Los tumores mesenquimatosos de partes blandas son entidades poco frecuentes como causa de tumores perineales y entre ellos, el Angiomixoma Perineal Agresivo es más raro aún. Afecta preferencialmente a mujeres en edad reproductiva y para su adecuado manejo se requiere una alta sospecha clínica, imágenes que permitan evaluar su localización y extención y finalmente una excéresis quirúrgica radical. Presentamos a continuación el caso de una paciente de 37 años de edad con un Angiomixoma Vulvar Agresivo que fue tratada en nuestro servicio. ABSTRACT The soft tissue benign mesenchymal tumor is a very rare perineal tumor and the perineal and vulvar angiomyxoma is even rarer. Most often found in women in reproductive age. It is required a high clinical suspicion, images to evaluate localization and extension and radical surgical treatment for a correct management of the disease. Below we present the case of a 37 year old with Vulvar Aggressive angiomyxoma was treated in our service. INTRODUCCIÓN: El Angiomixoma vulvar y perineal es una tumoración muy infrecuente, existen aproximadamente 150 casos publicados en todo el mundo, predominando en mujeres en edad fértil. Se trata de un tumor de estirpe benigno de tejidos blandos de origen mesenquimatoso que se comporta en forma localmente agresiva por su rápida infiltración y compresión de estructuras pelvianas, lo que obliga a realizar amplias resecciones a los fines de evitar su alta tasa de recidiva. CASO: Mujer de 37 años de edad, nulípara, que consulta en forma ambulatoria por tumoración perineal derecha de rápido crecimiento. Refiere cuadro de 6 meses de evolución de aparición de tumoración de forma ovalada localizada en proyección de labio mayor derecho de la vulva, en su inicio de pequeño tamaño, llegando a medir al Fig.1 momento de la consulta 20 cm de diámetro aproximadamente. Al examen físico (Fig. 1) se observa y se palpa tumoración de forma ovalada, de 15 x 20 cm de diámetro, límites netos, consistencia duro-elástica, indolora, irreductible a la cavidad pelviana y que Pag. 46 INTRODUCTION: The perineal and vulvar angiomyxoma is a very rare tumor with about 150 reported cases all over the world, most often found in women in reproductive age. It is a soft tissue benign mesenchymal tumor with a locally aggressive behavior shown in its fast infiltration and pressure on adjacent pelvic structures which requires large resections in order to prevent its high recurrence rate. CASE: A nulliparous 37-year-old woman consulted the outpatient service due to a fast-growing right perineal tumor. She stated that the signs and symptoms had been present for the past 6 months since the appearance of an oval-shaped tumor placed in projection of right labia majora, which was initially small and at the time of consultation was 20 cm of diameter approximately. On physical examination (Fig 1), a painless well-defined oval-shaped tumor of 15 x 20 cm of diameter, hard-elastic consistency, irreducible to the pelvic cavity and which was not propelled by efforts was visible and palpable. Se decide solicitar los siguientes estudios complementarios: ECOGRAFÍA DE TUMORACION: (Fig. 2) Se observa contenido ecogénico discretamente Fig 2 heterogéneo con presencia de elementos vasculares sin lograr definir etiología. ECOGRAFÍA TRANS VAGINAL: Útero desplazado hacia la izquierda, impresiona de tamaño, morfología y estructura conservados. Endometrio mide 7 mm. No se logran definir lesiones anexiales evidentes. TAC DE ABDOMEN Y PELVIS: (Fig. 3) Masa heterogénea expansiva ocupante de espacio en proyección a región abdomino-pelviana derecha protruyendo por fuera de la pared vaginal hacia la zona perineal. No puede identificarse órgano de origen, Se decide realizar excéresis radical por vía perineal. En el acto quirúrgico se constata masa adyacente a la vagina, que desplaza vagina, útero y vejiga hacia la izquierda, ocupando la pelvis sin pasar a la cavidad peritoneal. (Fig 4) La masa compromete los grupos musculares del piso pelviano y aparato esfinteriano peria- Fig 4 nal, por lo que se decide realizar colostomía sigmoidea terminal. Pieza quirúrgica de 29 x 20 x4,5 cm y peso de 1.370 kg de aspecto blanco nacarado. (Fig 5) La anatomía patológica informa angiomixoma agresivo. (Fig 6) La paciente cursa buena evolución postoperatoria, sin recidiva a los 12 meses de la cirugía. No se logró la recuperación del tono del esfínter anal, por lo que persiste colostomizada. DISCUSIÓN: El angiomixoma vulvar a pesar de ser una patología infrecuente, debe sospecharse ante lesiones mesenquimatosas de aspecto mixoide en el peri- TRANSVAGINAL ULTRASOUND: Uterus displaced to the left. Its size, morphology and structures appeared to be normal. Endometrium was 7 mm. The presence of evident injuries could not be determined. ABDOMEN AND PELVIS CTA: (Fig 3) Expansive heterogeneous mass placed in projection to right abdominopelvic region protruding from outside of the vaginal wall to the perineal area. The organ of origin could not be identified. Radical perineal excision was the treatment of choice. Surgery revealed mass adjacent to vagina, displacing vagina, uterus and bladder to the left, placed within the pelvis Fig 3 without reaching the peritoneal cavity. (Fig 4) As the mass was affecting the muscle groups of pelvic floor and perianal sphincter complex, the decision of performing a terminal sigmoid colostomy was taken. The sample obtained was of pearl-white aspect, 29 x 20 x4.5 cm and weighted 1,370 kg. (Fig 5) The pathologic anatomy reported an aggressive angiomyxoma. (Fig 6) The patient shows a good post-operative evolution, without recurrence 12 months after the surgery. Anal sphincter tone has not been recovered; therefore, she continues with the colostomy. DISCUSSION: Although the vulvar angiomyxoma is a rare pathology, it must be suspected whenever there are mesenchymal injuries of myxoid aspect in the perineum or pelvic floor, both in men and women. While imaging diagnostic methods may not be specific in relation to the tumor origin, they provide valuable information as to the tumor extension within the pelvic cavity. Cross-sectional studies (CT multislice – MR) are the preferred ones for that purpose and to differentiate it from other causes of perineal region tumor. Los métodos de diagnóstico por imágenes, si bien pueden ser inespecíficos en cuanto al origen de la tumoración, aportan información valorable sobre la extensión de la misma dentro de la cavidad pelviana. Especialmente los estudios transversales (TC multislice – RM), son de elección para tal fin y para diferenciarlo de otras causas de tumoraciones de la región perineal. Entre los diagnósticos diferenciales debemos considerar abscesos, quistes y neoplasias de glándulas de Bartholín, patologías anorrectales, hernias perineales, y otros tumores benignos y malignos de partes blandas. Si bien es un tumor benigno, tiene alto índice de recidiva local por lo que se recomienda tratamiento quirúrgico agresivo con márgenes amplios. Sin embargo, debido a sus límites imprecisos la cirugía radical es muy dificultosa. Frecuentemente infiltra los planos musculares del piso pelviano, con lo cual la excéresis conlleva a resecar amplias superficies del periné, con las consecuentes secuelas estéticas y funcionales que ello implica, sin lograr descartar por completo la posibilidad de recidiva local. La radio y quimioterapia tienen escasa utilidad, ya que presenta un escaso índice mitótico y no existen estudios que avalen esta terapéutica. BIBLIOGRAFÍA: tissue tumors. Even though it is a benign tumor, it has a high rate of local recurrence. Therefore, aggressive surgical treatment with wide margins is recommended. However, due to Fig 6 its imprecise limits, radical surgery is very difficult. The tumor frequently infiltrates muscle planes of the pelvic floor and, therefore, excision leads to resecting wide surfaces of perineum, with the corresponding aesthetic and functional after-effects, and without being able to completely disregard local recurrence. Radiotherapy and chemotherapy are not very useful due to their low mitotic rate and no studies guarantee said treatment. 1 Steeper TA, Rosai J.
