La emergencia de pandemias aunadas al creciente fenómeno de la migración ha hecho de la salud un tema que requiere respuestas intersectoriales y de carácter global. En la nueva arquitectura global, que cuenta con una amplia diversidad de actores gubernamentales y no gubernamentales, es preciso fortalecer la capacidad de innovación con nuevos esquemas de cooperación internacional en salud pública que atiendan los retos de la agenda global. La migración y la salud son una dimensión crítica del proceso referido en la que el gobierno mexicano ha emprendido acciones significativas, mediante políticas públicas innovadoras, con el fin de mejorar las condiciones de vida de los mexicanos en el exterior. Hemos trabajado de manera coordinada con la Secretaría de Relaciones Exteriores y la Embajada de México en los Estados Unidos de América en la construcción de un sólido entramado de 50 Ventanillas de Salud –una en cada consulado–, en el que se sustenta la promoción, prevención y atención de la salud de la comunidad migrante mexicana en aquel país. El programa de Ventanillas de Salud representa un esquema innovador de colaboración que se encuentra cofinanciado con recursos del gobierno federal mexicano, y en el que participan los consulados en Estados Unidos, así como proveedores locales de servicios de salud y clínicas comunitarias enfocadas en la atención a migrantes. En torno a la red de Ventanillas de Salud opera un conjunto de más de 500 agencias estadounidenses, públicas y privadas, que a través de alianzas locales permiten llevar a cabo acciones para mejorar la salud de los mexicanos que residen en los Estados Unidos de América. El programa se ha convertido en el principal difusor de acciones de prevención y promoción de la salud en Estados Unidos, con el propósito de garantizar el derecho constitucional a la salud a todo mexicano, sin importar el territorio donde se encuentre. 9 Prólogo Las Ventanillas de Salud se ubican en todos los consulados de México, que diariamente son visitados por miles de personas, muchas de ellas carentes de seguridad social (incluidos los servicios de salud) debido a su condición de migrantes y a las actividades laborales que en su mayoría realizan (la construcción, la agricultura y la industria manufacturera), las cuales se encuentran estadísticamente dentro de las más riesgosas para la salud. Durante los periodos de trámites en los consulados, las Ventanillas de Salud ofrecen a las familias mexicanas servicios como pruebas para medir la presión arterial y la glucosa, análisis de VIH/SIDA, revisiones para detectar el cáncer de seno, dotación de ciertas vacunas o, en caso necesario, la canalización a centros comunitarios de salud en la Unión Americana, sin importar la condición migratoria de los beneficiarios. La información y atención en las ventanillas están concentradas en los padecimientos de mayor incidencia de la población mexicana en Estados Unidos, que son enfermedades crónicas no transmisibles, derivadas de factores de riesgo como la obesidad, que se presenta en nuestros connacionales debido, sobre todo, a graves trastornos metabólicos por el cambio de dieta y estilo de vida. Para casos críticos, el programa ofrece referirlos, de acuerdo con su padecimiento, a centros de salud federales en la ciudad de México u hospitales de las entidades federativas, con la finalidad de brindarles la atención médica requerida en el menor tiempo posible. Asimismo, las Ventanillas de Salud han puesto en marcha programas preventivos y de consejería psicológica para reforzar factores de protección contra el consumo de drogas entre los jóvenes en edad escolar, tanto en las poblaciones con alto índice de migración como en las ciudades de destino en los Estados Unidos. Además cuentan con una plataforma electrónica para brindar a los migrantes la posibilidad de preafiliación al Seguro Popular desde el lugar donde se encuentren, y procurar que los miembros de su familia que permanezcan en México reciban servicios de salud en su lugar de origen. Gracias a estos servicios se está logrando un empoderamiento in situ de las comunidades mexicanas en Estados Unidos, en donde las prioridades locales se definen en cada ciudad, respondiendo con flexibilidad a sus necesidades específicas. El fin principal es aumentar los conocimientos de salud preventiva de que disponen las familias mexicanas en ambos lados de la frontera y promover su uso oportuno y efectivo. 10 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias La amplia participación de agencias ha consolidado una sólida red de salud preventiva para nuestras comunidades en el exterior, lo cual ha permitido una creciente cobertura de atención, que se ha visto enriquecida con las Ventanillas de Salud Móviles, que permiten acercar servicios de salud a trabajadores agrícolas en lugares distantes. Las ventanillas constituyen una prueba de la responsabilidad compartida que el gobierno de México asume para procurar el bienestar de los migrantes en Estados Unidos. Aún queda mucho por hacer. México y Estados Unidos requieren dar continuidad y mayor profundidad a los programas innovadores y efectivos que se han establecido, como las Ventanillas de Salud, y, sobre todo, precisan sumar esfuerzos bilaterales para seguir garantizando el derecho a la salud de su población. En esa dirección apunta el establecimiento de un programa piloto de Ventanillas de Segunda Generación, con el cual, entre otras acciones innovadoras, se busca la participación de los gobiernos de las entidades de la república mexicana donde se origina la migración. Esta publicación relata la historia de las Ventanillas de Salud y plantea el reto que tenemos por delante para la institucionalización de este modelo de atención con imaginación y compromiso, con el fin de contribuir a elevar la calidad de vida de nuestros connacionales que viven en el exterior, extendiendo los brazos generosos del gobierno mexicano y de sus asociados en los Estados Unidos de América. Ello considera las externalidades positivas de nuestras acciones para la sociedad de ese país en su conjunto. Salomón Chertorivski Woldenberg Secretario de Salud 11 Antecedentes E l permanente flujo de mexicanos a Estados Unidos y el creciente número de inmigrantes indocumentados pone de relieve la situación de vulnerabilidad a la que éstos están expuestos en el país vecino, dado que las condiciones climatológicas extremas y de seguridad pública pueden convertirse en riesgos potenciales que influyen en la situación de salud de los migrantes. En ocasiones, el viaje sin documentos hacia el norte del continente, más que una oportunidad, podría convertirse en un riesgo permanente por las condiciones a los que están expuestos los migrantes en todo el trayecto desde su partida del lugar de origen hasta su estancia en Estados Unidos. Dichas condiciones pueden afectar su salud, la cual constituye un eje prioritario para el pleno desarrollo de las potencialidades físicas, sociales y culturales del migrante y su familia, poniendo en riesgo factores particulares e identificables para la salud pública. De acuerdo con el censo poblacional de los Estados Unidos, en 2008, la población de origen mexicano tenía el porcentaje más alto (34.6%) de personas sin seguro médico viviendo en los Estados Unidos (U.S. Census Bureau, 2008). En particular, destaca el hecho de que casi la mitad (48.3%) de las personas de origen mexicano y 61% de los nacidos en México que se encuentran en el grupo de edad más productivo (de 18 a 64 años) reportan no contar con seguro médico. El acceso a seguros y servicios es limitado inclusive en el grupo de menores de 18 años. Por ejemplo, en la encuesta de necesidades de salud, realizada en el Consulado General de México en Los Ángeles (2004), sobresale el hecho de que 49% de los niños mexicanos nacidos en los Estados Unidos no tenían seguro médico, aun cuando el estado de California contaba con un programa de cobertura de seguro gratuito para niños de bajo ingreso entre 0 y 18 años de edad. Esta cifra refleja claramente la subutilización de servicios de salud de estas familias. La población objetivo de este programa son los mexicanos que viven en Estados Unidos y sus familiares. En 2010 se estimaron alrededor de 31.8 millones de personas de origen mexicano en Estados Unidos. El potencial de 13 Antecedentes alcance de las ventanillas es muy significativo, ya que la red consular atiende a un promedio de 1 500 000 personas al año a través de sus 50 Ventanillas de Salud operando en toda la red consular y a través de dos Ventanillas de Salud Móviles en los Estados Unidos. Ante este panorama, la Secretaría de Salud (2008) propone el Programa de Acción Específico 2007-2012. Salud del Migrante, que busca promover y fomentar la salud de los mexicanos en los Estados Unidos de América. Para la implementación de dicho programa se tuvo en cuenta el siguiente marco institucional, relacionado con los fundamentos jurídicos. El nivel general se enmarca en el artículo 4º de la Constitución Política de los Estados Unidos Mexicanos (H. Congreso de la Unión, 2012), en el que se estipula que “toda persona tiene derecho a la protección de la salud”. Asimismo, en lo particular, los artículos 3 y 13 de la Ley General de Salud (H. Congreso de la Unión, 2010) resaltan las obligaciones de la citada secretaría para el mantenimiento de la salud de los mexicanos. De manera específica, se consideró el “Reglamento interior de la Secretaría de Salud” (Ejecutivo Federal, 2011), en el que se incluyen las competencias, objetivos y acciones correspondientes a la Dirección General de Relaciones Internacionales de dicha secretaría y sus funciones propias relativas al quehacer para el mejoramiento de la salud de los connacionales en el exterior. Otro vínculo importante que tiene el Programa de Acción Específico 2007-2012. Salud del Migrante (Secretaría de Salud, 2008) es con el Plan Nacional de Desarrollo 2007-2012 (Gobierno de los Estados Unidos Mexicanos, 2007), en el que se establecen los objetivos nacionales, las estrategias y las prioridades que deben regir el accionar del gobierno en turno. Así, dicho programa está dirigido a promover los medios para la preservación y consecución de la salud del migrante, tanto de aquellos que llegan a México como la de los inmigrantes nacionales en la región de América del Norte, de tal forma que se cuente con un ambiente favorable para la prevención y atención a la salud que permita el desarrollo individual y social, así como el aprovechamiento de oportunidades para el beneficio del migrante y su familia. Finalmente, dentro de este marco institucional se ubica el Programa Sectorial de Salud 2007-2012 (Prosesa), el cual tiene como finalidad atender las necesidades de salud de los inmigrantes mexicanos, principalmente en América del Norte, y de aquellos que se internan en el territorio nacional. En su estrategia 4.9 indica la implementación de un programa para la salud 14 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias del migrante en el que se pueda “aprovechar la cooperación internacional en salud para contribuir a lograr los objetivos del sector”. Las acciones específicas propuestas por el Prosesa son las siguientes: • Fortalecer la colaboración y cooperación con las comisiones de Salud Fronteriza México-Estados Unidos y México-Guatemala en los temas de vigilancia y control epidemiológico. • Robustecer los vínculos multilaterales y bilaterales para ofrecer y recibir cooperación internacional para cumplir los objetivos del Prosesa 2007-2012. • Consolidar la organización y desarrollo de las semanas binacionales de salud, que han ampliado el acceso de la población migrante a los servicios de salud en los Estados Unidos. • Promover la afiliación de familias de migrantes al Seguro Popular, aprovechando las Ventanillas de Salud en los consulados en los Estados Unidos. • Mejorar la atención otorgada a los connacionales enfermos que tengan la necesidad de ser repatriados. • Fortalecer la participación activa de México en los principales organismos internacionales de salud, en particular en la Organización Mundial de la Salud (OMS) y la Organización Panamericana de la Salud (OPS). Con las anteriores acciones, el Programa de Acción Específico 2007-2012. Salud del Migrante (Secretaría de Salud, 2008) se convierte en un elemento clave para la consecución de los objetivos y el cumplimiento de metas establecidos en el Programa Sectorial de Salud 2007-2012. Salud del Migrante Misión Proteger la salud de los migrantes y sus familias en su lugar de destino mediante el establecimiento de estrategias específicas de colaboración binacional para la promoción y atención de la salud con sensibilidad cultural. 15 Antecedentes Visión Los migrantes mexicanos y sus familias cuentan con información oportuna sobre la promoción de su salud y la prevención de enfermedades, y en sus comunidades tienen acceso a servicios de salud con calidad para lograr el pleno desarrollo de sus potencialidades físicas, mentales y sociales. Objetivo general Garantizar el acceso a los servicios de salud y proteger la salud de los migrantes a través de acciones binacionales en sus lugares de origen, tránsito y destino. Objetivos específicos • Coordinar, apoyar y dar seguimiento a los diferentes programas de la administración pública federal que en materia de salud se realicen en favor de la población migrante mexicana. • Promover estrategias de comunicación e información en educación y promoción de la salud dirigidas a la población migrante mexicana con sensibilidad cultural. • Impulsar y fortalecer la vinculación binacional entre prestadores de servicios de salud, a fin de incrementar la accesibilidad a la atención médica de la población migrante. • Propiciar y facilitar la gestión de acuerdos y convenios binacionales con organizaciones gubernamentales y no gubernamentales, así como con instituciones académicas, que tengan como propósito promover la salud y mejorar el acceso y la calidad de los servicios de salud dirigidos a la población migrante mexicana. • Facilitar la atención médica y hospitalaria de connacionales enfermos en ambos lados de la frontera. • Fomentar la realización de investigaciones relacionadas con la salud del migrante. 16 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias Fundamentosjurídicos xConstituciónPolíticadelos EstadosUnidosMexicanos (art.4º) xLeyGeneraldeSalud(arts. 3y13) x“ReglamentoInteriordela SecretaríadeSalud” PlanNacional deDesarrollo 2007Ͳ2012 ProgramaSectorial deSalud2007Ͳ2012 (Prosesa). Comisión de Salud Fronteriza México-Estados Unidos (CSFMEU). Ventanillas de Salud Las Ventanillas de Salud concretan la estrategia 7 del Programa de Acción Específico 2007-2012. Salud del Migrante (Secretaría de Salud, 2008), 17 Antecedentes mediante la cual se pretende fomentar y consolidar la atención relacionada con la salud de la población migrante. Además es una contribución del gobierno mexicano, consistente en los servicios de prevención a sus connacionales en Estados Unidos. La creación de las VDS tiene como finalidad proporcionar información y educación en salud a los migrantes mexicanos en Estados Unidos, con el objetivo de facilitar y mejorar el acceso a los servicios de salud disponibles para dicha población y compatibilizar el conocimiento que ambos sistemas de salud –el mexicano y el estadounidense– tienen acerca de un mismo paciente. Dentro de las líneas de acción consideradas en el Programa de Acción Específico 2007-2012. Salud del Migrante (Secretaría de Salud, 2008) para esta estrategia, se resaltan las siguientes: 1. Consolidar el programa estratégico de Ventanillas de Salud, en coordinación con el Instituto de los Mexicanos en el Exterior, dependencia de la Secretaría de Relaciones Exteriores. 2. Incrementar los recursos financieros con el fin de poder operar este proyecto en todos los consulados de los Estados Unidos. 3. Supervisar y evaluar los servicios en los consulados que están en funcionamiento. 4. Conducir y promocionar el modelo VDS en todos los consulados en Estados Unidos. 5. Apertura de 25 Ventanillas de Salud en consulados de Estados Unidos. Descripción del Programa Ventanillas de Salud A partir del año 2000, el gobierno de México emprende una serie de acciones para mejorar las condiciones de vida de los mexicanos en el exterior. De esta manera, tres años después, mediante su representación consular en San Diego, California, y como una iniciativa de buena voluntad, se pone a prueba el programa Ventanillas de Salud como Inauguración de la primera Ventanilla de Salud. un centro para brindar informaSan Diego, California, 2003. ción en esta materia.1 El término Ventanilla de Salud deriva de la propuesta inicial de fungir como un canal de interlocución entre el migrante, el promotor de salud y el Consulado para proporcionar orientación e información en materia de salud. 1 18 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias Con el apoyo de la Comisión de Salud Fronteriza México-Estados Unidos (CSFMEU)2 y la Iniciativa de Salud México-Estados Unidos (hoy conocida como la Iniciativa de Salud Medición de la presión arterial a usuaria de las Ventanillas de Salud. de las Américas), de la Universidad de California y The California Endowment,3 se aprueba una subServicio de inmunización en las Ventanillas de Salud. vención para una fase piloto en dicho consulado. El Condado de San Diego apoya el Programa de Acción Específico 20072012. Salud del Migrante (Secretaría de Salud, 2008), proporcionando educación básica en salud, así como la realización de actividades de prevención y promoción de la salud a los migrantes mexicanos con el objeto de que cuenten con información oportuna en esta materia, teniendo acceso a servicios básicos con calidad para lograr el pleno desarrollo de sus potencialidades físicas, mentales y sociales. La Ventanilla de Salud es un programa del gobierno de México desarrollado por la Secretaría de Salud y la Secretaría de Relaciones Exteriores e instrumentado a través de los 50 consulados de México en los Estados Unidos y por diversas organizaciones de salud locales. Este programa provee información confiable sobre prevención de la salud, asesorías y referencias La Comisión de Salud Fronteriza México-Estados Unidos (CSFMEU) es un organismo binacional de la Secretaría de Salud dedicado a mejorar la salud de los fronterizos, siendo un espacio de encuentro entre los responsables estatales de salud y personalidades de la comunidad con sus contrapartes federales para compartir información tendiente a crear acuerdos sobre temas específicos de salud pública con los Estados Unidos. La CSFMEU cuenta con seis oficinas regionales a lo largo de la frontera de México con Estados Unidos (http://www.saludfronteriza.org.mx/). 3 The California Endowment es una institución con sede en Los Ángeles que busca ampliar el acceso a la atención de salud con calidad para los individuos y las comunidades, promoviendo mejoras fundamentales para el estado de salud de todos los californianos. 2 19 Antecedentes a servicios de salud disponibles y accesibles en su comunidad. Además fue diseñado para mejorar el bienestar físico y mental de los mexicanos que viven en los Estados Unidos e incrementar su acceso a servicios primarios y preventivos de salud, cobertura de seguros médicos y procurar servicios culturalmente sensibles, a fin de reducir el uso de la atención de emergencia. Después de dos años de desarrollo, dos estudios de evaluación y un proceso de adaptación de la etapa piloto, el gobierno de México inició la expansión de este modelo en otros consulados y, en 2004, proporcionó “dinero semilla” para mantener y abrir ventanillas en otros consulados de México en los Estados Unidos. Anualmente, el gobierno mexicano apoya al programa con alrededor de dos millones de dólares en aportaciones monetarias y aproximadamente un millón de dólares en especie. Ubicación geográfica de las Ventanillas de Salud en los Estados Unidos 20 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias El programa Ventanillas de Salud ha tenido una aceptación y crecimiento extraordinario desde su fase piloto en los consulados de México en San Diego y Los Ángeles. Actualmente se encuentran operando y oficialmente inauguradas 50 Ventanillas de Salud. Hoy en día, este servicio está presente en toda la red consular de México en los Estados Unidos. Esta cobertura, aunada a la naturaleza culturalmente sensible, el clima de confianza que inspiran los consulados a los connacionales y la vinculación de múltiples organizaciones y de Inauguración de la Ventanilla de Salud número 50. San Bernardino, California, 2011. dos países hacen de este programa un excelente vehículo para la promoción, prevención de la salud y el control de enfermedades en un ámbito binacional que incluye a México y Estados Unidos. En la figura 2 se muestra la ubicación de las Ventanillas de Salud en cada uno de los estados de este país. Misión Mejorar el acceso a los servicios primarios y preventivos de salud, aumentar la cobertura en seguros públicos y promover una cultura de prevención en salud de los mexicanos y sus familias que viven en los Estados Unidos, a través de información, educación, asesorías y referencias de calidad en un ambiente seguro y amable, mediante la creación de colaboraciones locales y binacionales entre Estados Unidos y México. Visión Mejorar el bienestar físico y mental de los mexicanos que viven en los Estados Unidos, aumentando el acceso a los servicios primarios y preventivos, la cobertura de seguros públicos, la oferta y calidad de servicios culturalmente sensibles, reduciendo el uso de servicios de emergencia y estableciendo la red de ventanillas como un programa sede confiable y sustentable que brinde información, educación y referencia en materia de salud. 21 Antecedentes Funciones esenciales • Difundir programas de prevención y promoción de la salud. • Promover el acceso de los migrantes a centros comunitarios de salud en Estados Unidos a través de una red de 6 500 unidades médicas. • Difusión y preafiliación al Seguro Popular para los migrantes y sus familias. • Brindar a los mexicanos en los Estados Unidos la posibilidad de afiliar, desde su lugar de residencia, a los miembros de su familia y a ellos mismos al Seguro Popular para recibir servicios de salud en el territorio nacional. • Reducir el consumo de drogas entre los jóvenes en edad escolar, tanto en las poblaciones con alto índice de migración como en las ciudades de destino en los Estados Unidos, reforzando factores de Sesión de difusión de los servicios de salud protección para evitar el uso de en Estados Unidos. drogas, considerando los aspectos culturales propios de la familia mexicana. • Difundir información respecto de los servicios que ofrecen los centros comunitarios de salud. • Llevar a cabo acciones en áreas específicas, incluyendo temas relacionados con la migración y la salud, así como en áreas de investigación e intercambio académico. Promoviendo el acceso a servicios de salud a través de los centros comunitarios en Estados Unidos. Los Ángeles, California, 2011. 22 • Dar seguimiento, hasta su recuperación, a pacientes con graves problemas de salud. Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias • Establecer un programa preventivo en escuelas de nivel básico y medio, dirigido a migrantes potenciales. • Implementar un programa de intervención psicológica temprana en los centros de atención existentes en las ciudades fronterizas de cruce. Programa de Jóvenes voluntarios para crear una comunidad latina saludable. VDS Móvil, Kansas, City, 2012 • Poner en marcha un programa preventivo y de consejería psicológica para reforzar los factores de protección asociados al uso de drogas. Los contenidos de este programa son sensibles a la cultura y familia mexicanas a través de redes, asociaciones y clubes de mexicanos en los Estados Unidos. • Canalizar a los pacientes graves, de acuerdo con su padecimiento, a centros de salud federales en la ciudad de México u hospitales de las entidades federativas con la finalidad de brindarles la atención médica requerida en el menor tiempo posible. Información sobre temas de salud Los temas prioritarios sobre los cuales se entrega información a los connacionales están relacionados estrechamente con el perfil de morbilidad de quienes se desplazan al norte del continente. Los temas son los siguientes: • Abuso de sustancias (alcoholismo/adicción a drogas). • Artritis/reumatismo. • Asma/alergias. • Cáncer. • Cuidado prenatal. • Diabetes • Enfermedad cardiaca • Enfermedades de transmisión sexual (VIH/SIDA y otras). • Hipertensión arterial. • Influenza. • Nutrición y actividad física. • Obesidad, síndrome metabólico y colesterol. • Planeación familiar. • Problemas de espalda y cuello. 23 Antecedentes • Salud de la mujer. • Salud dental. • Salud mental (depresión, ansiedad y estrés). • Salud ocupacional (prevención de accidentes laborales). Promoviendo y realizando la prueba de detección del VIH. • Tabaquismo. • Tuberculosis. • Úlcera/gastritis. • Violencia doméstica. • Salud visual. Promoviendo hábitos saludables para la prevención de enfermedades crónicas. Reunión sobre los servicios que ofrecen las Ventanillas de Salud y los retos a enfrentar por éstas. Denver, Colorado, 2012. Impacto del programa Ventanillas de Salud A continuación se presentan algunos datos con los que se pretende resaltar el impacto que han tenido las Ventanillas de Salud en los connacionales en Estados Unidos. La fuente de información utilizada para el presente apartado es el “Reporte de indicadores del programa Ventanillas de Salud”, el cual presenta datos promedio de 40 VDS entre julio de 2011 y septiembre de 2012. • El total de usuarios atendidos en el periodo de referencia fue de 903 919. Es preciso mencionar que las cifras disponibles en cuanto a la población cubierta por cada una de las Ventanillas de Salud son diferentes de acuerdo con el tamaño de la población y del Consulado. Según un diagnóstico 24 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias realizado entre 2010 y 2011 por la consultora Saber es Poder, 40.4% de las VDS atendieron en promedio a más de 200 personas a la semana. Asimismo, semanalmente, 19% de las VDS brindaron el servicio a un número de entre 50 y 100 usuarios, y un porcentaje igual de las ventanillas atendieron entre 100 y 150 personas. Solamente tres VDS cubren en promedio de 150 a 200 usuarios a la semana. • El número total de servicios fue de 3 108 379, distribuidos de la siguiente manera: - De los servicios ofrecidos, 68% se concentraron en la orientación y consejería en temas específicos sobre problemas de salud que son prioritarios para la población latina que vive en Estados Unidos, tales como diabetes mellitus; obesidad; hipertensión arterial; algunos condicionantes de GRÁFICA 1. Número de ventanillas por periodo: 35 de julio a septiembre de 2011, 39 de octubre a diciembre de 2011, 48 de enero a marzo de 2012, 50 de abril a junio de 2012 y 52 de julio a septiembre de 2012. - Asimismo, 16% ayudaron a la detección oportuna de padecimientos como diabetes mellitus, obesidad, hipertensión arterial, colesterol, VIH/SIDA o infecciones de transmisión sexual (véase la gráfica 1). - Del total de los atendidos, 9% se refirieron a servicios de salud en Estados Unidos (véase la gráfica 1). - Otro 6% de las personas fueron atendidas en consulados móviles, en donde recibieron orientaciones, detecciones o referencias a algún tipo de servicio de salud (véase la gráfica 1). - El restante 1% recibió otros servicios como vacunas o inscripción en algún seguro de salud en Estados Unidos (véase la gráfica 1). • En promedio se otorgaron tres atenciones de orientación, prevención o referencia a algún servicio, por persona que acude a una ventanilla de salud disponible. 27 Antecedentes • Específicamente, para el tema de la diabetes mellitus se proporcionaron 195 664 orientaciones o consejerías y se realizaron 64 435 mediciones de glucosa en la sangre, de las cuales 15% presentaron niveles altos (véanse la gráfica 2 y el cuadro 1). • Para la prevención de la obesidad se realizaron 297 239 orientaciones o consejerías en temas específicos como el síndrome metabólico, niveles de colesterol, nutrición y actividad física. Se efectuaron 41 967 mediciones del índice de masa corporal, en las que se registró una prevalencia de 36% de sobrepeso y obesidad (véanse la gráfica 3 y el cuadro 1). • En el tema de la prevención de la hipertensión arterial, se atendió a 125 781 usuarios y se les proporcionó orientación y consejería al respecto. A 84 171 personas se les midió la presión arterial y a 20% se les detectó presión alta (véanse la gráfica 4 y el cuadro 1). GRÁFICA 4. Se realizaron 589 pruebas de detección. En cuanto a las enfermedades del corazón, se brindaron 60 290 orientaciones y se registró una prevalencia de 22% entre aquellas personas a las que se les realizó algún examen de este órgano. • En temas específicos como VIH/SIDA e infecciones de transmisión sexual, se tomaron pruebas a 8 522 personas para el primero y 2 443 para las segundas. Es importante resaltar que 1% de estas pruebas resultaron positivas para el VIH, y 2% para otras infecciones de transmisión sexual (véanse la gráfica 5 y el cuadro 1). GRÁFICA 5. Número de ventanillas por periodo: 35 de julio a septiembre de 2011, 39 de octubre a diciembre de 2011, 48 de enero a marzo de 2012, 50 de abril a junio de 2012 y 52 de julio a septiembre de 2012. • En cuanto a la prevención de la tuberculosis, se orientó a 9 270 usuarios, de los cuales a 282 se les realizaron pruebas de detección, que arrojaron una prevalencia de 3% de esta enfermedad (véase el cuadro 1). 29 Antecedentes • Se brindó orientación y consejería a 311 378 mujeres sobre temas relacionados con su salud. Así, 43% de la atención correspondió a cuestiones específicas sobre su salud; una cuarta parte de ellas recibieron información sobre planificación familiar; 20% se relacionó con temas de violencia doméstica y 11% restante recibió información sobre control prenatal (véase la gráfica 6). Número de ventanillas por periodo: 35 de julio a septiembre de 2011, 39 de octubre a diciembre de 2011, 48 de enero a marzo de 2012, 50 de abril a junio de 2012 y 52 de julio a septiembre de 2012. • En cuanto a la salud mental y bucal se brindaron orientación y consejería a 67 570 y 63 307 personas, respectivamente, de las cuales 15% se remitieron para recibir servicios de segundo y tercer nivel de atención relacionados con salud mental, y 84% para tratamientos odontológicos especializados. • En materia de prevención del abuso en el consumo de sustancias psicoactivas, se refirió a 18% de las 73 053 personas que recibieron orientación (véase la gráfica 7). Asimismo se orientó a 48 854 usuarios en temas re- 30 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias lacionados con el tabaquismo y la prevención del asma y las alergias, y a 0.2% de ellos se les practicaron exámenes de detección del asma. • Cabe resaltar que en las VDS también se proporcionó orientación a 35 089 migrantes sobre la prevención de la influenza, y aproximadamente la mitad de ellos fueron vacunados. Para otro tipo de enfermedades se aplicaron 7 560 vacunas, de las cuales 63% las recibieron los adultos, y 37%, la población infantil. • Para la promoción de la salud ocupacional se orientó a 18 934 usuarios. En los programas de asistencia social se brindó orientación a 107 325 personas, de las cuales 22% recibieron información sobre asistencia pública, 13% fueron beneficiadas con servicios de transporte, 12% contaron con asistencia financiera, 10% con asistencia en discapacidad, y el porcentaje restante recibió asistencia en otros temas. Por otro lado, 80 723 personas recibieron orientación en programas de asistencia social; 65% obtuvieron descuento en medicinas; 17% correspondió a la referencia de mujeres, infantes y niños; 3% recibió asistencia médico-legal en deuda de servicios; igual porcentaje participó en el programa de entrega de desayunos o almuerzos, y 12% restante recibió otro tipo de servicios de asistencia social (véase la gráfica 8). Número de ventanillas por periodo: 35 de julio a septiembre de 2011, 39 de octubre a diciembre de 2011, 48 de enero a marzo de 2012, 50 de abril a junio de 2012 y 52 de julio a septiembre de 2012. • Finalmente, 6.02% de las 184 477 personas informadas se preafiliaron al Seguro Popular; 17.47% de los 29 823 adultos informados se inscribieron en algún seguro público en Estados Unidos, y 29.14% de los 19 595 menores de 18 años orientados se inscribieron en algún seguro público de salud. 32 Iniciativas para el fortalecimiento del programa D ados los resultados obtenidos con la implementación de las 50 Ventanillas de Salud (VDS) por parte de la Secretaría de Salud, la Secretaría de Relaciones Exteriores y el Instituto de Mexicanos en el Exterior, fue necesario desarrollar nuevas iniciativas que permitieran el fortalecimiento de los avances que se han logrado hasta el momento. A continuación se describen tales iniciativas. Proyecto Segunda Generación de Ventanillas de Salud Este proyecto se creó con el fin de fortalecer el programa de VDS en su segunda fase y mejorar el acceso a los servicios de salud de los migrantes en Estados Unidos, en colaboración con diversas iniciativas de salud y a través de la implementación de actividades de capacitación, difusión, acceso a servicios preventivos y asistencia técnica. En marzo de 2012 se implementa el proyecto Segunda Generación de las VDS, cuyos objetivos son los siguientes: • Atención especializada a través de un concepto global que permita uniformar criterios y conceptos en temas prioritarios, tales como cáncer, obesidad y diabetes, salud ocupacional, salud mental, adicciones, salud infantil, VIH /SIDA, tuberculosis y acceso a los servicios. • Capacitación al personal de las Ventanillas de Salud sobre la reforma de salud de Estados Unidos, con el fin de que puedan proveer mayor información a los usuarios sobre los beneficios y desventajas que ésta tendrá para la población migrante. • Proporcionar asistencia técnica para que las Ventanillas de Salud puedan concursar para obtener apoyos federales (grants). • Reforzar la difusión de las VDS y el Seguro Popular. 33 Iniciativas para el fortalecimiento del programa • Avanzar en la estandarización de los procesos y protocolos en cada Ventanilla de Salud. • Promover que las referencias hacia las clínicas comunitarias fluya en Medición de la presión arterial. ambos sentidos y se pueda lograr un canal privilegiado que permita a las VDS llegar a prerregistrar a los usuarios en las clínicas de salud. • Proseguir con la alineación de la red de VDS con las estrategias nacionales de salud pública, como es el caso de la National Prevention Strategy, y que se mantenga el reconocimiento de las VDS como socias de ésta. Promotora de salud dando orientación y consejería sobre prevención de la diabetes, 2012. • Analizar los retos y oportunidades del fortalecimiento de las VDS. La implementación de este proyecto inició en cinco VDS, ubicadas en las siguientes ciudades: Kansas City, K.C.; Nueva York, N.Y.; Washington D.C.; Portland, Oregón, y Fresno, California. Para la selección de estas Ventanillas de Salud se consideraron algunas variables, como la población total de cada una de estas ciudades, el número de personas atendidas, el tiempo de operación (seleccionando diferentes periodos de inicio), la experiencia de trabajo con las agencias fiscales, y las experiencias exitosas y buenas prácticas. En Washington D.C., el 18 y 19 de junio de 2012 se realizó la primera sesión de capacitación para la Segunda Generación de Ventanillas Proporcionando orientación sobre los servicios que ofrecen las Ventanillas de Salud de Salud, con el fin de cumplir los y el Seguro Popular. objetivos anteriormente señalados. 34 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias Los principales temas que se trataron en la sesión se describen en seguida: • En la parte inaugural se abordaron aspectos relacionados con los componentes de la Segunda Generación de las VDS, la participación financiera, el acompañamiento técnico a estados como Puebla, Oaxaca y Chihuahua, y el reforzamiento de elementos tecnológicos. Fue señalada la importancia de las VDS y su correlación con el Seguro Popular como mecanismo del gobierno mexicano para hacer efectivo el derecho a la salud en el país. • Por parte de Health and Human Services (HHS), se destacó la importancia de identificar las características de los dos países –específicamente, las necesidades de la población– y de incentivar buenas prácticas de colaboración en salud. Se presentó la experiencia con China denominada “The Management Manual Process; Getting to ‘Yes’”, la cual permitió identificar que los problemas de salud pública unen a las naciones, puesto que luchan por los mismos propósitos. En la tercera intervención, a cargo de la HHS Office of Minority Health, se señaló que su misión es mejorar la salud de poblaciones minoritarias raciales y étnicas, a través del desarrollo de políticas de salud y programas que ayuden a eliminar las disparidades en este campo. También se presentó el programa Promotores de Salud Initiative, el cual tomó en cuenta el quehacer de los promotores que operan en las Ventanillas de Salud, quienes se acercan a los miembros Primera sesión de capacitación para la Segunda Generación de Ventanillas de Salud, Washington D.C., 18 y 19 de junio de 2012. 35 Iniciativas para el fortalecimiento del programa de las comunidades latinas e hispanas de bajos ingresos para adelantar acciones de prevención y promoción de la salud. Se resalta que desde esta oficina se dirigen los esfuerzos para construir y financiar alianzas con diversas organizaciones para ayudar a individuos, familias y comunidades necesitados. Finalmente, desde la Embajada de México en Estados Unidos, se presentó la reciente reforma legal en salud realizada en dicho país, según la cual se reconoce el derecho de gozar de la mejor calidad posible de salud, sin discriminaciones, lo que puede implicar beneficios importantes en la salud de los latinos que viven en Estados Unidos. • Los temas presentados por la Secretaría de Salud de México fueron los siguientes: conformación del Sistema de Salud en México y la reforma a la Ley General de Salud en 2003, en la que se incluye la creación del Seguro Popular. Destacó que la función del Seguro Popular es atender a todas las personas que no tienen cobertura de salud ni acceso a servicios médicos debido a su estatus laboral. Por otra parte, se resaltaron las funciones de regulación y financiamiento de la Comisión Nacional de Protección Social en Salud. Finalmente, se destacaron los resultados obtenidos en dicha sesión de capacitación: • Confirmación del compromiso del secretario de Salud respecto de la salud de los migrantes mexicanos en Estados Unidos. • Capacitación a representantes de las cinco prioritarios y comunes de salud. VDS seleccionadas en temas • Se acordó la elaboración de un “Modelo lógico de las Ventanillas de Salud”. • Se llegó a acuerdos sobre temas prioritarios de salud, como son la obesidad, el cáncer, la diabetes, el VIH, la salud sexual y la vacunación. • Se concluyó que las Ventanillas de Salud son actualmente una red y deben avanzar hacia un modo de intervención. • Consolidación del convenio con Su Familia: National Hispanic Family Health Helpline (call center) y la respectiva capacitación de los asistentes. 36 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias Call center en colaboración con la National Alliance for Hispanic Health (NAHH), Estados Unidos Esta colaboración inició en junio de 2012, y consiste en que la National Alliance for Hispanic Health, a través del call center financiado por HHS, proporciona orientación sobre prevención y promoción de la salud del migrante, hace referencias a las VDS y a las clínicas comunitarias, y apoya en la difusión del Seguro Popular. Otro de los servicios que se ofrecen por medio de la colaboración con esta iniciativa es la difusión de información en salud a través de medios impresos, los cuales se distribuyen en todas las VDS. En el centro de llamadas Su Familia: National Hispanic Family Health Helpline, entre julio y octubre de 2012 se recibieron 874 llamadas. A todas se les otorgó información sobre salud, y a algunas personas se les refirió a servicios de atención de salud, independientemente de su país de origen; 40 de ellas se identificaron como migrantes mexicanos sin seguro de salud, y a 15 se les sugirió visitar las Ventanillas de Salud. Se les informó acerca de su elegibilidad para recibir beneficios de salud a través del Seguro Popular, servicio otorgado por el gobierno mexicano. Seguro Popular para las familias de migrantes mexicanos Este programa forma parte del Sistema de Protección Social en Salud o Seguro Popular, propuesto por la Secretaría de Salud, el cual tiene por objetivo “Garantizar el derecho constitucional a la protección de la salud de los mexicanos, a través del financiamiento de intervenciones y tratamientos que son operados por los servicios de salud de las 32 entidades federativas del país”. Específicamente, en el programa de Seguro Popular para la Familia de Migrantes Mexicanos se busca lo siguiente: • Dar a conocer a los mexicanos que residen en los Estados Unidos las acciones que realiza el gobierno federal para que sus familias que viven en México tengan acceso a los servicios de salud que brinda el Sistema de Protección Social en Salud (Seguro Popular). • Hacerles saber a los migrantes que por ser mexicanos tienen derecho a la protección de su salud en territorio nacional. 37 Iniciativas para el fortalecimiento del programa • Que los mexicanos que viven en los Estados Unidos no se preocupen por la salud de sus familiares en México, toda vez que ellos estarán atendidos por los servicios médicos financiados a través del Seguro Popular. • Como respuesta a la problemática señalada, el proyecto de difusión del Seguro Popular se acompaña de acciones de prevención en salud que se llevan a cabo con la población migrante, buscando disminuir la carga de enfermedades infecciosas, crónico-degenerativas y de las adicciones. Los beneficios del proyecto son los siguientes: • Atención médica a la familia del migrante que vive en México. • El migrante, cuando se encuentre de visita en México y lo requiera, podrá afiliarse y recibir atención médica. En el periodo de julio de 2011 a septiembre de 2012, se informó a 184 477 personas sobre la disponibilidad y beneficios del Seguro Popular, de las cuales 6% se preafiliaron a dicho programa (véase la gráfica 9). Estrategias de difusión La implementación de la estrategia tiene dos vertientes, una en Estados Unidos y la otra en México. Para Estados Unidos se tienen las siguientes estrategias: • El trabajo se llevó a cabo en estrecha colaboración con los consulados de México en las cuatro ciudades más importantes como receptoras de migrantes mexicanos: Los Ángeles, Chicago, Nueva York y Atlanta. • El Seguro Popular imprimió 100 000 folletos en forma de historieta, dirigidos a los migrantes que se encuentran en zonas urbanas, y un tiraje de 50 000 más para atender a la población rural. • En coordinación con el Instituto de Mexicanos en el Exterior (IME) de la Secretaría de Relaciones Exteriores, se realizó una cápsula informativa sobre los beneficios del Seguro Popular para los migrantes y sus familias, la cual es difundida a través de la red de pantallas de Saber es Poder, que se transmite en todos los consulados en Estados Unidos. • Se produjeron anuncios informativos, que son transmitidos por las radiodifusoras de México con presencia en Estados Unidos. • En los boletines mensuales de los consulados se ha incluido información sobre el Seguro Popular. • También se llevaron a cabo varias reuniones entre los titulares del Seguro Popular en las entidades federativas y organizaciones de migrantes (con sede en Jalisco, Zacatecas, Michoacán, Yucatán y Puebla, entre otros estados) residentes en las principales ciudades de destino de la migración en los Estados Unidos de América, con el fin de propiciar un acercamiento 39 Iniciativas para el fortalecimiento del programa entre quienes tienen la responsabilidad de operar el Seguro Popular y la población migrante, con el propósito de que, por una parte, los servidores públicos conozcan de primera mano las necesidades y expectativas de los migrantes y, por la otra, para que difundan los nuevos servicios con los que se cuenta en las comunidades de origen de los migrantes y propicien la afiliación y el ejercicio del derecho a la protección de la salud tanto de sus familiares en México como de ellos mismos. • Asimismo se llevó a cabo un proyecto de investigación entre el Baruch College de la Universidad de Nueva York y el Centro de Investigación y Docencia Económicas (CIDE) para identificar el conocimiento que los migrantes mexicanos tienen sobre el Seguro Popular. En cuanto a las estrategias propuestas para México, se destacan las siguientes: • Realización de reuniones con los directores de los regímenes de protección social en salud de los estados originarios de la migración en México para el desarrollo de materiales que se están entregando a los consulados. • Directorio de los módulos de atención y orientación por entidad federativa. • Catálogo de enfermedades e intervenciones financiadas por el Seguro Popular (CAUSES dinámico explicativo). • Cobertura del Seguro Popular por estados y municipios. • Inventario de la infraestructura médica en los niveles estatal, municipal y local. • Elaboración de un folleto informativo que se distribuirá masivamente a las familias de los migrantes que residen en los municipios originarios de la migración, a fin de que éstas les informen a sus migrantes en Estados Unidos sobre el Seguro Popular, con lo cual queda cerrado el círculo. Ventanillas móviles Se inició con el programa de Ventanilla de Salud Móvil en Kansas City, en la cual se pretende atender a trabajadores agrícolas que viven en locali- 40 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias dades rurales o muy alejadas de la ciudad. Además están presentes en consulados móviles, ferias de salud y eventos comunitarios. En noviembre de 2011, el Consulado de México en Kansas City, Missouri, y Juntos Projects firmaron un acuerdo para establecer la Ventanilla de Salud Móvil. La ceremonia inaugural se celebró el 6 de marzo Inauguración de la Ventanilla de Salud Móvil. Garden City, Kansas, 2012. de 2012 en el Garden City Community College. La Ventanilla de Salud Móvil se instaló en el Smoky Hills Education Service Center de Salina, Kansas, el 28 de abril de 2012. Como parte de las actividades de esta ventanilla, en dicho evento se incluyeron cuatro estaciones para la detección de padecimientos como la diabetes mellitus, hipertensión arterial, obesidad y VIH/SIDA. Se proporcionó información relacionada con la prevención del cáncer y el tabaquismo, la adopción de estilos de vida saludables, cuidados de la piel e higiene bucal. Se contó con la atención del doctor Edward Ellerbeck, quien proporcionó consultas individuales a los participantes del evento. En total asistieron 70 personas, quienes recibieron información sobre salud o pruebas de detección como glucosa en la sangre, presión arterial, índice de masa corporal (IMC) y VIH/SIDA. Ventanilla de Salud en Garden City, Kansas, 12 de mayo de 2012. 41 Iniciativas para el fortalecimiento del programa El 12 de mayo de 2012, en Garden City, Kansas, se contó con la colaboración de los promotores de la salud de la ciudad y se ofreció información a la población latina sobre la prevención del cáncer y el tabaquismo. Se tomaron pruebas de detección de VIH/SIDA, glucosa en la sangre, niveles de presión arterial, IMC y daños en la piel. Finalmente, cabe resaltar que durante el periodo de reporte se instalaron 51 337 consulados móviles con la participación de 2 672 agencias gubernamentales y no gubernamentales, en los cuales se atendió a 191 071 personas (véase la gráfica 10). Clínicas comunitarias En 2010, las secretarías de salud de México y Estados Unidos acordaron trabajar conjuntamente para que los migrantes mexicanos pudieran acceder a los servicios de salud ofrecidos por la Red de Clínicas Comunitarias (7 000 clínicas) operadas por el gobierno federal de los Estados Unidos. Esta colaboración inicia con la organización de varias reuniones regionales de Ventanillas de Salud (VDS) y la Health Resources and Services Ad- 42 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias ministration ( HRSA), con la finalidad de incrementar el acceso de los migrantes mexicanos a dicha red de servicios de atención médica en los Primera reunión regional para fortalecer el quehacer de las clínicas comunitarias. Para ello se establece una estrategia de difusión de los servicios que prestan los centros comunitarios, así como de capacitación respecto de estos servicios. Las actividades que se están realizando en colaboración con las clínicas comunitarias de HRSA son las siguientes: • Difusión del funcionamiento de las clínicas comunitarias que maneja la HRSA, los servicios ofrecidos, la referencia adecuada de los usuarios de las ventanillas a las clínicas comunitarias, así como la promoción y prevención de la salud. • Reuniones regionales con la participación de instituciones del gobierno federal de Estados Unidos y el de México, gobierno estatal, agencias fiscales y organizaciones de la sociedad civil que trabajan el tema de salud del migrante. A la fecha, se han llevado a cabo dos reuniones: la primera en Dallas, Texas, el 28 y 29 de julio de 2011, y la segunda en San Francisco, California, el 20 de enero de 2012. • En la reunión de Dallas, Texas, se realizó el primer acercamiento entre las agencias fiscales que operan las VDS y directivos de la HRSA en la región 6, cubriendo los estados de Texas, Nuevo México, Luisiana, Oklahoma y Arkansas, a fin de poder fortalecer las actividades y el funcionamiento de las VDS, en donde se pretende que, a través de las clínicas comunitarias que apoya la HRSA, un mayor número de migrantes mexicanos y sus familias tengan acceso a los servicios de salud, brindando una orientación 43 Iniciativas para el fortalecimiento del programa Participantes en la primera reunión regional en Dallas, Texas. específica para cada necesidad de salud de la población. A esta reunión asistieron alrededor de 70 personas, entre promotores de salud, representantes de este sector y del gobierno tanto de México como de Estados Unidos. Además se involucraron, a través de Internet, otras regiones en las que opera la HRSA, como Boston, Nueva York, Kansas, Filadelfia, Denver, Atlanta y San Francisco. El objetivo principal fue dar inicio a una cooperación fundamentada en la buena voluntad de servir a los migrantes en el tema específico del acceso a los servicios de salud. Se destacó la importancia de incluir, a través de las VDS y las clínicas comunitarias de la HRSA, a los migrantes para ofrecerles información sobre prevención y promoción de la salud. Se resaltó la importancia de la colaboración entre las agencias fiscales y organizaciones no gubernamentales para sensibilizar, capacitar y educar a la población para que se involucre Sesión plenaria sobre la colaboración del gobierno federal de los Estados Unidos (HHS y HRSA) con el programa Ventanillas de Salud. 44 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias en el cuidado de su salud, familia y entorno. Se presentaron programas como: el Seguro Popular y su mecanismo de preafiliación, mediante el cual se busca que las familias de los mexicanos que están en el exterior puedan recibir asistencia médica y orientación sobre promoción y prevención de la salud en México; Administration for Children and Families (ACF), Centers for Medicare and Medicaid (CMS), Food and Drug Administration (FDA), U.S. Administration on Aging y National Institute for Occupational Safety and Health (NIOSH), los cuales buscan empoderar, fortalecer, producir e integrar un trabajo específico a fin de mejorar las necesidades en servicios de salud y comprometerse con la comunidad, especialmente con los migrantes, para fortalecer las habilidades en niños y familias. Como conclusión, se destacó la importancia de continuar con los acercamientos con las otras regiones en las que opera la HRSA, con el propósito de promover el acercamiento de las más de 6 000 clínicas comunitarias, poder vincular las 50 VDS disponibles actualmente y firmar la carta de intención entre las dos instituciones. • El 20 de enero de 2012, en San Francisco, California, se celebró la Segunda Reunión Regional de Ventanillas de Salud y la Health Resources and Services Administration, a fin de fortalecer las actividades de colaboración entre estos organismos y el funcionamiento de las VDS, compartir las buenas prácticas realizadas y dar seguimiento al primer encuentro, efectuado en julio de 2011. Se contó con la participación de personal operativo y directivo de las VDS y la HRSA, así como de funcionarios de la Secretaría de Salud de México y de Estados Unidos. Las principales actividades que se desarrollaron fueron las siguientes: presentación de la perspectiva general del Instituto de Mexicanos en el Exterior (IME), estructura y servicios disponibles en las VDS para la población migrante, descripción de la infraestructura del actual sistema de salud mexicano, específicamente del tema relacionado con la cobertura universal en México y el servicio de preafi- Discusión llevada a cabo por el grupo de Arizona acerca de las áreas de oportunidad para liación al Seguro Popular para la el fortalecimiento de las Ventanillas de Salud. atención en este país. 45 Iniciativas para el fortalecimiento del programa • En grupos por estado se discutieron las áreas de oportunidad para el fortalecimiento de las VDS. - En el grupo de Arizona se destacaron los siguientes temas: a) mejorar el acceso a la comunidad de migrantes mediante el uso de clínicas móviles, el uso de anuncios de servicio público, la difusión de las organizaciones religiosas y grupos de la comunidad; b) mayores oportunidades de capacitación, las cuales deben ser culturalmente sensibles; c) mejorar la comunicación entre las Ventanillas de Salud por medio del desarrollo de un registro que incluya casos difíciles de atender, las mejores prácticas y las poblaciones que están siendo atendidas; d) difusión de información; e) disponibilidad de recursos, HHS-HRSA, recursos de información, oportunidades de financiación, fundaciones privadas y corporaciones; f) salud mental y abuso de sustancias dañinas para la salud; g) promotoras, y definir roles y oportunidades de formación; h) recaudación de fondos y cómo solicitar subvenciones a gran escala, entre otras oportunidades de financiamiento. Discusión y líneas de acción para el fortalecimiento de las Ventanillas de Salud. - En el grupo de California se identificaron oportunidades similares a las propuestas por el grupo anterior y además destacaron lo siguiente: a) mejorar el acceso de la comunidad migrante a las ventanillas, que deben ser utilizadas como una red de comunicación, y promover intervenciones como campañas de vacunación; b) crear alianzas con gobiernos locales (en este sentido, se señaló que las ventanillas pueden ser parte de la Ley 46 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias de Asistencia Asequible de Estados Unidos); c) compartir las mejores prácticas a través de boletines, informes trimestrales y página web; d) verificar oportunidades de capacitación; e) evaluar los materiales disponibles e incluir a la población indígena que no habla castellano, y f) salud mental. Finalmente, se destacan los siguientes compromisos: • Incrementar el conocimiento acerca de las ventanillas y servicios disponibles por parte de la población migrante, tales como: referencias, seguro de asistencia sanitaria, preafiliación al Seguro Popular, entre otras. Como siguientes pasos, se planteó reforzar el impacto y las alianzas estratégicas con instituciones como HRSA, ACS, NIOSH y CDC; el fortalecimiento de los esfuerzos y las actividades de difusión, así como la planeación y posterior ejecución de ventanillas de salud móviles que se acerquen más a la población migrante. • Trabajar en conjunto con el IME y la Secretaría de Salud para identificar temas relevantes como la prevención, el acceso a los servicios y la elaboración de propuestas de subvención para que el personal de las ventanillas pueda ser capacitado por la HRSA-HHS en temas relevantes y se desarrolle un plan de trabajo. • Realizar un directorio de recursos por estado dirigido al personal de las Ventanillas de Salud, con el fin de que se puedan efectuar las referencias adecuadas de la población migrante. • Detallar un programa de capacitación sobre el sistema de salud mexicano para el personal de las Ventanillas de Salud, que incluya el Seguro Popular. • Fortalecer la colaboración entre el programa Ventanillas de Salud con las secretarías de salud fronterizas, con el fin de poder brindar apoyo en conjunto a la población migrante y atender sus necesidades. • Elaborar una propuesta para incluir el fortalecimiento del programa de las VDS como parte de los proyectos de la Comisión de Salud Fronteriza México-Estados Unidos. 47 Iniciativas para el fortalecimiento del programa Es preciso mencionar que en los hogares médicos o clínicas comunitarias se orientó a 31 236 usuarios; 110 968 personas fueron referidas a dichos servicios, y de éstas sólo se pudo confirmar la atención de 12% en la primera visita. La mayor proporción de las referencias (84%) correspondió a personas adultas (véase la gráfica 11). GRÁFICA 11. Conformación del Consejo Asesor del programa federal de Ventanillas de Salud El 18 de septiembre de 2012 se llevó a cabo, en Washington D.C., la instalación del Consejo Asesor de las Ventanillas de Salud (CAVDS), conformado por especialistas, líderes en los sectores de salud y atención al inmigrante vinculados a sectores filantrópicos para brindar su experiencia y visión en temas de salud pública y fortalecer los objetivos de las Ventanillas de Salud. Para su debida operación se elaborará una propuesta de plan de acción del Consejo Asesor, así como un programa de seguimiento del Secretariado Técnico de éste. 48 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias El CAVDS es un cuerpo colegiado establecido con el fin de asesorar al programa VDS en diferentes procesos de gestión, innovación, gerenciales y de fortalecimiento binacional. Su propósito es contribuir con el gobierno de México al fortalecimiento de los objetivos del programa VDS, que garanticen día a día, a través de un mejor acceso a servicios de prevención, que los migrantes y sus familias tengan una mejor calidad de vida en los Estados Unidos de América y al regresar a México. Miembros del Consejo Asesor del programa Ventanillas de Salud. Las siguientes son las atribuciones que tiene el Consejo Asesor de las Ventanillas de Salud: • Mentener comunicación continua con la persona designada por la Secretaría de Salud, el Instituto de los Mexicanos en el Exterior y la Embajada de México en Estados Unidos, cuando alguno o todos los miembros del consejo tengan un tema de interés que consideren relevante para el fortalecimiento del programa VDS. • Informar sobre oportunidades de capacitación que como consejeros consideren que contribuyen al fortalecimiento de las VDS, así como facilitar, cuando sea necesario, el apoyo para la participación del programa VDS en dichas capacitaciones. 49 Iniciativas para el fortalecimiento del programa • Apoyar al programa VDS en la identificación de los puntos de contacto en las diferentes dependencias del Departamento de Salud de Estados Unidos para que puedan interactuar y resolver dificultades de las VDS al promover programas de salud pública estadounidense. Que estos puntos de contacto reconozcan el papel estratégico de las VDS en sus programas. Reunión del Consejo Asesor del programa Ventanillas de Salud, Washington D.C., 2012. • Apoyar en la difusión del quehacer del programa VDS y su contribución a los servicios de salud en la población hispana en Estados Unidos. • Apoyar en la gestión que se requiere para lograr una mayor cooperación técnica para que las VDS puedan concursar para obtener apoyos públicos y privados. Semana Binacional de Salud La Semana Binacional de Salud (SBS) es una de las mayores movilizaciones de esfuerzos de organizaciones comunitarias, agencias federales y estatales y voluntarios del continente americano para mejorar la salud y el bienestar de la población migrante latinoamericana en Estados Unidos, la cual se lleva a cabo anualmente en octubre en los 50 estados de la Unión Americana, bajo el liderazgo de la Secretaría de Salud, la Secretaría de Relaciones Exteriores y la Iniciativa de Salud las Américas, de la Universidad de California en Berkeley. 50 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias Inauguración de la XII Semana Binacional de Salud, Oaxaca, 2012. Sus objetivos son los siguientes: • Movilizar a las clínicas y organizaciones comunitarias para que durante una semana ofrezcan sus servicios especialmente a la población de origen mexicano en Estados Unidos. • Fomentar actividades de promoción de la salud diseñadas exclusivamente para los migrantes y sus familias. • Proveer información a las familias migrantes sobre los recursos y servicios de salud disponibles en su localidad. • Difundir, a través del Foro Binacional de Políticas Públicas, trabajos de investigación relevantes sobre migración y salud, promoviendo la creación de políticas públicas en favor de la población migrante mexicana. Del 1 al 15 de octubre de 2012 se llevó a cabo la XII Semana Binacional de Salud en los 50 consulados de México en Estados Unidos. La ceremonia inaugural y el Foro Binacional de Políticas Públicas tuvieron lugar el 1 y 2 de octubre en Oaxaca. Sesión plenaria sobre la migración de mexicanos a Estados Unidos y los servicios de salud. 51 Iniciativas para el fortalecimiento del programa Las campañas de la SBS en 2012 estuvieron enfocadas en las áreas siguientes: acceso a la salud, enfermedades infecciosas, enfermedades crónicas, salud de la mujer, salud mental/emocional, salud de los adolescentes y salud ocupacional. Dentro de estas áreas, con la ayuda de Mesa sobre enfermedades infecciosas. expertos, los siguientes temas de salud se han identificado como problemas crecientes entre la comunidad latina: barreras al acceso a servicios, VIH/SIDA, tuberculosis, diabetes, obesidad, ansiedad, depresión, violencia doméstica y embarazo en adolescentes. Se estima que durante la XII Semana Binacional de Salud se proporcionaron servicios y educación a aproximadamente 400 000 personas en más de 300 ciudades y localidades, ubicadas en 40 estados de la Unión Americana y tres provincias en Canadá. La SBS consta de una serie de actividades de promoción y educación de la salud –incluyendo talleres educativos, exámenes de salud y canalización a programas de seguro médico–, dirigidas a las poblaciones latinas más desfavorecidas en los Estados Unidos y Canadá. Se realizaron más de 2 000 actividades con la participación de 178 grupos de trabajo liderados por 134 consulados de México y de países latinoamericanos. Entre ellas destacan 350 ferias de salud comunitarias y más de 100 eventos en clínicas y hospitales, cerca de 300 talleres educativos para informar a la población sobre cómo mantenerse sana, y más de 100 unidades móviles brindan servicios de salud en los lugares más remotos en donde la población tiene menos oportunidad de acceder a éstos. Todas estas actividades se desarrollan con el apoyo de más de 4 500 agencias y 10 000 voluntarios. 52 Información, asesoría y referencias a servicios de salud en Estados Unidos para mexicanos y sus familias Programa de Investigación en Migración y Salud (Pimsa) Desde 2003, la Secretaría de Salud otorga financiamiento a proyectos de investigación sobre migración y salud a través de equipos binacionales de investigación. Participan la Secretaría de Salud, el Consejo Nacional de Ciencia y Tecnología (Conacyt), la Universidad Nacional Autónoma de México, la Universidad de California en Berkeley, la Universidad de El Paso y la Universidad de Minnesota. Se trata de generar conocimiento sobre el impacto de la migración en la salud que permita a los tomadores de decisiones implementar o fortalecer acciones de política pública en beneficio de la población migrante. Los temas centrales a financiar en este programa se seleccionan a partir de las prioridades de salud que más afectan a esta comunidad. Entre ellos podemos mencionar la salud mental, enfermedades crónico-degenerativas, enfermedades infecciosas, salud ocupacional, salud de la mujer y acceso a servicios. En 2011 se recibieron 39 propuestas de investigación, de las cuales se financiaron seis dirigidas a investigar sobre el acceso a los servicios de salud, enfermedades crónico-degenerativas, enfermedades infecciosas, nutrición y salud mental. 53 Retos y oportunidades del programa U na vez cumplida la meta de contar con una Ventanilla de Salud en cada uno de los consulados de México en Estados Unidos, el programa requiere la implementación de acciones que garanticen su continuidad y crecimiento en cuanto a la población atendida y el número de servicios, así como de otros proyectos que permitan ofrecer otros servicios de salud que actualmente no se pueden proporcionar. Entre estos proyectos resulta importante considerar la implementación de la telemedicina. Para lograr esto es necesario que exista un presupuesto etiquetado para el programa Ventanillas de Salud, porque no se cuenta con recursos asignados de forma específica para éste. Por lo anterior, los siguientes pasos del programa VDS se resumen en los retos y oportunidades que a continuación se describen: • Fortalecimiento de las Ventanillas de Salud para lograr su sustentabilidad como modelo de atención integral de salud de los connacionales en Estados Unidos. Dado que este programa ha tenido un gran impacto, las VDS deben trascender a su consolidación, autofinanciamiento y homologación de servicios, para lo cual se hacen necesarias varias actividades de fortalecimiento de los servicios que éstas ofrecen, como capacitación continua, asistencia técnica para gestión de recursos y acceso a servicios de salud, entre otras. • En cuanto al proyecto piloto de Ventanillas de Salud de Segunda Generación que se lleva en cinco ventanillas (Washington, Portland, Fresno, Nueva York y Kansas), el reto es ampliarlo gradualmente hasta llegar a cubrir las 50 ventanillas. • Mejorar el sistema de información del quehacer diario de las ventanillas, con el fin de contar con información oportuna que permita reorientar sus actividades, a partir de las necesidades específicas. 55 Retos y oportunidades del programa • Incrementar el número de Ventanillas de Salud Móviles que permita llevar servicios de prevención de la salud a nuestros connacionales que viven en localidades rurales o muy alejadas de la ciudad. • En cuanto a la ampliación de servicios se propone lo siguiente: - Diseñar una estrategia conjunta con HRSA para mejorar el acceso de la población migrante a los servicios de salud. - Canalizar a los migrantes a los centros comunitarios de salud para recibir atención médica. - Dar seguimiento a pacientes atendidos en los centros comunitarios de salud que por su gravedad hayan sido referidos a otro nivel de atención para recibir tratamiento y verificar éste. - Elaborar una línea basal sobre enfermedades que presenten los migrantes, así como la utilización de los centros comunitarios de salud, a fin de realizar la evaluación del programa. - Establecer un eje operativo entre este programa y las VDS. Ofrecer a la población migrante en los Estados Unidos consejería y consultas médicas por vía telefónica en español, a través de médicos y enfermeras certificados mediante una tarjeta de membrecía de servicios. 56 Legados del programa Ventanillas de Salud L os logros del programa Ventanillas de Salud han sido muy significativos en cuanto a su crecimiento geográfico, población atendida y servicios ofrecidos; también ha logrado posicionarse con diferentes agencias gubernamentales, académicas, privadas y organizaciones de la sociedad civil, principalmente, lo que ha permitido que cada vez más se incremente el número de organizaciones que colaboran con el quehacer diario de las VDS, todo esto gracias al esfuerzo que ha hecho el gobierno de México para que, a través de dicho programa, los connacionales que viven en Estados Unidos tengan servicios de salud más eficientes que les permitan una mejor calidad de vida. Gracias a este esfuerzo, el legado del programa Ventanillas de Salud se resume a continuación: • En el Programa de Acción Específico 2007-2012. Salud del Migrante (Secretaría de Salud, 2008) se tenía como meta lograr la apertura y operación de 50 Ventanillas de Salud. Además de cumplir esta meta, el 5 de marzo de 2012 se inauguró la Ventanilla de Salud Móvil en Kansas City, con lo cual se consigue llevar información de prevención y promoción de la salud a comunidades rurales de la zona. También se cuenta con la Ventanilla Móvil en New Jersey. • Implementación del proyecto de Segunda Generación de Ventanillas de Salud (en cinco VDS), con el fin de mejorar el acceso a servicios de salud de los migrantes en Estados Unidos, en colaboración con diversas iniciativas de salud y a través de la implementación de actividades de capacitación, difusión, acceso a servicios preventivos y asistencia técnica. • Instalación de un Consejo Asesor de las Ventanillas de Salud, conformado por especialistas, líderes en los sectores de salud y atención al inmigrante, vinculados a sectores filantrópicos para brindar su experiencia y visión en temas de salud pública y fortalecer los objetivos de las Ventanillas de Salud. 57 Legados del programa Ventanillas de Salud • A la par del desarrollo del plan de evaluación, se estableció una forma de reporte trimestral en línea, el cual permitirá tener información estadística oportuna sobre la población atendida y tipo de servicios otorgados, además de comparar los servicios entre ventanillas. The emergence of pandemics, combined with the growing phenomenon of migration, has made health a topic that requires inter-sector and global answers. Within the new global architecture, which has a wide variety of governmental and non-governmental organizations, it is necessary to strengthen the capacity for innovation with new patterns of international cooperation in public health that meet the challenges of the global agenda. Migration and health are a critical dimension of the referred process in which the Mexican government has taken significant actions through innovative public policies in order to improve the living conditions of Mexicans abroad. We have worked in a coordinated manner with the Secretariat of Foreign Relations and the Mexican Embassy in the United States of America in building a strong network of 50 Ventanillas de Salud (health modules), one in each Consulate, which supports health promotion, prevention and care of the Mexican immigrant community in the United States. The Ventanillas de Salud program represents an innovative collaboration design which is co-financed with resources of the Mexican Federal Government, involving the Consulates in the United States and local health service providers and community clinics that are focused on immigrant assistance. Around the Ventanillas de Salud network operates a set of more than 500 public and private U.S. agencies, which through local partnerships perform actions to improve the health of Mexicans residing in the United States of America. The program has become the main promoter of health prevention and promotion in the United States, in order to guarantee the constitutional right to health for all Mexicans, regardless of the country where they live. The Ventanillas de Salud are located within all the Mexican Consulates, which are visited daily by thousands of people, many of them lacking 9 Foreword social security (including health services) due to their immigration status and the work activities that most of them perform (construction, agriculture and manufacturing), which are statistically among the most hazardous to health. During periods of formalities at the Consulates, the Ventanillas de Salud offer Mexican families services such as tests to measure blood pressure and glucose, analysis of HIV / AIDS, screening for breast cancer, provision of certain vaccines, or if necessary, the referral to Community Health Centers in the United States, regardless the immigration status of the recipients. The information and assistance provided through the Ventanillas de Salud are focused on the higher incidence diseases of the Mexican population in the United States, which are non-communicable diseases caused by risk factors such as obesity, which is present among our fellow citizens due mainly to severe metabolic changes produced by changes in diet and lifestyle. For critical cases, the program provides referral, according to their condition, to federal health centers in Mexico City or hospitals within Mexico in order to provide the required medical care as quickly as possible. Likewise, the Ventanillas de Salud have implemented prevention programs and psychological counseling to strengthen protection factors against the use of drugs among school age young people in populations with high migration rate as in destination cities in the USA. Furthermore, the Ventanillas de Salud include an electronic platform to provide immigrants the possibility of pre-affiliation to the Seguro Popular (Public Health Insurance) from wherever they are, as well as to seek that family members who remain in Mexico may receive health services in their place of origin. Thanks to these services, an in situ empowerment of Mexican communities in the United States is being achieved, where local priorities are defined in each city, responding flexibly to their specific needs. The main purpose is to broaden the knowledge of preventive health that Mexican families have on both sides of the border and to promote its timely and effective use. The broad participation of agencies has consolidated a strong network of preventive health for our communities abroad, which has allowed a growing assistance coverage, which has been enriched with mobile Ventanillas de Salud, allowing to bring health services closer to agricultural workers in distant places. The Ventanillas de Salud are evidence of shared responsibility that the Mexican Government undertakes to seek the welfare of immigrants in 10 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families the United States. Much remains to be done. Mexico and the United States need to follow up and work further with the innovative and effective programs that have been implemented, such as the Ventanillas de Salud; and mainly they need to join bilateral efforts to continue guaranteeing the right to health for their population. The establishment of a pilot program of second generation: Ventanillas Second Generation, aims in this direction. This program, among other innovative actions, seeks the participation of Mexican entity governments where migration originates. This publication tells the story of the Ventanillas de Salud. It also raises the challenge ahead for the institutionalization of this assistance model with imagination and commitment, with the purpose of helping to raise the quality of life of our fellow citizens living abroad, extending the generous arms of the Mexican government and its associates in the United States of America. It considers the positive externalities of our actions for the society of that country as a whole. Salomón Chertorivski Woldenberg Secretary of Health 11 Background T he constant flow of Mexicans to the United States and the growing volume of illegal immigrants highlight the vulnerability to which they are exposed in the neighboring country. Extreme weather conditions and public safety can become potential hazards affecting the health situation of immigrants. Sometimes, the journey without legal documents to the north of the continent could become a permanent risk, more than an opportunity, due to the conditions to which immigrants are exposed to all the way from their departure point of origin to their destination in the United States. Such conditions may affect their health, which is a priority for the full development of the physical, social and cultural potential of the immigrant and his family, risking specific factors identifiable to public health. According to the population census of the United States for 2008, the Mexican-origin population had the highest percentage (34.6%) of uninsured people living in the U.S. (U.S. Census Bureau, 2008). It stands out that almost half (48.3%) of the people of Mexican origin and 61% of those born in Mexico in the most productive age group (18 to 64 years old) reported that they had no health insurance. Insurance and service access is limited even among the group of children under 18. For example, in the health needs survey conducted at the Mexican Consulate General in Los Angeles, in 2004 (Secretariat of Foreign Relations, 2004), it stands out that 49% Mexican children born in the United States had no health insurance, even when the State of California had a free insurance coverage program for low-income children between 0-18 years. This figure clearly reflects the underutilization of health services among these families. The target population of this program is formed by those Mexicans living in the United States and their families. In 2010, there were about 31.8 million people of Mexican origin in the United States. The potential scope of the Ventanillas is very significant, since the consular network serves an average of 1 500 000 people annually through its 50 Ventanillas de Salud and through two Ventanillas de Salud in the United States. 13 Background Overall, the Secretariat of Health proposed the Programa de Acción Específico 2007-2012. Salud del Migrante (Secretariat of Health, 2008), which seeks to promote the health of Mexicans in the United States of America. To implement this program, the following institutional framework, related to legal fundamentals, was considered. In its general sense, it is based in Article 4 of the Constitución Política de los Estados Unidos Mexicanos (Honorable Union Congress, 2012), which states that “every person has the right to health protection”. Likewise, specifically, Articles 3 and 13 of the Ley General de Salud (Honorable Union Congress, 2010), highlight the obligations of the Secretariat of Health for maintaining the health of Mexicans. In particular, the “Reglamento interior de la Secretaría de Salud” (Federal Executive, 2011) was considered, including the competencies, objectives and actions for the International Relations Department of the Secretariat and its functions in the work to improve the health of fellow citizens abroad. Another important link that the Programa de Acción Específico 20072012. Salud del Migrante (Secretariat of Health, 2008) has, is the Plan Nacional de Desarrollo 2007-2012 (Mexican United States Government, 2007)–, which establishes national objectives, strategies and priorities that should govern the actions of the current government. Thus, this program is aimed to promote the means to achieve the protection and attainment of immigrants’ health, for both, those who arrive in Mexico, and national immigrants in the North American region. In consequence, we will have a favorable environment for prevention and health care that allows individual and social development, as well as seizing of opportunities for the benefit of immigrants and their families. Lastly, the Programa Sectorial de Salud 2007-2012 (Prosesa, Spanish acronym) was considered within this international framework. Such program aims to address health needs of Mexican immigrants, primarily in North America and those who enter the country. Its strategy 4.9 points out the implementation of a program for immigrant’s health, through which “international cooperation regarding health may be seized to help achieve the objectives of the sector”. The following are specific actions proposed by Prosesa: • Strengthen collaboration and cooperation with the Border Health Commissions Mexico-United States and Mexico-Guatemala on issues of epidemiological surveillance and control. 14 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families • Strengthen multilateral and bilateral ties to offer and receive international cooperation to fulfill the objectives of Programa Sectorial de Salud 2007-2012. • Consolidate the organization and development of the Binational Health Weeks, which have broadened the access of immigrant population to health services in the United States. • Promote the affiliation of migrant families to the Seguro Popular through the Ventanillas de Salud within the consulates throughout the United States. • Improve the health care that is provided to ill fellow citizens who need to be repatriated. • Strengthen Mexico’s active participation in major international health organizations, including the World Health Organization (WHO) and the Pan American Health Organization (PAHO). Through these actions, the Migrant Health Program becomes a key element in achieving the goals and complying with the Programa Sectorial de Salud 2007-2012. Salud del Migrante Mission To protect the health of immigrants and their families in their destination places through the establishment of specific binational collaboration strategies to promote health care in a culturally sensitive manner. Vision Mexican immigrants and their families have timely information on health promotion and disease prevention, and in their communities they have access to quality health services to achieve the full development of their physical, mental and social potential. 15 Background General Purpose To guarantee access to health services and protect the health of immigrants through binational actions in their places of origin, transit and destination. Specific Objectives • Coordinate, support and monitor the various health related programs of the federal public administration that are carried out to benefit the Mexican immigrant population. • Promote communication strategies and information regarding education and health promotion aimed at the Mexican immigrant population with cultural sensitivity. • Promote and strengthen the binational relationship among health care providers in order to increase accessibility to health care of immigrants. • Encourage and facilitate the management of binational agreements with governmental and nongovernmental organizations and academic institutions, which have the purpose to promote health and improve the access to and quality of health services aimed at the Mexican immigrant population. • Provide medical care and hospital services to ill fellow citizens on both sides of the border. • Foster the research related to the health of immigrants. This program includes ten strategies, each with specific lines of action and the following subprograms: 1. Ventanillas de Salud (VDS). 2. Binational Health Week (SBS). 3. Repatriation of seriously ill fellow citizens. 4. Research Program on Migration and Health (Pimsa, Spanish acronym). 5. Modules for the prevention and health promotion of immigrants in border cities. 6. Seasonal Agricultural Workers Program Mexico-Canada (PTAT). 7. Institutional framework for the approach of Ventanillas de Salud Ventanillas de Salud The Ventanillas de Salud materialize strategy 7 of the Programa de Acción Específico 2007-2012. Salud del Migrante (Secretariat of Health, 2008), which seeks to promote and strengthen the health care of the immigrant population. It is also a contribution from the Mexican government to provide prevention services to their fellow citizens in the U.S. The establishment of the Ventanillas de Salud seeks to supply health information and education to Mexican immigrants in the United States, with the purpose of providing and improving access to health services that are available for this population and harmonize the information that both health systems –the Mexican and the American– have on a same patient. Regarding this strategy, and within the lines of action that are considered in the Programa de Acción Específico 2007-2012. Salud del Migrante (Secretariat of Health, 2008), the following are highlighted: 17 Background 1. Strengthen the strategic program Ventanillas de Salud in coordination with the Institute of Mexicans Abroad of the Secretariat of Foreign Relations. 2. Increase financial resources in order to implement this project in all U.S. Consulates. 3. Monitor and evaluate the proper functioning of Consulates services. 4. Manage and promote the Ventanillas de Salud (VDS) model in all U.S. Consulates. 5. Opening of 25 Ventanillas de Salud in U.S. Consulates. Description of the Ventanillas de Salud program Since 2000, the Mexican government began a series of actions to improve the living conditions of Mexicans abroad. Three years later, through its consular representative in San Diego, California, and as a goodwill initiative, the Ventanillas de Salud program is tested as a center to provide information on health issues.1 Having the support of the Mexico-United States Border Health Commission (CSFMEU)2 and the MexicoUnited States Health IniInauguration of the first Ventanilla de Salud, tiative (currently known as San Diego, California, 2003. the Health Initiative of the Americas), the University of California and The California Endowment,3 a subsidy for a pilot phase in the mentioned Consulate was approved. The term Ventanilla de Salud comes from the initial proposal to act as a channel of communication among the immigrant, the health promoter and the Consulate to provide guidance and information on health issues. 2 The Mexico-United States Border Health Commission is a binational agency of the Secretariat of Health committed to improve health on the border, being a meeting point between Health State Officials and personalities from the community and their federal counterparts to share information and create agreements regarding specific public health issues with the United States. Ventanillas de Salud is a Mexican government program developed by the Secretariat of Health and the Immunizations services at Ventanillas de Salud. Secretariat of Foreign Relations and implemented by the 50 Mexican Consulates in the United States and local health organizations. It provides reliable information regarding health prevention, counseling and referrals to health services that are available and accessible in the community. It was also designed to improve physical and mental health of Mexicans who live in the United States and to improve their access to primary and preventive health services and health insurance coverage, and guarantee culturally sensitive services to decrease the use of emergency services. After a two-year development, two evaluation studies and an adaptation process of the pilot phase, the Mexican government initiated the expansion of this model to other Consulates. In 2004, the Mexican government provided “seed money” to keep and open modules in other Mexican Consulates in the United States. Each year, the Mexican government provides the program with about two million dollars in cash contributions and approximately one million dollars of in-kind contributions. This resource is in turn matched at different levels by local organizations, foundations and sponsors of each Ventanilla de Salud. The Ventanillas de Salud program has had an extraordinary acceptance and growth since its pilot phase began in the Mexican Consulates in San 19 Background Diego and Los Angeles. Currently, 50 Ventanillas de Salud have been officially opened and are operating. This service is available throughout the whole Mexican consular network in the United States. The location of the Ventanillas de Salud throughout the country is shown in figure 2. Mission Improve access to primary and preventive health services, increase public insurance coverage, and promote a preventive health culture of Mexicans living in the United States and their families through information, education, counseling and quality referrals, in a safe and friendly environment, by creating local and binational collaborations between the U.S. and Mexico. Vision Improve the physical and mental health of Mexicans living in the United States by increasing access to primary and preventive services, public insurance coverage, the supply and quality of culturally sensitive services, reducing the use of emergency services and establishing the ventanillas network as a reliable and sustainable home-based program that provides information, education and referral on health issues. Essential Functions • Issue health prevention and promotion programs. • Promote access of migrants to Community Health Centers in the United States through a network of 6 500 medical units. • Dissemination and pre-affiliation to Seguro Popular for migrants and their families. • Provide Mexicans in the United States the ability to affiliate their family members and themselves to Seguro Popular, from their place of residence, in order to receive health services in national territory. Health services diffusion session in the United States. 21 Background • Reduce drug consumption among school age young people both in populations with high migration rates as well as in destination cities in the United States, reinforcing protection issues to prevent the use of drugs, considering the cultural aspects common in Mexican families. Promoting access to healt services through community centers in the United States, Los Angeles, California, 2011. • Issue information about the services offered by Community Health Centers. • Take action in specific areas, including issues related to migration and health, as well as in areas of research and academic exchange. • Give follow-up to patients with serious health problems until they recover. • Establish a preventive program in elementary and secondary level schools, targeting potential immigrants. • Implement an early psychological intervention program in existing care centers in border crossing cities. • Implement a preventive and psychological counseling program in order to strengthen protective factors associated with the use of drugs. The content of this program are sensitive to Mexican culture and families through networks, associations and clubs of Mexicans in the United States. Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families Health Information Priority issues on which information is delivered to the fellow citizens, are closely related to the disease profile of those who move to the north of the continent. These are: • Substance abuse (alcohol / drug addiction). • Arthritis / rheumatism. • Asthma / allergies. • Cancer • Prenatal care. • Diabetes. • Heart disease. • Sexually transmitted diseases ( HIV / AIDS, other). • High blood pressure. • Influenza. • Nutrition and physical activity • Obesity / metabolic syndrome / cholesterol Promoting and doing an HIV screening. • Family planning • Back and neck problems • Women’s health. • Dental health • Mental health (depression / anxiety / stress). • Occupational health (prevention of work accidents). • Smoking. • Tuberculosis. • Ulcer / gastritis. • Domestic violence. • Visual health. Denver, Colorado, 2012. 23 Background Impact of the Ventanillas de Salud Program The following data is intended to highlight the impact that the Ventanillas de Salud program has had among compatriots living in the U.S. The source of information used in this section corresponds to that contained in the “Indicators Report of the Ventanillas de Salud Program”, which presents average data of 40 Ventanillas de Salud modules between July 2011 and September 2012. • The total population covered in the reference period was 903 919 persons. Its should be mentioned that the population covered by each of the available Ventanillas de Salud is different according to the size of population and the consulate. According to a diagnosis made between 2010 and 2011 by Consultant Knowledge is Power, 40.4% of the VDS attended on average more than 200 people a week. 19% of the VDS attended weekly between 50 and 100 users, and another 19% between 100 and 150 people. Only three VDS serve on average 150 to 200 people. • The total number of 3 108 379 services was distributed as follows: - 68% focused on guidance and counseling on specific topics on health issues that are priorities for the Latino population living in the U.S., such as diabetes mellitus, obesity, high blood pressure, some determinants of risk for chronic diseases such as eating habits, sedentary lifestyle, HIV / AIDS and sexually transmitted infections, mental health, addictions, women’s health, access to health services: references, health insurance, etc. (see graphic 1). - Likewise, 16%, helped for an early detection of diseases such as diabetes mellitus, obesity, high blood pressure, cholesterol, HIV / AIDS and sexually transmitted infections (see graphic 1). - Of those treated, 9% were referred to health services in the United States (see graphic 1). - Another 6% were treated in mobile consulates where they received guidance, detection or reference to some kind of health service (see graphic 1). 24 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families - The remaining 1% received other services such as vaccines or affiliation to some health insurance in the United States (see graphic 1). In addition, 64 435 glucose level meassurings were made, 15% of which presented high levels (see graphic 2 and table 1). • For prevention of obesity, there were 297 239 guidances or counselings on specific issues such as metabolic syndrome, cholesterol levels, nutrition and physical activity. 41 967 body mass index meassurings were practiced in which a 36% prevalence of overweight and obesity was registered (see graphic 3 and table 1). • On the issue of the high blood pressure prevention, 125 781 users were helped and provided guidance and counseling on the subject. Number of Ventanillas per period: 35 July-August 2011, 39 October to December 2011, 48 January-March 2012, 50 April-June 2012 and 52 July-September 2012. 27 Background was measured on 84 171 people and high blood pressure was detected on 20% of them (see graphic 4 and table 1). • 15 078 blood cholesterol meassurings were performed and nearly a quarter of them presented high levels. • 150 334 people were oriented on breast, cervical uterine or prostate cancer. 589 detection tests were performed. As for heart diseases, 60 290 people were provided guidance and a prevalence of 22% was recorded among those people who underwent a heart test. • For specific issues such as HIV / AIDS and sexually transmitted infections, 8 522 people were tested for the first and 2 443 for the latter. It is important to highlight that 1% of these testings were positive on HIV and 2% were positive on sexually transmitted infections (see graphic 5 and table 1). GRAPHIC 5. Thus, 43% of the attention corresponded to specific issues on their health; a quarter of them received information on family planning; 20% was acquainted with domestic violence issues and the remaining 11% received information on prenatal care (see graphic 6). Number of Ventanillas per period: 35 July-August 2011, 39 October to December 2011, 48 January-March 2012, 50 April-June 2012 and 52 July-September 2012. • For guidance or counseling about mental and oral health, 67 570 and 63 307 people, respectively, were provided orientation and counseling, 15% of which received second and third level services related to mental health and 84% received specialized dental treatments. • Regarding prevention of the abuse of psychoactive substances consumption, 18% of the 73 053 people who received orientation were referred to 29 Background (see graphic 7). Likewise, 48 854 users were oriented on topics related to nicotine poisoning and prevention of asthma and allergies; 0.2% of them were tested for asthma detection. GRAPHIC 7. For other diseases, 7 560 vaccines were applied, 63% of which were for the adult population and 37% for the child population. • For the promotion of occupational health, 18 934 users were oriented. Social work programs provided 107 325 people with orientation, 22% of which received information on municipal health service, 13% were benefited with transportation services, 12% counted with financial support, 10% with disability assistance, and the remaining percentage received assistance in other topics. 30 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families • Regarding access to health services, second and third level services were referred to 29 969 users who needed specific health care. On the other hand, 80 723 people received orientation on social work programs; 65% obtained discounts on medication; 17% corresponded to women, infants, and children referral; 3% received medical-legal assistance regarding debt for services; the same percentage participated in the breakfast and lunch delivery program, and the remaining 12% received other welfare services (see graphic 8). Number of Ventanillas per period: 35 July-August 2011, 39 October to December 2011, 48 January-March 2012, 50 April-June 2012 and 52 July-September 2012. • Finally, 6.02% out of 184 477 informed people pre-affiliated to Seguro Popular; 17.47% out of 29 823 informed adults got registered for a public health insurance in the U.S. and 29.14% out of 19 595 oriented children under 18 enrolled at a public health insurance. 31 Initiatives for Strengthening the Program G iven the results obtained with the implementation of the 50 Ventanillas de Salud (VDS) by the Secretariat of Health, the Secretariat of Foreign Relations and the Institute of Mexicans Abroad, it became necessary to develop new initiatives to ensure the strengthening of the progress that has been achieved so far. Such initiatives are described as follows: Second Generation of Ventanillas de Salud Project This project was developed in order to strengthen the VDS Program in its second phase and to improve the access to health services for immigrants in the United States, in collaboration with various health initiatives, and through the implementation of training, dissemination, access to preventive services and technical assistance. In March 2012 the Second Generation of VDS Project is implemented and its objectives are: • Specialized care through a global concept that allows the standardization of criteria and concepts on priority issues, such as cancer, obesity and diabetes, occupational health, mental health, addictions, child health, HIV / AIDS, tuberculosis and access to services. • Training the Ventanillas de Salud staff on health care reform in the U.S., so they can provide more information to users about the benefits and disadvantages that such reform will bring for immigrant population. • Provide technical assistance so that the Ventanillas de Salud Program can compete for federal support (grants). • Strengthen the dissemination of VDS and Seguro Popular. • Progress in standardization of processes and protocols in each Ventanilla de Salud. 33 Initiatives for Strengthening the Program • Promote that referrals to community clinics flow in both directions and be able to reach for a privileged channel that enables VDS to pre-affiliate users in health clinics. Health promoter orienting and counseling about diabetes prevention, 2012. • Proceed with the alignment of the VDS network with national strategies for public health, such as the National Prevention Strategy, and to maintain recognition of the VDS as a member of such strategy. • Analyze the challenges and opportunities of strengthening VDS. The implementation of this project started in five VDS located in the following cities: Kansas City, KC, New York, NY, Washington, DC, Portland, Oregon, and Fresno, California. In order to select these Ventanillas de Salud, some variables were considered such as the total population of each of these cities, the number of population served, the operating time (choosing different periods of startup), experience working with prosecuting agencies, success stories and best practices. The first trainin session for the Second Generation of Ventanillas de Salud was held in Washington, DC, on June 18 and 19, 2012, in order to fulfil the aforementioned objectives. The main topics that were covered during the session are described as follows: • The inaugural address approached issues related to the second gen- 34 Providing information about services offered by Ventanillas de Salud and Seguro Popular. Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families eration of VDS, financial participation, technical support to states like Puebla, Oaxaca and Chihuahua, and the strengthening of technological elements. It was highlighted the importance of Ventanillas de Salud and its correlation with the Seguro Popular, as a Mexican government mechanism in order to implement the right to health in the country. • In behalf of the Health and Human Services ( HHS), it was highlighted the importance of identifying the characteristics of both countries, specifically the needs of the population, and to encourage good collaboration practices in Health. The experience with China, called “The Management Manual Process; Getting to ‘Yes’”, was presented, which allowed to identify that public health problems bring nations together, since fighting for the same purposes. During the third intervention by the HHS Office of Minority Health, it was stated that its mission is to improve the health of racial and ethnic minority populations through the development of health policies and programs that help eliminate health disparities. The Promotores de Salud Initiative Program was also presented, which considered the work of promoters working in Ventanillas de Salud, who are close to members of the Latino and Hispanic low-income communities to set forth prevention and health promotion actions. It is emphasized that this office leads efforts to build and finance partnerships with various organizations in order to help individuals, families and communities in need. Finally, from the Embassy of Mexico in the United States, the recent health reform law made in that country was First training session for the Second Generation of Ventanillas de Salud, Washington, DC, June 18 and 19, 2012. 35 Initiatives for Strengthening the Program presented, which recognizes the right to enjoy the best health quality possible, without discrimination, which may result in significant benefits on the health of Latinos living in the United States. • The issues presented by the Secretariat of Health of Mexico were: conformation of the Health System in Mexico and the reform to Ley General de Salud in 2003 that included the creation of Seguro Popular. It emphasized that the function of Seguro Popular is to serve every person who has no health coverage or access to services because of their employment status. On the other hand, the functions of regulation and financing of the National Health and Social Protection Commission were highlighted. Finally, the highlighted results obtained in this training session were: • Confirmation of the commitment of the Secretary of Health regarding health of Mexican immigrants in the United States. • Training of representatives of the five selected VDS on priority and common health issues. • It was agreed the development of a Logical Model of Ventanillas de Salud. • Agreements were reached on priority health issues such as obesity, cancer, diabetes, HIV, sexual health and vaccination. • It was concluded that Ventanillas de Salud are currently a network and should move towards a mode of intervention. • Consolidation of the agreement with Su Familia: National Hispanic Family Health Helpline (call center) and the respective training for attendants. Call Center in Collaboration with National Alliance for Hispanic Health (NAHH), United States This collaboration began in June 2012, and it consists in that the National Alliance for Hispanic Health, through the Call Center funded by HHS, provides guidance on prevention and health promotion of immigrants, and refers to the VDS and community clinics, and supports the diffusion of Seguro Popular. 36 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families Another service offered through collaboration with this initiative is to spread health information through printed media, which is distributed in all VDS. Su Familia: National Hispanic Family Health Helpline call center received 874 calls between July and October 2012. Every caller was provided with information on health, regardless of their country of origin; 40 of them identified themselves as Mexican immigrants without health insurance and 15 people were suggested to visit the Ventanillas de Salud. They were informed about their eligibility to receive health benefits through Seguro Popular, service provided by the Mexican government. Seguro Popular for Families of Mexican Immigrants This program is part of the social protection system on health or Seguro Popular, proposed by the Secretariat of Health, which aims to “Ensure the constitutional right to the protection of the health of Mexicans, through the financing of interventions and treatments operated by the health services of the 32 states of the country”. Specifically, the Seguro Popular for Families of Mexican Immigrants program seeks to: • Make known to Mexicans living in the United States about the actions taken by the Federal Government in order for their families, living in Mexico, to have access to health services provided by the Social Protection in Health System (Seguro Popular). • Let immigrants know that by being Mexicans they have the right to health protection in Mexico. • That Mexicans living in the United States do not worry about the health of their families in Mexico, since they will be treated through medical services funded through Seguro Popular. As a response to the aforementioned problems, the diffusion project of the Seguro Popular program is accompanied by health prevention actions that are carried out with the immigrant population, seeking to reduce the burden of infectious diseases, chronic degenerative and addictions. The benefits of the project are: 37 Initiatives for Strengthening the Program • Medical care for the immigrant’s family living in Mexico. • The immigrant, when visiting Mexico and requires it, may affiliate and receive medical attention. • If the immigrant is seriously ill, covered by the Fund for Protection against Catastrophic Expenses, he will be able to return to Mexico and receive the required treatment. During the period from July 2011 to September 2012, 184 477 people were informed about the availability and benefits of Seguro Popular, 6% of which were pre-affiliated to the program (see graphic 9). Advertising Strategies The implementation of the strategy has two aspects, one in the U.S. and one in Mexico. 38 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families For the U.S., we have the following strategies: • The work was carried out in close collaboration with the Consulate of Mexico in the four most important cities as recipients of Mexican immigrants: Los Angeles, Chicago, New York and Atlanta. • The Seguro Popular program printed 100 000 brochures in the form of comics targeted to immigrants who live in urban areas, and a print run of 50 000 more to serve rural areas population. • In coordination with the Institute of Mexicans Abroad (IME by its Spanish acronym) of the Secretariat of Foreign Relations, an informative broadcast was created about the benefits of Seguro Popular for immigrants and their families, which is transmitted through the network of screens of Saber es Poder (Knowledge is Power), which is transmitted on all U.S. Consulates. • Informative radio spots were produced, which are transmitted by radio stations of Mexico with presence in the United States. • Information about Seguro Popular has been included in the Consulate monthly newsletter. • Several meetings were also held between Seguro Popular officials in the states and immigrant organizations (with home base organizations in Jalisco, Zacatecas, Michoacan, Puebla and Yucatan, among others) residents in major immigration destination cities in the United States of America, in order to contribute to a close-up among those responsible for the operation of Seguro Popular and the immigrant population, in order that, on the one hand, public servers get to know first hand, the needs and expectations of immigrants, and on the other hand, to disseminate the available at the immigrants’ place of origin and promote affiliation and to exercise the right to health protection both for their families living in Mexico and for themselves. • An investigation project was also held between the Baruch College of the City University of New York and the Economic Teaching and Education Center (CIDE, by its Spanish acronym), to identify the knowledge that Mexican immigrants have on Seguro Popular. 39 Initiatives for Strengthening the Program As for the proposed strategies for Mexico, the following are highlighted: • Meetings held with the directors of health social protection from the place of origin of immigration in Mexico for the development of materials that are being delivered to the Consulate. • Directory of Information and Orientation Modules by State. • Catalogue of diseases and interventions financed by the Seguro Popular (CAUSES dynamic explanatory). • Coverage of Seguro Popular by State and Municipality. • Inventory of medical infrastructure by level of state, municipality and local. • Preparation of an informative brochure to be distributed massively to the families of immigrants residing in municipalities of immigration origin, so that families will inform their immigrants in the United States about Seguro Popular, thus closing the circle. Mobile Ventanillas de Salud The program Mobile Ventanillas de Salud began in Kansas City, which is intended to serve farmworkers living in rural areas or areas far from the city. They are also present in mobile consulates, health fairs and community events. In November 2011, the Consulate of Mexico in Kansas City, Missouri, and Juntos Projects signed an agreement to establish the Mobile Ventanilla de Salud. The opening ceremony was held on March 6, 2012 in Garden City Community College. The Mobile Ventanilla de Salud was installed in the Smoky Hills Education Service Center in Salina, Kansas, on April 28, 2011. The Mobile VDS counted with the presence of Dr. Edward Ellerbeck, who provided individual consultations to event participants. Seventy people attended in total, whom received information about health or detection screenings such as blood glucose, blood pressure, body mass index and HIV / AIDS. On May 12, 2012, in Garden City, Kansas, the collaboration of the city health promoters took place, offering information to the Latino population on cancer prevention and nicotine poisoning. They took tests for HIV / AIDS, blood glucose, blood pressure levels, BMI and skin damage. Finally, it may be highlighted that during the reporting period 51 337 mobile consulates were installed, involving 2 672 governmental and nongovernmental agencies, in which 191 071 people were helped (see graphic 10). Community Clinics In 2010, the Ministries of Health of Mexico and the United States agreed to work together in order that Mexican immigrants were able to access health services offered by the Community Clinics Network (7 000 clinics), operated by the United States federal government. This collaboration begins with the organization of Ventanillas de Salud (VDS) regional meetings with the Health Resource and Services Administration with the purpose of increasing the access of Mexican immigrants to the health care services network in community health centers for immigrant. Therefore, a strategy for dissemination of services offered by community centers as well as training provided in these centers is established. The activities being undertaken in collaboration with HRSAR Community Clinics are: • To disseminate information about the operation of community clinics managed by the HRSAR, services offered, appropriate reference of users of Ventanillas de Salud, as well as health promotion and prevention. • Regional meetings involving federal institutions in the United States and Mexico, state government, tax agencies and civil society organizations working in the area of immigrant’s health. Up to date, two meetings have been held: the first, in Dallas, Texas, on July 28 and 29, 2011, and the second on January 20, 2012, in San Francisco, California. 42 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families • During the meeting in, Texas, held on July 28 and 29, 2011, the first approach between Fiscal Agencies operating the VDS and HRSAR managers in region 6 took place, First Regional Meeting for strengthening the labor of community clinics, Dallas, Texas, July 28 and 29, 2011. covering the states of Texas, New Mexico, Louisiana, Oklahoma and Arkansas in order to strengthen the activities and operation of the VDS, where it is intended that, through community clinics supported by the HRSAR, a greater number of Mexican immigrants and their families have access to health services, providing specific guidance for every population’s health need. About 70 people attended the meeting, including health promoters, health sector and government representatives from Mexico and the United States. Also involved via Internet were other regions in which the HRSAR operates, such as Boston, New York, Kansas, Philadelphia, Denver, Atlanta and San Francisco. The main objective was to initiate a cooperation based on good will of serving immigrants on the specific issue of access to health services. It was highlighted the the importance of including immigrants, through the VDS and HRSAR Community Clinics, to offer them information on health prevention and promotion. It was emphasized the importance of collaboration between tax agencies and NGOs in order to sensitize, train and educate people to get involved in their health care, family and environment. Some programs were presented such as: Seguro Popular and its pre-enrollment mechanism, by which it is intended that the families of Mexicans who are abroad are able to receive medical care and counseling on health prevention and promotion in Mexico; Administration for Children and Families (ACF), Centers for Medicare and Medicaid (CMS), Food and Drug Administration (FDA), U.S. Administration on Aging and the National Institute for Occupational Safety and Health (NIOSH), which seek to empower, strengthen, produce, and integrate a specific job in order to improve the needs in health services 43 Initiatives for Strengthening the Program and commit to the community, specially with immigrants, to strengthen skills in children and families. As a conclusion, it was emphasized the importance of continuing with the approaching of other regions in which the HRSAR operates in order to promote the approach of the more than 6 000 community clinics and to link the 50 VDS currently available, and sign the Letter of Intent between both institutions. Participants on the First Regional Meeting in Dallas, Texas. • The second Regional Meeting between Ventanillas de Salud and the Health Resources and Services Administration was held in San Francisco, California, on January 20, 2012, in order to strengthen collaboration activities among these organizations, the VDS functioning, share best practices and give follow up on the first meeting, held on July, 2011. Operational and directive staff of the VDS and the HRSAR, as well as officials from the Secretariat of Health of Mexico and the United States, attended the meeting. The main activities developed were: presentation of the general overview of the Institute of Mexicans Abroad, structure and services available in the VDS for the immigrant population, description of the infrastructure of the current Mexican health system, especially on the subject of universal coverage in Mexico and the pre-affiliation service of Seguro Popular for health care in this country. • A discussion in groups by states, was held, in order to discuss the opportunity areas for the strengthening of the VDS. - In the group of Arizona, the following issues were highlighted: a) improve access to the immigrant community using mobile clinics, public service announcements, dissemination of religious organizations and community groups; b) increase training opportunities, which must be 44 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families culturally sensitive; c) improve communication among Ventanillas de Salud through the development of a record that includes difficult cases, best practices and populations that are being helped; d) dissemination of information; e) availability of resources, the HHS-HRSAR, information resources, funding opportunities, private foundations and corporations; f) Mental Health and abuse of health threatening substances; g) promoters, and defining roles and training opportunities; h) fundraising and how to apply for grants on a large scale, among other funding opportunities. - The California group, identified opportunities similar to those proposed by the previous group and also highlighted the following: a) improve access of immigrant population to the VDS, which must be used as a communication network, and promote interventions such as vaccination campaigns; b) create partnerships with local governments (it was noted that the VDS may be part of the “Affordable Care Act” of the United States); c) share best practices through newsletters, website and quarterly reports; d) verify training opportunities; e) evaluate the available materials and include the indigenous population that does not speak Spanish; and f) mental health. Finally, the following commitments were highlighted: • Increase awareness on immigrant population about VDS and the available services, such as: references, medical insurance, pre-affiliation to Seguro Popular, Plenary session about collaboration of the United States Federal Government (HHS and HRSA) with the Ventanillas de Salud program. 45 Initiatives for Strengthening the Program among others. As following steps it was considered to strenghten the impact and strategic partnerships with institutions such as HRSAR, ACS, NIOSH, and CDC; strengthening efforts and outreach activities as well as planning and subsequent execution of Mobil VDS that reach further to the immigrant population. • Work with the Institute of Mexicans Abroad and the Secretariat of Health to identify relevant topics such as: prevention, access to Arizona Group discussing opportunity areas for strengthening Ventanillas de Salud. services and the development of grant proposals, so that the staff of the VDS can be trained by HRSAR / HHS on relevant issues and a work plan is developed. Discussion and course of action for strengthening Ventanillas de Salud. • Create a directory by states, targeting VDS staff so that the appropriate references for immigrant population can be made. • Create a detailed training program about the Mexican health system for the VDS staff, including the Seguro Popular. • Strengthen collaboration between the VDS program and border health departments in order to jointly support to the immigrant population and their needs. 46 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families GRAPHIC 11. It should be mentioned that in medical homes or community clinics, 31 236 users were oriented; 110 968 people were referred to those services, and only the attention of 12% could be confirmed on their first visit. The major proportion of references corresponded to adult people (84%) (see graphic 11). Establishment of the Advisory Board of the Ventanillas de Salud Federal Program The establishment of the Advisory Board of Ventanillas de Salud (CAVDS by its Spanish acronym) was held on September 18, 2012, in Washington DC, comprised of specialists, leaders in the health care and immigrant attention sector, related to philanthropic sectors to provide their experience and 47 Initiatives for Strengthening the Program vision in public health issues and strengthen the objectives of the Ventanillas de Salud. For proper operation, a proposal of a plan of action of the Advisory Board will be developed, as well as a follow up program by its Technical Secretariat. The CAVDS, is a collegiate body established to advise the VDS Program in different management processes, innovation, management and binational strengthening. Its purpose is to contribute, along with the Government of Mexico, in strengthening VDS program objectives, which daily ensure, through better access to preventive services, that immigrants and their families enjoy a better life quality in the United States and when going back to Mexico. The following are the powers under the CAVDS: • Have regular communication with the person designated by the Secretariat of Health, the Institute for Mexicans Abroad and the Embassy of Mexico in the United States, where some or all members of the Council have a topic of interest which they consider relevant for the strengthening of the VDS Program. • Report on training opportunities which, as counselors, they consider that contribute for the strengthening of the VDS, as well as facilitate, when 48 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families Advisory Board Meeting for the Ventanillas de Salud Program, Washington, DC, 2012. necessary, the support for the participation of the trainings. VDS Program in such • Support the VDS Program in identifying contact points in the different units of the Department of Health in the United States so they can interact and solve problems of VDS when promoting U.S. public health programs. These contact points must recognize the strategic role of the VDS in their programs. • Assist in the dissemination of the role of the VDS Program and its contribution to health services in the U.S. Hispanic population. • Assist in the management required to achieve greater technical cooperation so that the VDS can compete for public and private support. Binational Health Week Binational Health Week ( BHW) is one of the largest efforts mobilization of community organizations, state and federal agencies, and volunteers in the Americas to improve the health and welfare of Latin American immigrant population in the United States, which is held annually in October in all 50 states of the USA, under the leadership of the Secretariat of Health, the Secretariat of Foreign Relations and the Americas Health Initiative, of the University of California, in Berkeley. 49 Initiatives for Strengthening the Program Inauguration of the XII Binational Health Week, Oaxaca, 2012. Its objectives are: • Mobilize clinics and community organizations for a week to offer their services, especially to the people of Mexican origin in the United States. • Promote activities of health promotion exclusively designed for immigrants and their families. • Provide information to immigrant families on health resources and services available in their community. • Disseminate through the Binational Public Policy Forum, relevant research work on immigration and health, promoting the creation of public policies in favor of the Mexican immigrant population. The XII Binational Health Week was held during October 1 to 15, 2012 in the 50 Mexico Consulates in the United States. The opening ceremony and the Binational Public Policy Forum took place in October 1 and 2 in Oaxaca, Oaxaca. The Binational Health Week campaigns this year were focused on 50 Information, Counseling and Referrals to Health Services in the United States for Mexicans and their Families the following areas: public health, infectious diseases, chronic diseases, women’s health, mental / emotional health, teenagers health and occupational health. It is estimated that during the XII Binational Health Week, service and education were provided to about 400 000 people in more than 300 cities and towns located in 40 states of the American Union and three provinces in Canada. The Binational Health Week is formed by a series of activities for health promotion and education, including educational workshops, health screenings and referral to health insurance programs, targeted to the most disadvantaged Latino populations in the United States Infectious diseases session and Canada. There were more than 2 000 activities, carried out by 178 working groups under the leadership of 134 consulates of Mexico and Latin American countries, highlighting, among others, 350 community health fairs and events in over 100 clinics and hospitals, nearly 300 educational workshops to inform the public on how to stay healthy and over 100 mobile health units provide services in the most remote places where the population has less opportunity to access health services. All these activities are supported by more than 4 500 agencies and 10 000 volunteers. 51 Initiatives for Strengthening the Program Research Program on Migration and Health (Pimsa by its Spanish accronym) Since 2003, the Secretariat of Health provides funding for research projects on migration and health through binational research teams. The Secretariat of Health, the National Council of Science and Technology (Conacyt by its Spanish acronym), the National Autonomous University of Mexico, the University of California at Berkeley, the University of El Paso and the University of Minnesota participate in this program. The purpose is to raise awareness on the impact of migration in health that allows decision makers to implement or strengthen public policy actions in favor of the immigrant population. The central issues to be funded with this program are health priorities that affect this community. Among them we may mention mental health, chronic degenerative diseases, infectious diseases, occupational health, women’s health and access to services. 39 research proposals were received in 2011, six of which were funded, and were directed to investigate access to health services, chronic degenerative diseases, infectious diseases, nutrition and mental health. 52 Challenges and Opportunities of the Program O nce that the goal of having one Ventanilla de Salud in each consulate of Mexico in the United States is reached, the program requires the implementation of actions that guarantee continuity and growth regarding the community that has been helped and the number of services, as well as other projects that allow the offering of other health services that are not currently available. Among these projects, it is important to consider the implementation of telemedicine. In order for this to be accomplished, the existence of a budget labeled for the Ventanillas de Salud program is necessary because there are no resources assigned specifically for the program. Said the above, the following steps of the VDS program are summarized on the following challenges and opportunities: • Strengthening Ventanillas de Salud to achieve sustainability as a model of comprehensive health care for Mexican nationals in the United States. Since this program has had a major impact, the VDS should transcend its consolidation, self-financing and standardization of services, for which several activities are necessary to strengthen the services they offer, such as ongoing training, technical assistance for resources management, and access to health services, among others. • For the pilot project of the Second Generation of Ventanillas de Salud which is being carried in five VDS (Washington, Portland, Fresno, New York and Kansas), the challenge is to gradually expand it to cover the 50 VDS. • Improve the information system of everyday tasks of the VDS, in order to have timely information that enables to reorient their activities, based on specific needs. • Increase the number of mobile VDS so that it is possible to take health prevention services to our compatriots who live in rural areas and areas that are far from the city. 53 Challenges and Opportunities of the Program • As for the expanding services, the following were proposed: - Design joint strategy with HRSAR to improve access of immigrant population to health services. - Refer immigrants to the Community Health Centers in order to receive medical care. - Follow up on patients helped in community health centers that due to serious illness have been referred to another level of care for treatment and verify it. - Develop a baseline of diseases presented by immigrants as well as the use of Community Health Centers in order to evaluate the program. - Establish an operational axis between this program and the VDS. - Provide the immigrant population in the United States with counseling and medical consultation via telephone in Spanish, through certified doctors and nurses using a membership services card. 54 Ventanillas de Salud Program Legacy T he achievements of Ventanillas de Salud program have been very significant in relation to geographic growth, population served and offered services; it has also managed to position itself with different governmental, academic, and private agencies and mainly with civil society organizations, which has made it possible for a continuing growth on the number of organizations that collaborate with the everyday labor of the VDS, all of this thanks to the effort made by the Mexican government in order that, through this program, our nationals living in the United States have more efficient health services that allow them a better quality of life. Thanks to this effort, the legacy of the Ventanillas de Salud program is summarized as follows: • In the Programa de Acción Específico 2007-2012. Salud del Migrante (Secretariat of Health, 2007), the goal was to reach an opening and operation of 50 VDS. In addition to having 50 operational VDS, the Mobile Ventanilla de Salud was inaugurated in Kansas City, which makes it possible to bring health prevention and promotion information to rural communities in the area. There is also a Ventanilla de Salud in New Jersey. • Implementation of the project Second Generation Ventanillas de Salud (in five VDS), in order to improve access to health services for immigrants in the United States, in collaboration with various health initiatives, and through the implementation of training, dissemination, access to preventive services and technical assistance activities. • Establishment of an Advisory Board of Ventanillas de Salud, consisting of specialists, leaders in the health care and immigrant services sector, related philanthropic sectors to offer their experience and vision in public health issues and strengthen the objectives of Ventanillas de Salud. • Along with the development of the evaluation plan, an online quarterly report was developed, which will make it possible to have timely statistical 55 Ventanillas de Salud Program Legacy information on the people that has been helped and the services that were offered, besides comparing services among VDS.
